2. Etiological aspects of chronic pathology of the lymphepithelial pharyngeal ring in children at the present stage

The lymphoepithelial pharyngeal ring is located in the entrance gate and is constantly exposed to infectious agents. Currently, chronic pathology of the lymphoepithelial pharyngeal ring remains one of the most common problems in the practice of a pediatric doctor. Hypertrophy of the pharyngeal tonsil, chronic adenoiditis, which occurs in 35.3% of children under the age of 7 years, and chronic tonsillitis, which occurs in 15-63% of cases, are among the most common diseases of childhood. Taking into account the wide range of infectious microbiota that contributes to the development of chronic pathology of the lymphoepithelial pharyngeal ring, the issue of differential diagnosis of the etiology of the disease and the selection of adequate treatment regimens remains relevant. In most cases, at the present stage, the approach to the treatment of patients with chronic inflammation of the lymphoid ring of the pharynx in children is reduced to replacing the surgical method with a conservative one with the appointment of immunocorrective and immunomodulatory drugs. The lack of a positive effect of the ongoing conservative treatment, clinical and laboratory signs of persistent streptococcal infection say us about the need for surgical treatment. At the same time, a group of frequently ill children requires special attention with the development of new regimens of the immunocorrecting and immunomodulating drugs usage.

3. Haemorrhoidal artery ligation with and without Doppler guidance in the treatment of haemorrhoidal disease: a single-centre randomized study

Aim. To evaluate the efficacy of haemorrhoidal artery ligation (HA) with a preliminary palpatory determination of its localization supplemented by mucopexy of haemorrhoids as a new surgical method in the treatment of haemorrhoidal disease (HD) and to compare it with HAL-RAR technology.Materials and methods. The randomized controlled clinical trial included patients over 18 years old with Goligher's grade II, III or IV symptomatic HD. We operated on patients in the study group (n = 75) using palpatory determination of the localization of HA and subsequent mucopexia. In the control group (n = 75) we used HAL-RAR. The primary endpoint (25-30 days after surgery): recurrence rate of HD symptoms. Secondary endpoints: postoperative complication rate, pain intensity on a visual-analogue scale from 1 to 10 points, patient satisfaction with the treatment results on a 10-point scale.Results. According to the initial characteristics (age, gender, body mass index, stage of HD, frequency of clinical symptoms), the groups did not differ. Anal bleeding relapse developed: study group - 11%, control group -14%; relapse of haemorrhoids prolapse: 3% and 5% respectively (p > 0.05). Postoperative complications were noted in 6 (8%) in the study group and 4 (5%) in the control group (p > 0.05). The intensity of pain on the 2nd and 25-30 days after surgery was 6.3 [4.8; 7.4] and 1.2 [0.6; 2.5] points in the study group and 6.5 [4.9; 7.3] and 2.1 [1.9; 4.1] in the control group, respectively (p > 0.05). Patient satisfaction with the treatment results after 12 months was estimated at 8.7 [7.9; 9.2] and 9.4 [8.2; 9.6] in the study and control groups, respectively (p > 0.05).Conclusion. HA ligation with a preliminary palpatory determination of its localization and supplemented with mucopexy of haemorrhoids is no less effective than HAL-RAR in preventing haemorrhoidal bleeding and prolapse of the nodes.

4. Hospital and mid-term results of simultaneous correction of coronary and carotid arteries

Study objective: to present the immediate and mid-term results of onestage surgical treatment of patients in the volume of CEE and CABG based on a differentiated approach to patient selection.Materials and methods: in FCCVS n.a. S.G. Suhanov, Perm developed an algorithm for choosing treatment tactics in patients with combined atherosclerotic lesions of the coronary and brachiocephalic arteries. According to this algorithm, for the period from 01.07.2014-01.01.2021, one-stage correction of CABG + CEE was performed in 104 patients. The primary endpoints were all-cause death, acute myocardial infarction (MI), transient ischemic attack (TIA), and stroke. Hospital and midterm results were analyzed. The average follow-up time in the study of mid-long-term results was 40.3 ± 20.4 months.Results: in the study of hospital outcomes, the mortality rate was 0%. There were recorded 3 (2.9%) cases of perioperative stroke and 1 (0.9%) case of myocardial infarction. There were no cases of TIA. The combined endpoint (death, acute MI, stroke, TIA) reached 4 (3.8%) patients. In the study of mid-term results, we were able to contact 96 patients out of 104 operated on (92.3%). The survival rate was 94.8%. 5 (5.2%) people died. There were 2 (2.1%) cases of myocardial infarction, 4 (4.1%) cases of stroke.Conclusions: simultaneous revascularization of the carotid and coronary regions of CABG + CEE is safe and allows adequate elimination of the lesion in both regions.

Background. The rapid development of laparoscopic surgery can significantly reduce trauma during operations in the retroperitoneal space. These goals are also achieved by retroperitoneoscopy (RPS), which has become widespread in the last twenty years. The main difficulties for surgeons were associated with a small workspace and a lack of clear anatomical landmarks. Different access methods have different features, advantages and disadvantages, the analysis of which helps to optimize surgical treatment and accelerate patient rehabilitation.Aim: to study the advantages and disadvantages of retroperitoneoscopic and transperitoneal laparoscopic surgical methods of treatment.Materials and methods: We analyzed 305 case histories of patients operated on for various kidney diseases. The patients were divided into 2 groups: Group 1 – patients who underwent surgery using an extraperitoneal retroperitoneoscopic approach, Group 2 – patients who used the transperitoneal laparoscopic approach. A comparison of two methods of surgical treatment of kidney diseases, retroperitoneoscopic and laparoscopic access, was carried out. The following indicators were compared: duration of surgery, length of hospital stay, need for analgesics, frequency of postoperative complications.Results. The duration of the operation with retroperitoneoscopic surgery was reduced by more than 15 % compared with the laparoscopic approach. Shorter periods of pain relief were required, and the number of complications also decreased. The duration of inpatient treatment did not differ significantly.Conclusion. Retroperitoneoscopic approach can be used as the method of choice for operations on the retroperitoneal space.

Introduction. Immunoenzyme assay of biochemical markers is one of the most important methods for examination of pa- tients with neuroendocrine tumors (NETs). Along with the generally accepted NET marker chromogranin A (CgA), another member of the granin family, chromogranin B (CgB), can serve as a complementary marker.Objectives. Analysis of CgB as an additional to CgA biochemical marker in the blood serum of patients with gastric and pancreatic neuroendocrine tumors.Materials and methods. We examined 79 patients with gastic (n = 14) and pancretic (n = 65) NETS, and 42 particularly healthy people, who were included in the control group. CgB and CgA were determined with ELISA method using the Human Chromogranin B (USCN, China) and Chromogranin A NEOLISA (Eurodiagnostica, Sweden) test systems., Results. CgB levels in gastric and pancreatic NETs were significantly higher than in control group. CgB concentrations were independent of tumor spread and its biological activity. ROC analysis in common group of NETs relative to control group showed AUC for CgB = 0.869 and for CgA AUC = 0.82. According to results in common group of NET patients when used isolated, CgA and CgB have comparable diagnostic sensitivity, which increases in complex use to 82.5 %. In the group of NET patients with low levels of CgA (15.7 ng/ml) was observed in 53.6 % of cases.Conclusion. The combination of CgB and CgA in gastric and pancreatic NETs could increase the diagnostic efficacy of bio- chemical diagnostics. The received data confirms the significance of CgB as a complementary biomarker of NETs.

In the design of multi-engine aircraft, one of the important issues is the interaction between the propellers and airframe configuration components, especially in take-off and go-around procedure modes. Modern propeller-driven aircraft concepts in the pulling configuration are characterized by a high disk loading and an increased number of propeller blades used to increase cruising speed and reduce excessive noise. The first problem arising due to high disk loading is the direct impact of forces by operating propellers (thrust, normal force) on fixed-wing stability, especially at angles of attack different from a zero value. The second one involves a high-energy level of the propeller slipstream, having a significant indirect impact on the aircraft’s aerodynamics, stability and controllability. This impact is primarily associated with the interaction of propellers slipstream with other aircraft’s configuration elements. The complexity of taking into account the slipstream-wing interaction and other airframe components stipulated the application of experimental methods to study the problems of propellers – airframe interaction while designing propeller-driven aircraft configurations. This article presents an analysis of the experimental studies results of the operating propellers- airframe interaction for a light twin-engine transport aircraft. The aerodynamic aircraft’s configuration is executed using the conventional pattern of a high-wing and the carrier-on deck type empennage. The high-lift wing device is a fixed-vane doubleslotted flap. The wind-tunnel tests of the model in the cruising, takeoff and landing configurations were carried out in TsAGI lowspeed wind-tunnel T-102. Measurement of forces and moments, acting on the model, was performed by means of an external sixcomponent wind-tunnel balance. Measurement of forces and moments, acting on the propeller, was conducted using strain gauge weighers installed inside the engine nacelles of power plant simulators. The simultaneous combined use of external and internal balances allowed researchers to determine the direct and indirect contribution of operating propellers to the model longitudinal aerodynamic characteristics under variation of loading factor B ranging from 0 to 2.

In 46 patients with arterial hypertension, arrhythmias and amiodarone-associated type I thyrotoxicosis, it was confirmed that prolonged premedication with long-acting benzodiazepines and magnesium preparations reduces the risk of cardiovascular complications and instability of systemic hemodynamics, mitigates the adverse effects of amiodarone dysfunction of the thyroid gland on the cardiovascular system. Postoperatively, under prolonged premedication with long-acting benzodiazepines and magnesium preparations, postoperative cardiac arrhythmias were found to be less common. When the pre-operative period was significantly reduced, the appointment of long-acting benzodiazepines had advantages over short-acting benzadiazepines due to the prevention of cardiac arrhythmias and increased blood pressure in the early post-operative period.

Osteoporosis and sarcopenia are age-associated diseases of the musculoskeletal system. Osteosarcopenia, the presence of osteopenia/osteoporosis and sarcopenia. The prevalence of osteosarcopenia in older adults with failing was 37% and associated with higher rate of death. Diagnosis of osteosarcopenia consists of describing medical history of fractures, providing x-ray of the spine (if it is needed) and bone densitometry, calculation of Fracture Risk Assessment Tool (FRAX), evaluating muscle strength, mass, function. The most common exam which is used to measure bone mineral density (BMD) is dual-energy x-ray absorptiometry (DXA or DEXA). Screening using the FRAX is recommended in all postmenopausal women and mеn over 50 in order to identify individuals with high probability of fractures. It is recommended to diagnose osteoporosis in patients with fragility fracture of large bones of the skeleton. Diagnosis of sarcopenia is consist of measures for three parameters: muscle strength, muscle quantity/quality and physical performance as an indicator of severity. Muscle strength can be measured with carpal dynamometry. Muscle mass can be evaluated dual-energy X-ray absorptiometry (program «Whole body»). Muscle function can be evaluated with short physical performance battery (SPPB) tests. In this article described algorithm of diagnosis of osteosarcopenia.

11. Prolonged QT interval on electrocardiogram and fainting — is there always a relationship?

Using a clinical example, the article draws the attention of doctors to the problem of the prolonged QT interval (long QT) and the related problem of fainting (syncope). Syncope is a component of long QT syndrome, and syncope is a precursor of sudden cardiac death. However, syncope in a patient with long QT syndrome may have pathogenesis that is completely unrelated to abnormalities of cardiac ion channels. In other words, such a patient may have a second disease as a syntropy relates to prolonged QT interval, to an extent mimicking long QT syndrome. The presented medical history of a 33-year-old patient S. shows the complexity of differential diagnosis of the causes of syncope. The crucial part in the diagnosis, in addition to the clinical picture, was the so-called tilt test, little-known to general medical practice, as well as the laboriousness of making a final diagnosis of the long QT Syndrome type 2, which required a molecular genetic study whole-exome sequencing. Patient S. had vasovagal syncope that not associated with long QT syndrome, but she has a risk of sudden cardiac death, and the article identifies therapeutic and other measures to reduce this risk.

The high mortality rate in COVID-19 can be explained by the development of a hyperinflammatory syndrome, characterized by a cytokine storm and extensive thrombus formation. The main direction for preventing the development of hyperinflammatory syndrome and reducing mortality from COVID-19 is immune therapy, however, the data on the efficacy and criteria for prescribing immune drugs is very heterogeneous. The purpose of this review is to analyze the results of clinical trials on the use of various types of immune therapy and possible criteria for its prescription. Analysis of literature data showed that the most effective among the existing variants of immune therapy were monoclonal antibodies to IL-6, the use of donor plasma in the early stages of treatment. Janus kinase inhibitors, intravenous immunoglobulin improved the clinical characteristics of patients, but did not affect the mortality rate. An analysis of possible predictor-markers of the development of a cytokine storm revealed an increase in the number of neutrophils > 11 × 103/ml, a decrease in the number of lymphocytes > 1000 × 103/ml, an increase in the level of IL-6 > 24 pg/ml, LDH > 300 IU/L, D-dimer > 1000 ng/ml, and CRP > 10 mg/dL as the most informative and accessible in clinical practice at the moment.

The scientific program of the symposium included 150 reports devoted to both the results of scientific and experimental research and advanced applied medical technologies aimed at improving the results of chronic wounds by various etiology and localization treatment.

The results of both numerical simulations and experimental study of the traveling-wave amplification in the ${W}$ -band circuit with the nonuniform grating are presented and discussed. The circuit supports a wide sheet electron beam that is promising for a signal gain increase in terahertz (THz) amplifiers. The small-signal gain up to 30 dB is predicted by the simulations of the two-section amplifier in the frequency range from 94 to 99 GHz. The experimental tests of the 14.3-mm input section, powered by a 3.8 kV, 100-mA sheet electron beam, revealed the linear gain of 12 dB and instantaneous bandwidth of 2 GHz. The full bandwidth of 5 GHz has been experimentally observed by adjusting the beam voltage from 3.6 to 4.1 kV.

Introduction The problem of complications after surgical treatment of pertrochanteric fractures in elderly patients is relevant and far from a solution. Materials and methods The retrospective study was based on the analysis of the results of treatment of 129 patients with pertrochanteric femoral fractures (average age 76 years). All fractures in the early time from trauma were fixed with two types of cephalomedullary nails, either dynamic or static. All patients could not limit the load on the operated limb after surgery because of different reasons. Results The results of treatment were evaluated in 109 patients after one year. In dynamic cephalomedullary fixator group (59 patients), there were 7 orthopedic complications with a functional Harris scale result of 68 points (range, 26 to 94 points). In static cephalomedullary fixator group (50 patients), there were 14 orthopedic complications with a functional Harris score of 56.5 points (range, 15 to 92 points). Discussion Higher results of treatment in the group of dynamic fixator in the condition of full-weight bearing on the operated limb may be associated with the possibility of dynamization of the part of fixator in response to bone resorption in the contact area of bone fragments. The use of dynamic cephalomedullary fixators instead of static ones for treatment of pertrochanteric femoral fractures in elderly patients exercising full weight-bearing leads to a decrease in orthopedic complications (from 28 to 11.9 %) and improves the functional results of treatment.

In conditions of limited finance resources building optimum finance structure of capital is considered the most important objective, as the availability of the necessary amount of own funds can provide finance sustainability of the organization, the potential to meet timely liabilities to creditors, finance and develop new lines of work. However, any resources (both borrowed and own) have value, as they can be invested into the profitable investment project. It is obvious that the finance structure of capital can influence indirectly the level of organization profit, as in other expenses interest costs or interests payable are shown. Therefore, it is necessary to build such a correlation of own and borrowed sources, which can provide the max profit for the enterprise. As tools for building such a correlation the article proposes methods of mathematic modeling and, namely, the correlation analysis to find the interconnection between the dependent variable, i.e. work profitability and different factors - a share of own and borrowed funds. On the basis of this interconnection regression equation was built, which allows us to find out how the amount of net profit changes with a certain correlation of own and borrowed resources. The analysis was carried out based on published finance reports of the limited company ‘Gazprom Bureniye'.

Chronic diffuse liver diseases (CDLD) of various etiologies remain a serious socio-economic and clinical-epidemiological health problem. Every year more than 1 million people die from cirrhosis of the liver of various etiology all over the world. According to the results of a study by a group of authors, it was found that in the Siberian Federal District there is an unfavorable situation in terms of gastroenterological mortality, in particular, hepatological mortality. CCM is characterized by lengthening of the QT interval, electromechanical dyssynchronization, and chronotropic insufficiency. The duration of the QTc interval in the subgroup of patients with liver cirrhosis who received basic therapy and the drug lisinopril (Diroton) was (418.21 ± 2.74 ms) and was significantly lower than in the subgroup of patients receiving only basic therapy. Evaluating the parameters of transthoracic echocardiography among patients with liver cirrhosis, it was noted that during treatment with basic drugs in combination with lisinopril (Diroton), the volume of LV EDV significantly decreased (p < 0.05). The appointment of an ACE inhibitor lisinopril to patients with cirrhosis of the liver as part of complex therapy leads to an improvement in general well-being, a decrease in shortness of breath, an increase in exercise tolerance, an improvement in basic laboratory parameters, as well as an improvement in some morphometric parameters (EDV) and cardiac electrical systole.

Hundreds of thousands of bariatric surgeries are performed worldwide every year. Th ey have long been proven to be safe and eff ective in treating obesity and type 2 diabetes. Along with an unconditional positive eff ect, these interventions, especially shunting ones, are characterized by specifi c complications. In the absence of proper correction, they can become fatal for patients. One of these complications is malabsorption leading to a defi ciency of vitamins and microelements, which in most cases, is amenable to timely correction in the postoperative period. However, there are situations when it is not possible to carry out an eff ective correction and it becomes necessary to perform reconstructive interventions with the reverse inclusion of the small intestine in the digestion, which is associated with great diffi culties. Th e authors demonstrated this situation in the description of clinical observation of a patient with postoperative hypothyroidism and history of postoperative hypoparathyroidism, who underwent bariatric surgery. Impaired absorption of drugs (L-thyroxine, calcium, and vitamin D), and therefore, uncompensated hypothyroidism and hypocalcemia was an indication for reconstructive surgery.

24. Correction of Motor Disability Impairments by Physical Therapy Means in Women after Cesarean Section

In modern obstetrics, the frequency of abdominal accouchement is constantly increasing. Therefore, the problem of restoring the state of health of women after surgical delivery requires the introduction of new medical knowledge and technologies into practical rehabilitation, in particular, methods of physical therapy. The purpose of the study was to determine the influence of the developed program of physical therapy on the dynamics of the parameters of the motor ability of women in the postpartum period after cesarean section. Materials and methods. 112 women were examined after childbirth. The comparison group consisted of 47 women who had a natural childbirth. The main group 1 consisted of 32 women who did not want to carry out the proposed program of physical therapy; they were provided with general recommendations for self-observation in the postpartum period after cesarean section. The main group 2 consisted of 33 women after cesarean section who underwent postpartum physical therapy for 12 months (kinesitherapy (therapeutic exercises, functional training to improve motor skills and abilities), abdominal wall bandage, kinesiotaping, abdominal and general massage, wellness nutrition, psychological relaxation, education of women). Determination of motor ability according to the Functional Movement Screen was performed in the late postpartum period (6-8 weeks), 6 and 12 months after childbirth. Results and discussion. In women in the late postpartum period, regardless of the type of delivery, signs of impaired motor ability are revealed, which was established by the results of the Functional Movement Screen test exercises. Within a year after childbirth, there is a gradual improvement in the motor ability of women, regardless of the type of delivery and their performance of active restorative interventions. The use of a physical therapy program starting from the early postpartum period in women after cesarean section revealed advantages regarding the state of their motor ability in relation to women who underwent natural delivery and those who recovered from cesarean section on their own, according to separate Functional Movement Screen tests already at the end of the late postpartum period. After 1 year after childbirth, women who underwent cesarean section and performed the developed physiotherapy intervention on all Functional Movement Screen tests demonstrated a statistically significantly better result compared to women who underwent natural delivery and those who recovered from cesarean section on their own. Women who underwent cesarean section, but recovered on their own with the help of general recommendations, according to the signs of impaired movement capacity 1 year after childbirth could not reach the levels of women who gave birth by vaginal delivery and women who underwent a physical therapy program. Conclusion. It is advisable to prescribe physical therapy means from the first day of postpartum rehabilitation in order to level the signs of impaired motor capacity for quick postpartum recovery of women and their return to full functioning

Currently, rhinitis accompanied by nasal obstruction is one of the most common diseases in children and adolescents. Regardless of the cause of its occurrence - an infectious or non-infectious (allergic) process, each time there is a vascular (vasomotor) reaction. In children of preschool and primary school age, the most common is infectious rhinitis, which develops with viral and bacterial infections. The group of non-allergic rhinitis, which develops more often in adolescents, is united by the term idiopathic (vasomotor) rhinitis.Purpose of the study. Evaluation of the effectiveness of the use of a complex preparation containing calendula, witch hazel, esculus, menthol and zinc oxide in the treatment of vasomotor rhinitis in children.Materials and methods. The study included 40 patients of childhood and adolescence, of both sexes (23 girls and 17 boys) aged 6 to 14 years with an established diagnosis of vasomotor (idiopathic) rhinitis, without concomitant pathology, not registered with specialists, without constant admission The study drug was used to lubricate the mucous membrane of the nasal cavity or put turundas with ointment for 3-5 minutes 2-3 times a day. The course of treatment was 7-10 days. The effectiveness of treatment was assessed by the dynamics of clinical data and the severity of the subjective symptoms of the disease.Results. A comparisons of these scores for re-assessing the status of patients relative to the timing of the study drug use showed a uniform positive trend in terms of the clinical picture and subjective complaints of patients.Conclusions. The inclusion in the complex therapy of pediatric patients with idiopathic (vasomotor) rhinitis of the drug Fleming’s Ointment, or monotherapy with this agent, contributes to a rapid and pronounced weakening of the clinical manifestations of the disease, allowing you to quickly achieve positive treatment results in the form of relief of nasal obstruction, rhinorrhea, edema of the mucous membrane of the cavity nose, sneezing. There is a positive trend in the state of the mucous membrane itself.

We discuss the physicochemical characteristics of aerosol over seas of the North Atlantic and Arctic Ocean, measured in the 80th cruise and two preceding (67th and 71st) cruises of RV Akademik Mstislav Keldysh. Most aerosol characteristics (concentrations of aerosol, black carbon, elemental and organic carbon, elemental composition) are shown to be larger in the atmosphere of the Baltic and North Seas as compared to the Norwegian and Barents Seas. The isotopic composition of carbon in aerosol samples was dominated by light isotopes, indicating the predominant effect from combustion of oil products and natural gas.

The aim of the study was to carry out a meta-analysis of randomized clinical trials in order to combine the results of clinical trials on Triazavirin® (Riamilovir) efficacy in etiotropic therapy of influenza.Materials and methods. The analysis was carried out in accordance with the PRISMA principles regarding the quality of information presentation on the results of systematic reviews and meta-analyzes of works assessing medical interventions. The study included 471 patients with a confirmed diagnosis of influenza (with laboratory confirmed presence of influenza virus antigens).Results. The conducted meta-analysis showed that the use of Triazavirin® (Riamilovir) has a statistically significant effect on the severity of clinical symptoms in patients with influenza, therefore Riamilovir can be used in the initial therapy of adult patients with influenza.Conclusion. Clinical studies have shown that the use of Triazavirin® (Riamilovir) is effective both in the initial and late stages of the disease, and therefore Riamilovir can be used in the treatment of adult patients with respiratory diseases of viral etiology, in particular, those diagnosed with influenza. The meta-analysis of the collected data showed that therapy with Triazavirin® (Riamilovir) has statistically significant advantages in various aspects both in comparison with the placebo group and with the Tamiflu® (Oseltamivir) group.

The aimof the study was to carry out meta-analysis of randomized controlled trials in order to combine the results of clinical trials on Triazavirin®(Riamilovir) efficacy in the etiotropic therapy of acute respiratory viral infection.Materials and methods.The studies included 435 patients with a confirmed diagnosis of acute respiratory viral infection or ARVI (with laboratory confirmed absence of influenza virus antigens). The research was carried out in 27 centers. In studies, patients were divided into 3 groups in a 1:1:1 ratio (a total of 145 people per group). Each study included a group taking 100 mg of the medication 5 times a day, a group taking 250 mg of the medication 3 times a day and a placebo 2 times a day, as well as a group taking a placebo 5 times a day. The analysis was carried out in accordance with the PRISMA principles regarding the quality of information presentation on the results of systematic reviews and meta-analyzes of works evaluating the effects of medical interventions.Results. The conducted meta-analysis showed that the use of Triazavirin®(Riamilovir) has a statistically significantly effects on the severity of clinical symptoms in patients with ARVI. The performed meta-analysis confirmed reliable associations between the use of Triazavirin®(Riamilovir) in both doses and the chance of a persistent improvement in clinical symptoms on the 5thday of therapy. The meta-analysis also confirmed the statistical significance of the clinical effects of Riamilovir by such indicators as the area under the curve «point on the scale showing the severity of the condition in a patient with ARVI in relation to time», the proportion of patients with complete alleviation of all symptoms by the end of the 5th day from the start of therapy.Conclusion.The clinical trials proved that the use of Triazavirin®(Riamilovir) is effective both in the initial and in the late stages of the disease, therefore, the drug can be used in the initial therapy of adult patients with respiratory diseases of viral etiology.

The study objective is to study the role of multiparametric magnetic resonance imaging (mpMRI), to predict extracapsular extension, infiltration of the seminal vesicles, neurovascular bundles and status of positive surgical margin (PSM) in robot-assisted radical prostatectomy (RARP) with fascio- and nerve-sparing and anatomy-sparing techniques in patients with low and intermediate oncological risk; to evaluate safety of RARP with anatomy-sparing techniques in the context of oncological results in the studied patient cohort.Materials and methods. Fifty-four males underwent mpMRI in the device with magnetic field density 3 Tesla without endorectal coil. Results of mpMRI were analyzed using PI-RADS v.2. Prostate-specific antigen level, results of prostate biopsy, nomogram were analyzed. All patients were included into groups with low and intermediate oncological risk. The patients underwent RARP with anatomy-sparing techniques. Gross specimen removed during the surgery was stained with tissue stain DECOLA and subjected to stepwise pathomorphological analysis. Comparative analysis of tumor lesion localization was performed for mpMRI and pathomorphological examination. The study concentrated on the largest tumor lesion locations. The anterior surface of the prostate was examined in detail. Patients were classified per risk groups in accordance with the National Comprehensive Cancer Network (NCCN) and D'Amico criteria.Results. Full or partial overlap of the lesions per mpMRI and pathomorphological examination were observed in 48 (88.9 %) patients; locations of tumor lesions disagreed in 6 (11.1 %) patients. PSM was detected in 8 (14.8 %) patients; among them in 7 (12.9 %) the size was between 0.1 and 0.4 cm. On the anterior surface of the prostate 3 (5.5 %) PSM were observed, 2 of which were between 0.14 and 0.4 cm, 1 was multifocal, larger than 0.3 cm. Per mpMRI results and histological examination, the anterior surface (fibromuscular stroma) was affected in 14 (25.9 %) and 11 (20.4 %) cases, respectively. Among them, lesion locations overlapped in 11 observations. Results of anatomy-sparing RARP show increased PSM rate compared to traditional nerve-sparing based on the anterior surface of the prostate.Conclusion. Routine mpMRI for patients who are planned to undergo RARP allows to justify selection of anatomy-sparing RARP or rejection of modification in favor of more thorough dissection. mpMRI plays a significant role on planning of anatomy-sparing RARP, achievement of negative surgical margin in patients with low and intermediate risk of oncological progression. Evaluation of the location and size of cancer lesion in the prostate allows to plan for special aspects of anatomy- and/or nerve-sparing techniques, achieve lower PSM rate, optimize oncological and functional surgical results in localized prostate cancer.

Cell and serum nucleases are considered as a natural biological barrier, one of the first links in the body’s antiviral defense: nucleases act on viruses that the usual immunological barrier cannot resist. Inactivation of the enzymatic properties of nucleases also leads to the loss of their antiviral activity. It was found that the nucleases do not inactivate the native virus outside the cell and act mainly on the virus that multiplies in the cells. There is a direct dependence of the antiviral activity of endonuclease on the concentration of the enzyme in the medium. The first antiviral drug created on the basis of the Serratia marcescens endonuclease was called bacterial endonuclease. Its technology was developed by joint research of NIKTI BAS of the Ministry of Medical Industry (Berdsk, Novosibirsk region) and ICIG SB of the USSR Academy of Sciences in 1973–1984. Serratia marcescens endonuclease is capable of cleaving the nucleic acids of both RNA and DNA-containing viruses. The enzyme inhibits the reproduction of vesicular stomatitis and smallpox vaccine viruses in chicken fibroblast cell culture, respectively. It was the first antiviral drug in the history of beekeeping, designed to prevent acute and chronic paralysis and other viral diseases of bees. The drug was also intended as an antiviral agent for a wide range of diseases in various organisms. The effectiveness of bacterial endonuclease as a means of prevention and treatment of respiratory viral diseases in calves was studied. The research was carried out at the industrial complex for growing heifers of the Gosnensky state farm in the Leningrad region on calves of a black-and-white breed of 20–60 days old. For the experiments, we used bacterial endonuclease produced by the Vyshnevolotsky Plant of Enzyme Preparations or NICTI BAS of the Ministry of Medical Industry. In the future, NIKTI BAV together with Diafarm LLC developed a second – generation antiviral drug, endoglucin, based on bacterial endonucleosis. Production experiments to study its effect as a therapeutic and prophylactic agent for respiratory diseases of calves were conducted during 2007– 2011. in the agricultural complex «Prichulymsky» of the Achinsky district of the Krasnoyarsk Territory with the participation of the Department of Epizootology and Parasitology of the Faculty of Veterinary Medicine of KrasGAU and in CJSC «Suzdalskoye» of the Dovolensky district of the Novosibirsk region. Prophylactic use of endoglucin in calves with bronchopneumonia can reduce the incidence from 2.2 to 3.1 times. The drug endoglucin in combination with drugs indicated for the treatment of pulmonary pathology, has therapeutic effectiveness in bronchopneumonia of calves.

The subject of the study is the problem of the spatial distribution of the population in Russia and its regions. The relevance of research is determined by the key trends in the processes of distribution and movement of human capital in the context of cities. The paper aims to analyze the key trends and prospects for the development of the Russian settlement system at the federal, regional, and local levels. The research methods include a critical analysis of approaches to territorial settlement optimization, the modern system of urban settlement in Russia and its regions based on the use of statistical data of the Federal State Statistics Service for 2011-2019. The authors carried out a comparative analysis of the studied processes by countries using the Zipf method, as well as by Russian regions using the the Lorenz coefficient. It has been determined that the group of regions with an increase in the level of differentiation of urban settlement is characterized by a high level of depopulation of small and medium-sized cities with a contraction and concentration of the population in the largest city of the region, which creates additional risks for the sustainable development of the territory. A decrease in the level of differentiation of urban settlement is observed in regions where the share of the population of both small and medium-sized cities (but at a slower pace) and large ones is decreasing. The scientific novelty of the study lies in the development of an approach to improving the settlement system in Russia based on the application of the Lorenz coefficient and modelling methods. The authors conclude that an uneven system of settlement has developed in Russia, creating prerequisites for the emergence of new imbalances and threats to the complex sustainable development of the country’s territory. In this regard, it is advisable to develop an appropriate document in the field of state policy at the federal level or clarify similar issues within the framework of existing documents, as well as to increase the scientific validity of the measures taken using formalized methods of forecasting and planning. A promising direction in this area is the development of an agent-based model that allows increasing the efficiency of the distribution of financial resources for the development of social infrastructure. The results of the study justify the expediency of reallocating financial resources of the budget to ensure state policy in the field of development of the settlement system in the country.

Ferritin is one of the biomarkers requiring special attention; its blood level increases significantly especially in the severe course of COVID-19. Information on the effect of hyperferritinemia on the disease outcome is very contradictory as are the ideas about the causes of its development.The objective: to study the effect of hyperferritinemia on the disease outcome and analyse the possible causes of its development in severe COVID-19.Subjects and Methods. Data on 479 patients with severe course of coronavirus infection treated in intensive care units (ICU) were retrospectively analyzed. Of them, the proportion of patients with a favorable outcome (Group 1) was 51.0% (n = 241), and with an unfavorable outcome (Group 2) - 49.0% (n = 235). The following parameters were assessed: the levels of ferritin, C-reactive protein, fibrinogen, IL-6, IL-10, IL-18, procalcitonin, complement C5a, total, direct and indirect bilirubin, ALT, AST, and the blood level of lactate dehydrogenase (LDH). The changes of erythrocyte count and hemoglobin blood level were also monitored. In order to form a clear view of iron metabolism, free iron, transferrin, and myoglobin levels were assessed in several patients with high ferritin values (more than 1,500 pg/L).Results. In the unfavorable outcome, ferritin levels increase very significantly, reaching maximum by day 6 of patients' stay in ICU. The difference in the level of ferritin between the groups of survivors and deceased during this period is clear and significant (p = 0.0013). The association of ferritin values with the outcome is detected as early as day 1, but by day 4 it becomes highly significant (the cut-off point is 1,080 pg/l). No data have been obtained that would indicate the association of hyperferritinemia with impaired iron metabolism, the development of hepatic dysfunction, or cellular destruction. In contrast to survivors, those who died on day 6 had elevated IL-6 while C5a level remained unchanged. Conclusions. The rapid increase in ferritin blood levels to 1,000 pg/L or higher is an unfavorable prognostic sign indicating a high probability of a lethal outcome. When assessing genesis of hyperferritinemia in COVID-19, the crucial significance should be attributed to the cytokine storm rather than disorders of iron metabolism or hemotoxic effects of the virus. The persistent increase of ferritin level in blood during 4-6 days of stay in ICU should be considered as a reason to intensify anticytokine therapy.

The paper evaluates the effectiveness of the use of therapeutic laser exposure (photobiomodulation therapy – PBMT) to minimize acute pain in the early postoperative period in patients after septoplasty. The study included two groups of patients. Patients of the first group (31 patients) underwent septoplasty with standard management in the postoperative period. Patients of the second group (31 patients) also underwent septoplasty, and then added PBMT to the standard measures of the postoperative period at 3, 6 and 24 h after septoplasty (λ = 0.890 μm, P = 10 W, 2 min) and then intranasally 48 h after septoplasty (λ = 0.630 μm, P = 8 W, 2 min). In patients of both groups, heart rate variability and pain were assessed using a visual analog scale within 48 hours after septoplasty. In patients of the second group, after the use of PBMT, the indicators of heart rate variability had a significantly lower total power, compared with patients of the first group. Background.Severe fractures of the shin bones are often accompanied by the formation of defects in the tibia, suppuration and soft tissue necrosis. In the case of surgical treatment of fractures, infectious complications reach 3.6-9.1%. One of the methods of treatment of infected defects is resection of the ends of the tibia with shortening. This operation has proven to be effective in the treatment of fresh fractures. The relevance of the work is due to the prospects of using this technique in the treatment of the consequences of fractures with the formation of infected defects of the tibia.Aim of the study. Improving the results of treatment of patients with post-traumatic defects of the tibia complicated by osteomyelitis by performing a shortening resection with simultaneous or sequential correction of the segment length.Material and methods.The results of treatment of 65 patients with diaphyseal post-traumatic tibial defects complicated by osteomyelitis were analysed. They were divided into 2 groups. Group 1 was formed by 31 (47.7%) patients, they underwent shortening resection of the ends of bone fragments in the defect zone with simultaneous lengthening at another level. Group 2 included 34 (52.3%) patients who underwent a shortening resection of the tibia without lengthening. In all cases, the Ilizarov apparatus was used as a fixator.Results.The technique for assessing the size of the true defect of the tibia was optimized taking into account the initial shortening of the segment and the distance between the proximal and distal fragments after resection of their ends. A treatment regimen was developed depending on the level of localization of the tibial defect, and the results of treatment of patients in the compared groups were assessed.Conclusion.Shortening resection is an effective treatment for patients with post-traumatic tibial defects complicated by osteomyelitis. Depending on the level of localization of the defect, it is advisable to carry out treatment according to one of two possible options. When the defect is localized in the upper and middle third of the tibia, shortening resection in an isolated form is shown. If the defect is localized in the lower third of the tibia, it is possible to supplement the shortening resection with an osteotomy in the upper third with Ilizarov lengthening.

We studied the influence of combined preconditioning (compound pQ-4 and moderate hypoxia) on morphometrical parameters of neuronal populations in hippocampal fields CA1 and CA3 in rats after bilateral ligation of the common carotid artery. Preconditioning produced a neuroprotective effect, improved survival of pyramidal neurons in the early and delayed periods of modeled ischemia, prevented the formation of necrotic and apoptotic neurons and hyperactivation of microglia, and protected endotheliocytes. The positive influence of preconditioning factors on the morphometric parameters of the brain under ischemic conditions agrees with the results of behavioral tests (open field and elevated plus maze) that demonstrated increased locomotor activity and exploratory behavior of animals.

The objective: to identify laboratory markers of systemic perfusion in newborns with functional single ventricle on mechanical ventilation after surgical correction. Subjects and methods. Blood gas parameters were retrospectively analyzed in 52 newborns with congenital heart defects with univentricular hemodynamic after surgical correction. All samples were divided into three groups based on arterial blood saturation (SaO2): Group 1 – hypoxia (SaO2 ≤ 65%); Group 2 – normoxemia (SaO2 = 65-85%); Group 3 – hyperoxemia (SaO2 > 85%). Stroke volume and cardiac index were evaluated with echocardiography. The oxygen consumption and carbon metabolism were evaluated by arterial and venous blood gases. Results. The mixed central venous pO2 (PvO2) > 29.5 mm Hg, mixed central venous O2 (SvO2) > 54.5%, arteriovenous difference in saturation (Sa-vO2) < 15.8%, total oxygen content in venous blood (CvO2) > 119 ml/l, oxygen extraction ratio (O2ER) < 19% and the arteriovenous difference in partial pressure of carbon dioxide (dPCO2) < 5.4 mm Hg are cut off criteria for adequate systemic perfusion. PvO2 < 26 mm Hg, SvO2 < 44.5%, Sa-vO 2 > 27%, CvO2 < 88 ml/l, O2ER > 27.7%, dPCO2> 7.9 mm Hg have been associated with decreased systemic perfusion. The logistic regression model including combination of O2ER and dPCO2 predicts adequate systemic flow accuracy of 94.3% (sensitivity 87.5%, specificity 94.7%, p = 0.001). Graphics allow to adapt the mathematical model to clinical practice to verify systemic hypoperfusion in newborns with functional single ventricle. Conclusion: The following cut off parameters allow to assess systemic perfusion in newborns with functional single ventricle: PvO2, SvO2, CvO2, Sa-vO 2, O2ER, and dPCO2. The model for predicting the adequacy of systemic perfusion can be used as an effective tool to monitor hemodynamic status in newborns with functional single ventricle.

Objective. Development of automated balance flow-sheets in commodity and feedstock depots. Methods. Methods for modelling balance flow-sheets of commodity and feedstock depots, taking into account petroleum products movement and shipping plans, were applied. Results. Examples of automated balance flow-sheets of commodity and feedstock depots specific to refineries are given with the description of technological processes or oil product shipment processes. Rail, road, water, and pipelines shipment automation processes are shown. Operational and daily summaries of balance objects, such as a tank or a group of tanks (commodity group), are graphically shown. An example of an automated dispatch sheet formed on the basis of corresponding automated balance flow-sheets is given. Conclusion. The implementation of automated flow-sheets for all significant production facilities at a refinery, in particular for commodity and feedstock depots, will ensure the visibility and transparency of information on balance objects, which is subsequently used to close a balance sheet for the enterprise as a whole.

Background. Physical activity is associated with better asthma control and life quality in asthma. Osteoarthritis is one of the less studied comorbidities in asthmatic patients. Methods. The study included 38 patients diagnosed with asthma, 65 patients with asthma and osteoarthritis, and 40 volunteers who did not suffer from asthma and osteoarthritis. During the study, 3 groups were formed: Group 1 consisted of patients with asthma; Group 2 included patients with both asthma and osteoarthritis, Control group consisted of volunteers. Spirometry, Asthma Quality of Life Questionnaire (AQLQ), Asthma Control Test (АСТ) were used in asthmatic patients. International Physical Activity Questionnaire (IPAQ) in its short from was filled by all the participants. Results. Both Group 1 and Group 2 did not engage in vigorous physical activity. The spectrum and the frequencies of cytogenetic abnormalities in young cattle immunized with a vaccine against salmonellosis of calves were investigated. The study was carried out on the farm of Novosibirsk region on 10 clinically healthy Holstein black-and-white calves of 10-17 days of age. A concentrated formol-alum vaccine against salmonellosis (paratyphoid) of calves was used at a dose of 2 ml (reimmunization at a dose of 2 ml) with an interval of 10 days between injections. The vaccine was made from the culture of bacteria of the Salmonella dublin strain № 373, inactivated with formalin with the addition of potassium alum and calcium chloride. Cytogenetic analysis of peripheral blood in calves was carried out before vaccination (control), 2 and 9 days after vaccination, 2 and 9 days after revaccination. It was found that the spectrum of somatic chromosomal instability in peripheral blood lymphocytes of calves after vaccination and revaccination is represented by polyploidy, hypoploidy and hyperploidy, chromatid and chromosomal breaks, single and paired fragments of chromosomes. It was revealed that the spectrum of somatic chromosomal instability after double immunizations with an inactivated vaccine against salmonellosis did not differ from the spectrum of spontaneously occurring mutations in this species. Vaccination and subsequent revaccination of calves in comparison with the pre-vaccination period did not lead to a significant increase in the frequency of aneuploid and polyploid cells. During double immunization of calves, a wave pattern in the variation of genomic mutation frequencies from maximum to minimum values in the lymphocytic blood cells of animals was noted, similar to prolonged mutagenesis. A tendency was found for the frequency of structural chromosome abnormalities to increase 2 and 9 days after vaccination and 2 days after revaccination. There was a credible 2.9-fold increase in the frequency of cells with chromosomal aberrations in the blood lymphocytes of animals 9 days after their repeated immunization due to breaks and paired fragments of chromosomes. After vaccination and revaccination, chromatid breaks were most often recorded in the medial regions of one of the chromatids, and chromosomal breaks in the medial and telomeric regions of both chromatids.

Relevance. There is limited evidence on the effect of various vaccines on the human reproductive system. Potential adverse effects of vaccines on fertility are associated with autoimmune disorders, which might cause gonadal damage. Currently, several studies are underway investigating the impact of COVID-19 vaccines on human fertility. To our best knowledge, there is only one study published which demonstrated no adverse effects of the COVID-19 vaccine on assisted reproductive technology outcomes. Aim. To investigate the effect of the Gam-COVID-Vac (Sputnik V) vaccine on ovarian reserve and antiphospholipid antibody level in reproductive-age women. Materials and methods. The prospective study included 51 women vaccinated against COVID-19 with Gam-COVID-Vac (Sputnik V) vaccine. The inclusion criteria were age from 18 to 45, preserved menstrual function, normal ovarian reserve, no history of COVID-19, negative PCR test result for SARS-CoV-2 and negative SARS-CoV-2 IgG antibody test before vaccination, no pregnancy, and no history of serious illnesses. Clinical evaluation was carried out twice-immediately before immunization and 90 days after the first vaccine component administration. The antral follicle count was measured by pelvic ultrasound. Serum levels of AMH, FSH, TSH, estradiol, antiphospholipid antibodies (aPL) M and G isotypes against cardiolipin (aCL), β2-glycoprotein-1 (aβ2-GP-1), annexin V (aAn V), phosphatidylserine (aPS), and IgG antibodies against SARS-CoV-2 were measured by enzyme immunoassay. Results. There were no significant changes in hormones levels and antral follicle counts before and after vaccination, including in women of advanced reproductive age (≥37 years). After immunization, aPL antibody levels did not differ significantly from the baseline. There was no correlation between aPL antibody level dynamics and level of FSH and AMH, which indirectly demonstrates no possible autoimmune effect of vaccination on women's fertility. Conclusion. This is the first study investigating the effect of the Gam-COVID-Vac vaccine on ovarian reserve parameters and aPL antibody levels. The preliminary results prove that the Gam-COVID-Vac vaccine in women of reproductive age does not adversely impact ovarian reserve.

Statement of the problem. One of the relevant scientific and technical issues for improving the state-of-the-art and advanced monitoring methods is alarm construction (including in the minds of ordinary minds) with the emergence of uncompromising objects, and the future of unfinished control. It is quite common that conditions arise when it is not possible to perform a full-fledged monitoring of certain types of deformations of building structures by means of traditional methods. It is required to develop universal, superior and manual methods for the control of different geometric parameters of structures for taking timely measures to ensure the stability and stability of the structure and enabling the transition of the structure in the interconnection of the robotic mill as well. Results. All of the considered methods of statistics of the ways to control the output and have been revised based on the maintenance and repair of the conserved Voronezh nuclear power plant for heat supply.Conclusions. The methods of calculating deformations and the quality of the preparation of alarm constructions for metric photography allowance to carry out repeated monitoring of the technical mill with the necessary accuracy.

Estimations of the frequency of acute kidney injury in SARS-CoV-2 (COVID-19) coronavirus disease are very diverse. The literature data is highly inconsistent and shows AKI ranging from 0.5 to 80%.The objective: to analyze the incidence of renal dysfunction in COVID-19 patients and assess the significance of systemic inflammation for its development.Subjects and methods: The analysis was performed basing on data of 3,806 patients with COVID-19 treated at the Pavlov State Medical University, 395 of them were admitted to the intensive care units (ICU). The criterion for establishing renal dysfunction (RD) is the increase in blood creatinine level above the upper limit of reference values (0.115 mmol/l). Patients with end-stage chronic kidney disease who needed to continue routine long-term dialysis were not included in the study. We analyzed the incidence of renal dysfunction, changes in blood levels of creatinine, urea, and electrolytes during 8 days. In addition, glomerular filtration rate, diuresis volume, levels of hematocrit, hemoglobin, LDH, CRP, ferritin, and procalcitonin were evaluated.Results. The frequency of RD among all patients was 19.0%, among patients in the ICU – 41.0%. In 79% and 81%, respectively, it was detected on the first day of hospitalization. The increase in the number of patients with RD and the aggravation of the existing dysfunction occurred after 6 days. At the initial stage of the disease, the manifestations of RD in most cases were not expressed even in those with an unfavorable course of the disease but the level of creatinine showed a weak but significant (p < 0.5) correlation with changes in CRP (r = 0.110), ferritin (r = 0.137), and procalcitonin (PCT, r = 0.418). The difference in the level of creatinine in patients with PСT level above and below 0.5 ng/ml was observed on the first day only; the value of this parameter returned to normal faster in the subgroup of patients whose procalcitonin level did not exceed 0.5 ng/ml.Conclusion. In case of the signs indicative of RD, it is advisable to distinguish between primary and secondary injury. In the first case, it is primarily due to systemic inflammation, in the second case it is caused by additional impact of other aggressive factors. This will make it possible to clarify the renal and non-renal indications for renal replacement therapy (RRT) in patients with COVID-19, and to evaluate the results adequately since the effectiveness of RRT at different stages of the disease cannot be the same.

Diagnosis of vitamin D deficiency and its management is the most difficult problem of modern neonatology.The objective: to study the concentration of 25(OH)D3 in newborns of St. Petersburg depending on the presence of congenital malformations (CM).Subjects and methods. 60 newborns were examined, their gestational age made 39.4 (38-41) weeks. Children were divided into 2 groups: Group 1 – healthy children, Group 2 – children with CM.Results. Regardless of the presence of CM, the calcifedol plasma concentration was below 15 ng/ml. It was found that children with CM whose plasma calcifedol concentration was below 8.0 ng/ml needed long-term invasive mechanical ventilation (80.0 vs 40.0 hours; p < 0.005) and longer treatment in ICU (7.0 vs 4.0; p < 0,002). Negative correlations were revealed between calcifedol concentration, duration of narcotic analgesics administration (R = -0.44; p = 0.01), duration of mechanical ventilation (R = -0.49; p = 0.003) and stay in ICU (R = -0.54; p = 0.001).Conclusion. The most pronounced deficiency of calcifedol was in children with severe CM requiring long-term treatment in ICU.

For correct and reliable experimental in vivo assessment of antistress effect of various bioactive substances, appropriate biomodels reproducing stress and organism response to stress in laboratory animals should be chosen. We chose treadmill test for simulating exhaustive physical load and forced immobilization accompanied by disorders of physiological and psychological condition. Verification of the models used indicates their wide applicability for testing certain biological manifestations under reproduced stress exposure.

Islands in the form of truncated triangular pyramids on the surface of an epitaxial Ge3Sb2Te6 layer grown on a Si(111) substrate are identified by scanning electron microscopy. It is established that sides of their bases are oriented along the Si〈110〉 directions, and sizes vary from several tens to several hundred nanometers. It is shown by high-resolution electron microscopy of cross-sectional specimens that the (111) planes of the cubic phase or (0001) planes of the hexagonal phase of Ge3Sb2Te6 in the epitaxial layer are oriented parallel to Si(111) and, in surface domains, can make an angle of 70.5° with the layer–substrate interface. It is established, using an atomic structure analysis and energy-dispersive X-ray microanalysis, that the composition in islands changes from Ge3Sb2Te6 to GeSb2Te4 along the growth direction.

40 adolescents aged 15–17 years with functional cardiovascular pathology of vegetative origin underwent a 24-hour blood pressure monitoring (using the oscillometric method BPLabVasotens, Peter Telegin LLC, Nizhny Novgorod) with an assessment of central blood pressure parameters and rigidity of the main arteries.The scientists found significantly higher values of central systolic pressure during the day and night hours in the group of adolescents with arterial hypertension (n=13) compared to adolescents with normal blood pressure (n=27). They determined significantly higher values of the pulse wave velocity both during 24 hours and in the day and night hours in the group with arterial hypertension compared to the group of adolescents with normal blood pressure. No differences were found in the parameters of the propagation time of the reflected pulse wave and the augmentation index. The time of the reflected pulse wave propagation was significantly lower at nighttime compared to the daytime. In the group with arterial hypertension, the rate of pulse wave propagation in the aorta correlated only with the values of diastolic pressure over 24 hours and diastolic pressure in the daytime. In the adolescents with normal blood pressure, the pulse wave velocity correlated with systolic and pulse blood pressure. The augmentation index in the group of adolescents with arterial hypertension correlated with diastolic pressure, in contrast to the group of adolescents with normal blood pressure, where such a correlation was not detected.

Background. An urgent problem is the development and introduction of cultivars for universal use that can meet the demand of animal husbandry for various types of feed and that of food industry for raw materials. Of great interest in this regard is ‘Tobolyak’, a new spring oat cultivar characterized by a high yield of grain and green biomass.Materials and methods. The cultivar was developed at the Research Institute of Agriculture for the Northern TransUrals, a branch of the Tyumen Scientific Centre, Siberian Branch of the RAS, using hybridization techniques with subsequent selection. The cultivars used as source material for hybridization were ‘Tayoznik’ (Narym Agricultural Station, Tomsk Province) and ‘Orion’ (Siberian Research Institute of Agriculture, Omsk Province).Results. It is a mid-ripening cultivar, with the growing season of 71 to 83 days from sprouting to wax ripeness; its plants are medium-tall (89.6 to 120.2 cm), and resistant to lodging. The average grain yield for the years of study (2014–2019) under the environmental conditions of the northern forest-steppe (Tyumen Province) was 5.88 t/ha (+0.41 t/ha to the reference). The maximum grain yield (7.97 t/ha) was obtained in 2019 at the Lower Tavda Variety Testing Plot, Tyumen Province. The cultivar formed a grain with the test weight of 459.5–527.0 g/l and a low hull content of 22.1–25.1%. The yield of green biomass varied from 31.4 to 47.3 t/ha, depending on the growing conditions, and averaged 37.7 t/ha in 2014–2019 (+7.3 t/ha to the reference). Dry matter harvest averaged 11.76 t/ha (+1.54 t/ha to the reference), ranging from 8.62 to 14.56 t/ha.Conclusion. The new spring oat cultivar ‘Tobolyak’ for universal use has been listed the State Register for Selection Achievements since 2020 and recommended for cultivation in regions 10, 11 and 12 of the Russian Federation.
