The incidence of and risk factors for acquiring community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) among patients staying in intensive care units (ICUs) remain unclear. We enrolled patients staying in two ICUs at the Far Eastern Memorial Hospital during the period of 1 September 2008 to 30 September 2009 to clarify this issue. Surveillance cultures for MRSA were taken from nostril, sputum or throat, axillae, and the inguinal area in all enrolled patients upon admission to the ICU, every 3 days thereafter, and on the day of discharge from the ICU. For each MRSA isolate, we performed multilocus sequence typing, identified the type of staphylococcal cassette chromosome mec, detected the presence of the Panton-Valentine leukocidin gene, and conducted drug susceptibility tests. Among the 1,906 patients who were screened, 203 patients were carriers of MRSA before their admission to the ICU; 81 patients acquired MRSA during their stay in the ICU, including 31 who acquired CA-MRSA. The incidence rates of newly acquired MRSA and CA-MRSA during the ICU stay were 7.9 and 3.0 per 1,000 patient-days, respectively. Prior usage of antipseudomonal penicillins and antifungals and the presence of a nasogastric tube were found to be independent risk factors for acquiring CA-MRSA during the ICU stay when data for CA-MRSA carriers and patients without carriage of MRSA were compared (P=0.0035, 0.0330, and 0.0262, respectively). Prior usage of carbapenems was found to be a protective factor against acquiring CA-MRSA when data for patients with CA-MRSA and those with health care-associated MRSA acquired during ICU stay were compared (P=0.0240).

The present study was carried out to determine how active surveillance for methicillin-resistant Staphylococcus aureus (MRSA) could be improved by the use of enrichment broth and the inclusion of extra-nasal sites with nares cultures. Molecular typing was also performed to identify colonization by single or multiple strains. Surveillance cultures for MRSA were obtained from 650 patients on admission to a medical and surgical intensive care unit (ICU) in Taiwan. MRSA was detected on directly plated vs. broth-enrichment cultures in any site at 10.0% vs. 24.2%, nares 8.2% vs. 17.5%, throat 4.8% vs. 13.4%, axilla 1.2% vs. 9.1%, and perineum 1.8% vs. 9.5%, respectively. Nares cultures alone detected only 81.5% and 72.5% of all colonized patients by direct and broth-enriched cultures, respectively. The molecular typing of 68 isolates from 17 patients revealed that multisite isolates were largely indistinguishable within each patient, but four patients had multiple subtypes and another three patients had different clonotypes. The detection of MRSA carriers was considerably enhanced by broth-enrichment cultures at multiple anatomic sites and simultaneous colonization by multiple strains at different sites can occur. Epidemiological studies are needed to determine the likelihood of subsequent nosocomial infection among colonized patients detected via direct nasal versus broth-enriched cultures from multiple sites.

Among 234 isolates comprising 26 different Candida species colonizing the oropharynx of 181 (54.3% of 399 surveyed) HIV-infected outpatients, 27 (11.7%) were fluconazole resistant. Antibacterial treatment was associated with increased rates of yeast colonization, while antiretroviral therapy and pneumococcal vaccination protected patients from yeast colonization.

In order to determine the prevalence of methicillin (meticillin)-resistant Staphylococcus aureus (MRSA) colonization among adults in community settings in Taiwan and identify its risk factors, we conducted the present study. For a 3-month period, we enrolled all adults who attended mandatory health examinations at three medical centers and signed the informed consent. Nasal swabs were taken for the isolation of S. aureus. For each MRSA isolate, we performed multilocus sequence typing, identification of the staphylococcal cassette chromosome mec, tests for the presence of the Panton-Valentine leukocidin gene, and tests for drug susceptibilities. Risk factors for MRSA colonization were determined. The results indicated that the MRSA colonization rate among adults in the community settings in Taiwan was 3.8% (119/3,098). Most MRSA isolates belonged to sequence type 59 (84.0%). Independent risk factors for MRSA colonization included the presence of household members less than 7 years old (P < 0.0001) and the use of antibiotics within the past year (P = 0.0031). Smoking appeared to be protective against MRSA colonization (P < 0.0001).

Background and Purpose: To evaluate the impact of 23-valent polysaccharide pneumococcal vaccine on Streptococcus pneumoniae colonization in the upper airway of patients with human immunodeficiency virus (HIV)-1 in Taiwan., Methods: 302 HIV-infected patients aged 15 years or older underwent swab cultures of the posterior pharynx and tonsils for S. pneumoniae between June 1, 2000 and June 30, 2002. 198 patients (65.6%) had received 23-valent polysaccharide pneumococcal vaccine with a median interval of 420 days (range, 27-634 days) before cultures were performed. Clinical characteristics and laboratory findings were analyzed to determine the factors associated with S. pneumoniae colonization in the upper airway., Results: Twenty patients (6.6%) had colonization with S. pneumoniae: 15 of 198 patients (7.6%) who had received 23-valent polysaccharide pneumococcal vaccine and 5 of 104 patients (4.8%) who had not received the vaccine (p = 0.37). According to the multivariate analysis, smoking was the only factor that was statistically significantly associated with S. pneumoniae colonization (adjusted odds ratio, 4.03; 95% confidence interval, 1.04-15.64; p = 0.04); pneumococcal vaccination was not statistically significantly associated with S. pneumoniae colonization., Conclusions: Among HIV-infected patients, smoking was the only factor significantly associated with a higher prevalence of upper airway colonization by S. pneumoniae. As previous receipt of 23-valent polysaccharide pneumococcal vaccine was not associated with a lower prevalence of S. pneumoniae colonization, a better vaccination strategy, in addition to smoking cessation, may be needed to reduce S. pneumoniae colonization in HIV-infected adults.

Background and Purpose: Methicillin-resistant Staphylococcus aureus (MRSA) has been the leading cause of nosocomial infections in many hospitals. To investigate the impact of carriage by health care workers (HCWs) on patient transmission, surveillance culture was performed following an outbreak of MRSA in a pediatric intensive care unit (PICU)., Methods: Isolates from 61 HCWs and 10 environmental sites were collected. Pulsed-field gel electrophoresis (PFGE) and antibiogram analysis were performed to determine the clonal relationship between isolates and potential routes of transmission., Results: The overall carriage rate of HCWs was 67.2% (41/61) for S. aureus and 26.2% (16/61) for MRSA. One MRSA was isolated from the 10 environmental sites sampled. Two major MRSA clusters were identified based on the PFGE patterns. Isolates with indistinguishable PFGE patterns (pulsotype A) were found in all patient isolates from the outbreak, from several HCWs plus the environmental isolate; all were resistant to ciprofloxacin, clindamycin, erythromycin, gentamicin, tetracycline, and trimethoprim-sulfamethoxazole. Interestingly, the isolate from a patient who had prolonged hospitalization in PICU had PFGE patterns (pulsotype B) distinct from the strains involved in the outbreak. This strain was susceptible to ciprofloxacin and trimethoprim-sulfamethoxazole, and was also found in several HCWs. Thus, there appeared to be 2 main MRSA clones circulating in the PICU of our hospital., Conclusions: Person-to-person and environment-to-person (or vice versa) transmissions are documented in this study. Strict hand washing before and after patient contact must be enforced and closely monitored, as it is the principal preventive measure in containing the spread of MRSA. To prevent the emergence of vancomycin-resistant MRSA and the further transmission of multidrug-resistant organisms, implementation of periodic and routine active surveillance cultures as part of infection control measures may also be evaluated.

This report describes the serotypes and antimicrobial resistance patterns of 860 strains of Streptococcus pneumoniae isolated from nasopharyngeal (NP) carriers and clinical specimens collected from Taiwanese children during the years 1997 to 2003. The 6 most common serotypes/groups were 23F, 19F, 6B, 14, 6A, and 3. These accounted for 652/716 (91.1%) of the NP and 131/144 (91.0%) of the clinical isolates. Serotype 23F was the most common isolate in the NP carriers (25.7%, 184/716). Serogroup14 was most common in the clinical isolates (29.2%, 42/144) and the most frequent invasive isolate (43.4%, 33/76). It was more frequently associated with invasive infection than all other serotypes/groups (odds ratio = 7.2; 95% confidence interval, 4.16-12.46; P < .0001). Resistance to macrolides was high in all serotypes/groups, which ranged from 70% to 97%. Resistance to penicillin varied among the 6 leading serotypes/groups, ranging from 3% in serogroup 3 to 99% in serotype 19F. Serotype 23F was most likely to be multidrug resistant to penicillin, macrolides, and chloramphenicol compared with all others (107/150 [71%] versus 105/461 [23%], P < .0001). The potential coverage by the pentavalent and heptavalent vaccines was 83% for all isolates, but was significantly lower for NP than clinical isolates (81% versus 92%, P< .01). These findings provide baseline data to compare trends in the distribution of pneumococcal serotypes and antibiotic resistance patterns with the introduction of childhood pneumococcal vaccination in Taiwan and other countries.

Background: The Taiwan19F-14 Streptococcus pneumoniae clone and its variants are being found with increasing frequency in the Asia-Pacific region. A 5-year old child with S. pneumoniae meningitis caused by a high-level penicillin resistant strain (MIC = 4 microg/ml) was admitted to a hospital in southern Taiwan. We carried out a study to determine the potential source of this strain., Methods: Nasopharyngeal cultures were obtained from all children attending the same kindergarten as the index case. To determine their relatedness all isolates were compared by serotype, antimicrobial susceptibility profile and pulsed field gel electrophoresis (PFGE)., Results: A high proportion of the children including the index case (32/78, 41.0%) carried S. pneumoniae in their nasopharynx (NP). The most common serotype was 19F (13/32, 40.6%). The PFGE types of the 19F serotype isolates obtained from the patient's blood, CSF and NP were identical and were related to 11 other serotype 19F NP isolates including 10 that were indistinguishable from the Taiwan19F-14 clone. All 14 isolates had similar high-level penicillin and multi-drug resistance. The serotypes of the other 19 NP isolates included 6A (2), 6B (10), 23F (5), 9V (1) and 3 (1). The overall rate of penicillin resistance in these S. pneumoniae from these children was 87.5% (28/32), with an MIC50 of 2 and MIC90 of 4 ug/ml. In addition, multi-drug resistant-isolates (isolates resistant to 3 different classes of antimicrobials) accounted for 87.5% (28/32) of all isolates., Conclusion: The high carriage rate of high-level penicillin- and multi-drug- resistant S. pneumoniae in a kindergarten associated with a case of pneumococcal meningitis emphasizes the need for restraint in antibiotic use and consideration of childhood immunization with conjugate pneumococcal vaccine to prevent the further spread of resistant S. pneumoniae in Taiwan.
