Background: Preliminary studies have raised the question of whether certain prosthetic biomaterials used in total hip arthroplasty (THA) bearings are associated with increased risk of periprosthetic joint infection (PJI). For example, some observational data suggest the risk of PJI is higher with metal-on-metal bearings. However, it is not known whether other bearings-including ceramic bearings or metal-on-polyethylene bearings-may be associated with a higher or lower risk of PJI., Questions/purposes: The objective of this study was to use a national arthroplasty registry to assess whether the choice of bearings-metal-on-polyethylene (MoP), ceramic-on-polyethylene (CoP), ceramic-on-ceramic (CoC), or metal-on-metal (MoM)-is associated with differences in the risk of revision for deep infection, either (1) within 6 months or (2) over the entire period of observation, which spanned 15 years., Methods: Data from primary THAs were extracted from the New Zealand Joint Registry over a 15-year period. A total of 97,889 hips were available for analysis. Inclusion criterion was degenerative joint disease; exclusion criteria were previous surgery, trauma, and any other diagnosis (12,566 hips). We also excluded a small group of ceramic-on-metal THAs (429) with short followup. The median observation period of the selected group of hips (84,894) was 9 years (range, 1-15 years). The mean age of patients was 68 years (SD ± 11 years), and 52% were women. There were 54,409 (64%) MoP, 16,503 (19%) CoP, 9051 (11%) CoC, and 4931 (6%) MoM hip arthroplasties. Four hundred one hips were revised for deep infection. A multivariate assessment was carried out including the following risks factors available for analysis: age, sex, operating room type, use of body exhaust suits, THA fixation mode, and surgeon volume. Because of late introduction of data collection in the Registry, we were unable to include body mass index (BMI, recording started 2010) and medical comorbidities according to the American Society of Anesthesiologists class (ASA, recording started 2005) in the multivariate analysis., Results: The rate of early PJI (< 6 months) did not differ by bearing surface. In contrast, we observed a difference over the total observation period. Within the first 6 months after the index surgery, CoC THAs were not associated with a lower risk of revision for PJI (p = 0.118) when compared with CoP (hazard ratio [HR], 1.31; 95% confidence interval [CI], 0.50-3.41), MoP (HR, 2.10; CI, 0.91-4.82), and MoM (HR, 2.04; CI, 0.69-6.09). When the whole observation period was considered, CoC hips were associated with a lower risk of revision for deep infection when compared with CoP (HR, 1.30; CI, 0.78-2.18; p = 0.01), MoP (HR, 1.75; CI, 1.07-2.86; p = 0.02), and MoM (HR, 2.12; CI, 1.23-3.65; p = 0.008)., Conclusions: Our finding associating CoC THA bearings with a lower risk of infection after THA must be considered very preliminary, and we caution readers against attributing all of the observed difference to the bearing surface. It is possible that some or all of the observed difference associated with bearing type may have been driven by other factors such as ASA and BMI, which could not be included in our multivariate analysis, and so future registry studies on this topic must assess those variables carefully., Level of Evidence: Level III, therapeutic study.

Background: Poor survival of THA implants in very young patients has been attributed to use of cemented implants, wear of conventional polyethylene, and the presence of morphologic deformities in the proximal femur or in the acetabulum. Few studies have reported the long-term results of ceramic-on-ceramic implants in THAs in patients younger than 20 years., Question/purposes: We determined: (1) the proportion of patients who experienced complications related to the ceramic bearing (squeaking, fracture); (2) the survivorship free from loosening and free from revision for any reason; (3) whether patients with osteonecrosis had inferior survivorship compared with patients whose surgical indication was all other diagnoses including sequelae of pediatric hip disorders (developmental dysplasia of the hip, Legg-Calvé-Perthes disease, slipped capital femoral epiphysis); and (4) clinical function., Methods: Between 1979 and 2013, we performed 113 primary THAs in 91 patients younger than 20 years at the time of surgery. Of those, 105 THAs (83 patients) were done with ceramic-on-ceramic bearings (91% of the 91 patients); during that period, a ceramic-on-ceramic bearing couple was indicated in all patients younger than 20 years. In eight patients (eight hips), a cemented polyethylene cup was implanted because the diameter of the acetabulum was smaller than the smallest available ceramic cup (46 mm), or because adequate fixation of a ceramic press-fit cup could not be achieved despite careful reaming of the acetabulum. The most common diagnosis indicating THA was avascular necrosis of the femoral head (56.2%; 59 hips). Thirty-five patients (40 hips) had undergone previous operations before the replacement. Three patients (4%; four hips) died from unrelated causes, nine patients (11%; 13 hips) were lost to followup, and four patients (five hips) had a followup greater than 8.5 years but have not been seen in the last 5 years. Patients were assessed clinically (using the Merle d'Aubigne-Postel score, Hip disability and Osteoarthritis Outcome score (HOOS), and the SF-12(®) Health Survey, and radiographically for signs of radiolucencies, subsidence, or osteolysis on plain films. The mean followup was 8.8 ± 6.1 years (range, 2-34.4 years)., Results: Five patients experienced transient noise generation, defined as a snap in four patients and squeaking in one. Seventeen hips underwent revision surgery-16 for aseptic loosening and one for septic loosening. The implant survival rate at 10 years with aseptic loosening as the endpoint was 90.3% (95% CI, 82.4%-98.9%). No hip had acetabular or femoral osteolysis. Survivorship in patients with osteonecrosis did not differ from survivorship in patients with other diagnoses. The Merle d'Aubigne-Postel score increased from 10.1 ± 4.0 to 17.6 ± 1.1 (p < 0.01); the mean HOOS score was 79.3 ± 13.8 (range, 50.6-100); the mean SF-12(®) physical and mental component scores were 48.1 ± 7.9 (range, 21.4-57.6), and 47.4 ± 12.2 (range, 24.5-99.4), respectively., Conclusions: We found that patient-reported outcomes scores improved in most patients undergoing THA in this very young study group. Underlying diagnosis did not affect long-term survivorship. However, the revision-free survival rate at 10 years is lower than published estimates in older patients, and with 11% of patients lost to followup, our estimates may represent a best-case scenario. Therefore, we believe THA should be performed as a last resort in this population., Level of Evidence: Level IV, therapeutic study.

Background: Dislocation is a major complication after primary total hip arthroplasty (THA), but little is known about the potential relationships between bearing materials and risk of dislocation. Dislocation within the first year after surgery is typically related to either surgical error or patient inattention to precautions, but the reasons for dislocation after the first year are often unclear, and whether ceramic bearings are associated with an increased or decreased likelihood of late dislocation is controversial., Questions/purposes: The purpose of this study was to use a national registry to assess whether the choice of bearings-metal-on-polyethylene (MoP), ceramic-on-polyethylene (CoP), ceramic-on-ceramic (CoC), or metal-on-metal (MoM)-is associated with differences in the risk of late dislocation., Methods: Data from primary THAs were extracted from the New Zealand Joint Registry over a 10-year period. The mean age of patients was 69 years (SD ± 12 years), and 53% were women. The median followup in this population was 7 years (range, 1-13 years). The surgical approach used was posterior in 66% of THAs, lateral in 29%, and anterior in 5%. The primary endpoint was late revision for dislocation with "late" defined as greater than 1 year postoperatively. A total of 73,386 hips were available for analysis: 65% MoP, 17% CoP, 10% CoC, and 7% MoM. In general, patients receiving CoC and MoM bearings were younger compared with patients receiving CoP and MoP bearings., Results: Four percent of the hips were revised (3130 THAs); 867 THAs were revised for dislocation. Four hundred seventy THAs were revised for dislocation after the first postoperative year. After adjusting for head size, age, and surgical approach, only CoP (hazard ratio [HR], 2.10; p = 0.021) demonstrated a higher proportion of revision, whereas MoP did not (HR, 1.76; 95% p = 0.075). There were no differences of revisions for dislocation in the CoC (HR, 1.60; p = 0.092) and MoM cohorts (HR, 1.54; p = 0.081)., Conclusions: Dislocation is a common reason for revision after THA. The relationships between bearing materials and risk of revision for late dislocation remain controversial. This large registry study demonstrated that bearing surface had little association with the incidence of late dislocation. Future studies with longer followups should continue to investigate this question., Level of Evidence: Level III, therapeutic study.

We analyzed wear pattern of long-term retrieved alumina-alumina hip prostheses from Osteal, which were implanted for 15-19 years. A comparison was carried out with our previous study of 17-year Biolox alumina-on-alumina hip prostheses, (Shishido et al., J Biomed Mater Res B 2003;67:638-647) and all-alumina total hip replacement run under microseparation simulator tests. Of particular interest was the occurrence of stripe wear in these first generation alumina ceramic bearings. Two balls of Osteal revealed only one stripe wear as did the respective liners on their rim areas. In these latter balls, the stripes were shallower than those previously observed in Biolox implants. A microscopic analysis of the bearing surface was carried out using scanning electron microscopy and fluorescence microprobe spectroscopy. On average, the Osteal retrievals had one grade lower wear than Biolox retrievals. Fluorescence microprobe maps showed that Biolox ball surfaces had higher compressive stress than the Osteal likely due to severe impingement and microseparation promoted by the bulky implant design.

Alumina-on-alumina total hip arthroplasty has been used for 32 years in Europe. The theoretical advantages of this combination are represented by its remarkable sliding characteristics, its very low wear debris generation, and its improved fracture toughness. These advantages are achieved if the material is processed properly with high density, high purity and small grains. We summarize the results obtained with the alumina-on-alumina combination concerning in vitro and in vivo wear behavior with special emphasis on wear debris characterization and quantification and histologic tissue examinations. Alumina-on-alumina seems to be one of the best choices in young and active patients provided that sound socket fixation is maintained in the long term.

During a period of 25 years (1977 to 2001), 13 fractures of an alumina component were recorded retrospectively. During the same period, approximately 5500 alumina components were implanted (3300 with all-alumina bearings and 1200 with alumina-on-polyethylene). These events occurred in seven women and six men, with a mean age of 59 years and a mean weight of 71 kg. There were eight fractures of the femoral head and five fractures of the socket component. Three fractures clearly were related to trauma (two fractures of the socket and one fracture of the head) and two were related to an abnormal design (one 22-mm head and one extra-long neck). Five fractures occurred without any rational explanation, but two fractures that occurred in the early phase could have been caused by a weaker alumina material. Finally three recent fractures of the liner were related to a change in the design of the material. This has been corrected and no additional fractures were observed. Although this dramatic event is of concern, it is infrequent and easy to solve by a limited revision procedure if done emergently. This rare complication should be balanced with the other complications of THR, including mechanical failures and osteolysis, often described with metal-on-polyethylene prostheses.

Melt-derived bioactive glasses have been used with success in various clinical applications for over 10 years. Recently, particles of sol-gel-derived bioactive glasses with an initial high specific surface area have been shown to exhibit excellent osteoconductive properties as well as significant degradability. In this work, we explored the long-term in vivo bioactivity and degradability of bulk sol-gel-derived glasses in a rabbit model. Two sol-gel compositions (58S and 77S Bioglass) were used. Bulk 45S5 Bioglass was used as a control. Both sol-gel-derived glasses demonstrated osteoconductive properties similar to 45S5 Bioglass. In addition, absorbability was observed for both sol-gel-derived glasses starting after 12 weeks of implantation. Total absorption reached 40% after 52 weeks. No degradation could be measured in the case of bulk 45S5 melt-derived Bioglass within 1 year of implantation. The degradation process was highly time dependent, as demonstrated by regression analysis. New bone formation was found to fill in areas that had been resorbed, similar to bone remodeling. This absorbability can be assumed to be at least partially related to an osteoclastic resorption as viable osteoclasts-like cells were found to be in direct contact with the glass surfaces.

Despite their impact on the healing of soft connective tissue, fibrin sealants have not been shown conclusively to have an important role in the healing of bone defects. We report the positive influence of fibrin sealants on repair of cancellous bone cavities filled with a porous, resorbable ceramic. We studied two fibrin sealants: Autocolle and Tissucol. Autocolle is enriched in platelet factors during its preparation. Tissucol is a commercially available fibrin glue prepared from pooled human plasma that has no enrichment in platelet factors. Cavities 10 mm in depth and 5 mm in diameter were drilled in lateral condyles of 45 New Zealand rabbits. These defects were filled with either coral granules or a mixture of fibrin sealant (Autocolle or Tissucol) and coral granules or left empty. At 1 month addition of a fibrin sealant (Autocolle or Tissucol) to the coral led to a significant increase in bone formation in comparison to coral alone. At 2 months significant fibrin sealant mediated enhancement of bone repair was observed with Autocolle only. At 6 months bone formation was similar to the adult bone amount in nonoperated animals, whatever the initial material. Control cavities, on the other hand, were invaded with fibrous tissue only at each time period.

We made a semiquantitative study of the comparative histology of pseudomembranes from 12 loose cemented ceramic-ceramic and 18 metal-polyethylene total hip replacements. We found no significant difference in cellular reaction between the two groups, but there was a major difference in the origin of the particulate debris. In the metal-polyethylene group, polyethylene of articular origin was predominant, while in the ceramic-ceramic group the cellular reaction appeared to be a response to zirconia ceramic particles used to opacify cement used for fixation. Isolation and characterisation of the debris showed that the zirconia particles formed the greatest proportion (76%) in ceramic-ceramic hips, while alumina debris of articular origin formed only 12%. Our study has indicated that aseptic loosening of ceramic cups is not due to a response to debris generated at the articular interface, but to mechanical factors which lead to fragmentation of the cement.

In contrast to the much-studied mechanism of aseptic loosening of the metal-polyethylene joint couple, the mechanism responsible for failure of ceramic-ceramic (CC) total hip arthroplasties (THAs) has not been evaluated. The aim of this study was to conduct a systematic characterization of the in vivo wear debris from 15 cases of CC THAs revised for aseptic loosening. Two methods were used to evaluate the wear debris; a semiquantitative histological analysis of H&E-stained periprosthetic pseudomembranes; and an evaluation of isolated debris particles using SEM, energy-dispersive X-ray analysis, and image analysis. The three main types of particulate debris identified were titanium alloy (TiAlV) and alumina ceramic (Al2O3) of prosthetic origin, and zirconium dioxide (ZrO2) from the contrast agent used in the cement for prosthetic fixation. Alumina debris was present in the smallest proportion (12%) and was consistent with the low wear rate of the CC joint couple. Zirconium dioxide debris was present in the greatest proportion (76%) and was an unexpected finding. The ZrO2 debris represented microstructural grains of the original ZrO2 particles added as contrast agent to the cement. The presence of a histiocytic foreign body reaction to ZrO2 debris on histologic sections leads us to believe that these particles play an important role in aseptic loosening of the CC THAs evaluated in this study.

In the first 187 consecutive alumina-alumina combination hip arthroplasties performed from 1977 to 1979, both components were cemented with conventional techniques. At ten-year follow-up evaluation, 87 patients were reviewed or interviewed by telephone, 37 were dead, 39 were lost to follow-up evaluation, and 24 failures were reoperated on before the end of ten years. The major cause of failure was aseptic loosening of the acetabular component (15 failures). Fracture of the socket and of the femoral head occurred in five patients in this series. However, these complications were not seen with components manufactured after 1979. At the end of ten years, survivorship analysis depicted a 82.59% survival rate when reoperation was considered as failure and a 88.57% rate when reoperation for aseptic loosening was considered as failure. The femoral component had a 99.16% survival rate and the acetabular component had an 88.57% survival rate when reoperation for aseptic loosening was considered as failure. Age, appearance of a two- or three-zone demarcation at the intermediate follow-up evaluation, and outer diameter of the acetabular component were the major parameters influencing the results. Better results observed in the population younger than 50 years of age may be related to the small amount of wear debris produced by the alumina-alumina combination. This combination in hip prosthesis is secure, but should be implanted in young and active patients; the outer diameter of the acetabular component must be at least 50 mm. The major problem that remains is the socket's fixation. It could be improved by a design modification, by choosing another mode of fixation, or both.

87 consecutive cases of sealed total hip prosthesis with an alumina/alumina friction couple, placed in 76 patients less than 50 years old, have been studied. The surveillance period ranged between 2 and 13 years for 74 of them. 51 have been followed over more than 5 years, 10 have been lost out of sight and 3 died. 5 failures are to be deplored, one by fracture of a 22 mm ceramic head, the normal size in this series being 32 mm; the 4 others occurred after 6, 6, 10 and 11 years and are the consequence of a cotyloid loosening. Compared to the other series in literature concerning young patients and therefore those considered to be "at risk", the results obtained are better, especially if the high number of cases re-operated upon in this series (18 prosthesis exchanges out of 87) is taken into account. The actuarial method of Kaplan Meier gives a probability of 94% of keeping the prosthesis for 8 years. It is undoubtedly the quality of the design of the titanium femoral stem and especially the small amount of erosion scraps produced by the alumina/alumina couple which explain this result. The hip replacement must however remain an exceptional indication in a young subject.

The purpose of this article was to review the laboratory and clinical performances since 1970 of a total hip prosthesis using alumina-alumina combination. The chemical and physical properties of dense alumina ceramic were studied in relation to biocompatibility, mechanical strength, and surface properties. Through the examination of 35 retrieved implants, it was found that the long-term success of alumina-alumina total hip replacement depends on both the ceramic microstructure (small grain size with uniform distribution, minimum porosity, absence of inclusions) and implant geometry (sphericity deviation +/- 1 micron, radius tolerance between components 7-10 microns). Alumina component wear and fractures have disappeared with the use of high-performance materials and severe manufacturing quality control. Examination of human biopsies from well-fixed prostheses showed that alumina particles deposits increase with time with only a low-grade macrophagic reaction. When loosening occurred, an inflammatory reaction appeared; this reaction was less striking than with loose metal-polyethylene prostheses, however. The long-term behavior of cementless alumina cup fixation depends upon initial positioning and stability; survivorship analysis of the cemented ceramic cups showed an 88% survival probability after 8 years with a 1.6% average annual probability of revision. The percentage of surviving was 100% after 8 years in patients who were less than 50 years old. Aseptic loosenings occurring at the cup-cement interface were assumed to be related to stress protection secondary to the high rigidity of the ceramic leading to a weakening of the spongious bone supporting the cement mantle. Good bone stock quality as well as high-quality ceramic appear to be the prerequisites for durable fixation of alumina sockets.

Alumina on the alumina sliding system in a total hip replacement is recognized as a breakthrough in orthopaedic surgery. Advantages and problems with this material are explained and discussed in the light of 36 years of clinical application. Laboratory data as well as clinical ones are summarized. [ABSTRACT FROM AUTHOR]
