Resumen Objetivo Revision y descripcion de la afectacion otoaudiologica en el seguimiento de 23 ninos con diagnostico de mucopolisacaridosis (MPS) tipo I , II , III y IV . Metodos Estudio retrospectivo de los hallazgos clinicos, audiologicos y tratamiento (medico y/o quirurgico) de 23 ninos con diagnostico de MPS tipo I , II , III o IV en seguimiento en un hospital terciario entre 1997 y 2015. Resultados Seis casos de MPS I , 8 de MPS II , 4 de MPS III y 5 de MPS IV fueron revisados. Al inicio del seguimiento el 71,2% de los pacientes presentaban otitis media serosa (OMS) y el 54% de los casos presentaban algun tipo de hipoacusia. El comportamiento de la hipoacusia fue fluctuante en cada uno de los subgrupos de MPS, encontrando mayor afectacion y variabilidad en los tipos I y II . Conclusiones Los ninos afectos de MPS tienen un alto riesgo de hipoacusia, siendo MPS tipo I y II los casos con mayor porcentaje de afectacion audiologica y con un comportamiento menos homogeneo, mostrando un importante porcentaje de hipoacusias transmisivas que progresan a componentes mixtos o neurosensoriales. Se requiere un seguimiento periodico dada la importante repercusion de esta patologia en la calidad de vida y en el desarrollo de estos pacientes.

Objective The aim of our study is to reflect hearing impairment of 23 children diagnosed with mucopolysaccharidosis (MPS) type I, II, III and IV. Methods Retrospective study of the clinical, audiological and treatment (medical vs surgical) findings of 23 children diagnosed with MPS type I, II, III and IV followed at a Tertiary Referral Hospital between 1997 and 2015. Results Six cases of MPS I, 8 of MPS II, 4 of MPS III and 5 of MPS IV were reviewed. 71.2% of patients had secretory otitis media (SOM) and 54% of patients had some type of hearing loss (HL). The behaviour of hearing loss was variable in each of the subgroups of MPS, finding greater involvement and variability in types I and II. Conclusions Children with MPS have a high risk of hearing loss. A significant percentage of transmissive HL progressing to mixed or sensorineural HL was observed. This was more common in types I and II. Periodic follow up of these patients is mandatory because of hearing impairment and consequences for their development and quality of life.

Summary Objectives To compare the immunogenicity, safety, and interchangeability of two pediatric hepatitis A vaccines, Avaxim 80U-Pediatric ® and Havrix 720 ® , in Chilean children. Methods In this randomized trial, 332 hepatitis A virus (HAV) seronegative children from 1 to 15 years of age received two doses of Avaxim, two doses of Havrix, or Havrix followed by Avaxim, 6 months apart. Anti-HAV antibody titers were measured before and 14 days after the first dose of vaccine, and before and 28 days after the second dose of vaccine. Immediate reactions were monitored; reactogenicity was evaluated from parental reports. Results Seroconversion rates after the first vaccination were 99.4% and 100% for Avaxim and Havrix, respectively. Anti-HAV geometric mean concentrations (GMCs) were 138mIU/ml for Havrix (95% confidence interval (CI): 120; 159) and 311mIU/ml for Avaxim (95% CI: 274; 353). GMCs increased to 4008mIU/ml after two doses of Havrix, 8537mIU/ml following two doses of Avaxim, and 7144mIU/ml in children who received Havrix with Avaxim as the second dose. Following the first injection, 36% of subjects given Avaxim and 44% given Havrix reported local reactions; 38% of subjects in the Avaxim group and 40% in the Havrix group reported systemic reactions related to vaccination. Solicited reactions were less frequent after the second dose of Avaxim or Havrix, occurring in 27% to 37% of subjects. Conclusions No significant difference in seroconversion rates was seen 14 days after a single dose of vaccine. A two-dose schedule with either vaccine or with Havrix/Avaxim provided a strong booster response. Both vaccines were well tolerated and can be recommended for routine vaccination of Chilean children. Avaxim 80 may be used to complete a vaccine schedule begun with Havrix 720.

Resumen El linfoma anaplasico de celulas grandes es una patologia rara en la infancia. La localizacion mas frecuente es en la piel y nodulos linfaticos, siendo infrecuente la afectacion del pabellon auricular. Presentamos el caso de un nino de 6 anos con tumoracion en el lobulo de la oreja. Se realizo tratamiento quirurgico y el resultado de la anatomia patologica mostro linfoma anaplasico de celulas grandes. Los estudios radiologicos fueron negativos y no hubo afectacion sistemica.

Background Current hepatitis A vaccines are either licensed for children >2 years of age, as in the U.S. or Chile, or >1 year of age, as in Europe and other parts of the world. Recent recommendations for immunization against hepatitis A have included routine vaccination of children in areas or regions of higher endemicity. However, data on hepatitis A vaccination in toddlers aged between 1 and 2 years are scarce. Methods This open clinical study investigated the reactogenicity and immunogenicity of two doses (0, 6-month schedule) of an inactivated hepatitis A vaccine (Havrix™ pediatric, Glaxco SmithKline Biologicals, Rixensart, Belgium) in 120 seronegative children aged 12–24 months. Results Pain at the injection site and irritability were the most frequently reported local and general symptoms, respectively. No serious adverse events related to the study vaccine were reported. One month after the first dose, all but one subject had antibodies against hepatitis A with a GMT of 159 mIU/mL. After the booster dose, all had antibodies with a GMT of 2,939 mIU/mL. Conclusions Our data show that the inactivated hepatitis A vaccine was well tolerated by these toddlers and that the vaccine elicits a good immune response.

Anaplastic large T-cell lymphoma is a very rare disease in childhood. The most common locations are lymph nodes and skin, while the external ear location is uncommon. We present the case of a 6-year-old child with an earlobe tumour. Surgical treatment was performed and the anatomopathological results showed anaplastic large cell lymphoma. Radiological tests were negative and there was no systemic involvement.
