1. Continuous glucose monitoring of a runner during five marathons

Abstract

Summary Introduction Now in the global running boom, many people enjoy full-marathon, from top athletes to city runners. To understand glucose fluctuations during full-marathon would be useful to discuss the running performance and exercise intensity. There is a report concerning the continuous glucose monitoring (CGM) of three runners with type 1 diabetes during full-marathon, and that of two elite runners during 100 km running race. However, there are little CGM data of healthy subjects during full-marathon as far as we know. The purpose of this study is to observe glucose fluctuation during full-marathon by CGM with running performance and exercise intensity. Summary of facts and results CGM was performed five times during full-marathon of one same healthy runner and the heart rate and running speed were monitored. Five CGM data during full-marathon were obtained. Three of them showed the flat pattern. One of the left two showed the ascending pattern, in which the glucose level went up to more than 140 mg/dL. The other one showed the descending pattern. The best running performance was observed in the flat pattern. Conclusion CGM is a useful tool for monitoring glucose fluctuation during long distance running. These data would be helpful as a pilot study to understand glucose metabolism and performance during endurance exercises.

Abstract

BACKGROUND: TREATMEN The contribution of eosinophilic esophagitis (EoE) to refractory gastroesophageal reflux disease (GERD) remains unknown. When EoE and GERD overlap, the clinical, endoscopic and histological findings are nonspecific and cannot be used to distinguish between the two disorders. Limited data are available on this topic, and the interaction between EoE and GERD is a matter of debate. AIM: We have conducted a prospective study of adult patients with refractory GERD to evaluate the overlap of reflux and EoE. METHODS: Between July 2006 and June 2008, we consecutively and prospectively enrolled 130 male and female patients aged 18 to 70 years old who experienced persistent heartburn and/or regurgitation more than twice a week over the last 30 days while undergoing at least six consecutive weeks of omeprazole treatment (at least 40 mg once a day). The patients underwent an upper digestive endoscopy with esophageal biopsy, and intraepithelial eosinophils were counted after hematoxylin/eosin staining. The diagnosis of EoE was based on the presence of 20 or more eosinophils per high-power field (eo/HPF) in esophageal biopsies. RESULTS: Among the 103 studied patients, 79 (76.7%) were females. The patients had a mean age of 45.5 years and a median age of 47 years. Endoscopy was normal in 83.5% of patients, and erosive esophagitis was found in 12.6%. Only one patient presented lesions suggestive of EoE. Histological examination revealed >;20 eo/HPF in this patient. CONCLUSION: Our results demonstrated a low prevalence of EoE among patients with refractory GERD undergoing omeprazole treatment.

Abstract

A microwave reflectometer is developed for electron density profile measurement in Heliotron J. An amplitude modulation (AM) type system is adopted to reduce density fluctuation effects. The carrier frequency ranges from 33 to 56 GHz, and the modulation frequency is 200 MHz. The X-mode is selected as the propagation mode in order to measure a hollow density profile which is typically observed in ECH plasmas. A test-bench examination using an aluminum reflection plate shows that the measured phase shift agrees well with that expected from the change in the plate position. The initial measurement results show that the reconstructed density profile has hollow profile at low density and steep gradient at the edge in ECH plasma. Abstract

Background It is well-known that FK506 strongly inhibits cytokine production by T cells in vitro. However, less evidence is available from in vivo studies of ocular allergy. Objective To study the anti-inflammatory effect of FK506 eye drops on late and delayed-type responses in several animal models of ocular allergy. Methods Rats and guinea-pigs were sensitized with egg albumin (EA) in adjuvant and later challenged by topical EA application to their eyes to examine the late response. Biopsy specimens of conjunctiva were stained with haematoxylin-eosin or stained for T cells and eosinophils. In addition, rats, rabbits and guinea-pigs were sensitized with complete Freund's adjuvant and later challenged by injecting purified protein derivatives for the delayed-type response. Bulbar conjunctival oedema and hyperaemia were graded by score in rabbits, and Evans blue (EB) extravasation was measured in rats and guinea-pigs. FK506 (0.01-1%) and steroid (0.1%) eye drops were instilled in the eyes of animals several times, before and after challenge. Results FK506 eye drops inhibited T cell and eosinophil infiltration in the late response and EB extravasation in the delayed-type response in rats. Also, they inhibited conjunctival oedema, hyperaemia and ocular mucus in the delayed-type response in rabbits. These effects were similar to those of steroid eye drops (betamethasone sodium phosphate, fluorometholone). FK506 eye drops also inhibited inflammatory cell infiltration, the loss of conjunctival epithelium and decrease of goblet cells in the late response as well as EB extravasation in the delayed-type response in guineapigs, a steroid-resistant species. Conclusion FK506 eye drops inhibit late and delayed-type responses in animal models of ocular allergy.

Abstract

A single-chip 1.25-Gb/s 32:1, 1:32 transceiver, as specified in the emerging American National Standards Institute fiber channel standard, has been developed using 0.25-/spl mu/m CMOS technology. Newly developed features contributing to the achievement of 4.25-Gb/s operations include: 1) an asynchronous tree-type 1:8 demultiplexer, 2) an 8-10 bit parallel-to-parallel frequency conversion circuit, and 3) comma detection and word alignment logic. The transceiver consumes 600 mW in 4.25-Gb/s operations with a 2.5 V supply. This design helps achieve higher speed operations and only half the power dissipation of the fastest previously reported CMOS fiber channel design that includes an 8B10B encoder and a 10B8B decoder.

A BiCMOS FCL circuit using depression mode NMOS transistors is proposed. An FCL circuit operates stably at multi-GHz frequency with 1.5 V power supply using low cost silicon technologies, which has never been achieved by any other circuit techniques. The FCL logic circuits can be implemented with low voltage CMOS on a single chip.

By combining the high transconductance and good matching of bipolar transistors with the circuit flexibility of MOS switched capacitor (SC) circuits, a 1.5 V BiCMOS video ADC shows analog BiCMOS has potentially higher performance than CMOS even at 1.5 V.

Abstract

We have developed a 0.13 /spl mu/m embedded DRAM technology, which targets high-speed and low-voltage operation, with logic performance fully compatible with 0.13 /spl mu/m pure logic. Parasitics (resistance, capacitance) of the DRAM macro limit its performances due to RC delay and IR drop. The parasitics of the new DRAM have been successfully minimized by employing the Full Metal embedded DRAM (FMD) technology which consists of a Metal-Insulator-Metal (MIM) capacitor with metal contact, and a newly developed low resistivity bitline. The results of performance simulation using a 16 Mbit DRAM macro show 1.2 V, 450 MHz of high-performance random access operation.

Abstract

We have developed embedded DRAM technology, in which 0.15 /spl mu/m logic transistor performance is fully compatible with that of pure logic processes. The key technology is the newly developed MIM capacitor element with W-TiN-Ta/sub 2/O/sub 5/-TiN structure. Temperatures as low as 500/spl deg/C are sufficient for the formation process for this MIM capacitor element. Excellent leakage current characteristics of 8/spl times/10/sup -15/ A//spl mu/m/sup 2/ at 125/spl deg/C with T/sub eq/ (equivalent oxide thickness) of 17 /spl Aring/ have been obtained. This technology has been actually implemented into a 4 Mbit test chip with cell size of 0.425 /spl mu/m/sup 2/. Over 50% yield without redundancy was obtained, confirming that there are no basic issues in process integration.

Abstract

Background and Study Aims: Intraoperative changes in circulatory hemodynamics and autonomic nervous activity were evaluated in 33 patients with cholelithiasis who underwent laparoscopic cholecystectomy. Patients and Methods: Of these patients,18 were treated using a pneumoperitoneum (group G) and 15 using the abdominal wall-lifting method (group WL). Their ECG, blood pressure, arterial oxygen saturation, and expiratory carbon dioxide partial pressure were monitored. Autonomic nervous function was evaluated by spectral analysis of the heart rate. Results: Mean blood pressure increased significantly in group G during surgery, but did not vary in group WL during any stage of surgery. The high-frequency (HF) power, an index of parasympathetic activity, decreased significantly in group G after pneumoperitoneum. However, the HF power did not decrease significantly in group WL. The LF/HF ratio, an index of sympathetic activity, increased significantly in group G after pneumoperitoneum, but did not vary in group WL. In addition, the incidence of ventricular or supraventricular arrhythmias and the severity of the arrhythmias as determined by Lown's classification were higher in group G than in group WL. These findings suggest that intraoperative changes in autonomic nervous activity, due to increased intra-abdominal pressure, were smaller in patients undergoing laparoscopic cholecystectomy using the abdominal wall-lifting method than in those undergoing laparoscopic cholecystectomy using pneumoperitoneum. The results also demonstrated that hemodynamic changes were smaller in patients undergoing the abdominal wall-lifting method than in those undergoing pneumoperitoneum. Conclusions: It was concluded that hemodynamics should be carefully monitored during pneumoperitoneum, and that the abdominal wall-lifting approach in laparoscopic cholecystectomy is a method worthy of consideration for elderly patients or those with cardiopulmonary complications.

Abstract

Histamine release and cytokine production by mast cells and basophils are thought to be closely involved in the pathogenesis of allergic diseases. Some reports show that FK506 (tacrolimus hydrate) inhibited histamine release and cytokine production by mast cells and basophils. However, as the effects of FK506 has not been compared with those of clinically used drugs in those reports, the clinical relevancy of FK506 inhibition remained unclear. In this paper, we compared the actions of FK506 with those of steroids or disodium cromoglycate (DSCG) which has been clinically used. FK506 inhibited histamine release by Brown–Norway rat peritoneal mast cells more potently than steroids and especially DSCG. FK506 also inhibited histamine release by a mast rat basophilic leukemia (RBL)-1 cell line and human peripheral blood basophils, whereas steroids failed to inhibit histamine release by human basophils. FK506 as well as steroids inhibited TNF-α and IL-4 production by RBL-1 cells. FK506 was therefore more effective than steroids and DSCG in inhibiting histamine release, and it also had the ability of inhibiting cytokine production by mast cells as steroids do. We concluded that FK506 might regulate allergic diseases via these actions, judging from the viewpoint of clinical relevancy.

Abstract

We describe the construction and performance of a scintillating fiber detector used in the near detector for the K2K (KEK to Kamioka, KEK E362) long baseline neutrino oscillation experiment. The detector uses 3.7 m long and 0.692 mm diameter scintillating fiber coupled to image-intensifier tubes (IIT), and a CCD camera readout system. Fiber sheet production and detector construction began in 1997, and the detector was commissioned in March, 1999. Results from the first K2K runs confirm good initial performance : position resolution is estimated to be about 0.8 mm, and track finding efficiency is $98 \pm 2$ % for long tracks (i.e., those which intersect more than 5 fiber planes). The hit efficiency was estimated to be $92 \pm 2$ % using cosmic-ray muons, after noise reduction at the offline stage. The possibility of using the detector for particle identification is also discussed., Comment: In total 21 pages with 18 figures. Abstract

SRPX2 (Sushi repeat-containing protein, X-linked 2) has recently emerged as a multifunctional protein that is involved in seizure disorders, angiogenesis and cellular adhesion. Here, we analyzed this protein biochemically. SRPX2 protein was secreted with a highly post-translational modification. Chondroitinase ABC treatment completely decreased the molecular mass of purified SRPX2 protein to its predicted size, whereas heparitinase, keratanase and hyaluroinidase did not. Secreted SRPX2 protein was also detected using an anti-chondroitin sulfate antibody. These results indicate that SRPX2 is a novel chondroitin sulfate proteoglycan (CSPG). Furthermore, a binding assay revealed that hepatocyte growth factor dose-dependently binds to SRPX2 protein, and a ligand–glycosaminoglycans interaction was speculated to be likely in proteoglycans. Regarding its molecular architecture, SRPX2 has sushi repeat modules similar to four other CSPGs/lecticans; however, the molecular architecture of SRPX2 seems to be quite different from that of the lecticans. Taken together, we found that SRPX2 is a novel CSPG that is overexpressed in gastrointestinal cancer cells. Our findings provide key glycobiological insight into SRPX2 in cancer cells and demonstrate that SRPX2 is a new member of the cancer-related proteoglycan family.

Abstract

Introduction: Refractory GERD describes those patients who continue to have symptoms despite treatment with proton-pump inhibitors (PPI)1. The participation of eosinophilic esophagitis (EE) in the refractory GERD has been scarcely studied. When both disorders are simultaneously present, the clinical, endoscopic and histologic findings may be nonspecific and as such the overlap of refractory GERD and EE has been matter of debate. Aim: To study the occurrence of EE in patients with refractory GERD. Methods: Upper digestive endoscopies (UDE) were performed, obtaining mucosal biopsies of distal, middle and lower esophagus and stomach (body and antrum) in 97 adult consecutive patients. Erosive esophagitis was classified according to Los Angeles classification. Seventy three (75.3%) were female, with mean age of 44,4 (+ 9.09) of years. Eighty seven patients (89.6%) were submitted to esophageal manometry and pHmetry. The patients (82, 84%) have been receiving high doses of PPI (40 mg omeprazol b.i.d.) for at least 6 weeks. The main complaint was heartburn and regurgitation. The diagnosis of GERD was based on UDE and pHmetry. The diagnosis of EE was based on the presence of ≥20 eosinophils per high power field (eo/hpf) in the esophageal biopsies. Results: UDE was normal in 78 patients (81%). Erosive esophagitis (grade A in 9, B in 2 and C in 2 of LA) was found in 13 (13%) patients. All the others 6 (6%) were classified as non-erosive esophagitis (NERD). One patient ( 1 %) presented > 20 eo/hpf. In 25 (27.8%) patients the pHmetry was altered during PPI treatment (80 mg/day). The manometry showed hypomotility before the treatment for EE and, inespecific motor disturbances after the decreasing of eosinophils post the appropriate treatment. Abstract

A conversational data access method is proposed in order to avoid the complexity of procedures for data access of CRT operator consoles. The procedure proposed here is composed of three hierarchical steps (tree structural steps), related to the systems of plant components and operational modes. It is concluded from the simulation experiment that the procedure proposed here is superior in many respects to that of the long code entry method.

Abstract

An information display system for start-up and shut-down operation of a commercial BWR power plant has been designed for application to a prototype CRT-based operators console having three CRTs. Problems in designing the display system include determining the process variables to be displayed for each period of the plant operation, and selecting the display formats. Variables to be displayed have been chosen on the basis of work analysis of the plant operation, in the form of an operational sequence diagram. The variables have been assigned to nineteen display formats, after consideration of accessibility requirements and the number of times display pictures must be exchanged. Design criteria of the display formats have also been derived from the work analysis, by observing the operator's manner of monitoring process variables for each operational period. The information display system designed has been evaluated by simulational tests. It has been concluded that the nineteen display formats are sufficient for operators to start-up and shut-down a 460MWe class BWR power plant.

Abstract

Up to now, surgery was the only possible treatment for choledochoduodenal fistulas, which are seen more often in Japan than in Europe. This paper presents the value and effect of endoscopic fistulotomy (EFT) as an alternative treatment in those cases. Parapapillary choledochoduodenal fistulas are abserved usually on the longitudinal fold of the papilla or on its oral side. The papillotome is inserted into the common bile duct through the orifice of the duodenal papilla, then the wall between its orifice and the fistula is cut to open widely the distal portion of the choledochus. EFT was performed successfully in 7 cases. The procedure led to a wide open stoma of the distal common bile duct with free bile outflow. Residual stones, a common occurrence in cases of parapapillary choledochoduodenal fistulas, may pass spontaneously after EFT or can be removed with a basket catheter. In cases of parapapillary choledochoduodenal fistula, EFT is a reliable method, especially in high-risk patients, and an alternative to surgical treatment.

Abstract

The effort consisted of determining the procedures for vertical climb and yaw maneuverability tests and recommendeding the compliance test procedure. The flight testing was primarily conducted with an AH-1G helicopter at Edwards Air Force Base and the Naval Weapons Center at China Lake, California from November 1970 through September 1971. The test results show that both vertical climb testing and yaw maneuverability testing involve limiting factors which result in uncertainty in the data. A test procedure was developed to demonstrate the yaw maneuverability and calculate the vertical climb capability, utilizing tethered hover test techniques to reduce the data uncertainty. This recommend procedure provided results which were in good agreement with the flight test results and should be adequate for determination of the hot day hover performance margin.

Abstract

Airspeed calibration tests were conducted on an experimental model of the Elliott low-airspeed system to determine its suitability for use as a helicopter airspeed instrument. The airspeed system was mounted in various locations on the UH-1C helicopter. Emphasis was placed on the low-speed flight regimes where the sensor operated in rotor downwash. The evaluation required 13.7 productive test flight hours. The system provides reliable, accurate airspeed data from hover to 120 knots in the direction for which the sensor is mounted, and results indicate an omnidirectional system is feasible. The system is simple, is highly reliable, should be relatively inexpensive in production quantities, and has high potential for development into a standard aircraft instrument.

Abstract

We have demonstrated three key integration technologies of thermally stable dual-gate CMOSFETs for DRAM-embedded ASICs. These technologies include: (1) a thermally stable W-polycide gate for every MOSFET and CoSi/sub 2/ diffusion for logic CMOS to maintain low resistance, (2) nitrogen implantation into WSi/sub 2/ to prevent lateral dopant diffusion without gate depletion, and (3) a Si/sub 3/N/sub 4//TEOS-BPSG stacked interlayer for self-aligned contacts (SAC) without boron penetration in PMOSFETs. High-performance CMOSFETs using these technologies and 5 metal layers result in a flexible circuit design which can achieve 6.8 ns access speed in a 64 Mb DRAM-embedded macro with a 0.25 /spl mu/m design rule.

A series of passive fiber-optic devices, which includes a new type multiplexer–demultiplexer for WDM transmission, has been developed.
