Ailevi Akdeniz ateşi otozomal resesif geçişli; tekrarlayan ateş ve peritonit,artrit, plörit gibi poliserözit atakları ile karakterize otoinflamatuar bir hastalıktır.Türk, Ermeni, Arap ve Yahudi gibi Akdeniz kökenli toplumlarda sıkgörülmektedir.Hastalıkla ilgili gen (MEFV geni) 1997‟de iki grup tarafındantanımlanmıştır. Genotiplendirme AAA‟nın çok farklı klinik bulgular ile ilişkiliolduğunu göstermiştir.Bu çalışmanın amacı kliniğimizde AAA tanısı ile izlenen hastaların klinik,laboratuar, genetik ve tedavi sonuçlarının değerlendirilmesi, genotip fenotipilişkisinin araştırılmasıdır.Çalışmaya 2013-2015 yılları arasında Ankara Üniversitesi Tıp FakültesiÇocuk Romatoloji Polikliniği‟nde AAA tanısı ile izlenen hastalar dahil edildi vehastaların dosyaları geriye dönük olarak incelendi.Genotip fenotip değerlendirmesi yapmak amacıyla hastalar mutasyonsonuçlarına göre alt gruplara ayrıldı. Hastalar iki mutasyonu ve tek mutasyonuolanlar, 2 mutasyonu olan hastalar; M694V homozigot, M694V/diğer mutasyon,M694V mutasyonu taşımayanlar ve tek mutasyonu olan hastalar; M694V taşıyanve taşımayanlar olarak sınıflandırıldı. Gruplar arasında demografik, klinik velaboratuar bulguları, tedavi yanıtı, hastalık ağırlık skoru parametreleri açısındankarşılaştırmalar yapıldı.Çalışmaya AAA tanısı ile izlenen 317 (170 kız, 147 erkek, ortalama yaş12,5±5,7) hasta dahil edildi.Hastaların 299‟unda genetik inceleme yapılmış; 181‟inde (%61) ikimutasyon, 82‟sinde (%27) tek mutasyon saptanmıştır. Otuzaltı hastada (%12)mutasyon saptanmamıştır. İki ve tek mutasyonu olan AAA hastalarının karşılaştırıldığında; ikimutasyonu olan hastaların ailesinde AAA görülme sıklığı, son kullandığı kolşisindozu, PRAS skoru ve şiddetli hastalık sıklığı oranının istatistiksel olarak anlamlıderecede yüksek olduğu görüldü. İki mutasyonu olan hastalar M694V mutasyonu taşımalarına göredeğerlendirildiğinde M694V taşıyan hastalarda ateş semptomunun daha sıkolduğu saptandı. Homozigot M694V mutasyonu taşıyan hastaların yakınmalarınındaha erken yaşta başladığı ve bu hastalarda eşlik eden hastalık, egzersizle bacakağrısı ve topuk ağrısı görülme sıklığının anlamlı derecede daha yüksek olduğugörüldü. Bu hastaların son kullandıkları kolşisin dozları ve PRAS skoru diğergruplardan anlamlı derecede daha yüksek, hafif PRAS evresi sıklığı ise diğergruplardan daha düşük saptandı. M694V heterozigot mutasyonu olan hastalarda artralji, egzersizle bacakağrısı, topuk ağrısı görülme sıklığı diğer heterozigot mutasyonu olanlara göreanlamlı derecede daha yüksek bulunmuştur. Sonuç olarak AAA‟da genotip fenotip ilişkisinin olduğu; özellikle ikimutasyon taşıyan hastaların tek mutasyonu olanlara göre ve homozigot M694Vmutasyonu taşıyan hastaların diğer iki mutasyon taşıyan hastalara göre daha ağır hastalar olduğu görüldü. M694V mutasyonunun kas iskelet sistemi yakınmaları ileilişkili olduğu saptandı. Familial Mediterranean fever (FMF) is an autosomal recessive and autoinflammatorydisease, characterized by recurring attacks of fever and polyserositissuch as peritonitis, arthritis and pleuritis. It is frequently seen in Mediterraneanpopulations such as Turkish, Armenian, Arabic ve Jewish.The gene of the disease (MEFV gene) was identified by two groupsin1997. Genotyping has showed that FMF is associated with various clinicalfindings.The aim of this study was to evaluate the clinical, laboratory, genetical andtreatment findings of patients with FMF who were followed in our clinic and toinvestigate the effect of genotype on phenotype.Files of patients with FMF, who were followed in Ankara UniversityMedical Faculty Pediatric Rheumatology outpatient clinics between the dates of2013 and 2015, were evaluated retrospectively.Patients were classified into subgroups according to genetic results inorder to make a genotype - phenotype evaluation. Patients were categorized ashaving two mutations and one mutation, patients with two mutations were categorized as having homozygous M694V, M694V/other mutation and withoutM694V mutation and patients with one mutation were categorized as having andnot having M694V. Comparisons were done among groups in terms ofdemographic, clinical and laboratory findings, treatment response, diseaseseverity scores.317 patients (170 female, 147 male, average age 12,5 ± 5,7 years) with thediagnosis of FMF were included to the study.Genetic analysis was done in 299 patients; 181 (61%) had two mutationsand 82 (27%) carried one mutation. No mutation was detected in 36 (12%)patients.Patients with two mutations had more family history of FMF, neededmore colchicine inorder to control attacks, had higher PRAS scores and higherfrequency of severe disease according to PRAS scores when compared to patientswith heterozygous mutations.Among patients with two mutations; fever was more frequently detected inpatients with M694V. Patients with homozygous M694V mutation had diseaseonset at earlier ages and associated diseases, leg pain and ankle pain were morefrequently detected in these patients. Moreover, final colchicine dosages andPRAS scores were found to be higher than other groups and frequency of mildPRAS phase was found to be lower in these patients. Patients with heterozygousM694V mutation had increased frequency of arthralgia, leg pain and ankle pain 67when compared to patients with other heterozygous mutations.As a conclusion, there seems to be a genotype – phenotype relation inpatients with FMF; especially patients with two mutations when compared topatients with one mutation and patients with homozygous M694V mutations withregard to those having other two mutations had more severe disease. M694Vmutation was found to be a susceptibility factor for musculoskeletal systemcomplaints. 97
