Objective: The PEDSPINE I and PEDSPINE II scores were developed to determine when patients require advanced imaging to rule out cervical spine injury (CSI) in children younger than 3 years of age with blunt trauma. This study aimed to evaluate these scores in an institutional cohort., Methods: The authors identified patients younger than 3 years with blunt trauma who received cervical spine MRI from their institution's prospective database from 2012 to 2015. Patient demographics, injury characteristics, and imaging were compared between patients with and without CSI using chi-square and Wilcoxon rank-sum tests., Results: Eighty-eight patients were identified, 8 (9%) of whom had CSI on MRI. The PEDSPINE I system had a higher sensitivity (50% vs 25%) and negative predictive value (93% vs 92%), whereas PEDSPINE II had a higher specificity (91% vs 65%) and positive predictive value (22% vs 13%). Patients with CSI missed by the scores had mild, radiologically significant ligamentous injuries detected on MRI. Both models would have recommended advanced imaging for the patient who required halo-vest fixation (risk profile: no CSI, 81.9%; ligamentous, 10.1%; osseous, 8.0%). PEDSPINE I would have prevented 52 (65%) of 80 uninjured patients from receiving advanced imaging, whereas PEDSPINE II would have prevented 73 (91%). Using PEDSPINE I, 10 uninjured patients (13%) could have avoided intubation for imaging. PEDSPINE II would not have spared any patients intubation., Conclusions: Current cervical spine clearance algorithms are not sensitive or specific enough to determine the need for advanced imaging in children. However, these scores can be used as a reference in conjunction with physicians' clinical impressions to reduce unnecessary imaging.

Background: C5 palsy affects approximately 5% to 10% of patients undergoing cervical spine surgery. It has a significant negative impact on patient quality-of-life outcomes and healthcare costs. Although >80% of patients improve, some are left with persistent, debilitating deficits. Our objective was to examine if electrodiagnostic testing could be used to successfully identify patients likely to experience complete, partial, and no recovery., Methods: Patients undergoing posterior cervical decompression and fusion at a single institution over a 10-year period were identified. Those experiencing postoperative C5 palsy were included. Outcomes examined included motor recovery of the affected deltoid as a function of time, and changes in electrodiagnostic testing as a function of time since injury. Electrodiagnostic testing included electromyography and was sub-analyzed by time of acquisition postinjury. Deltoid strength was graded on manual motor testing using the 5-point medical research council grading system., Results: Of 77 patients experiencing C5 palsy, 29 had postoperative electrodiagnostic testing. Patients experiencing complete recovery on average achieved functional (4/5) strength by 6-weeks post injury and 4+ per 5 strength by 6-months. Those experiencing partial recovery only achieved antigravity strength (3/5) by 6-weeks and low-function (4-/5) strength by 6-months. Electrodiagnostic testing performed 6-weeks to 6-months postinjury demonstrated that those experiencing complete recovery were more likely to have normal motor unit (MU) recruitment than those experiencing partial (p<.001) or no recovery (p=.008). The presence of ≥2+ fibrillation on tests acquired ≤6-weeks of injury identified patients unlikely to experience any recovery with a positive predictive value (PPV) of 88.9%. The presence of normal MU recruitment on tests acquired 6-weeks to 6-months postinjury identified patients likely to experience complete recovery with a PPV of 87.5%., Conclusions: Electrodiagnostic testing may be a valuable means of differentiating between patients with C5 palsy likely to experience complete, partial, or no recovery. Testing between 6-weeks and 6-months post onset may aid in identifying those least likely to have a complete recovery. No MUs at 4 to 6-months, or reduced units with strength that is not improving, portends a poor long-term outcome. Background: Postoperative C5 palsy is a debilitating complication following posterior cervical decompression., Objective: To create a simple clinical risk score predicting the occurrence of C5 palsy., Methods: We retrospectively reviewed all patients who underwent posterior cervical decompressions between 2007 and 2017. Data was randomly split into training and validation datasets. Multivariable analysis was performed to construct the model from the training dataset. A scoring system was developed based on the model coefficients and a web-based calculator was deployed., Results: The cohort consisted of 415 patients, of which 65 (16%) developed C5 palsy. The optimal model consisted of: mean C4/5 foraminal diameter (odds ratio [OR] = 9.1 for lowest quartile compared to highest quartile), preoperative C5 radiculopathy (OR = 3.5), and dexterity loss (OR = 2.9). The receiver operating characteristic yielded an area under the curve of 0.757 and 0.706 in the training and validation datasets, respectively. Every characteristic was worth 1 point except the lowest quartile of mean C4/5 foraminal diameter, which was worth 2 points, and the factors were summarized by the acronym F2RaD. The median predicted probability of C5 palsy increased from 2% in patients with a score of 0 to 70% in patients with a score of 4. The calculator can be accessed on https://jhuspine2.shinyapps.io/FRADscore/., Conclusion: This study yielded a simplified scoring system and clinical calculator that predicts the occurrence of C5 palsy. Individualized risk prediction for patients may facilitate better understanding of the risks and benefits for an operation, and better prepare them for this possible adverse outcome. Background: Anterior cervical diskectomy and fusion (ACDF) is the main surgical treatment of cervical radiculopathy. Controversy exists about the need to resect the posterior longitudinal ligament (PLL) to directly decompress the nerve roots, or if it is sufficient to indirectly decompress with diskectomy and graft placement. The objective of this study was to determine the effect of PLL resection after ACDF., Methods: A retrospective review was performed of all patients that underwent first-time ACDF for cervical radiculopathy at a single tertiary care institution between 1999 and 2013. Comparative analyses and multivariable logistic regression were performed., Results: Two hundred patients were included with a mean follow-up of 39 months. Average age was 54 years, 62% were women, and diabetes and current smoking status were noted in 11% and 15%, respectively. PLL resection was performed in 127 patients (64%), and no significant differences in baseline characteristics were observed between the 2 cohorts. One durotomy occurred in the resected PLL cohort, and none were seen in the unresected PLL group. No differences were seen in perioperative complications. At the time of last follow-up, improvement in radiculopathy was observed in 94% of the resected PLL group compared with 81% of the unresected PLL group (P = 0.008). After controlling for confounders, PLL resection had 3.8 times greater odds of leading to postoperative improvement in radiculopathy., Conclusions: ACDF leads to a high rate of success in improvement of preoperative radiculopathy. Background: Of the more than 30,000 posterior cervical spine fusions performed annually, 7%-12% will be complicated by postoperative C5 palsy, a condition characterized by new-onset deltoid weakness with or without C5 dermatomal findings and biceps weakness. Posterior translation of the cervical spinal cord has been proposed as a risk factor for this complication., Purpose: To evaluate if C5 palsy can be predicted by spinal cord float back., Study Design/setting: Retrospective cohort., Patient Sample: Patients ≥18 years of age undergoing posterior cervical decompression between 2002 and 2017 for degenerative cervical spine pathologies., Outcome Measures: Occurrence of C5 palsy as evaluated by manual motor testing (MMT)., Methods: We recorded baseline neurological status, operative notes, details of postoperative course, and both pre- and postoperative magnetic resonance imaging images. Float back was defined by the change in the distance between the spinal cord and posterior face of the C4/5 annulus from preoperative to postoperative imaging. C5 palsy was defined by new-onset deltoid weakness on MMT., Results: We identified 242 patients with a mean age of 62.4 years and mean follow-up of 27.9 months. Forty-two (17.4%) experienced postoperative C5 palsy. On univariable analysis, significant predictors of postoperative C5 palsy were mean C4/5 foraminal diameter (2.8 vs. 3.2 mm; p<.001), anterior projection of the C5 superior articular process (4.12 vs. 3.70 mm; p=.04), cord float back (0.35 vs. 0.28 cm; p=.02), undergoing laminectomy of the C5 (p=.02) or C4 and C5 levels (p=.02), and undergoing instrumented fusion extending one level above and below the C4/5 level. Foraminotomy of the C4/5 level was not predictive of postoperative palsy. On multivariable analysis mean C4/5 foraminal diameter (odds ratio=0.38 per mm; p<.01) predicted C5 palsy; cord float back at the C4/5 level was not predictive of C5 palsy., Conclusions: Spinal cord float back was not an independent predictor of C5 palsy on multivariable analysis. Only smaller foraminal diameter was independently predictive of postoperative C5 palsy. This suggests that chronic preoperative compression of the C5 roots, not postdecompression float back may be the biggest contributor to the etiology of postoperative C5 palsy., (Copyright © 2019 Elsevier Inc. Background: Horner syndrome is an infrequently seen complication of anterior cervical discectomy and fusion (ACDF). Multicenter studies have reported a very low incidence, less than 0.1%., Objective: To identify the incidence in, characteristics of, and postoperative course in patients in whom postoperative Horner syndrome developed after ACDF., Methods: We performed a retrospective review of all patients who experienced Horner syndrome after ACDF for cervical degenerative disease at a single tertiary care institution between 2017 and 2018. A systematic review was then performed to identify studies investigating prevalence, diagnosis, and treatment of postoperative Horner syndrome after ACDF., Results: Of 1116 patients at our institution who underwent ACDF, the incidence of Horner syndrome was 0.45%. C4/5 and C5/6 were the 2 most common surgical levels. The complication was noted to occur immediately after surgery, and at least partial improvement was identified in all patients an average 3.5 months after surgery (range, 10 days to 6 months). These findings were consistent with our systematic review of 21 studies that showed an incidence of 0.6% (range, 0.02% to 4.0%), the most common surgical level C5/6 (64%), and 82% of patients experiencing at least partial resolution of symptoms within 1 year (60.7% complete, 21.4% partial resolution)., Conclusion: Horner syndrome occurs in 0.6% of patients undergoing ACDF. Careful postoperative examination should reveal this complication, which may be underdiagnosed or underreported in larger multicenter case series. Objective: Postoperative C5 palsy affects 7%-12% of patients who undergo posterior cervical decompression for degenerative cervical spine pathologies. Minimal evidence exists regarding the natural history of expected recovery and variables that affect palsy recovery. The authors investigated pre- and postoperative variables that predict recovery and recovery time among patients with postoperative C5 palsy., Methods: The authors included patients who underwent posterior cervical decompression at a tertiary referral center between 2004 and 2018 and who experienced postoperative C5 palsy. All patients had preoperative MR images and full records, including operative note, postoperative course, and clinical presentation. Kaplan-Meier survival analysis was used to evaluate both times to complete recovery and to new neurological baseline-defined by deltoid strength on manual motor testing of the affected side-as a function of clinical symptoms, surgical maneuvers, and the severity of postoperative deficits., Results: Seventy-seven patients were included, with an average age of 64 years. The mean follow-up period was 17.7 months. The mean postoperative C5 strength was grade 2.7/5, and the mean time to first motor examination with documented C5 palsy was 3.5 days. Sixteen patients (21%) had bilateral deficits, and 9 (12%) had new-onset biceps weakness; 36% of patients had undergone C4-5 foraminotomy of the affected root, and 17% had presented with radicular pain in the dermatome of the affected root. On univariable analysis, patients' reporting of numbness or tingling (p = 0.02) and a baseline deficit (p < 0.001) were the only predictors of time to recovery. Patients with grade 4+/5 weakness had significantly shorter times to recovery than patients with grade 4/5 weakness (p = 0.001) or ≤ grade 3/5 weakness (p < 0.001). There was no difference between those with grade 4/5 weakness and those with ≤ grade 3/5 weakness. Patients with postoperative strength < grade 3/5 had a < 50% chance of achieving complete recovery., Conclusions: The timing and odds of recovery following C5 palsy were best predicted by the magnitude of the postoperative deficit. The use of C4-5 foraminotomy did not predict the time to or likelihood of recovery.

Background: Postoperative C5 palsy affects 8% of patients undergoing posterior cervical decompression. It is associated with a period functional disability that may exceed 12 months and increase direct care costs > $2000., Methods: All patients undergoing posterior cervical decompression at a single tertiary-care facility for degenerative conditions were evaluated for preoperative imaging, clinical presentation, surgical operation, and postoperative course. We sought to identify those variables predictive of postoperative C5 palsy., Results: Of 221 included patients (mean age, 63 years; 54% male), 12.2% experienced C5 palsy. On univariate analysis, C5 palsy was associated with foraminal diameter (P = 0.0005), spinal cord cross-sectional area (P = 0.11), number of levels undergoing laminectomy (P = 0.14), and clinical presentation of dropping objects (P = 0.07), hand clumsiness (P = 0.13), or paresthesias in the upper extremities (P = 0.08). Foraminal diameter (odds ratio, 0.31 per mm increase; 95% confidence interval, 0.16-0.60; P < 0.001) and patient report of gait disturbance (odds ratio, 0.53; 95% confidence interval, 0.33-0.85; P = 0.008) were the only significant predictors on multivariate analysis. A foraminal diameter <2 mm had 2-fold greater odds of postoperative C5 palsy. A receiver operating curve for the multivariate logistic model had an associated C-statistic of 0.7818. The absolute error of this model was 9.3% on internal validation., Conclusions: Foraminal stenosis most strongly predicted postoperative C5 palsy. A proof-of-concept model incorporating foramen size, as well as clinical complaints of paresthesias, hand clumsiness, and gait abnormality, successfully predicts the occurrence of postoperative palsy with an overall accuracy of 78%., (Copyright © 2019 Elsevier Inc. Background: Posterior atlantoaxial fusion is an important armamentarium for neurosurgeons to treat several pathologies involving the craniovertebral junction. Although the potential advantages of recombinant human bone morphogenetic protein-2 (rhBMP-2) are well documented in the lumbar spine, its indication for C1-C2 fusion has not been well characterized. In our institution, we apply rhBMP-2 to the C1-C2 joint either alone or with hydroxyapatite, locally harvested autograft chips, and/or morselized allogenic bone graft for selected cases-without conventional posterior structural bone graft. We report the clinical outcomes of the surgical technique to elucidate its feasibility., Methods: We performed a single-center, retrospective review of data from 2008 to 2016 and identified 69 patients who had undergone posterior atlantoaxial fusion with rhBMP-2. The clinical records of these patients were reviewed, and the baseline characteristics, operative data, and postoperative complications were collected and statistically analyzed., Results: The average age of the 69 patients was 60.8 ± 4.5 years, and 55.1% were women. With an average follow-up period of 21.1 ± 4.2 months, the C1-C2 fusion rate was 94.3% (65 of 69), and the average time to fusion was 11.4 ± 2.6 months (range, 5-23). The overall reoperation rate was 10.1% (7 of 69), with instrumentation failure in 7 patients (10.1%), adjacent segment disease in 2 (2.9%), and postoperative dysphagia and dyspnea in 2 patients (2.9%). No ectopic bone formation or soft tissue edema developed., Conclusions: Although retrospective and from a single center, our study has shown that rhBMP-2 usage at the C1-C2 joint without posterior structural bone grafting is a safe and reasonable surgical option., (Copyright © 2018 Elsevier Inc. OBJECTIVE The authors assessed the rate of vertebral growth, curvature, and alignment for multilevel constructs in the cervical spine after occipitocervical fixation (OCF) in pediatric patients and compared these results with those in published reports of growth in normal children. METHODS The authors assessed cervical spine radiographs and CT images of 18 patients who underwent occipitocervical arthrodesis. Measurements were made using postoperative and follow-up images available for 16 patients to determine cervical alignment (cervical spine alignment [CSA], C1-7 sagittal vertical axis [SVA], and C2-7 SVA) and curvature (cervical spine curvature [CSC] and C2-7 lordosis angle). Seventeen patients had postoperative and follow-up images available with which to measure vertebral body height (VBH), vertebral body width (VBW), and vertical growth percentage (VG%-that is, percentage change from postoperative to follow-up). Results for cervical spine growth were compared with normal parameters of 456 patients previously reported on in 2 studies. RESULTS Ten patients were girls and 8 were boys; their mean age was 6.7 ± 3.2 years. Constructs spanned occiput (Oc)-C2 (n = 2), Oc-C3 (n = 7), and Oc-C4 (n = 9). The mean duration of follow-up was 44.4 months (range 24-101 months). Comparison of postoperative to follow-up measures showed that the mean CSA increased by 1.8 ± 2.9 mm (p < 0.01); the mean C2-7 SVA and C1-7 SVA increased by 2.3 mm and 2.7 mm, respectively (p = 0.3); the mean CSC changed by -8.7° (p < 0.01) and the mean C2-7 lordosis angle changed by 2.6° (p = 0.5); and the cumulative mean VG% of the instrumented levels (C2-4) provided 51.5% of the total cervical growth (C2-7). The annual vertical growth rate was 4.4 mm/year. The VBW growth from C2-4 ranged from 13.9% to 16.6% (p < 0.001). The VBW of C-2 in instrumented patients appeared to be of a smaller diameter than that of normal patients, especially among those aged 5 to < 10 years and 10-15 years, with an increased diameter at the immediately inferior vertebral bodies compensating for the decreased width. No cervical deformation, malalignment, or detrimental clinical status was evident in any patient. CONCLUSIONS The craniovertebral junction and the upper cervical spine continue to present normal growth, curvature, and alignment parameters in children with OCF constructs spanning a distance as long as Oc-C4.

OBJECTIVE If left untreated, occipitocervical (OC) instability may lead to serious neurological injury or death. Open internal fixation is often necessary to protect the neurovascular elements. This study reviews the etiologies for pediatric OC instability, analyzes the radiographic criteria for surgical intervention, discusses surgical fixation techniques, and evaluates long-term postoperative outcomes based on a single surgeon's experience. METHODS The charts of all patients < 18 years old who underwent internal OC fixation conducted by the senior author were retrospectively reviewed. Forty consecutive patients were identified for analysis. Patient demographic data, OC junction pathology, radiological diagnostic tools, surgical indications, and outcomes are reported. RESULTS The study population consisted of 20 boys and 20 girls, with a mean age of 7.3 years. Trauma (45% [n = 18]) was the most common cause of instability, followed by congenital etiologies (37.5% [n = 15]). The condyle-C1 interval had a diagnostic sensitivity of 100% for atlantooccipital dislocation. The median number of fixated segments was 5 (occiput-C4). Structural bone grafts were used in all patients. Postsurgical neurological improvement was seen in 88.2% (15/17) of patients with chronic myelopathy and in 25% (1/4) of patients with acute myelopathy. Preoperatively, 42.5% (17/40) of patients were neurologically intact and remained unchanged at last follow-up, 42.5% (17/40) had neurological improvement, 12.5% (5/40) remained unchanged, and 2.5% (1/40) deteriorated. All patients had successful fusion at 1-year follow-up. The complication rate was 7.5% (3/40), including 1 case of vertebral artery injury. CONCLUSIONS Occipitocervical fixation is safe in children and provides immediate immobilization, with excellent survival and arthrodesis rates. Of the radiographic tools evaluated, the condyle-C1 interval was the most predictive of atlantooccipital dislocation.

OBJECTIVE The rule of Spence is inaccurate for assessing integrity of the transverse atlantal ligament (TAL). Because CT is quick and easy to perform at most trauma centers, the authors propose a novel sequence of obtaining 2 CT scans to improve the diagnosis of TAL impairment. The sensitivity of a new CT-based method for diagnosing a TAL injury in a cadaveric model was assessed. METHODS Ten human cadaveric occipitocervical specimens were mounted horizontally in a supine posture with wooden inserts attached to the back of the skull to maintain a neutral or flexed (10°) posture. Specimens were scanned in neutral and flexed postures in a total of 4 conditions (3 conditions in each specimen): 1) intact (n = 10); either 2A) after a simulated Jefferson fracture with an intact TAL (n = 5) or 2B) after a TAL disruption with no Jefferson fracture (n = 5); and 3) after TAL disruption and a simulated Jefferson fracture (n = 10). The atlantodental interval (ADI) and cross-sectional canal area were measured. RESULTS From the neutral to the flexed posture, ADI increased an average of 2.5% in intact spines, 6.25% after a Jefferson fracture without TAL disruption, 34% after a TAL disruption without fracture, and 25% after TAL disruption with fracture. The increase in ADI was significant with both TAL disruption and TAL disruption and fracture (p < 0.005) but not in the other 2 conditions (p > 0.6). Changes in spinal canal area were not significant (p > 0.70). CONCLUSIONS This novel method was more sensitive than the rule of Spence for evaluating the integrity of the TAL on CT and does not increase the risk of further neurological damage.

OBJECTIVE Craniovertebral junction (CVJ) injuries complicated by transverse atlantal ligament (TAL) disruption often require surgical stabilization. Measurements based on the atlantodental interval (ADI), atlas lateral diameter (ALD1), and axis lateral diameter (ALD2) may help clinicians identify TAL disruption. This study used CT scanning to evaluate the reliability of these measurements and other variants in the clinical setting. METHODS Patients with CVJ injuries treated at the authors' institution between 2004 and 2011 were evaluated retrospectively for demographics, mechanism and location of CVJ injury, classification of injury, treatment, and modified Japanese Orthopaedic Association score at the time of injury and follow-up. The integrity of the TAL was evaluated using MRI. The ADI, ALD1, and ALD2 were measured on CT to identify TAL disruption indirectly. RESULTS Among the 125 patients identified, 40 (32%) had atlas fractures, 59 (47.2%) odontoid fractures, 31 (24.8%) axis fractures, and 4 (3.2%) occipital condyle fractures. TAL disruption was documented on MRI in 11 cases (8.8%). The average ADI for TAL injury was 1.8 mm (range 0.9-3.9 mm). Nine (81.8%) of the 11 patients with TAL injury had an ADI of less than 3 mm. In 10 patients (90.9%) with TAL injury, overhang of the C-1 lateral masses on C-2 was less than 7 mm. ADI, ALD1, ALD2, ALD1 - ALD2, and ALD1/ALD2 did not correlate with the integrity of the TAL. CONCLUSIONS No current measurement method using CT, including the ADI, ALD1, and ALD2 or their differences or ratios, consistently indicates the integrity of the TAL. A more reliable CT-based criterion is needed to diagnose TAL disruption when MRI is unavailable.

Objective: Hypertension and cervical spondylosis are diseases of the adult population that are approaching near pandemic proportions. However, the interactions between these two disease processes are poorly understood. We set out to determine the associations among systemic hypertension, clinical status, and imaging findings of spinal cord damage for patients with cervical stenosis., Methods: A retrospective chart review was performed on patients with symptomatic cervical stenosis related to degenerative disease and divided on the basis of outpatient blood pressure control (normal <140/<90 mm Hg). Sagittal T2-weighted magnetic resonance imaging (MRI) of the cervical spine was analyzed to determine the degree of maximal canal stenosis (MCS; %), surface area of increased signal intensity (ISI; cm(2)), and signal intensity ratio (SIR). Functional status was evaluated using the modified Japanese Orthopaedic Association (mJOA) scale and the Nurick scale., Results: One hundred twenty-two patients were identified (64 hypertensive, 58 nonhypertensive). Likelihood of ISI was higher in hypertensive patients (P < 0.05). Average ISI was significantly higher in patients with uncontrolled blood pressure (P = 0.02) despite MCS being identical between the two groups. The mJOA and Nurick scores were worse for patients with systemic hypertension (P < 0.02). Diabetes mellitus and smoking history did not affect these findings., Conclusions: Persistent hypertension in outpatients is associated with worsened clinical status and increased markers of spinal cord damage on MRI. Perioperative management of blood pressure may serve to improve clinical outcomes. Larger prospective trials are necessary to further validate these findings., (Copyright © 2015 Elsevier Inc. A 13-year-old boy presented with fever and neck pain and stiffness, which was initially misdiagnosed as culture-negative meningitis. Magnetic resonance images of the brain and cervical spine demonstrated what appeared to be an intradural extramedullary mass at the C1-3 level, resulting in moderate cord compression, and a Chiari Type I malformation. The patient underwent a suboccipital craniectomy and a C1-3 laminectomy with intradural exploration for excisional biopsy and resection. The lesion containing the parasite was extradural, extending laterally through the C2-3 foramina. Inflammatory tissue secondary to Onchocerca lupi infection was identified, and treatment with steroids and doxycycline was initiated. At the 6-month follow-up, the patient remained asymptomatic, with MR images demonstrating a significant reduction in lesional size. However, 10 weeks postoperatively, the infection recurred, necessitating a second operation. The patient was treated with an additional course of doxycycline and is currently maintained on ivermectin therapy. This is the second reported case of cervical O. lupi infection in a human. In the authors' experience, oral doxycycline alone was insufficient in controlling the disease, and the addition of ivermectin therapy was necessary.

The craniovertebral junction consists of the occiput, atlas, and axis, along with their strong ligamentous attachments. Because of its unique anatomical considerations, trauma to the craniovertebral junction requires specialized care. Children with potential injuries to the craniovertebral junction and cervical spinal cord demand specific considerations compared to adult patients. Prehospital immobilization techniques, diagnostic studies, and spinal injury patterns among young children can be different from those in adults. This review highlights the unique aspects in diagnosis and management of children with real or potential craniovertebral junction injuries.

Background Context: In vitro nondestructive flexibility testing of the CerviCore total disc replacement (TDR) was performed. It was hypothesized that TDR would not significantly alter biomechanics relative to intact, whereas rigid fixation would cause significant changes., Purpose: To assess the ability of a cervical metal-on-metal saddle-shaped TDR to replicate normal biomechanics in vitro., Study Design: Human cadaveric flexibility experiment., Methods: Nine human cadaveric C3-T1 specimens were tested intact, after TDR and after anterior plating. Flexion, extension, lateral bending, and axial rotation were induced by pure moments; flexion-extension was then repeated using a simplified muscle force model with 70-N follower load. Optical markers measured three-dimensional intervertebral motion, and eight points of laminar surface strain were recorded near the left and right C5-C6 facet joints. Biomechanical parameters studied included range of motion (ROM), lax zone (LZ), angular coupling pattern, sagittal instantaneous axis of rotation (IAR), and facet loads normal to the facet joint plane. Mean values of parameters were compared statistically using repeated measures analysis of variance and Holm-Sidak tests., Results: Total disc replacement caused significant reduction in ROM during extension (p=.004) and significant reduction in LZ during lateral bending (p=.01). However, plating significantly reduced both ROM and LZ during flexion, extension, and lateral bending (p<.006). Sagittal IAR shifted relative to intact by 3.6 mm after TDR (p>.05) and 6.5 mm after plating (p>.05). Coupled axial rotation/degree lateral bending was 99% of intact after TDR but 76% of intact after plating (p=.15). Coupled lateral bending/degree axial rotation was 95% of intact after TDR but 85% of intact after plating (p=.43). Neither construct altered facet loads from intact., Conclusions: With regard to ROM, LZ, IAR, and coupling, deviations from intact biomechanics were less substantial after TDR than after plating. Facet load alterations were minimal with either construct. Objectives: Juxtafacet cysts (JFCs) of the subaxial cervical spine are rare causes of neurological deficits. Their imaging characteristics, relationship to segmental instability, and potential for inducing acute symptomatic deterioration have only been described in a few case reports and small case series. The objective of the current study was to review the surgical experience at our center and across the literature to better define these variables., Methods: A single-institution, multisurgeon series of 12 consecutive patients (mean age 63.4 years, range 52-83 years) harboring 14 JFCs treated across 9 years was retrospectively reviewed. Clinical history, neurological status, preoperative imaging, operative findings, pathology, and postoperative outcomes were obtained from medical records. The mean follow up was 9.2 ± 7.8 months. A literature review identified 35 studies with 89 previously reported cases of surgically treated subaxial cervical JFCs., Results: Consistent with previously reported cases, most JFCs in our series involved the C7/T1 level. Nine patients reported axial neck pain, 12 patients had radicular symptoms, four patients had myelopathy, and one patient experienced rapid neurological decline attributable to cystic hemorrhage. Cyst expansion without hemorrhage caused subacute deterioration in one patient. All patients experienced sensory and/or motor improvement following surgical decompression. Preoperative axial neck pain improved in eight of nine patients (89 %). Seven out of 12 patients (58 %) underwent fusion either at the time of decompression (six patients) or at a delayed timepoint within the follow-up period (one patient). Prior history of cervical instrumentation, hypermobility on dynamic imaging, and other risk factors for segmental instability were more common in our series than in previous reports., Conclusions: Our findings lead us to advocate for early decompression rather than prolonged conservative treatment, for pre- and postoperative dynamic imaging, and for fusion in selected cases as an initial surgical consideration.

Traumatic cervical spondyloptosis is a rare clinical entity typically associated with complete neurological deficit. The inherent mechanics of this fracture-dislocation pattern contorts the vertebral arteries in such a way that it may result in dissection or compromised flow through those vessels. Thus, intimal injury or thrombus from stasis of flow may result. Reduction of the spondyloptosis restores flow to the vertebral arteries, but it also may mobilize thrombus or propagate an intimal dissection within the previously contorted vessel. The authors review their experience in the care of a 43-year-old man who sustained C4-5 spondyloptosis while riding an all-terrain vehicle. On arrival, the patient demonstrated no motor function below C-4 but had sensation to the nipple line (American Spinal Injury Association Spinal Cord Injury Classification B). The patient's cranial nerve examination was unremarkable. Computed tomography of the cervical spine demonstrated complete spondyloptosis at C4-5. The patient was immediately placed in cervical traction and taken to the operating room for open reduction of the fracture dislocation, decompression of the spinal cord, and stabilization with an interbody graft and cervical plate. Preoperative cervical traction was successful in only partial reduction of the fracture dislocation. Open reduction was achieved with exposure of the C-4 and C-5 bodies and sequential distraction. After anatomical alignment was achieved, an interbody graft was placed and a cervical plate secured. A subsequent decline in the patient's level of consciousness prompted CT of the head, which showed evidence of a basilar artery thrombosis. A CT angiographic study demonstrated patency of the vertebral arteries, but a mid-basilar artery thrombosis. The patient progressed to brain death 24 hours after reduction of the fracture dislocation. The degree of contortion of the vertebral arteries in cervical spondyloptosis in the upper cervical spine may result in stasis of flow with subsequent formation of thrombus or intimal injury. After anatomical reduction, restoration of flow within the vertebral arteries may mobilize the thrombus or propagate an intimal dissection and result in subsequent embolic events. Endovascular evaluation may be warranted immediately after anatomical reduction of a high cervical spondyloptosis for evaluation of the vertebral arteries and possible thrombus dissolution or retrieval.

Objective: To evaluate risk factors for the development of dysphagia after anterior cervical surgery., Methods: The records of 249 patients who underwent anterior cervical surgery were reviewed. The presence and severity of dysphagia were assessed with the Dysphagia Disability Index 6 weeks and 3, 6, and 12 months after surgery. Age; sex; ethnicity; cigarette smoking; previous cervical surgeries; reoperation for same pathology; type of procedure, incision, and instrumentation; number and levels involved; side of procedure, length of surgery; and use of postoperative bracing were analyzed., Results: During the first 6 months after surgery, 27 (10.8%) patients developed dysphagia. From these patients the presence of dysphagia at 6 weeks and at 3 and 6 months was 88.8%, 29.6%, and 7.4%, respectively. By 12 months, dysphagia had resolved in all cases. The mean age of patients with dysphagia was 55 years (SD 12.98) and 50 years (SD 12.07) in patients without dysphagia (P = 0.05). Dysphagic patients had an average of 2.2 (SD 1.15) levels operated compared with 1.84 (SD 0.950) in nondysphagic patients (P = 0.05). Patients who developed dysphagia were most often treated at C4-5 (67%) and C5-6 (81%: P < 0.001). Although mean operative time was slightly longer in patients with dysphagia (186 minutes) compared with those without (169 minutes), the difference was not significant., Conclusions: In our patients, the incidence of dysphagia was low, and it had completely resolved at 12 months in all cases. Object: Schwannomas occupying the craniocervical junction (CCJ) are rare and usually originate from the jugular foramen, hypoglossal nerves, and C-1 and C-2 nerves. Although they may have different origins, they may share the same symptoms, surgical approaches, and complications. An extension of these lesions along the posterior fossa cisterns, foramina, and spinal canal--usually involving various cranial nerves (CNs) and the vertebral and cerebellar arteries--poses a surgical challenge. The primary goals of both surgical and radiosurgical management of schwannomas in the CCJ are the preservation and restoration of function of the lower CNs, and of hearing and facial nerve function. The origins of schwannomas in the CCJ and their clinical presentation, surgical management, adjuvant stereotactic radiosurgery, and outcomes in 36 patients treated at Barrow Neurological Institute (BNI) are presented., Methods: Between 1989 and 2009, 36 patients (mean age 43.6 years, range 17-68 years) with craniocervical schwannomas underwent surgical resection at BNI. The records were reviewed retrospectively regarding clinical presentation, radiographic assessment, surgical approaches, adjuvant therapies, and follow-up outcomes., Results: Headache or neck pain was present in 72.2% of patients. Cranial nerve impairments, mainly involving the vagus nerve, were present in 14 patients (38.9%). Motor deficits were found in 27.8% of the patients. Sixteen tumors were intra- and extradural, 15 were intradural, and 5 were extradural. Gross-total resection was achieved in 25 patients (69.4%). Adjunctive radiosurgery was used in the management of residual tumor in 8 patients; tumor control was ultimately obtained in all cases., Conclusions: Surgical removal, which is the treatment of choice, is curative when schwannomas in the CCJ are excised completely. The far-lateral approach and its variations are our preferred approaches for managing these lesions. Most common complications involve deficits of the lower CNs, and their early recognition and rehabilitation are needed. Stereotactic radiosurgery, an important tool for the management of these tumors as adjuvant therapy, can help decrease morbidity rates.

Introduction: Cervical spine injuries can be life-altering issues in the pediatric population. The aim of the present paper was to review this literature., Conclusions: A comprehensive knowledge of the special anatomy and biomechanics of the spine of children is essential in diagnosis and treating issues related to spine injuries.

Object: Ossification of the posterior longitudinal ligament (OPLL) is a rare disease that results in progressive myeloradiculopathy related to pathological ossification of the ligament from unknown causes. Although it has long been considered a disease of Asian origin, this disorder is increasingly being recognized in European and North American populations. Herein the authors present demographic, radiographic, and comorbidity data from white patients with diagnosed OPLL as well as the outcomes of surgically treated patients., Methods: Between 1999 and 2010, OPLL was diagnosed in 36 white patients at Barrow Neurological Institute. Patients were divided into 2 groups: a group of 33 patients with cervical OPLL and a group of 3 patients with thoracic or lumbar OPLL. Fifteen of these patients who had received operative treatment were analyzed separately. Imaging analysis focused on signal changes in the spinal cord, mass occupying ratio, signs of dural penetration, spinal levels involved, and subtype of OPLL. Surgical techniques included anterior cervical decompression and fusion with corpectomy, posterior laminectomy with fusion, posterior open-door laminoplasty, and anterior corpectomy combined with posterior laminectomy and fusion. Comorbidities, cigarette smoking, and previous spine surgeries were considered. Neurological function was assessed using a modified Japanese Orthopaedic Association Scale (mJOAS)., Results: A high-intensity signal on T2-weighted MR imaging and a history of cervical spine surgery correlated with worse mJOAS scores. Furthermore, mJOAS scores decreased as the occupying rate of the OPLL mass in the spinal canal increased. On radiographic analysis, the proportion of signs of dural penetration correlated with the OPLL subtype. A high mass occupying ratio of the OPLL was directly associated with the presence of dural penetration and high-intensity signal. In the surgical group, the rate of neurological improvement associated with an anterior approach was 58% compared with 31% for a posterior laminectomy. No complications were associated with any of the 4 types of surgical procedures. In 3 cases, symptoms had worsened at the last follow-up, with only a single case of disease progression. Laminoplasty was the only technique associated with a worse clinical outcome. There were no statistical differences (p > 0.05) between the type of surgical procedure or radiographic presentation and postoperative outcome. There was also no difference between the choice of surgical procedure performed and the number of spinal levels involved with OPLL., Conclusions: Ossification of the posterior longitudinal ligament can no longer be viewed as a disease of the Asian population exclusively. Since OPLL among white populations is being diagnosed more frequently, surgeons must be aware of the most appropriate surgical option. The outcomes of the various surgical treatments among the different populations with OPLL appear similar. Compared with other procedures, however, anterior decompression led to the best neurological outcomes.

Background: An alternative route must be used for atlantoaxial arthrodesis to avoid the risks of transoral route or when posterior approaches are contraindicated., Objective: To assess relevant quantitative anatomic parameters for C1-C2 anterolateral transarticular fixation and to demonstrate the nuances of an anterolateral approach to the upper cervical spine., Methods: Five cadaveric necks were dissected bilaterally to demonstrate anatomic landmarks and surgical technique. The C2 pars interarticularis was used as the entry for inserting screws toward the C1 lateral mass. Ten computed tomography scans were analyzed to quantify working area and optimal angles of approach., Results: The medial surface of sternocleidomastoid muscle was dissected extensively but not divided. The C2 transverse process was a landmark for guiding dissection posterior to the carotid sheath. In all specimens, the gray ramus communicans from the superior cervical ganglion to the C2 nerve was a landmark for locating the C2 pars. Slightly below that branch, the longus capitis muscle could be displaced medially to reach the C2 pars. The ideal angles for screw placement were 22.9 +/- 5.7 degrees medial to the sagittal plane and 25.3 +/- 7.4 degrees posterior to the coronal plane. The mean working area was 71.2 mm (range, 49-103 mm)., Conclusion: We propose a new anterolateral stabilization technique for atlantoaxial instability based on less traumatic dissection of the upper cervical region, different instrumentation, and guidance by reliable landmarks. For anterolateral transarticular C1-C2 screw fixation, the gray ramus communicans to the C2 nerve is a reliable landmark for locating the entry for a screw on the C2 pars.

OBJECT The objective of this work was to search a national health care database of patients diagnosed with cervical spine fractures in the US to analyze discharge, demographic, and hospital charge trends over a 10-year period. METHODS Clinical data were derived from the Nationwide Inpatient Sample (NIS) for the years 1997 through 2006. The NIS is maintained by the Agency for Healthcare Research and Quality and represents a 20% random stratified sample of all discharges from nonfederal hospitals within the US. Patients with cervical spine fractures with and without spinal cord injury (SCI) were identified using the appropriate ICD-9-CM codes. The volume of discharges, length of stay (LOS), hospital charges, total national charges, discharge pattern, age, and sex were analyzed. National estimates were calculated using the HCUPnet tool. RESULTS Approximately 200,000 hospitalizations were identified. In the non-SCI group, there was a 74% increase in hospitalizations and charges between 1997 and 2006, but LOS changed minimally. There was no appreciable change in the rate of in-hospital mortality (< 3%), but discharges home with home health care and to skilled rehabilitation or nursing facilities increased slightly. In the SCI group, hospitalizations and charges increased by 29 and 38%, respectively. There were no significant changes in LOS or discharge status in this group. Spinal cord injury was associated with increases in LOS, charges, and adverse outcomes compared with fractures without SCI. Total national charges associated with both groups combined exceeded $1.3 billion US in 2006. CONCLUSIONS During the studied period, increases in hospitalizations and charges were observed in both the SCI and non-SCI groups. The percentage increase was higher in the non-SCI group. Although SCI was associated with higher adverse outcomes, there were no significant improvements in immediate discharge status in either group during the 10 years analyzed.

Object: The small diameter of the pedicle can make C-7 pedicle screw insertion dangerous. Although transfacet screws have been studied biomechanically when used in pinning joints, they have not been well studied when used as part of a C7-T1 screw/rod construct. The authors therefore compared C7-T1 fixation using a C-7 transfacet screw/T-1 pedicle screw construct with a construct composed of pedicle screws at both levels., Methods: Each rigid posterior screw/rod construct was placed in 7 human cadaveric C6-T2 specimens (14 total). Specimens were tested in normal condition, after 2-column instability, and once fixated. Nondestructive, nonconstraining pure moments (maximum 1.5 Nm) were applied to induce flexion, extension, lateral bending, and axial rotation while recording 3D motion optoelectronically. The entire construct was then loaded to failure by dorsal linear force., Results: There was no significant difference in angular range of motion between the 2 instrumented groups during any loading mode (p > 0.11, nonpaired t-tests). Both constructs reduced motion to < 2 degrees in any direction and allowed significantly less motion than in the normal condition. The C-7 facet screw/T-1 pedicle screw construct allowed a small but significantly greater lax zone than the pedicle screw/rod construct during lateral bending, and it failed under significantly less load than the pedicle screw/rod construct (p < 0.001)., Conclusions: When C-7 transfacet screws are connected to T-1 pedicle screws, they provide equivalent stability of constructs formed by pedicle screws at both levels. Although less resistant to failure, the transfacet screw construct should be a viable alternative in patients with healthy bone.

Study Design: In vitro human cadaveric study simultaneously quantifying sagittal plane flexibility and spinal canal stenosis., Objective: To compare biomechanical stability and the change in cross-sectional area during flexion and extension after laminectomy and open-door laminoplasty., Summary of Background Data: Spinal canal stenosis has been quantified in vitro but has not been quantified in studies of laminectomy or laminoplasty., Methods: Cadaveric specimens were loaded in physiologic-range flexion and extension using nonconstraining pure moments while recording segmental angles optoelectronically. Custom flexible tubing was placed within the spinal canal, and water was continuously pumped through the tubing while measuring upstream pressure. Spinal canal cross-sectional area correlated to water pressure, allowing continuous monitoring of the smallest cross-sectional area of the canal. Specimens were tested (1) normal, (2) after modeling stenosis by inserting hemispherical wooden beads in the spinal canal at 3 levels, (3) after open-door laminoplasty at 5 levels, and (4) after expanding laminoplasty to laminectomy., Results: Range of motion (ROM) in the normal, stenotic, and laminoplasty conditions did not differ significantly. However, laminectomy increased ROM significantly more than other conditions. ROM after laminectomy was 13% greater than after laminoplasty. After modeling stenosis, the cross-sectional area decreased to 52% +/- 12% of normal. Laminoplasty restored the cross-sectional area to 70% +/- 12% of normal whereas laminectomy restored cross-sectional area to 101% +/- 4% of normal. Among all conditions, areas differed significantly except normal versus laminectomy., Conclusion: Laminoplasty leaves the spine in a significantly more stable condition than laminectomy. However, laminoplasty failed to relieve stenosis completely. In this study, stenosis was modeled as about 50% occlusion of the spinal canal. The degree of stenosis should be considered in clinical decisions of whether laminectomy or laminoplasty is more appropriate.

The relative rarity of pediatric cervical spine injuries can impede rapid response and efficient care of this patient population. An understanding of the unique anatomical, radiographic, and biomechanical characteristics of the pediatric cervical spine is essential to the appropriate care of these challenging patients. Patterns of injury, diagnosis, and issues related to operative and nonoperative management are discussed with a focus on the developing spine. Our aim is to improve the understanding of traumatic cervical spine injuries in children for all practitioners involved with their care.

Object: Lateral mass screws are traditionally used to fixate the subaxial cervical spine, while pedicle screws are used in the thoracic spine. Lateral mass fixation at C-7 is challenging due to thin facets, and placing pedicle screws is difficult due to the narrow pedicles. The authors describe their clinical experience with a novel technique for transfacet screw placement for fixation at C-7., Methods: A retrospective chart review was undertaken in all patients who underwent transfacet screw placement at C-7. The technique of screw insertion was the same for each patient. Polyaxial screws between 8- and 10-mm-long were used in each case and placed through the facet from a perpendicular orientation. Postoperative radiography and clinical follow-up were analyzed for aberrant screw placement or construct failure., Results: Ten patients underwent C-7 transfacet screw placement between June 2006 and March 2007. In all but 1 patient screws were placed bilaterally, and the construct lengths ranged from C-3 to T-5. One patient with a unilateral screw had a prior facet fracture that precluded bilateral screw placement. There were no intraoperative complications or screw failures in these patients. After an average of 6 months of follow-up there were no hardware failures, and all patients showed excellent alignment., Conclusions: The authors present the first clinical demonstration of a novel technique of posterior transfacet screw placement at C-7. These results provide evidence that this technique is safe to perform and adds stability to cervicothoracic fixation.

Pseudomeningoceles rarely develop after cervical trauma; in all reported cases the lesions have extended outside the spinal canal. The authors report the first known cases of anterior cervical pseudomeningoceles contained entirely within the spinal canal and causing cord compression and neurological injury. The authors retrospectively reviewed the cases of three patients with traumatic cervical spine injuries and concomitant compressive anterior pseudomeningoceles. The lesion was recognized in the first case when the patient's neurological status declined after he sustained a severe atlantoaxial injury; the pseudomeningocele was identified intraoperatively and decompressed. After the decompressive surgery, the patient's severe tetraparesis partially resolved. In the other two patients diagnoses of similar pseudomeningoceles were established by magnetic resonance imaging. Both patients were treated conservatively, and their mild to moderate hemiparesis due to the pseudomeningocele-induced compression abated. The high incidence of anterior cervical pseudomeningoceles seen at the authors' institution within a relatively brief period suggests that this lesion is not rare. The authors believe that it is important to recognize the compressive nature of these lesions and their potential to cause devastating neurological injury.

Object: The risk factors of halo fixation in elderly patients have never been analyzed. The authors therefore retrospectively reviewed data obtained in the treatment of such cases., Methods: A discharge database was searched for patients 70 years of age or older who had undergone placement of a halo device. In a search of cases managed between April 1999 and February 2005, data pertaining to 53 patients (mean age 79.9 years [range 70-97 years]) met these criteria. Forty-one patients were treated for traumatic injuries. Ten patients had deficits ranging from radiculopathy to quadriparesis, and 43 had no neurological deficit. Adequate follow-up material was available in 42 patients (mean treatment duration 91 days). Halo immobilization was the only treatment in 21 patients, and adjunctive surgical fixation was undertaken in the other 21 patients. There were 31 complications in 22 patients: respiratory distress in four patients, dysphagia in six, and pin-related complications in 10. Eight patients died; in two of these cases, the cause of death was clearly unrelated to the halo brace. The other six patients died of respiratory failure and cardiovascular collapse (perioperative mortality rate 14%). Three patients who died had sustained acute trauma and three had undergone surgical stabilization., Conclusions: External halo fixation can be used safely to treat cervical instability in elderly patients. The high complication rate in this population may reflect the significant incidence of underlying disease processes.

Object: In this study the authors evaluated the mechanisms and patterns of injury and the factors affecting management and outcome of pediatric subaxial cervical spine injuries (C3-7)., Methods: Fifty-one pediatric patients (38 boys and 13 girls; mean age 12.4 years, range 10 months-16 years) with subaxial cervical spine injuries were reviewed retrospectively. Motor vehicle accidents (MVAs) were the most common cause of injury. Overall, 12% presented with a dislocation, 63% with a fracture, 19% with a fracture-dislocation, and 6% with a ligamentous injury. The most frequently injured level was C6-7 (33%); C3-4 (6%) was least frequently involved. Sixty-four percent of patients were neurologically intact, 16% had incomplete spinal cord injuries (SCIs), 14% had complete SCIs, and three patients (6%) died after admission and before assessment. Treatment was conservative in 64%: seven (13%) wore a halo vest and 26 (51%) wore a rigid cervical orthosis. Surgery was performed in the other 18 patients (36%), with the breakdown as follows: 15 (30%) underwent an anterior approach, two (4%) had posterior approaches, and one (2%) had a combined approach. Postoperatively, four patients (8% who had a neurological deficit improved. The overall mortality rate was 8%; all deaths were related to MVAs. There were no surgery-related deaths or complications., Conclusions: Subaxial cervical spine injuries are common in children 9 to 16 years of age, and occur principally between C-5 and C-7. Multilevel injury is more common in children 8 years of age and older than in younger children and infants. Most patients with subaxial cervical spine injuries can be treated conservatively. Both anterior and posterior approaches are safe and effective.

Because skeletal dysplasias are primary disorders of bone, they have not been commonly understood as neurosurgical diseases. Nevertheless, neurosurgical complications are commonly encountered in many cases of dysplasia syndromes. The authors present two cases of skeletal dysplasia that caused overt instability of the cervical spine. One patient with a diagnosis of Gorham disease of the cervical spine was treated with prolonged fixation in a halo brace after an initial attempt at instrumentation with a posterior occiput--C4 fusion. The other patient, who at birth was identified to have camptomelic dysplasia, has been treated conservatively from the outset. Although these two patients presented with different disorders--in one patient adequate mature bone never formed and in the other patient progressive bone loss became apparent after a seemingly normal initial development--these cases demonstrate unequivocally that surgical options for fusion are ultimately limited by the quality of the underlying bone. In patients in whom the bone itself is inadequate for use as a substrate for fusion, there are currently limited treatment options. Future improvements in our understanding of chondrogenesis and ossification may lead to the design of superior methods of encouraging fusion in these patients; however, at the present time, long-term maintenance in a halo brace may, in fact, be the only treatment.
