The exact contribution of the insula to risky decision making remains unclear, as are the specific outcome parameters and inter-individual characteristics that modulate insular activity prior to a risky choice. This fMRI study examines the contributions of outcome valence, magnitude, probability, and expected value (EV) to insular activity during risky decision making, and explores the influence of sensitivity to reward and to punishment, and anxiety, to insular activity. Participants (N = 31) performed a gambling task requiring choice between two roulettes with different outcome magnitude, probability and EV, under gain and loss conditions separately, and filled questionnaires assessing sensitivity to punishment/reward, and state/trait anxiety. Parametric analyses were conducted to examine the modulation of brain activity during decision making in relation to each task parameter. Correlations were examined between insular activity and psychometric questionnaires. EV of the selected roulette was associated with right posterior insula activation during decision making. Higher sensitivity to punishment was associated with lower bilateral insular activation. These findings suggest that the right posterior insula is involved in tracking the EV of a risky option during decision making. The involvement of the insula when making risky decisions also appears to be influenced by inter-individual differences in sensitivity to punishment.

Functional neuroimaging studies using auditory stimuli consistently show activation of the insular cortex. However, due to the limited temporal resolution of non-invasive neuroimaging techniques, the role(s) of the insula in auditory processing remains unclear. As the anterior insula (aI) and the posterior insula (pI) have different connections and are thought to be functionally distinct, it is likely that these two areas contribute differently to auditory processing. Our study examines the spatiotemporal dynamics of auditory processing in the insula using intracranial electroencephalography (EEG). Eight epileptic patients completed two passive listening tasks and one three-stimulus auditory oddball detection task during the intracranial EEG monitoring of their drug-resistant seizures. Recordings were obtained from depth electrodes implanted in 11 insulae. Event-related potentials (ERPs) were analyzed using permutation analyses during the N100 and the P300 intervals, and modulations of alpha, theta, and gamma band responses were compared using Wilcoxon/Mann-Whitney analyses. N100 responses to auditory stimuli were mostly observed in the pI and were little affected by task conditions. Auditory target detection was associated with P300 ERPs, and alpha, theta, high- and low-gamma responses, preferentially at aI contacts. Results suggest that the aI is involved in voluntary attentional processing of task-relevant information, whereas the pI is involved in automatic auditory processing.

Functional neuroimaging studies suggest that the insular cortex-and more especially the anterior insula (aI)-is involved in attentional processes and plays a crucial role in the "salience network". However, its specific role in attentional processing remains unclear, which is partly attributable to the low temporal resolution of non-invasive neuroimaging techniques. This study aims to examine the spatio-temporal dynamics of visual target processing using intracranial EEG recorded directly from the insula. Eight epileptic patients (four women, age 18-44 years) completed a three-stimulus visual oddball task during the extraoperative invasive intracranial EEG (iEEG) monitoring of their drug-resistant seizures. Depth electrodes were implanted in ten insular lobes (5 left and 5 right) and provided a total of 59 recording contacts in the insula. Event-related potentials (ERPs) and high-gamma-band responses (GBRs) were processed offline. Permutation analyses were performed to compare ERP signals across conditions during the P300 (225-400) interval, and modulations of GBRs (70-150 Hz) were computed for separate 100 ms time windows (from 0 to 1000 ms post-stimulus) and compared across conditions using non-parametric Wilcoxon test. Target stimuli were associated with a P300 (250-338 ms) component for 39% of contacts implanted in the aI, most probably reflecting voluntary attentional processing. Amplitude was significantly greater for target than for standard stimuli for all of these contacts, and was greater than for novel stimuli for 72%. In the posterior insula (pI), 16% of contacts showed preferential responses to target stimulus in the P300 interval. Increased GBRs in response to targets were observed in 53% of aI contacts (from ≈ 200 to 300 ms) and in 43% of pI contacts (from ≈ 400 to 500 ms). This study is the first to characterize the spatio-temporal dynamics of visual target processing in the insula using iEEG. Results suggest that visual targets elicit a P300 in the aI which corresponds in latency to the P3b component, suggesting that this region is involved in top-down processing of task-relevant information. GBRs to visual targets occur earlier in the aI than in the pI, further characterizing their respective roles in voluntary attentional processing.

Background: Having a parent with an anxiety disorder increases the risk of anxiety symptoms and anxiety disorders during the lifespan. Moreover, childhood and adolescence anxiety disorders and symptoms have been linked to a range of brain structure abnormalities. However, to date, no study has investigated brain anatomy in adolescents at high risk based on parental anxiety disorders and in adolescents with an anxiety disorder but without any treatment or therapy., Methods: Anatomical images from magnetic resonance imaging of 68 adolescents with anxiety disorders without any treatment (N = 20), at risk for anxiety because of their parents' anxiety disorders (N = 21), and comparison youths (N = 27), were analyzed using Freesurfer., Results: Compared to comparison group, smaller cortical thickness of the rostral anterior cingulate cortex and of the insula was observed in anxious and at-risk groups; smaller amygdala volume was observed in the anxious group only., Limitations: The age range studied is large (10 to 17 years old). Moreover, this study is cross-sectional. Since adolescence is one of the biggest periods of cerebral reorganization, longitudinal follow-up of these youths would be necessary., Conclusions: Smaller rostral anterior cingulate cortex and insula cortical thickness appear to be cerebral markers of the risk of developing an anxiety disorder in adolescence. Arithmetic reasoning activates the occipital cortex of congenitally blind people (CB). This activation of visual areas may highlight the functional flexibility of occipital regions deprived of their dominant inputs or relate to the intrinsic computational role of specific occipital regions. We contrasted these competing hypotheses by characterizing the brain activity of CB and sighted participants while performing subtraction, multiplication and a control letter task. In both groups, subtraction selectively activated a bilateral dorsal network commonly activated during spatial processing. Multiplication triggered activity in temporal regions thought to participate in memory retrieval. No between-group difference was observed for the multiplication task whereas subtraction induced enhanced activity in the right dorsal occipital cortex of the blind individuals only. As this area overlaps with regions showing selective tuning to auditory spatial processing and exhibits increased functional connectivity with a dorsal "spatial" network, our results suggest that the recruitment of occipital regions during high-level cognition in the blind actually relates to the intrinsic computational role of the activated regions., (Copyright © 2018 Elsevier Inc. The insula is now regarded as a potential site of epileptogenesis in drug-resistant epilepsy, and the advent of microsurgical techniques has allowed insular cortectomy to become a treatment of choice when the insular cortex is involved in the seizure focus. However, considering the evidence of an insular role in socio-emotional processing, it remains unknown whether these cortical resections disturb personality and social behavior as experienced in daily life. We examined such changes in a group of patients (n=19) who underwent epilepsy surgery involving partial or complete resection of the insula, and compared them to a group of patients who underwent standard temporal lobe epilepsy (TLE) surgery (n=19) as a lesion-control group. Participants were assessed on the Iowa Scales of Personality Change, filled by a close relative at least six months after surgery. While postoperative changes did not significantly differ between groups on any of the ISPC items, insular resections were associated with mild but significant increases in irritability, emotional lability, anxiety, and frugality postoperatively, which, apart from anxiety, were not significant among TLE patients. Our results are congruent with the idea that the insula contributes to emotion processing. We used BrainVisa software in an exploratory analysis measuring the depth and sulcal profile of the central sulci of congenitally blind and sighted individuals. We found the greatest differences between the groups at locations on the central sulcus corresponding with the pli de passage fronto-parietal moyen (PPFM), suggesting a cortical reorganization of the primary sensorimotor area of the hand within the central sulcus. This may be in response to the congenitally blind individuals' mastery of Braille or general increase of hand use in everyday life.

Besides the prefrontal cortex, the insula and medial structures of the temporal lobe are thought to be involved in risky decision-making. However, their respective contributions to decision processes remain unclear due to the lack of studies involving patients with isolated insular damage. We assessed adult patients who underwent resection of the insula (n = 13) or of the anterior temporal lobe (including medial structures) (n = 13) as part of their epilepsy surgery, and a group of healthy volunteers (n = 20), on the Iowa Gambling Task (IGT) and on the Cups Task. Groups were matched on sociodemographic, estimated-IQ and surgery-related factors. On the IGT, patients with temporal lobe resection performed significantly worse than both the insular and healthy control groups, as they failed to learn which decks were advantageous on the long-term. On the Cups Task, the insular and temporal groups both showed impaired sensitivity to expected value in the loss domain, when compared with healthy controls. These findings provide clinical evidence that the insula and mesiotemporal structures are specifically involved in risky decision-making when facing a potential loss, and that temporal structures are also involved in learning the association between behavior and consequences in the long-term., (© The Author (2016). The insula is a multisensory area involved in various brain functions, including central auditory processing. However, its specific role in auditory function remains unclear. Here we report three cases of persistent hypersensitivity to auditory stimuli following damage to the insular cortex, using behavioral and neurophysiological measures. Two patients who complained of auditory disturbance since they suffered an isolated unilateral insular stroke, and one epileptic patient who underwent right insular resection for control of drug-resistant seizures, were involved in this study. These patients, all young adult women, were tested for auditory function more than one year after brain injury, and were compared to 10 healthy control participants matched for age, sex, and education. The assessment included pure-tone detection and speech detection in quiet, loudness discomfort levels, random gap detection, recognition of frequency and duration patterns, binaural separation, dichotic listening, as well as late-latency auditory event-related potentials (ERPs). Each patient showed mild or moderate hyperacusis, as revealed by decreased loudness discomfort levels, which was more important on the side of lesion in two cases. Tests of temporal processing also revealed impairments, in concordance with previous findings. ERPs of two patients were characterised by increased amplitude of the P3b component elicited during a two-tone auditory oddball detection task. This study is the first to report cases of persistent hyperacusis following damage to the insular cortex, and suggests that the insula is involved in modulating the perceived intensity of the incoming auditory stimuli during late-stage processing., (Copyright © 2015 Elsevier B.V. The insula has been implicated in social cognition and empathy in several neuroimaging paradigms. Impairments in social information processing, including specific deficits in disgust recognition, have been described following isolated insular damage, although the evidence remains limited to a few case studies. The present study examines social cognition and empathy in a group of fifteen patients for whom the insula was removed as part of their epilepsy surgery. These patients were compared to a lesion-control group of 15 epileptic patients who had a surgery in the anterior temporal lobe that spared the insula, and to 20 healthy volunteers matched on age, sex, and education. Participants were assessed on an Emotion Recognition Task (ERT), the Reading the Mind in the Eyes test, and a self-administered empathy questionnaire. Patients who underwent insular resection showed poorer ability to recognize facial expressions of emotions and had lower scores of perspective taking on the empathy questionnaire than healthy controls. Using results from healthy controls as normative data, emotion recognition deficits were more frequent in insular patients than in both other groups. Specific emotion analyses revealed impairments in fear recognition in both groups of patients, whereas happiness and surprise recognition was only impaired in patients with insular resection. There was no evidence for a deficit in disgust recognition. The findings suggest that unilateral damage to the operculo-insular region may be associated with subtle impairments in emotion recognition, and provide further clinical evidence of a role of the insula in empathic processes. However, the description of 15 consecutive cases of insula-damaged patients with no specific deficit in disgust recognition seriously challenges the assumptions, based on previous case reports, that the insula is specifically involved in disgust processing., (Copyright © 2015 Elsevier Ltd. Resection of the insular cortex is becoming more frequent as it is increasingly recognized that a nonnegligible proportion of surgical candidates with drug-resistant epilepsy have an epileptogenic zone that involves the insula. In the last decades, however, the insula has been proposed to be involved in several neuropsychological functions, and there is a lack of documentation on whether partial or complete insulectomy results in permanent cognitive impairments in this clinical population. In this study, we conducted standard preoperative and postoperative neuropsychological assessments in 18 patients undergoing epilepsy surgery that included the removal of the insula in the right (n=13) or the left (n=5) hemisphere. Postoperative testing was conducted at least five months after surgery. Cognitive impairments were common and heterogeneous prior to surgery, with language and verbal memory impairments being especially frequent among patients in whom epileptic seizures originated from the left hemisphere. After surgery, declines and improvements occurred on a variety of outcomes, although new deficits were relatively infrequent among patients who had obtained normal performance at baseline. Statistical comparisons between preoperative and postoperative assessments revealed significant deterioration of only one outcome - the color naming condition of the Stroop test - which relies on oro-motor speed and lexical access. These findings suggest that partial or complete resection of the insular cortex in patients with drug-refractory epilepsy can be conducted without major permanent neuropsychological impairments in a vast majority of patients. However, small decrements in specific cognitive functions can be expected, which should also be taken into account when considering the surgical option in this clinical population., (Copyright © 2014 Elsevier Inc. Our comprehension of the neural mechanisms underlying emotional information processing has largely benefited from noninvasive electrophysiological and functional neuroimaging techniques in recent years. However, the spatiotemporal dynamics of the neural events occurring during emotional processing remain imprecise due to the limited combination of spatial and temporal resolution provided by these techniques. This study examines the modulations of high-frequency activity of intracranial electroencephalography recordings associated with affective picture valence, in epileptic patients awaiting neurosurgery. Recordings were obtained from subdural grids and depth electrodes in eight patients while they viewed a series of unpleasant, pleasant and neutral pictures from the International Affective Picture System. Broadband high-gamma (70-150 Hz) power was computed for separate 100-ms time windows and compared according to ratings of emotional valence. Compared to emotionally neutral or pleasant pictures, unpleasant stimuli were associated with an early and long-lasting (≈200-1,000 ms) bilateral increase in high-gamma activity in visual areas of the occipital and temporal lobes, together with a late and transient (≈500-800 ms) decrease found bilaterally in the lateral prefrontal cortex (PFC). Pleasant pictures were associated with increased gamma activity in the occipital cortex, compared to the emotionally neutral stimuli. Consistent with previous studies, our results provide direct evidence of emotion-related modulations in the visual ventral pathway during picture processing. Results in the lateral PFC also shed light on the neural mechanisms underlying its role in negative emotions processing. Little is known about the relation of alpha rhythms and object recognition. Alpha has been generally proposed to be associated with attention and memory and to be particularly important for the mediation of long-distance communication between neuronal populations. However, how these apply to object recognition is still unclear. This study aimed at describing the spatiotemporal dynamics of alpha rhythms while recognizing fragmented images of objects presented for the first time and presented again 24 hr later. Intracranial electroencephalography was performed in six epileptic patients undergoing presurgical evaluation. Time-frequency analysis revealed a strong alpha activity, mainly of the evoked type, propagating from posterior cerebral areas to anterior regions, which was similar whether the objects were recognized or not. Phase coherence analysis, however, showed clear phase synchronization specific for the moment of recognition. Twenty-four hr later, frontal regions displayed stronger alpha activity and more distributed phase synchronization than when images were presented for the first time. In conclusion, alpha amplitude seems to be related to nonspecific mechanism. Phase coherence analysis suggests a communicational role of alpha activity in object recognition, which may be important for the comparison between bottom-up representations and memory templates.

Visual perception is often challenged by various difficulties that act concomitantly and whose respective impacts may therefore be hard to distinguish. We used event-related potentials to dissociate the impact of target saliency, generated by occlusion, from that of interference produced by incongruent nontargets. In one block, the target (a square) partially occluded another square tilted by 45 degrees. This nontarget square interfered only to a small extent with target perception. In another block, the target was the occluded stimulus, and interference from the nontarget was substantial. Blocks including two kinds of overlapping shapes (a cross and a square) were added to control for the interference effect. Block comparisons revealed that occlusion modulated an occipital N250 and reaction times. In contrast, interference modulated a parietal N380 but not reaction times.

Early blind persons have often been shown to be superior to sighted ones across a wide range of non-visual perceptual abilities, which in turn are often explained by the functionally relevant recruitment of occipital areas. While voice stimuli are known to involve voice-selective areas of the superior temporal sulcus (STS) in sighted persons, it remains unknown if the processing of vocal stimuli involves similar brain regions in blind persons, or whether it benefits from cross-modal processing. To address these questions, we used fMRI to measure cerebral responses to voice and non-voice stimuli in blind (congenital and acquired) and sighted subjects. The global comparison of all sounds vs. silence showed a different pattern of activation between blind (pooled congenital and acquired) and sighted groups, with blind subjects showing stronger activation of occipital areas but weaker activation of temporal areas centered around Heschl's gyrus. In contrast, the specific comparison of vocal vs. non-vocal sounds did not isolate activations in the occipital areas in either of the blind groups. In the congenitally blind group, however, it led to a stronger activation in the left STS, and to a lesser extent in the fusiform cortex, compared to both sighted participants and those with acquired blindness. Moreover, STS activity in congenital blind participants significantly correlated with performance in a voice discrimination task. This increased recruitment of STS areas in the blind for voice processing is in marked contrast with the usual cross-modal recruitment of occipital cortex.

Until only a few decades ago, researchers still considered sensory cortices to be fixed or "hardwired," with specific cortical regions solely dedicated to the processing of selective sensory inputs. But recent evidences have shown that the brain can rewire itself, showing an impressive range of cross-modal plasticity. Visual deprivation is one of the rare human models that allow us to explore the role of experience-dependent plasticity of a sensory cortex deprived of its natural inputs. The objective of this paper is to describe recent results regarding the spatial processing of sounds in blind subjects. These studies suggest that blind individuals may demonstrate exceptional abilities in auditory spatial processing and that such enhanced performances may be intrinsically linked to the recruitment of occipital areas deprived of their normal visual inputs. Such results highlight the brain's remarkable ability to rewire its components to compensate for the challenging neurological condition that is visual deprivation. Moreover, we shall discuss that such cross-modal recruitment may, to some extent, follow organizational principles similar to the functional topography of the region observed in the sighted. Even if such recruitment is especially present in individuals having lost their sight in early infancy, occipital regions also show impressive plastic properties when vision is lost at a later age. This observation will be related to recent results demonstrating that occipital regions play a more important role than previously expected in the spatial processing of sounds, even in sighted subjects. Putative physiological mechanisms underlying such cross-modal recruitment will then be discussed. All these results have important implications for understanding the role of visual experience in shaping the development of occipital regions and may guide the implementation of rehabilitative methods such as sensory substitution or neural implants.

The aim of this study was to investigate cortical activation in response to binaural stimulus presentations in an individual (FX) with a circumscribed traumatic hemorrhagic lesion of the right inferior colliculus. FX and control subjects were exposed to complex sounds while undergoing a functional magnetic resonance imaging assessment. Whereas normally-hearing individuals show well-balanced bilateral activation patterns in response to binaural auditory stimulation, the same stimuli produced stronger activation in the left hemisphere in FX. Combined with previous data, these findings reinforce the notion that the inferior colliculus is an essential auditory relay and that its loss cannot be significantly compensated.

Previous studies have shown that unilateral painful but not tactile stimulation produces ipsilateral cortical activation in callosotomized patients. Here, we used functional magnetic resonance imaging (fMRI) to compare activation evoked by tactile and thermal pain stimulation in two individuals with callosal agenesis, one callosotomized patient, and six control subjects. Bilateral tactile activation was found in S1 and/or S2 of both hemispheres in control and acallosal subjects whereas no ipsilateral activation was detected in these structures in the callosotomized participant. In contrast, although there was some inter-individual variability in the pattern of responses to pain, all subjects including the callosotomized patient showed ipsilateral responses in at least two of the target pain-related areas (S1, S2, insula and/or cingulate cortex). These findings are consistent with the plasticity of the touch system in callosal agenesis and further confirm that ipsilateral activation of pain-related regions does not require the integrity of the corpus callosum.

Human blind individuals have demonstrated cross-modal plasticity in research over the past decade. In one such study, we showed that early-blind subjects were able to localize sound sources accurately despite the lack of visual input for the calibration of their auditory space. A further ERP study with these subjects also revealed N1 and P3 components during a sound localization task to be more posteriorly distributed than for sighted controls, indicating an involvement of posterior regions in sound localization for blind subjects not present for sighted subjects. In the current study, we analyzed these data for EEG power and coherence in theta, alpha, beta, and gamma frequency bands to see whether blind individuals would show increased coherence reflecting increased connectivity between the central and posterior cortical regions. Blind and sighted subjects did not differ with respect to overall EEG power in any frequency range. However, EEG coherence was significantly increased in blind subjects compared to sighted in the theta, alpha, and beta frequency bands. These results have implications for cortical plasticity affected by sensory deprivation in humans.

Modality specificity of neuronal responses to visual, somesthetic and auditory stimuli was investigated in the anterior ectosylvian cortex (AEC) of cats, using single-unit recording techniques. Seven classes of neurons were found, and according to their responsiveness to sensory stimuli regrouped into three categories: unimodal, bimodal and trimodal. Unimodal cells that responded to only one of the three stimulus modalities formed 59% of the units; 30.2% were bimodal, in that they showed a clear increase of neuronal discharges to two of the three stimulus types; 10.8% were defined as trimodal because they responded to all three stimulus modalities. Although the different categories of cells were intermingled within the AEC, indicating a certain degree of overlap between sensory modalities, some clustering of cell types was nonetheless evident. Thus, the somatosensory responsive cells were mainly located in the anterior two-thirds of the dorsal bank of the anterior ectosylvian sulcus. Visually responsive cells were concentrated on the ventral bank of the sulcus, whereas neurons with an auditory response occupied the banks and fundus of the posterior three-quarters of the sulcus. The histological distribution and physiological properties of AEC neurons suggest that this cortical region is a higher-order associative area whose function may be to integrate information from different sensory modalities.

Sensory interactions, namely, the responses of single cells to stimulations originating from the two sides of the body or from the two visual fields, or from more than one sensory modality (namely, visual, auditory and somatosensory), were evaluated within the anterior ectosylvian cortex (AEC) of cats. Results showed that responses of single neurons to a stimulus of one modality can be enhanced or inhibited by the presentation of another stimulus of either the same or another modality. This facilitatory or inhibitory modulation seems to depend upon temporal and/or spatial relationships between the stimuli. These results, taken together with those previously obtained in our laboratory and by others, suggest that neurons in the AEC may be involved in integrating inputs from various modalities and possibly linking sensory input with action.

The effects of bilateral removal of cortical areas 17-18 were investigated in the cat; these areas represent the central portion of the visual field and the effect of their removal was evaluated with reference to the perception of Julesz random-dot stereograms. Animals were trained in a two-choice discrimination box to choose between two stereotargets made out of random dots. When appropriately viewed, one produced a vertical rectangle and the other an horizontal one, which appeared to float out in space (crossed stereopsis). The results indicated that all normal cats could solve the random-dot task. Following the cortical lesions, stereoscopic perception was abolished. We also tested for the possibility that this inability to solve the random-dot problem was due to a more general acuity loss. Vernier-type acuity comparing a continuous to a disjointed line showed this to be within the animals' discriminative ability. Offset acuity of the lines was better than that of the stereodot patterns. On the other hand, the ability to determine the preoperatively acquired brightness and pattern discriminations was preserved, although some retraining was necessary for the more difficult patterns. It is therefore suggested that the primary visual cortex, at least in the cat, is involved in the perception of global stereopsis independently of its implication in the discrimination of bidimensional patterns.

Ipsilateral cerebellar and pyramidal signs suddenly developed in three patients. Computed tomography revealed small infarcts of the corona radiata in two patients and a small infarct in the posterior limb of the internal capsule in the third. Ataxic hemiparesis can result from lesions of the corona radiata as well as many other points along the corticopontine pathways.

Abstract: The present study investigated the spatial properties of cells in the postero-lateral lateral suprasylvian (PLLS) area of the cat and assessed their sensitivity to edges defined by motion. A total of one hundred and seventeen (117) single units were isolated. First, drifting sinusoidal gratings were used to assess the spatial properties of the cells'' receptive fields and to determine their spatial frequency tuning functions. Second, random-dot kinematograms were used to create illusory edges by drifting textured stimuli (i.e. a horizontal bar) against a similarly textured but static background. Almost all the cells recorded in PLLS (96.0%) were binocular, and a substantial majority of receptive fields (79.2%) were end-stopped. Most units (81.0%) had band-pass spatial frequency tuning functions and responded optimally to low spatial frequencies (mean spatial frequency: 0.08 c./degree). The remaining units (19.0%) were low-pass. All the recorded cells responded vigorously to edges defined by motion. The vast majority (96.0%) of cells responded optimally to large texture elements; approximately half the cells (57.3%) also responded to finer texture elements. Moreover, 38.5% of the cells were selective to the width of the bar (i.e., the distance between the leading and the trailing edges). Finally, some (9.0%) cells responded in a transient fashion to leading and to trailing edges. In conclusion, cells in the PLLS area are low spatial frequency analyzers that are sensitive to texture and to the distance between edges defined by motion. [Copyright &y& Elsevier]
