Abstract

Fog computing is a promising technique to reduce the latency and power consumption issues of the Internet of Things (IoT) ecosystem by enabling storage and computational resource close to the end-user devices with additional benefits such as improved execution time and processing. However, with an increase in IoT devices, the resource allocation and job scheduling became a complicated and cumbersome task due to limited and heterogeneous resources along with the locality restriction in such computing environment. Therefore, this paper proposes a cache-based approach for efficient resource allocation in fog computing environment, while maintaining the quality of service. The proposed algorithm is realized using iFogSim simulator and a comprehensive comparison is presented with the traditional First Come First Served and Shortest Job First policies. The performance evaluation revealed that with the proposed scheme the execution time, latency, processing delays and power consumption decreased by 38%, 11.1%, 6%, and 17.8%, respectively, as compared to those of the traditional schemes.

Abstract

Purpose: To compare RNLF structural changes by Spectral Domain- OCT and functional visual eld defects by automated perimetry in Primary Open Angle Glaucoma cases Materials And Methods: A prospective and comparative study was conducted to quantitatively measure the peripapillary retinal nerve ber layer thickness using SD-OCT and compared it with eld changes plotted by Humphrey Field analyser 24-2. The study was conducted at the department of Glaucoma of a tertiary eye care hospital for a period of one year. 60 eyes of 30 patients who diagnosed to have POAG between the age group of 40-65 were included in the study. Patients having substantial media opacity, Retinal pathology and who underwent intra ocular surgeries were excluded from the study. Informed consent obtained from patients who were included in the study. All were examined using Slit lamp, Snellen's chart, Applanation tonometer, Gonioscope, 90 D slit lamp biomicroscope to study Fundus oculi. Fields were plotted using Humphrey eld analyser 24-2 , RNFL thickness measured using SD-OCT. Results: 60 eyes of 30 patients 19 were males 11 were females. 14(23.33%) eyes showed normal visual elds but there was thinning of RNFL was noted in SD-OCT. 41 eyes (68.3%) had visual eld defects which corresponded to thinning of RNFL in that quadrant. 5 eyes (8.3%) showed advanced eld changes and had corresponding thinning of RNFL in that quadrant. Discussion: Results of this study shows a signicant difference in RNFL thickness among all three grades of glaucoma. It was observed that the average RNFL thickness value in moderate glaucoma (66.34 microns) and all quadrant thickness values were similar to Sihota et al study (RNFL-66.07 microns). The mean RNFL thickness +/- Sd (58 +/-5.52) in advanced glaucoma of present study were closely similar with the ndings of Sihota et al study (53.65 +/-14.2). Conclusion: It is concluded that RNFL thickness changes corresponds to Field changes plotted on Humphrey eld analyser in moderate glaucoma. Even in mild cases changes RNFL thickness is noted even though the elds plotted on Humphrey eld analyser doesn't show any changes. So RNFL thickness can be taken as Pre perimetric changes as diagnostic criteria for the diagnosis of POAG.

Abstract

Niemann-Pick disease has an autosomal recessive inheritance pattern and occurs due to a deficiency of a lysosomal enzyme, sphingomyelinase. It causes variable clinical signs and symptoms such as hepatosplenomegaly, delayed milestones, and peripheral cytopenia due to bone marrow involvement. Here, we report a case of a child who presented with hepatosplenomegaly and pancytopenia, who was later found to have Niemann-Pick disease on bone marrow examination. This case highlights the case presentations of this rare disease and the importance of bone marrow trephine in prompt diagnosis and management of a patient.

Abstract

A 15-year-old girl presented with the complaint of a cosmetically disfigured right eye since birth. The initial assessment was carried out by way of a history and physical examination. The patient's history revealed no significant findings. Physical examination revealed that the right ophthalmic socket was seemingly devoid of an identifiable globe and ocular tissue, which is described as an anophthalmic socket. The conjunctival fornices appeared short, a finding which was more noticeable in the inferior conjunctival fornix. The patient's orbital volume was decreased. To increase the orbital volume, a dermis fat graft (DFG) was carefully taken from the gluteal region. The graft was then transposed to the anophthalmic socket and sutured to the conjunctiva and Tenon's capsule. A prosthetic eye was placed in the socket. Later, a second surgical intervention was performed to deepen the inferior fornix, for which a mucous membrane graft was taken from the lower lip. As a result of these interventions, all cosmetic and medical concerns of the patient regarding the anophthalmic socket were addressed. The success of this procedure speaks to the efficacy of DFGs in the repair of an anophthalmic socket.

Abstract

Incontinentia pigmenti (IP) is a rare X-linked dominant disorder. We present a case of an infant with IP who was brought to the operating room for panretinal diode photocoagulation under general anesthesia. The anesthesia team was unable to obtain intravenous access even with instruments such as a vein finder and ultrasound. Anesthesia for IP patients also poses challenges such as prevention of the oculocardiac reflex, obesity and airway management, and preemptive measures for intravenous access due to skin manifestations. Patients with IP may present with many challenges for the anesthesiologist during all phases of anesthetic management.

Abstract

Coronavirus - 2019 (COVID-19)-a respiratory syndrome caused by the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-COV2)-is responsible for the world's latest pandemic unlike any seen since the 1980s HIV/AIDs pandemic. While significant effort has been made towards acute management, long-term management has received less attention. To address this, our study identified patients diagnosed with COVID-19 and recorded the time to enrollment into a formal pulmonary rehabilitation program at an academic center;the differences in time allowed us to assess whether earlier enrollment led to differences in physiologic and psychologic outcomes. First, we hypothesize that enrollment into a pulmonary rehabilitation program is essential to a patient's long term treatment and management of post-infectious, chronic COVID-19 symptoms. Second, we aim to demonstrate that early participation leads to improvements in dyspnea, anxiety and depression as supported by the UCSD SOBQ, GAD-7 and PHQ-9 scores respectively. Through chart review, we collected data from 24 patients enrolled in a post-COVID recovery program centered around pulmonary rehabilitation at an academic medical center in Texas. We administered: (1) pulmonary function test (PFT);(2) University of California - San Diego (UCSD) Dyspnea Questionnaire;and (3) six-minute walk (6MW) tests when a patient entered and completed the program. We used this data to assess for subjective and objective changes in respiratory function postrehabilitation. To analyze the indirect benefits of pulmonary rehabilitation on mental health effects of COVID-19, we used the Generalized Anxiety Disorder 7 (GAD7) and Patient Health Questionnaire (PHQ9) questionnaires to evaluate a patients' anxiety and depression respectively at the same time points. Our results showed that it took patients an average 67.8 days to initiate rehabilitation after initial diagnosis. 13 of 24 patients began rehab earlier than the average and had an average decrease in UCSD SOBQ score of 35.6 and 6MW increase of 64.5 m compared to the group averages of 17.8 and 74.7 m respectively. The average initial GAD7 and PHQ9 scores were 6.7 and 8.7, with an average decrease of 1.5 and 2.2 respectively. Through our study, we highlight the benefit of early initiation of pulmonary rehabilitation in the post-COVID 19 recovery period. Through both subjective and objective measurements of patient respiratory function and mental health, our study supports inclusion and standardization of early pulmonary rehabilitation as a component of COVID-19 recovery.

Abstract

Introduction Face of healthcare, patient safety and protection of healthcare providers has been completely transformed with global health pandemic. However, emergency surgical procedures must still be performed, with the commonest being appendicectomy. It is common practice across National Health Service trusts to collect a group and save (G&S) sample pre-operatively which increases healthcare staff exposure to the patient and increased use of personal protective equipment in this pandemic. Method Prospective study of adult patients undergoing emergency appendicectomy since the transformation of emergency care with COVID-19 induced restrictions compared with patients undergoing the same operation before the pandemic. Results 179 adult patients underwent emergency appendicectomy over 6-months in 2019–2020, 60 patients in the 12 weeks period from the start of the transformed emergency services due to the pandemic. Pre-operative G&S samples were taken for 60(33.5%) patients in the pre COVID-19 period, whereas 7(11.6%) were taken for patients undergoing appendicectomy during the pandemic. None of the patients in either group had intra-operative blood loss of more than 500 millilitres and none of them required peri-operative blood transfusion Conclusions Our study demonstrates that the routine pre-operative G&S can safely be abandoned as a routine practice for all patients undergoing emergency appendicectomy.

Introduction NICE (National Institute of Clinical Excellence) guidelines currently recommend the use of CT coronary Angiogram (CTCA) as the initial test to investigate coronary artery disease in patients with new onset of chest pain. Our aim was to evaluate the relationship between the CT coronary angiogram findings on index presentation, and hospital admissions and re-referral to outpatient clinics in following 2 years. Method Data was accrued via a retrospective analysis of electronic medical records at Sunderland Royal Hospital pertaining to patients who presented to the Rapid Access Chest Pain Clinic (RACPC) and underwent CTCA in 2017.Data included: Presentation – Typical & atypical angina Risk factors profile Investigations including ECG, ECHO, CTCA, perfusion scan and invasive coronary angiography Severity of coronary artery lesion on CTCA Hospital admissions or re-referral to outpatient clinics in 2 year follow up Results In the 235 patients studied, mean age was 56 years with 130 (55.5%) men and 195 (82.9%) presented with atypical angina as shown in table. Out of 195 patients with atypical chest pain only 17 (8.7%) were diabetics and most of them 178 (91%) had Coronary Calcium score of 1-400. Most patients (184) underwent CT coronary angiogram with 39 (21%) having normal coronary arteries, 126 (68%) with mild to moderate coronary artery disease and 19 (11%) with severe coronary artery disease. Subsequent assessments with invasive coronary angiography, myocardial perfusion scan and Treadmill exercise did not reveal significant disease warranting coronary revascularization. Patients with normal or mild -moderate CAD on CTCA 24 (15%) represented with acute chest pain (only one needed PCI) and 6 (3.5%) were referred to outpatient clinics over 2 years follow up. In patients with severe CAD on CTCA, 6 (32%) presented with acute chest pain and 4 (21%) needed PCI. Almost all patients were treated with statins and antiplatelets following CTCA results. Conclusion CT coronary angiography is sensitive and specific in assessment of hemodynamically significant coronary artery disease in non-diabetic patients presenting with angina in outpatient setting. CTCA in patients with normal or mild to moderate CAD also gives confidence to the clinician and prevents further un-necessary investigation and hospital admissions/outpatient referrals.

Background and importance The importance of health promotion and disease prevention among the general public has been reinforced following the COVID-19 pandemic Although national campaigns have been active for years, reports have highlighted the opportunities for the greater use of pharmacy teams for improving this, in light of their location, accessibility, convenience and relationship with the public Aim and objectives To assess the level of knowledge on important health topics of patients and learn their preferences for future learning in order to develop a targeted and effective health promotion programme Material and methods In July 2019, patients waiting for a prescription to be filled in a hospital outpatient pharmacy were approached for inclusion in the study Those who consented were interviewed via a confidential questionnaire (revised following a pilot on 5 patients) until 100 patients were recruited The results were submitted into Excel for analysis Results The participation rate was approximately 30% (47% men and 53% women, aged 18-70 years) Approximately 10% of patients were unaware of the risks of high blood pressure and 28% had never had their blood pressure monitored 28% did not know the maximum recommended units of alcohol permitted per week All smokers (28%) had been unsuccessful in previous attempts to stop smoking Although all patients were aware of the correct signs of breast cancer, 17% of patients were unsure of the signs of prostate cancer 40% of patients were unable to give two correct symptoms of depression and some patients mentioned inaccurate ones Although over 75% of patients preferred to receive health promotion information via a one-to-one consultation with pharmacy staff, 74% of patients thought watching health promotion videos while waiting for a prescription was a good idea All patients had access to a mobile phone or a computer and were happy to receive information via their electronic devices Conclusion and relevance The study highlighted gaps in knowledge, particularly in the areas of alcohol intake, depression and prostate cancer, giving ideas of where to target future health promotion campaigns Although patients prefer personal consultations with pharmacy staff, novel ways of delivering health promotion, including the use of phones and electronic devices, should be considered

Abstract

Background Coronavirus disease 2019 (COVID-19) was declared a pandemic by the WHO on 11 March 2020 and global surgical practice was compromised. This Commission aimed to document and reflect on the changes seen in the surgical environment during the pandemic, by reviewing colleagues’ experiences and published evidence. Methods In late 2020, BJS contacted colleagues across the global surgical community and asked them to describe how severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) had affected their practice. In addition to this, the Commission undertook a literature review on the impact of COVID-19 on surgery and perioperative care. A thematic analysis was performed to identify the issues most frequently encountered by the correspondents, as well as the solutions and ideas suggested to address them. Results BJS received communications for this Commission from leading clinicians and academics across a variety of surgical specialties in every inhabited continent. The responses from all over the world provided insights into multiple facets of surgical practice from a governmental level to individual clinical practice and training. Conclusion The COVID-19 pandemic has uncovered a variety of problems in healthcare systems, including negative impacts on surgical practice. Global surgical multidisciplinary teams are working collaboratively to address research questions about the future of surgery in the post-COVID-19 era. The COVID-19 pandemic is severely damaging surgical training. The establishment of a multidisciplinary ethics committee should be encouraged at all surgical oncology centres. Innovative leadership and collaboration is vital in the post-COVID-19 era.

Abstract

Background Grisel’s syndrome is a non-traumatic atlantoaxial subluxation associated with inflammatory conditions of the head and neck, which occurs primarily in children. Increased flexibility of the ligaments during inflammation is implicated in the pathogenesis of the subluxation between the axis and atlas. The potential sequelae may be severe, and early diagnosis and treatment of Grisel’s syndrome can prevent tragic outcomes. Case presentation We present a case of torticollis in an 8-year-old child. She had a two-week history of a streptococcal throat infection. The patient was treated with several different methods of conservative care, including muscle relaxation, cervical halter traction, and Halo application. However, the torticollis persisted. The patient then required surgical correction involving cervical spine fusion. She had no complications and experienced no reoccurrence of the torticollis to date. Conclusion Grisel’s syndrome is a pathology for which conservative management is successful in most cases. Cases requiring surgical intervention are rarely documented in the literature. Our case is significant, as in spite of aggressive conservative management, the patient required surgical correction. Patients requiring surgical management of Grisel’s syndrome may require additional anesthetic exposure for diagnostic interventions like magnetic resonance imaging or neck manipulations for closed reduction. We discuss the features of Grisel’s syndrome and specific anesthetic management considerations for procedures such as magnetic resonance imaging, application of cervical traction, and surgical correction of torticollis.

Abstract

Background Idiopathic nodular mesangial sclerosis, also called idiopathic nodular glomerulosclerosis (ING), is a rare clinical entity with an unclear pathogenesis. The hallmark of this disease is the presence of nodular mesangial sclerosis on histology without clinical evidence of diabetes mellitus or other predisposing diagnoses. To achieve insights into its pathogenesis, we queried the clinical, histopathologic and transcriptomic features of ING and nodular diabetic nephropathy (DN). Methods All renal biopsy reports accessioned at Indiana University Health from 2001 to 2016 were reviewed to identify 48 ING cases. Clinical and histopathologic features were compared between individuals with ING and DN (n = 751). Glomeruli of ING (n = 5), DN (n = 18) and reference (REF) nephrectomy (n = 9) samples were isolated by laser microdissection and RNA was sequenced. Immunohistochemistry of proline-rich 36 (PRR36) protein was performed. Results ING subjects were frequently hypertensive (95.8%) with a smoking history (66.7%). ING subjects were older, had lower proteinuria and had less hyaline arteriolosclerosis than DN subjects. Butanoate metabolism was an enriched pathway in ING samples compared with either REF or DN samples. The top differentially expressed gene, PRR36, had increased expression in glomeruli 248-fold [false discovery rate (FDR) P = 5.93 × 10−6] compared with the REF and increased 109-fold (FDR P = 1.85 × 10−6) compared with DN samples. Immunohistochemistry revealed a reduced proportion of cells with perinuclear reaction in ING samples as compared to DN. Conclusions Despite similar clinical and histopathologic characteristics in ING and DN, the uncovered transcriptomic signature suggests that ING has distinct molecular features from nodular DN. Further study is warranted to understand these relationships.

Abstract

We present a rare case of pancreatic panniculitis in a 59-year-old male simultaneous pancreas-kidney (SPK) recipient with failed allografts. The patient presented with fever and painful erythematous nodules on his leg 1 month after stopping all immunosuppression. A thorough infectious and rheumatological workup was negative. He had pancreas rejection 4 years after SKP transplant and was restarted on dialysis after 14 years when his renal allograft failed due to chronic allograft nephropathy. His chronic immunosuppression (tacrolimus, azathioprine) was stopped and prednisone was weaned over 3 months at that time. A skin biopsy revealed saponification of the subcutaneous fat with inflammation pathognomonic of pancreatic panniculitis. Concurrent allograft pancreatitis confirmed with elevated lipase and a computed tomography scan finding of peripancreatic graft stranding and atrophic native pancreas. He was started on pulse steroid therapy for 3 days followed by oral taper. This resulted in dramatic resolution of all skin lesions and normalization of lipase levels within 1 week, followed by resumption of low-dose tacrolimus and azathioprine. This is an extremely rare occurrence of panniculitis in pancreas allograft after 10 years of pancreatic failure associated with stopping immunosuppression.

Abstract

Purpose To determine the increase in clinically significant cancer detection in the prostate with increasing number of core samples obtained by using cognitive MRI-targeted transrectal US biopsy. Materials and Methods This retrospective cross-sectional study included 330 consecutive patients (mean age, 64.3 years; range, 42-84 years) who underwent multiparametric prostate MRI from March 2012 to July 2017 and had an index lesion that subsequently underwent cognitive MRI-targeted biopsy using transrectal US with at least five core samples (which were sequentially labeled) per lesion. The detection rate of clinically significant cancer was calculated on sequential biopsy cores, comparing the first core alone versus three cores versus five cores per target. Clinically significant cancer was defined as International Society of Urological Pathology Grade Group 2 or higher. Results Increasing the number of biopsy core samples from one to three per target and from three to five per target increased the detection rate of clinically significant cancer by 6.4% (21 of 330) and 2.4% (eight of 330), respectively. The target yield for clinically significant cancer was 26% (87 of 330), 33% (108 of 330), and 35% (116 of 330) for one, three, and five cores, respectively. Subgroup analysis showed no significant difference in upgrade rates as a function of multiparametric MRI lesion size (P = .53-.59) or location (P = .28-.89). Abstract

Coal bottom ash (CBA) generated from power plants have become a major environmental concern and hence its usage by recycling it is the increasing demand for research nowadays. CBA is collected from bottom of the boiler and is normally as coarse sand to fine gravel size material which is generally used as a low cost replacement material. In this paper CBA waste material is introduced, considered and its utilization is reviewed. Information is provided on the various reuse options of this waste material which has been implemented. To replace the material as an ingredient in concrete is the key aim of this paper and has been emphasized throughout. To reduce the burden on natural aggregates and cement use of such CBA can prove to be a good option in future. It is also concluded that some pre-defined treatment method is needed to be developed to implement the use of CBA in concrete. This paper would help beginners to understand the scope and development in use of CBA in concrete as an ingredient.

Abstract

Background Adherence to an oncology regime is vital and hence it is important that patients understand how and when to take their medicines before their first cycle of chemotherapy as well as know the signs of toxicity that can be life-threatening. An oncology counselling training programme was developed and an audit was undertaken to assess whether its use by pharmacists in a hospital outpatient pharmacy could improve medicines’ optimisation. Purpose To investigate whether the use of an oncology training programme for pharmacists can improve medicines’ optimisation for patients taking first-cycle chemotherapy. Material and methods A counselling tool was developed containing proformas of the most commonly dispensed oral chemotherapy medicines. Pharmacists were trained via a specific programme which taught them how to counsel effectively using the tool. The proformas contained detailed information about how and when to take the drug, what to do if they miss or vomit after taking a dose, what the most common side-effects are and how to manage those side-effects. Fifty-two patients were included in the study. All patients were interviewed to assess how much they knew about their medicines and how confident they were before and after the pharmacists’ counselling. Results Seventy-five per cent of patients were confident/very confident after counselling by pharmacists compared to 35% prior to it. Patients gained information (despite prior counselling from other health professionals) on: how to take the medicines (31%); what to do if they miss a dose (82%); what to do if they vomit after a dose (86%); how to handle chemotherapy (66%); and what the side-effects are (14%). Interventions, including clarifying the frequency of dosing, alerting doctors about changes in patients’ weight and providing information about drug interactions and how the medicines work were made in 42% of cases. Counselling by the newly trained pharmacists helped 100% of patients. Conclusion Utilisation of an oncology counselling training programme for pharmacists can significantly enhance patients’ knowledge about their first-cycle chemotherapy and boost their confidence about taking their medicines, which can improve medicines’ optimisation and patient safety. In this paper, we propose to integrate cloud computing into cognitive radio adhoc vehicular networking (CRAVENET) so that the vehicles can share network resources and can benefit wider scope network information for decision making. The proposed architecture includes a cloud enabled CRAVENET. Then, we study the IoV application management system in this cloud enabled CRAVENET. This architecture facilitates to recognize the available resources in real time. Also it provides cloud enabled IoV applications to CRAVENET enabled vehicles. The presented work demonstrates the security protocol in cloud enabled CRAVENET. We propose a distributive and comparative methodology for a secure vehicular network to communicate. We demonstrate the potential of our architecture for real time access of IoV applications in cloud enabled CRAVENET environment.

This is a case report of a patient with history of vague chest pain and facial numbness. When the patient was subjected for evaluation he was found to have cervical Schwannoma. He was operated and was successfully treated. Schwannomas are extramedullary-intradural tumors composed of Schwann cells, which can arise from spinal nerves at any level (cervical, thoracic, lumbar, or cauda equina) and most often arise from a posterior (sensory) nerve root. Abstract

Purpose Surgical bleeding in the immediate post-operative period presents a frequent challenge in the management of LVAD recipients. We hypothesized that implementing an aggressive approach to surgical hemostasis would reduce the rate of early mortality. Methods We conducted a single-center retrospective review of 78 patients who underwent LVAD implantation between 9/13/11 and 8/24/17. Logistic regressions were utilized to estimate the odds ratio for RVAD requirement and in-hospital mortality based on risk factors of six-hour chest tube output and return to operating room for bleeding. Chest tube output of greater than 600 cc in the first six hours was considered to be significant. Results When comparing chest tube outputs in the first six hours, there were no significant differences in age, gender, heart failure etiology, or device type in patients with >600 cc and those with lower outputs. Greater than 600 cc chest tube output in the first six hours was an independent predictor for RVAD requirement (15.2% vs. 2.3%, p=0.04) and in-hospital mortality (24.2% vs. 4.7%, p=0.012) than those with lower outputs. Patients who returned to the operating room for bleeding complications experienced a similar in-hospital mortality rate (17.6% vs. 9.3%, p=0.28) but had a higher RVAD requirement (14.7% vs. 2.3%, p=0.04). Multivariate analysis showed a statistical significance for in-patient mortality in patients with chest tube output >600 cc in the first 6 hours (p = 0.013). There was a close correlation between the amount of chest tube output during the first six hours and the risk of in-hospital mortality ( Figure 2 ). Conclusion Minimizing the degree of post-operative bleeding after LVAD surgery may result in a reduced rate of right ventricular failure and in-hospital mortality. These findings suggest that efforts should be undertaken to identify surgical strategies for achieving improved hemostasis at the time of implantation.

Abstract

Ludwigs angina is a potentially lethal deep neck infection associated with rapid airway obstruction due to swelling of the neck, tongue and submandibular areas. Despite the name, it has no relation to angina pectoris. We report a case of an 80 -year-old gentleman who presented with the unusual symptom of chest pain and previous history of diabetes mellitus and treated lymphoma, who was later diagnosed with Ludwigs angina. Despite of early intravenous antibiotics, airway support and intensive care treatment patient died within 24 hours. This case highlights the need for a high index of suspicion for Ludwigs angina in patients with chest pain and neck swelling, as althoughits lethal conditionbut early, rapid and aggressive intervention can save lives.

Abstract

Robotic surgical technology has been adopted by surgeons with and without previous standard laparoscopic experience. The necessity or benefit of prior training and experience in laparoscopic surgery is unknown. We hypothesized that laparoscopic training enhances performance in robotic surgery.Fourteen medical students with no surgical experience were instructed to incise a spiral using the da Vinci(®) surgical robot with time to completion and errors recorded. Each student was then trained for 1 month in standard laparoscopy, but with no further robotic exposure. Training included a validated laparoscopic training program, including timed and scored parameters. After completion of the month-long training, the students repeated the cutting exercise using the da Vinci robot as well as with standard laparoscopic instruments and were scored within the same parameters.The mean time to completely incise the spiral robotically before training was 16.72 min with a mean of 6.21 errors. After 1 month of validated laparoscopic training, the mean robotic time fell to 9:03 min (p=0.0002) with 3.57 errors (p=0.02). Laparoscopic performance after 1 month of validated laparoscopic training was 13.95 min with 6.14 errors, which was no better than pretraining robotic performance (p=0.20) and worse than post-training robotic performance (p=0.01).Formal laparoscopic training improved the performance of a complex robotic task. The initial robotic performance without any robotic or laparoscopic training was equivalent to standard laparoscopic performance after extensive training. Additionally, after laparoscopic training, the robot allowed significantly superior speed and precision of the task. Laparoscopic training may improve the proficiency in operation of the robot. This may explain the perceived ease with which robotics is adopted by laparoscopically trained surgeons and may be important in training future robotic surgeons.

Introduction: The purposes of this investigation were to determine the: (1) reproducibility of the patterns of responses for electromyographic (EMG) amplitude and mean power frequency (MPF); and (2) intraclass correlation coefficient (ICC) of the EMG amplitude and MPF during three incremental cycle ergometer tests separated by 48 hours. Methods: Ten men performed incremental cycle ergometry tests to exhaustion on three separate occasions. Surface EMG signals were recorded simultaneously from the three superficial quadriceps muscles at each trial. Results: Polynomial regression indicated that, for >95% of the cases, the best-fit model was the same at each trial for EMG amplitude but not for EMG MPF. The ICC values were high for EMG amplitude but low for EMG MPF. Conclusions: These results indicate that EMG amplitude is a reliable measure of motor unit activation strategy during incremental cycle ergometry, whereas the EMG MPF was unreliable. Abstract

Directional Drilling (DD) achieves higher well production and improves accessibility of oil reservoirs in complex locations. Quad Motor drilling heads was recently proposed to achieve better steering control of the drilling operation and higher dogleg severity. The current state of the art in DD is the Rotary Steerable System (RSS), where steering actuators displace the centerline of the drilling system away from the centerline of the hole by a controllable amount of eccentricity. The drilling bit is powered by the rotation of the drilling string. However, in the quad motor drilling head, four drilling bits are independently controlled by 4 downhole motors. This paper proposes a real time control and optimization of the quad bit directional drilling system. The control commands include the angular velocity and torque of each of the 4 motors. The control is updated at regular distance intervals. The optimization procedure determines the drilling parameters to minimize the drilling time and the deviation from the planned trajectory.

PurposeThe purpose of this audit was to determine the most common medication related prescription errors in ophthalmic practice, to determine avoidable prescription errors and evaluate the effect of preventative methods against these errors.MethodsThe first audit was conducted prospectively over a four week period in Oct 2009 and a re-audit after 6months on May 2010, to determine the effects of preventative measures suggested in the first audit.ResultsThere were 4.7% (29/623) prescription errors during the initial four week audit period. A method of check and counter check of prescriptions was implemented and re-audit showed a reduction in incidence of errors to 0.77% (5/651) errors.ConclusionsThe majority of prescribing errors occurred at the stage of writing the prescription and our findings suggest that the intervention of check and counter check can reduce the rate of error significantly.

Abstract

Data publication is a simple and cost-effective approach for data sharing across organizations. Data anonymization is a central technique in privacy preserving data publications. Many methods have been proposed to anonymize individual datasets and multiple datasets of the same data publisher. In real life, a dataset is rarely isolated and two datasets published by two organizations may contain the records of the same individuals. For example, patients might have visited two hospitals for follow-up or specialized treatment regarding a disease, and their records are independently anonymized and published. Although each published dataset poses a small privacy risk, the intersection of two datasets may severely compromise the privacy of the individuals. The attack using the intersection of datasets published by different organizations is called a composition attack. Some research work has been done to study methods for anonymizing data to prevent a composition attack for independent data releases where one data publisher has no knowledge of records of another data publisher. In this chapter, we discuss two exemplar methods, a randomization based and a generalization based approaches, to mitigate risks of composition attacks. In the randomization method, noise is added to the original values to make it difficult for an adversary to pinpoint an individual’s record in a published dataset. In the generalization method, a group of records according to potentially identifiable attributes are generalized to the same so that individuals are indistinguishable. We discuss and experimentally demonstrate the strengths and weaknesses of both types of methods. We also present a mixed data publication framework where a small proportion of the records are managed and published centrally and other records are managed and published locally in different organizations to reduce the risk of the composition attack and improve the overall utility of the data usc

Abstract

This was a pilot study to establish a protein difference between thyroglossal duct cyst fluid relative to oral saliva, in order to differentiate patients with residual thyroglossal cyst from those with salivary fistula in the case of post-Sistrunk procedure incisional drainage. Past immunohistochemistry studies on archived tissue blocks from post-Sistrunk procedure patients have shown MUC5AC to have elevated concentrations in thyroglossal duct relative to salivary tissue; secretory immunoglobulin A (IgA) was also chosen as a candidate protein based on previously published reports of its increased concentration in saliva relative to respiratory secretions.Diagnostic tests were assessed. Thyroglossal duct cyst fluid and oral saliva were obtained from 12 patients. Enzyme-linked immunosorbent assay (ELISA) was then performed on the samples to identify the presence of MUC5AC and IgA.Tertiary care hospital.Patient fluid samples were taken immediately postoperatively for patients undergoing Sistrunk procedure at one institution. Presence of thyroglossal duct cyst was confirmed in all cases by pathology.Seven of 12 thyroglossal duct cyst fluid samples were shown to have elevated absorbance with ELISA for MUC5AC. In experiments for IgA, 10 of the 12 saliva samples had elevated absorbance compared with thyroglossal duct fluid samples obtained from the same patient.IgA is an accurate identifier of saliva compared with thyroglossal duct fluid in secreted samples obtained from the same patient.

Purpose: The development of an effective nontoxic intravesical agent that may be used immediately after bladder tumor resection to prevent the implantation of tumor cells would be a significant clinical advancement. We report the cytotoxic effects of curcumin on bladder tumor cell lines as well as its effects on the intravesical implantation of tumor cells in C3H mice.Materials and Methods: UMUC human and MBT-2 mouse bladder cancer lines were incubated with 0 to 100 μM. curcumin in dimethyl sulfoxide for 30 minutes and cell viability was determined by clonal assay. Additional culture dishes were incubated with curcumin and processed for electron microscopy. A comparison of blood pressure response with exercise stress, thallium scintigraphy, and 24-hour electrocardiographic monitoring between 5 patients with left ventricular hypertrophy associated with glycogen storage disease type III and 10 matched patients with hypertrophic cardiomyopathy revealed normal results in the former group. These data highlight the importance of the etiology of left ventricular hypertrophy before the application of risk stratification.

Abstract

AIM: Myocarditis and dilated cardiomyopathy may be phases of an organ-specific autoimmune disease of the myocardium. To provide evidence for autoimmune involvement in myocarditis, cardiac autoantibodies were detected in patient sera from the Myocarditis Treatment Trial. METHODS AND RESULTS: Cardiac antibody status was assessed by indirect immunofluorescence and by anti-alpha-myosin enzyme-linked immunosorbent assay in 53 patients from the Myocarditis Treatment Trial (35 males, aged 42 +/- 15 years); all had clinical myocarditis, but only 24 were classified as having histological myocarditis (Dallas criteria). By immunofluorescence, cardiac antibodies were more common in myocarditis (13/53) than in ischaemic (11/186, P = 0.0001) or in normal controls (24/270, P = 0.001). Abnormally raised anti-alpha-myosin antibodies were also more frequent in myocarditis (9/53) than in ischaemic (4/92, P = 0.01) or normal controls (4/203, P = 0.001); 34% of myocarditis patients were positive with one or both tests. Similar proportions of patients with and without histological myocarditis had antibodies by immunofluorescence (8/24 vs 5/29, P = ns) and by enzyme-linked immunosorbent assay (4/24 vs 5/29, P = ns). CONCLUSION: The detection of disease-specific cardiac autoantibodies supports autoimmune involvement in a subset of patients with clinical myocarditis. The lack of correlation of antibody with biopsy features suggests that diagnosis of myocarditis should not be made on histology alone. Autoimmune markers may provide adjunct diagnostic tools and identify patients in whom immunosuppression is of potential benefit.
