1. Disaggregating Data on Pacific Islander Gastric Cancer Patients Reveals Survival Disparity

Abstract

The incidence and prognosis of Pacific Islanders with gastric cancer is not well documented as previous studies have often aggregated this population with Asians. The purpose of our study was to describe patient and tumor characteristics, as well as prognostic factors of Pacific Islanders with gastric cancer. Patients diagnosed with gastroesophageal junction or gastric adenocarcinoma between 2000 and 2014 were identified in the tumor registry of the largest hospital in Hawaii. Overall survival of Asians, Whites, and Pacific Islanders were calculated using the Kaplan-Meier method and log-rank test. Cox proportional hazards regression models were constructed to assess predictors of survival adjusting for clinical and pathological factors. A total of 615 patients were included in the final analysis. Pacific Islanders were found to present at a younger age, were more often uninsured or had Medicaid insurance, and were diagnosed with a higher stage of cancer compared to their Asian and White counterparts. Pacific Islanders were less likely to undergo surgery even after adjusting for stage. Race was a prognostic factor and survival was lowest among Pacific Islanders, but only if the model was unadjusted for treatment. We present an analysis of the largest cohort of Pacific Islander gastric cancer patients. Pacific Islanders have different sociodemographic characteristics and inferior survival compared to Asian patients and should be independently studied.

Abstract

Coronavirus disease 2019 (COVID-19) is now an epidemic of global proportion, with major adverse impacts on older adults, persons with chronic diseases, and especially residents of long-term care facilities. This health catastrophe has challenged healthcare facilities' capacity to deliver care to not only COVID-19 patients but all patients who need hospital care. Abstract

Despite the current understanding of the pathophysiology of sepsis and advances in its treatment, the rate of sepsis is increasing globally. Sepsis is a common cause of hospitalization in older adults, and infections are among the most common diagnoses among residents transferred to the hospital from long-term care facilities (LTCFs). LTCFs and hospitals are facing financial and regulatory requirements to reduce potentially preventable emergency department visits, hospitalizations, and hospital readmissions due to infections and other causes. In addition, the human and financial costs of these events are substantial. Current criteria for early identification of sepsis have low sensitivity and specificity among LTCF residents. Early diagnosis must focus on changes in clinical, mental, and functional status, and vital signs including pulse oximetry. Laboratory data can increase the suspicion of sepsis, but the availability of testing and timing of results limits its usefulness in most LTCFs.While new diagnostic criteria for sepsis are being developed and validated in the LTCF setting, clinical practice and decision support tools are available to guide management. Most LTFCs do not have the capabilities to manage sepsis based on current guidelines despite availability of qualified nursing staff and clinicians. Thus excluding circumstances in which a resident's desire is palliative or hospice care without transfer to a hospital, most LTCFs will continue to transfer residents with severe infections at risk for evolving into sepsis to an acute hospital setting. Abstract

The University of Tokyo Atacama Observatory Project is to construct a 6.5 m infrared-optimized telescope at the summit of Co. Chajnantor (5640 m altitude) in northern Chile. The telescope optics uses a Ritchey-Chretien type layout, with an under-sized secondary mirror to reduce stray light caused by thermal emission from the telescope structure. The primary mirror is a F/1.25 lightweight borosilicate glass (Ohara E6) mirror with honeycomb structure, which is developed by Steward Observatory Richard F. Caris Mirror Lab. The telescope has two Nasmyth foci and two folded-Cassegrain foci, which can be switched by rotating a tertiary mirror. The final focal ratio is 12.2 with a field of view of 25 arcmin in diameter. The telescope mount is a tripod-disk alt-azimuth mount. Both the azimuth and elevation axes are supported by and run on hydrostatic bearings, and they are driven by friction drives with servo motors, which are controlled by the telescope control system. It also controls the hexapod mount of the secondary mirror and the pneumatic actuators of the primary mirror support to keep good image quality during the observation. An off-axis Shack-Hartmann sensor installed in each focus measures the wavefront aberration of the telescope optics, then the misalignment between the secondary and primary mirrors is corrected by adjusting the hexapod mount while other aberrations are corrected by the deformation of the primary mirror. The force distribution of the actuators for correction will be calculated by fitting the wave-front errors with a series of bending modes of the primary mirror.

Abstract

Two European phase III trials comparing D1 and D2 demonstrated that D2 did not improve the overall survival and was associated with a high mortality related to splenectomy. However, a long-term follow-up study showed that the gastric cancer-related death rate was significantly higher in D1 than D2. Based on these findings, the standard surgery in Europe became D2 without pancreatico-splenectomy to prevent mortality. In contrast, the JCOG9501 phase III comparing D2 and D2 plus para-aortic nodal dissection did not showed a survival efficacy of extended lymphadenectomy, but the mortality rate was quite low in both surgeries. Subsequently, the JCOG0110 phase III study comparing D2 and spleen-preserving D2 for upper gastric cancer not invading the greater curvature clearly showed the non-inferiority of spleen preservation. Thus, spleen-preserving D2 was made the standard surgery for these tumors in Japan. However, splenectomy is often selected for complete dissection of the splenic-hilar nodes, a frequent metastatic site for upper gastric tumors invading the greater curvature. Recently, an approach involving splenic hilar nodal dissection without splenectomy has been developed.

Abstract

Inappropriate antibiotic use is common in older adults (aged >65 y), and they are particularly vulnerable to serious antibiotic-associated adverse effects such as cardiac arrhythmias, delirium, aortic dissection, drug-drug interactions, and Clostridioides difficile. Antibiotic prescribing improvement efforts in older adults have been primarily focused on inpatient and long-term care settings. However, the ambulatory care setting is where the vast majority of antibiotic prescribing to older adults occurs. To help improve the clinical care of older adults, we review drivers of antibiotic prescribing in this population, explore systems aspects of ambulatory care that can create barriers to optimal antibiotic use, discuss existing stewardship interventions, and provide guidance on priority areas for future inquiry. Abstract

The reinstatement of the Journal of the American Geriatrics Society Section on Ethnogeriatrics coincides with more rapid growth in older populations that are classified as ethnic and racial minorities in the United States than those classified as non-Hispanic white. By 2060, 40% of older Americans are predicted to belong to a minority. Important needs for ethnogeriatric research and publication include: making sure research populations are meaningful and precise rather than using categories that include many unrelated groups; and increasing research among smaller ethnic populations of older adults. Topics in need of attention include epidemiology of common geriatric illness among the smaller populations, and social determinants for those in which disparities have been established; the extent and effectiveness of use of Culturally and Linguistically Appropriate Services Standards in geriatric care; examination of communication strategies used in provider/patient interaction, especially use of interpreters; ethnic differences in treatment of older patients; and models of use of community health workers from older patients' own cultural communities. Stresses related to caregiving in cultures emphasizing the importance of family care are important to understand acceptable models of long-term care for diverse families. The new Ethnogeriatric Section opens the opportunity for geriatric researchers, especially those from diverse backgrounds, to explore important issues in cross-cultural geriatric care; their findings can then form the basis of expanded ethnogeriatric curriculum for training future providers for the growing population of diverse older Americans.

Abstract

Norovirus infection usually results in acute gastroenteritis, often with incapacitating nausea, vomiting, and diarrhea. It is highly contagious and resistant to eradication with alcohol-based hand sanitizer. Appropriate preventative and infection control measures can mitigate the morbidity and mortality associated with norovirus infection. Clostridium difficile infection is the leading cause of health care-associated diarrhea in the United States. Antibiotic use is by far the most common risk factor for C difficile colonization and infection. Appropriate preventive measures and judicious use of antibiotics can help mitigate the morbidity and mortality associated with C difficile infection.

Abstract

Noroviruses have emerged as one of the leading causes of viral gastroenteritis worldwide, affecting community-dwelling and institutionalized older adults. Recent global epidemics present a growing challenge to the healthcare system and to long-term care facilities. Noroviruses spread readily and rapidly through multiple routes (e.g., person-to-person contact, contact with contaminated surfaces, airborne dissemination of vomitus) and thus are able to sustain an epidemic efficiently and successfully. Although norovirus gastroenteritis is a short self-limited illness in healthy immunocompetent individuals, it can result in significant morbidity and mortality in vulnerable compromised persons such as frail elderly persons and older residents of nursing homes. Diagnosis is made by clinical assessment and confirmed primarily by stool evaluation using polymerase chain reaction. Treatment is confined to supportive measures. Public health prevention and control strategies provide guidance regarding surveillance and the necessary steps to curb the clinical effect and spread of norovirus infections in various settings, including long-term care.

Abstract

The diagnosis, treatment and prevention of infectious diseases in older adults in long-term care facilities (LTCFs), particularly nursing facilities, remain a challenge for all health providers who care for this population. In this review, the authors provide updated information on the currently most important issues of infectious diseases in LTCFs. With the increasing prescribing of antibiotics in older adults, particularly in LTCFs, the topic of antibiotic stewardship is presented “front and center” in this review. Following this discussion salient points on the clinical relevance, clinical presentation, diagnostic approach, therapy, and prevention are discussed for skin and soft tissue infections, infectious diarrhea (Clostridium difficile and norovirus infections), bacterial pneumonia, and urinary tract infection as well as some of the newer approaches to preventive interventions in the LTCF setting.

Abstract

Introduction Multiple chemical sensitivity (MCS), often called idiopathic environmental intolerance, is a chronic acquired disorder characterised by nonspecific symptoms attributed to exposure to common odorous chemicals. We previously reported significant activations in the prefrontal cortex (PFC) during olfactory stimulation using several different odorants in patients with MCS compared with controls. Previous clinical observations demonstrated differences in oxygen partial pressure in peripheral venous blood between patients with MCS and controls. Our objective is to investigate peripheral blood oxygen levels during olfactory stimulation to characterise patients with MCS. Methods We investigated changes in the prefrontal area using near-infrared spectroscopic (NIRS) imaging and those in peripheral arterial blood oxygen saturation (SpO2) using a pulse oximeter during olfactory stimulation with odorants (γ-undecalactone) at three concentrations (zero, odour recognition threshold, and normal perceived odour level) in 11 patients with MCS and 10 controls. We also examined their oxygen partial pressures in peripheral venous blood at normal condition before the test. Results We observed significant activations in the PFC during olfactory stimulation in patients with MCS. The SpO2 in peripheral arterial blood in patients with MCS remained higher than that in controls during olfactory stimulation tests. The SpO2 remarkably decreased in patients with MCS compared with that in controls during olfactory stimulus at normal perceived odour level and the recovery of the SpO2 after the stimulus was delayed in these patients. No significant differences in oxygen partial pressures of peripheral venous blood were observed. Conclusion Patients with MCS exhibited stronger brain responses to odours at normally perceived levels. Changes in SpO2 in the peripheral arterial blood suggest that MCS might result in poor oxygen supply to the peripheral tissues or inadequately control the oxygen supply.

Abstract

International audience; Background: Dumping syndrome is a prevalent complication of oesophageal and gastric surgery characterised by early (postprandial tachycardia) and late (hypoglycaemia) postprandial symptoms. Aim: To evaluate efficacy and safety of the somatostatin analogue, pasireotide in patients with dumping syndrome after bariatric or upper gastrointestinal cancer surgery. Methods: A single-arm, open-label, multicentre, intrapatient dose-escalation, phase 2 study with 4 phases: screening, 3-month SC (subcutaneous), 3-month IM (intramuscular) and 6-month optional extension IM phase. Primary endpoint was the proportion of patients without hypoglycaemia (plasma glucose \textless3.3 mmol/L [60 mg/dL] during an oral glucose tolerance test, OGTT) at the end of 3-month SC phase. A \textgreater= 50% response rate was considered clinically relevant. Results: Forty-three patients with late dumping were enrolled; 33 completed the 3-month SC phase and 23 completed the 12-month study. The proportion of patients without hypoglycaemia at month 3 (primary endpoint) was 60.5% (26 of 43; 95% confidence interval, 44.4%-75.0%). Improvement in quality of life was observed during SC phase, which was maintained in the IM phase. The proportion of patients with a rise in pulse rate of \textgreater= 10 beats/min during OGTT reduced from baseline (60.5%) to month 3 (18.6%) and month 12 (27.3%). Overall (month 0-12), the most frequent (\textgreater20% of patients) adverse events were headache (34.9%); diarrhoea, hypoglycaemia (27.9% each); fatigue, nausea (23.3% each); and abdominal pain (20.9%). Conclusion: These results suggest that pasireotide is a promising option in patients with dumping syndrome after bariatric or upper gastrointestinal cancer surgery.

Abstract

Background Plaintiffs of the gefitinib (Iressa) lawsuits in Japan started in 2004 were defeated in the Supreme Court in 2013. The Court judged it was not possible to foresee the outbreak of deaths caused by interstitial pneumonia due to gefitinib from death cases before approval of this drug. Objective We attempted to verify validity of this judgment. Methods We estimated the 95% confidence interval (CI) of the proportion of onset and death cases among 23 onset and 13 death cases occurring from "within 1 week" to "within 4 weeks" from clinical data before approval of this drug using data admitted to the Court. Results For death cases, all of the upper limits of the 95% CI exceeded 50% within 1-4 weeks. This fact suggested that the cases of acute interstitial pneumonia were included in the clinical trial before the approval of gefitinib. Conclusion It was possible to foresee the outbreak of death cases after drug approval. This conclusion showed the Court's ruling was not reasonable and was unscientific.

Abstract

Home energy management system (HEMS) is expected as one of the promising technologies to satisfy the demand. We are developing HEMS assisted by a sensor network having an efficient wireless power supply in order to be spreading around the people. Then we have studied the resonant-type wireless power transmission applying for HEMS because the transmitting power is comparatively high in the middle range by using resonant way. In our study, we have derived the formula of transmitting power efficiency in the system of including exciting coil and resonant coil. And the transmitting characteristic using single loop coil is measured. The calculated value with derived equation and measured value are in good agreement. Also we have tried to calculate using electromagnetic numerical method. After showing the validation of both calculation methods, we have examined the way of coil arrangement that transmitting distance is more than 1 m under the condition of human body safety protection guideline from electromagnetic wave. Finally we have arrived at the design of using the pair resonant coils which are very large coil for transmitting and the small coil for receiving. Then we have calculated the transmitting characteristic using electromagnetic numerical method and concluded this design is very useful for HEMS.

Near-infrared (NIR) high-resolution spectroscopy is a fundamental observational method in astronomy. It provides significant information on the kinematics, the magnetic fields, and the chemical abundances, of astronomical objects embedded in or behind the highly extinctive clouds or at the cosmological distances. Scientific requirements have accelerated the development of the technology required for NIR high resolution spectrographs using 10 m telescopes. WINERED is a near-infrared (NIR) high-resolution spectrograph that is currently mounted on the 1.3 m Araki telescope of the Koyama Astronomical Observatory in Kyoto-Sangyo University, Japan, and has been successfully operated for three years. It covers a wide wavelength range from 0.90 to 1.35 μm (the z-, Y-, and J-bands) with a spectral resolution of R = 28,000 (Wide-mode) and R = 80,000 (Hires-Y and Hires-J modes). WINERED has three distinctive features: (i) optics with no cold stop, (ii) wide spectral coverage, and (iii) high sensitivity. The first feature, originating from the Joyce proposal, was first achieved by WINERED, with a short cutoff infrared array, cold baffles, and custom-made thermal blocking filters, and resulted in reducing the time for development, alignment, and maintenance, as well as the total cost. The second feature is realized with the spectral coverage of Δλ/λ~1/6 in a single exposure. This wide coverage is realized by a combination of a decent optical design with a cross-dispersed echelle and a large format array (2k x 2k HAWAII- 2RG). The Third feature, high sensitivity, is achieved via the high-throughput optics (>60 %) and the very low noise of the system. The major factors affecting the high throughput are the echelle grating and the VPH cross-disperser with high diffraction efficiencies of ~83 % and ~86 %, respectively, and the high QE of HAWAII-2RG (83 % at 1.23 μm). The readout noise of the electronics and the ambient thermal background radiation at longer wavelengths could be major noise sources. The readout noise is 5.3 e- for NDR = 32, and the ambient thermal background is significantly reduced to ~ 0.05 e- pix-1 sec-1 at 273 K. As a result, the limiting magnitudes of WINERED are estimated to be mJ = 13.8 mag for the 1.3 m telescope, mJ = 16.9 mag for the 3.6 m telescope, and mJ = 19.2 mag for 10 m telescope with adoptive optics, respectively. Finally, we introduce some scientific highlights provided by WINERED for both emission and absorption line objects in the fields of stars, the interstellar medium, and the solar system.

Abstract

The purpose of out study is to establish the way for making vibration power generation floor. In the consideration of our study is composed of three processes. The first is to establish the design method obtaining the desirable maximum power from cantilever piezoelectric vibration unit. The 2nd is to construct the vibrating floor. The 3rd is to estimate the charging efficiency using the capacitor. After all we examine the average output power by the vibration power generation floor. The applications of the vibration power generation floor are assumed as power source of sensor network nodes and foot light at night and so on. Considering for the use of power supplier for sensor network node, the desirable generated power has been estimated more than 100uW averagely. That is our target. We have performed above three processes and succeeded in making the cantilever typed vibration power generating floor which can generate 120uW averagely.

Abstract

Despite lower femoral neck bone mass, Japanese women have a substantially lower incidence of hip fracture than North American whites. Reasons for this discrepancy were sought in a study of 57 Japanese and 119 white American women aged 50-79. All women were in good health. Bone mineral content (BMC) in the femoral neck, femoral neck length (NL), femoral neck angle (theta), cross-sectional moment of inertia (CSMI), safety factor (SF), and fall index (FI) were calculated using dual x-ray absorptiometry. Height and weight were greater in Americans than in Japanese (1.62 versus 1.52 m; p < 0.0001 and 66.0 versus 49.4 kg; p < 0.0001, respectively). Mean BMC in the femoral neck and CSMI were greater in Americans than in Japanese (3.91 versus 3.02 g; p < 0.0001 and 0.99 versus 0.57 cm4; p < 0.0001, respectively). NL was longer in Americans (5.6 versus 4.4 cm; p < 0.0001) and theta was larger in Americans (130 versus 128 degrees; p < 0.01), whereas SF and FI were less in Americans than in Japanese (3.41 versus 5.12; p < 0.0001 and 1.00 versus 1.40; p < 0.0001, respectively). These results indicate that despite lower bone mass, Japanese women have lower risks of structural failure in the femoral neck, attributable primarily to shorter femoral necks and, to a lesser degree, a smaller femoral neck angle. Geometric characteristics of the femoral neck in Japanese women are associated with their lower hip fracture risk, and the measurement of proximal femoral geometry, combined with bone mass, may provide further clinical information about the risk of hip fracture.

Abstract

Residents of long-term care facilities (LTCFs) are at great risk for infection. Most residents are older and have multiple comorbidities that complicate recognition of infection; for example, typically defined fever is absent in more than one-half of LTCF residents with serious infection. Furthermore, LTCFs often do not have the on-site equipment or personnel to evaluate suspected infection in the fashion typically performed in acute care hospitals. In recognition of the differences between LTCFs and hospitals with regard to hosts and resources present, the Infectious Diseases Society of America first provided guidelines for evaluation of fever and infection in LTCF residents in 2000. The guideline presented here represents the second edition, updated by data generated over the intervening 8 years. It focuses on the typical elderly person institutionalized with multiple chronic comorbidities and functional disabilities (e.g., a nursing home resident). Specific topic reviews and recommendations are provided with regard to what resources are typically available to evaluate suspected infection, what symptoms and signs suggest infection in a resident of an LTCF, who should initially evaluate the resident with suspected infection, what clinical evaluation should be performed, how LTCF staff can effectively communicate about possible infection with clinicians, and what laboratory tests should be ordered. Finally, a general outline of how a suspected outbreak of a specific infectious disease should be investigated in an LTCF is provided.

Abstract

Background Atrial fibrillation (AF) is a cardiac rhythm disturbance arising from disorganized electrical activity in the atria, and it is accompanied by an irregular and often rapid ventricular response. It is the most common clinically significant dysrhythmia in the general and older population. Types of Studies Reviewed The authors conducted a MEDLINE search using the key terms ‘atrial fibrillation,’ ‘epidemiology,’ ‘pathophysiology,’ ‘treatment’ and ‘dentistry.’ They selected contemporaneous articles published in peer-reviewed journals and gave preference to articles reporting randomized controlled trials. Clinical Implications The anticoagulant warfarin frequently is prescribed to prevent stroke caused by cardiogenic thromboemboli arising from stagnant blood in poorly contracting atria. Most dental procedures and a limited number of surgical procedures can be performed without altering warfarin dosage if the international normalized ratio value is within the therapeutic range of 2.0 to 3.0. Certain analgesic agents, antibiotic agents, antifungal agents and sedative hypnotics, however, should not be prescribed without consultation with the patient's physician because these medications may alter the patient's risk of hemorrhage and stroke. Conclusions AF affects nearly 2.5 million Americans, most of who are older than 60 years. Consultation with the patient's physician to discuss the planned dental treatment often is appropriate, especially for people who frequently have comorbid diseases such as coronary artery disease, congestive heart failure, diabetes and thyrotoxicosis, which are treated with multiple drug regimens.

Abstract

Infectious diseases in general in the aged are associated with higher morbidity and mortality rates. Decremental biologic changes with age affect host defenses and responses to infection, and the frequent presence of comorbidities also may adversely impact host defenses, especially in frail older persons. Infections may present differently in older persons than in younger populations, making early diagnosis difficult. Within this context, the article explores the importance of how fever of unknown origin (FUO) in the old differs significantly from FUO in younger adults because the etiology is different. Moreover, it is important to aggressively determine the etiology of FUO in this older population because it is often treatable.

Abstract

It is well known that one drawback for stress-ribbon bridges can be that significant horizontal reactions at the abutment can be generated and that they have low flexural stiffness. The latter prevents them from being used as roadway bridges. A new hybrid structure of stress-ribbon deck and truss has been proposed and was adopted in the construction of Nozomi Bridge in Japan, a roadway bridge opened to traffic in 2003. Static and dynamic behaviours of the hybrid structure have been studied analytically and experimentally. The results show that the hybrid bridge has advantages over the stress-ribbon deck bridge as it generates much less horizontal force in suspension cables and has higher flexural stiffness, suitable for use as a roadway bridge, and that the hybrid bridge has advantages over the truss bridge since it can be constructed without extensive falsework and without large erection equipment. Abstract

Adults aged 65 and over comprise the fastest growing segment of the U.S. population, and older adults experience greater morbidity and mortality due to infection than young adults. While this factor is well established, most clinical investigation of infectious diseases in the aged focuses on microbiology, and crude endpoints of clinical success such as cure rates or mortality, but often fails to assess functional status, a critical variable in geriatric care. Functional status can be evaluated as a risk factor for infectious disease or an outcome of interest following specific interventions utilizing well-validated instruments. This paper outlines the currently available data suggesting a link between infection, immunity and impaired functional status in the elderly, summarizes commonly employed instruments used to determine specific aspects of functional status, and provides recommendations for a new paradigm in which clinical trials of older adults include functional assessment.

Abstract

Objective. Although DCC has been considered as a candidate tumor suppressor, the roles it plays in the uterine endometrium and in the carcinogenic process remains unclear. To define these roles more clearly, we examined the expression of DCC and its ligand, netrin-1, in the normal endometrium and in endometrial cancer. Methods. The expression of DCC and netrin-1 in normal endometrial glands and in cancer cell lines was examined by RT-PCR and immunohistochemistry. The effects of exogenous DCC and netrin-1 expression were observed together with the respective expression vector transfection. Results. Endometrial glands in the proliferative and early secretory phase expressed both DCC and netrin-1, but glands in the late-secretory phase tended to silence DCC expression. In addition, all of the endometrial cancer cell lines lost normal DCC expression. Restored DCC expression in the cancer cell lines in the absence of netrin-1 induced apoptosis. However, no changes were observed in the presence of netrin-1. Conclusion. Our observations suggest that DCC/netrin-1 signaling may commit cells to the transition of endometrial gland architecture or function from a proliferating to a secretory phase. In addition, the silencing of DCC expression may contribute to the escape of endometrial cancer cells from a DCC-regulated apoptotic program, thereby promoting malignant phenotypes.

Abstract

Stability of a haptic interface is an important issue in virtual reality because an operator directly touches haptic interface devices. Stability is influenced by the sampling period and the discrete-time property of the control system. For decreasing the sampling system influence, this paper proposes a haptic device with an analog circuit which is placed between the computer and the haptic device. The circuit functions as springs and dampers. The control system can specify stiffness, damping coefficients, and their equilibrium. Since the impedance generated by the electric spring and damper can work continuously within the sampling period, it is effective for making the system more stable. We also develop a control method for displaying a static virtual object with the proposed device. Further, we discuss the effects of the proposed approach analytically, using passivity analysis for a 1-degree-of-freedom display system. Finally, some experimental results in a two-dimensional virtual environment are presented.
