We estimate the value of non-personally identifying information to consumers of online services through a discrete choice experiment based on hypothetical streaming video services. Non-personally identifying information for online services is typically information on the ways in and times at which customers use the service, and is distinct from personally identifying information such as email addresses or telephone numbers. For most of our survey respondents, we find no evidence that they were willing to pay to avoid sharing their non-personally identifying information with third parties. A smaller group of respondents never selected a service that shared information with third parties.

The National Association of School Nurses (NASN) developed the Framework for 21st Century School Nursing Practice to reflect current school nurse practice. The Framework of practice was introduced in June 2015, and feedback was requested and obtained from practicing school nurses in a variety of ways. The final version of the Framework is introduced in this article. This article updates (and replaces) the articles in the July 2015 NASN School Nurse related to the Framework. Central to the Framework is student-centered nursing care that occurs within the context of the students’ family and school community. Surrounding the student, family, and school community are the nonhierarchical, overlapping key principles of Care Coordination, Leadership, Quality Improvement, and Community/Public Health.These principles are surrounded by the fifth principle, Standards of Practice, which is foundational for evidence-based and clinically competent quality care. Each of these principles is further defined by practice components. Suggestions are provided regarding how the Framework can be used in a variety of settings to articulate and prioritize school nursing practice. The ultimate goal is to provide a resource to guide school nurses in their practice to help students be healthy, safe, and ready to learn.

A 25% risk of acute esophagitis is experienced by patients as a result of the volume of esophagus encompassed within a conformal radiotherapy technique. We reviewed our institutional experience administering the radioprotectant amifostine prior to daily radiotherapy to determine its effects on the onset of esophagitis. Materials and methods: From 2005 to 2016, 49 patients diagnosed with stage 3 small cell lung carcinoma received concurrent chemoradiotherapy. Chemotherapy (CT) consisted of cisplatin and etoposide with radiotherapy (RT) encompassing CT-identified gross tumor volume. In 32 patients (group 1), amifostine was delivered (500 mg subcutaneously divided in two injections) prior to the second daily RT fraction. The remaining 17 patients (group 2) did not receive amifostine due to choice or drug intolerance. Results: Metrics of esophagitis included weight loss and opiate requirement during treatment. About 31% of group 1 required opiates at a median RT dose of 3300 cGy, and 41% of group 2 required opiates at a median dose of 2250 cGy. The dose of radiotherapy delivered to 50% of the esophageal volume for group 1 was significantly greater than that in group 2 (3000 cGy vs 576 cGy). Conclusion: In this modern retrospective series of thoracic chemoradiotherapy in the treatment of stage 3 small cell lung cancer, amifostine that was delivered subcutaneously postponed the onset of esophagitis. Incontinence is associated with mental illness and neuroleptic medications but diagnosis and treatment is often poor or non-existent. Problems of incontinence are compounded in secure psychiatric services for women by poor health, obesity, and a sedentary lifestyle. Addressing the physical health of this group necessitates a more accurate picture of the nature, incidence, and management of incontinence. A point-in-time survey of 108 women who agreed to be interviewed (93%) covered presence, frequency, and nature of incontinence, and information on management case note data was used to gather demographic and previous medical history, comparisons were made between patients with and without problems of incontinence. Findings indicate a problem of incontinence in 48% of women with a dominance of problems of stress and urge enuresis. Of modifiable factors that contribute to enuresis, the current study highlighted the contribution of obesity, smoking and clozapine medication. A further finding was the preference for managing rather than treating problems of incontinence. Actions to improve the detection and treatment of this problem are described.

The purpose of this project was to determine the effectiveness of the Helping Administer to the Needs of the Student with Diabetes in Schools (H.A.N.D.S.sm) continuing education program in improving the level of experience and competence in performing services associated with diabetes care. This program is a live course for school nurses providing clinical information about diabetes management and their professional role in the care of students with diabetes. Pre- and post-surveys were administered via e-mail to assess their level of experience and competence in diabetes care. A total of 105 nurses completed both surveys and were included in the analysis. The changes between pre- and post-survey questions were assessed. The H.A.N.D.S. participants’ levels of experience and competence for each of the four categories of diabetes care improved significantly, and a greater number of nurses reported being able to perform the services independently and having the ability to teach others.

Over the past 20 years, market-based choice initiatives have become a popular approach to education reform. Since 2002, the New York City Department of Education has opened over 250 high schools, creating a marketplace so widespread that many students no longer have a zoned or neighborhood school. This article uses two New York City–based case studies to examine branding or marketing practices at new small schools. It explores how and to whom schools market themselves and the nature and type of information provided to students and parents. The article raises questions about the implications of marketing and branding practices for public education.

The need to avert unacceptable and irreversible environmental change is the most urgent challenge facing society. Landscape ecology has the capacity to help address these challenges by providing spatially-explicit solutions to landscape sustainability problems. However, despite a large body of research, the real impact of landscape ecology on sustainable landscape management and planning is still limited. In this paper, we first outline a typology of landscape sustainability problems which serves to guide landscape ecologists in the problem-solving process. We then outline a formal problem-solving approach, whereby landscape ecologists can better bring about disciplinary integration, a consideration of multiple landscape functions over long time scales, and a focus on decision making. This framework explicitly considers multiple ecological objectives and socio-economic constraints, the spatial allocation of scarce resources to address these objectives, and the timing of the implementation of management actions. It aims to make explicit the problem-solving objectives, management options and the system understanding required to make sustainable landscape planning decisions. We propose that by adopting a more problem-solving approach, landscape ecologists can make a significant contribution towards realising sustainable future landscapes.

Current diabetes regimens require more effort than ever before. The level of diabetes control students are able to maintain is affected greatly by their ability to care for their diabetes during the school day. This article reviews use of School Health Plans and Diabetes Medical Management Plans in schools. Students with diabetes, their families, health care providers and school personnel all have responsibilities that should be outlined in these plans. School nurses coordinate school-based diabetes care, provide training to school staff members, advocate for students and monitor implementation of students’ school plans. Normal growth and development, prevention of complications and full participation in academic and social opportunities should be possible for students with diabetes. A variety of resources that support students with diabetes are described and referenced.

The use of computed tomography (CT) to evaluate obesity in canines is limited. Traditional CT image analysis is cumbersome and uses prediction equations that require manual calculations. In order to overcome this, our study investigated the use of advanced image analysis software programs to determine body composition in dogs with an application to canine obesity research. Beagles and greyhounds were chosen for their differences in morphology and propensity to obesity. Whole body CT scans with regular intervals were performed on six beagles and six greyhounds that were subjected to a 28-day weight-gain protocol. The CT images obtained at days 0 and 28 were analyzed using software programs OsiriX, ImageJ, and AutoCAT. The CT scanning technique was able to differentiate bone, lean, and fat tissue in dogs and proved sensitive enough to detect increases in both lean and fat during weight gain over a short period. A significant difference in lean : fat ratio was observed between the two breeds on both days 0 and 28 (P<0.01). Therefore, CT and advanced image analysis proved useful in the current study for the estimation of body composition in dogs and has the potential to be used in canine obesity research.

A new device, the ThAIRapy® Bronchial Drainage System, enables patients with cystic fibrosis to self-administer the technique of high frequency chest compression (HFCC) to assist with mucociliary clearance. We review the literature on HFCC and outline a case study of a patient currently using the ThAIRapy® Bronchial Drainage System. While mucociliary clearance and lung function may be enhanced by HFCC therapy, more research is needed to determine its efficacy, cost benefits, and optimum treatment guidelines. Although our initial experience with the patient using this device has been positive, we were unable to accurately evaluate the ThAIRapy® Bronchial Drainage System. © 1995 Wiley-Liss, Inc.

In the school setting, the school nurse facilitates and coordinates the team that implements the nutritional plan for students with diabetes. It is important that the school nurse is familiar with basic principles of medical nutrition therapy, glucose metabolism, carbohydrate counting, and insulin coverage for meals.

A best evidence topic was constructed according to a structured protocol. The question addressed was: In children undergoing umbilical hernia repair is a rectus sheath block (RSB) better than local anaesthetic infiltration of the surgical site, at reducing post-operative pain? From a total of 34 papers, three studies provided the best available evidence on this topic. One randomised clinical trial showed RSB had a better analgesic effect in the immediate post-operative period. In another randomised trial opioid consumption in the peri-operative period was found to be significantly lower in patients administered RSB. These improvements in pain and analgesia consumption need to be balanced against the expertise, training, equipment required, time implications and complications of performing a RSB.
