Midwives and doctors require effective information-sharing strategies to provide safe and evidence-based care for women and infants, but this can be difficult to achieve. This article describes maternity care professionals' perceptions of communication in their current workplace in Australia. We invoke social identity theory (SIT) to explore how these perceptions affect interprofessional practice. A survey was conducted with 337 participants (281 midwives and 56 doctors). Using exploratory factor analysis we developed three scales that measured interprofessional workplace practice collaboration. Results indicated an intergroup environment in maternity care in which the professionals found exchange of ideas difficult, and where differences with respect to decision making and professional skills were apparent. Although scores on some measures of collaboration were high, the two professions differed on their ratings of the importance of team behaviors, information sharing, and interprofessional socialization as indicators of collaborative practice. These results highlight the complexities among maternity care providers with different professional identities, and demonstrate the impact of professional identity on interprofessional communication.

Problem: Postnatal follow-up screening is recommended for all women diagnosed with gestational diabetes mellitus (GDM). However follow-up rates are poor and highly variable in Australia and internationally. The reasons that mothers are not completing recommended postnatal screening after GDM diagnosis are not well understood or studied. The quality of communication may be an important influence on the completion of postnatal GDM follow-up., Aim: To explore and assess women's communication experiences of postnatal GDM follow-up., Methods: Theoretical, purposeful sampling was used to identify women diagnosed with GDM. Convergent interviews explored participants' communication experiences with GDM and postnatal follow-up. Transcripts were provided to and updated by participants. Data was analysed with Leximancer(®) (V4, 2011) automated content analysis software., Setting and Participants: This research was conducted at a major tertiary referral hospital in Queensland, Australia, between December 2012 and July 2013. Women participating in maternity shared care and diagnosed with GDM were interviewed (n=13)., Findings: Five themes, all concerned with obtaining information, were identified: diagnosis of GDM; seeking GDM information; accessing specialist services; need for postnatal GDM follow-up; and completing GDM follow-up. Results were interpreted using Communication Accommodation Theory (CAT) to explore whether and how the information needs of women were accommodated. Women's interpretations of communication events influenced their knowledge, perceptions and motivation to complete recommended postnatal follow-up., Conclusion: Accommodation of the communication and information needs of women with GDM may be an effective strategy for clinicians to encourage completion of recommended postnatal GDM follow-up., (Copyright © 2015 Australian College of Midwives. Background: Miscommunication in the healthcare sector can be life-threatening. The rising number of migrant patients and foreign-trained staff means that communication errors between a healthcare practitioner and patient when one or both are speaking a second language are increasingly likely. However, there is limited research that addresses this issue systematically. This protocol outlines a hospital-based study examining interactions between healthcare practitioners and their patients who either share or do not share a first language. Of particular interest are the nature and efficacy of communication in language-discordant conversations, and the degree to which risk is communicated. Our aim is to understand language barriers and miscommunication that may occur in healthcare settings between patients and healthcare practitioners, especially where at least one of the speakers is using a second (weaker) language., Methods/design: Eighty individual interactions between patients and practitioners who speak either English or Chinese (Mandarin or Cantonese) as their first language will be video recorded in a range of in- and out-patient departments at three hospitals in the Metro South area of Brisbane, Australia. All participants will complete a language background questionnaire. Patients will also complete a short survey rating the effectiveness of the interaction. Recordings will be transcribed and submitted to both quantitative and qualitative analyses to determine elements of the language used that might be particularly problematic and the extent to which language concordance and discordance impacts on the quality of the patient-practitioner consultation., Discussion: Understanding the role that language plays in creating barriers to healthcare is critical for healthcare systems that are experiencing an increasing range of culturally and linguistically diverse populations both amongst patients and practitioners. The data resulting from this study will inform policy and practical solutions for communication training, provide an agenda for future research, and extend theory in health communication.

In this study, 3,531 Queensland women, who had recently given birth, completed a questionnaire that included questions about their participation in decision making during pregnancy, their ratings of client-centered care, and perceived quality of care. These data tested a version of the Linguistic Model of Patient Participation in Care, adapted to the maternity context. The authors investigated how age and education influenced women's perceptions of their participation and quality of care. Hierarchical multiple regressions revealed that women's perceived ability to make decisions, and the extent of client-centered communication with maternity care providers, were the most influential predictors of participation and perceived quality of care. Participation in care predicted perceived quality of care, but the influence of client-centered communication by a care provider and a woman's confidence in decision making were stronger predictors of perceived quality of care. Age and education level were not important predictors. These findings extend and support the use of Linguistic Model of Patient Participation in Care in the maternity context.

Background: People with dementia have a range of needs that are met by informal caregivers. A DVD-based training program was developed using research-based strategies for memory and communication in dementia. The effectiveness of the training on the caregiver experience and the well-being of the person with dementia was evaluated., Methods: A pre-test/post-test controlled trial was undertaken with caregiver-care-recipient dyads living in the community. Measures of the carers' knowledge of memory and communication strategies, burden, positive perceptions of caregiving, and perceptions of problem behaviors were taken pre- and three months post-intervention. The depression and well-being of the person with dementia were also evaluated. Satisfaction with the training and feedback were measured., Results: Twenty-nine dyads (13 training group, 16 control group) participated. Bonferroni's correction was made to adjust for multiple comparisons, setting α at 0.00385. A significant improvement was found in caregivers' knowledge for the training group compared to the control group (p = 0.0011). The training group caregivers reported a reduction in the frequency of care recipient disruptive behaviors (p = 0.028) and increased perceptions of positive aspects of caregiving (p = 0.039), both at a level approaching significance. The training group care recipients had increased frequency of verbally communicated depressive behaviors at a level approaching significance (p = 0.0126). The frequency of observed depressive behaviors was not significantly different between groups., Conclusions: This approach to training for caregivers of people with dementia appears promising for its impact on knowledge and the caregiving experience. Further research could monitor the impact of the training on broader measures of depression and well-being, with a larger sample.

Effective communication between healthcare professionals and patients is critical to patients' health outcomes. The doctor/patient dialogue has been extensively researched from different perspectives, with findings emphasising a range of behaviours that lead to effective communication. Much research involves self-reports, however, so that behavioural engagement cannot be disentangled from patients' ratings of effectiveness. In this study we used a highly efficient and time economic automated computer visualisation measurement technique called Discursis to analyse conversational behaviour in consultations. Discursis automatically builds an internal language model from a transcript, mines the transcript for its conceptual content, and generates an interactive visual account of the discourse. The resultant visual account of the whole consultation can be analysed for patterns of engagement between interactants. The findings from this study show that Discursis is effective at highlighting a range of consultation techniques, including communication accommodation, engagement and repetition.

This study examined the ways in which health care providers (general practitioners and specialists) and patients communicate with each other about managing musculoskeletal (MSK) disorders, a major cause of long-term pain and physical disability. In managing their illness, patients must interact closely with health care providers, who play a large role in transferring knowledge to them. In-depth interviews with patients, general practitioners, and specialist rheumatologists in Australia and Canada were analyzed using Leximancer (a text-mining tool). Results indicated that, in their communication, doctors subtly emphasized accepting and adjusting to the illness ("new normal"), whereas patients emphasized pain relief and getting "back to normal." These results suggest that doctors and patients should accommodate in their communication across subtle and often unexpressed differences in the priorities of provider and patient, or they are likely to be at cross purposes and thus less effective.

Background: Difficulties with memory and communication are prominent and distressing features of dementia which impact on the person with dementia and contribute to caregiver stress and burden. There is a need to provide caregivers with strategies to support and maximize memory and communication abilities in people with dementia. In this project, a team of clinicians, researchers and educators in neuropsychology, psychogeriatrics, nursing and speech pathology translated research-based knowledge from these fields into a program of practical strategies for everyday use by family and professional caregivers., Methods: From the available research evidence, the project team identified compensatory or facilitative strategies to assist with common areas of difficulty, and structured these under the mnemonics RECAPS (for memory) and MESSAGE (for communication). This information was adapted for presentation in a DVD-based education program in accordance with known characteristics of effective caregiver education., Results: The resultant DVD comprises (1) information on the nature and importance of memory and communication in everyday life; (2) explanations of common patterns of difficulty and preserved ability in memory and communication across the stages of dementia; (3) acted vignettes demonstrating the strategies, based on authentic samples of speech in dementia; and (4) scenarios to prompt the viewer to consider the benefits of using the strategies., Conclusion: Using a knowledge-translation framework, information and strategies can be provided to family and professional caregivers to help them optimize residual memory and communication in people with dementia. Future development of the materials, incorporating consumer feedback, will focus on methods for enabling wider dissemination.

We examined conversations between people with schizophrenia (PwS) and family or professional carers with whom they interacted frequently. We allocated PwS to one of two communication profiles: Low-activity communicators talked much less than their conversational partners, whereas high-activity communicators talked much more. We used Leximancer text analytics software to analyze the conversations. We found that carers used different strategies to accommodate to the PwS's behavior, depending on the PwS's communication profile and their relationship. These findings indicate that optimal communication strategies depend on the PwS's conversational tendencies and the relationship context. They also suggest new opportunities for qualitative assessment via intelligent text analytics technologies.

The article discusses impact of COVID-19 pandemic on communication and social psychology. Topics discussed include need for balance between containing the epidemic and salvaging economies; lack of culture-based understanding of behavioural drivers among diverse population cohorts during pandemic; and rise in conspiracy theories, fake news, and misinformation.

The study had two goals: First, a 20-item “Refugee Parent-Child Relational Communication Scale ” was developed for parents from refugee backgrounds. Former refugee parents from Ethiopia, Congo, and Burma (= 221) completed this and other questionnaires measuring demographics, acculturation, and acculturative stress. Exploratory factor analysis indicated three factors: “Engagement,” “Hope and Aspirations,” and “Dissonance.” The subscales were internally consistent. Second, this newly developed scale was used to explore whether the parents’ communication and relations with their children were associated with demographics, acculturation, and acculturative stress. Results of multiple regressions indicated that mothers and parents with higher acculturation were more engaged with their children and also encouraged their children to aspire for higher goals in life. Although parents with higher acculturative stress were also developing aspirations and hopes in their children; they reported strained relations with their children. Objective: Futile treatment, which by definition cannot benefit a patient, is undesirable. This research investigated why doctors believe that treatment that they consider to be futile is sometimes provided at the end of a patient's life.Design: Semistructured in-depth interviews.Setting: Three large tertiary public hospitals in Brisbane, Australia.Participants: 96 doctors from emergency, intensive care, palliative care, oncology, renal medicine, internal medicine, respiratory medicine, surgery, cardiology, geriatric medicine and medical administration departments. Participants were recruited using purposive maximum variation sampling.Results: Doctors attributed the provision of futile treatment to a wide range of inter-related factors. One was the characteristics of treating doctors, including their orientation towards curative treatment, discomfort or inexperience with death and dying, concerns about legal risk and poor communication skills. Second, the attributes of the patient and family, including their requests or demands for further treatment, prognostic uncertainty and lack of information about patient wishes. Third, there were hospital factors including a high degree of specialisation, the availability of routine tests and interventions, and organisational barriers to diverting a patient from a curative to a palliative pathway. Doctors nominated family or patient request and doctors being locked into a curative role as the main reasons for futile care.Conclusions: Doctors believe that a range of factors contribute to the provision of futile treatment. A combination of strategies is necessary to reduce futile treatment, including better training for doctors who treat patients at the end of life, educating the community about the limits of medicine and the need to plan for death and dying, and structural reform at the hospital level. People with dementia living in residential care often face the dual disadvantage of increasing difficulty with communication and reduced opportunities for conversation. Social interaction is central to wellbeing of residents with dementia, so it is important that care staff have the skills to engage in conversation with them. We studied conversations in 20 care staff–resident dyads, to examine conversation structure and content, patterns of engagement within conversations, including the topics around which engagement occurred, and communication behaviours by care staff that appeared to facilitate (or impede) participation by residents. The transcripts were analysed using Discursis, a computational information visualisation tool that allows interactive visual inspection, in context, of the contributions by each speaker, the turn-taking dynamics, and the content recurring within and between speakers. We present case examples (a) where care staff did most of the talking, initiated topics and were responsible for most recurrence of content; (b) where talk was more evenly shared between partners, with some topics initiated and/or elaborated by participants with dementia; and (c) where participants with dementia talked most, with care staff supporting the conversation. We identified accommodative strategies used by care staff, such as reflecting back the other person's responses to sustain engagement. We also noted care staff behaviours that impeded communication, such as not listening attentively and not allowing sufficient time for responses. The results from this study highlight aspects of social communication within the aged care context and suggest ways in which rewarding interactions between staff and residents with dementia might be encouraged. The growing Chinese economy has attracted foreign ventures from around the world to engage in business partnerships with Chinese firms. However, many foreign businesses confront the liability of foreignness of operating in China. This paper examines how cultural, institutional, and socio-economic influences are communicated through Chinese business people's intentions to engage in business with foreign firms. Interviews and a survey were conducted with business executives across different industries and regions in China. The findings show that Chinese business people's intentions to engage in business with foreign firms are most likely to be influenced by the extent of personal trust, institutional network ties, government support, and common understanding of business norms. This study generates insights on how foreign ventures can minimize the liability of foreignness and effectively manage business relationships with Chinese partners through effective communication. The author discusses intergroup contact as a communication phenomenon in interpersonal and mediated communication. The idea of the existence of conditions for intergroup contact that mitigate perceptions across issues such as gender, generation and race is explored. The systematic approach of the contact hypothesis is brought forward to a more comprehensive theory of intergroup contact. Mediated communication is also examined through concepts such as extended contact and imaginary contact.

An introduction to a series of articles about communication is presented. One article discusses the synthesis of terror management theory (TMT) with work for fear appeals. Another article offers a critical review of the attribution theory as well as the incorporation of social psychology. One article explores the practice of deception.

Communication failures in the complex environment of hospital care affect the quality of care and occurrence of inadvertent harm. This study investigated doctors' written communication using a sample of medical records, specifically doctors' progress notes, and the frameworks of social identity and communication accommodation theories. These records include standardized and stylized language, and are intended to record assessment and treatment of patients according to known guidelines for practice. An interpretive analysis of the language and discourse in these records revealed that doctors used medical record entries both to express their specialty identity and to negotiate intergroup conflict. Nonaccommodation and interspecialty conflict sometimes took precedence over facilitation of patient treatment and management. Thus, intergroup communication in this context can constitute a serious threat to the quality of patient care. [ABSTRACT FROM AUTHOR]

Rumors collected from a large public hospital undergoing change were content analyzed, and a typology comprising the following five broad types of change-related rumors was developed: rumors about changes to job and working conditions, nature of organizational change, poor change management, consequences of the change for organizational performance, and gossip-rumors. Rumors were also classified as positive or negative on the basis of their content. As predicted, negative rumors were more prevalent than positive rumors. Finally, employees reporting negative rumors also reported more change-related stress as compared to those who reported positive rumors and those who did not report any rumors. Organizational communication covers an eclectic mix of approaches, theories, and methodologies, developed with in organizational settings or applied from other areas. The authors start their assessment of recent literature with 6 challenges for the field that have been delineated in previous reviews: (a) to innovate in theory and methodology, (b) to acknowledge the role of ethics, (c) to move from micro- to macro level issues, (d) to examine new organizational structures and technologies, (e) to understand the communication of organizational change, and (f) to explore diversity and intergroup aspects of communication. All 6 challenges implicate the importance of considering the intergroup level of analysis as well as the interpersonal and organizational levels, to undertake multilevel research in con- text, and to consider the role and place of voice in organizations. This study examines the role of social group processes in perceptions of effective communication in Australian Cooperative Research Centres (CRCs). Communication professionals in 25 CRCs discussed the barriers and opportunities for communication in their diverse networked organizations. Thematic analysis of the transcripts highlighted the contribution of social group processes to both barriers and opportunities. Communication challenges implicated the social identity of organizational members, many of which were associated with distinct structural aspects of these organizations. Opportunities for communication frequently involved features that implicated social identity, including taking advantage of existing organizational or industry identities, preventing conflicting identities from becoming salient, and promoting a collective CRC identity. The December 31, 2003 issue of the "Australian Journal of Communication," does much to bring Australian and New Zealand research, theory and opinion about disability and diversity to the fore. Papers published in this edition engage a plethora of issues around determining the intergroup and interpersonal relations around disability; the problematic of disability and identity; the need for and impact of social change, education, law and legal issues; and the pivotal role of the media. When one considers disability as diversity, the enormous diversity among people with disabilities themselves comes immediately to the fore. Diversity appears on every conceivable dimension. The diversity among people with disabilities also means that there are serious clashes of viewpoints, goals and action, which can threaten the solidarity of advocates for more recognition and a better deal. Disability is diversity; and disability matters in achieving a diverse community. Theory and research must accommodate this disagreement.

Problem: Postnatal screening rates to detect type two diabetes following gestational diabetes are low. The quality of communication is an important element to consider in developing targeted strategies that support women in completing recommended follow-up care.Aims: To explore the communication perspectives, practices and preferences of women, hospital clinicians and general practitioners, to determine strategies that may promote completion of recommended postnatal GDM follow-up, in Queensland Australia.Method: We used an exploratory, three-phase, mixed-methods approach, interpreted through intergroup communication theory. Phase one: convergent interviews explored perspectives of the communication experience in GDM care among new mothers (n = 13), hospital clinicians (n = 13) and general practitioners (n = 16). Phase two: a retrospective chart audit assessed current practice in postnatal discharge summaries of women (n = 86). Phase three: an online survey identified the preferences of general practitioners and hospital clinicians who provide maternity care in Queensland. Triangulation of the findings from the interviews, audit and surveys was used to clarify results and increase the robustness of the findings.Results: Three themes: Seeking information, Written hospital discharge summary (discharge summary) and Clarity of follow-up requirements, provide direction for pragmatic strategies to promote follow-up. Practical recommendations include continued discussion about care with women from the point of GDM diagnosis into the postnatal period; discharge summaries that give primacy to diagnosis and ongoing treatment; and provision of explicit directions for recommended testing and timing.Implications: This research informs seven practical recommendations to help promote completion of recommended postnatal GDM follow-up.
