Background Docetaxel (DTX) exhibits antitumor effects against breast cancer by stabilizing microtubules and increasing the accumulation of reactive oxygen species (ROS). DTX extravasation during infusion often causes skin injury. The present study aimed to investigate the effects and mechanisms of icaritin (ICT) on DTX‐induced skin injury. Methods The effects of ICT on the viability and apoptosis of HaCaT cells were measured by SRB assay and flow cytometry, respectively. Endogenous LC3 puncta and microtubules were determined by immunofluorescence. The number of mitochondria was measured by MitoTracker orange staining. ROS were determined by dihydroethidium staining. The expression of markers of ROS and autophagy were measured by western blotting. Chloroquine, compound D, and tamoxifen were employed as the inhibitor for autophagy and AMPK, estrogen receptors (ERs) modulator, respectively. Results DTX inhibited the viability and decreased apoptosis of HaCaT cells, which can be rescued by ICT. ICT decreased microtubule bundles, increased the number of mitochondria, and attenuated ROS of HaCaT cells induced by DTX. ICT blocks autophagy and the autophagic flux. Compound C or tamoxifen diminished the protection effects of ICT on DTX‐treated HaCaT cells. Conclusion ICT alleviates DTX‐induced skin injury by suppressing ROS, reducing microtubule bundles, and blocking autophagy via ERs. Our study indicated that ICT may be a potential candidate for DTX‐induced skin injury., ICT alleviates DTX‐induced skin injury by alleviating the microtubule bundles, while the specific mechanism is unknown. At the same time, ICT, as an ER modulator, promotes the phosphorylation of the α subunit of AMPK to activate AMPK, thereby regulating the level of autophagy. ICT also reduces the level of ROS by acting as an ER modulator to reduce cell apoptosis.

During invasive mechanical ventilation, knowledge of the patient’s respiratory effort is valuable in guiding the clinical team to perform a personalized therapy. The same adjustment of the ventilator can produce excessive transpulmonary pressures for different patients, capable of generating or aggravating pre-existing lung injuries. The measurement of transpulmonary pressure (the difference between airway and pleural pressures) is not easily performed in practice. Although airway pressure measurement is available on most current mechanical ventilators, pleural pressure measurement is indirectly performed using an esophageal balloon. In many cases, esophageal pressure reading takes other phenomena into account and is not a reliable measure of pleural pressure. This work will study system identification techniques to obtain reliable pleural pressures based on esophageal pressure readings, aiming to provide the clinical team with information about the patient’s ventilatory therapy’s real status. By estimating transfer function models, autoregressive with external input (ARX) and output error (OE) polynomial models, waveforms were adjusted in two different patients’ occlusion maneuver data. Different metrics were used to assess the quality of the models obtained. The analysis results showed that the estimation methods used provided plausible representations of the underlying dynamic system. Therefore, there are indications that the system identification techniques presented in this work can be used in general, in a clinical environment, to provide reliable estimates of pleural pressure based on esophageal pressure measurements.

Local anesthesia (LA) is the most important pain management process in oral and maxillofacial surgery. Safe and effective LA not only enable patients to obtain high-quality treatment, but also relieve the anxiety of patients when they come to the clinic. The choices of local anesthetic and injection methods determine the success of LA to a great extent. At present, in most countries or regions, common local anesthetics used in oral and maxillofacial surgery belong to amides and they are injected into patients' body mainly through block or infiltration anesthesia. In addition, the operators' technique level, patient's subjective psychology and anatomical variation of maxillofacial structure also have a strong influence on LA in dental clinic. Due to the existence of above factors, the worldwide success rates of LA in oral and maxillofacial surgery is very different. There are no specific LA methods that ensure 100% successful LA rates. Fortunately, the development of new local anesthetic and injection technology are providing us with new ideas to solve this problem. This review mainly report the new research progress on LA in oral and maxillofacial surgery in recent decades and help clinicians with dental LA operation.

Background Laparoscopic repair is a well-accepted treatment modality for perforated peptic ulcer (PPU). However, intraoperative conversion to laparotomy is still not uncommon. We aimed to identify preoperative factors strongly associated with conversion. Methods A retrospective review of records of all PPU patients treated between January 2011 and July 2019 was performed. Patients were divided into three groups: laparoscopic repair (LR), conversion to laparotomy (CL), and primary laparotomy (PL). Patient demographics, operative findings, and outcomes were compared between the groups. Logistic regression analyses were performed, taking conversion as the outcome. Results Of 822 patients, there were 236, 45, and 541 in the LR, CL, and PL groups, respectively. The conversion rate was 16%. Compared with those in the LR group, patients in the CL group were older (p p p p = 0.003). PULP score was the only independent risk factor for conversion. The area under the curve (AUC) for the PULP score to predict conversion was 75.3%, with a best cut-off value of ≥ 4. The operative time was shorter for PL group patients than for CL group patients with PULP scores ≥ 4. For patients with PULP scores Conclusion The PULP score may have utility in predicting and minimizing conversion for laparoscopic PPU repair. Laparoscopic repair is the procedure of choice for PPU patients with PULP scores

BACKGROUND Nerve diseases and injuries, which are usually accompanied by motor or sensory dysfunction and disorder, impose a heavy burden upon patients and greatly reduce their quality of life. Dental pulp stem cells (DPSCs), derived from the neural crest, have many characteristics that are similar to those of neural cells, indicating that they can be an ideal source for neural repair. AIM To explore the potential roles and molecular mechanisms of DPSCs in crushed nerve recovery. METHODS DPSCs were isolated, cultured, and identified by multilineage differentiation and flow cytometry. Western blot and immunofluorescent staining were applied to analyze the expression levels of neurotrophic proteins in DPSCs after neural induction. Then, we collected the secretions of DPSCs. We analyzed their effects on RSC96 cell proliferation and migration by CCK8 and transwell assays. Finally, we generated a sciatic nerve crush injury model in vivo and used the sciatic function index, walking track analysis, muscle weight, and hematoxylin & eosin (H&E) staining to further evaluate the nerve repair ability of DPSCs. RESULTS DPSCs highly expressed several specific neural markers, including GFAP, S100, Nestin, P75, and NF200, and were inclined toward neural differentiation. Furthermore, neural-induced DPSCs (N-DPSCs) could express neurotrophic factors, including NGF, BDNF, and GDNF. The secretions of N-DPSCs could enhance the proliferation and migration of Schwann cells. In vivo, both DPSC and N-DPSC implants alleviated gastrocnemius muscle atrophy. However, in terms of anatomy and motor function, as shown by H&E staining, immunofluorescent staining, and walking track analyses, the repair effects of N-DPSCs were more sustained, potent, and effective than those of DPSCs and the controls. CONCLUSION In summary, this study demonstrated that DPSCs are inclined to differentiate into neural cells. N-DPSCs express neurotrophic proteins that could enhance the proliferation and migration of SCs. Furthermore, our results suggested that N-DPSCs could help crushed nerves with functional recovery and anatomical repair in vivo. Thus, DPSCs or N-DPSCs could be a promising therapeutic cell source for peripheral nerve repair and regeneration.

Background: With a global rising trend in prevalence of allergic diseases, more attention has been paid to investigation of environmental risk factors. Many risk factors have so far been identified. However, novel risk factors specific to Taiwanese environment and lifestyle were still relatively unknown. Objective: To investigate the potential effects of a number of little-known indoor risk factors on the frequency of doctor's visit for respiratory problems in context of Taiwanese environment and lifestyle. Methods: A cross-sectional, population-based study was performed on a 861 participants around Kaohsiung area, Taiwan. Survey investigation was employed to assess the household environment and the frequency of doctor's visit for respiratory problems. Results: Participants who performed “daily cleaning” was shown to have a significantly (p=0.007) higher mean number of doctor's visits in comparison to those who did not. Similar observation was made for participants who periodically took out beddings (p=0.042). Age had a significant positive correlation (linear regression β 0.089) with frequency of respiratory problems. Conclusion: The habit of daily cleaning was implicated as a potential indoor risk factor due to the unique nature of Taiwanese cleaning habit and close contact with cleaning supplies, which could serve as chemical irritants. Bedding takeout was predicted to be an indicator of chronic allergies rather than an actual risk factor. However, both were controversial in their role as potential indoor risk factor, and required further examination.

In order to realize underwater optical wireless communication (UOWC) under long-distance, single photon avalanche diode (SPAD) is used as the receiver. Since the SPAD outputs discrete single photon pulse signals, the key issue is how to recover data from the discrete single photon pulses. Traditional photon-counting UOWC uses SPAD array with high gain to recover data directly. The disadvantage of this method is that the number of photons required to transmit 1 bit data is large, and the transmission distance is also limited. In order to reduce the number of photons required to transmit 1 bit data, we proposed a method of extracting the time slot synchronization clock (TSSC) from the received discrete photon-counting pulses and recovering the data. Based on the random Poisson process of the receiver and the attenuation process of the underwater channel, a photon-counting UOWC model was established. Meanwhile, to verify the method proposed, an underwater light emitting diode (LED) communication system based on SPAD was built. The experimental results show that without considering the background light noise, the photon-counting UOWC system can achieve a satisfactory performance of 0.875 photons/bit with a symbol error rate (SER) of 1.8 × 10−3 by using 256-pulse-position modulation (PPM) and the TSSC extraction method we proposed.

Nerve injury is a critical problem in the clinic. Nerve injury causes serious clinic issues including pain and dysfunctions for patients. The disconnection between damaged neural fibers and muscles will result in muscle atrophy in a few weeks if no treatment is applied. Moreover, scientists have discovered that nerve injury can affect the osteogenic differentiation of skeletal stem cells (SSCs) and the fracture repairing. In plastic surgery, muscle atrophy and bone fracture after nerve injury have plagued clinicians for many years. How to promote neural regeneration is the core issue of research in the recent years. Without obvious effects of traditional neurosurgical treatments, research on stem cells in the past 10 years has provided a new therapeutic strategy for us to address this problem. Adipose stem cells (ASCs) are a kind of mesenchymal stem cells that have differentiation potential in adipose tissue. In the recent years, ASCs have become the focus of regenerative medicine. They play a pivotal role in tissue regeneration engineering. As a type of stem cell, ASCs are becoming popular for neuroregenerative medicine due to their advantages and characteristics. In the various diseases of the nervous system, ASCs are gradually applied to treat the related diseases. This review article focuses on the mechanism and clinical application of ASCs in nerve regeneration as well as the related research on ASCs over the past decades.

The knee joint is extremely susceptible to injury, which is usually identified by magnetic resonance imaging (MRI). In the present study, MRI was applied to quantitatively detect the association between anterior cruciate ligament (ACL) rupture and anatomic morphologic changes of the intercondylar notch. Forty patients with unilateral ACL rupture who were treated between July, 2013 and October, 2014 were enrolled in the present study. The patients were divided into the observation (affected side) and control (healthy side) groups. MRI measurements were undertaken based on parameters associated with intercondylar notch of double knee joints. The results showed that intercondylar notch width (ICW) in the observation group was significantly smaller than that in the control group, and differences were statistically significant (P0.05). Notch width index (NWI) and notch shape index (NSI) in the observation group were significantly less than those in the control group and differences were statistically significant (P0.05). The differential value of ICW in the observation group was 2.6±1.3 mm and the ACL rupture time of the affected knee was 20.4±1.3 months on average. The correlation was statistically significant (P0.05). In conclusion, after ACL rupture, ICW on the affected knee had significant stenosis, NSI and NWI were significantly reduced and the stenosis degree was aggravated with the prolongation of course. By contrast, Lysholm and Tegner scoring of patients with different degrees of stenosis had no correlation.

Cardiovascular complications represent a leading cause of mortality in patients with type 2 diabetes mellitus (T2DM). During such complicated progression, subtle variations in the cardiovascular risk (CVR)-related biomarkers have been used to identify cardiovascular disease at the incipient stage. In this study we attempt to integrally characterize the progression of cardiovascular complications and to assess the beneficial effects of metformin combined with salvianolic acid A (Sal A), in Goto-Kakizaki (GK) rats with spontaneous T2DM. The rats were treated with metformin (200 mg·kg -1 ·d -1 , ig) alone or in combination with Sal A (1 mg·kg -1 ·d -1 , ip) at ages from 8 to 22 weeks. During the treatment, the levels of asymmetric dimethylarginine, L-arginine, superoxide dismutase, malondialdehyde, glucose, high density lipoprotein and low density lipoprotein were assessed. Based on alterations in these biomarkers, a mini-network balance model was established using matrixes and vectors. Radar charts were created to visually depict the disruption of CVR-related modules (endothelial function, oxidative stress, glycation and lipid profiles). The description for the progression of cardiovascular disorder was quantitatively represented by u, the dynamic parameter of the model. The modeling results suggested that untreated GK rats tended to have more severe cardiovascular complications than the treatment groups. Metformin monotherapy retarded disease deterioration, whereas the combination treatment ameliorated the disease progression via restoring the balance. The current study, which focused on the balance of the mini-network and interactions among CVR-related modules, proposes a novel method for evaluating the progression of cardiovascular complications in T2DM as well as a more beneficial intervention strategy.

The correlation techniques of nodes cooperative transmission strategy in wireless sensor networks with virtual MIMO are presented. A communication model between clusters is proposed. From it, we can see the correlation techniques include coding, forwarding, combining and decoding. The coding is generally divided into three categories: Layered Space-Time Coding, Space-Time Trellis Coding and Space-Time Block Coding. Forwarding can be broken down into Amplifying Forwarding, Decode Forwarding, and Selecting Dynamic Relay. Select combining, equal gain combining and maximum ratio combining are the common combine methods. The decode method is maximum likelihood algorithm.

Objective To discuss the effect of lesser-known potential risk factors, such as bedroom showers, on the prevalence of allergic rhinitis. Method A cross-sectional, population-based study was performed using both survey and fungal culturing in southern Taiwan. There were 998 participants enrolled in the survey, and 513 sets of fungal culture obtained. With score for allergic rhinitis (SFAR) more than 7, the patient was defined to have allergic rhinitis. Risks of allergic rhinitis were calculated as odds ratios for various predicted risk factors by logistic regression. Correlation between predicted risk factors and fungal level were examined with linear regression. Results The adjusted odds ratio of frequently using bedroom shower to having allergic rhinitis was 1.572 (95% confidence interval: 1.090–2.265), and 0.962 for people with older age to have AR (95% confidence interval: 0.949–0.976). As to the 24-hour fungal level, the standardized coefficient was 0.254 for frequent use of bedroom shower, and 0.106 for window open hours. Conclusions Use of bedroom shower is a potential risk factor for allergic rhinitis development.

We demonstrate a single photon compressive imaging system with the image plane up to the entire digital micro-mirror device (DMD) work area. A parallel light source is designed to reduce the influence of light scattering on imaging resolution and a photon counting photomultiplier tube (PMT) with a large photosensitive area is used to effectively collect light reflected from the full screen of DMD. A control and counting circuit, based on Field-Programmable Gate Array (FPGA), is developed to load binary random matrix into the DMD controller for each measurement, and to count single-photon pulse output from PMT simultaneously. To reduce imaging time and huge memory occupation for image reconstruction, a multiple micro-mirrors combination imaging method is proposed. The signal-to-noise ratio and detection limit of the imaging system is theoretically deduced. Theoretical analysis and experimental results show that micro-mirrors combination imaging method is more suitable for faster imaging in a weaker-light-level environment. In order to achieve high imaging quality, the size of the combined pixels and the average time of each measurement should be moderate, so that the impact of Poisson shot noise is minimized.

In this paper, constructive neural network for landmine classification using ultra wideband (UWB) ground penetrating radar (GPR) is presented. GPR echo signal is composed of three parts: ground bounce, clutter and target echo signal, the target echo signal is deteriorated by the clutter. Firstly WP-based preprocessing algorithm is used to ground bounce removal and clutter reduction and feature extraction of GPR echo signal. Then wrapper based approach is adopted to feature subset selection of GPR echo signal using genetic algorithm(GA) in conjunction with constructive neural network learning algorithm, and at the meanwhile, the result of classification of landmine is obtained. Experiment result based on GPR measured data shows that the feasibility and advantage of the presented algorithm.
