Objectives: To investigate the physiological pattern of sleep through polysomnography, to verify pulmonary function and ventilatory muscle strength through spirometry and manovacuometry, and to perform the translation, cultural adaptation and validation of two specific scales to evaluate the clinical evolution of patients with MG, denominated "Quantitative Test for Myasthenia Gravis" and "Composite Scale for Myasthenia Gravis". Material and methods: This is a cross-sectional study involving patients with MG. Clinical evaluations were performed at the UNINOVE Sleep Laboratory, at the Neuromuscular Diseases Research Unit at UNIFESP, at the Santa Casa de S??o Paulo Department of Neurology and at the Department of Neurology at the Hospital do Servidor P??blico Estadual of S??o Paulo. Results: The results of this thesis are separated into three studies. Study I: article entitled "Sleep, lung function and quality of life in patients with Myasthenia gravis: A cross-sectional study" published in the journal Neuromuscular Disorders. Study II: entitled "Brazilian- Portuguese translation, cross-cultural adaptation and validation of the Myasthenia Gravis Composite scale. A multicentric study." published in the journal Arquivos de Neuropsiquiatria. Study III: entitled "Quantitative Myasthenia gravis Score: a Brazilian multicenter study for translation, cultural adaptation and validation", published in the journal Arquivos de Neuropsiquiatria. Conclusion: Patients with MG presented a considerable presence of DRS with worsening quality of life. They presented reduced values in the maximum ventilatory pressures associated with normal values of pulmonary function. The translation and validation of the scales were performed successfully and we believe that the use of our Portuguese version from Brazil will assist in the care of patients with MG, including a more accurate monitoring of the clinical rehabilitation process. Introdu????o: Nossa hip??tese ?? que os pacientes com MG apresentam um sono de m?? qualidade devido a presen??a de dist??rbios respirat??rios, comprometido pela fraqueza dos m??sculos ventilat??rios acarretando em piora da condi????o clinica e qualidade de vida. Objetivos: Investigar o padr??o fisiol??gico do sono atrav??s da polissonografia, verificar a fun????o pulmonar e a for??a muscular ventilat??ria atrav??s de espirometria e manovacuometria e realizar a tradu????o, adapta????o cultural e valida????o de duas escalas espec??ficas para avaliar a evolu????o cl??nica de pacientes com MG, denominados ???Teste Quantitativo para Miastenia gravis" e "Escala composta para Miastenia gravis". Material e m??todos: Trata-se de um estudo transversal envolvendo pacientes com MG. As avalia????es cl??nicas foram realizadas no Laborat??rio do sono da UNINOVE, no Setor de Investiga????o de Doen??as Neuromusculares da UNIFESP, no Departamento de Neurologia da Santa Casa de S??o Paulo e no Departamento de Neurologia do Hospital do Servidor P??blico Estadual de S??o Paulo. Resultados: Os resultados desta tese s??o separados em tr??s estudos. Estudo I: artigo intitulado "Sono, fun????o pulmonar e qualidade de vida em pacientes com Miastenia gravis: Um estudo transversal" publicado no peri??dico Neuromuscular Disorders. Estudo II: intitulado "Tradu????o, adapta????o cultural e valida????o da escala Composta de Miastenia gravis para a l??ngua portuguesa do Brasil: estudo multic??ntrico" publicado no peri??dico Arquivos de Neuropsiquiatria. " publicado no peri??dico Arquivos de Neuropsiquiatria. Conclus??o: Os pacientes com MG apresentaram uma presen??a consider??vel de DRS com piora na qualidade de vida. It is also known that the amount of nocturnal rostral fluid displacement from the lower limbs is related to increased neck circumference and the severity of obstructive sleep apnea (OSA) in patients with end-stage renal disease. Method: A cross-sectional study was carried out to verify the degree of upper airway collapsibility in patients with ESRD. Negative expiratory pressure and baseline nocturnal polygraphy will be performed before and after the hemodialysis session. Results: There was a significant difference in pre and post hemodialysis in the various apnea / hypopnea index, neck circumference, waist circumference, percentage of exhaled flow at the time of 0.2 seconds (V0.2) sitting and lying down. The VO, 2 sitting pre-hemodialysis presented a correlation with the pre-AHI (p = 0.047, r = -0.953) and post hemodialysis (p = 0.010; r = -0.990). In addition, V0.2 after hemodialysis showed a correlation with AHI after hemodialysis (p = 0.029; r = -0.971). Discussion: The NEP test was promising for clinical use, since it has a rapid application and presents information relevant to the collapse of the airway of patients with suspected OSA. Introdu????o: Atualmente, a doen??a renal em estagio terminal (DRET) ?? um dos mais s??rios problemas de sa??de p??blica, tornando-se uma epidemia global. Sabe-se tamb??m que a quantidade de deslocamento do fluido rostral durante a noite, proveniente dos membros inferiores, est?? relacionada ao aumento da circunfer??ncia do pesco??o e a gravidade da apn??ia obstrutiva do sono (AOS) em pacientes com doen??a renal em est??gio final. M??todo: Foi proposto um estudo clinico transversal com o objetivo de verificar o grau de colapsabilidade da via a??rea superior em pacientes com DRET. Foi realizado o teste da press??o expirat??ria negativa (NEP) e a poligrafia basal noturna domiciliar antes e ap??s a sess??o de hemodi??lise. Resultados: Houve diferen??a significativa em pr?? e p??s hemodi??lise nas v??riaveis ??ndice de apneia / hipopneia, circunfer??ncia de pesco??o, circunfer??ncia de cintura, porcentagem de fluxo exalada no momento de 0,2 segundos (V0,2) sentado e deitado. Material and Method: Thirty children with spastic hemiparesis, aged 5 to 12 years, were randomly allocated into three groups: Group 1: anodic tDCS over the primary motor cortex (M1) and cathode over the contralateral supraorbital region associated with Treadmill training; Group 2: anodic sham tDCS over the M1 and cathode over the contralateral supra-orbital region associated with treadmill training; Group 3: Anodal tDCS over M1 of the injured hemisphere and cathodal stimulation in contralateral M1 associated with treadmill training. The evaluation (before, after and one month after the protocol) consisted of three-dimensional gait analysis, static balance, functional balance, functional performance, quality of life and electromyography of the lower limbs. The protocols were performed for two consecutive weeks, with five training sessions per week and duration of 20 minutes per session. Estudo III: artigo de protocolo de estudo com metodologia de interven????o com estimula????o transcraniana por corrente continua associada a treino motor de realidade virtual, publicado no peri??dico BMJOPEN. Estudo VI: Os resultados da an??lise da cinem??tica do movimento de alcance manual entre crian??as com desenvolvimento t??pico e Sindrome de Down apresentaram diferen??as no aspecto da velocidade, tempo e acur??cia do movimento. Estudo V: Os resultados deste estudo demonstraram que o grupo de crian??as com SD que recebeu o treinamento com o protocolo de 10 sess??es de 20 minutos de ETCC an??dica sobre o c??rtex motor prim??rio (C3 e C4) com montagem extracefalica bilateral apresentou respostas positivas a esta interven????o nos aspectos funcionais e do movimento dos MMSS nas avalia????es realizadas logo ap??s o t??rmino do protocolo, bem como ao follow-up de um m??s p??s interven????o em compara????o com o grupo que recebeu ETCC Sham durante o mesmo tipo de treino motor. Material and Methods: Study I: A systematic review was performed of articles published between 2011 and 2016 retrieved from four databases using the following keywords and synonyms: Parkinson???s Disease, Gait, and Virtual Reality Exposure Therapy. Study II: Detailed description of the study protocol. Study III: A population-based sample was composed of 37 individuals with Parkinson???s randomly allocated to three groups (control, treadmill and virtual reality). The participants were evaluated on four occasions: pre-intervention; immediately after first intervention session; post-intervention; and one month after last intervention session). Evaluations involved the administration of the Hoehn & Yahr scale, the motor section of the Unified Parkinson???s Disease Rating Scale and Parkinson ???s disease Questionnaire (PDQ-39 for the evaluation of quality of life). Long-distance walking was evaluated using the Six-Minute Walk Test with the aid of the G-WALK (BTS) inertial sensor and monitoring of heart rate, blood pressure and perceived exertion (modified Borg scale). Training was performed in 20-minute sessions at a frequency of three sessions per week for four weeks. Study IV: Thirty-seven individuals with Parkinson???s were allocated to a control group, treadmill group and Xbox group Training in each group was performed in 12 sessions. The participants were evaluated on four occasions: pre-intervention; immediately after first training session; after 12 sessions; and one month after last training session). The evaluations involved the administration of the Hoehn & Yahr scale, the motor section of the Unified Parkinson???s Disease Rating Scale, Parkinson???s Disease Questionnaire (PDQ-39 for the evaluation of quality of life), the Berg Balance Scale, and the Tinetti Test. For the evaluation of spatiotemporal gait variables, the Timed Up and Go test was performed using the G-WALK (BTS) inertial sensor. Results: Study I: Twenty-one studies with a methodological quality of 5 or more points on the PEDro scale were analyzed and the review was published in the journal Archives of Neuropsychiatry. Study II: Protocol study published in the journal Manual Therapy. Study III: Significant improvements in the distance travelled and velocity on the Six-Minute Walk Test were found in the virtual reality and treadmill groups in comparison to the control group (p < 0.05), whereas no significant differences were found between the virtual reality and treadmill groups. Study IV: Improvements in the Berg Balance Scale, Tinetti balance and cognition subscales and mobility domain of the PDQ-39 were found in the treadmill and Xbox groups in comparison to the control group. Conclusion: The results of Studies III and IV suggest that, when used with the aim of improving mobility, balance and spatiotemporal gait variables in individuals with Parkinson???s disease, training with virtual reality is as effective as treadmill training. Objetivo: Verificar o efeito do treino de marcha com realidade virtual em compara????o com treino de marcha em esteira e treino de marcha livre sobre o equil??brio e a marcha de indiv??duos com doen??a de Parkinson (DP). Material e m??todo: Estudo I: Foi realizada uma revis??o sistem??tica com uma busca em quatro bases de dados com as seguintes palavras chave e seus sin??nimos: doen??a de Parkinson, Marcha, Terapia de exposi????o ?? realidade virtual, publicados entre 2011 e 2016. Estudo II: Descri????o detalhada do protocolo do estudo. Estudo III: A amostra populacional foi composta de 37 indiv??duos com DP, alocados randomicamente em tr??s grupos (Grupo Controle; Grupo Esteira e Grupo Realidade Virtual). Os participantes foram avaliados: pr??-interven????o, efeito imediato, p??s-interven????o e um m??s ap??s a ??ltima interven????o. A avalia????o foi composta pela Escala de Hoehn e Yahr, pela se????o motora da Escala Unificada de Avalia????o da Doen??a de Parkinson e a percep????o da qualidade de vida pelo question??rio de Qualidade de Vida na Doen??a de Parkinson (PDQ-39). Os treinos foram realizados durante quatro semanas, com tr??s sess??es por semana, com dura????o de 20 minutos de treino. Thus, the objective of this study was to evaluate the pain, cervical mobility and electromyographic activity of the upper trapezius (UT) muscle in patients with NS-NP pre and post immediately visceral manipulation (VM) and after 7 days. This study was a placebo-controlled, randomized, blind study of subjects with IC. Thirty individuals with NS-NP were divided into two groups: Manipulation Group: 15 subjects treated with visceral manipulation and Placebo Group: 15 subjects receiving placebo treatment. The possible effects of the therapy were verified by the analysis of the pain performed through the Numeric Pain Rating Scale (NPRS) and by the measurement of the area of pain, the electromyographic activity of the UT muscle and the cervical mobility. In the analysis of mobility and pain verified by the area of pain, NPRS and algometry, no significant difference (p> 0.05) was found between the studied groups. To assess the immediate effects of two related techniques, transcranial direct-current stimulation (tDCS) and functional electrical stimulation (FES), the electrical activity of the tibialis anterior (TA) and static balance of individuals with post stroke hemiparesis. Methods: clinical trial, controlled, randomized, double-blind, 30 individuals hemiparesis due to stroke. Rating: median frequency and RMS TA by electromyography (EMG) and evaluation of static balance (area, speed and frequency of oscillation in open eyes (OE) and closed (CE)) by stabilometry. Interventions: 4 protocols with 48h interval: 1 (tDCS anodic + FES sham), 2 (tDCS sham + FES active), 3 (tDCS anodic + FES active) and 4 (tDCS sham + FES sham). The anode was applied on etcc C3 and / or C4 and cathode on the contralateral supraorbital region and FES on hemiparetic TA for 20 minutes. Results: There was no statistically significant difference (p> 0.05) of the median frequency and RMS for the acquisition of isotonic and maximal voluntary isometric contraction (MVIC) of TA and static balance both OE and CE condition of the protocols by the Friedman test . Conclusion: There was no difference in the electrical activity of the TA muscle and static balance after application of the associated technical or isolated. The application time may not be enough to verify a motor learning, moreover, it is suggested that the tDCS may have a cortical hyperexcitability generated response any competitor inhibiting action; and a second hypothesis is that the stimulation of both the FES or active contraction has generated supposed peripheral fatigue, even with preconization contractions to avoid that this factor. As treatment , there are therapeutic approaches aimed at normalizing muscle tone and prevent change in the relation between bone growth and muscle, that can lead to poor posture and structured deformities. The aim this study was to investigate the effects of botulinum toxin type A (TB-A) for spasticity and functionality in spastic CP childrem. This was a prospective, controlled, randomized, consisting of 24 children (aged 5 to 12 years) with spastic CP, being divided into two groups: experimental group (EG) consisting of 12 patients (mean age 7.83 ?? 2, 07 years) treated with TB-A toxin and physiotherapy and control group (CG) with 12 patients (mean age 8.50 ?? 2.17 years) treated only with physical therapy. All participants were assessed through motor and functional scales (GMFCS, GMFM-88, Ashworth, Berg Balance Scale, Time up and go -TUG and Inventory Assessment Pediatric Disorders - ASK) at three different times: before treatment, 30 days and 3 months after the treatment proposed. By analysis of variance (ANOVA) for repeated measures it was observed significant differences (p < 0.001) between groups (groups vs treatment) to the data obtained in the GMFCS, GMFM-88, BERG, TUG, Ashworth and ASK, being observed functional improvement only for the GE group. In this study it was concluded that the use of TB-A provides a significant improvement on spasticity and child functionality with spastic CP, over a period of up to 3 months after application. The children were divided randomly into two groups (control group: mobility training with virtual reality and use transcranial stimulation placebo; Experimental Group: mobility training using virtual reality and transcranial stimulation active) and evaluated at four different times (pre -intervention, immediately after the first session, after the intervention and one month after intervention). Results: The analysis of the immediate effect of treatment with transcranial Direct Current Stimulation (tDCS) only sway velocity showed a significant interaction. The sample population that was part of this project consisted of 24 children with CP between five and ten years old. Children were be randomly allocated into two groups (Group 1: gait training on the treadmill with transcranial stimulation activates, Group 2: gait training on the treadmill with transcranial stimulation placebo) and evaluated at three different times (pre-intervention, immediately after and one month after the proposed training). The evaluation consisted of the quantification of static balance, functional balance (Pediatric Balance Scale) and functional performance (Inventory assessment of pediatric disorders). The trainings was done for two consecutive weeks, with five training sessions per week, for 20 minutes per session. The transcranial stimulation and treadmill training was realized simultaneously, in each session during twenty minutes, with an intensity of 1 mA. The results were statistically analyzed assuming a significance level of 0.05 (p < 0.05). The experimental group exhibited better results in comparison to the control group with regard to anteroposterior sway (eyes open and closed; p,0.05), mediolateral sway (eyes closed; p,0.05) and the Pediatric Balance Scale both one week and one month after the completion of the protocol. Gait training on a treadmill combined with anodal stimulation of the primary motor cortex led to improvements in static balance and functional performance in children with cerebral palsy.
