Gestational diabetes mellitus (GDM), a frequently-occurring disease during pregnancy, may cause some adverse healthy outcome of both mother and offspring. However, the knowledge about metabolite alterations during the pathogenesis and development process is limited. Here, a large longitudinal non-targeted metabolomics study of 195 pregnant women (64 women with subsequently developed GDM and 131 healthy controls) was conducted. Each participant provided urine samples at three timepoints during early, middle and late pregnancy, respectively. The metabolic profiles of 585 urine samples (195 × 3) were measured by using ultra-high performance liquid chromatography coupled with Orbitrap high-resolution mass spectrometry. Among the 56 identified metabolites, the levels of eight metabolites increased and three ones decreased in the first trimester, the concentration of one metabolite increased and those of 20 decreased in the second trimester, as well as the levels of five metabolites increased and two decreased in the third trimester. After false discovery rate correction, the levels of valine and 5-acetamidovalerate in GDM group significantly increased in the first trimester, the levels of 1-methylguanine and 1,3-dihydro-(2H)-indol-2-one significantly decreased in the second trimester and three metabolites (threonine, OH-octanedioyl-carnitine and pimelylcarnitine) increased and N-acetyltryptophan decreased in the third trimester, respectively. Six metabolites, such as pantothenic acid and threonine, had significant interaction effects between gestational stage (different trimester) and group (GDM or control). The differential metabolites were involved in “tryptophan metabolism”, “purine metabolism”, “valine, leucine and isoleucine degradation” and other pathways. The findings may provide insights into further pathogenesis study of GDM.

Inorganic fibers and opacifiers are indispensable for improving the strength and high temperature insulation performance of the fumed silica-based thermal insulation material. However, zirconia fiber enhances the strength of the fumed silica-based thermal insulation material and reduces the radiative heat transfer to replace the opacifier. The sample of fumed SiO2/Al2O3 doped with 7% zirconia fiber (FZ7) has a lower density of 0.70 g/cm3 and a high porosity of 75.0%. In addition, the thermal conductivity of FZ7 at 800 °C is 0.077 W/(m·K), which is lower than the sample of fumed SiO2/Al2O3 doped with 7% glass fiber (FG7) and 0.089 W/(m·K) at 800 °C. The effective extinction coefficient of the thermal insulation material containing zirconia fiber is larger than that of the glass fiber by Fourier transform infrared spectroscopy analysis and calculation, indicating that the zirconia fiber has a distinct absorption and scattering effect on infrared radiation to reduce the radiative heat transfer. Therefore, zirconia fiber enhances the strength and decreases the high temperature thermal conductivity of the composites with the dual effect on the insulation and mechanical performance of the fumed silica-based thermal insulation material.

Existing studies for multi-source neural machine translation (NMT) either separately model different source sentences or resort to the conventional single-source NMT by simply concatenating all source sentences. However, there exist two drawbacks in these approaches. First, they ignore the explicit word-level semantic interactions between source sentences, which have been shown effective in the embeddings of multilingual texts. Second, multiple source sentences are simultaneously encoded by an NMT model, which is unable to fully exploit the semantic information of each source sentence. In this paper, we explore multi-stage information interactions for multi-source NMT. Specifically, we first propose a multi-source NMT model that performs information interactions at the encoding stage. Its encoder contains multiple semantic interaction layers, each of which sequentially consists of (1) monolingual semantic interaction sub-layer, which is based on the self-attention mechanism and used to learn word-level monolingual contextual representations of source sentences, and (2) cross-lingual semantic interaction sub-layer, which leverages word alignments to perform fine-grained semantic transitions among hidden states of different source sentences. Furthermore, at the training stage, we introduce a mutual distillation based training framework, where single-source models and ours perform information interactions. Such framework can fully exploit the semantic information of each source sentence to enhance our model. Extensive experimental results on the WMT14 English-German-French dataset show our model exhibits significant improvements upon competitive baselines.

Background Early mobilization is incorporated into many enhanced recovery pathways. Inadequate analgesia or excessive opioids may restrict postoperative mobilization. The authors tested the hypotheses that in adults recovering from abdominal surgery, postoperative pain and opioid consumption are inversely related to postoperative mobilization, and that postoperative mobilization is associated with fewer potentially related complications. Methods The authors conducted a subanalysis of two trials that enrolled adults recovering from abdominal surgery. Posture and movement were continuously monitored for 48 postoperative hours using noninvasive untethered monitors. Mobilization was defined as the fraction of monitored time spent sitting or standing. Results A total of 673 patients spent a median [interquartile range] of 7% [3 to 13%] of monitored time sitting or standing. Mobilization time was 1.9 [1.0 to 3.6] h/day for patients with average pain scores 3 or lower, but only 1.2 [0.5 to 2.6] h/day in those with average scores 6 or greater. Each unit increase in average pain score was associated with a decrease in mobilization time of 0.12 (97.5% CI, 0.02 to 0.24; P = 0.009) h/day. In contrast, there was no association between postoperative opioid consumption and mobilization time. The incidence of the composite of postoperative complications was 6.0% (10 of 168) in the lower mobilization quartile, 4.2% (7 of 168) in the second quartile, and 0% among 337 patients in the highest two quartiles (P = 0.009). Conclusions Patients recovering from abdominal surgery spent only 7% of their time mobilized, which is considerably less than recommended. Lower pain scores are associated with increased mobility, independently of opioid consumption. Complications were more common in patients who mobilized poorly. OBJECTIVE We sought to investigate the clinical efficacy and safety of a novel endoscopic closure technique in repairing gastric wall defects after endoscopic full-thickness resection (EFTR) of gastric submucosal tumors (SMTs) originating from the muscularis propria layer. METHODS From December 2016 to December 2019, patients with gastric submucosal tumors were enrolled and clinicopathological data were collected. All SMTs were resected by EFTR and gastric wall defects were closed using a novel endoscopic closure technique. The feasibility, efficacy, and safety were evaluated. RESULTS A total of 21 patients with gastric SMTs were included in this study. Among the included SMTs, 15 tumors were located in the fundus of the stomach, and 6 were located in the upper body of the stomach. The average size of the lesions was 2.3 cm (range: 1.9-2.5 cm). All patients underwent EFTR and the gastric wall defect was closed by endoscopic closure. The average endoscopic closure time was 9 min (range: 7-15 min) and the average hospitalization stay length was five days (range: 4-6 days). One patient developed abdominal pain on the first day after the procedure and their body temperature increased; he received treatments such as anti-infection, antacid, and gastrointestinal decompression and was cured and discharged after 4 days. No instance of delayed bleeding, postoperative gastrointestinal fistula, or abdominal infection occurred. No case was transferred to surgery. The postoperative pathology profile included 18 stromal tumors and 3 leiomyomas. During the follow-up period (6-24 months), no case of residual or recurrence was recorded. CONCLUSION The described endoscopic loop ligation technique is feasible, effective, and safe for repairing gastric wall defects after EFTR for gastric submucosal tumors originating from the muscularis propria layer.

Occupying the most significant portion of global data traffic, video is being generated in almost every aspect of our life. Because of its huge volume, we are depending much more heavily on machine intelligence based analysis. In the meantime, video coding technology has been continuously improved for better compression efficiency. However, the state-of-the-art video coding standards, such as H.265/HEVC and versatile video coding (VVC), are still designed assuming that the compressed video will be watched by a human later. Such a design is not optimal when the compressed video will be used by computer vision applications. While the human visual system (HVS) is consistently sensitive to the content with high contrast, the impact of pixels on computer vision algorithms is task driven. For example, because of the different categories of objects used to train detection algorithms, the influence of the same image content on those detectors also varies. Therefore, human oriented video coding strategies may not be optimal when the compressed signal is further processed by algorithms, as the encoder is unaware of the task specific information. In this paper, taking object detection as an example, we propose a novel video coding strategy for computer vision. By protecting the information according to its importance for an object detector rather than for the human visual system, our proposed method has the potential to achieve a better object detection performance with the same bandwidth. The main contributions of our paper are: 1) the modeling of the relationship between object detection accuracy and bit rate; 2) a back propagation based method to analyze the influence of each pixel on the detection of target objects; 3) an object detection oriented bit allocation and codec control parameter determination scheme; 4) an evaluation metric to compare the impact of video coding strategies on a given object detector over a predefined range of bit rate. Experimental results demonstrate that our proposed algorithm can better preserve the video content vital for object detection than state-of-the-art video coding schemes.

The treatment options for cancer of unknown primary (CUP) are challenging due to the lack of knowledge about the primary sites, often resulting in a poor prognosis. The emerging next-generation sequencing (NGS) technique has provided a reliable approach to facilitate tumor primary site prediction and targetable gene alteration identification for CUP patients. In this report, we described a 63-year-old female patient who experienced recurrent CUP. NGS-based genetic profiling results revealed a pathogenic germline BRCA1 R71K mutation. Accordingly, the patient received the poly(adenosine diphosphate [ADP]-ribose) polymerase (PARP) inhibitor olaparib treatment and demonstrated a favorable response to this treatment. Our case suggests that NGS holds great promise for providing improved diagnosis and treatment options to patients with CUP, warranting further clinical investigation.

With the discovery of the lung microbiota, its study in both pulmonary health and disease has become a vibrant area of emerging research interest. Thus far, most studies have described the lung microbiota composition in lung disease quite well, and some of these studies indicated alterations in lung microbial communities related to the onset and development of lung disease and vice versa. However, the underlying mechanisms, particularly the cellular and molecular links, are still largely unknown. In this review, we highlight the current progress in the complex cellular and molecular mechanisms by which the lung microbiome interacts with immune homeostasis and pulmonary disease pathogenesis to advance our understanding of the elaborate function of the lung microbiota in lung disease. We hope that this work can attract more attention to this still-young yet very promising field to facilitate the identification of new therapeutic targets and provide more innovative therapies. Additional accurate standard-based methodologies and technological breakthroughs are critical to propel the field forward to ultimately achieve the goal of maintaining respiratory health.

BACKGROUND There are limited studies comparing budesonide inhalation suspension (BIS) with montelukast in real-world settings where treatment adherence and persistency may be suboptimal. This real-world study aims to investigate the control effectiveness of montelukast or BIS as a monotherapy in Chinese children with mild asthma. METHODS Data were derived from a retrospective questionnaire-based analysis of 2‒14-year-old children with mild persistent asthma, who received either 500 µg of BIS (n = 153) or 4‒5 mg of montelukast (n = 240) once daily. The indicators of asthma control, the Asthma Control Test (ACT)/Childhood ACT (C-ACT) score, and the asthma-related medical costs were assessed. The differences between the two groups were compared using an unpaired t-test (normally distributed), Mann-Whitney U test (non-normally distributed) or chi-squared test (categorical variables). RESULTS Medication compliance in the past 3-month period was better in the montelukast group than in the BIS group (P = 0.042). The montelukast group exhibited better asthma control in the past 4-week period, including lower percentages of asthmatic children with symptoms more than twice a week (P = 0.021), had night waking or night coughing (P = 0.022), or required reliever medication more than twice a week (P

Fatigue failure caused by unsteady loads on the shaft is an important limiting factor in the development of large VAWT. This study proposes an active control method that relies on movable mass blocks to reduce the unsteady loads. Then on the basis of the aerodynamic load of the VAWT simulated by computational fluid dynamics, the motion laws of the mass blocks with different masses are investigated. When the mass of mass block is 0.075 kg, the radial load of the shaft can be reduced by 97.1% and the shaft deformation can be eliminated by 99% without affecting the power output of wind turbine. However, further research on the start-up performance of the VAWT indicates that the active control method also prolongs the start-up time. Thus a method of adjusting the moment of inertia of wind turbine with a rotation speed of 15.6 m/s is proposed to save 40.6% of the start-up time with respect to the method used for direct start-up with large moment of inertia of 0.036 kg m2. After the start-up, a transition displacement curve is plotted to aid in the switching design of the operation mode of mass blocks between the start-up phase and the normal operation phase.

Postoperative nutritional therapy is of paramount importance for patients undergoing esophagectomy. The jejunostomy and nasoenteral tube are the popular routes for nutritional therapy. However, which one is the preferred route is unclear. This study aims to analyze the differences in safety and efficacy of the two routes for nutritional therapy.PubMed, Web of Science, Cochrane Library, and EMBASE (till September 17, 2020) were searched. The primary outcome was postoperative pneumonia. Secondary outcomes were the length of hospital stays (LOS), bowel obstruction, catheter dislocation, anastomotic leakage, overall postoperative complications, and postoperative albumin. Weighted mean differences (WMD) and odds ratios (OR) were calculated for statistical analysis.Ten studies involving a total of 1,531 patients in the jejunostomy group and 1,375 patients in the nasoenteral tube group were included. Compared with patients in the nasoenteral tube group, those in the jejunostomy group had a lower incidence of postoperative pneumonia (OR = 0.68, P0.001), shorter LOS (WMD = -0.85, P0.001), and lower risk of catheter dislocation (OR = 0.15, P = 0.001). There were no significant differences in the incidence of anastomotic leakage (OR = 0.84, P = 0.43), overall postoperative complications (OR = 0.87, P = 0.59), and postoperative albumin (WMD = -0.40, P = 0.24). However, patients in the jejunostomy group had a higher risk of bowel obstruction (OR = 8.42, P = 0.002).Jejunostomy for enteral nutrition showed superior outcomes in terms of postoperative pneumonia, LOS, and catheter dislocation. Jejunostomy may be the preferred enteral nutritional route following esophagectomy.

Background Sjögren’s syndrome (SjS) associated with systemic lupus erythematosus (SjS-SLE) was considered a standalone but often-overlooked entity. Purpose To assess altered spontaneous brain activity in SjS-SLE and SjS using amplitude of low-frequency fluctuation (ALFF). Material and Methods Sixteen patients with SjS-SLE, 17 patients with SjS, and 17 matched controls underwent neuropsychological tests and subsequent resting-state functional magnetic resonance imaging (fMRI) examinations. The ALFF value was calculated based on blood oxygen level dependent (BOLD) fMRI. Statistical parametric mapping was utilized to analyze between-group differences and multiple comparison was corrected with Analysis of Functional NeuroImages 3dClustSim. Then, the ALFFs of brain regions with significant differences among the three groups were correlated to corresponding clinical and neuropsychological variables by Pearson correlation. Results ALFF differences in the bilateral precuneus/posterior cingulate cortex (PCC), right parahippocampal gyrus/caudate/insula, and left insula were found among the three groups. Both SjS-SLE and SjS displayed decreased ALFF in the right parahippocampal gyrus, right insula, and left insula than HC. Moreover, SjS-SLE showed wider decreased ALFF in the bilateral precuneus and right caudate, while the SjS group exhibited increased ALFF in the bilateral PCC. Additionally, patients with SjS-SLE exhibited lower ALFF values in the bilateral PCC and precuneus than SjS. Moreover, ALFF values in the right parahippocampal gyrus and PCC were negatively correlated to fatigue score and disease duration, respectively, in SjS-SLE. Conclusion SjS-SLE and SjS exhibited common and different alteration of cerebral functional segregation revealed by AlFF analysis. This result appeared to indicate that SjS-SLE might be different from SjS with a neuroimaging standpoint.

The growing demands for optical anti-counterfeiting technology require the development of new environmentally friendly materials with single component, multimodal fluorescence and high stability. Herein, the Bi/Sb codoped Cs2Ag0.1Na0.9InCl6 lead-free double perovskite material is reported as an efficient multimodal luminescence material with excitation-wavelength-dependent emission. When excited by 360 nm UV light, dual-emission is observed at 455 and 560 nm, which comes from the 3P1-1S0 transition of Sb3+ ions and self-trapped excitons (STEs), respectively. Under the 320 nm UV lamp, the microcrystals show only a blue emission centered at 455 nm. Therefore, the Bi/Sb codoped Cs2Ag0.1Na0.9InCl6 double perovskite emits blue and yellow lights under the 320 and 360 nm UV lamp, respectively. Moreover, the obtained double perovskite shows a high PLQY up to 41% and excellent stability against both air and high temperature, which make it a promising anti-counterfeiting material.

Objectives Abnormal expression of C-C chemokine motif ligand 5 (CCL5) occurs in many solid tumors. The mechanism by which CCL5 affects hepatocellular carcinoma (HCC) tumor progression is still unclear. Here, we aimed to evaluate the expression level of CCL5 in HCC, explore the function of CCL5 in regulating the immune microenvironment, and elaborate its regulatory mechanism involved in tumor immunity. Methods The relative expression level of CCL5 in the tissues of HCC patients was measured by quantitative polymerase chain reaction (qPCR) assay, and its related protein concentration was exanimated by immunohistochemistry. Serum CCL5 expression was measured by enzyme-linked immunosorbent assay (ELISA) test. C57BL/6 wild-type (WT) mice and CCL5 knockout (Ccl5-/- ) mice were utilized to establish the DEN-induced HCC mouse model. The population of the immune cells was determined by flow cytometry, and peripheral serum IgM level was quantified by ELISA test. Results CCL5 expression was low in HCC patients' tissues and peripheral blood, and its expression was correlated with the overall survival (OS), recurrence, and metastasis. In the HCC mouse model, tumor sizes of the Ccl5-/- group were smaller than that of the WT group. Moreover, compared to the WT group, the population size of B cells in the tumor tissues of the Ccl5-/- group was smaller, while there was no significant difference in the other immune cell populations. Furthermore, the IgM level in the peripheral blood of the Ccl5-/- group was significantly lower than that of the WT group. Conclusions CCL5-deficiency in HCC reduces B-cell recruitment and decreases IgM secretion, leading to rapid tumor progression. This article is protected by copyright. All rights reserved.

Meibomian gland, the largest sebaceous gland of the body, is responsible for the biosynthesis of lipid layer of the tear film to prevent excessive evaporation. The loss of normal functions of meibomian gland, known as meibomian gland dysfunction (MGD), is a chronic disease and is the leading cause of dry eye symptoms in the clinics. Studies have found sex hormones, especially androgen, play vital roles in the regulation of the functions of meibomian gland. Recently, androgen has also been preliminarily applied in clinics for the treatment of MGD and showed promising results, especially in people with endogenous androgen deficiency. This review summarized the mechanisms of the function of androgen on meibomian gland based on molecular, animal, and clinical studies, and proposed evidence-based views about its potential applications for the treatment of MGD.

The liver is a vital metabolic and detoxifying organ and suffers diverse endogenous or exogenous damage. Hepatocyte mitochondria experience various structural and functional defects from liver injury, bearing oxidative stress, metabolic dysregulation, and the disturbance of mitochondrial quality control (MQC) mechanisms. Mitochondrial malfunction initiates the mitochondria-mediated apoptotic pathways and the release of damage signals, aggravating liver damage and disease progression via inflammation and reparative fibrogenesis. Removal of mitochondrial impairment or the improvement of MQC mechanisms restore mitochondrial homeostasis and benefit liver health. This review discusses the association of mitochondrial disorders with hepatic pathophysiological processes and the resultant potential of mitochondrion-targeting therapeutics for hepatic disorders. The recent advances in the MQC mechanisms and the mitochondrial-derived damage-associated molecular patterns (DAMPs) in the pathology and treatment of liver disease are particularly focussed.

Pitch angle greatly affects the performance of VAWT. Aiming at the maximum value of CT, this study first obtains the theoretical optimal angles of attack, i.e., 17.7° and −18.4° in upwind and downwind zones. A 2.5D model is simulated using Fluent to determine the magnitude and direction of induced velocity, and an initial pitch angle adjustment law is obtained combining the proposed local TSR. Compared with a prototype wind turbine, CP is increased by 2.69% after applying the initial adjustment law. The fitting curve angle δ at the abrupt part of pitch angle is analyzed. The smaller the angle δ is, the more evident the performance improvement of VAWT becomes. CP is increased by 11.68% when the angle is δ = 0°. On this basis, the adjustment law corresponding to δ = 0° of 100°

Aiming at reducing the energy absorption of the hydrogen cylinder for fuel cell vehicles in collision, which may lead to fierce explosion, this paper introduces an energy-absorbing protective shell combining with negative Poisson’s ratio (NPR) structure to the hydrogen cylinder. It adopts three honeycomb structures of the same mass and three kinds of aluminum alloy to determine the optimal structure and material scheme of the protective shell. Besides, sensitivity analysis is selected to screen the design parameters of the optimal structure scheme. Based on this, the overall mass of the designed protective shell, the specific energy absorption of NPR structure, the maximum impact force of rigid wall, and the maximum stress of the liner are taken to establish the optimization mathematical model. Furthermore, adaptive mutation genetic algorithm (AMGA), non-dominated sorting genetic algorithm-ii (NSGA-II), and adaptive simulated annealing algorithm (ASA) are exerted as the optimization algorithms separately to conduct optimization design and a comparison is made between the optimization results. The results show that the optimized energy-absorbing protective shell has better crashworthiness and energy absorption effect, which can effectively improve the collision safety of the hydrogen cylinder. It provides a new idea for the protection design of hydrogen storage system in fuel cells in the future.

In this work, an auto-afterpulsing correction system for single photon avalanche photodiode (SPAD) is proposed and implemented which can be used in real-time to minimize the afterpulsing effect in SPAD. The system is based on passive quenching circuit and retriggerable monostable multivibrator and can be used for any SPADs with different bias voltages (Vb). Two different SPADs are used to evaluate the proposed system, one is a planar SPAD fabricated in-house (Vb is around 26V) and the other one is a commercial reach-through SPAD (Vb is around 126V). Experimental results show that for both SPADs, the afterpulsing probabilities can be effectively reduced to below 0.5% from 5% and 4% respectively. Results also show that, with a proper time interval threshold, the SPAD’s afterpulsing probability can be maintained below 0.5% with the increasing of the bias voltage. The system developed is simple and can be used for effectively minimizing the afterpulsing effect for any SPADs that makes it very useful in low cost and compact SPAD based photon counting applications.

The north-south transitional zone in China mainly consists of the Qinling-Daba Mountains. It is the most important West-East geo-ecological space in China, containing protected areas vital for biodiversity conservation and ecological security of China. The protection and rational development of its natural habitat is of great significance to China’s ecological security and integration of protected areas based on mountain forest ecosystems on a global scale. In this study, five important types of protected areas in the transitional zone were selected, and their spatial patterns were analysed. Spatial analysis methods, such as kernel density estimation and accessibility analysis, were employed for both point and areal data, and focused on four aspects: land use scale, shift in the centre of gravity, spatial agglomeration, and accessibility. In addition, policy background and evolution of spatial and temporal characteristics of the protected area system in the transitional zone from 1963 to 2017 were also examined. We analysed the characteristics and geographical significance of the West-east corridor using the spatial pattern of the protected area system from the perspective of ecological and economic spaces. We focused on spatial shape, type intersection, and key areas to analyse the spatial overlap of the protected areas. Protected area establishment was divided into three stages: initial (1956–1980), rapid development (1981–2013), and national park transformation (2014–present). These stages reflected the change in the concept of ‘simple protection—sustainable use—integration and upgrade’ for protected areas of China. The spatial centre of gravity of the protection zone system was located in the west Qinling-Daba Mountains, and its high-density core exhibited a relatively stable N-shaped structure composed of four gathering areas. Affected by factors such as geographic environment and socio-economic development density, the average access time for protected areas was high (1.56 h); wetland parks and scenic areas are located closer to the city centre. As the West-east corridor in the transitional zone extends from west to east, there is a clear spatial dislocation between the development of protected areas and the intensity of human activities. During development, differentiated goal orientation should be adopted based on the idea of zoning and classified governance. With the advancement of the construction of protected areas, the spatial overlap of protected areas in the transition zone has become more prominent. At present, the spatially overlapped protected areas in the transitional zone remain prominent, with inclusion overlap being the most common, and forest parks exhibiting the highest probability of overlap with other protected areas, we should focus on in the integration process of the corridor-type ecological space based on the mountain forest ecosystem.

The rapid increase in the number of chemical science literature has brought challenges to researchers in search and data analysis. For many chemical scientific literature, extracting information from text and using knowledge is the focus of research. However, the existing Chinese text word segmentation methods have low recognition rate for chemical terms. The reason is that the addition of many new vocabulary and mixed professional vocabulary of Chinese and English brings challenges to word segmentation. In this paper, we propose a word segmentation method of Chinese chemical literature based on hybrid feature fusion learning Model(HFFLM). HFFLM first establishes chemical science corpus (Chem-pku) to train Chinese word segmentation (CWS) tasks. In addition, HFFLM uses BiLSTM and CNN to extract document features and fuse them. Then, HFFLM combines boundary features to construct conditional random field to train the end-to-end CWS model. In the end, HFFLM makes visual analysis of the word segmentation results. The experimental results indicate that HFFLM has high accuracy and recall rate, and is suitable for chemical industry vocabulary extraction with mixed Chinese and English.

Tamoxifen (TAM) resistance has indicated a significant challenge during endocrine therapy for hormone-sensitive breast cancer. Thus, it is significant to elucidate the molecular events endowing TAM resistance to endocrine therapy. In this study, we found that epithelial-mesenchymal transition (EMT) was an important event to confer TAM resistance, and attenuating EMT by elevating connexin (Cx) 43 expression could reverse TAM resistance. Specifically, Cx43 overexpression improved TAM sensitivity, while Cx43 depletion facilitated TAM insensitivity by modulating EMT in T47D TAM-resistant and -sensitive cells, and transplanted xenografts. Importantly, we found a novel reciprocal regulation between Cx43 and c-Src/PI3K/Akt pathway contributing to EMT and TAM resistance in breast cancer. Moreover, we identified that Cx43 deficiency was significantly correlated with poor relapse-free survival in patients undergoing TAM treatment. Therefore, Cx43 represents a prognostic marker and an attractive target for breast cancer treatments. Therapeutic strategies designed to increase or maintain Cx43 function may be beneficial to overcome TAM resistance.

Parkinson protein 7 (PARK7)/DJ-1 (DJ-1) is a redox sensitive molecular and stabilizer of nuclear factor erythroid 2-related factor 2 (Nrf-2). Nrf-2 regulates the downstream antioxidant defense system and exerts a significant function in patients with chronic obstructive pulmonary disease (COPD). Vitamin D receptor (VDR) is the nuclear receptor that regulates the downstream target genes. This study aimed to analyze the associations among pulmonary function, DJ-1, VDR and Nrf-2 in COPD patients. Serum was collected from 180 COPD patients and control subjects. Thirty-five lung tissues were obtained. DJ-1 was measured using ELISA and western blotting. Nrf-2 and VDR were detected by immunohistochemistry. Serum and pulmonary DJ-1 levels were lower in COPD patients than those in control subjects. Pulmonary VDR-positive nuclei were reduced in COPD patients. Nrf-2-positive nuclei were reduced in lung tissues of COPD patients. On the contrary, Nrf-2-related downstream target proteins were elevated in COPD patients. Further correlation analysis indicated that forced expiratory volume in 1 second (FEV1) was positively associated with pulmonary DJ-1, VDR and Nrf-2 in patients with COPD. In addition, there were positive correlations among DJ-1, VDR and Nrf-2 in lung tissues of COPD patients. In conclusion, DJ-1, VDR and Nrf-2 were decreased in COPD patients compared with control subjects. The reduction of DJ-1 and VDR associating with Nrf-2 downregulation may be involved in the process of COPD.

Background: Renal flare of lupus nephritis (LN) is strongly associated with poor kidney outcomes, and predicting renal flare and stratifying its risk are important for clinical decision-making and individualized management to reduce LN flare. Methods: We randomly divided 1,694 patients with biopsy-proven LN, who had achieved remission after treatment, into a derivation cohort (n = 1,186) and an internal validation cohort (n = 508), at a ratio of 7:3. The risk of renal flare 5 years after remission was predicted using an eXtreme Gradient Boosting (XGBoost) method model, developed from 59 variables, including demographic, clinical, immunological, pathological, and therapeutic characteristics. A simplified risk score prediction model (SRSPM) was developed from important variables selected by XGBoost model using stepwise Cox regression for practical convenience. Results: The 5-year relapse rates were 39.5% and 38.2% in the derivation and internal validation cohorts, respectively. Both the XGBoost model and the SRSPM had good predictive performance, with a C-index of 0.819 (95% confidence interval [CI]: 0.774–0.857) and 0.746 (95% CI: 0.697–0.795), respectively, in the validation cohort. The SRSPM comprised 6 variables, including partial remission and endocapillary hypercellularity at baseline, age, serum Alb, anti-dsDNA, and serum complement C3 at the point of remission. Using Kaplan-Meier analysis, the SRSPM identified significant risk stratification for renal flares (p < 0.001). Conclusions: Renal flare of LN can be readily predicted using the XGBoost model and the SRSPM, and the SRSPM can also stratify flare risk. Both models are useful for clinical decision-making and individualized management in LN.

Summary Smoking is mainly sustained by nicotine dependence (ND), which varies across ethnic groups principally due to genetic as well as environmental factors. The Fagerström Test for Nicotine Dependence (FTND) and biomarkers of tobacco exposure are two important approaches to assess ND. However, the relationship between ND and FTND of Chinese smokers has not been studied. The aim of this study was to assess the relationship between FTND scores and nicotine, cotinine, 3′-hydroxycotinine (3HC) and nicotine metabolite ratio (NMR, the concentration ratio of 3HC to cotinine) in Chinese smokers. FTND was carried out and general characteristics were collected using a self-administered smoking questionnaire with 289 smokers. Nicotine, cotinine and 3HC in urine were simultaneously determined by liquid chromatography–mass spectrometry/mass spectrometry (LC-MS/MS). The concentrations of nicotine, cotinine and 3HC in the urine of smokers with a high FTND score were higher than in the urine of those with a low FTND score. There were significant correlations between urinary biomarker and FTND scores. Except for FTND item 2 (difficulty to refrain), the other items showed significant associations with the urinary biomarkers. No relationship was found between the nicotine metabolite ratio (NMR, 3′-hydroxycotinine/cotinine) and FTND scores or general characteristics of the participants. In conclusion, biomarkers of tobacco exposure levels are significantly associated with FTND scores. However, FTND Item 2 and NMR were not found to be associated with nicotine dependence in Chinese smokers.

Feature selection is a technique to select a subset of the most relevant features for modeling training. In this paper, a new concept of TDR is firstly proposed to improve the classification accuracy. Then, a TDR-based algorithm for text classification is advanced. Finally, the extensive experiments are made on seven datasets (K1a, K1b, WAP, R52, R8, 20NewGroups, and Cade12) for two classifiers of Naive Bayes and Support Vector Machine. The experimental results indicate that the new approach can improve the classification accuracy by an average percent of 7.9%.

OBJECTIVE Unilateral complete cleft lip and palate (UCCLP) is associated with apparent nasal deformities before the cheilorrhaphy. The aim of this study was to determine whether preoperative correction techniques are effective in the treatment of nasal deformities in infant with unilateral UCCLP used by the systematic review and meta-analysis. METHODS We searched Medline, Cochrane Library, EMBASE, PubMed, and Chinese BioMedical Literature Database (CBM) until January 31, 2019, to identify studies that compared the effectiveness of preoperative correction techniques in the treatment of nasal deformities in infant with UCCLP. Two authors individually extracted the data and performed the quality assessments. The height of nasal columella, the width of the affected side nasal ala and the inclination of the nasal columella were evaluated. RESULTS Seven articles were incorporated into the systematic review, and 5 (274 participants) in the meta-analysis according to the inclusion criteria. Thermionic electron sources are scaled down to the microscale on a chip and batch fabricated on 4-in silicon wafers by utilizing microfabrication technologies and exploiting carbon nanotubes as microscopic filaments of thermionic electron emission. The microfabricated on-chip thermionic electron sources not only satisfy the metrics of compactness and ease of batch fabrication, but also exhibit the advantages of good performance reproducibility (±6.9% variation over 100 test cycles under a driven voltage of 3.5 V) and high emission stability (fluctuation < 5% for emission current level of ≈10−8 A over 900 s) under a relatively low vacuum condition (10−4–10−2 Pa). Furthermore, to extract electrons and tune emission current, an extraction gate with a mesh is monolithically integrated with the thermionic electron sources using anodic bonding technique. The integrated electron sources exhibit a strong gate controllability and a considerable electron transmission ratio of ≈76% through the extraction gate. All these results make our devices a promising type of on-chip electron source in the applications of miniature vacuum electronic devices/systems.

Background:. Although breast-conserving surgery is one of the standard treatments for breast cancer, few studies have assessed its recent implementation in China. We aimed to clarify the current real-world status of breast-conserving surgery in China. Methods:. This cross-sectional survey relied on data collected by the Chinese Society of Breast Surgery (CSBrS) to examine patients who underwent this surgery between January 2018 and December 2018. The survey was conducted using a uniform electronic questionnaire to collect information, including clinical and pathological data on these patients. Results:. Overall, 4459 breast-conserving surgeries were performed in 34 member units of CSBrS, accounting for 14.6% of all breast cancer surgeries performed in these units during the study period. In patients who underwent breast-conserving surgery with information on tumor size available, more than half (61.2%) of the tumors were smaller than 2 cm in diameter, and only 87 (3.2%) tumors were larger than 4 cm in diameter. Among patients who underwent breast-conserving surgeries, 457 (10.2%) patients received neoadjuvant therapy before the surgery. Among patients with a reported margin width, 34 (2.0%) patients had a margin of ≤2 mm, and 1530 (88.2%) of them had a margin of >5 mm. Conclusions:. This study demonstrated the rates of breast-conserving surgery in member units of the CSBrS, and introduced the characteristics and surgical margins of patients who underwent this surgery. Microbial ecosystem comprises a complex community in which bacteria interact with each other. The potential roles of the intestinal microbiome play in human health have gained considerable attention. The imbalance of gut microbial community has been looked to multiple chronic diseases. Cardiovascular diseases (CVDs) are leading causes of morbidity worldwide and are influenced by genetic and environmental factors. Recent advances have provided scientific evidence that CVD may also be attributed to gut microbiome. In this review, we highlight the complex interplay between microbes, their metabolites, and the potential influence on the generation and development of CVDs. The therapeutic potential of using intestinal microbiomes to treat CVD is also discussed. It is quite possible that gut microbes may be used for clinical treatments of CVD in the near future.

Background Lymphoplasmacyte-rich meningioma (LPRM) is one of the rarest variants of meningioma and is classified as grade I (benign) tumor. It is characterized by abundant infiltrates of lymphocytes and plasma cells. Here, we report an extremely rare case of LPRM with an atypical imaging finding of multiple cysts around a solid mass. Case summary The patient was a 36-year-old man with intermittent headache, dizziness, and vomiting for 2 years. Computed tomography and magnetic resonance imaging presented a cystic solid mass in the right frontal lobe with heavy peritumoral edema and obvious contrast enhancement. The patient was treated with right frontotemporal craniotomy, and gross total resection of the tumor was achieved without adjuvant therapy. There was no clinical or neuroradiological evidence of recurrent or residual tumor for 3 years after initial surgery. Conclusion LPRM is one of the rarest variants of meningioma. Although, the mass of this case had common features, multiple cysts with nonuniform size and thin wall around the solid part are uncommon imaging finding, increasing the rate of misdiagnosis. The definitive diagnosis of LPRM relies on histopathological findings.

Pancreatic cancer is a disease with a high mortality rate. Although survival rates for different types of cancers have improved in recent years, the five-year survival rate of pancreatic cancer stands at 8%. Moreover, the current first-line therapy, gemcitabine, results in low remission rates and is associated with drug resistance problems. Alternative treatments for pancreatic cancer such as surgery, chemotherapy and radiation therapy provide marginal remission and survival rates. This calls for the search of more effective drugs or treatments. Traditional Chinese medicine contains numerous bioactive ingredients some of which show activity against pancreatic cancer. In this review, we summarize the mechanisms of five types of traditional Chinese medicine monomers. In so-doing, we provide new potential drug candidates for the treatment of pancreatic cancer.

Purpose To determine the role of NLRP3 inflammasome activation in low-dose radiation-induced radiation pneumonitis (RP) and to assess whether inhibition or deletion of the NLRP3 inflammasome is critical for conferring protection against RP. Methods and Materials The human monocytic THP-1 cells were treated with increasing doses of radiation to assess the activation of NLRP3 by Western blot and enzyme-linked immunosorbent assay. Reactive oxygen species (ROS) production was measured by flow cytometry, with or without ROS inhibitor treatment. A mouse thoracic radiation model that received different doses of radiation was used, and the lung tissues of thoracic-irradiated nlrp3-/- and wild-type C57BL/6 mice were examined by hematoxylin and eosin and immunofluorescence staining. The concentrations of cytokines in the bronchoalveolar lavage fluid were measured by enzyme-linked immunosorbent assay and Luminex multiplex assays. Lipopolysaccharide (LPS) was administered intranasally 28 days after thoracic irradiation, and NLRP3 inhibitor, MCC950 was administered intraperitoneally after irradiation at 2 different doses. Results (1) The NLRP3 inflammasome was activated in 2 Gy irradiated THP-1 cells; NLRP3 and cleaved-caspase-1 levels were not associated with dose escalation of irradiation. (2) Activation of the NLRP3 inflammasome was mediated by ROS, and ROS inhibitor treatment decreased the production of IL-1β and IL-18 in vitro. (3) NLRP3 was activated in mouse lungs by irradiation at 2 Gy, 4 Gy, and 16 Gy, and NLRP3 activation was continuous for 8 weeks. (4) NLRP3 deletion protects against LPS-mediated monocyte infiltration in the mouse lung. (5) The administration of MCC950 decreased the inflammation score of the mice irradiated with 2 Gy or 16 Gy in vivo. Conclusions Our results indicate that the NLRP3 inflammasome is activated by low-dose irradiation both in vitro and in vivo. Inhibition or deletion of NLRP3 can specifically alleviate the mouse lung inflammation caused by radiation and LPS treatment. This study reveals the mechanism of low-dose radiation therapy–induced RP and offers a possible treatment strategy.

Distribution of SARS-CoV-2 virus and pathological features of multiple organs in COVID-19 patients remains unclear, which interferes with the improvement of COVID-19 diagnosis and treatment. In this article, we summarize the pathological findings obtained from systematic autopsy (37 cases) and percutaneous multiple organ biopsy (“minimally invasive autopsy”, 54 cases). These findings should shed light on better understanding of the progression of COVID-19 infection and the means of more effective intervention.

Objective To explore the effects of electroacupuncture pretreatment on mitochondrial energy metabolism in the rats with myocardial ischemia reperfusion injury (MIRI). Methods A total of 60 SPF Wistar rats were randomly divided into a sham-operation group (sham group), a myocardial ischemia reperfusion injury group (MIRI group) and an electroacupuncture pretreatment group (EA group), 20 rats in each one. The rats in the sham group and the MIRI group were binded for 7 days, once a day, 20 min each time. On the 8th day, the sample was collected after the heart exposed for 50 min in thoractomy in the sham group and the sample was collected after ischemia for 20 min and reperfusion for 30 min in thoractomy in the MIRI group. In the EA group, the pretreatment intervention with electroacupuncture was applied at "Neiguān (内关PC6)", "Guānyuan (关元CV4)" and "Zusānlĭ (足三里ST36)" in the rats for 7 days, once a day, 20 min each time. On the 8th day, after ischemia for 20 min and reperfusion for 30 min in thoractomy, the sample was collected in the EA group. The changes in STⅡ segment of electroacardiogram (ECG) were observed and measured. Using enzyme-linked immunosorbent assay (ELISA), the concentrations of cardiac troponin T (cTnT) and cardiac troponin I (cTnI)were detected. Using nitro blue tetrazolium chloride monohydrate (NBT) staining, the myocardial infarction weight percentage was measured. Using ELISA, the concentrations of mitochondrial adenosine monophosphate (AMP), adenosine diphosphate (ADP)and adenosine triphosphate (ATP)were detected. Results (1) STⅡ changes: in 20 min of ligation, compared with the sham group, the STⅡ segment of electrocardiograph (ECG) was elevated significantly in the MIRI group and EA group (both P Conclusions Electroacupuncture pretreatment reduces the elevation of ECG STⅡ segment, decreases the concentrations of myocardial injury markers, cTnT and cTnI and regulates the transfer among AMP, ATP and ADP. The protective effect of electroacupuncture pretreatment may result from the regulation of mitochondrial energy metabolism.

Biological signals generated during various biological processes are critically important for providing insight into the human physiological status. Recently, there have been many great efforts in developing flexible and stretchable sensing systems to provide biological signal monitoring platforms with intimate integration with biological surfaces. Here, this review summarizes the recent advances in flexible and stretchable sensing systems from the perspective of electronic system integration. A comprehensive general sensing system architecture is described, which consists of sensors, sensor interface circuits, memories, and digital processing units. The subsequent content focuses on the integration requirements and highlights some advanced progress for each component. Next, representative examples of flexible and stretchable sensing systems for electrophysiological, physical, and chemical information monitoring are introduced. This review concludes with an outlook on the remaining challenges and opportunities for future fully flexible or stretchable sensing systems.

Electronic devices with configurability to providing multiple functions are of great interests for their superior adaptability to the ever-changing and multifarious application scenarios. Here, we report flexible integrated circuits (ICs) possessing configurable functions constructed with dual-material gate (DMG) devices based on carbon nanotube thin films, which can serve as either transistors or diodes, on a 2-μm-thick parylene substrate. When configured as a transistor, the DMG device has great advantages over the normal-gated (NG) devices regarding the current on/off ratio (Ion/Ioff), the subthreshold swing (SS) and the drain-induced barrier lowering (DIBL) due to the regulated energy band distribution in channel area. When operating as a diode, a typical DMG device demonstrates a sufficient rectification ratio of 8 × 10 4 and a diode-on-current of over 26 μA. Scalable manufacturing of DMG devices was also demonstrated with great uniformity both in diode and transistor configurations. Finally, multifunctional integrated circuits, which can dynamically switch their function from rectifier to follower or from OR gate to voltage adder by changing controlling signals, were constructed. The functional-configurability, together with scalable manufacturing and the realization on ultrathin flexible substrates, will open up great opportunity for the future environmentally-adaptive system in the field of flexible electronics.

Background The objectives of present study were to examine the level of disagreement between self-reported and objective measurement of sensitization to common inhalant allergens, and to explore the potential risk factors that might contribute to this discrepancy. Methods A total of 215 children were enrolled from pediatric clinics at a tertiary pediatric center in Beijing, China. A survey questionnaires regarding self-perceived sensitization was completed by participants’ parents/caregiver, meanwhile, skin prick testing(SPT) was performed as objective assessment of sensitization. Extent of agreement between self-reported versus SPT-measured sensitization to individual allergen was calculated using Cohen’s kappa (κ) coefficient. Multivariable regression analyses were used to determine the factors associated with discrepancy between self-reported and objective measurement of sensitization. Results 119(55.3%) patients have reported to be sensitized to at least one of inhalant allergen, whereas 167(77.7%) patients had a positive skin testing response. Agreement between self-perceived and actual aeroallergen sensitization was moderate for mites(κ = 0.518) and grass pollen mix(κ = 0.451), moreover, fair agreement was observed for mold(κ = 0.316) and cockroach(κ = 0.297), respectively. There was a least agreement between perceived and actual sensitization observed for pet dander, with a kappa coefficient of 0.005. Subjects’ age, atopy history, ownership of pet may increase the risk of disagreement, moreover, background factors of informant, like: age, education level, and the relationship with enrolled subjects, were linked to the incidence of disagreement between self-reported sensitization in comparison with SPT results. Conclusion Questionnaire-based self-assessment is easy way to collect clinical information on allergen sensitization; however, the accuracy of questionnaire-derived information is more likely to be influenced by respondent’s background factors. The information from the questionnaire report is considered to be more reliable when in combination with objective assessment of sensitization, including blood IgE testing and SPT.

Background HOXD9, a Hox family member, is involved in cancer growth and metastasis. But, its regulation mechanism at the molecular level particularly in colorectal cancer (CRC), is mostly unknown. Methods The HOXD9 protein expression levels were analyzed using immunofluorescence, immunohistochemistry (IHC) assays, and western blot. The in vivo and in vitro roles of HOXD9 in CRC were determined using colony formation and EdU incorporation, CCK‐8, wound scratch and transwell invasion assay, and animal models. Results Expression of HOXD9 was higher in CRC than in matched healthy tissues. High expression of HOXD9 has significantly associated with the American Joint Committee on Cancer (AJCC) stages, tumor differentiation, lymph node metastasis, and other serious invasions, and it had a poor prognosis. In vitro, HOXD9 encouraged proliferation, movement and EMT processes in cells of CRC. Also, TGF‐β1 promoted the expression of HOXD9 and this effect was dependent on the dose and downregulation of HOXD9 repressed TGF‐β1 ‐induced EMT. In vivo, HOXD9 promoted the invasive and metastasis of CRC cells via orthotopic implantation. Conclusions The ectopic expression of HOXD9 promoted the invasion metastasis in cells of the colorectal tumor by induction of EMT in vitro and vivo., HOXD9 is overexpressed in CRC and increases tumorigenicity and tumor proliferation. Also, ectopic HOXD9 expression influences EMT and tumor spread both in vivo and in vitro. Therefore, the HOXD9 gene could act as an assuring therapeutic approach for treating patients suffering from CRC.

Video display processing is an indispensable module in a variety of multimedia systems. With the increase of video format to 4K $\times$ 2K@60fps, the design of efficient display processing circuit faces severe challenges. This paper presents a video display processor which supports a variety of video formats and integrates multiple advanced algorithms for perpetual image quality improvement. By leveraging algorithm and architecture co-design, this paper efficiently implements these computational and memory intensive display processing algorithms in a single chip. In particular, we describe three representative design modules in detail, including motion compensated frame rate up-conversion, content-adaptive contrast enhancement, and backlight local dimming. Extensive experiments demonstrate that the proposed design modules outperform the previous works in terms of either image quality or hardware efficiency. The overall chip is fabricated in GF 55nm CMOS technology, and can work at the maximum operating frequency of 594 MHz. The maximum input and output video formats reach up to 4K $\times$ 2K@60fps.

Nasopharyngeal carcinoma is an endemic disease with a high prevalence in Southeast Asia, Mediterranean countries, and Northern Africa. With substantial advances in screening and diagnosis, increasingly more early-stage (stage I~II) patients are being diagnosed. The undebated treatment modality for stage I patients is radiotherapy alone. However, controversies exist for patients with stage II disease, mostly revolving around the management of chemotherapy. However, the use of intensity-modulated radiotherapy for the treatment of nasopharyngeal carcinoma has increased recently, which has drastically improved survival outcomes. Thus, many oncologists have considered omitting chemotherapy for stage II patients in the intensity-modulated radiotherapy era. Unfortunately, prospective studies comparing concurrent radio-chemotherapy with intensity-modulated radiotherapy alone are limited. Notably, stage II nasopharyngeal carcinoma consists of three subgroups, among which stage T2N1M0 disease is unique and potentially warrants additional treatment including chemotherapy. Additionally, molecular biology techniques are advancing at an incredible speed. Instead of adopting a one-size-fits-all recommendation, exploring potential predictive biomarkers to select patients who are likely to derive benefit from chemotherapy is a better choice. In this review, we summarize the data from studies and reviews regarding chemotherapy for stage II nasopharyngeal carcinoma in the intensity-modulated radiotherapy era and discuss chemotherapy utility. Eventually, we conclude that IMRT alone may be sufficient for stage II nasopharyngeal carcinoma, but this needs to be verified by prospective studies in the near future, the evidence collected thus far suggests that concurrent chemo-radiotherapy without induction or adjuvant chemotherapy is yet to be necessary for patients with stage II disease.

We theoretically demonstrate a broadband mode converter and multiplexer based on plasmonic waveguides loaded with structured dielectrics. The proposed device can realize conversions between a fundamental TM0 mode and a first order TM1 mode, as well as (de)multiplex them with another TM0 mode. Our design exhibits as wide as 400 nm bandwidth and as short as 6~7 μm coupling length. This work has potential application in high density photonic integrated devices for both computing and communication applications.

Delivery by cesarean section (CS) is linked to an increased incidence of food allergies in children and affects early gut microbiota colonization. Furthermore, emerging evidence has connected disordered intestinal microbiota to food allergies. Here, we investigated the impact of CS on a rat model for food allergy to ovalbumin (OVA). Rats delivered by CS were found to be more responsive to OVA sensitization than vaginally born ones, displaying a greater reduction in rectal temperature upon challenge, worse diarrhea, and higher levels of OVA-specific antibodies and histamine. 16S rRNA sequencing of feces revealed reduced levels of Lactobacillus and Bifidobacterium in the CS rats. Preventative supplementation with a probiotic combination containing Lactobacillus and Bifidobacterium could protect CS rats against an allergic response to OVA, indicating that the microbiota dysbiosis contributes to CS-related response. Additionally, probiotic intervention early in life might help to rebuild aberrant Th2 responses and tight junction proteins, both of which have been linked to CS-related high allergic reactions. Taken together, this study shows that disordered intestinal microbiota plays an essential role in the pathogenesis of food allergy mediated by CS. More importantly, interventions that modulate the microbiota composition in early life are therapeutically relevant for CS-related food allergies.
