Study Design: Retrospective cross-sectional review of a large database., Objective: Little is known regarding extension K-lines for treatment of cervical myelopathy. Therefore, this study seeks to examine differences between K-lines drawn in neutral and extension., Summary of Background Data: The modified K-line is a radiological tool used in surgical planning of the cervical spine. As posterior cervical decompression and fusion often results in patients being fused in a more lordotic position than the preoperative neutral radiograph, a K-line measured in the extension position may offer better utility for these patients., Materials and Methods: Total of 97 patients were selected with T2-weighted, upright cervical magnetic resonance imaging taken in neutral and extension. For each patient, the K-line was drawn at the mid-sagittal position for both neutral and extension. The distance from the most posterior portion of each disk (between C2 and C7) to the K-line was measured in neutral and extension and the difference was calculated. Paired t test was used to assess significant differences., Results: Across all levels between C2 and C7 there was an increase in the distance between the dorsal aspect of the disk and K-line when comparing neutral and extension radiographs. The average change in difference (extension minus neutral) at each cervical spinal level was 0.9 mm (C2-C3), 2.5 mm (C3-C4), 2.6 mm (C4-C5), 2.0 mm (C5-C6), and 0.9 mm (C6-C7). A paired t test showed that the K-line increase from neutral to extension was statistically significant across all disk levels ( P <0.001)., Conclusion: When positioned in extension, patients experience a significant increase in distance from the dorsal aspect of a disk to the K-line compared to when positioned in neutral, especially between C3 and C6. Purpose: To evaluate the reliability and validity of the multi-positional magnetic resonance imaging in measuring occipitocervical parameters using the standard cervical dynamic radiographs as a reference., Methods: Patients were included if they underwent both dynamic radiograph and cervical multi-positional MRI within a 2-week interval from January 2013 to December 2016. Twelve occipitocervical parameters were measured on both image modalities in all positions (neutral, flexion and extension): Posterior Atlanto-Dental Interval, Anterior Atlanto-Dental Interval (AADI), Dens-to-McRae distance, Dens-to-McGregor distance, Occipito-atlantal Cobb angle (C01 angle), Occipito-axis Cobb angle (C02 Cobb angle), Atlas-axis Cobb angle (C12 angle), Redlund-Johnell, Modified Ranawat, Clivus canal angle, Occiput inclination, and Occiput cervical distance. Pearson correlation and linear regression analysis were used to evaluate the correlation of both modalities for each parameter. A p value of < 0.05 was considered statistically significant., Results: Cervical images of 70 patients were measured and analyzed. There was a significant positive correlation between dynamic X-ray and multi-positional MRI for all parameters (p < 0.05) except AADI. Dens-to-McGregor distance and Redlund-Johnell parameter demonstrated a very strong correlation in the neutral position (r = 0.72, r = 0.79 respectively) and moderate to very strong correlation(r > 0.4) for Modified Ranawat, Clivus canal angle, C02 Cobb angle and C02 distance in all neck position. The intra-class correlation (ICC) of intra- and inter-observer showed good to excellent reliability, and ICCs were 0.67-0.98., Conclusions: Multi-positional MRI can be a reliable imaging option for diagnosis of occipitocervical instability or basilar invagination compared to standard dynamic radiographs.

Study Design: Retrospective database study., Objective: To assess the intra- and postoperative complications of cervical laminoplasty and to evaluate the effect of intraoperative neuromonitoring use on postoperative limb paralysis incidence., Summary of Background Data: Cervical laminoplasty is a known procedure for the management of cervical spondylotic myelopathy (CSM)., Methods: This was a retrospective study of 532 patients with CSM who underwent cervical laminoplasty between 2007 and the first quarter of 2016 using the Humana subset of the PearlDiver Database. The database was queried using the relevant International Classification of Diseases (ICD-9 and ICD-10) codes for CSM and Current Procedural Terminology (CPT) codes for cervical laminoplasty. The intra- and postoperative incidence of surgical and medical complications and reoperations was then determined and was compared with a propensity score-matched cohort of patients who had posterior laminectomy and fusion (490 patients in each group), using multivariate logistic regression analysis., Results: Laminoplasty was associated with a lower incidence of dysphagia (odds ratio [OR] = 0.37, 95% confidence interval [CI] = 0.16-0.79; P = 0.014), 30-day readmission (OR = 0.51, 95% CI = 0.35-0.75; P < 0.001), urinary tract infection (OR = 0.58, 95% CI = 0.37-0.93; P = 0.023), and incision and drainage, exploration or evacuation (OR = 0.28, 95% CI = 0.08-0.79; P = 0.026). The use of intraoperative neuromonitoring was associated with a non-significant lower incidence of limb paralysis within 1 and 3 months postoperatively (OR = 0.52 and 0.51, 95% CI = 0.23-1.19 and 0.23-1.11; P = 0.119 and 0.091, respectively)., Conclusion: Compared with posterior laminectomy and fusion, laminoplasty had lower rates of dysphagia, urinary tract infection, and 30-day readmission. The use of intraoperative neuromonitoring was associated with a lower risk of postoperative limb paralysis, but it did not achieve statistical significance., Level of Evidence: 4.

Purpose: To evaluate which cervical level is the most appropriate level to measure occipitocervical inclination (OCI)., Methods: Sixty-two patients with multi-positional MRI: 24 males and 38 females, who had cervical lordosis and had a disk degeneration grade of 3 or less were included. We measured patient's OCI at C3, C4, and C5, occipitocervical angle (OCA), occipitocervical distance (OCD), C2-7 angle, and cervical sagittal vertical axis (cSVA) in neutral, flexion, and extension position. The correlation between OCI and OCA, OCD, C2-7 angle, and cSVA on each cervical level in all three positions was tested using Pearson's correlation coefficient test. The difference between OCIs at each cervical level was tested by Wilcoxon signed-rank test. p value of less than 0.05 was set as a statistically significant level., Results: C5 OCI showed statistically significant correlation with OCA, OCD, C2-7 angle, and cSVA in all three positions (p < 0.05, r = 0.214-0.525). C3 OCI in flexion (p = 0.393, r = 0.081) and C4 OCI in neutral and flexion (neutral p = 0.275, r 0.104; flexion p = 0.987, r = 0.002) did not show significant correlation with C2-7 angle. There was a statistically significant difference between C3, C4, and C5 OCIs in neutral and extension position (p < 0.05). At the same time, OCI showed statistically strong correlation between adjacent cervical levels (p < 0.001, r = 0.627-0.822)., Conclusion: C5 cervical level is the most appropriate level for OCI measurement. OCI should be measured at the same cervical level at all time. C4 OCI can reliably substitute C5 OCI in case C5 which is invisible on radiographic image. Study Design: This is a retrospective analysis of kinematic magnetic resonance images (kMRI)., Objective: To assess the association of cervical paraspinal muscle with cervical sagittal balance and degenerative spinal disorders., Summary of Background Data: Although the effects of spinal disorders and cervical imbalance on patient's symptoms have been well described, the relationship of the cervical muscle quality and volume to cervical imbalance or spinal disorders is not well established., Materials and Methods: In total, 100 kMRI taken in a neutral weight-bearing position were analyzed. The adjusted cross-sectional area ratio (aCSA ratio: the value of muscle CSA divided by the vertebral CSA) and fat infiltration ratio of the transversospinalis muscles at C4 and C7 vertebral levels were measured using axial slice of kMRI. The correlation with cervical balance parameters [Oc-C2 angle, C2-C7 angle, C7-T1 angle, C7 slope, T1 slope, cranial tilt, cervical tilt, thoracic inlet angle (TIA), and neck tilt] and cervical degenerative disorders (disk degeneration, Modic change, and spondylolisthesis) were evaluated., Results: The aCSA ratio at C4 correlated with C2-C7 angle (r=0.267), C7 slope (r=0.207), T1 slope (r=0.221), disk degeneration at C3-4, C4-5, C5-6 (r=-0.234, -0.313, -0.262) and spondylolisthesis at C3 (anterior: r=-0.206, posterior: r=-0.249). The aCSA ratio at C7 correlated with disk degeneration at C3-4, C4-5, C5-6, C6-7 (r=-0.209, -0.294, -0.239, -0.209). The fat infiltration ratio at C4 correlated with TIA (r=0.306) and neck tilt (r=0.353), likewise the ratio at C7 correlated with TIA (r=0.270) and neck tilt (r=0.405). All correlations above were statistically significant with P<0.05., Conclusions: The paraspinal muscle volume showed significant relationship with the cervical balance parameters and disk degeneration. While, paraspinal muscle quality related to the thoracic inlet parameters. Our findings can be an important step to develop the knowledge of the association between cervical muscle and cervical degenerative disorders, as well as the sagittal balance of the cervical spine., Level of Evidence: Level III.

Study Design: It is a retrospective analysis of 1806 consecutive cervical magnetic resonance images taken in weight-bearing flexion, neutral, and extension positions., Objective: The aim was to identify the kinematic characteristics of patients with cervical imbalance. Additionally, factors were analysed in the neutral position that could predict the characteristics. Little is known about the kinematic characteristics during cervical flexion and extension positions of the patient with cervical imbalance (cervical sagittal vertical axis (cSVA) in neutral position ≥ 40 mm)., Methods: After evaluating the whole images, cervical imbalance group (cSVA ≥ 40 mm, n = 43) and matched control group (< 40 mm, n = 43) were created using propensity score adjusting for age, gender, and cervical alignment. They were compared for cervical motion, changes in disc bulge, and ligamentum flavum (LF) bulge from flexion to extension. Multinomial logistic regression analysis and receiver operating characteristic curve analysis were calculated to verify the predictive factors and cut-off value of the identified characteristics., Results: There were no significant differences in range of motion and the change in bulged discs. There was significant difference in the presence of LF bulge from flexion to extension (p = 0.023); the incidence of LF bulge increased sharply from neutral to extension in imbalance group, while there was linear increase in control group. The canal diameter (odds ratio = 0.61, p = 0.002) and disc height (odds ratio = 1.60, p = 0.041) showed significant relationship with the segments with LF bulge observed in extension but not in neutral position in the imbalance group; the cut-off values were 10.7 mm for canal diameter (sensitivity 82.5%, specificity 66.7%) and 7.1 mm for disc height (70.8%, 58.5%)., Conclusion: Patients with cervical imbalance had a stark increase in LF bulge from the neutral to extension position. Canal diameter < 10.7 mm and disc height > 7.1 mm on neutral images may predict the segments with LF bulge observed in extension, but not in the neutral position., Level of Evidence: II (Diagnostic: individual cross-sectional studies with consistently applied reference standard and blinding). Study Design: Retrospective study of consecutive 113 cervical kinetic magnetic resonance images (kMRIs) and 57 radiographs., Objective: To elucidate the relationship between C2-6 or C2-7 Cobb angle and cervical sagittal alignment parameters using kMRI, and evaluate the visibility of C6 and C7 inferior endplates on cervical radiographs., Summary of Background Data: Several studies have used C2-6 Cobb angle instead of C2-7 Cobb angle as C7 inferior endplate is not always visible because of overlying shadows. However, the relationship between C2-6 or C2-7 Cobb angle and cervical sagittal alignment parameters remains unclear. Moreover, visibility of C6 inferior endplate remains unknown., Methods: C2-6 Cobb angle, C2-7 Cobb angle, occiput-C2 angle, Atlas-dens interval (ADI), narrowest oropharyngeal airway space (nPAS), cervical sagittal vertical axis (cSVA), cervical tilt, cranial tilt, neck tilt, thoracic inlet angle (TIA), and T1 slope were measured using kMRI, and analyzed for their relationship with C2-6 or C2-7 Cobb angle. Visibility rates of C6 or C7 inferior endplates were evaluated using cervical radiographs., Results: Linear regression analysis showed high association between C2-6 and C2-7 Cobb angle (R = 0.696, P < 0.01). C2-6 Cobb angle was significantly correlated with occiput-C2 angle, TIA, T1 slope, neck tilt, cSVA, and cervical tilt; but not with nPAS, ADI, and cranial tilt. C2-7 Cobb angle resembled C2-6 Cobb angle regarding the relationships with other parameters. In our study, 94.7% C6 and 50.9% C7 inferior endplate were clearly visible; 1.8% C6 and 24.6% C7 inferior endplate were invisible. Chi-square test and residual analysis showed significant difference between the two groups (P < 0.01)., Conclusion: C2-6 Cobb angle highly resembled C2-7 Cobb angle regarding its relationships with parameters of craniovertebral, cervical and thoracic inlet alignment. C2-6 Cobb angle could be an alternative to C2-7 Cobb angle because of its significantly higher visibility rate., Level of Evidence: 3.

Study Design: Retrospective cohort study., Objective: To demonstrate the relationship between perioperative use of catheters and the incidence of surgical site infection (SSI) after cervical spine surgery, after adjusting for patient's age, severity of comorbidity, surgical approach, and use of instrumentation., Summary of Background Data: Although the association between SSI and the use of arterial catheters (ACs) or central venous catheters (CVCs) is established in cardiac surgery, the relation in the cervical spine was not well elucidated., Methods: A private insurance database was analyzed. The incidence of SSI within 1 month postoperatively and the crude odds ratio (cOR) and 95% confidence interval (95% CI) were calculated based on the use of catheters. Subsequently, logistic regression analysis was performed to identify independent factors for SSI. Independent variables of the regression analysis included Charlson comorbidity index with the score of age, the use of CVC, the use of AC, surgical approach (anterior or posterior), and instrumentation (fusion or decompression alone)., Results: A total of 39,893 patients received cervical surgery between 2007 and 2015. Of these, 1.6% patients experienced an SSI. The incidence of SSI in patients treated with and without AC was 3.2% and 1.3%, respectively (cOR 2.44, 95% CI: 2.05-2.99, P < 0.001). Likewise, incidence of SSI in patients with and without CVC was 5.8% and 1.5%, respectively (cOR 2.61, 95% CI: 2.97-5.55, P < 0.001). Multivariate logistic regression analysis demonstrated that the adjusted OR was 1.66 in CVC use (95% CI: 1.08-2.46, P = 0.016), whereas the AC use was not significant variable (P = 0.086)., Conclusion: The use of CVC can be a potential risk factor for SSI regardless of age, severity of comorbidity, surgical approach, or presence of instrumentation. Although the essential benefits of catheters are undisputed, our data can bring up the surgeon's attention to appropriate management of the CVC., Level of Evidence: 3.

Background Context: With the changing landscape of health care, outpatient spine surgery is being more commonly performed to reduce cost and to improve efficiency. Anterior cervical discectomy and fusion (ACDF) is one of the most common spine surgeries performed and demand is expected to increase with an aging population., Purpose: The objective of this study was to determine the nationwide trends and relative complication rates associated with outpatient ACDF., Study Design/setting: This is a large-scale retrospective case control study., Patient Sample: The patient sample included Humana-insured patients who underwent one- to two-level ACDF as either outpatients or inpatients from 2011 to 2016 OUTCOME MEASURES: The outcome measures included incidence and the adjusted odds ratio (OR) of postoperative medical and surgical complications within 1 year of the index surgery., Materials and Methods: A retrospective review was performed of the PearlDiver Humana insurance records database to identify patients undergoing one- to two-level ACDF (Current Procedural Terminology [CPT]-22551 and International Classification of Diseases [ICD]-9-816.2) as either outpatients or inpatients from 2011 to 2016. The incidence of perioperative medical and surgical complications was determined by querying for relevant ICD and CPT codes. Multivariate logistic regression adjusting for age, gender, and Charlson Comorbidity Index was used to calculate ORs of complications among outpatients relative to inpatients undergoing ACDF., Results: Cohorts of 1,215 patients who underwent outpatient ACDF and 10,964 patients who underwent inpatient ACDF were identified. The median age was in the 65-69 age group for both cohorts. The annual relative incidence of outpatient ACDF increased from 0.11 in 2011 to 0.22 in 2016 (R 2 =0.82, p=.04). Adjusting for age, gender, and comorbidities, patients undergoing outpatient ACDF were more likely to undergo revision surgery for posterior fusion at both 6 months (OR 1.58, confidence interval [CI] 1.27-1.96, p<.001) and 1 year (OR 1.79, CI 1.51-2.13, p<.001) postoperatively. Outpatient ACDF was also associated with a higher likelihood of revision anterior fusion at 1 year postoperatively (OR 1.46, CI 1.26-1.70, p<.001). Among medical complications, postoperative acute renal failure was more frequently associated with outpatient ACDF than inpatient ACDF (OR 1.25, CI 1.06-1.49, p=.010). Adjusted rates of all other queried surgical and medical complications were comparable., Conclusions: Outpatient ACDF is increasing in frequency nationwide over the past several years. Nationwide data demonstrate a greater risk of perioperative surgical complications, including revision anterior and posterior fusion, as well as a higher risk of postoperative acute renal failure. Candidates for outpatient ACDF should be counseled and carefully selected to reduce these risks., (Published by Elsevier Inc.)

Study Design: Retrospective study., Objective: To evaluate how each type of sagittal cervical alignment affects the motion of the upper and subaxial cervical spine using kinematic magnetic resonance imaging., Summary of Background Data: The sagittal malalignment of the cervical spine from degeneration in the subaxial cervical spine reflects a disruption in the kinematic properties of the cervical spine and affects the motion of adjacent segments. Changes in the sagittal parameters and kinematics of the upper cervical spine and upper thoracic spine due to the kyphosis are unknown., Methods: Kinematic magnetic resonance imaging of the cervical spine in neutral position from 311 patients, including 90 lordotic, 90 straight, 90 global kyphotic, and 41 segmental kyphotic were analyzed. The lordotic angle at the upper and lower cervical spine, and T1 slope were measured in the neutral position and again in flexion and extension for dynamic analysis., Results: The number of levels with significant disk degeneration was higher in the global kyphosis group. In the global kyphosis group, neutral sagittal parameters showed some characteristics of compensation to the malalignment. Compared with the lordotic group, patients with global kyphosis demonstrated significantly higher lordotic angle of the upper cervical spine and more horizontal T1 slope. The dynamic evaluation showed greater range of motion of the entire cervical spine and subaxial cervical spine in younger patients. However, we still found greater range of motion of the occipito-atlanto-axial complex in global kyphosis, even when controlling for age and number of levels with significant disk degeneration., Conclusion: Sagittal malalignment of the cervical spine affects all parts of the cervical motion complex. The global kyphotic alignment of subaxial cervical spine affects the kinematic properties of the occipito-atlanto-axial complex and upper thoracic spine to compensate for the alteration of cervical alignment. These differences are not seen in straight and segmental kyphosis., Level of Evidence: Level 3.

Purpose: Although the T1 vertebra is considered as an important factor of cervical balance, little is known about its motion between flexion and extension. The purpose of present study was to analyze the T1 sagittal motion using kinematic magnetic resonance imaging (kMRI), and to identify factors that relate to T1 sagittal motion., Methods: We retrospectively analyzed 145 kMR images taken in weight-bearing neutral, flexion and extension positions. Cervical balance parameters were evaluated in each position. The degree of T1 sagittal motion was defined as [(T1 slope at extension) - (T1 slope at flexion)]. All patients were divided into three groups: Positive group (T1 followed the head motion, T1 sagittal motion > 5°), Stable group (5 ≥, ≥ - 5) and Negative group (T1 moved in the opposite direction from the head motion, > - 5). The groups were compared and multivariate logistic regression analysis was calculated., Results: There were 57 (40%) patients in the positive, 56 (39%) in the stable and 32 (22%) in the negative group. The positive group had the largest C2-7 sagittal vertical axis in flexion (p < 0.001) and the shortest in the extension (p = 0.023). Similar trends were seen in cranial tilt and cervical tilt. The value of T1 height < 27 mm was a significant independent factor for the negative group (p = 0.008, adjusted odds ratio = 5.958)., Conclusion: Based on T1 sagittal motion, 40% of the patients were classified in positive group (the T1 vertebra followed the head motion in flexion and extension), and 20% were classified in the negative group (the T1 vertebra moved in the opposite direction from the head motion). T1 height < 27 mm was a potential predictor of negative group.

Purpose: To test the reliability and validity of the multi-positional magnetic resonance imaging (MRI) in measuring cervical angular parameter using the standard dynamic cervical X-ray as a reference., Methods: All patients who underwent both cervical dynamic plain radiograph and multi-positional MRI on the same day between 2010 and 2016 were included in this study. The C2-7 angle and the segmental angles of the C2-3 to C6-7 segments were measured in all three positions (neutral, flexion, and extension) using multi-positional MRI and dynamic radiograph. The Pearson's correlation coefficients and linear regression analysis were used for statistical analysis., Results: 46 patients were enrolled in this study. All angular parameters showed significant positive correlation between multi-positional MRI and dynamic X-ray (p < 0.05). The angle of C2-7 showed significantly positive correlation between multi-positional MRI and X-ray (r = 0.552-0.756). All segmental angles from C2-3 to C6-7 showed moderate correlation (r = 0.401-0.636). The linear regression analysis showed that C2-7 angles and all angular parameters had significant correlation between multi-positional MRI and dynamic X-ray (p < 0.05, R 2  = 0.107-0.571)., Conclusions: The C2-7 angle and segmental cervical angles measured by multi-positional MRI were valid, and reliability substituted the dynamic X-ray measurement within the acceptable range of error. Multi-positional MRI can be used as a reliable tool for angular parameter measurement and detection of angular instability in the cervical spine.

Study Design: Retrospective analysis of consecutive 45 radiographs and 120 kinematic magnetic resonance images (kMRI) OBJECTIVE.: The aim was to assess the visibility of C7 and T1 endplates on radiographs, and to verify the correlation between C7 or T1 slope and cervical balance parameters using kMRI., Summary of Background Data: Because the T1 slope is not always visible due to the anatomical interference, several studies have used C7 slope instead of T1. However, it is still unclear whether the C7 endplate is more visible on radiographs than T1, and if C7 slope has similarity with T1 slope., Methods: The endplate visibility was determined using weight-bearing radiography. Subsequently, using weight-bearing MR images, the C7 slope of upper and lower endplate, T1 slope, C1 inclination, C2 slope, atlas-dens interval (ADI), C2-C7 lordotic angle, cervical sagittal vertical axis (cSVA), cervical tilt, cranial tilt, neck tilt, thoracic inlet angle (TIA) were measured, for the analysis of correlation between three types of slopes and cervical balance parameters., Results: 82% of the upper C7, and 18% of T1 endplate were clearly visible. The upper C7 endplate was significantly visible, whereas T1 endplate was significantly invisible (residual analysis, P < 0.01). Linear regression analysis showed correlation between the upper C7 slope and T1 slope (R = 0.818, P < 0.01) and, lower C7 slope and T1 slope (R = 0.840, P < 0.01). T1 slope significantly correlated with neck tilt, TIA, C2-C7 angle, cSVA, cervical and cranial tilt, but not with the C1 inclination, C2 slope, and ADI. Upper and lower C7 slopes showed the close resemblance with T1 slope in terms of correlation with those parameters., Conclusion: Both, upper and lower C7 slope correlated strongly with T1 slope and showed similar relationship with cervical balance parameters as T1 slope. Therefore, C7 slope could potentially substitute T1 slope, especially upper C7 slope due to the good visibility., Level of Evidence: 3.

Study Design: Epidemiologic Study., Objectives: To identify the trends in utilization of outpatient discharge for single level anterior cervical discectomy and fusion (ACDF), between 2007 and 2014, and to compare the costs and incidence of complications against a cohort of inpatients., Methods: We retrospectively reviewed 18,386 patients from the PearlDiver database from between 2007 and 2014. Discharge status was determined from billing codes. The total cost of all procedures and diagnostic tests, was determined for the global period from the time of diagnosis up until 90-days post-operatively, and the incidence of complications was recorded for 30-days., Results: The proportion of outpatient discharges was stable around 20% from 2007 to 2014 (range17-23%). The mean 90-day cost was lower for outpatients ($39,528 v. $47,330) but reimbursement fell nearly 1/3 from 2007-2014 for both groups, and the difference between the two narrowed over time ($13,745 difference in 2008, to $3,834 in 2014). Outpatients had a lower incidence of overall 30-day complications (9.5% v. 18.6%, p<0.0001), but were also significantly less comorbid (mean Charlson comorbidity index 2.32 v. 3.85, p<0.001). Older patient age, obesity, cardiac, renal, and pulmonary comorbidity were each more common in the inpatients (p<0.05 for each)., Conclusions: Outpatient discharge after ACDF is a viable treatment option with a reasonable safety profile and decreased costs relative to inpatient admission. Appropriate patient selection is key, and the standard of care nationally for the comorbid patient remains inpatient admission. The objective was to evaluate motion, disc degeneration and Space Available for Cord (SAC) at the adjacent segments of degenerative cervical spondylolisthesis (DCS) using kinematic Magnetic Resonance Imaging (kMRI). The cervical spine kMRI of sixty-one DCS spinal levels (38 anterolisthesis and 23 retrolisthesis) were analyzed at the listhesis level and its adjacent segments in three position using kMRI. MRAnalyzer3 was used to analyze translation, angular motion and SAC. The caudad level had significantly less translation motion than the listhesis level in overall DCS or grade 2 anterolisthesis group (P<0.05). The cephalad level had significant more translational motion than the caudad level in overall DCS or grade 1 retrolisthesis group (P<0.05). For disc degeneration, the cephalad level had the least disc degeneration and showed significant difference with the listhesis level in overall DCS or overall anterolisthesis or overall retrolisthesis or grade 2 anterolisthesis or grade 1 or 2 retrolisthesis (P<0.05). For SAC, the listhesis level had the narrowest space in overall DCS groups. In neutral position, grade 1 anterolisthesis had significantly larger SAC at the listhesis and the cephalad level than grade 1 retrolisthesis (P<0.05). In conclusion, DCS affected cervical spine motion and kinematics. Grade 1 retrolisthesis showed tendency of narrower SAC at the cephalad level more than the same grade anterolisthesis. Study Design: A retrospective cohort study., Objective: To compare the complications between posterior cervical fusions with and without recombinant human bone morphogenetic protein 2 (rhBMP2)., Summary of Background Data: Use of rhBMP2 in anterior cervical spinal fusion procedures can lead to potential complications such as neck edema, resulting in airway complications or neurological compression. However, there are no data on the complications associated with the "off-label" use of rhBMP2 in upper and lower posterior cervical fusion approaches., Materials and Methods: Patients from the PearlDiver database who had a posterior cervical fusion between 2005 and 2011 were identified. We evaluated complications within 90 days after fusion and data was divided in 2 groups: (1) posterior cervical fusion including upper cervical spine O-C2 (upper group) and (2) posterior cervical fusion including lower cervical spine C3-C7 (lower group). Complications were divided into: any complication, neck-related complications, wound-related complications, and other complications., Results: Of the 352 patients in the upper group, 73 patients (20.7%) received rhBMP2, and 279 patients (79.3%) did not. Likewise, in the lower group of 2372 patients, 378 patients (15.9%) had surgery with rhBMP2 and 1994 patients (84.1%) without. In the upper group, complications were observed in 7 patients (9.6%) with and 34 patients (12%) without rhBMP2. In the lower group, complications were observed in 42 patients (11%) with and 276 patients (14%) without rhBMP2. Furthermore, in the lower group the wound-related complications were significantly higher in the rhBMP2 group (23 patients, 6.1%) compared with the non-rhBMP2 group (75 patients, 3.8%)., Conclusions: Our data showed that the use of rhBMP2 does not increase the risk of complications in upper cervical spine fusion procedures. However, in the lower cervical spine, rhBMP2 may elevate the risk of wound-related complications. Overall, there were no major complications associated with the use of rhBMP2 for posterior cervical fusion approaches., Level of Evidence: Level III.

Purpose: The purpose of this study was to examine the movement of the spinal cord and its relationship to the spinal canal in patients with mild spondylosis using kinetic magnetic resonance imaging (kMRI)., Methods: Weight-bearing, multiposition kMRI was performed on symptomatic patients through a full range of flexion-extension. A total of 52 study patients were selected based on the C2-C7 Cobb angle of sagittal alignment: lordotic (from 30 to 45 degrees). We evaluated dynamic changes in different parameters from flexion-extension: spinal canal diameter (CD), spinal cord diameter (SCD), space available for the cord, anterior space available for the cord (ASAC), posterior space available for the cord (PSAC), average distance between the anterior canal and the cord (d-value), and global angle for the spinal canal and cord., Results: The CD tended to decrease from flexion to extension from C3/C4 to C6/C7, however, there were no significant differences at the proximal and distal levels, C2/C3 and C7/T1. There were no significant differences of SCD between different postures. The SCD tended to decrease from C2/C3 to C7/T1. The ASAC followed the same pattern as CD-values. The ASAC was narrowest at C4/C5 and C5/C6. The PSAC tended to increase from C2/C3 to C7/T1. The spinal cord shifted anteriorly with extension and posteriorly with flexion. In addition, the spinal cord maintained its curve with the movement., Conclusions: The kinematics of spinal cord motion may be associated with the pathogenesis of cervical spondylotic myelopathy. However, the spinal cord maintains its curve with position changes. Consequently, different motions of the cervical spine may affect spinal cord migration and cause changes in ASAC and PSAC.

Purpose: This study defined the incidence and trends of surgeries performed for patients with cervical disc degeneration with and without tobacco use disorder (TUD)., Methods: This study utilized the Humana Inc. database between 2007 and 2013 to identify patients with cervical disc degeneration with or without myelopathy. International Classification of Diseases, ninth revision (ICD-9) and Current Procedural Terminology (CPT) codes determined the initial diagnosis of disc degeneration, myelopathy status and TUD, whether patients received surgery, and TUD status at surgery., Results: The prevalence of disc degeneration with myelopathy increased by 32.8% between 2007 and 2013, while disc disease with myelopathy and TUD increased by 91.6%. For patients without myelopathy, the prevalence of disc degeneration alone increased by 65.4%, and disc degeneration with myelopathy increased by 148.7%. Of myelopathy patients, 1717 (6.4%) had TUD and 1024 (59.6%) received surgery, compared to 6508 patients without TUD (26.1%). For patients without myelopathy, 11,337 (3.5%) had TUD and 787 (6.9%) underwent surgery, compared to 9716 patients (3%) without TUD. Of surgical patients, 781 (76.3%) with myelopathy and TUD still had a TUD diagnosis at surgery, and 542 (68.9%) of patients without myelopathy still had a TUD diagnosis at surgery., Conclusions: The prevalence of degenerative disc disease and TUD has increased more than disc disease alone. Patients with TUD were more likely to get surgery, and to have surgeries earlier than patients without TUD. Patients with TUD at the time of the diagnosis of their disc degeneration likely still had a TUD diagnosis at the time of surgery.

Background Context: T1 slope is a novel thoracic parameter used to assess cervical spine sagittal balance. Thoracic index (TI) parameters including T1 slope and cervical sagittal alignment parameters may play an important role in degenerative cervical spondylolisthesis (DCS). Current literature regarding the relationship between TI and cervical sagittal alignment parameters in patients with DCS is limited., Purpose: (1) To evaluate the T1 slope, cervical sagittal alignment, and thoracic inlet parameter in patients with DCS using kinematic magnetic resonance imaging (kMRI), and (2) to find a correlation between the T1 slope, TI, and other cervical sagittal parameters in patients with DCS., Design/setting: Retrospective kMRI study, Level III., Patient Sample: Fifty-two patients with DCS from 1,128 patients from a cervical kMRI database., Outcome Measures: T1 slope, C2-C7 angle, sagittal vertical axis C2-C7 (SVA C2-C7), cranial tilt, cervical tilt, neck tilt, and thoracic inlet angle (TIA)., Methods: Cervical spine kMRIs of 52 patients with DCS (mean age 51.7±standard deviation) were analyzed in neutral, flexion, and extension positions. Patients with DCS were divided into two groups: anterolisthesis (N=33) and retrolisthesis (N=19). Each listhesis group was subclassified into grade 1 (slip 2-3 mm) and grade 2 (slip>3 mm)., Results: Grade 2 retrolisthesis had the largest T1 slope followed by grade 1 retrolisthesis, grade 2 anterolisthesis, and grade 1 anterolisthesis. Significant differences were found between the anterolisthesis and the retrolisthesis groups in the neutral position (p=.025). The flexion position had the largest T1 slope and showed a significant difference with anterolisthesis in the neutral position (p=.041). Sagittal vertical axis C2-C7 showed strong correlation with cranial tilt in all DCS groups and all positions., Conclusions: In our study, T1 slope was larger in grade 2 DCS, and the retrolisthesis group had larger T1 slope than the anterolisthesis group. Study Design: This study was an in vivo kinematic magnetic resonance imaging analysis of cervical spinal motion in human subjects., Objective: The objective of the study was to identify associations between disk degeneration in the subaxial cervical spine and upper cervical spinal motion in patients with general age-related cervical spondylosis., Summary of Background Data: The kinematic relationship between the occipital-atlantoaxial complex and subaxial cervical spine in patients with cervical spondylosis and decreased cervical motion is not well understood., Methods: A total of 446 symptomatic patients who had neck pain with and without neurogenic symptoms were included in this study. Kinematic magnetic resonance imaging was performed with dynamic motion of the cervical spine in upright, weight-bearing neutral, flexion, and extension positions. Intervertebral disk degeneration for each segment from C2-3 to C7-T1 and sagittal angular motion between flexion and extension for each segment from Oc-C1 to C7-T1 was evaluated. Depending on the amount of sagittal subaxial angular motion, the patients were classified into 3 groups by sagittal angular motion using cutoff points based on tertile (<36-degree group: 149 cases; 36-47-degree group: 148 cases; and >47-degree group: 149 cases)., Results: A significant correlation was found between subaxial angular motion and intervertebral disk degeneration, indicating that the subaxial motion decreases according to the degree of disk degeneration. Mean angular motion of the occipital-atlantoaxial complex, especially of Oc-C1, was significantly higher in the <36-degree and 36-47-degree group than in the >47-degree group, whereas no significant difference was found at C1-C2., Conclusions: Our study demonstrates that decreased subaxial cervical spinal motion is associated with intervertebral disk degeneration in a symptomatic population. This decrease in mobility at the subaxial cervical spine is compensated for by an increase in angular mobility of the upper cervical spine at the occipital-atlantoaxial complex, especially at Oc-C1.

Study Design: A retrospective kinematic magnetic resonance imaging (kMRI) study., Objective: To evaluate the utility of kMRI in determining the relationship between cervical sagittal balance and TI alignment., Summary of Background Data: Thoracic inlet parameters play an important role in cervical spine sagittal balance. However, most of the literature is based on lower resolution cervical X-rays or CT scans in the supine position., Methods: Cervical spine kMRI of 83 patients with degenerative cervical spine conditions (20-68 yr of age) was analyzed for: (1) cervical spine parameters: C2-C7 angle, C2-C7 sagittal vertical axis (SVA), cranial tilt, and cervical tilt; and (2) T1 parameters: thoracic inlet angle (TIA), T1 slope, and neck tilt (NT). Multiple logistic regression analysis and Pearson correlation coefficients were performed., Results: The mean TIA, T1 slope, and NT were 78.0, 33.2, and 44.8°, respectively. The mean C2-7 angle, SVA of C2-C7, cervical tilt, and cranial tilt were -15.4°, 22.0 mm, 18.1°, and 15.1°, respectively. The ratio of cervical:cranial tilt was maintained as 55:45%. A significant correlation was found between the C2-C7 angle and T1 slope (r = 0.731), TIA and C2-C7 angle (r = 0.406), cervical tilt with C2-C7 angle (r = 0.671), T1 slope with TIA (r = 0.429), TIA with neck tilt (r = 0.733), TIA with cervical tilt (r = 0.377), SVA C2-C7 with cervical tilt (r = -0.480), SVA C2-C7 with cranial tilt (r = 0.912), and C2-7 SVA with the ratio of cranial tilt to cervical tilt (r = 0.694)., Conclusion: An individual with a large T1 slope required large cervical lordosis to preserve physiologic sagittal balance of the cervical spine. Cranial tilt was the cervical parameter most strongly correlated with SVA C2-C7, and thus may be a good parameter to assess decompensation of cervical sagittal balance., Level of Evidence: 3.

Purpose: Few studies have reported the characteristics of Modic changes (MCs) in the cervical spine in contrast to the lumbar spine. The purpose of this study was to identify the prevalence of MCs in the cervical spine and to elucidate the relationship of MCs with spinal canal stenosis and angular motion., Methods: 437 consecutive, symptomatic patients with neck pain with or without neurogenic symptoms were included in this study. MRI in multiple positions was performed with dynamic motion of the cervical spine in upright, weight-bearing neutral, flexion and extension positions. Type of MC, intervertebral disc degeneration grade, spinal cord compression grade and sagittal angular motion between flexion and extension for each segment from C2-3 to C6-7 were evaluated., Results: MCs were observed in 84 out of 437 patients (19.2%) and in 109 out of 2,185 motion segments (5.0%) with type 2 changes predominating. Disc degeneration grades and spinal cord compression grades of segments with MCs were significantly higher than those without MCs. Sagittal angular motion of segments with MCs were significantly lower than those without MCs. Multiple logistic regression analysis revealed that significantly elevated odds ratios for MCs were observed in segments with severe disc degeneration, severe spinal canal compression and less angular motion., Conclusion: The cervical segments with MCs were significantly more likely to have disc degeneration and spinal canal stenosis. In addition, the segments with MCs had significantly less angular motion, which suggests MCs may correlate with loss of mobility.

Purpose: To determine if adding flexion and extension MRI studies to the traditional neutral views would be beneficial in the diagnosis of cervical disc bulges., Methods: Five hundred patients underwent MRI in neutral, flexion and extension positions. The images were analyzed using computer software to objectively quantify the amount of disc bulge., Results: Compared to the neutral position, cervical disc bulges were significantly increased in the extension position (P < 0.05), but on flexion position, there was no significant difference (P > 0.05). For patients without or <3 mm of disc bulge in neutral, 2.97% demonstrated an increase in bulge to ≥3 mm bulge in flexion, and 16.41% demonstrated an increase to ≥3 mm bulge in extension. For patients in the neutral view that had a baseline disc bulge of 3-5 mm, 3.73% had increased bulges to ≥5 mm in flexion and 11.57% had increased bulges to ≥5 mm in extension., Conclusion: A significant increase in the degree of cervical disc bulge was found by examining extension views when compared with neutral views alone. Kinematic MRI views provide valuable added information, especially in situations where symptomatic radiculopathy is present without any abnormalities demonstrated on traditional neutral MRI.

Study Design: Retrospective analysis of kinematic magnetic resonance images., Objective: To provide baseline data on the segmental angular and translational motion of the degenerated cervical spine by subtype of kyphotic cervical deformity and to elucidate the relationship between motion and degree of spinal cord compression., Summary of Background Data: Kyphotic deformities of the cervical spine are relatively common and are classified as either global or focal. Nevertheless, the effects of kyphotic subtype on cervical segmental motion and degree of spinal cord compression are unknown., Methods: A total of 1171 symptomatic patients (618 females, 553 males) underwent cervical kinematic magnetic resonance imaging in the neutral, flexion, and extension positions. Cervical spines demonstrating kyphosis were included and classified into 3 groups: (1) "global kyphotic deformity" (C-type) (n = 54); (2) "sigmoid deformity" (S-type) with kyphotic upper and lordotic lower cervical segments (n = 29); and (3) "reverse sigmoid deformity" (R-type) with lordotic upper and kyphotic lower cervical segments (n = 39). Translational motion, angular motion, and degree of spinal cord compression were evaluated for each cervical level along with the changes associated with flexion and extension., Results: In the C- and R-types, angular motion with extension was increased in the upper cervical spine, where there was kyphosis; when compared with the S-type, in which there was lordosis in the upper segments. The results were opposite for flexion angular motion. R-type displayed more translational motion at C3-C4 and C5-C6. Degree of static spinal cord compression of R-type was higher than the others at C3-C4. The dynamic spinal cord compression increased in extension more than flexion in all subtypes., Conclusion: Cervical spine studies that aim to investigate kyphotic deformities should make efforts to discern the different subtypes of kyphotic deformities to more accurately characterize and study the effects that the sagittal alignment has on the kinematics of the spine and the degree of spinal cord compression.

Study Design: Retrospective analysis of kinematic magnetic resonance (MR) images., Objective: To elucidate the distribution and risk factors associated with missed dynamic stenosis in cervical spine., Summary of Background Data: Motion of the cervical spine is widely accepted to be associated with cervical spondylotic myelopathy; however, the distribution and the risk factors for dynamic spinal stenosis are not well understood., Methods: A total of 435 symptomatic patients (2610 cervical segments) obtained upright kinematic MR images in neutral, flexion, and extension postures. Spinal cord compression (SCC), spondylolisthesis, disc bulging, angular motion, translational motion, disc degeneration grade, Modic changes, segmental alignment, and developmental stenosis were all evaluated. Cervical segments C2-C3 to C7-T1 were divided into 2 groups, determined by the presence of SCC. After excluding segments with SCC in the neutral position, a multivariate logistic regression model was used to evaluate for associated risk factors of SCC in flexion and extension that were not present in the neutral position., Results: SCC in neutral position was observed in 5.3% (139/2610) of segments. After excluding these segments, missed dynamic stenosis was found in 8.3% (204/2471) of segments in extension and 1.6% (40/2471) in flexion. Missed dynamic stenosis in both extension and flexion was most frequent at C5-C6. Multivariate logistic regression analysis for dynamic stenosis in extension revealed that disc bulge greater than 2.4 mm, angular motion greater than 4.8°, moderate and severe disc degeneration, segmental kyphosis, and developmental stenosis were significant risk factors. In flexion, significant risk factors were a disc bulge of 1.9 mm or greater, moderate to severe disc degeneration, and segmental kyphosis., Conclusion: Dynamic cord compression was most common at the C5-C6 segment. Cervical segments with greater disc bulge, more severe disc degeneration, greater angular motion, segmental kyphosis, and developmental stenosis may be at risk for missed dynamic stenosis.

Study Design: Retrospective review., Objective: The objective of the study was to determine the morbidity and mortality rate associated with same day versus staged cervical circumferential approach., Summary of Background Data: A combined approach to the cervical spine is often indicated for complex cervical pathologies. Previous studies suggested superior results associated with same day combined surgery for thoracolumbar patients. This study examines the usefulness of p-Physiological and Operative Severity Score for enumeration of Morbidity and Mortality (POSSUM), an estimated mortality risk assessment for cervical spine patients and will compare same day surgery to staged procedures., Methods: This is a retrospective chart review including patients who underwent ventral and dorsal approach within 2 weeks. Estimated mortality was calculated using p-POSSUM. The cohort was divided into same day surgery group and staged group. Risk factors were compared between groups. Mean p-POSSUM was calculated and compared with the actual mortality rate. Univariate analysis was used to compare the risk factors between groups and the groups' outcomes. Multivariable analysis was used to adjust for risk factor differences when comparing group outcomes., Results: One hundred thirty-five patients were included, 106 patients were in the same day surgery group whereas 29 patients were in the staged group. Mean p-POSSUM was 2.8% predicted mortality with a 95% confidence interval of 1.6% to 4.1%. The actual mortality rate was 3.7%. The groups did not vary in most risk factors assessed. Univariate analysis demonstrated a statistically significantly higher rate of major complications (0.62 vs. 0.34, P=0.0369), infection (41.4% vs. 9.4%, P<0.0001), and length of hospital stay (9.3 vs. 6.8 d, P=0.0120) in the staged group. Multivariable analysis demonstrated significantly higher infection rate in the staged group., Conclusions: P-POSSUM mortality estimate may serve as a useful and valid tool for spine surgery studies. Staged combined cervical surgery harbors a higher complication rate and may be associated with lengthier hospitalization.

These techniques have the theoretical advantages of reducing morbidity, blood loss, perioperative pain, and length of hospital stay associated with conventional open posterior spinal exposure., Methods: Minimally invasive access to the posterior cervical spine was performed with exposure through a paramedian muscle-splitting approach. With the assistance of a specialized tubular retraction system with a deep soft tissue expansion mechanism, multilevel posterior cervical decompression and fusion can be accomplished., Results: Minimized access to perform multilevel posterior cervical foraminotomy and fusion can be safely accomplished with tubular retraction systems. Complications associated with these techniques can include inadequate decompression, improper instrumentation placement, or neurologic injury due to poor access and visualization., Conclusion: Multilevel foraminotomy and instrumented fusion using lateral mass screw fixation can be safely achieved using these techniques. Complications associated with these strategies are typically due to inadequate visualization, incomplete decompression, or poor placement of instrumentation. As with all minimally invasive spine techniques, the surgeon must ensure that goals of the surgery, both technical and clinical outcomes, are comparable to those of a conventional open procedure.

Multiple factors contribute to the determination for surgical intervention in the setting of cervical spinal injury, yet to date no unified classification system exists that predicts this need. The goals of this study were twofold: to create a comprehensive subaxial cervical spine injury severity numeric scoring model, and to determine the predictive value of this model for the probability of surgical intervention. In a retrospective cohort study of 333 patients, neural impairment, patho-morphology, and available spinal canal sagittal diameter post-injury were selected as injury severity determinants. A common numeric scoring trend was created; smaller values indicated less favorable clinical conditions. Neural impairment was graded from 2-10, patho-morphology scoring ranged from 2-15, and post-injury available canal sagittal diameter (SD) was measured in millimeters at the narrowest point of injury. Logistic regression analysis was performed using the numeric scores to predict the probability for surgical intervention. Complete neurologic deficit was found in 39 patients, partial deficits in 108, root injuries in 19, and 167 were neurologically intact. The pre-injury mean canal SD was 14.6 mm; the post-injury measurement mean was 12.3 mm. The mean patho-morphology score for all patients was 10.9 and the mean neurologic function score was 7.6. There was a statistically significant difference in mean scores for neural impairment, canal SD, and patho-morphology for surgical compared to nonsurgical patients. At the lowest clinical score for each determinant, the probability for surgery was 0.949 for neural impairment, 0.989 for post-injury available canal SD, and 0.971 for patho-morphology. The unit odds ratio for each determinant was 1.73, 1.61, and 1.45, for neural impairment, patho-morphology, and canal SD scores, respectively. The subaxial cervical spine injury severity determinants of neural impairment, patho-morphology, and post-injury available canal SD have well defined probability for surgical intervention when scored separately. Our data showed that each determinant alone could act as a primary predictor for surgical intervention.

Multilevel cervical spondylosis resulting in myelopathy is a complex condition to treat surgically. Several anterior and posterior procedures have been described. Cervical laminaplasty is a procedure that provides multilevel posterior cord decompression while attempting to eliminate the postoperative development of instability and kyphosis by retaining the posterior elements. Because laminaplasty does not involve arthrodesis, more postoperative motion is preserved and early range of motion is encouraged to avoid stiffness. Although laminaplasty is a relatively straightforward procedure, understanding several key points can help avoid common technical challenges and ensure the best possible outcomes for patients. A variety of fixation techniques, each with advantages and limitations, are available to keep the laminaplasty door open. The surgeon should be aware of the complications associated with cervical laminaplasty.

Minimally invasive approaches and operative techniques are becoming increasingly popular for the treatment of cervical spine disorders. Minimally invasive spine surgery attempts to decrease iatrogenic muscle injury, decrease pain, and speed postoperative recovery with the use of smaller incisions and specialized instruments. This article explains in detail minimally invasive approaches to the posterior spine, the techniques for posterior cervical foraminotomy and arthrodesis via lateral mass screw placement, and anterior cervical foraminotomy. The lower cervical segments are commonly the level responsible for cervical spondylotic myelopathy; however, we rarely encounter stenosis at the upper cervical segment in a clinical setting. We assumed that there might be some differences between the pathogenetic mechanisms underlying the development of cervical canal stenosis at different segments. We performed positional MRI in the weight-bearing position for 295 consecutive symptomatic patients. All subjects were classified into four groups (A: normal; B: C3-4 stenosis; C: C5-6 stenosis; D: two-level cervical segments stenosis, stenosis at C3-4 and C5-6). Age, sagittal cervical canal diameter, cervical intervertebral disc degeneration, cervical cord compression, and cervical mobilities were evaluated for each group. Group B showed a narrow cervical spinal canal structure at the C3 to C4 pedicle levels, while groups C and D showed narrow structures at the C4 to C6 pedicle levels in the cervical spine. Additionally, the sagittal cervical canal diameters at all pedicle levels, except C7, in group D were significantly smaller than those observed in group C. We demonstrated the differences in the pathogenetic processes for the development of cervical spinal canal stenosis between C3-4, C5-6, and two-level cervical segments stenosis. Our results suggest that the developmental morphological structure of the cervical spinal canal plays an important role in the development of cervical canal stenosis at different segments. Moreover, individuals with sagittal cervical canal diameters of less than 13 mm may be exposed to an increased risk for future development of cervical spinal canal stenosis at the upper cervical segments following stenosis at the lower cervical segments.

Object: The C-7 lateral mass has been considered difficult to fit with instrumentation because of its unique anatomy. Of the methods that exist for placing lateral mass screws, none particularly accommodates this anatomical variation. The authors have related 12 distinct morphological measures of the C-7 lateral mass to the ability to place a lateral mass screw using the Magerl, Roy-Camille, and a modified Roy-Camille method., Methods: Using CT scans, the authors performed virtual screw placement of lateral mass screws at the C-7 level in 25 male and 25 female patients. Complications recorded included foraminal and articular process violations, inability to achieve bony purchase, and inability to place a screw longer than 6 mm. Violations were monitored in the coronal, axial, and sagittal planes. The Roy-Camille technique was applied starting directly in the middle of the lateral mass, as defined by Pait's quadrants, with an axial angle of 15° lateral and a sagittal angle of 90°. The Magerl technique was performed by starting in the inferior portion of the top right square of Pait's quadrants and angling 25° laterally in the axial plane with a 45° cephalad angle in the sagittal plane. In a modified method, the starting point is similar to the Magerl technique in the top right square of Pait's quadrant and then angling 15° laterally in the axial plane. In the sagittal plane, a 90° angle is taken perpendicular to the dorsal portion of the lateral mass, as in the traditional Roy-Camille technique., Results: Of all the morphological methods analyzed, only a combined measure of intrusion of the T-1 facet and the overall length of the C-7 lateral mass was statistically associated with screw placement, and only in the Roy-Camille technique. Use of the Magerl technique allowed screw placement in 28 patients; use of the Roy-Camille technique allowed placement in 24 patients; and use of the modified technique allowed placement in 46 patients. No screw placement by any method was possible in 4 patients., Conclusions: There is only one distinct anatomical ratio that was shown to affect lateral mass screw placement at C-7. This ratio incorporates the overall length of the lateral mass and the amount of space occupied by the T-1 facet at C-7. Based on this virtual study, a modified Roy-Camille technique that utilizes a higher starting point may decrease the complication rate at C-7 by avoiding placement of the lateral mass screw into the T1 facet.

Study Design: Retrospective radiographic review., Objective: To investigate the effects of cervical disc herniation on kinematics at adjacent vertebral motion segments., Summary of Background Data: Kinetic magnetic resonance imaging (kMRI) is an alternative method to conventional MRI, which allows evaluation of the cervical spine in a more physiologic, weight-bearing position, and acquisition of images in flexion, extension, and neutral alignment. kMRI has previously been used to evaluate the effects of disc degeneration on cervical kinematics., Methods: A total of 407 patients with neck pain without prior history of surgery were evaluated using kMRI. Translational motion, angular variation, and disc height were measured at each segment from C2-C3 through C7-T1. Other factors including the degree of disc degeneration, age, gender, and vertebral segment location were analyzed in order to determine any predisposing risk factors for segmental instability adjacent to disc herniations., Results: Spinal levels above the disc herniation exhibited, on average, a 7.2% decrease in translational motion per mm of disc herniation (P = 0.0113), without significant change in angular motion. Levels below the herniation demonstrated a 5.2% decrease in angular motion per mm of disc herniation (P = 0.0236) without significant change in translational motion. The degree of disc degeneration had no significant effect on adjacent level motion. Disc herniation had no significant impact on disc height at adjacent levels, although disc degeneration correlated with decreased disc height above and increased disc height below., Conclusion: Although disc height, translational motion, and angular variation are significantly affected at the level of a disc herniation, no significant changes are apparent in adjacent segments. Our results indicate that herniated discs have no effect on ROM at adjacent levels regardless of the degree of disc degeneration or the size of disc herniation, suggesting that the natural progression of disc degeneration and adjacent segment disease may be separate, unrelated processes within the cervical spine.

A congenitally narrow cervical spinal canal has been established as an important risk factor for the development of cervical spondylotic myelopathy. However, few reports have described the mechanism underlying this risk. In this study, we investigate the relationship between cervical spinal canal narrowing and pathological changes in the cervical spine using positional magnetic resonance imaging (MRI). Two hundred and ninety-five symptomatic patients underwent cervical MRI in the weight-bearing position with dynamic motion (flexion, neutral, and extension) of the cervical spine. The sagittal cervical spinal canal diameter and cervical segmental angular motion were measured and calculated. Each segment was assessed for the extent of intervertebral disc degeneration and cervical cord compression. Based on the sagittal canal diameter, the subjects were classified into three groups: A, subjects with a congenitally narrow canal, diameter of less than 13 mm; B, subjects with a normal canal, diameter of 13-15 mm; C, subjects with a wide canal, diameter of more than 15 mm. When compared with Groups A and B, the disc degeneration grades at the C3-4, C5-6, and C6-7 segments and the cervical cord compression scores at the C3-4 and C5-6 segments showed significant differences. Additionally, when compare with Groups A and C, the disc degeneration grades at all segments, except C2-3, and the cervical cord compression scores at all segments, except C2-3, showed significant differences. With respect to the cervical kinematics, few differences in the kinematics were observed between Groups B and C, however, the kinematics in Group A was different with other two groups. In Group A, the segmental mobility at the C4-5 and C6-7 segments were significantly higher than those observed in Group B, and the segmental mobility at the C3-4 segment was significantly lower than that observed in Groups B or C. We demonstrated the unique pathological and kinematic traits of cervical spine that exist in a congenitally narrow canal. We hypothesize that kinematic trait associated with a congenitally narrow canal may greatly contribute to pathological changes in the cervical spine. Our results suggest that cervical spinal canal diameter of less than 13 mm may be associated with an increased risk for development of pathological changes in cervical intervertebral discs. Subsequently, the presence of a congenitally narrow canal can expose individuals to a greater risk of developing cervical spinal stenosis.

Treatment of cervical pathology requires a clear understanding of the biomechanical benefits and limitations of cervical plates, their indications, and their associated complications. The use of anterior cervical plates has evolved significantly since their early application in cervical trauma. They have become widely used for anterior cervical decompression and fusion for cervical spondylosis. Plate design has undergone significant refinement and innovation, from the initial unlocked plates requiring bicortical purchase to the latest rotationally and translationally semiconstrained dynamic plates. Excellent clinical results have been reported for single-level anterior cervical decompression and fusion with or without plate fixation; however, the addition of an anterior cervical plate clearly leads to earlier fusion and better clinical results in longer fusions. Longer fusions should ideally consist of corpectomies and strut grafting because the decreased number of fusion surfaces tends to lead to higher fusion rates. Although anterior plate fixation leads to higher fusion rates in fusions of three or more levels, the associated pseudarthrosis rate is still high. The use of dynamic plates, through increased load sharing across the graft and decreased stress shielding, may improve fusion rates, particularly in long fusions. Nevertheless, adjuvant posterior fixation is recommended for fusions of more than three vertebral levels. Anterior plate fixation may be of particular benefit in the management of traumatic injuries, in revision settings, and in the treatment of smokers. Complications unique to plate fixation include hardware breakage and migration as well as ossification of the adjacent disk levels.

Study Design: Retrospective analysis using kinetic magnetic resonance images (MRIs)., Objective: To investigate the relationship of changes in the sagittal alignment of the cervical spine on the kinematics of the functional motion unit and disc degeneration., Summary of Background Data: Normal lordotic alignment is one of the most important factors contributing to effective motion and function of the cervical spine. Loss of normal lordotic alignment may induce pathologic changes in the kinematics and accelerate degeneration of the functional motion unit. However, the relationship of altered alignment on kinematics and degeneration has not been evaluated., Methods: Kinetic MRIs in flexion, neutral, and extension were performed. Study participants were classified into 5 groups based on the C1-C7 Cobb angle of sagittal alignment--Group A: Kyphosis (n = 19), Group B: Straight (n = 29), Group C: Hypolordosis (n = 38), Group D: Normal (n = 63), and Group E: Hyperlordosis (n = 52).Intervertebral disc degeneration was graded (Grades 1-5), and the kinematics of the functional spinal unit were obtained., Results: When the alignment shifted from normal to less lordotic, the translational motion and angular variation tended to decrease at all levels. The contribution of the C1-C2, C2-C3, and C3-C4 levels to total angular mobility tended to be higher in Group C than Group D. However, the contribution of the C4-C5, C5-C6, and C6-C7 levels tended to be lower in Group C than in Group D. The grade of disc degeneration associated with loss of lordosis tended to be higher than that associated with normal alignment at the C2-C3 and C3-C4 levels., Conclusion: The present study demonstrated that the changes in sagittal alignment of the cervical spine affect the kinematics. Consequently, it may cause changes in the segment subjected to maximum load for overall motion and accelerate its degeneration.

Study Design: This was a radiographic reliability study of a novel grading system for cervical intervertebral disc degeneration., Objectives: This study aimed to develop and test the reliability of a reproducible grading system for cervical intervertebral disc degeneration on the basis of the routine magnetic resonance imaging (MRI)., Summary of Background Data: Cervical disc degeneration is common after middle age, and the morphology of cervical disc degenerative disease has often been studied using MRI. There are few specific MRI-based grading systems for cervical intervertebral disc degeneration despite the clinical importance of this problem. This study proposes a novel reproducible grading system for cervical disc degeneration and demonstrates the reliability of this classification scheme., Methods: A grading system for cervical intervertebral disc degeneration was developed based on relevant previous literature. MRI grading of 300 cervical intervertebral discs using T2-weighted sagittal images was performed by 4 spinal surgeons (observers) in a blinded fashion. Intraobserver and interobserver reliabilities were assessed by calculating kappa statistics., Results: Grade I degeneration was observed in 27 discs (9.0%); grade II, in 56 (18.7%); grade III, in 124 (41.3%); grade IV, in 67 (22.3%); and grade V, in 26 (8.7%) discs. Kappa coefficients for intraobserver and interobserver agreements ranged from substantial to excellent (intraobserver, 0.907 to 0.950 and interobserver, 0.730 to 0.826). Complete agreement was obtained, on an average, in 72.1% of the discs. A difference of 1, 2, and 3 grades was observed in 22.4%, 3.3%, and 0.2% discs, respectively., Conclusions: This grading system is comprehensive and easily applicable with sufficient reproducibility. It can be used as a common nomenclature for research and discussions.

Study Design: The sagittal kinematics of the cervical spine was evaluated using kinematic magnetic resonance imaging (kMRI)., Objective: To investigate the effect of degenerative changes in the functional spinal unit on cervical kinematics by using kMRI., Summary of Background Data: Few studies have, thus far, by using MR images, described the contribution of degenerative changes in the functional spinal unit to cervical kinematics; however, the exact cervical kinematics remains uncertain., Methods: A total of 289 consecutive symptomatic patients underwent dynamic cervical MRI in flexion, neutral, and extension postures. All digital measurements and calculations of the variations in segmental angular motion were automatically performed by an MR analyzer using true MR images with 77 predetermined points marked on each image. Each segment was assessed based on the extent of intervertebral disc degeneration (Grades 1-3) and cervical cord compression (groups A-C) observed on T2-weighted MR images., Results: The segmental mobility of the segments with severe cord compression and moderate disc degeneration tended to be lower than that of the segments with severe cord compression and severe disc degeneration, and a significant difference was observed in the segmental mobility of the C5-C6 segment. Moreover, in all segments with moderate disc degeneration, the segmental mobility was significantly reduced in the presence of severe cord compression, as compared with no compression. However, in segments with severe disc degeneration, no significant differences were observed between the segmental mobility of the cord compression groups., Conclusion: Our results suggest that cervical cord compression may cause deterioration of cervical cord function and kinematic changes in the cervical spine. We hypothesize that the spinal cord may potentially protect its functions from dynamic mechanical cord compression by restricting segmental motion, and these mechanisms may be closely related to the intervertebral discs.

Background: Cervical spondylotic myelopathy is a potentially serious neurologic disorder that commonly presents with gait difficulty and hand dysfunction. Because the development of CSM is in large part related to advanced spondylosis and degenerative disk disease, elderly patients appear to be at an increased risk to develop this condition. The surgical outcomes of this patient population have been understudied; the authors seek to report their clinical results in a series of patients with CSM older than 75 years who underwent surgical treatment., Methods: This report is composed of a cohort of 36 elderly patients (older than 75 years) and 34 younger patients (younger than 65 years) who underwent decompressive surgery for CSM at one institution between 2001 and 2005. The patients' functional status was evaluated preoperatively and postoperatively using the mJOA disability scale., Results: The mean follow-up time in the elderly group was 24 months, with a range from 12 to 48 months. There was a statistically significant improvement between mean preoperative (11.3) and postoperative (14.4) mJOA scores (P< .0001). The younger group had a higher neurologic recovery rate (71%) than the elderly group (59%); however, this was not statistically significant (P= .29). The postoperative complication rate in the elderly population (38%) was higher than in the younger group (6%; P= .002)., Conclusion: Elderly patients with CSM are likely to obtain neurologic improvement after decompressive surgery. Their postoperative complication rate is higher than that of younger patients, yet most complications appear to be self limiting and do not adversely affect neurologic outcome.

Study Design: Kinetic MRIs of cervical spines were obtained and analyzed according to the amount of motion and the degenerative grade of the intervertebral disc., Objective: To define the relationship between the grade of disc degeneration and the motion unit of the cervical spine and elucidate changes in the role of each cervical spine unit during flexion-extension motion caused by degeneration., Summary of Background Data: Degenerative changes in the cervical disc occur with age. The correlation between the degree of cervical disc degeneration and extent of cervical spine mobility has not yet been determined. The effect of degeneration on the overall motion of the functional spinal unit also remains undefined., Methods: We studied 164 patients with symptomatic neck pain. The cervical intervertebral discs were graded by spine surgeons according to the degenerative grading system (Grades I to V). All radiologic data from kinetic MRIs were recorded on a computer for subsequent measurements. All measurements and calculations for translational motion and angular variation of each segment were automatically performed by a computer analyzer., Results: The translational motion in discs with Grade II degeneration (mild degeneration) increased to Grade III degeneration (higher degeneration). However, the translational motion and angular variation significantly decreased for the Grade V (severe degeneration). For patients with relatively low grades of degeneration, Grades I and II discs, the C4-C5 and C5-C6 segmental units contributed the majority of total angular mobility of the spine. However, for the severely degenerated segments, Grade V discs, the contributions of the C4-C5 and C5-C6 U significantly decreased., Conclusion: The changes that occur with disc degeneration progress from the normal state to an unstable phase with higher mobility and subsequently to an ankylosed stage. This study evaluated the contribution of different levels to the changes in overall motion that occur with degeneration.

Introduction: The management of cervical radiculopathy has undergone significant evolution, and the most recent advancement is the integration of minimally invasive surgical techniques. There have been relatively few reports in the medical literature describing the clinical results of minimally invasive cervical spine surgery. The authors describe the surgical indications, technique, and preliminary clinical outcomes in a series of patients who underwent the 2-level minimally invasive posterior cervical foraminotomy procedure., Methods: This report is composed of 21 consecutive patients with cervical radiculopathy who underwent a minimally invasive 2-level posterior cervical foraminotomy at our institution between 2003 and 2005. Magnetic resonance imaging demonstrated foraminal or posterolateral pathology at 2 ipsilateral adjacent spinal levels in each patient. Radicular arm pain was the most common presenting symptom, and was encountered in all 21 patients., Results: The mean follow up for the patients was 23 months (range 12 to 36). Complete resolution of preoperative symptoms was achieved in 19 out of 21 patients (90%). Sixteen patients were discharged home the same day of surgery, and the mean estimated blood loss was 35 mL (range 10 to 100 mL). There were no perioperative complications., Conclusions: Minimally invasive 2-level posterior cervical foraminotomy can be safely performed on an outpatient basis with results comparable to that of conventional foraminotomy. This procedure should be considered as a potential alternative to 2-level anterior cervical discectomy and fusion or open foraminotomy in selected patients.

Study Design: A retrospective review of 33 consecutive patients treated with posterior fusion and selective nerve root decompression for the treatment of pseudarthrosis following anterior cervical discectomy and fusion., Objectives: Use standardized outcome measures to evaluate the results of posterior fusion with selective nerve root decompression as a treatment option for symptomatic pseudarthrosis of the cervical spine., Summary of Background Data: Pseudarthrosis after anterior cervical discectomy and fusion has been recognized as a cause of continued cervical pain and unsatisfactory outcomes. Debate continues as to whether a revision anterior approach or a posterior fusion procedure is the best treatment for symptomatic cervical pseudarthrosis. To our knowledge, standardized outcome measures have not been used to evaluate the results of either surgical treatment option; therefore, it is difficult to evaluate outcomes in these patients, let alone compare surgical treatment options. Data on fusion rates in these two surgical treatment groups suggest a trend of a higher fusion rate with utilization of a posterior revision procedure, but the largest study to date includes the study of only 19 patients treated with a posterior fusion., Methods: Thirty-three consecutive patients with symptomatic pseudarthrosis following anterior cervical discectomy and fusion were treated with selective nerve root decompression and posterior fusion using iliac crest or local bone graft as well as posterior wiring and/or lateral mass plating. The average follow-up period was 46 months (range, 20-86 months). Patients were assessed using physical examination, flexion-extension lateral radiographs, and standardized outcome measures including the SF-36, Arthritis Impact Measurement Scales 2, and Cervical Spine Outcomes Questionnaire., Results: All 33 patients (100%) demonstrated a solid fusion at their most recent follow-up, and all 33 patients noted significant improvement in their preoperative symptoms. No difference in fusion status was noted between those treated with iliac crest versus patients treated with local bone graft--all had a solid fusion; 72% of the patients were satisfied with the result of their surgery. Cervical Spine Outcomes Questionnaire pain scales demonstrated 52% of patients reported mild or nopain at follow-up, whereas 20% described their pain as "discomforting" and 28% of the patients continued to report moderate to severe pain., Conclusions: This is the first study to our knowledge to use standardized outcome measures to assess clinical outcome in patients treated with posterior fusion for pseudarthrosis after anterior cervical discectomy and fusion. Patients and surgeons need to understand the potential for success with this revision procedure but also be aware of the relatively high rate of continued moderate to severe pain observed in this patient population even after a solid fusion is achieved. All of the patients in this study fused with a single posterior fusion procedure, further supporting the relatively higher fusion rates observed in the literature using posterior fusion as a treatment for cervical pseudarthrosis. Our results also support the ability of surgeons to use local bone graft without iliac crest in a posterior fusion for cervical pseudarthrosis and therefore avoid the morbidity associated with iliac crest bone graft harvest.

Study Design: A retrospective review of consecutive patients with graft migration or displacement after anterior cervical corpectomy surgery was performed., Objectives: To examine the associated risk factors and results of treatment among patients who sustained graft displacement or migration after anterior cervical corpectomy surgery., Summary of Background Data: Graft migration or displacement after anterior cervical corpectomy is a potential complication that may require revision surgery, but because of the low incidence, the factors associated with graft movement and the results of treatment are not well defined., Methods: All patients who had undergone a cervical corpectomy were examined for graft migration or displacement. None of the patients had a previous cervical laminectomy or prior posterior cervical surgery. All the patients were treated with autogenous strut grafting after decompression., Results: Over a 25-year period, 249 consecutive patients underwent one- to five-level anterior cervical corpectomies and strut grafting. All the patients were fused using autogenous bone grafts (iliac crest or fibula). During the postoperative period, 16 of the patients (10 women and 6 men; average age, 61.4 years) experienced migration of their grafts. The average follow-up period was 4.7 years (range, 2-12 years). The graft migration rates increased with more levels of fusion (odds ratio of 1.65 for having a displaced graft with each additional level): 4 of 95 single-level grafts, 4 of 76 two-level grafts, 7 of 71 three-level grafts, and 1 of 6 for four-level grafts. Of the 16 patients with graft migration, 14 had procedures involving a corpectomy of C6 with a fusion inferiorly extending to the C7 vertebral body (P = 0.001, statistically significant difference). Of these 16 patients, 5 underwent revision surgeries acutely for displacement and associated fracture of the inferior graft and vertebral body junction. None of the patients sustained a neurologic or respiratory complication as a result of graft migration ordisplacement. All of the patients went on to successfulfusion., Conclusions: This study demonstrated that a greater number of vertebral bodies removed and a longer graft are directly related to an increased frequency of graft displacement. Graft displacement may require revision surgery, but no patient in this study experienced a permanent adverse result from this complication. Corpectomies involving a fusion ending at the C7 vertebral body were associated with a higher rate of graft migration.

Background: As the elderly population grows, the increasing prevalence of osteoporosis presents a unique challenge for surgeons. Decreased bone strength and quality are associated with hardware failure and impaired bone healing, which may increase the rate of revision surgery and the development of complications. The purpose of this review is to determine the impact of osteoporosis on postoperative outcomes for patients with cervical degenerative disease or deformity. Methods: A systematic review using Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and Medical Subject Headings terms involving spine surgery for cervical degenerative disease and osteoporosis were performed. This review focused on radiographic outcomes, as well as surgical and medical complications. Results: There were 16 studies included in the degenerative group and 9 in the deformity group. Across degenerative studies, lower bone mineral density was associated with increased rates of cage subsidence in osteoporotic patients undergoing operative treatment for cervical degenerative disease. Most studies reported varied results on the relationship between osteoporosis and other outcomes such as revision and readmission rates, costs, and perioperative complications. Our meta-analysis suggests that osteoporotic patients carry a greater risk of reduced fusion rates at 6 months and 1 year postoperatively. With respect to cervical deformity correction, although individual complication rates were unchanged with osteoporosis, the collective risk of incurring any complication may be increased in patients with poor bone stock. Conclusions: Overall, the literature suggests that outcomes for osteoporotic patients after cervical spine surgery are multifactorial. Osteoporosis seems to be a significant risk factor for developing cage subsidence and pseudarthrosis postoperatively, whereas reports on medical and hospital-related metrics were inconclusive. Our findings highlight the challenges of caring for osteoporotic patients and underline the need for adequately powered studies to understand how osteoporosis changes the risk index of patients undergoing cervical spine surgery. Clinical Relevance: In patients undergoing cervical spine surgery for degenerative disease, osteoporosis is a significant risk factor for long-term postoperative complications--notably cage subsidence and pseudarthrosis. Given the elective nature of these procedures, interdisciplinary collaboration between providers should be routinely implemented to enable medical optimization of patients prior to cervical spine surgery. Background: This review outlines clinical data and characteristics of current Food and Drug Administration (FDA)-approved implants in cervical disc replacement/cervical disc arthroplasty (CDR/CDA) to provide a centralized resource for spine surgeons. Methods: Randomized controlled trials (RCTs) on CDR/CDA were identified using a search of the PubMed, Web of Science, and Google Scholar databases. The initial search identified 69 studies. Duplicates were removed, and the following inclusion criteria were applied when determining eligibility of RCTs for the current review: (1) discussing CDR/CDA prosthesis and (2) published within between 2010 and 2020. Studies without clinical data or that were not RCTs were excluded. All articles were reviewed independently by 2 authors, with the involvement of an arbitrator to facilitate consensus on any discrepancies. Results: A total of 34 studies were included in the final review. Findings were synthesized into a comprehensive table describing key features and clinical results for each FDA-approved CDR/CDA implant and are overall suggestive of expanding indications and increasing utilization. Conclusions: RCTs have provided substantial evidence to support CDR/CDA for treating single- and 2-level cervical degenerative disc disease in place of conventional anterior cervical discectomy and fusion. Purpose: Current decision-making in multilevel cervical fusion weighs the potential to protect adjacent levels and reduce reoperation risk by crossing the cervicothoracic junction (C7/T1) against increased operative time and risk of complication. Careful planning is required, and the planned distal and adjacent levels should be assessed for degenerative disc disease (DDD). This study assessed whether DDD at the cervicothoracic junction was associated with DDD, disc height, translational motion, or angular variation in the adjacent superior (C6/C7) or inferior (T1/T2) levels. Methods: This study retrospectively analyzed 93 cases with kinematic MRI. Cases were randomly selected from a database with inclusion criteria being no prior spine surgery and images having sufficient quality for analysis. DDD was assessed using Pfirrmann classification. Vertebral body bone marrow lesions were assessed using Modic changes. Disc height was measured at the mid-disc in neutral and extension. Translational motion and angular variation were calculated by assessing translational or angular motion segment integrity respectively in flexion and extension. Statistical associations were assessed with scatterplots and Kendall's tau. Results: DDD at C7/T1 was positively associated with DDD at C6/C7 (tau = 0.53, p < 0.01) and T1/T2 (tau = 0.58, p < 0.01), with greater disc height in neutral position at T1/T2 (tau = 0.22, p < 0.01), and with greater disc height in extended position at C7/T1 (tau = 0.17, p = 0.04) and at T1/T2 (tau = 0.21, p < 0.01). DDD at C7/T1 was negatively associated with angular variation at C6/C7 (tau = − 0.23, p < 0.01). No association was appreciated between DDD at C7/T1 and translational motion. Conclusion: The association of DDD at the cervicothoracic junction with DDD at the adjacent levels emphasizes the necessity for careful selection of the distal level in multilevel fusion in the distal cervical spine. Purpose: To describe the incidence of complications associated with cervical spine surgery and post-operative physical therapy (PT), and to identify if the timing of initiation of post-operative PT impacts the incidence rates. Methods: MOrtho PearlDiver database was queried using billing codes to identify patients who had undergone Anterior Cervical Discectomy and Fusion (ACDF), Posterior Cervical Fusion (PCF), or Cervical Foraminotomy and post-operative PT from 2010–2019. For each surgical procedure, patients were divided into three 12-week increments for post-operative PT (starting at post-operative weeks 2, 8, 12) and then matched based upon age, gender, and Charlson Comorbidity Index score. Each group was queried to determine complication rates and chi-square analysis with adjusted odds ratios, 95% confidence intervals, and p-values were used. Results: Following matching, 3,609 patients who underwent cervical spine surgery at one or more levels and had post-operative PT (ACDF:1784, PCF:1593, and cervical foraminotomy:232). The most frequent complications were new onset cervicalgia (2–14 weeks, 8–20 weeks, 12–24 weeks): ACDF (15.0%, 14.0%, 13.0%), PCF (18.8%, 18.0%, 19.9%), cervical foraminotomy (16.8%, 16.4%, 19.4%); revision: ADCF (7.9%, 8.2%, 7.4%), PCF (9.3%, 10.6%, 10.2%), cervical foraminotomy (11.6%, 10.8% and 13.4%); wound infection: ACDF (3.3%, 3.4%, 3.1%), PCF (8.3%, 8.0%,7.7%), cervical foraminotomy (5.2%, 6.5%, < 4.7%). None of the comparisons were statistically significant. Conclusion: The most common post-operative complications included new onset cervicalgia, revision and wound infection. Purpose: To evaluate the trends and patterns of cervical degenerative disc disease in the cervical spine in different age groups of symptomatic patients.Methods: Overall, 1300 symptomatic patients who had undergone cervical spine MRI from February 2019 to November 2021 were included. Pfirrmann grading was used to evaluate the IVD degeneration. Patients were divided into five groups based on age: group 1 from 20 to 29 years, group 2 from 30 to 39 years, group 3 from 40 to 49 years, group 4 from 50 to 59 years, and group 5 from 60 years and above. The patterns, severity, and the number of degenerated levels in each age group were analysed.Results: The total number of degenerated IVDs (grades IV and V) increased significantly with age, ranging from 76 (4.9%) in group 1 to 677 (43.4%) in group 5. The most common degenerated levels were C2/3 (25% of the total degenerated levels) in group 1, C5/6 (29.0%) in group 2, C5/6 (33.1%) in group 3, C5/6 (31.3%) in group 4, C5/6 (24.8%), in group 5. The number of degenerated IVDs increased significantly with age (P < 0.001). In patients with two or more degenerated levels, contiguous-level disc degeneration was more common than skip lesions.Conclusion: This study evaluated the severity and pattern of cervical disc degeneration at each level in different age groups. The severity and number of degenerated levels increased significantly with increased age. Study Design: Systematic review. Objectives: The study's primary objective was to determine how osteobiologic choice affects fusion rates in patients undergoing anterior cervical discectomy and fusion (ACDF). The study's secondary objectives were to 1) determine the optimal timing of fusion assessment following ACDF and 2) determine if osteobiologic type affects the timing and optimal modality of fusion assessment. Methods: A systematic search of PubMed/MEDLINE was conducted for literature published from 2000 through October 2020 comparing anterior fusion in the cervical spine with various osteobiologics. Both comparative studies and case series of ≥10 patients were included. Results: A total of 74 studies met the inclusion criteria. Seventeen studies evaluated the efficacy of autograft on fusion outcomes, and 23 studies assessed the efficacy of allograft on fusion outcomes. 3 studies evaluated the efficacy of demineralized bone matrix, and seven assessed the efficacy of rhBMP-2 on fusion outcomes. Other limited studies evaluated the efficacy of ceramics and bioactive glasses on fusion outcomes, and 4 assessed the efficacy of stem cell products. Most studies utilized dynamic radiographs for the assessment of fusion. Overall, there was a general lack of supportive data to determine the optimal timing of fusion assessment meaningfully or if osteobiologic type influenced fusion timing. Conclusions: Achieving fusion following ACDF appears to remain an intricate interplay between host biology and various surgical factors, including the selection of osteobiologics. While alternative osteobiologics to autograft exist and may produce acceptable fusion rates, limitations in study methodology prevent any definitive conclusions from existing literature.
