1. First-in-Human Trial of Epichaperome-Targeted PET in Patients with Cancer

Abstract

Purpose: 124I-PU-H71 is an investigational first-in-class radiologic agent specific for imaging tumor epichaperome formations. The intracellular epichaperome forms under cellular stress and is a clinically validated oncotherapeutic target. We conducted a first-in-human study of microdose 124I-PU-H71 for PET to study in vivo biodistribution, pharmacokinetics, metabolism, and safety; and the feasibility of epichaperome-targeted tumor imaging. Experimental Design: Adult patients with cancer (n = 30) received 124I-PU-H71 tracer (201±12 MBq, Results: 124I-PU-H71 PET detected tumors of different cancer types (breast, lymphoma, neuroblastoma, genitourinary, gynecologic, sarcoma, and pancreas). 124I-PU-H71 was retained by tumors for several days while it cleared rapidly from bones, healthy soft tissues, and blood. Radiation dosimetry is favorable and patients suffered no adverse effects. Conclusions: Our first-in-human results demonstrate the safety and feasibility of noninvasive in vivo detection of tumor epichaperomes using 124I-PU-H71 PET, supporting clinical development of PU-H71 and other epichaperome-targeted therapeutics.

Abstract

Patients with type-2 diabetes mellitus (T2DM) have normal or increased bone mineral density (BMD) but despite that, they are characterized by an increased hip and vertebral fracture risk that involves the alteration of bone quality and not the reduction in bone mass. BMD is utilized for the diagnosis and evaluation of osteoporosis, but BMD itself cannot provide an accurate diagnosis of the individuals at increased risk of fracture and, therefore, studies have focused on identifying other risk factors that are partially or fully independent of BMD. The fracture risk score tool-FRAX® models provide information about a 10-year probability of osteoporotic fractures, but do not include risk factors specific to illness such as diabetes duration, diabetes drug therapy, glycemic control, or the presence of micro-vascular complications. Multiple markers have been investigated to provide information on the risk of fractures in patients with T2DM such as: advanced glycation end products (AGEs), insulin-like growth factor-I (IGF-I), osteocalcin (OC), adiponectin, and sclerostin, but epidemiological studies did not provide homogeneous information regarding the link between these markers and bone fragility in T2DM subjects. Markers that increase the accuracy of fracture risk estimation in patients with T2DM need to be identified and employed in current medical practice.

Abstract

Diseases are a manifestation of how thousands of proteins interact. In several diseases, such as cancer and Alzheimer’s disease, proteome-wide disturbances in protein-protein interactions are caused by alterations to chaperome scaffolds termed epichaperomes. Epichaperome-directed chemical probes may be useful for detecting and reversing defective chaperomes. Here we provide structural, biochemical, and functional insights into the discovery of epichaperome probes, with a focus on their use in central nervous system diseases. We demonstrate on-target activity and kinetic selectivity of a radiolabeled epichaperome probe in both cells and mice, together with a proof-of-principle in human patients in an exploratory single group assignment diagnostic study (ClinicalTrials.gov Identifier: NCT03371420). The clinical study is designed to determine the pharmacokinetic parameters and the incidence of adverse events in patients receiving a single microdose of the radiolabeled probe administered by intravenous injection. In sum, we introduce a discovery platform for brain-directed chemical probes that specifically modulate epichaperomes and provide proof-of-principle applications in their use in the detection, quantification, and modulation of the target in complex biological systems., Here, the authors show structural, biochemical, and functional insights into the discovery of epichaperome‐ directed chemical probes for use in central nervous system diseases. Probes emerging from this work have translated to human clinical studies in Alzheimer’s disease and cancer.

Abstract

Haematological and biochemical parameters have not yet been analysed in professional padel players. The aim of this study was to determine the basal values of these parameters and to observe the effect of a simulated competition on them, including gender-related differences. A total of 14 male professional players (age: 28.2 ± 7.9 years), and 16 female professional players (age: 29.7 ± 3.7 years) participated in this study. Players were allowed to hydrate ad libitum during the matches. Haematological and biochemical values were obtained before and after a simulated competitive padel match. The men’s group showed higher baseline values in red blood cells, haematocrit, haemoglobin, urea, creatinine, uric acid, albumin, glutamic oxaloacetic transaminase (GOT), glutamic-pyruvic transaminase (GPT), lactate dehydrogenase (LDH) and creatine kinase (CK) (p < 0.01) than the women’s group. Attending to match effect, significant differences were obtained in urea, creatinine, CK and glucose (p < 0.05). Finally, the group x match interaction revealed significant differences in serum concentrations of sodium and chloride (p < 0.05). In conclusion, high-level padel matches provoke several changes in biochemical parameters related to muscle damage and protein catabolism. Recovery and fluid intake strategies could be added regarding gender. The results obtained could be due to the differences in the intensity and volume of the simulated competition.

Abstract

The recovery of the motor deficit caused by neurapraxia is indispensably linked to the removal of the compression on the nerve and to a recovery strategy based on the facilitation of a motor response. The study was carried out on a 32-year-old patient with comminuted fracture of the lower third of the left femur, for which she had a surgical intervention, the fracture being stabilized using a metal plate and screws. The programme was made up of: stimulating massage, facilitation techniques in an open kinetic chain, then in a closed kinetic chain using the gym ball, the medicinal ball and the cane, exercises for the re-education of sensitivity, exercises using different objects, exercises using the wall ladder, pedalling on the ergonomic bike, etc. The results achieved confirm the hypothesis formulated in the beginning; thus, the recovery of the sensory-motor deficit was achieved in 3 months, compared to the 6-9-month period normally estimated, thus halving the recovery period.

Abstract

Problem statement: Physical exercise is considered one of the main strategies in management of Fibromyalgia (FM); While many studies have shown relevant improvements in the patients’ physical condition and psychological status, the efficacy of these treatments have not yet been completely determined and whether or not these improvements are related with symptom severity is still debated. The aim of this study was to evaluate the positive effect of exercise training on physical fitness and second to determine whether any change in aerobic capacity, strengthening or flexibility after exercise training is associated with improvements in symptoms severity of patients with FM. Approach: Thirty women with FM were randomized to either combined exercise group (CE, n = 15) which performed twice-weekly sessions of combined aerobic and resistance exercises, or usual care control group (CG, n = 15). Participants had to complete a Visual Analogue Scale (VAS) for stiffness, sleep disturbances, pain, anxiety and depression. Physical fitness (aerobic capacity, handgrip strength and flexibility) was also assessed at baseline and immediately after 12-week intervention. Results: Aerobic capacity and Range Of Motion (ROM) in both shoulders and in the right hip were significantly greater than controls. Inter-group differences were significant in anxiety and depression. Linear regressions of physical fitness variables and symptomatology showed positive correlations. Conclusions: Physical fitness seems to be related with the improvements in symptom´ severity and due to regular physical activity has demonstrated to be beneficial for individuals with FM it should be included in treatment plans for this patients group.

Abstract

It is not well known how local fatigue can impair the contractile properties of skeletal muscle, so the aim of this study was to determine the Tensiomyographic (TMG) characteristics of Rectus Femoris (RF) after a single bout of intense exercise. Approach: Twelve healthy male volunteers (mean±SEM; age, 24.16±0.62 years, height 172.02±1.21 cm, body mass 71.84±2.19 kg, BMI 24.42±0.70 kg m-1) were recruited. Results: In session 1, a graded exercise test on cycle ergometer was performed in order to determine subject’s maximal oxygen uptake (VO2max) and power at this key point (pVO2max). In a second testing session, TMG rest values were assessed and then all subjects carried out 2 min of cycling at pVO2max after which TMG analysis was applied again. The results showed subject’s VO2max values of 53.44±2.67 ml kg-1 min-1, reaching a mean pVO2max of 270.83±8.61 W. Maximal radial displacement (Dm) and time from the onset of electrical stimulus to 10% of Dm (Td) were significantly lower at post-test (from 7.577.57±0.92-4.37±0.67 mm; p = 0.01, for Dm and from 25.25±0.57-22.44±0.89 ms; p≤0.05, for Td). Conclusion/Recommendations: It can be concluded that a brief and intense exercise consisting in 2 min of cycling at PVO2max provokes a reduction in contractile properties of RF considering TMG parameters such as Dm and Td. More studies are needed to clarify the role of muscle fatigue on temporal TMG parameters.

11. Exercise Participation Motives and Engaging In Sports Activity among University of Ljubljana Students

Abstract

AIM: The main aim of this study was to examine differences in sport participation motives, the frequency of engaging in sports activities according to gender, region and field of study, but also the association between the incidence of engaging in sports activity and the motivation for sports activity of students at the University of Ljubljana.MATERIAL AND METHODS: Five thousand two hundred seventy-one students completed The Exercise Motivations Inventory (EMI-2), with additional questions about 12 socio-demographic parameters.RESULTS: The results reveal that most of the students are engaged in unorganized sports activities. Male students engage in sports activity more often than female students do. For male students, dominant participation motives are enjoyment, challenge, social recognition, affiliation, competition and strength but also endurance, for female students these are: stress and weight management, revitalisation, ill-health avoidance, positive health, appearance and nimbleness. Gender differences in participation motives are partly reflected also in differences according to the field of study. The correlations between the frequency of engaging in sports activity and the participation motives are mainly statistically significant. We did not find any significant differences in participation motives by region.CONCLUSION: In spite of these discouraging findings, increasing physical activity among students continues to be a national priority.m

Abstract

Heat shock protein 90 (HSP90) serves as a critical facilitator for oncogene addiction. There has been augmenting enthusiasm in pursuing HSP90 as an anticancer strategy. In fact, since the initial serendipitous discovery that geldanamycin (GM) inhibits HSP90, the field has rapidly moved from proof-of-concept clinical studies with GM derivatives to novel second-generation inhibitors.The authors highlight the current status of the second-generation HSP90 inhibitors in clinical development. Herein, the authors note the lessons learned from the completed clinical trials of first- and second-generation inhibitors and describe various assays attempting to serve for a more rational implementation of these agents to cancer treatment. Finally, the authors discuss the future perspectives for this promising class of agents.The knowledge gained thus far provides perhaps only a glimpse at the potential of HSP90 for which there is still much work to be done. Lessons from the clinical trials suggest that HSP90 therapy would advance at a faster pace if patient selection and tumor pharmacokinetics of these drugs were better understood and applied to their clinical development. It is also evident that combining HSP90 inhibitors with other potent anticancer therapies holds great promise not only due to synergistic antitumor activity but also due to the potential of prolonging or preventing the development of drug resistance.

Abstract

Heat shock protein 90 (Hsp90) is a molecular chaperone exploited by cancer cells to aid the function of numerous oncoproteins. The recognition of Hsp90 as a critical facilitator for oncogene addiction and survival of the cancer cell has opened a promising new niche for cancer treatment. The serendipitous discovery that the broad spectrum anticancer activity of the natural products geldanamycin (GM) and radicicol (RD) was a result of inhibition of Hsp90 resulted in the development of improved derivatives of these natural products. One of these was 17 allylaminogeldanamycin (17-AAG), a closely related analog of GM, and was in fact the first Hsp90 inhibitor to enter the clinic. However, GM and its analogs suffer from poor “drug-like” properties and this served as a strong impetus for the development of novel synthetic Hsp90 inhibitors. These efforts resulted in the development of numerous potent synthetic small molecule inhibitors with significant scaffold diversity as well as superior pharmacokinetic and toxicity profile to have entered clinical trials. This review highlights the drug discovery efforts pertaining to the development of the first and second-generation Hsp90 inhibitors, and also gleans over their individual promise as clinical agents for anticancer therapy.
