Hemophagocytic lymphohistiocytosis (HHL), a severe hyperinflammatory syndrome caused by aberrant activation of macrophages and cytotoxic T cells, is clinically manifested as a febrile onset along with cytopenias, high ferritin serum level and splenomegaly. In adult patients, secondary causes of it should be looked for, such as autoimmune and infectious diseases and neoplasms. Prompt initiation of treatment is important due to the high mortality of this syndrome.A case of a 53-year-old patient diagnosed with HHL associated with T-cell lymphoma is presented as a sudden onset of several symptoms and signs of HHL, along with infectious complications.Results: Once the diagnosis of HHL was made, treatment with Etoposide and Dexamethasone was started, and with the diagnosis of T-cell lymphoma established, chemotherapy treatment with a CHOEP scheme was started, with a favorable initial evolution.As HHL is a rare entity associated with high mortality, initial suspicion must be high when facing an acute onset of fever, cytopenia and splenomegaly. Nonetheless, diagnosing HHL is challenging and often appear superimposed on multiple infectious diseases. Ante su diagnóstico en adultos se deben buscar causas secundarias como neoplasias hematológicas. El inicio precoz del tratamiento es importante debido a su elevada mortalidad.Se presenta el caso de una paciente de 53 años de edad que se le realizó diagnóstico de LHH asociada a Linfoma de células T. La forma de presentación de dicho cuadro fue de manera abrupta con varios síntomas y signos de dicho síndrome, además de múltiples complicaciones infecciosas asociadas.Ante el diagnóstico inicial de LHH se inició tratamiento con etopósido y dexametasona, y al obtener el diagnóstico de Linfoma de células T, se instauró tratamiento quimioterápico con esquema de CHOEP, con evolución inicial favorable.La LHH es una entidad poco frecuente asociada a una elevada mortalidad, por lo que su índice de sospecha debe de ser elevado ante un cuadro clínico y analítico compatible. Sin embargo, su diagnóstico constituye un gran desafío clínico, pudiendo presentarse en algunas ocasiones superpuesto a cuadros infecciosos múltiples. El inicio precoz del tratamiento es importante debido a su alta mortalidad.

O linfoma ou linfossarcoma é uma neoplasia hematopoiética de elevada prevalência em caninos que apresenta diferentes características e classificações, com predisposição racial e genética, sendo o linfoma multicêntrico o mais acometido. Objetivou-se com esse trabalho relatar um caso de neoplasia linfóide ocorrida, onde o paciente ainda se encontra livre de doença superando as estatísticas. A suspeita diagnóstica de neoplasia foi confirmada pela citologia e no tratamento foram utilizadas medicações como Vincristina, Ciclofosfamida, Doxorrubicina e Prednisona.

Introduction. The identification and treatment of patients with hyperkalemia is necessary to prevent the development of arrhythmias. Pseudohyperkalemia is most commonly due to specimen haemolysis and is often recognised by laboratory scientists who subsequently report test results with cautionary warnings. The authors present a case of pseudohyperkalemia in a patient with chronic lymphocytic leukaemia. Report case: the technical factors and method of transport are a potential cause of pseudohyperkalemia. Pseudohyperkalemia has been associated with hyperleukoctosis, in cancer patient populations, more commonly in CLL in adults, but also acute lymphoblastics leukemia in children. This places the patient at risk of unnecessary and potentially dangerous treatments. Conclusion: Physicians should consider pseudohyperkalemia as the underlying cause of elevated potassium levels in patients with malignant leucocytosis who do not have signs or symptom of systemic hyperkalemia. Introducción. La identificación y el tratamiento de pacientes con hiperpotasemia son necesarios para prevenir el desarrollo de arritmias. La pseudohiperpotasemia se debe más comúnmente a la hemólisis de la muestra y a menudo es reconocida por los laboratoristas que posteriormente informan los resultados de las pruebas con advertencias de precaución. Los autores presentan un caso de pseudohiperpotasemia en un paciente con leucemia linfocítica crónica. Reporte de caso: los factores técnicos y el método de transporte son una causa potencial de pseudohiperpotasemia. La pseudohiperpotasemia se ha asociado también con hiperleucoctosis, en poblaciones de pacientes con cáncer, más comúnmente en Leucemia linfocítica crónica en adultos, pero también con leucemia linfoblástica aguda en niños. Esto pone al paciente en riesgo de tratamientos innecesarios y potencialmente peligrosos. Conclusión: Los médicos deben considerar la pseudohiperpotasemia como la causa subyacente de los niveles elevados de potasio en pacientes con leucocitosis maligna que no presentan signos o síntomas de hiperpotasemia sistémica.

Resumen Introducción. Los trastornos linfoproliferativos después de un trasplante se caracterizan por la proliferación descontrolada de linfocitos como consecuencia del tratamiento inmunosupresor posterior a este. Objetivo. Caracterizar clínica y patológicamente los casos de trastornos linfoproliferativos después de trasplante (Post-Transplant Lymphoproliferative Disorders, PTLD) en una cohorte de pacientes adultos con trasplante de hígado atendidos a lo largo de 15 años en el Hospital Universitario Fundación Santa Fe de Bogotá. Materiales y métodos. Se hizo un estudio observacional retrospectivo a partir de la revisión de las bases de datos de la Unidad de Trasplante Hepático y del Departamento de Patología del Hospital en busca de los casos de PTLD diagnosticados durante el periodo de estudio. Se recolectó la información epidemiológica, clínica y patológica, y se adelantaron los análisis estadísticos. Resultados. Durante el periodo de estudio, hubo 572 pacientes con trasplante de hígado, la incidencia de trastornos linfoproliferativos fue de 2,44 %, el 79 % en hombres, y la edad promedio en el momento del diagnóstico fue de 62,5 años. El 71 % de los casos se presentó durante los primeros 12 meses después del trasplante y el mismo porcentaje fue seropositivo para el virus de Epstein-Barr (EBV). El fenotipo patológico más frecuente fue el monomorfo y la mayoría de los tumores se detectaron en el hilio hepático. La supervivencia al año fue del 50 %. Conclusiones. Llamó la atención el alto porcentaje de casos de presentación temprana, así como la gran frecuencia de seropositividad para el EBV tanto en los donantes como en los receptores. Deben adelantarse estudios más detallados para una mejor comprensión de esta enfermedad en el país. Este es el primer análisis clínico y patológico de PTLD en pacientes con trasplante de hígado adelantado en Colombia hasta la fecha. Abstract Introduction: The post-transplant lymphoproliferative disorders (PTLD) are characterized by an uncontrolled pathological lymphoid proliferation as a consequence of transplant immunosuppression therapy. Objective: To characterize the clinical and pathological characteristics of PTLD in a cohort of adult patients with liver transplant during a 15 year period at the Hospital Universitario Fundación Santa Fe de Bogota. Materials and methods: We conducted an observational retrospective study by searching for the PTLD cases diagnosed during the study period in the databases of the Liver Transplantation Unit and the Pathology Department. We collected the epidemiological, clinical, and pathological information and performed the corresponding statistics analyses. Results: During the research period, 572 patients were transplanted; the incidence of PTDL was 2.44%; 79% of them were man and the average age at the time of diagnosis was 62.5 years; 71% of the cases were diagnosed during the first year after the transplant and the same percentage EBV-seropositive patients. The most frequent pathological phenotype was monomorphic and the majority of tumors was detected in the hepatic hilum. The one-year survival was 50%. Conclusion: The high proportion of early cases and the high frequency of Epstein-Barr virus seropositivity both in donors and receptors drewour attention. More studies are necessary to have a better understanding of this condition in Colombia. This is the first PTLD clinical and pathological analysis in liver-transplant patients from Colombia to date.

Purpose: The aim of this study is to assess the diagnostic value of magnetic resonance imaging (MRI), alone or combined with positron emission tomography (PET), for evaluation of mediastinal disease in lymphoma in comparison with PET/computed tomography (CT). Material and methods: Eighteen prospective studies with sample size ranging between 17 and 140 patients were included in this analysis. MRI, regardless of the sequence obtained, where used for the evaluation of lymphoma. Histopathology results and clinical or imaging follow-up were used as the reference standard. Studies were excluded if the sample size was less than 15 cases or if less than 15 lymph nodes assessment were presented. Papers not reporting sufficient data were also excluded. Results: As compared to PET/CT, MRI proved to be an alternative to use in evaluation of mediastinal disease of lymphoma. MRI has high sensitivity and specificity for nodal and extranodal involvement in lymphoma and the agreement between MRI with the standard technique was high. The ALARA principle reinforces these findings because it is a radiation-free technique and its use is preferred in young population at risk of being subjected to several studies. Conclusion: MRI seems to be a promising and free-radiation alternative due to its high sensitivity and specificity, as well as its high level of agreement with the standard reference. This suggests that MRI could play a significant role in lymphoma study. Resumen: Objetivo: El objetivo de este estudio es evaluar el valor diagnóstico de la resonancia magnética (RM), sola o combinada con la tomografía por emisión de positrones (PET), para la valoración de la afectación mediastínica en el linfoma en comparación con la tomografía computarizada (TC)/PET. Material y métodos: Se incluyeron en el análisis 18 estudios prospectivos con un tamaño de muestra que oscilaba entre los 17 y los 140 pacientes. Se utilizó la resonancia magnética, independientemente de la secuencia obtenida para la evaluación del linfoma. Los resultados de histopatología y el seguimiento clínico o de imagen se utilizaron como estándar de referencia. Se excluyeron los estudios si el tamaño de la muestra era inferior a 15 casos, o si se presentaron evaluaciones de menos de 15 ganglios linfáticos. Los artículos que no proporcionaban datos suficientes también fueron excluidos. Resultados: En comparación con PET/TC, la RM demostró ser una alternativa útil en la evaluación de la afectación mediastínica del linfoma. La RM tiene una alta sensibilidad y especificidad para detectar afectación nodal y extranodal en el linfoma y la concordancia entre la RMy la técnica estándar fue alta. El principio ALARA refuerza estos hallazgos porque es una técnica libre de radiación y su uso se prefiere en la población joven en peligro de ser sometida a varios estudios. Conclusión: La RM parece ser una alternativa prometedora y libre de radiación debido a su alta sensibilidad y especificidad, así como a su alto nivel de concordancia con la referencia estándar. Esto sugiere que la RM podría desempeñar un papel importante en el estudio del linfoma.

Lymphomas are the tumors most frequently associated with the death or euthanasia of dogs in most parts of the world. In dogs, they almost always occur as disseminated (multicentric lymphoma), gastrointestinal (alimentary lymphoma), or nodal mediastinal (mediastinal lymphoma) diseases. However, other uncommon presentations can occasionally occur. This study aimed to establish the prevalence of these unusual types of lymphoma and demonstrate to veterinary pathologists how they present pathologically. From a total of 100 cases of lymphoma in dogs diagnosed between 1965 and 2017, 16 cases (16/100) were considered by us as non-traditional presentations of the disease: follicular lymphoma, (5/100), peripheral T-cell lymphoma, NOS (2/100), angiocentric lymphoma (2/100), intravascular large T-cell lymphoma (2/100), lymphomatoid granulomatosis (1/100), anaplastic large-cell lymphoma (1/100), hepatosplenic T-cell lymphoma (1/100), and chronic small B-cell lymphocytic lymphoma, intermediate type (1/100). We hope that the results presented here can help veterinary pathologists to recognize such cases of “atypical lymphoma” in their diagnostic routines. RESUMO: Linfomas são os tumores mais associados a morte ou eutanásia de cães na maior parte do mundo. Nessa espécie animal ocorrem quase sempre como uma doença disseminada (linfoma multicêntrico), gastrintestinal (linfoma alimentar) ou nodal mediastinal (linfoma mediastínico), entretanto, ocasionalmente, outras apresentações bem menos comuns podem ser encontradas. O objetivo deste artigo é estabelecer a prevalência desses pouco usuais tipos de linfoma e demonstrar aos patologistas veterinários como eles se apresentam anatomopatologicamente. De um total de 100 casos de linfoma em cães diagnosticados entre os anos de 1965 e 2017, 16 casos (16/100) foram considerados como apresentações não tradicionais da doença: linfoma folicular (5/100), linfoma de células T periférico inespecífico (2/100), linfoma angiocêntrico (2/100), linfoma intravascular de grandes células T (2/100), granulomatose linfomatoide (2/100), linfoma de grandes células anaplásicas (1/100), linfoma hepatoesplênico de células T (1/100) e linfoma linfocítico crônico de pequenas células B - tipo intermediário (1/100). Esperamos que os resultados aqui demonstrados auxiliem patologistas veterinários a reconhecerem tais casos de “linfomas atípicos” em suas rotinas diagnósticas.

El síndrome de lisis tumoral es considerado la urgencia oncohematológica más común tanto en población pediátrica como en adultos. Se encuentra principalmente en neoplasias hematológicas de rápido recambio celular, aunque también en tumores sólidos de gran tamaño, con alta sensibilidad a la quimioterapia, metástasis y con niveles elevados de ácido úrico y deshidrogenasa láctica sin embargo se puede presentar en todos los tipos de tumores tanto posterior a la terapia citotóxica, la radiación, la cirugía o de manera espontánea. El principal mecanismo fisiopatológico se fundamenta en la lisis de células tumorales, y la subsecuente liberación masiva de componentes intracelulares al torrente sanguíneo. Lo que genera alteraciones metabólicas como trastornos hidroelectrolíticos (hiperpotasemia, hiperfosfatemia, hipocalcemia secundaria), hiperuricemia debido al metabolismo de los ácidos nucleicos y lesión renal aguda; dichas alteraciones producen manifestaciones clínicas a nivel muscular, renal, cardiaco y sistema nervioso central, que pueden progresar a falla multiorgánica y muerte. El diagnóstico de síndrome de lisis tumoral se basa en criterios clínicos y de laboratorio; además es fundamental la estratificación del riesgo en todos los pacientes. En cuanto al tratamiento consiste en tratar la lesión renal aguda y las alteraciones hidroelectrolíticas que son la fisiopatología del síndrome, esto se consigue con hidratación abundante y mantenimiento de la diuresis, los inhibidores de la urato-oxidasa y en casos refractarios con la terapia de diálisis. El síndrome de lisis tumoral es potencialmente mortal si no se trata y tiene como principal predictor de morbimortalidad la lesión renal aguda asociada y el pronóstico depende de la severidad del cuadro.

Introdução: Os linfomas são um conjunto de neoplasias linfoides em que há o acúmulo de linfócitos malignos nos linfonodos, causando linfonodomegalia. Os linfomas são responsáveis por 3% a 4% dos cânceres no mundo. Estimativa mundial apontou 390 mil novos casos e 199 mil óbitos por linfoma não Hodgkin e 65 mil novos casos e 25 mil óbitos por Linfoma de Hodgkin no ano de 2012. Sendo a busca por cuidados terapêuticos e a tentativa de solucionar problemas de saúde conduzem indivíduo e familiares por caminhos compreendidos como itinerário terapêutico Objetivo: Conhecer o itinerário terapêutico de pacientes portadores de linfoma atendidos na rede pública por serviço de saúde especializado. Material e Métodos: Estudo prospectivo e descritivo com dados coletados em um hospital de referência em oncologia na Zona da Mata Mineira, no estado de Minas Gerais, Brasil. Resultados: Participaram do estudo 32 pacientes, sendo 18 do sexo masculino (56,3%), com idade entre 7 e 80 anos, caracterizados principalmente por baixos níveis de escolaridade e renda. Prevaleceu o linfoma não Hodgkin representado por 23 sujeitos (71,9%). Um total de 24 pacientes (75%) necessitou passar por três ou mais médicos até a obtenção do diagnóstico definitivo. O tempo médio entre o aparecimento dos sintomas e a realização do diagnóstico foi de 9,77 meses. A baixa renda dos pacientes contribuiu para a demora na busca ou obtenção do primeiro atendimento à saúde (p = 0,05) e consequente diagnóstico do linfoma. Conclusão: A dificuldade de acesso aos serviços de atenção à saúde e a baixa suspeita diagnóstica para a doença por parte dos profissionais possivelmente influenciaram o atraso no estabelecimento do diagnóstico.

International audience; High dose methotrexate (HD-MTX) based chemoimmunotherapy is a central part of the standard approach to treatment of primary central nervous system lymphoma (PCNSL). Renal dysfunction leads to delayed MTX complete elimination and critical MTX concentration. Despite the recommendations, hemodialysis status should not exclude HD-MTX. We report the case of a 64 years old woman on chronic hemodialysis with PCNSL successfully treated with HD-MTX-based chemoimmunotherapy with an adjusted dose of 100 mg/m(2), instead of the usual dose of 3500 mg/m(2), and daily hemodialysis started 24 h later. The patient had no significant toxicity and was in complete remission at 1 year after the end of the treatment. We argue that ESRD is not an absolute pitfall to the use of HD-MTX for hematological malignancies. Experts should consider the use of adjusted dose at 100 mg/m(2) as a viable therapeutic modality in ESRD patients. (C) 2021 Sociedad Espanola de Nefrologia. Published by Elsevier Espana, S.L.U.

Resumen Introducción. El linfoma puede afectar el tracto gastrointestinal de manera primaria o secundaria, y representa hasta el 2 % de todas las neoplasias malignas del intestino delgado y colon. El tracto gastrointestinal es la ubicación extraganglionar primaria más común en el linfoma no Hodgkin. Métodos. Se realizó una búsqueda de la literatura en las principales bases de datos académicas, con revisión de textos publicados sobre el tema en los últimos 5 años. Discusión. La presentación clínica del linfoma con compromiso gastrointestinal es inespecífica y, hasta en la mitad de los pacientes, se puede presentar de manera inicial con complicaciones que requieran manejo quirúrgico. Entre las principales se encuentran la perforación intestinal, el sangrado digestivo y la obstrucción intestinal. Abstract Introduction. Lymphoma can affect the gastrointestinal tract, primarily or secondarily, and accounts for up to 2% of all malignant neoplasms of the small intestine and colon. The gastrointestinal tract is the most common primary extranodal location in non-Hodgkin lymphoma. Methods. A literature search performed in the main academic databases, with a review of texts published on the subject in the last 5 years. Discussion. The clinical presentation of lymphoma with gastrointestinal involvement is nonspecific and, in up to half of the patients, it can present initially with complications that require surgical management. Among the main ones are intestinal perforation, digestive bleeding and intestinal obstruction.

Chronic ulcerations of the lower extremities are quite a common condition amongst adults, most often caused by chronic venous insufficiency. Irrespective of the main underlying cause, chronic limb ulcerations are usually associated with significant symptoms, impairing daily functioning. Improper or delayed diagnosis and inadequate treatment increase the risk of serious complications, including limb amputations. Malignancies can develop secondary to chronic leg ulcers. About 2.4% of ulcers arising from chronic venous stasis undergo malignant transformation. Squamous cell carcinoma is the most common type of malignancy found in chronic leg ulceration biopsies. Basal cell carcinoma, sarcoma, and melanoma have all been documented infrequently. In the case described here, we found lymphoma of the marginal zone of mucosa-associated lymphoid tissue (MALT), which is an extremely rare cutaneous neoplasm of the lower extremities, but one that may have an association with autoimmune diseases.

Introduction: To determine the correlation between tobacco control policies and mortality of haematological malignancies: leukemia, lymphoma and multiple myeloma (MM). Methods: Ecological study with the countries as the unit of analysis. Tobacco Control Scale (TCS) scores from 2010, 2013 and 2016 were used as measures for the level of tobacco control policy implementation in 27 European countries. Mortality rates for leukemia, lymphoma, and MM, were obtained from the WHO Mortality Database and the European Cancer Information System for each country for 2010, 2013, 2015 and 2018. Correlation between yearly TCS scores and mortality rates from the same and prospective years were calculated using Spearman’s rank correlation coefficients (rsp) and 95% confidence intervals (95% CI) (2010 TCS scores vs 2010, 2013, 2015, 2018 mortality rates; 2013 TCS scores vs 2013, 2015, 2018 mortality rates; and 2016 TCS scores vs 2018 mortality rates). Results: The 2010 TCS scores were significantly negatively associated with leukemia mortality rates in 2013 (rsp=-0.58; 95% CI: -0.79, -0.24; p=0.002), 2015 (rsp=-0.65; 95% CI: -0.85, -0.30; p=0.001) and 2018 (rsp=-0.44; 95% CI: -0.71, -0.06; p=0.021). TCS scores from 2013 and 2016 had significant negative associations with leukemia mortality in all prospective years. TCS scores did not demonstrate consistent correlations with lymphoma and MM mortality. Conclusions: The level of tobacco control policies in European countries correlates negatively with leukemia mortality at ecological level, with no correlation seen for lymphoma and MM. La leucemia linfoblástica aguda representa más del 30% de todos los tipos de cáncer en niños entre 0 y 9 años a nivel global. En el Perú, no se ha evaluado si la lactancia materna exclusiva es un factor protector para la leucemia linfoblástica. Objetivo: Identificar los factores protectores y de riesgo asociados a la leucemia linfoblástica aguda en niños y adolescentes entre 0 y 13 años en un hospital nacional, Lima, Perú. Pacientes y Método: Estudio observacional, analítico, diseño caso-control. Se evaluaron 112 casos con diagnóstico de leucemia linfoblástica y 229 controles. Los datos fueron recogidos por entrevistas a las madres de ambos grupos. Se estimó la magnitud de la asociación entre la leucemia y la lactancia exclusiva mediante el odds ratio (OR) y regresión logística multivariado en Stata v 12. Resultados: El 50,9% (57/112) de los casos y el 51,5% (118/229) de los controles fueron del género masculino. La media de edad entre los casos fue 6,7 ± 3,2 años y entre los controles 5,7 ± 3,5 años. La media de edad de la madre entre los casos fue 35,9 ± 6,5 y entre los controles fue 34,1 ± 7,1 años. La lactancia materna exclusiva se asocia a menor riesgo de leucemia en un 44% respecto a aquellos que no tuvieron, OR 0,56, p = 0,017, IC 95% (0,35-0,90). La educación secundaria completa se asocia a menor riesgo de leucemia en un 62%, OR 0,38 IC 95% (0,15-0,61). Conclusiones: La lactancia materna exclusiva y la educación secundaria completa de la madre son factores asociados a menor riesgo de leucemia linfoblástica aguda en niños y en adolescentes. Globally, Acute Lymphoblastic Leukemia (ALL), represents more than 30% of all types of cancers in children aged between 0 and 9 years. In Peru, it has not been evaluated whether exclusive breastfeeding (EB) is a protective factor for ALL. Objective: To identify the protective and risk factors associated with acute lymphoblastic leukemia in children aged between 0 and 13 years in a national hospital in Lima, Peru. Patients and Method: Observational, analytical study, case-control design. 112 cases diagnosed with ALL and 229 controls were evaluated. The data were collected by interviews with the mothers of both groups. The magnitude of the association between ALL and EB was estimated using the odds ratio (OR) and multivariate logistic regression in Stata v 12. Results: 50.9% (57/112) of the cases and 51.5% (118/229) of the controls were male. The mean age of the cases was 6.7 ± 3.2 years and of the controls 5.7 ± 3.5 years. The mean age of the mothers of the cases was 35.9 ± 6.5 and of the controls was 34.1 ± 7.1 years. EB reduces the risk of ALL by 44% compared with those who did not receive it, OR 0.56, p = 0.017, 95% CI (0.35-0.90). Complete secondary education reduces the risk of ALL by 62%, OR 0.38 CI 95% (0.15-0.61). Conclusions: Exclusive breastfeeding and the mother’s complete secondary education are protective factors for the development of ALL in children and adolescents.

Hypercalcemia is common in patients after kidney transplantation (KTx) and is associated with persistent hyperparathyroidism in the majority of cases. This retrospective, single-center study evaluated the prevalence of hypercalcemia after KTx. KTx recipients were evaluated for 7 years after receiving kidneys from living or deceased donors. A total of 301 patients were evaluated; 67 patients had hypercalcemia at some point during the follow-up period. The median follow-up time for all 67 patients was 62 months (44; 80). Overall, 45 cases of hypercalcemia were classified as related to persistent post-transplant hyperparathyroidism (group A), 16 were classified as “transient post-transplant hypercalcemia” (group B), and 3 had causes secondary to other diseases (1 related to tuberculosis, 1 related to histoplasmosis, and 1 related to lymphoma). The other 3 patients had hypercalcemia of unknown etiology, which is still under investigation. The immunophenotype is regarded as an independent prognostic factor in high-grade lymphomas, seeing that lymphomas of T-cell origin are associated with shorter survival time. Although a number of studies have evaluated the immunophenotypical profile of lymphoma in the USA and Europe, Brazilian research on the matter remains scarce. Exact characterization of the histopathological type is crucial to establish proper treatment and prognosis. This study evaluated the database of immunohistochemistry laboratories that perform immunophenotyping of canine lymphoma in Brazil. A total of 203 cases of multicentric lymphoma were classified according to the WHO classification. Immunophenotyping was able to identify 71.4% lymphomas of B-cell line, 27.1% of T-cell line and 1.5% of non-B cells and non-T cell lines. Diffuse large B-cell lymphoma was the most common with 59.1% of the cases. Among T-cell lymphomas, lymphoblastic was the most common (11.33% of the cases). Even though canine lymphomas tend to be high-grade, indolent lymphomas comprised 11.82% of the cases and T-zone lymphoma was the most prevalent (8.86%). The immunophenotype of multicentric lymphoma in Brazil is similar to those in other parts of the world, which suggests similar etiologic factors to the development of this disease. RESUMO O imunofenótipo é considerado um fator prognóstico independente em linfomas de alto grau, visto que os linfomas de origem de células T estão associados a menor tempo de sobrevida. Apesar de vários estudos terem avaliado o perfil imunofenotípico do linfoma nos EUA e na Europa, a pesquisa brasileira sobre o assunto ainda é escassa. A caracterização exata do tipo histopatológico é crucial para estabelecer o tratamento e o prognóstico adequados. Este estudo avaliou a base de dados de laboratórios de imuno-histoquímica que realizam imunofenotipagem do linfoma canino no Brasil. Um total de 203 casos de linfoma multicêntrico foi classificado de acordo com a classificação da OMS. A imunofenotipagem foi capaz de identificar 71,4% dos linfomas da linhagem de células B, 27,1% da linhagem de células T e 1,5% das linhagens de células não B e não T. O linfoma difuso de grandes células B foi o mais comum em 59,1% dos casos. Entre os linfomas de células T, o linfoblástico foi o mais comum (11, 33% dos casos). Embora os linfomas caninos tendam a ser de alto grau, os linfomas indolentes representaram 11,82% dos casos e o linfoma da zona T foi o mais prevalente (8,86%). O imunofenótipo do linfoma multicêntrico no Brasil é semelhante ao de outras partes do mundo, o que sugere fatores etiológicos semelhantes ao desenvolvimento dessa doença.

Objetivo Verificar as alterações no estado nutricional em adolescentes de 10-19 anos, observadas após 12 meses, no mínimo, da conclusão de tratamento oncológico de leucemias e linfomas. Métodos Trata-se de estudo quantitativo de delineamento longitudinal realizado no Hospital de Clínicas de Porto Alegre. Foram incluídos adolescentes de 10-19 anos, que haviam concluído tratamento oncológico para leucemias e linfomas há no mínimo doze meses. Foram coletadas as medidas de peso, estatura, circunferência braquial, dobra cutânea tricipital, circunferência muscular do braço e circunferência abdominal. Resultados A amostra foi constituída por cinquenta adolescentes sobreviventes de leucemias e linfomas. No follow up, 38% dos pacientes foram classificados como excesso de peso de acordo com o índice de massa corporal para a idade. Houve um aumento significativo no índice de massa corporal para a idade, ocorrida entre o início do tratamento, seu fim e follow up (p=0,013) nos indivíduos do sexo feminino, quando comparados a indivíduos do sexo masculino. Os resultados apontam uma diminuição nas médias de Escore-Z de estatura para a idade, com diferenças significativas entre o início do tratamento e follow up (p=0,016) e entre término do tratamento e follow up (p=0,006), em pacientes de ambos os sexos. Conclusão Os indicadores antropométricos demonstram uma frequência importante de excesso de peso e aumento da dobra cutânea tricipital, um aumento significativo do índice de massa corporal para a idade, bem como um deficit de crescimento entre os sobreviventes. Objective To examine the changes in the nutritional status of adolescents aged 10-19 years after a minimum 12 months interval following oncological treatment for leukemias and lymphomas. Methods Longitudinal design quantitative study conducted at Hospital de Clínicas, Porto Alegre. Adolescents aged 10-19 years after a minimum 12 months interval following oncological treatment for leukemias and lymphomas were included. The measures of weight, height, brachial circumference, triceps skinfold thickness, arm muscle circumference and abdominal circumference were collected. Results The sample comprised 50 adolescents who had survived leukemias and lymphomas. In the follow up 38% of the patients were classified as overweight according to the body mass index for their age. There was a significant increase in body mass index for age between the beginning and the end of treatment and follow up (p=0.013) in female individuals, compared to males. The results indicate a reduction in the Z-score means of height for age, with significant differences between the beginning of treatment and follow up (p=0.016); and end of treatment and follow up (p=0.006) in patients of both genders Conclusion The anthropometric indicators show an important frequency of excess weight and increased tricipital skinfold, as well as a significant increase of the body mass index for age and also a growth deficit among the survivors.

Introduction The gastrointestinal lymphoma can be classified in primary or secondary, and this is important regarding diagnosis and subsequent treatment. Primary gastrointestinal lymphoma of the rectum is rare and therefore lacks data in medical literature. Its incidence has been increasing and that fact may be related to a higher incidence in immunosuppressive therapy and immunosuppressive diseases (such as AIDS). Metodology 19 articles have been reviewed, searched online on the Scielo and PubMed databases. The goal was to increase data available regarding this pathology and improve its therapy. Discussion Primary GI lymphoma of the rectum presents as hematochezia, rectal pain, change in bowel habits. PET/CT is the first choice exam to pursue investigation; however abdominal CT and MRI reveal sufficient information and are much more available in daily practice. Plasmablastyc lymphoma is an aggressive subtype and is usually associated with AIDS patients. There are no available treatment protocols for this specific type of lymphoma and colonic lymphoma’s therapy is usually used for this patient (such as ECHOP and CHOP). Conclusion As rare as this pathology is, this article aims to improve the available data and provide useful information regarding diagnosis and therapy. RESUMO Introdução O linfoma do TGI pode ser dividido entre primário e secundário, com importância diagnóstica e terapêutica. O linfoma primário de reto é patologia rara, pouco relatada em literatura médica. Sua incidência tem aumentado e possivelmente esse fenômeno esteja associado ao aumento no numero de pacientes com imunossupressão (seja por SIDA ou drogas imunossupressoras). Metodologia Foram revisados 19 artigos nas bases de dados Scielo e PubMed, com o objetivo de aumentar o número de relatos dessa patologia e consequentemente expandir o conhecimento disponível, visando melhorar a terapêutica e, principalmente, o diagnóstico desse tipo de linfoma. Discussão Quando o linfoma tem seu sítio primário no reto, as principais manifestações são sangramento, dor retal, tenesmo e mudança nos hábitos intestinais (diarreia ou constipação). O exame de investigação de escolha é o PET/CT, porém a TC e RNM fornecem as informações necessárias e são mais disponíveis na prática clínica. O linfoma plasmablástico é um subtipo bastante agressivo e associado aos pacientes com SIDA. Não existem ainda protocolos definidos para o tratamento do linfoma primário de reto, sendo optado por seguir a mesma terapêutica dos linfomas de cólon com esquemas EPOCH e CHOP. Conclusão Por se tratar de patologia rara e pouco descrita na literatura, espera-se que este relato contribua na formação de protocolos de tratamento específicos.

Introdução: As neoplasias hematológicas, leucemias e linfomas são patologias que afetam o sangue ou tecidos formadores dele. Durante o período de hospitalização, os pacientes podem desenvolver redução da capacidade funcional que pode interferir na sua função respiratória. Objetivo: Avaliar a influência do tempo de internamento sobre a força muscular respiratória e o nível funcional de adultos com leucemia e linfoma. Método: Estudo observacional, com delineamento longitudinal e abordagem quantitativa, realizado na enfermaria onco-hematológica do Complexo Hospitalar Universitário Professor Edgard Santos (Hupes). A avaliação da força muscular respiratória foi mensurada pelo manovacuômetro e a capacidade funcional pela escala de desempenho de Karnofsky (KPS). Resultados: No decorrer do tempo de internamento dos pacientes, houve uma diminuição da pressão expiratória máxima (PEM) (p=0,000), porém não foi observada diferença significativa na pressão inspiratória máxima (PIM) (p>0,05). Em relação à KPS, os pacientes apresentaram nível de funcionalidade de 70%. Conclusão: Este estudo demonstrou que a PEM foi alterada durante o internamento, porém não houve modificação da PIM e da funcionalidade dos pacientes.

Objective: To investigate, in a large prospective multicenter study, whether 2-[18F]-fluoro-2-deoxy-d-glucose-positron emission tomography is sufficiently accurate to identify clinically important bone marrow involvement by Hodgkin's lymphoma to replace routine bone marrow biopsy in a developing tropical country. Methods: Patients newly diagnosed with Hodgkin's lymphoma were recruited from six cancer centers in Brazil. All were staged by the results of positron emission tomography/computed tomography that were centrally reviewed and by iliac crest bone marrow biopsy. Patients were classified as having marrow disease if they had lymphoma identified by marrow biopsy histology or had focal 2-[18F]-fluoro-2-deoxy-d-glucose marrow uptake that resolved following chemotherapy. Results: A total of 246 participants were recruited from six different centers and 62 (25.2%) were judged to have Hodgkin's lymphoma in the bone marrow. Positron emission tomography and biopsies were concordant in 206 patients (83%). Positron emission tomography correctly identified marrow disease in 59/62 patients (95.1%) and marrow biopsy in 25/62 patients (40.3%). In 22/62 (35.4%) patients, the two techniques were concordant in the diagnosis of marrow involvement. Of the forty discordant results, positron emission tomography found bone marrow involvement in 37 patients, upstaging 22 to stage IV and having an impact on therapeutic decision in nine cases given their reallocation from early to advanced stage. Three false negative positron emission tomography results were obtained with bone marrow biopsy giving positive findings. All three cases were classified as stage IV regardless of bone marrow findings implying no modification in the clinical management. The sensitivity, specificity and accuracy of positron emission tomography for detecting bone marrow disease were 95%, 100% and 98% and for bone marrow biopsy they were 40%, 100% and 84%, respectively. Conclusion: We conclude that positron emission tomography can replace marrow biopsy in Brazilian patients with Hodgkin's lymphoma without compromising clinical management. Background Primary central nervous system lymphomas (PCNSL) are infrequent. The traditional treatment of choice is chemotherapy. Complete resections have generally not been recommended, because of the risk of permanent central nervous system deficits with no proven improvement in survival. The aim of the current study was to compare survival among patients with PCNSL who underwent biopsy versus surgical resection. Methods A retrospective study was conducted on 50 patients with a confirmed diagnosis of PCNSL treated at our center from January 1994 to July 2015. Results Patients in the resection group exhibited significantly longer median survival time, relative to the biopsy group, surviving a median 31 months versus 14.5 months; p = 0.016. Conclusions In our series, patients who had surgical resection of their tumor survived a median 16.5 months longer than patients who underwent biopsy alone. RESUMO Introducción Los linfomas primarios del sistema nervioso central (LPSNC) son infrecuentes. Tradicionalmente el tratamiento de elección es la quimioterapia. Existe un paradigma de no indicar resección, por el riesgo de déficit neurológico sin aumento de la sobrevida. El objetivo del presente estudio es comparar la sobrevida de pacientes con LPSNC sometidos a biopsia versus resección. Métodos Estudio retrospectivo que incluye 50 pacientes con diagnóstico confirmado de LPSNC tratados en nuestra Institución desde enero de 1994 hasta julio de 2015. Resultados Los pacientes del “grupo resección” mostraron una sobrevida media significativamente mayor respecto a los del “grupo biopsia”; 31 meses versus 14,5 meses respectivamente, p = 0,016. Conclusiones En nuestra serie, los pacientes que con resección quirúrgica de su tumor tuvieron una sobrevida media de 16,5 meses superior que los pacientes biopsiados.

Resumen El cáncer en la población pediátrica representa la segunda causa de muerte en niños mayores de 5 años en nuestro país, después del trauma. La tomografia por emisión de positrones/tomograña computada (PET/CT) es una técnica hibrida que involucra radiación y que progresivamente se está implementando en niños. Esta técnica permite hacer evaluaciones morfológicas y funcionales del cuerpo con aplicaciones en patología neoplásica y no neoplásica. Su principal rol en la oncología se encuentra en la evaluación y control de linfomas, sarcomas y neuroblastomas, entre otros. La calidad de los resultados depende de la realización de procesos rigurosos para obtener las imágenes. En este artículo se revisan los pasos para la obtención de las imágenes, los aspectos técnicos involucrados, las fuentes de mala interpretación y las principales indicaciones que debe conocer el médico radiólogo y pediatra.

Objectives Multidrug resistance 1 (MDR1) encodes a protein called P-glycoprotein (P-gp), which serves as an efflux pump membrane protein implicated in intestinal homeostasis and drug resistance. Cyclooxygenase-2 (COX2) is a key enzyme in the synthesis of proinflammatory prostaglandins, tumourigenesis and in mucosal defence. Despite the importance of MDR1 and COX2, changes in their mRNA levels have not been studied in cats with inflammatory bowel disease (IBD) and low-grade alimentary lymphoma (LGAL). The present study aimed to determine the mRNA levels of MDR1 and COX2 in cats with IBD and LGAL, and to evaluate their correlation with clinical signs, histological severity and between genes. Methods Cats diagnosed with IBD (n = 20) and LGAL (n = 9) between 2008 and 2015 were included in the current study. Three healthy animals composed the healthy control cats group in which endoscopy was performed immediately before the ovariohysterectomy. All duodenal biopsy samples were obtained by endoscopy. Feline chronic enteropathy activity index was calculated for all cases. IBD histopathology was classified according to severity. MDR1 and COX2 mRNA levels were determined by absolute reverse transcriptase-quantitative real-time PCR. Results Statistically significant differences were observed for MDR1 and COX2 mRNA levels between the IBD and LGAL groups. No correlations were observed between molecular gene expression, feline chronic enteropathy activity index and histological grading for IBD, and between MDR1 and COX2 genes. However, a positive statistically significant correlation was observed between MDR1 and COX2 expression in the duodenum of cats. Conclusions and relevance MDR1 and COX2 gene expression is increased in cats with LGAL compared with cats with IBD. The control group tended to have lower values than both diseased groups. These results suggest that these genes may be involved in the pathogenesis of IBD or LGAL in cats.

Background: Acquired angioedema due to C1 inhibitor deficiency (AAE-C1-INH) is a very rare disease. In clinical practice, it may be difficult to differentiate AAE-C1-INH from hereditary angioedema due to C1-INH deficiency (HAE-C1-INH). In both conditions, patients are at an increased risk of death from asphyxiation due to upper airway obstruction. The association of AAE-C1-INH with lymphoproliferative and autoimmune diseases, and with presence of anti-C1-INH antibodies has been well documented, and treatment of the underlying condition may result in complete remission of angioedema. Objectives: To discuss the clinical evaluation, diagnosis, and treatment outcomes of AAE-C1-INH in the context of the care of 2 patients with recurrent isolated angioedema. Methods: Two patients were followed up prospectively at our clinic. Measurements of C3, C4, C1-INH, and C1q levels were carried out by nephelometry, and the functional activity of C1-INH was determined by a chromogenic assay. Hematological investigation included morphological and immunophenotyping analysis of peripheral blood, bone marrow, and spleen histopathology. Sequencing of the 8 exons and adjacent intronic regions of the SERPING1 gene was performed using the Sanger method. Results: Two patients were diagnosed with AAE-C1-INH associated with splenic marginal zone lymphoma during follow-up. Conclusions: Close follow-up, including detailed clinical history, physical examination, and laboratory tests, of our patients with AAE-C1-INH was essential for the early diagnosis and successful treatment of the lymphoproliferative disease, leading to the resolution of the angioedema attacks.

A 61-year-old female patient presented a nodular lesion located in the right buccal mucosa with a 3-month evolution. Clinical hypotheses of salivary duct cyst and mucocele were proposed, and the patient underwent excisional biopsy. Microscopically, a well-circumscribed and encapsulated lymphoid aggregate fragment was observed, characterized by layers of well-differentiated small lymphocytes and collections of reactive lymphoblasts. These findings, associated with immunohistochemistry, established the diagnosis of follicular lymphoid hyperplasia. Currently, the patient is well, under follow-up after six months.

Hepatic lymphoma can be divided into its primary and secondary forms. To be classified as primary hepatic lymphoma, the disease should be limited to the liver and hilar lymph nodes, with no distant involvement (to the bone marrow or other sites). Primary hepatic lymphoma is quite rare, accounting for less than 1% of all cases of non-Hodgkin lymphoma, whereas secondary hepatic lymphoma affects nearly half of such cases. Familiarity with the imaging features of hepatic lymphoma is important for its early diagnosis and appropriate management. In this essay, we aim to review the roles of computed tomography and magnetic resonance imaging in identifying and monitoring hepatic lymphomas, using sample cases to describe and illustrate the wide spectrum of imaging findings in the primary and secondary forms of this disorder.O linfoma hepático pode ser dividido em primário e secundário. Para ser classificada como linfoma hepático primário, a doença deve estar limitada ao fígado e linfonodos hilares, sem acometimento de outros órgãos (incluindo a medula óssea). O linfoma hepático primário é muito raro, correspondendo a menos de 1% de todos os linfomas não Hodgkin. Já o envolvimento secundário do fígado ocorre em quase metade dos casos de linfoma não Hodgkin. O conhecimento dos aspectos de imagem do linfoma hepático é importante para o diagnóstico precoce e manejo adequado do paciente. Com base em imagens de casos, nosso objetivo é revisar o papel da imagem por tomografia computadorizada e ressonância magnética na identificação e segmento do linfoma hepático, ilustrando e descrevendo o amplo espectro de achados de imagem observados nas formas primária e secundária da doença.

Lymphoma is a neoplasm that originates from solid hematopoietic tissues and is one of the most common tumors in dogs. The goal of the present study was to perform a retrospective study of canine lymphomas diagnosed at the “Laboratório Regional de Diagnóstico”, at the “Faculdade de Veterinária” of the “Universidade Federal de Pelotas” (LRD-UFPel) from 2000 to 2017, to determine the epidemiology and anatomical distribution, and to evaluate the histopathological and immunohistochemical aspects of each case according to the adapted Kiel classification. The protocols for necropsies and biopsies in the laboratory were reviewed. Lymphoma was diagnosed in 77 dogs. Approximately 37.7% (29/77) of affected dogs had no defined breed, while dogs with defined breeds accounted for 58.4% (45/77) of the diagnoses. The occurrence in males (40/77) was slightly higher than that in females (36/77), and the mean age was 8.1 years (1.4-17 years). The most affected age group was between six and 10 years of age with 31 cases (40.2%). Regarding the anatomical classification, the multicentric form was the most prevalent, accounting for 71.4% (55/77) of the diagnoses. In 40 cases that immunophenotyping was performed, B-cell lymphomas represented 62.5% of the diagnoses (25/40), while T-cell lymphomas corresponded to 37.5% of the diagnoses (15/40). The degree of malignancy according to the modified Kiel classification was low in 35% of lymphomas (14/40) and high in 65% of cases (26/40). The multicentric form was more frequent in the region of influence of the LRD-UFPel. Identification of the immunophenotype can improve the quality of life and survival in affected dogs since it allows the most appropriate treatment for each patient. RESUMO: O linfoma é uma neoplasia com origem nos tecidos hematopoiéticos sólidos e é um dos tumores mais frequentes em cães. O objetivo do presente trabalho foi efetuar um estudo retrospectivo dos linfomas caninos recebidos no Laboratório Regional de Diagnóstico, da Faculdade de Veterinária da Universidade Federal de Pelotas (LRD-UFPel) de 2000 a 2017, determinando a epidemiologia e a distribuição anatômica, bem como os aspectos histopatológicos e imuno-histoquímicos de cada caso de acordo com a classificação de Kiel adaptada. Foram revisados os protocolos de necropsias e biópsias recebidos no laboratório identificando-se 77 casos de cães com diagnóstico de linfoma. A doença afetou cães sem raça definida em 37,7% (29/77) dos casos, enquanto os cães com raças definidas tiveram 58,4% (45/77) dos diagnósticos. A ocorrência em machos (40/77) foi discretamente maior do que em fêmeas (36/77) e a idade média foi de 8,1 anos (1,4-17 anos). A faixa etária mais acometida foi entre seis e 10 anos de idade com 31 casos (40,2%). Quanto à classificação anatômica a forma multicêntrica foi a mais prevalente atingindo 71,4% (55/77) dos diagnósticos. Em 40 casos em que a imunofenotipagem foi realizada, os linfomas de células B representaram 62,5% dos casos (25/40), enquanto os linfomas de células T equivaleram a 37,5% dos diagnósticos (15/40). O grau de malignidade de acordo com a classificação de Kiel modificada foi baixo em 35% dos linfomas (14/40) e alto em 65% dos casos (26/40). Conclui-se que a forma multicêntrica é mais frequente na região de influência do LRD-UFPel e que a identificação do imunofenótipo pode melhorar a qualidade de vida e dar maior sobrevida aos cães afetados uma vez que permite o tratamento mais adequado para cada caso.

Objective: Evaluation of enlarged mediastinal lymph nodes is crucial for patient management. Malignant lymphoma and sarcoidosis are often difficult to differentiate. Our objective was to determine the diagnostic accuracy of MRI for differentiating between sarcoidosis and malignant lymphoma. Methods: This was a retrospective study involving 47 patients who underwent chest MRI and were diagnosed with one of the diseases between 2017 and 2019. T1, T2, and diffusion-weighted signal intensity were measured. Apparent diffusion coefficients (ADCs) and T2 ratios were calculated. The diagnostic performance of MRI was determined by ROC analysis. Results: Mean T2 ratio was significantly lower in the sarcoidosis group than in the lymphoma group (p = 0.009). The T2-ratio cutoff value that best differentiated between lymphoma-related and sarcoidosis-related enlarged lymph nodes was 7.1, with a sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of 58.3%, 95.6%, 76.5%, 93.3%, and 68.7%, respectively. The mean ADC was significantly lower in the lymphoma group than in the sarcoidosis group (p = 0.002). The ADC cutoff value that best differentiated between lymphoma-related and sarcoidosis-related enlarged lymph nodes was 1.205, with a sensitivity, specificity, positive predictive value, negative predictive value and accuracy of 87.5%, 82.6%, 85.1%, 84.0% and 86.3%, respectively. No significant differences were found between the two groups regarding T1 signal intensity, T2 signal intensity, and lymph node diameter. Conclusions: MRI parameters such as ADC, diffusion, and T2 ratio can be useful in the differentiation between sarcoidosis and lymphoma in the evaluation of enlarged lymph nodes. RESUMO Objetivo: A avaliação de linfonodomegalias mediastinais é crucial para o manejo dos pacientes. O linfoma maligno e a sarcoidose costumam ser difíceis de diferenciar. Nosso objetivo foi determinar a precisão diagnóstica da RMN para diferenciar sarcoidose de linfoma maligno. Métodos: Estudo retrospectivo com 47 pacientes submetidos a RMN de tórax e diagnosticados com uma das doenças entre 2017 e 2019. Foi medida a intensidade do sinal em T1, T2 e difusão. Foram calculados o coeficiente de difusão aparente (ADC, do inglês apparent diffusion coefficient) e a razão T2. O desempenho diagnóstico da RMN foi determinado pela análise da curva ROC. Resultados: A média da razão T2 foi significativamente menor no grupo sarcoidose que no grupo linfoma (p = 0,009). A razão T2 = 7,1 foi o melhor valor de corte para diferenciar linfonodomegalias relacionadas com linfoma daquelas relacionadas com sarcoidose, com sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo e precisão de 58,3%, 95,6%, 76,5%, 93,3% e 68,7%, respectivamente. A média do ADC foi significativamente menor no grupo linfoma que no grupo sarcoidose (p = 0,002). O ADC = 1,205 foi o melhor valor de corte para diferenciar linfonodomegalias relacionadas com linfoma daquelas relacionadas com sarcoidose, com sensibilidade, especificidade, valor preditivo positivo, valor preditivo negativo e precisão de 87,5%, 82,6%, 85,1%, 84,0% e 86,3%, respectivamente. Não houve diferença significativa entre os grupos quanto à intensidade do sinal em T1, intensidade do sinal em T2 e diâmetro dos linfonodos. Conclusões: Parâmetros de RMN como ADC, difusão e razão T2 podem ser úteis na diferenciação entre sarcoidose e linfoma na avaliação de linfonodomegalias.

High levels of circulating EBV load are used as a marker of post-transplant lymphoproliferative disorders (PTLD). There is no consensus regarding the threshold level indicative of an increase in peripheral EBV DNA. The aim of the study was to clinically validate a developed EBV quantification assay for early PTLD detection.Transversal study: paired peripheral blood mononuclear cells (PBMC), plasma and oropharyngeal lymphoid tissue (OLT) from children undergoing a solid organ transplant with (n=58) and without (n=47) PTLD. Retrospective follow-up: 71 paired PBMC and plasma from recipients with (n=6) and without (n=6) PTLD history. EBV load was determined by real-time PCR. The diagnostic ability to detect all PTLD (categories 1–4), advanced PTLD (categories 2–4) or neoplastic PTLD (categories 3 and 4) was estimated by analyzing the test performance at different cut-off values or with a load variation greater than 0.5log units.The higher diagnostic performance for identifying all, advanced or neoplastic PTLD, was achieved with cut-off values of 1.08; 1.60 and 2.47log EBVgEq/105 PBMC or 2.30; 2.60; 4.47loggEq/105 OLT cells, respectively. EBV DNA detection in plasma showed high specificity but low (all categories) or high (advanced/neoplastic categories) sensitivity for PTLD identification. Diagnostic performance was greater when: (1) a load variation in PBMC or plasma was identified; (2) combining the measure of EBV load in PBMC and plasma.The best diagnostic ability to identify early PTLD stages was achieved by monitoring EBV load in PBMC and plasma simultaneously; an algorithm was proposed.

Primary extranodal lymphoma is defined as a lymphoma at a solitary extranodal site, with or without involvement of the lymph nodes. The clinical and radiological features of extranodal lymphoma have been documented in recent studies. In this pictorial essay, we reviewed imaging findings of extranodal lymphoma in the head and neck region.Linfoma extranodal primário é definido como um sítio extranodal solitário de linfoma, com ou sem envolvimento dos gânglios linfáticos. Background Consensus is lacking regarding the optimal salvage therapy for patients with follicular lymphoma who relapse after or are refractory to immunochemotherapy. Methods This phase II trial evaluated the efficacy and safety of response‐adapted therapy with rituximab, bendamustine, mitoxantrone, and dexamethasone (RBMD) in follicular lymphoma patients who relapsed after or were refractory to first‐line immunochemotherapy. Sixty patients received three treatment cycles, and depending on their response received an additional one (complete/unconfirmed complete response) or three (partial response) cycles. Patients who responded to induction received rituximab maintenance therapy for 2 years. Results Thirty‐three (55%) and 42 (70%) patients achieved complete/unconfirmed complete response after three cycles and on completing induction therapy (4‐6 cycles), respectively (final overall response rate, 88.3%). Median progression‐free survival was 56.4 months (median follow‐up, 28.3 months; 95% CI, 15.6‐51.2). Overall survival was not reached. Progression‐free survival did not differ between patients who received four vs six cycles (P = .6665), nor between patients who did/did not receive rituximab maintenance after first‐line therapy (P = .5790). Median progression‐free survival in the 10 refractory patients was 25.5 months (95% CI, 0.6‐N/A) and was longer in patients who had shown progression of disease after 24 months of first‐line therapy (median, 56.4 months; 95% CI, 19.8‐56.4) than in those who showed early progression (median, 42.31 months; 95% CI, 24.41–NA) (P = .4258). Thirty‐six (60%) patients had grade 3/4 neutropenia. Grade 3/4 febrile neutropenia and infection were recorded during induction (4/60 [6.7%] and 5/60 [8.3%] patients, respectively) and maintenance (2/43 [4.5%] and 4/43 [9.1%] patients, respectively). Conclusions This response‐adapted treatment with RBMD followed by rituximab maintenance is an effective and well‐tolerated salvage treatment for relapsed/refractory follicular lymphoma following first‐line immunochemotherapy. El linfoma difuso de células B grandes, es una neoplasia maligna linfoproliferativa caracterizada por la reproducción monoclonal de linfocitos de la estirpe B en diferentes órganos y tejidos. Se describe el caso de un paciente femenino de 63 años de edad, quien ingresa al servicio de urgencias por cuadro de dolor abdominal, asociado a pérdida de peso y presencia de melenas. Durante la hospitalización presenta cuadro de obstrucción intestinal y agravamiento del cuadro clínico, por lo que se indica realizar laparotomía exploratoria en donde se encuentra una gran masa con múltiples ganglios en meso de yeyuno de aspecto tumoral. El estudio de patología con inmunohistoquímica de la muestra reporta reactividad difusa para CD20 y para vimentina, además de CD3 y BCL2, hallazgos compatibles con Linfoma de células B grandes, un linfoma no Hodgkin gastrointestinal de presentación infrecuente en yeyuno.

Although the primary purpose of periodic mammograms in screening programs is to identify lesions suspected of being carcinomas, the findings are often related to systemic (benign or malignant) diseases, rather than breast cancer. Although the involvement of breast structures in systemic diseases is unusual, it can be included in the differential diagnosis of masses, skin changes, calcifications, asymmetry, and axillary lymphadenopathy. The main diagnostic entities that can be associated with such involvement are diabetes, chronic kidney disease, heart diseases, connective tissue diseases, HIV infection, lymphoma, leukemia, and metastases from primary tumors at other sites. In many cases, information related to knowledge and treatment of chronic diseases is not available to the radiologist at the time of evaluation of the mammography findings. The purpose of this essay is to offer relevant pictorial information to the general radiologist about systemic diseases involving the breast, expanding the range of differential diagnoses in order to avoid unnecessary invasive procedures. Resumo Embora o objetivo primário da realização periódica da mamografia nos programas de rastreamento seja a identificação de lesões suspeitas para carcinoma mamário, muitas vezes as alterações encontradas não estão relacionadas ao câncer de mama, e sim, a doenças sistêmicas benignas e malignas secundárias de outros sítios. O envolvimento das estruturas mamárias nas doenças sistêmicas é incomum, mas pode ser incluído no diagnóstico diferencial de nódulos, alterações cutâneas, calcificações, assimetrias e linfonodomegalias axilares. As principais entidades diagnósticas que podem estar associadas ao acometimento mamário são o diabetes, a nefropatia crônica, as cardiopatias, as colagenoses, as infecções pelo vírus HIV ou parasitas, o linfoma, a leucemia e as metástases de tumores primários de outros órgãos. Muitas vezes as informações relacionadas ao conhecimento e/ou tratamento de doenças crônicas não estão disponíveis para o radiologista no momento da avaliação da mamografia. O objetivo deste ensaio é oferecer informações iconográficas relevantes a respeito de doenças sistêmicas com envolvimento mamário, permitindo ampliar o leque de diagnósticos diferenciais e evitar eventuais procedimentos invasivos desnecessários.

Background Canine breeds may be considered good animal models for the study of genetic predisposition to cancer, as they represent genetic clusters. From epidemiologic and case collection studies it emerges that some breeds are more likely to develop lymphoma or specific subtypes of lymphoma but available data are variable and geographically inconsistent. This study was born in the context of the European Canine Lymphoma Network with the aim of investigating the breed prevalence of canine lymphoma in different European countries and of investigating possible breed risk of lymphoma overall and/or different lymphoma subtypes. Results A total of 1529 canine nodal lymphoma cases and 55,529 control cases from 8 European countries/institutions were retrospectively collected. Odds ratios for lymphoma varied among different countries but Doberman, Rottweiler, boxer and Bernese mountain dogs showed a significant predisposition to lymphoma. In particular, boxers tended to develop T-cell lymphomas (either high- or low-grade) while Rottweilers had a high prevalence of B-cell lymphomas. Labradors were not predisposed to lymphoma overall but tended to develop mainly high-grade T-cell lymphomas. In contrast with previous studies outside of Europe, the European golden retriever population did not show any possible predisposition to lymphoma overall or to specific subtypes such as T-zone lymphoma. Conclusion Further prospective studies with more precise and consistent subtype identification are needed to confirm our retrospective results and to create the basis for the investigation of possible genes involved in different predispositions. RESUMEN Introducción: Los tumores primarios de mediastino son una entidad poco frecuente, que representa sólo el 3% de la patología torácica. Pueden manifestarse por una variedad de síntomas, según sea su efecto sobre los órganos y estructuras adyacentes de acuerdo a su localización. El objetivo de este trabajo es establecer la frecuencia y características de este tipo de tumores en el Hospital San Jorge. Materiales y Métodos: Estudio observacional, descriptivo, retrospectivo de corte transversal. Fueron incluidos todos los pacientes intervenidos quirúrgicamente en el Servicio de Cirugía Torácica en el periodo comprendido entre julio de 2013 a junio de 2018. Resultados: De los 476 pacientes intervenidos en el Servicio, 45 pacientes padecían patologías tumorales de mediastino (frecuencia de 9,4%); siendo 30 pacientes mujeres (67%), entre 51 y 60 años. La localización topográfica más frecuente corresponde al mediastino anterior (83%). Los tipos histológicos más frecuentes fueron los linfomas de Hodgkin con un 31 %. Conclusiones: La frecuencia de tumores de mediastino en nuestra serie corresponde al 9,4%. La localización más frecuente fue en el mediastino anterior, se halló un mayor número de casos en el sexo femenino. El tipo histológico más frecuente es el linfoma de Hodgkin, la vía de abordaje más utilizada fue la toracotomía. ABSTRACT Introduction: Primary mediastinal tumors are a rare entity, representing only 3% of the thoracic pathology. They can be manifested by a variety of symptoms, depending on their effect on adjacent organs and structures according to their location. The objective of this work is to establish the frequency and characteristics of this type of tumors in the San Jorge Hospital. Materials and Methods: Observational, descriptive, retrospective cross-sectional study. All the patients who underwent surgery in the Thoracic Surgery Service were included in the period from July 2013 to June 2018. Results: Of the 476 patients operated on in the Service, 45 patients suffered from mediastinal tumor pathologies (frequency of 9.4 %); 30 patients being women (67%), between 51 and 60 years old. The most frequent topographic location corresponds to the anterior mediastinum (83%). The most frequent histological types were Hodgkin's lymphomas with 31%. Conclusions: The frequency of mediastinal tumors in our series corresponds to 9.4%. The most frequent location was in the anterior mediastinum, a greater number of cases was found in the female sex. The most frequent histological type is Hodgkin's lymphoma, the most commonly used approach was thoracotomy.

Introdução: O filgrastim, que desempenha um papel fundamental na coleta de células progenitoras de sangue periférico (CPSP), está disponível há quase 25 anos, e existem vários biossimilares de filgrastim sendo comercializados. Objetivo: Avaliar se um filgrastim biossimilar (Filgrastine®) foi associado com mobilização efetiva em pacientes submetidos à coleta de CPSP para transplante autólogo de medula óssea. Método: Foram revisados os prontuários de pacientes com mieloma múltiplo e linfomas tratados em três instituições no Brasil. O desfecho primário (taxa de sucesso de mobilização) foi a proporção de pacientes na população intenção de tratar (ITT), em que pelo menos 2 x 106 células CD34+/kg foram coletadas por leucaférese nos dias 5 e/ou 6. A população per protocolo (PP) foi composta por pacientes que receberam pelo menos quatro dias de Filgrastine e tiveram pelo menos uma contagem de CD34+ nos dias 5 ou 6. Resultados: A dose diária de Filgrastine (no D1, com pequenas alterações subsequentes) variou de 8,5 a 28,9 mcg/Kg nos 52pacientes na população ITT, com uma mediana de 13,8 mcg/Kg; 51 pacientes receberam pelo menos quatro doses. Uma média de 2,84±1,97 x 106 células CD34+/kg foram coletadas. A taxa de sucesso de mobilização foi de 53,9% (IC 95%, 39,5% a 67,8%) na população ITT e 62,2% (IC 95%, 46,5% a 76,2%) nos 45 pacientes da população PP. A mobilização foi considerada efetiva pelos pesquisadores em 80,8% dos pacientes da população ITT e 88,9% daqueles na população PP. The per-protocol (PP) population comprised patients who received at least 4 days of Filgrastine andhad at least one CD34+ count on Days 5 or 6. Results: The daily dose of Filgrastine (on D1, with few changes thereafter) ranged from8.5 to 28.9 mcg/Kg in the 52 patients in the ITT population, with a median of 13.8 mcg/Kg; 51 patients received at least four doses. Amean of 2.84±1.97 x 106 CD34+cells/Kg were harvested. The MSR was 53.9% (95% CI, 39.5% to 67.8%) in the ITT population and62.2% (95% CI, 46.5% to 76.2%) in the 45 patients in the PP population. The mobilization was considered effective by investigators in80.8% of patients in the ITT population and 88.9% of those in the PP population. Conclusion: Despite its observational nature, thisstudy suggests that Filgrastine® is associated with the expected success rates in PBPC harvest for autologous transplantation.

En la clasificación de la Organización Mundial de la Salud (OMS) del 2016 se reconocen dos entidades cutáneas relacionadas con el EBV: el desoerden linfoproliferativo tipo Hydroa vaciniforme y el linfoma extraganglionar NK / T de células nasales. Se han informado casos raros de linfoma T primario CD8 cutáneo EBV positivo. Estas proliferaciones linfoides no encajan en ninguno de los dos linfomas clásicos relacionados con EBV . Presentamos un caso único de un trastorno linfoproliferativo de células T primario cutáneo-CD4-cutáneo asociado con el virus de Epstein-Barr que afecta los oídos bilaterales con curso indolente y recaída posterior en la piel de la lengua y el tronco. Rare cases of EBV primary cutaneous CD8-positive T lymphomas have been reported. These lymphoid proliferations do not fit into any of the two classical EBV-related lymphomas .We report a unique case of an Epstein-Barr Virus-associated Primary Cutaneous –CD4-positive T-Cell Lymphoproliferative Disorder affecting the bilateral ears with indolent outcome and subsequent relapse on the tongue and trunk skin. In the appropriate clinical setting, certain aspects of extranodal abdominal lymphoma, as revealed by current cross-sectional imaging techniques, should be considered potentially diagnostic and can hasten the diagnosis. In addition, diagnostic imaging in the context of biopsy-proven lymphoma can accurately stage the disease for its appropriate treatment. The purpose of this article was to illustrate the various imaging aspects of extranodal lymphoma in the abdomen. Resumo O linfoma extranodal abdominal tem um espectro de manifestações radiológicas que, no contexto clínico adequado, permite que o radiologista, ao considerar corretamente esta hipótese, otimize o seu diagnóstico e sua terapia. Além disso, depois de comprovado por biópsia, o linfoma pode ser estadiado e a resposta ao tratamento, avaliada pelos métodos de imagem. O presente ensaio iconográfico tem como objetivo ilustrar os diferentes achados por imagem do acometimento extranodal no linfoma abdominal.

RESUMENLa Enfermedad Celiaca (EC) tiene una prevalencia cercana al 1% de la población general y se considera que hay un número importante de pacientes asintomáticos no diagnosticados. Su presentación clínica es variable comprendiendo el clásico síndrome de malabsorción, formas menores y la EC silente. El diagnóstico serológico tiene una elevada sensibilidad y especificidad y siempre debe confirmarse con biopsia. El diagnóstico en pacientes en dieta libre de gluten incluye test de tipificación de HLA y prueba de dieta con gluten con estudio serológico e histológico posterior. El pilar del tratamiento es la dieta libre de gluten, que debe ser supervisada por un nutriólogo con experiencia. La monitorización de la terapia debe realizarse con serología. La EC mal controlada puede determinar complicaciones como linfoma y adenocarcinoma de intestino delgado. En el futuro es probable que nuevas terapias farmacológicas sean de utilidad en el manejo de la EC.SUMMARYCeliac disease has a prevalence near to 1% of general population and there is an important amount of asymptomatic people not yet diagnosed. Clinical presentation includes the classical malabsorption syndrome, minor and silent celiac disease. Serologic diagnosis has an elevated sensitivity and specificity, and must be confirmed by biopsy. Diagnosis in those on gluten free diet includes HLA type and gluten challenge with posterior serologic and histologic evaluation. The core of the treatment is the gluten free diet that must be supervised by an expert nutritionist. Monitoring is with serology. Poor disease control can determine complications such as lymphoma and small bowel adenocarcinoma. In the future, it is likely that new pharmacologic therapies will be available for the management of celiac disease.

Introdução: O médico de família (MF) depara-se regularmente com sinais e sintomas comuns que podem ser a tradução de patologias de curso menos benigno. Com este caso pretende-se demonstrar como o achado relativamente comum de edema facial pode ser interpretado erroneamente como angioedema, quando a patologia subjacente é a síndroma da veia cava superior (SVCS). Descrição do caso: Homem, 61 anos, recorreu ao MF por história de síncope. Foi enviado ao serviço de urgência (SU) hospitalar, onde foi assumido o diagnóstico de síncope vasovagal e detetada anemia microcítica hipocrómica. O estudo da anemia revelou ferropenia e uma úlcera no antro gástrico. Passados três meses voltou à Unidade de Saúde Familiar por edema facial, tendo sido medicado com anti-histamínico. Por persistência das queixas, recorreu ao SU onde foi feito o diagnóstico de angioedema. Um mês depois foi internado para tratamento de uma hemorragia digestiva alta com origem na úlcera gástrica. Voltou posteriormente ao SU por queixas respiratórias, onde se efetuou telerradiografia torácica que revelou alargamento do mediastino superior. No internamento diagnosticou-se um linfoma B que terá condicionado compressão da veia cava superior. Comentário: A dificuldade em diferenciar angioedema de SVCS revelou-se fulcral no desenvolvimento do presente caso. Situações de confusão diagnóstica entre estas duas entidades são já descritas na literatura. Questões relativas à complexidade das apresentações clínicas e às dificuldades sentidas pelo MF na gestão do presente caso serão discutidas. Introduction: The family doctor often sees signs and symptoms of an apparently benign nature that may turn out to be part of more severe disease. This case presents the common finding of facial edema that was interpreted as angioedema, when the underlying disease was the superior vena cava syndrome (SVCS). Case description: A 61-year-old man consulted his family doctor because of an episode of syncope. He was sent to the hospital emergency department, where vasovagal syncope was diagnosed and a microcytic hypochromic anemia was found. Subsequent investigations revealed iron deficiency anemia and a gastric antral ulcer. Three months later, he returned to the Family Health Unit with facial edema and was treated with an antihistamine. Because of persistence of his complaints, he consulted at the hospital emergency department, where angioedema was diagnosed. One month later, he was admitted to the hospital for treatment of gastrointestinal bleeding from the gastric ulcer. He later returned to the hospital emergency department with respiratory symptoms. A chest radiograph revealed enlargement of the superior mediastinum. Following admission to hospital, the diagnosis of a B-cell lymphoma was made after an incisional biopsy of an enlarged cervical lymph node. Comment: This case illustrates the diagnostic challenge presented by lymphoproliferative disorders. Distinguishing between angioedema and superior vena cava syndrome was a central challenge. Cases of diagnostic confusion between these two entities have been described in the literature. Questions related to the complexity of clinical presentations and the difficulties experienced by the family doctor in the management of this case are discussed.

Resumo: Neoplasmas em suínos são raros. Esse trabalho descreve os neoplasmas encontrados em suínos na rotina diagnóstica de um laboratório de patologia veterinária localizado na Região Central do Rio Grande do Sul. Durante um período de 49 anos, 2.266 casos de várias afecções em suínos foram encontrados, dos quais 37 (1,6%) eram neoplasmas. Em ordem decrescente de frequência, os seguintes neoplasmas foram encontrados: Linfoma (11/37), nefroblastoma (11/37), melanoma (8/37) e papiloma (2/37). Adicionalmente, um caso de cada um dos seguintes tumores foi observado: Adenoma hepatocelular, carcinoma hepatocelular, colangiocarcinoma, histiocitoma fibroso maligno e sarcoma granulocítico. O aspecto macroscópico e histológico desses tumores é descrito e a sua epidemiologia é comparada com os dados disponíveis na literatura para neoplasia suína. Abstract: Neoplasms in swine are rare. This paper describes neoplasms found in swine in the diagnostic routine of a veterinary pathology laboratory in the Central Region of Rio Grande do Sul, Brazil during a 49-year period, during which 2,266 cases of the various affections in swine were diagnosed. Of those 37 cases (1.6%) were neoplasms. In decreasing order of prevalence, the following neoplasms were found: lymphoma (11 out of 37 cases), nephroblastoma (11/37), melanoma (8/37), and papilloma (2/37). Adenoma hepatocelular, carcinoma hepatocelular, cholangiocarcinoma, malignant fibrous histiocytoma, granulocytic sarcoma were each found in one case out of the 37 cases. The gross and histological aspects of these tumors are described and their epidemiology is compared with the data available in the literature for neoplasia in swine.

Fundamentos: Los factores relacionados con la recepción de cuidado informal (CI) en pacientes oncológicos es una cuestión poco estudiada. El objetivo fue analizar los factores sociodemográficos y clínicos asociados a la recepción de cuidado informal en pacientes con neoplasia hematológica a lo largo de las diferentes etapas del tratamiento. Métodos: 139 pacientes diagnosticados de neoplasia hematológica que recibieron un trasplante de células madre durante el período 2006-2011 en dos centros sanitarios españoles completaron la encuesta elaborada para el estudio. Se estimó un modelo de regresión logística binaria para cada una de las cuatro etapas de tratamiento: pretrasplante, primer año, segundo y tercer año, cuarto al sexto año postrasplante. La variable dependiente fue recibir o no cuidado informal. Resultados: Pacientes diagnosticados de leucemia aguda presentaron mayor probabilidad de recibir CI durante la etapa pretrasplante (OR = 6,394) y durante el segundo y tercer año postrasplante (OR = 42,212). A largo plazo (4o-6o año) los pacientes con mieloma múltiple fueron los que requirieron mayor cuidado (OR = 15,977). El estado de salud fue estadísticamente significativo en la mayoría de las etapas. Ser hombre (OR = 0,263), tener pareja (OR = 0,137) y estar empleado (OR = 0,110) se asociaron a una menor probabilidad de recibir CI a largo plazo. Conclusiones: El CI está presente en más del 75% de los pacientes con neoplasia hematológica durante la etapa pretrasplante y primer año postrasplante. El diagnóstico y estado de salud son decisivos en la probabilidad de recibirlo, pero no el tipo de trasplante. Los factores sociodemográficos cobran protagonismo a largo plazo. Background: There is little information on factors related to use of to informal care in cancer patients. Our objective is to study sociodemographic and clinical factors associated with use of informal care in patients with hematologic malignancy and analyze how these changes throughout different phases of the treatment. Methods: 139 patients diagnosed with hematologic malignancy who received an haematopoietic stem cell transplantation between 2006-2011 in two Spanish hospitals completed the developed postal questionnaire. A binary logistic regression model was used to analyse the factors associated with use of informal care each of four phases of the treatment (pretransplant, first year, second and third year, and from the fouth to sixth year postransplant). Dependent variable was receive vs. not receive informal care. Results: Patients diagnosed with acute leukemia had higher probability of receiving informal care during pretransplant period (OR = 6.394) and during the second and third year postransplantation (OR = 42.212). In the long-term (4-6 years), multiple myeloma patients were the ones who required more informal care (OR = 15.977). Health status was statistically significant during all phases. Being male (OR = 0.263), having partner (OR = 0.137) and being employed (OR = 0.110) were associated with lower likelihood of receiving informal care in the long-term. Conclusions: Over 75% of patients diagnosed with hematologic malignancy received informal care during pretrasplant and first year postransplant. Type of diagnosis and health status are decisive factors in the probability of receiving informal care at all phases, while the type of transplantation is not. CONTEXT AND OBJECTIVE: Positron emission tomography with [18]F-fluoro-2-deoxyglucose (FDG-PET/CT) has been advocated as the method of choice for lymphoma staging, since it enables whole-body analysis with high sensitivity for detection of affected areas and because it combines capacities for anatomical and functional assessment. With technological advances, magnetic resonance imaging (MRI) has emerged as an alternative to FDG-PET/CT. This systematic review with meta-analysis aimed to compare whole-body diffusion-weighted MRI (WB-MRI) with FDG-PET/CT for lymphoma staging. DESIGN AND SETTING: Systematic review on diagnostic test accuracy studies conducted at a public university. METHODS: The Medline, Scopus, Embase and Lilacs databases were searched for studies published up to September 2013 that compared WB-MRI and FDG-PET/CT for lymphoma staging. The reference lists of included studies were checked for any relevant additional citations. RESULTS: Six studies that evaluated the initial lymphoma staging in 116 patients were included. WB-MRI and FDG-PET/CT agreed in 90.5% of the cases (κ = 0.871; P < 0.0001). In most of the studies, when there was disagreement between the methods, WB-MRI overstaged in relation to FDG-PET/CT. The sensitivity of WB-MRI and FDG-PET/CT, in comparison with the clinical-radiological standard, ranged from 59 to 100% and from 63 to 100% respectively. CONCLUSION: WB-MRI is a highly sensitive method for initial lymphoma staging. It has excellent agreement with FDG-PET/CT and is a great alternative for managing lymphoma patients, without using ionizing radiation or an intravenous contrast agent. CONTEXTO E OBJETIVO: A tomografia por emissão de pósitrons com 2-[18F]-fluoro-2-deoxi-D-glicose (FDG-PET/CT) tem sido defendida como método de escolha para o estadiamento do linfoma por realizar o estudo do corpo inteiro com boa sensibilidade para detecção das áreas acometidas e por combinar as capacidades de avaliação anatômica e funcional. Com os avanços tecnológicos, a ressonância magnética tem se apresentando como alternativa à FDG-PET/CT. Esta revisão sistemática com metanálise visa comparar a ressonância magnética de corpo inteiro (WB-MRI) com difusão com a FDG-PET/CT no estadiamento do linfoma. TIPO DE ESTUDO E LOCAL: Revisão sistemática de estudos de acurácia diagnóstica conduzida em universidade pública. MÉTODOS: Foi conduzida uma busca nos bancos de dados Medline, Embase, Scopus e Lilacs por estudos publicados até setembro de 2013 comparando a WB-MRI com a FDG-PET/CT no estadiamento do linfoma. As referências bibliográficas dos estudos incluídos foram checadas com a finalidade de encontrar citações adicionais relevantes. RESULTADOS: Foram incluídos seis estudos que avaliaram o estadiamento inicial do linfoma de 116 pacientes. A WB-MRI e a FDG-PET/CT concordaram em 90,5% dos casos (κ = 0,871; P < 0,0001). Na maioria dos estudos, quando houve discordância, a WB-MRI estabeleceu estadiamento superior à FDG-PET/CT. A sensibilidade da WB-MRI e da FDG-PET/CT, em relação ao padrão clínico-radiológico, variou de 59% a 100% e de 63% a 100%, respectivamente. CONCLUSÃO: A WB-MRI apresenta alta sensibilidade no estadiamento inicial do linfoma, excelente concordância com a FDG-PET/CT e representa uma ótima alternativa no manejo de pacientes com linfoma, sem utilizar radiação ionizante ou meio de contraste intravenoso.

Background Cyclooxygenase 2 (COX-2) is an inducible isoform by cellular activation, proinflammatory cytokines and growth factors. The aims of the current study were to evaluate COX-2 immunoexpression in epithelial and lamina propria (LP) of cats with inflammatory bowel disease (IBD) and low grade alimentary lymphoma (LGAL), as well as to correlate them with clinical signs and histopathological scoring. Cats diagnosed with IBD and LGAL (2007–2013) were included in the current study. Feline chronic enteropathy activity index (FCEAI) was calculated for all cases. Control group was composed by 3 healthy indoor cats and 5 sick cats died or were euthanized (non-gastrointestinal illness). Diagnosis and classification of IBD and LGAL was established according to the WSAVA gastrointestinal standardization group template and the National Cancer Institute formulation, respectively. Furthermore, a modified WSAVA template was applied for LGAL evaluation. Immunolabelling for COX-2 (polyclonal rabbit anti-murine antibody) was performed on biopsy samples. Epithelial and LP (inflammatory or neoplastic cells) COX-2 immunolabelling was calculated according to the grade and intensity. The most representative segment scored by the WSAVA and the modified WSAVA were used for statistical analysis. Results Significant difference was found regarding COX-2 intensity overexpression in the epithelial cells of IBD and LGAL groups when compared to control cats, but not between the groups of sick cats, whereas no differences were found regarding the grade of immunoreactivity between groups. No difference was found for COX-2 immunoexpression at the LP between all groups. However, 3 cats from LGAL group showed COX-2 expression in neoplastic cells at the LP. There were no correlations between epithelial or LP COX-2 expression and FCEAI and histological alterations. Conclusions Increased COX-2 intensity at the epithelial cells observed in cats with IBD and LGAL may be secondary to the inflammatory response or a protective function in the intestinal reparation. COX-2 expression at the LP was presented in 33% of LGAL. This result provides a reason for further investigation concerning the role of COX-2 expression in feline alimentary lymphoma.

Purpose There is no consensus on the optimal systemic treatment of patients with extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue. The IELSG-19 phase III study, to our knowledge, was the first such study to address the question of first-line treatment in a randomized trial. Patients and Methods Eligible patients were initially randomly assigned (1:1 ratio) to receive either chlorambucil monotherapy (6 mg/m2/d orally on weeks 1 to 6, 9 to 10, 13 to 14, 17 to 18, and 21 to 22) or a combination of chlorambucil (same schedule as above) and rituximab (375 mg/m2 intravenously on day 1 of weeks 1, 2, 3, 4, 9, 13, 17, and 21). After the planned enrollment of 252 patients, the protocol was amended to continue with a three-arm design (1:1:6 ratio), with a new arm that included rituximab alone (same schedule as the combination arm) and with a final sample size of 454 patients. The main end point was event-free survival (EFS). Analysis of chlorambucil versus the combination arm was performed and reported separately before any analysis of the third arm. Results At a median follow-up of 7.4 years, addition of rituximab to chlorambucil led to significantly better EFS (hazard ratio, 0.54; 95% CI, 0.38 to 0.77). EFS at 5 years was 51% (95% CI, 42 to 60) with chlorambucil alone, 50% (95% CI, 42 to 59) with rituximab alone, and 68% (95% CI, 60 to 76) with the combination ( P = .0009). Progression-free survival was also significantly better with the combination ( P = .0119). Five-year overall survival was approximately 90% in each arm. All treatments were well tolerated. No unexpected toxicities were recorded. Conclusion Rituximab in combination with chlorambucil demonstrated superior efficacy in mucosa-associated lymphoid tissue lymphoma; however, improvements in EFS and progression-free survival did not translate into longer overall survival.

Purpose: To determine the feasibility of mRNAs (C-MYC, BCL-XL, BCL-6, NF-κβ, PTEN and AKT) in exosomes of plasma as a liquid biopsy method for monitoring and prognostic evolution in B-cell lymphomas. Patients and Methods: Exosomes were isolated from 98 patients with B-cell Lymphoma and 68 healthy controls. mRNAs were analyzed by quantitative PCR. An additional 31 post-treatment samples were also studied. Results: In the general and follicular lymphoma series, the presence of AKT mRNA was associated with poor response to rituximab-based treatment. Patients with first relapse or disease progression showed a lower percentage of PTEN and BCL-XL mRNA. The presence of BCL-6 mRNA was associated with a high death rate. The absence of PTEN mRNA in the general series, and presence of C-MYC mRNA in follicular lymphomas, were associated with short progression-free survival. BCL-6 and C-MYC mRNA were independent prognostic variables of overall survival. C-MYC mRNA may provide prognostic information with respect to overall survival. BCL-XL mRNA and increase of BCL-6 mRNA in post-treatment samples could serve as molecular monitoring markers. Conclusions: This is the first large study to evaluate the prognostic and predictive values of pretreatment tumor-associated mRNA in exosomes. BCL-6 and C-MYC mRNA positivity in pretreatment samples were predictors of worse PFS compared to patients with mRNA negativity. Clinical case The case is presented of 37-year-old male with a history of nasal obstruction with right rhinorrhea, headache, hearing loss and right exophthalmos of 4 months progression. The MRI revealed that the ethmoidal and maxillary sinuses contained inflammatory tissue extending into the orbital region. The biopsy confirmed a non-Hodgkin lymphoma of natural killer (NK) T cells. Discussion Non-Hodgkin's T NK lymphoma is a rare tumour in the orbital area that requires an early detection and multi-disciplinary care to ensure appropriate monitoring and treatment.

Por meio de um estudo retrospectivo, os aspectos epidemiológicos, clínicos e anatomopatológicos de 128 casos de linfoma bovino são descritos. Dos protocolos que informavam o sexo (n=111), 84,7% correspondiam a fêmeas e 15,3% a machos. Dos protocolos em que constava a raça (n=108), a mais prevalente foi a holandesa (63%). Em relação à idade (n=107), houve uma variação entre um e 14 anos. A maioria dos bovinos era adulta (89,7%) e a maior concentração dos casos ocorreu ao redor de 5-8 anos (57,9%). Em relação aos sinais clínicos (n=89), linfadenomegalia foi o achado mais frequentemente observado (74,1%). Outros sinais clínicos, principalmente aqueles relacionados com os sistemas respiratório (dispneia, estertoração pulmonar e taquipneia), cardiovascular (taquicardia, edema subcutâneo e pulso venoso positivo), digestório (atonia ruminal, timpanismo e diarreia) e nervoso (paresia dos membros pélvicos e andar cambaleante), foram pouco prevalentes. Na necropsia (n=125), 71,2% dos bovinos apresentavam aumento de volume dos linfonodos; essa linfadenomegalia foi classificada como localizada em 89,6% dos casos e generalizada em 10,3% dos casos. Dos protocolos que informavam os linfonodos acometidos (n=58), a distribuição foi a seguinte: mesentéricos (51,7%), mediastínicos (37,9%), pré-escapulares (29,3%), ilíacos internos (27,6%), inguinais superficiais (25,8%) e traqueobrônquicos (18,9%). Além dos linfonodos, outros órgãos comumente afetados pelo linfoma neste estudo incluíram: coração (40%), fígado (15,2%), rim (14,4%), abomaso (12,8%), útero (11,2%), intestino (10,4%) e pulmão (7,2%). A presença de massas tumorais no canal vertebral foi observada em poucos casos (3,2%). Com base na epidemiologia e na localização das lesões, a maioria dos casos (96%) foi classificada como linfoma enzoótico e o restante (4%) como linfoma esporádico. Os resultados encontrados neste estudo irão auxiliar clínicos de grandes animais e patologistas veterinários na suspeita e no diagnóstico definitivo do linfoma na espécie bovina.
