Abstract

Clinical trials are slow and costly, built around the research centers that study local participants. Building clinical trials around patients in their homes and community through remote visits and monitoring could enhance recruitment and increase convenience for participants. This study evaluated different trial settings, a decentralized arm via telemedicine center (virtual study conduct), a conventional arm via health clinic (onsite study conduct) and a mixed model arm. Acute low-back pain patients (20–65 years) were recruited to this non-interventional trial in Switzerland. The study consisted of a screening period and a 2-week data collection period using direct data capture (eSource), electronic informed consent form (eICF), electronic diary (eDiary) and wearable actigraphy sensor.A higher number of patients were enrolled in the decentralized arm (N = 18) compared to the conventional arm (N = 5) and none in the mixed model arm. The decentralized arm consisted of a diverse population with increased participation from rural areas. In the decentralized arm 89% of enrolled patients completed the study compared to 60% in the conventional arm. All the patients reported satisfaction with the use of eICF, eDiary and remote visits; whereas patients reported a lower level of satisfaction with the wearable sensor.The decentralized setting was operationally feasible and well accepted by patients. Faster recruitment and improved access to patients was observed in the decentralized arm. This study supports broader adoption of the decentralized model in clinical trials, though further investigations in larger interventional trials are needed to confirm the benefits from this patient-centric approach. Abstract

After 3.5 years of cruise, and about 3 months in the vicinity of its target, the MASCOT lander was deployed successfully on October 3, 2018 by the Hayabusa2 spacecraft onto the C-type near-Earth asteroid (162173) Ryugu. After a free-fall of 5 ​min 51 ​s from an altitude of 41 ​m MASCOT experienced its first contact with the asteroid hitting a big boulder. The lander bounced for ~11 ​min 3 ​s before it came to rest. MASCOT was able to perform science measurements with its payload suite at 3 different locations on the surface of Ryugu. It investigated the fine-scale structure, multispectral reflectance, thermal characteristics and magnetic properties. The surface consists of very rugged terrain littered with large surface boulders. The in-situ measurements confirmed the absence of fine particles and dust as already implied by the remote sensing instruments aboard the Hayabusa2 spacecraft. After about 17 ​h of operations, the MASCOT mission terminated with the last communication contact as its primary batteries depleted. This paper summarizes the MASCOT mission covering its four years of in-flight operations, its preparation for the descent, landing and in-situ investigation on the asteroid Ryugu until the end of its operation.

Abstract

20 years after the successful ground deployment test of a (20 m)2 solar sail at DLR Cologne, and in the light of the upcoming U.S. NEAscout mission, we provide an overview of the progress made since in our mission and hardware design studies as well as the hardware built in the course of our solar sail technology development. We outline the most likely and most efficient routes to develop solar sails for useful missions in science and applications, based on our developed ‘now-term’ and near-term hardware as well as the many practical and managerial lessons learned from the DLR-ESTEC Gossamer Roadmap. Mission types directly applicable to planetary defense include single and Multiple NEA Rendezvous ((M)NR) for precursor, monitoring and follow-up scenarios as well as sail-propelled head-on retrograde kinetic impactors (RKI) for mitigation. Other mission types such as the Displaced L1 (DL1) space weather advance warning and monitoring or Solar Polar Orbiter (SPO) types demonstrate the capability of near-term solar sails to achieve asteroid rendezvous in any kind of orbit, from Earth-coorbital to extremely inclined and even retrograde orbits. Some of these mission types such as SPO, (M)NR and RKI include separable payloads. For one-way access to the asteroid surface, nanolanders like MASCOT are an ideal match for solar sails in micro-spacecraft format, i.e. in launch configurations compatible with ESPA and ASAP secondary payload platforms. Larger landers similar to the JAXA-DLR study of a Jupiter Trojan asteroid lander for the OKEANOS mission can shuttle from the sail to the asteroids visited and enable multiple NEA sample-return missions. The high impact velocities and re-try capability achieved by the RKI mission type on a final orbit identical to the target asteroid's but retrograde to its motion enables small spacecraft size impactors to carry sufficient kinetic energy for deflection.

Abstract

After its release and a descent and bouncing phase, the Hayabusa2 lander MASCOT came to a final rest and MASCOT’s camera MASCam acquired a set of images of the surface of Ryugu. With MASCam’s instantaneous field of view of about 1 mrad, the images provide pixel scales from 0.2 to 0.5 mm pixel−1in the foreground and up to 1 cm pixel−1for surface parts in the background. Using a stereo-photogrammetric analysis of the MASCam images taken from slightly different positions due to commanded and unintentional movements of the MASCOT lander, we were able to determine the orientation for the different measurement positions. Furthermore, we derived a 3D surface model of MASCOT’s vicinity. Although the conditions for 3D stereo processing were poor due to very small stereo angles, the derived 3D model has about 0.5 cm accuracy in the foreground at 20 cm distance and about 1.5 cm at a distance of 40–50 cm.

Abstract

Any effort which intends to physically interact with specific asteroids requires understanding at least of the composition and multi-scale structure of the surface layers, sometimes also of the interior. Therefore, it is necessary first to characterize each target object sufficiently by a precursor mission to design the mission which then interacts with the object. In small solar system body (SSSB) science missions, this trend towards landing and sample-return missions is most apparent. It also has led to much interest in MASCOT-like landing modules and instrument carriers. They integrate at the instrument level to their mothership and by their size are compatible even with small interplanetary missions.\ud \ud The DLR-ESTEC Gossamer Roadmap NEA Science Working Groups‘ studies identified Multiple NEA Rendezvous (MNR) as one of the space science missions only feasible with solar sail propulsion. Parallel studies of Solar Polar Orbiter (SPO) and Displaced L1 (DL1) space weather early warning missions studies outlined very lightweight sailcraft and the use of separable payload modules for operations close to Earth as well as the ability to access any inclination and a wide range of heliocentric distances.\ud \ud These and many other studies outline the unique capability of solar sails to provide access to all SSSB, at least within the orbit of Jupiter. Since the original MNR study, significant progress has been made to explore the performance envelope of near-term solar sails for multiple NEA rendezvous.\ud \ud However, although it is comparatively easy for solar sails to reach and rendezvous with objects in any inclination and in the complete range of semi-major axis and eccentricity relevant to NEOs and PHOs, it remains notoriously difficult for sailcraft to interact physically with a SSSB target object as e.g. the Hayabusa missions do.\ud \ud The German Aerospace Center, DLR, recently brought the Gossamer solar sail deployment technology to qualification status in the Gossamer-1 project. Development of closely related technologies is continued for very large deployable membrane-based photovoltaic arrays in the GoSolAr project.\ud \ud We expand the philosophy of the Gossamer solar sail concept of efficient multiple sub-spacecraft integration to also include landers for one-way in-situ investigations and sample-return missions. These are equally useful for planetary defence scenarios, SSSB science and NEO utilization. We outline the technological concept used to complete such missions and the synergetic integration and operation of sail and lander.\ud \ud We similarly extend the philosophy of MASCOT and use its characteristic features as well as the concept of Constraints-Driven Engineering for a wider range of operations.

Abstract

Images from the Optical Navigation Camera system (ONC) onboard the Hayabusa2 spacecraft show the MASCOT lander during its descent to the surface of asteroid (162173) Ryugu. We used results from a previous stereo-photogrammetric analysis that provided precise ONC image orientation data (camera position and pointing), ONC orthoimages, and an ONC-based 3D surface model to combine them with the visibilities of MASCOT itself and its shadow on-ground within the ONC images. We integrated additional information from instruments onboard MASCOT (MASMag, MARA, MASCam) and derived MASCOT’s release position and modeled its free-fall descent path and its velocity over 350 s from its release at ∼41 m altitude above ground until its first contact with the surface of Ryugu. After first contact, MASCOT bounced over the surface of Ryugu for 663 s and came to rest at its first settlement point after four intermediate surface contacts. We again used ONC images that show MASCOT and partly its shadow and reconstructed the bouncing path and the respective velocities of MASCOT. The achieved accuracy for the entire descent and bouncing path is ∼0.1 m (1σ).

Abstract

MASCOT (‘Mobile Asteroid Surface Scout’) is a 10 kg mobile surface science package on board JAXA’s Hayabusa2 sample return mission, currently on its way to the near-Earth asteroid (162173) Ryugu. The mission was launched in December 2014 from Tanegashima Space Center, Japan. The Hayabusa2 spacecraft will reach the target asteroid in summer 2018. Hayabusa2 will return its samples to Earth in December 2020. After arrival at the target asteroid ‘Ryugu’ a detailed mapping phase will be performed and the landing site of MASCOT will be selected. The deployment of MASCOT to the asteroids surface is planned for the beginning of October 2018. MASCOT has been developed by the German Aerospace Center (DLR) in cooperation with the Centre National d’Etudes Spatiales (CNES). The main objective of MASCOT is to perform in-situ investigations of the asteroid surface and to support the sampling site selection for the mother spacecraft. Mascot is equipped with four scientific instruments a wide angle camera, a hyperspectral infrared microscope, a radiometer and a magnetometer. The camera (MasCam) provides ground truth for the orbital measurements of the Hayabusa2 orbiter instruments and the in-situ MASCOT sensor suite as well as geological context of the samples. The radiometer (MARA) determines the surface brightness temperature, the thermal inertia of the surface material and the spectral slope in infrared. The radiometer field of view is correlated with the wide angle camera field of view. The magnetometer (MasMAG) observes the magnetic field profile during descent and bouncing and determines any global and local magnetization of the asteroid.

Abstract

JANUS (Jovis Amorum Ac Natorum Undique Scrutator) is a high-resolution visible camera designed for the ESA space mission JUICE (Jupiter Icy moons Explorer). The main scientific goal of JANUS is to observe the surface of the Jupiter satellites Ganymede and Europa in order to characterize their physical and geological properties. During the design phases, we have proposed two possible optical configurations: a Three Mirror Anastigmat (TMA) and a Ritchey-Chrétien (RC) both matching the performance requirements. Here we describe the two optical solutions and compare their performance both in terms of achieved optical quality, sensitivity to misalignment and stray light performances.

Sphingosine-1-phosphate receptor 1 (S1P1 ) modulators sequester circulating lymphocytes within lymph nodes, thereby preventing potentially pathogenic autoimmune cells from exiting into the blood stream and reaching inflamed tissues. S1P1 receptor modulation may thus offer potential to treat various autoimmune diseases. The first nonselective S1P1-5 receptor modulator FTY720/fingolimod/Gilenya® has successfully demonstrated clinical efficacy in relapsing forms of multiple sclerosis. However, cardiovascular, hepatic, and respiratory side-effects were reported and there is a need for novel S1P1 receptor modulators with better safety profiles. Here, we describe the discovery of cenerimod, a novel, potent and selective S1P1 receptor modulator with unique S1P1 receptor signaling properties and absence of broncho- and vasoconstrictor effects ex vivo and in vivo. Cenerimod dose-dependently lowered circulating lymphocyte counts in rats and mice after oral administration and effectively attenuated disease parameters in a mouse experimental autoimmune encephalitis (EAE) model. Cenerimod has potential as novel therapy with improved safety profile for autoimmune diseases with high unmet medical need.

Abstract

The JUICE (JUpiter ICy moons Explorer) satellite of the European Space Agency (ESA) is dedicated to the detailed study of Jupiter and its moons. Among the whole instrument suite, JANUS (Jovis, Amorum ac Natorum Undique Scrutator) is the camera system of JUICE designed for imaging at visible wavelengths. It will conduct an in-depth study of Ganymede, Callisto and Europa, and explore most of the Jovian system and Jupiter itself, performing, in the case of Ganymede, a global mapping of the satellite with a resolution of 400 m/px. The optical design chosen to meet the scientific goals of JANUS is a three mirror anastigmatic system in an off-axis configuration. To ensure that the achieved contrast is high enough to observe the features on the surface of the satellites, we also performed a preliminary stray light analysis of the telescope. We provide here a short description of the optical design and we present the procedure adopted to evaluate the stray-light expected during the mapping phase of the surface of Ganymede. We also use the results obtained from the first run of simulations to optimize the baffle design.

Abstract

Background—Interleukin (IL)-6 regulates various aspects of the immune response. In the context of heart diseases, it has been recognized as a prognostic factor for dilated cardiomyopathy, which often results from myocarditis.Methods and Results—Using IL-6–deficient mice, we studied the role of IL-6 in a model of autoimmune myocarditis resulting from immunization with a peptide derived from cardiac α-myosin. Prevalence and severity of myocarditis were markedly reduced in the absence of IL-6. CD4+T cells from immunized IL-6–deficient mice proliferated poorly on restimulation with specific antigen in vitro and did not mediate disease on adoptive transfer into IL-6–competent RAG-2–deficient mice, which otherwise lack B cells and T cells. Production of complement C3, a crucial factor for the development of myocarditis, was strongly upregulated in IL-6+/+but not in IL-6–deficient mice after immunization.Conclusions—Our results demonstrate that IL-6 is required for the expansion of autoimmune CD4+T cells and the pathogenesis of autoimmune myocarditis, possibly by upregulation of complement C3.

Abstract

Background: Fixed appliance therapy often extends over several years. Debonding is warmly welcomed and is often seen by the patient as the end of treatment. Yet both patients and parents often underestimate the importance of the subsequent retention period and the speed at which negligence in this treatment phase results in relapse. Bonded retainers guarantee excellent longterm stability at least while they are in situ. The reliable attachment of lingual retainers with modern bonding techniques has led to widespread application of this retention method. The present study investigated its influence on tooth mobility and on the damping properties of the periodontal tissue, by means of a dynamic measuring method (Periotest). Patients and Method: For this purpose two groups with mandibular bonded retainers and one control group were formed. The control group wore removable retention appliances. In all groups, active treatment with fixed appliances had been completed at least half a year before baseline. Results: The results showed that bonded retainers had a negative impact on the damping properties of the periodontal tissue and thus in the broader sense on tooth mobility. Tooth mobility decreased with the number of teeth to which the retainer was bonded but remained, as in the control group, within the physiologic range.

Abstract

The present OSS mission continues a long and bright tradition by associating the communities of fundamental physics and planetary sciences in a single mission with ambitious goals in both domains. OSS is an M-class mission to explore the Neptune system almost half a century after flyby of the Voyager 2 spacecraft. Several discoveries were made by Voyager 2, including the Great Dark Spot (which has now disappeared) and Triton's geysers. Voyager 2 revealed the dynamics of Neptune's atmosphere and found four rings and evidence of ring arcs above Neptune. Benefiting from a greatly improved instrumentation, it will result in a striking advance in the study of the farthest planet of the Solar System. Furthermore, OSS will provide a unique opportunity to visit a selected Kuiper Belt object subsequent to the passage of the Neptunian system. It will consolidate the hypothesis of the origin of Triton as a KBO captured by Neptune, and improve our knowledge on the formation of the Solar system. The probe will embark instruments allowing precise tracking of the probe during cruise. It allows to perform the best controlled experiment for testing, in deep space, the General Relativity, on which is based all the models of Solar system formation. OSS is proposed as an international cooperation between ESA and NASA, giving the capability for ESA to launch an M-class mission towards the farthest planet of the Solar system, and to a Kuiper Belt object. The proposed mission profile would allow to deliver a 500 kg class spacecraft. Abstract

Epstein-Barr virus (EBV)-associated B-cell post-transplantation lymphoproliferative disorder (PTLD) is a severe complication following stem cell transplantation. This is believed to occur as a result of iatrogenic immunosuppression leading to a relaxation of T-cell control of EBV infection and thus allowing viral reactivation and proliferation of EBV-infected B-lymphocytes. In support of this notion, reduction of immunosuppressive therapy may lead to regression of PTLD.We present a case of an 18-year-old male developing a monomorphic B-cell PTLD 2 months after receiving an allogenic stem cell transplant for acute lymphoblastic leukemia. Reduction of immunosuppressive therapy led to regression of lymphadenopathy. Nevertheless, the patient died 3 months afterwards due to extensive graft-vs.-host-disease and sepsis. As a diagnostic lymph node biopsy was performed only after reduction of immunosuppressive therapy, we are able to study the histopathological changes characterizing PTLD regression. We observed extensive apoptosis of blast cells, accompanied by an abundant infiltrate comprising predominantly CD8-positive, Granzyme B-positive T-cells. This observation supports the idea that regression of PTLD is mediated by cytotoxic T-cells and is in keeping with the observation that T-cell depletion, represents a major risk factor for the development of PTLD.

Abstract

Primary liver tumors in children are rare with malignant hepatoblastoma being the most common neoplasm. In this report, we describe the diagnosis and clinical management of a large liver tumor in a 3-year-old child that displayed the features of both, conventional hepatoblastoma and malignant teratoma. Pathological assessment on a pre-operative bioptical specimen showed an immature teratoid tumor with no area of hepatoblastic differentiation present. Histological and immunhistological examination of the resected tumor specimen additionally showed tumor areas of very different differentiation pattern intermixed with each other, namely areas of hepatoblastoma-typical and neuroblastoma-like morphology as well as areas of rhadomyosarcomatous differentiation. After chemotherapy the tumor size increased and an extended right hemihepatectomy was performed. Post-operatively, the general condition of the child improved and adjuvant chemotherapy was started two weeks later. 36 months after initial diagnosis the patient is healthy, in good general condition, and without any sign of residual tumor disease. Overall, we describe the diagnosis and clinical management of a large liver tumor in a 3-year-old child that displayed the features of both, conventional hepatoblastoma and malignant teratoma and was designated as mixed hepatoblastoma and teratoma. Though mesenchymal tumor portions can occur within hepatoblastomas, most commonly osteoid or chondroid, our case is different as it presents a large spectrum of mesenchymal and epithelial differentiation pattern in most of the lesion.

Abstract

We report a case of a 57-year-old male patient presenting with a painful mass in the popliteal fossa of the left knee, while the X-ray being unremarkable, MRI suggested a paraarticular lesion such as an inflamed paraarticular ganglion. A biopsy showed a poorly differentiated metastasis of a papillary thyroid carcinoma, the patient had been operated on 8 years ago. This case emphasizes that in patients with malignancies such as papillary thyroid carcinomas long-term courses (over years) with several phases of tumor spread occur finally leading to filiae in any location. Thus, in such patients, a metastatic lesion even in unusual places such as the periarticular soft tissue should be included in the differential diagnosis.

Abstract

Interleukin-5 (IL-5) is a cytokine which is essential for the maturation of eosinophils in bone marrow and for their release into the blood. Eotaxin is a CC type chemokine implicated in the recruitment of eosinophils in a variety of inflammatory disorders. Since eosinophil-activity is governed by these two pathways, we targeted both IL-5 and eotaxin by active vaccination to block eosinophilia. We produced two vaccines by chemically cross-linking IL-5 or eotaxin to a virus-like particle (VLP) derived from the bacteriophage Qbeta, yielding highly repetitive arrays of these cytokines on the VLP surface. Both vaccines overcame self-tolerance and induced high antibody titers against the corresponding self-molecules in mice. Immunization with either of the two vaccines reduced eosinophilic inflammation of the lung in an ovalbumin (OVA) based mouse model of allergic airway inflammation. Animals immunized with the two vaccines at the same time developed high antibody titers against both cytokines and also reduced eosinophil-infiltration of the lung. These data demonstrate that targeting either IL-5 or eotaxin may lower eosinophilia. Simultaneous immunization against IL-5 and eotaxin demonstrates that such a therapeutic approach may be used to treat complex disorders in which multiple mediators are involved.

Abstract

ASTEX is a feasibility study of an in-situ exploration mission to two near-Earth asteroids (NEAs) funded by the German Space Agency DLR. The mission objectives call for targets with different mineralogical compositions: one asteroid should be of “primitive'' nature, the other one should be a fragment of a differentiated asteroid. The goals of the mission are to explore the physical, geological and mineralogical nature of the asteroids and to provide information and constraints on the formation and evolution history of our planetary system. In addition, the detailed knowledge of the properties of NEAs will allow for the selection of potential mitigation systems when required. The mission scenario consists of an orbiting and landing phase at each target. Aims of the study: The primary aim of the study is to define the high-level goals of the ASTEX mission as well as to identify and to analyze mission scenarios which support these goals. The study includes: 1) Identification of potential target pairs, optimized trajectories and orbits around NEAs. 2) Identification of the optimum propulsion system. 3) Selection of the scientific strawman payload of orbiter and lander systems. 4) Analysis of the requirements and options for the spacecraft bus and the lander system. 5) Identification of the requirements for the operational ground segment.

Abstract

IL-1 alpha and IL-1 beta are potent pro-inflammatory cytokines that regulate many physiological systems by binding and signaling to the same receptor termed IL-1 receptor type 1 (IL-1R1). We have investigated the role of IL-1 for pulmonary immune responses in models of allergic asthma using IL-1R1-deficient (IL-1R1(-/-)) mice. In a model of mild asthma, based on repeated sensitization of mice with low doses of ovalbumin in the absence of any adjuvant and multiple intranasal challenges, the pulmonary eosinophilic inflammation and goblet cell hyperplasia were strongly reduced in IL-1R1(-/-) as compared to control BALB/c mice. Moreover, priming of CD4(+) T cells in bronchial lymph nodes and their recruitment to the lung was affected in IL-1R1(-/-) mice associated with impaired antibody responses including IgG, IgE, and IgA. In contrast, sensitization of mice in the presence of alum adjuvant, a more severe asthma model, rendered the IL-1 pathway dispensable for the development of pulmonary allergic Th2 responses, as eosinophilic inflammation, antibody responses, and CD4(+) T cell priming in lymph nodes were comparable between IL-1R1(-/-) and wild-type mice. These results suggest a critical role of IL-1/IL-1R1 for development of allergic Th2 responses, but its requirement can be overcome by using alum as adjuvant for sensitization.
