Abstract: Edoxaban is used for venous thromboembolism (VTE) treatment. Real-life data are lacking about its use in long-term therapy. We aimed to assess the efficacy and the safety of edoxaban for long-term VTE treatment in a real-life setting. Patients with VTE included in the Registro Informatizado Enfermedad TromboEmbólica (RIETE) registry, receiving edoxaban 60 or 30 mg daily were prospectively followed up to validate the benefit of using different dosages. The main outcome was the composite of VTE recurrences or major bleeding in patients with or without criteria for dose reduction. Multivariable analysis to identify predictors for the composite outcome was performed. From October 2015 to November 2019, 562 patients received edoxaban for long-term therapy. Most (94%) of the 416 patients not meeting criteria for dose reduction received 60 mg daily, and 92 patients meeting criteria (63%) received 30 mg daily. During treatment, two patients developed recurrent VTE, six had major bleeding and nine died (2 from fatal bleeding). Among patients not meeting criteria for dose reduction, those receiving 30 mg daily had a higher rate of the composite event (hazard ratio (HR) 8.37; 95% confidence interval (CI) 1.12–42.4) and a significant higher mortality rate (HR 31.1; 95% CI 4.63–262) than those receiving 60 mg. Among patients meeting criteria for dose reduction, those receiving 60 mg daily had no events, and a nonsignificantly higher mortality rate (HR 5.04; 95% CI 0.54–133) than those receiving 30 mg daily. In conclusion, edoxaban seems to be effective and safe for long-term VTE treatment in real life. Criteria for dose reduction should be reformulated., No funding was received for this work

Abstract: The association between elevated liver enzymes or FIB-4 (fibrosis index 4) and outcome in patients with venous thromboembolism (VTE) has not been evaluated. Data from patients in RIETE (Registro Informatizado Enfermedad TromboEmbólica) were used to assess the association between elevated liver enzymes or FIB-4 levels and the rates of major bleeding or death in apparent liver disease-free patients with acute VTE under anticoagulation therapy. A total of 6206 patients with acute VTE and without liver disease were included. Of them, 92 patients had major bleeding and 168 died under anticoagulation therapy. On multivariable analysis, patients with elevated liver enzymes were at increased mortality risk (HR: 1.58; 95% CI: 1.10–2.28), while those with FIB-4 levels > 2.67 points were at increased risk for major bleeding (HR: 1.69; 95% CI: 1.04–2.74). Evaluation of liver enzymes and FIB-4 index at baseline in liver disease-free patients with VTE may provide additional information on the risk for major bleeding or death during anticoagulation. Abstract: OBJECTIVES: Despite extensive clinical use, limited data are available on optimal loading and maintenance doses of vancomycin in critically ill patients. This study aimed to develop a rational approach for optimised dosage of vancomycin given in a continuous infusion in critically ill patients. METHODS: Vancomycin pharmacokinetic (PK) data (total serum concentrations) were obtained from 55 intensive care unit (ICU) patients (Bach Mai Hospital, Hanoi, Vietnam) receiving a 20 mg/kg loading dose followed by continuous infusion stratified by creatinine clearance (CLCr). Population PK modelling and Monte Carlo simulations were performed using a nonlinear mixed-effects modelling (NONMEM) program for a target of 20-30 mg/L to optimise efficacy and minimise nephrotoxicity. RESULTS: A two-compartment model with first-order elimination best fitted the PK data with central and peripheral volumes of distribution of 1.01 and 2.39 L/kg, respectively (allometric scaling to a 70 kg standard subject). The population total clearance of 3.63 L/h was only explained by renal function in the covariate and final model. The simulations showed that a 25-mg/kg loading dose infused over 90 minutes was optimal to reach the target range. The optimal maintenance dose for low renal function (CLCr < 45 mL/min) was 1000-1500 mg/day. For augmented renal clearance (CLCr > 130 mL/min) the dose should be up to 3500 mg/day or even 4500 mg/day to achieve adequate exposure. These simulated maintenance doses were larger than previously proposed for non-ICU patients. CONCLUSION: Large loading and maintenance doses of vancomycin are generally needed in critically ill patients. Because of high interindividual variability in vancomycin PK, drug monitoring may still be necessary.

Abstract: In the current era of patient empowerment and precision medicine, access to timely information is critical to decision-making. Unfortunately, we currently lack patient-specific, real-time data about clinical presentation, risk of thrombotic or hemorrhagic events, key risk factors, and adverse outcomes in patients with venous thromboembolism (VTE). Accordingly, the Registro Informatizado Enfermedad TromboEmbólica (RIETE) investigators developed a tool to provide an open-source, real-time graphic representation of VTE-related data derived from over 90 000 patients with confirmed VTE. This information is intended to facilitate discussion in the informed decision-making process. The current article describes the aims, rationale, methods, and ongoing and future efforts of the real-time VTE infographics developed by the RIETE registry collaborators.

Abstract: Background Patients with venous thromboembolism (VTE) secondary to transient risk factors may develop VTE recurrences after discontinuing anticoagulation. Identifying at‐risk patients could help to guide the duration of therapy. Methods We used the RIETE database to assess the prognostic value of d‐dimer testing after discontinuing anticoagulation to identify patients at increased risk for recurrences. Transient risk factors were classified as major (postoperative) or minor (pregnancy, oestrogen use, immobilization or recent travel). Results In December 2018, 1655 VTE patients with transient risk factors (major 460, minor 1195) underwent d‐dimer measurements after discontinuing anticoagulation. Amongst patients with major risk factors, the recurrence rate was 5.74 (95% CI: 3.19–9.57) events per 100 patient‐years in those with raised d‐dimer levels and 2.68 (95% CI: 1.45–4.56) in those with normal levels. Amongst patients with minor risk factors, the rates were 7.79 (95% CI: 5.71–10.4) and 3.34 (95% CI: 2.39–4.53), respectively. Patients with major risk factors and raised d‐dimer levels (n = 171) had a nonsignificantly higher rate of recurrences (hazard ratio [HR]: 2.14; 95% CI: 0.96–4.79) than those with normal levels. Patients with minor risk factors and raised d‐dimer levels (n = 382) had a higher rate of recurrences (HR: 2.34; 95% CI: 1.51–3.63) than those with normal levels. On multivariate analysis, raised d‐dimers (HR: 1.74; 95% CI: 1.09–2.77) were associated with an increased risk for recurrences in patients with minor risk factors, not in those with major risk factors. Conclusions Patients with raised d‐dimer levels after discontinuing anticoagulant therapy for VTE provoked by a minor transient risk factor were at an increased risk for recurrences. Abstract: Background: This study was conducted in order to explore the use of quantitative ultrasound (QUS) based higher-order texture derivatives in predicting recurrence for patients with locally advanced breast cancer (LABC) early during neoadjuvant chemotherapy (NAC). Methods: Eighty-three patients with LABC were scanned with 7 MHz ultrasound before starting NAC (week 0) and during treatment (week 4). Spectral parametric maps were generated corresponding to tumor volume. Twenty-four textural features (QUS-Tex1) were determined from parametric maps acquired using grey-level co-occurrence matrices (GLCM) for each patient, which were further processed to generate 64 texture derivatives (QUS-Tex1-Tex2), leading to a total of 95 features from each time points. Analysis was carried out on week 4 data and compared to baseline (week 0) data. ∆Week 4 data was obtained from the difference in QUS parameters, texture features (QUS-Tex1), and texture derivatives (QUS-Tex1-Tex2) of week 4 data and week 0 data. Patients were divided into two groups based on their clinical status: recurrence and no-recurrence. Machine learning algorithms using k-nearest neighbor (k-NN) and support vector machines (SVM) were used to generate radiomic models. Results: With a median follow up of 69 months (range 7-118 months), 28 patients had disease recurrence. The k-NN classifier was the best performing algorithm at week 4 with sensitivity, specificity, accuracy, and area under curve (AUC) of 87%, 75%, 81%, and 0.83, respectively. The inclusion of texture derivatives (QUS-Tex1-Tex2) in week 4 QUS data analysis led to the improvement of the classifier performances. The AUC increased significantly from 0.70 (without texture derivatives) to 0.83 (with texture derivatives). The most relevant features in separating the two groups were higher-order texture derivatives obtained from scatterer diameter and acoustic concentration-related parametric images. Conclusion: This is the first study highlighting the utility of QUS radiomics in the prediction of recurrence during the treatment of LABC. It reflects that the ongoing treatment-related changes can predict clinical outcomes with higher accuracy as compared to pretreatment features alone. 7. Total nodule number is an independent prognostic factor in resected stage III non-small cell lung cancer: a deep learning powered study

Abstract: Almost every lung cancer patient has multiple pulmonary nodules while the significance of nodule multiplicity in locally advanced non-small cell lung cancer (NSCLC) remained unclear. This study explores the relationship between deep learning detected total nodule number (TNN) and survival outcomes in patients with surgical resected stage I-III NSCLC. Patients who underwent surgical resection for stage I-III NSCLC with accessible preoperative chest CT scan from 2005 to 2018 were identified from our database. Deep learning-based AI algorithms using convolutional neural networks (CNN) was applied for pulmonary nodule (PN) detection and classification. Of the 2126 patients, a total number of 33410 PNs were detected by AI. Median TNN detected per person was 12 (IQR 7-20). AI-detected TNN (analyzed as continuous variable) was independent prognostic factor for both RFS (HR 1.012, 95% CI 1.002-1.022, p = 0.021) and OS (HR 1.013, 95% CI 1.002-1.025, p = 0.021) in multivariate analyses of stage III cohort; while it was not significantly associated with survival in stage I and II cohorts. In terms of nodule categories, the numbers of upper-lobe nodule, same-side nodule, other-side nodule, solid nodule, and even solid nodule at small size (≤ 6mm) were independent prognostic factors; while the numbers of middle/lower-lobe nodule, same-lobe nodule, subsolid nodule, calcific nodule and perifissural nodule were not associated with survival. In survival tree analysis, rather than using traditional IIIA and IIIB classification, the model grouped cases by AI-detected TNN (lower vs. higher: log-rank p < 0.001), which showed superior discrimination of survival in stage III cohort. In conclusion, AI-detected TNN was significantly associated with survival in patients with surgical resected stage III NSCLC. Lower TNN detected on preoperative CT scan indicated better prognosis in patients who underwent complete surgical resection.

Abstract: International hospitals and healthcare facilities are facing catastrophic financial challenges related to the COVID-19 pandemic. The American Hospital Association estimates a financial impact of $202.6 billion in lost revenue for America's hospitals and healthcare systems, or an average of $50.7 billion per month. Furthermore, it could cost low- and middle-income countries ~ US$52 billion (equivalent to US$8.60 per person) each four weeks to provide an effective healthcare response to COVID-19. In the setting of the largest daily COVID-19 new cases in the US, this burden will influence patient care, surgeries, and surgical outcomes. From a global economic standpoint, The World Bank projects that global growth is projected to shrink by almost 8% with poorer countries feeling most of the impact, and the United Nations projects that it will cost the global economy around 2 trillion dollars this year. Overall, a lack of preparedness was a major contributor to the struggles experienced by healthcare facilities around the world. Items such as personal protective equipment (PPE) for healthcare workers, hospital equipment, sanitizing supplies, toilet paper, and water were in short supply. These deficiencies were exposed by COVID-19 and have prompted healthcare organizations around the world to invent new essential plans for pandemic preparedness. In this paper, we will discuss the economic impact of COVID-19 on US and international hospitals, healthcare facilities, surgery, and surgical outcomes. In the future, the US and countries around the world will benefit from preparing a plan of action to use as a guide in the event of a disaster or pandemic.

Abstract: BackgroundCurrent guidelines suggest treating cancer patients with incidental pulmonary embolism (PE) similarly to those with clinically suspected and confirmed PE. However, the natural history of these presentations has not been thoroughly compared.MethodsWe used the data from the RIETE (Registro Informatizado de Enfermedad TromboEmbólica) registry to compare the 3-month outcomes in patients with active cancer and incidental PE versus those with clinically suspected and confirmed PE. The primary outcome was 90-day all-cause mortality. Secondary outcomes were PE-related mortality, symptomatic PE recurrences and major bleeding.ResultsFrom July 2012 to January 2019, 946 cancer patients with incidental asymptomatic PE and 2274 with clinically suspected and confirmed PE were enrolled. Most patients (95% versus 90%) received low-molecular-weight heparin therapy. During the first 90 days, 598 patients died, including 42 from PE. Patients with incidental PE had a lower all-cause mortality rate than those with suspected and confirmed PE (11% versus 22%; OR 0.43, 95% CI 0.34–0.54). Results were consistent for PE-related mortality (0.3% versus 1.7%; OR 0.18, 95% CI 0.06–0.59). Multivariable analysis confirmed that patients with incidental PE were at lower risk of death (adjusted OR 0.43, 95% CI 0.34–0.56). Overall, 29 (0.9%) patients developed symptomatic PE recurrences, and 122 (3.8%) had major bleeding. There were no significant differences in PE recurrences (OR 0.62, 95% CI 0.25–1.54) or major bleeding (OR 0.78, 95% CI 0.51–1.18).ConclusionsCancer patients with incidental PE had a lower mortality rate than those with clinically suspected and confirmed PE. Further studies are required to validate these findings, and to explore optimal management strategies in these patients.

Abstract: Underuse of the antipsychotic clozapine for schizophrenia is an impediment to improving outcomes for patients. Because of its possible severe side effects, including granulocytopenia or even agranulocytosis, clozapine treatment entails regular WBC monitoring, which can be a major drawback for patients and practitioners. The HemoCue WBC DIFF system is a point-of-care device using capillary blood sampling which provides WBC counts with differentials, including granulocytes. We investigated if capillary sampling instead of conventional venous sampling might diminish the burden for patients and practitioners and motivate them to continue clozapine treatment. A randomized cross-over trial design was used to compare the two sampling methods. Patients׳ subjective experiences of various aspects of blood sampling were rated on a 10-cm visual analogue scale (VAS). Patients and practitioners were also asked if they had any preference for venous or capillary sampling and patients were asked if the sampling method influenced their motivation to continue clozapine treatment. Abstract: International audience; This paper proposes a full-rate cooperative scheme adapted to the slow fading semi-orthogonal multiple access relay channel (MARC). It is assumed that the sources transmit on orthogonal channels and the relay is half duplex transmitting on the same channels as the one of the sources. The sources employ LDPC to encode their messages before transmitting them, while the relay uses a simple demodulate-and-forward strategy to transmit the combination of the received codewords. The presence of the relay does not affect the time-scheduling of the sources. Therefore, this scheme is backward compatible to existing non-cooperative systems. In order to cancel the interference at the destination, we propose a joint network-channel decoder that uses maximum a posteriori (MAP) detection and channel decoding. Through numerical results, we show the benefits of joint network-channel decoder. We also show that the proposed scheme significantly outperforms, in terms of BER, the non-cooperative system as well as the classical relay-assisted orthogonal channel network and that it achieves the maximum code diversity.

Abstract: Androgen deprivation therapy by bilateral orchiectomy (surgical castration) or luteinizing hormone-releasing hormone agonist therapy (medical castration) is recommended for advanced or metastatic prostate cancer. Both methods aim at reducing serum testosterone concentrations to a castrate level which is currently defined as less than 50 ng/dl. The results of previous studies are based on testosterone immunoassays that have insufficient accuracy in the low range. In this study we reevaluated serum testosterone concentrations in men on androgen deprivation therapy using isotope dilution-liquid chromatography-tandem mass spectrometry, an accurate method of measuring testosterone in the castrate range. Subjects underwent surgical castration (34) or received a luteinizing hormone-releasing hormone agonist (32). Serum samples were obtained more than 3 months after surgery or initiation of luteinizing hormone-releasing hormone agonist therapy. Testosterone levels were determined using isotope dilution-liquid chromatography-tandem mass spectrometry. Dihydroepiandrosterone sulfate, androstenedione, sex hormone-binding globulin and inhibin B levels were determined. All subjects had serum testosterone values less than 50 ng/dl and 97% had testosterone concentrations less than 20 ng/dl. Abstract: What's known on the subject? and What does the study add? The male steroid hormone metabolism is an important target in the treatment of prostatic diseases. However, present literature is inconsistent about intraprostatic concentrations of steroid hormones and the role of intraprostatic steroid hormones in the arise and the course of prostatic diseases is yet to be determined. Part II of this two-piece mini-review reviews the effect of medication that alters the male steroid hormone metabolism (i.e. 5-alpha reductase inhibitors, androgen deprivation therapy) on intraprostatic steroid hormone concentrations. Better knowledge of the intraprostatic steroid hormone concentrations might lead to more individualized treatment and even to new medical targets. Androgen deprivation therapy (ADT) and 5-α-reductase (5AR) inhibition are used in the treatment of men with advanced or metastatic prostate cancer and benign prostatic hyperplasia (BPH), respectively. These drugs exert their effect by lowering androgen levels in the serum and allegedly, the prostate gland. It is, however, unknown whether (increased) intraprostatic androgen levels are associated with the pathogenesis of BPH and with the initiation and progression of prostate cancer. Also, it is unclear whether intraprostatic dihydrotestosterone (DHT) levels correlate with a response to initial hormonal therapy or with patient outcome. These uncertainties have resulted from the finding that serum testosterone levels do not necessarily reflect those in the prostate gland. Intraprostatic DHT levels of men being treated with 5AR inhibition, of those treated with ADT for hormone-naive prostate cancer, and of those with castration-resistant prostate cancer are all altered in an equivalent manner because of hormonal manipulation. Increased knowledge of the mechanisms of the androgenic steroid pathways in prostatic diseases, with a special focus on intraprostatic androgen levels, may lead to treatment that is tailored to the needs of the individual patient, and probably to new therapeutic targets as well.

Abstract: Owing to inconsistencies and methodological differences, the present peer-reviewed literature lacks conclusive data on the intraprostatic levels of androgens, in particular dihydrotestosterone (DHT), in untreated benign prostatic hyperplasia (BPH) and prostate cancer. To date, no difference has been shown between DHT concentrations in normal prostatic tissue and BPH, and nor has a difference been shown in DHT concentrations between the histologically distinct regions of the prostate. Recent literature has also failed to show a consistent difference in androgen level between BPH and prostate cancer. The role of intraprostatic DHT in the pathogenesis of BPH and in the initiation and progression of prostate cancer thus remains to be established. Increased knowledge of the mechanisms of the androgenic steroid pathways in prostatic diseases, with a special focus on intraprostatic androgen levels may lead to more optimized and more personalized forms of treatment, and probably new therapeutic targets as well.

Abstract: Objective: The objective of our study was to determine reference intervals and biologic variation for testosterone (T), free testosterone (fT), and free androgen index (FAI) in women with accurate methods and to test the discriminative value of these parameters in a polycystic ovary syndrome (PCOS)-population. Methods: Serumwas obtained daily during a normal menstrual cycle from 25 healthy women (677 data-points). A single serum sample was obtained from 44 PCOS-patients. T was measured by LC-MS/MS and by Architect® 2nd generation T Immunoassay. Sex hormone-binding globulin was measured to calculate fT and FAI. Results: Reference intervals which were established in healthy women with an ovulatory menstrual cycle were T = 0.3-1.6 nmol/L and 0.5-2.0 nmol/L, fT = 5.2-26 pmol/L and 7.2-33 pmol/L, and FAI = 0.4-2.9 and 0.6-4.4, by LC-MS/MSand immunoassay, respectively. T, fT and FAIwere higher in PCOS patients than in controls (p b 0.0001). The areas under the curve of receiver operator characteristic (ROC) plots were not different for T, fT, or FAIwhen Twasmeasured by LC-MS/MSversus immunoassay based on prediction of PCOS. FAI and fTwere the strongest predictors of PCOS. Conclusions: When based upon the appropriate reference intervals and ROC analysis, LC-MS/MS and second generation immunoassay have equivalent clinical utility for the diagnosis of PCOS.

Abstract: Our goal was to characterize pathological tissues of liver by magnetic resonance (MR)-related parameters such as T1 and magnetization transfer (MT) indices and to evaluate the clinical efficacy of MT contrast (C) in diagnosis of liver diseases via binomial pulsed saturation, with and without the administration of the paramagnetic agent gadolinium diethylenetriamine pentaacetic acid (Gd-DTPA). Fifty-one cases of liver disorders were included in this study. Among the more important findings were the following: a) cirrhotic livers have significantly higher MT indices than normal liver, while hepatoma, metastatic tumor and fatty liver have sub-normal MT indices; b) in general, although with notable exceptions, images with MT give significantly better contrast indices than control images; and c) MT with Gd-DTPA rarely fares any better than the MT technique alone, although again with notable exceptions. MTC is a potentially powerful technique for diagnosing liver diseases, provided it can be optimally exploited for each individual disease type.

Abstract: Introduction In our hospital, female-to-male (FtM) transgender adolescents from the age of 16 are treated with two- or four-weekly intra-muscular injections of testosterone-esters. Some patients treated with four-weekly injections have complaints of fatigue and experience mood swings towards the end of the inter-injection period, which calls for an evaluation of the time-course of testosterone levels between injections. Evaluation of salivary testosterone is a practical approach for sequential measurements. Since only ∼2% of total serum testosterone is present in saliva, a sensitive assay is necessary. The objective was to develop an isotope dilution-liquid chromatography–tandem mass spectrometry method (ID-LC–MS/MS) for salivary testosterone measurements and to evaluate the testosterone profiles after testosterone-ester mixture injections in FtM-adolescents. Experimental FtM treated with 125 mg/2 weeks or with 250 mg/4 weeks depots of testosterone-ester mixture collected saliva at different time intervals. Salivary testosterone was measured by a thoroughly validated ID-LC–MS/MS assay. Results An ID-LC–MS/MS method for measuring salivary testosterone was developed with adequate accuracy and specificity. The reference range was established at 135–400 pmol/L. Testosterone levels peaked supra-physiologically immediately post-injection, and decreased to levels within the male reference range after nine days in all patients. 250 mg/4 weeks depots resulted in values below the reference range at the end of the 4 weeks. Discussion The development of an adequate ID-LC–MS/MS method for measuring salivary testosterone allowed us to investigate the testosterone profile in FtM-adolescents after testosterone-esters mixture injections. These injections lead to extreme concentrations which may affect the wellbeing of the patients.

Abstract: Objective To evaluate the relationship between the body mass index (BMI) and serum testosterone concentrations in men receiving luteinizing hormone-releasing hormone (LHRH) agonist therapy for prostate cancer. Materials and Methods A total of 66 white men were included in the present study. All subjects had received LHRH agonist therapy for ≥3 months. The BMI was calculated, and the subjects were classified as normal weight (i.e. BMI 2 ), overweight (BMI 25-30 kg/m 2 ), or obese (BMI >30 kg/m 2 ). The serum testosterone concentration was determined using the highly sensitive isotope dilution-liquid chromatography-tandem mass spectrometry technique. The sex hormone-binding globulin level was determined using an immunometric assay, and the free serum testosterone concentration was calculated. Results The median serum testosterone concentration of the patients with a BMI 2 was 5.5 ng/dL. The patients with a BMI of 25-30 kg/m 2 had a median serum testosterone concentration of 3.8 ng/dL. Those patients with a BMI >30 kg/m 2 had a median concentration of 5.7 ng/dL. No significant difference in the serum testosterone concentrations among the 3 groups was found. The sex hormone-binding globulin levels declined with an increasing BMI. The concentration of free testosterone was significantly greater in the obese men. Conclusion Using an ultrasensitive technique of serum testosterone measurement, the present data have shown that no difference exists in the serum testosterone concentration in the castrate range among normal weight, overweight, and obese patients receiving LHRH agonist therapy for prostate cancer. From our findings and current knowledge, more stringent follow-up or changes in dosage or dosage intervals of LHRH agonist therapy in those with a greater or high BMI is not warranted.

Group 1 included nine patients with cervical-drainage varices draining into brachiocephalic vein; group 2 included 41 patients with varices draining into azygos vein and was subdivided as group 2A, 15 patients with variceal hemorrhage, and group 2B, 26 without variceal hemorrhage.Azygos flow rate and velocity were increased in patients compared with that in control subjects. ABF in group 2A was faster than that in group 2B (cutoff, 15 cm/sec; sensitivity, 80%; specificity, 89%). Group 1 had ABF close to that of group 2B, but variceal hemorrhage occurred in seven of nine patients in group 1 versus 15 of 41 patients in group 2 (78% vs 37%; P = .029).Patients with cirrhosis with cervical-drainage varices or ABF greater than 15 cm/sec may have higher risk for variceal hemorrhage.

Abstract: To the Editor: The monitoring of antiandrogen treatment in patients with prostate cancer, investigation of hyperandrogenism in women, and evaluation of infants with ambiguous genitalia require accurate measurement of low testosterone concentrations. However, testosterone immunoassays have been shown to be inaccurate and often to overestimate testosterone concentrations in the low range (1). A working group of the Endocrine Society recently reviewed this concern and presented several recommendations to ensure the accuracy of future testosterone testing for improvement of diagnosis and treatment of disease (2). In the present study we evaluated the current situation with regard to the accuracy of 7 testosterone immunoassays, including 2 second generation assays, by comparison with isotope-dilution liquid chromatography–tandem mass spectrometry (ID-LC-MS/MS). In addition, we investigated the possible improvement of these immunoassays by diethyl ether sample extraction. Serum from 50 men, 50 women, and 16 children (age 4–16 years) was collected, divided into aliquots, and stored (−20 °C) until analysis. Abstract: A method of performing magnetization transfer contrast (MTC) using a pulse sequence based on a series of on-resonance binomial pulses preceding a conventional spin-echo sequence has been recently described. We investigated this technique in the evaluation of circumscribed hepatic neoplasms on a 0.5 T imager. Conventional spin-echo imaging was performed in 18 patients with hepatic neoplasms, 15 with primary or metastatic hepatic neoplasms, and 3 with cavernous hemangiomas. The T1-weighted sequence was repeated following MTC saturation in 7 patients and the proton density sequence was repeated following MTC saturation in 13 patients (both were used in two patients). The mean lesion-to-liver contrast for neoplasms worsened with T1-MTC (0.87 vs. 1.04), but improved slightly with proton density-MTC (1.10 vs. 1.22) when compared to the corresponding non-MTC spin-echo images. However, these differences were not statistically significant (p = .19 and p = .16, respectively). The limited data on cavernous hemangiomas does not suggest these have consistently different MTC imaging characteristics from other hepatic neoplasms. In conclusion, MTC imaging using a binomial saturation pulse did not offer significant improvement in contrast between hepatic neoplasms and normal liver. Refinements of the binomial pulse sequence will be necessary if a consistent improvement in lesion sensitivity is to be achieved. Evaluation of its role in increasing diagnostic specificity will require further investigation.

Abstract: P in chemotherapy-naive patients (pts) with mCRPC, and to assess the frequency of interpretation of bone scans that is discordant with PSA and clinical response. METHODS: COU-AA-002 was a single-arm, open-label, multicenter, phase II study of 1000 mg AA with 5 mg po P BID in continuous 28-day cycles. Primary end point: pts achieving a 50% PSA decline by Week 12. Secondary end points: objective radiographic response (RECIST), time to PSA progression (TTPP), overall survival and clinical benefit of disease stabilization, change in ECOG status and safety. Bone scan flare was evaluated for all pts who had scans available at baseline, 4, and 7 months. RESULTS: Between October 2007 and May 2008, 33 pts with mCRPC were enrolled. A 50% PSA decline at Week 12 was seen in 22 pts (67%). Maximal PSA declines of 50% and 90% were seen in 26 (79%) and 15 (46%) pts, respectively, with undetectable PSA levels ( 0.1 ng/mL) in 2 pts. Overall, an objective tumor response occurred in 9/33 (27%) as partial responses; 2 had stable disease. The median time on therapy was 63 weeks (range, 8 –104 weeks), with 15 (46%) pts still on treatment. Median TTPP was 71 weeks (95% CI; 40 weeks, not estimable). ECOG-PS improved from 1 to 0 in 8 (24%) pts. Bone scan flare was observed in 30% overall and in 10/23 (43.5%) of those who experienced a decline in PSA. Adverse events were mainly grade 1 to 2, with the most common being fatigue (46%), hot flush (30%), bone pain (24%), peripheral edema (24%), arthralgia (21%), dizziness (21%), and hypokalemia (21%). CONCLUSIONS: Clinical response to AA P was frequent and durable in chemotherapy-naive pts with mCRPC, with over 50% on therapy for 1 year. The high rate of bone scan flare indicates that further investigation is needed to clarify the clinical meaning of this phenomenon. AA P was well tolerated. Maturing data from the COU-AA-302 phase 3 study comparing AA/P to placebo/P will further clarify the effects of abiraterone in chemotherapy-naive mCRPC.

Abstract: An alternative scheme in acquiring 13C spectroscopic images using inverse detection via polarization transfer through protons was proposed and experimentally verified by a phantom using a heteronuclear multiple-quantum coherence technique. This scheme has some features that, in special circumstances, can be exploited to one's advantages. These features are: (1) signal enhancement, a feat realizable under favorable conditions; (2) spectroscopic encoding via constant time; and (3) improvement of the time efficiency of the constant-time method via optimization by singular-value decomposition analysis. Features (1) and (2) can, in some cases, yield better sensitivity than the conventional 3DFT technique without sacrificing the most important attribute of 13C, the enormity of the chemical shifts. Such is the case because spectroscopic images acquired by the technique proposed are based on the chemical shifts of carbon, in contrast to the other more prevalent inverse-detection schemes. Feature (2) can also offer higher spatial resolution by shifting the emphasis of signal encoding from a spectral-resolution-first viewpoint to one that favors spatial resolution. Feature (3) was adopted from a method previously known as MILS (metabolite imaging of lines in a spectrum), or the inverse of SLIM, an economical scheme originally proposed by Hu et al. (1988, Magn. Abstract: We studied 20 patients with intracranial tumours (meningioma, astrocytoma, pituitary adenoma and adenocarcinoma metastasis with MTC (magnetisation transfer contrast) using binomial-pulse saturation at 1.0 T before and after intravascular Gd-DTPA. Our goal was to evaluate the use of MTC (alone and combination with Gd-DTPA) as a potential means of enhancing lesion contrasts in brain tumours. Our preliminary results, based on the image intensity attenuation measured quantitatively, indicate that MTC with (and without) Gd-DTPA cannot be used to differentiate tumour type. However, perifocal oedema in intra-parenchymal tumours is generally found to have a smaller intensity attenuation ratio than that in extra-parenchymal tumours. This observation, if verified by further study or other means, may be used for better delineation of the fringe area of the tumour than is possible with conventional MRI techniques.

Abstract: Two classes of low impedance, non-glass membrane electrodes for pH measurement were evaluated: (I) Metal-metal oxide electrodes and (II) Reduction-oxidation electrodes. The fundamental causes of oxygen sensitivity of metal-metal oxide electrodes were examined and three approaches for its suppression were proposed. For the case of Sb-Sb2Ox electrodes, oxygen sensitivity can be attenuated partially by cell loading, either directly across the reference electrode or indirectly across a third slave electrode. In a Po2 range of 8–54 kPa, more than 95% of the Po2 response can be suppressed by loading the cell emf to half of its open-circuit value. The oxygen sensitivity also was observed to be diminished by grinding the metal-metal oxide and pressing it under high pressure into a pellet electrode. Other metal-metal oxide electrodes that have promise in transcutaneous measurement are the Pd-Pdo2 electrodes. The redox electrodes are typified by the Quinhydrone electrode. A membrane Quinhydrone electrode showed a sensitivity of 56 mV/decade at 37dC and no oxygen sensitivity up to 50 kPa and a drift of 1 mV/h over a 24-h period. However, the stability deteriorated over a long-term period.

Abstract: because of the peculiar process through which the Ryukyu Islands (of which Okinawa is the largest) were subjected to American rule, and partly because of the peculiar nature and functions of the American bases on Okinawa, the problem of Okinawa reversion was one of the most debated issues in Japanese politics until Okinawa was returned to Japan on May 15, 1972. Although the Japanese and Okinawans were united in their demand for the earliest possible return of Okinawa to Japan, their opinions were deeply divided as to the proper terms of reversion to be obtained from the United States. Since the terms of reversion would affect not only the future status of the Okinawa bases but also the Japanese-American defense relations, the Sat6 Government, and its opposition, could not agree on the terms of reversion. In order to brine about a satisfactory solution to the Okinawa problem, the Sata Government had to take into consideration both the strategic thinking of the U.S. and Japanese public opinion concerning the status of the American bases on Okinawa after reversion, or else there could be no satisfactory solution to the reversion issue. The Japanese demanded the reversion of "nuclear-free" Okinawa with the bases placed under the same restrictions as those in Japan proper, while the U.S. insisted initially on the continued unrestricted use of the Okinawa bases, including the deployment of nuclear

Abstract: AFTER NEARLY A quarter-century of diplomatic estrangement and abnormality, Japan and China embarked on a new era of diplomatic relations as a result of the Sino-Japanese Joint Communique issued at Shanghai on September 29, 1972. The epoch-making event was possible due to a number of important international developments: the rapid thaw in Sino-American relations after President Nixon's announced intention on July 15, 1971, to visit China (or the "Nixon shock" as it is known in Japan); the seating of the People's Republic of China and the expulsion of Nationalist China from the United Nations in October 1971; and the Nixon-Chou Joint Communique issued at Shanghai on February 28, 1972. The Sino-Japanese rapprochement was also made possible by the more flexible China policy pursued by the Japanese Government after July 7, 1972. Following former Prime Minister Tanaka's historic trip to Peking in September 1972, several important steps were undertaken by Tokyo and Peking to facilitate normalization of Sino-Japanese relations. The Japanese Government closed down its embassy in Taipei on November 28, 1972, and established a new embassy in Peking on January 11, 1973, while the People's Republic of China (PRC) opened an embassy in Tokyo on February 1, 1973. By March 28, 1973, the exchange of ambassadors between Japan and China had taken place. The Japanese Government also carried out a series of negotiations with Peking for the purpose of concluding various administrative agreements stipulated in the Sino-Japanese Joint Communique of September 29, 1972 (e.g., trade, fisheries, aviation, navigation, etc.). Despite initial optimism expressed by officials of both governments, little progress was made in hammering out these agreements, regarded as essential for the full-fledged normalization of relations between Tokyo and Peking, until January 1974, largely due to Japan's existing non-diplomatic relations with Taiwan. Abstract: Advancements in computer science and magnet design have resulted in the recent development of high resolution NMR imaging systems. Using our primate model we evaluated the ability of NMR scanning to detect early changes following middle cerebral artery (MCA) occlusion. Serial NMR scans documented progressive changes secondary to edema and swelling beginning ninety minutes after MCA occlusion. NMR was also able to readily demonstrate the area of cerebral infarct 10 days after a six hour episode of MCA occlusion. Soft tissue contrast and image resolution were superb. Correlation with pathologic sections was excellent.

30. An Improved Sensor and a Method for Transcutaneous CO2Monitoring

Abstract: Since CO2 diffuses readily through intact skin along a reasonably short diffusion path, measurement of transcutaneous CO2 (tcPco2) is possible, provided the CO2 consumption of the electrode is minimal and epidermal capillary blood flow and stratum corneum permeability are maintained well. In this paper, a sensor and a methodology for measurement of arterial Pco2 (Paco2) by a continuous non-invasive transcutaneous technique is described. The sensor detects changes in pH which in turn is inversely proportional to the logarithm of partial pressure of CC2. The pH is measured by a pH-sensitive glass electrode whose half cell potential is sensitive to hydrogen ion concentration. The sensor consisted of a pH-sensitive electrode, a Ag/AgCl reference electrode, a thermistor to measure true sensor temperature, a buffer amplifier to increase signal-to-noise ratio, a servo-controlled heater unit, and an in situ calibration unit. Response time (90%) was 20 ± 2.7 sec. Maximum deviation in slope at 37dC during the four day period was from 59.16±3.38 to 60.64 ± 3.34 mV/decade. Temperature coefficient was specific for each electrode and varied between +0.37 to -2.12 mV/dC. The overall four day drift based on the first day slope and temperature coefficient was 0.87±0.47 kPa Pco2. Changes in mean drift value during 24 h were within 0.27 kPa Pco2. The sensor was applied on rabbits and newborn patients. For the best correlation between Paco2 and tcPco2, the skin surface temperature in rabbits was maintained between 40 and 42dC. Based on the in situ calibration, the fitted regression line of tcPco2 versus Paco2 had a slope of 0.92, an intercept of 0.3 kPa, and a standard error of 0.51 kPa. Preliminary data obtained on newborn patients suggest that for optimal tcPco2 to Paco2 correlation a temperature of 38dC is required. These data indicate the usefulness of tcPco2 monitoring to indicate Paco2 changes under normal physiologic conditions.

Abstract: Background: The association between heart rate (HR) and pulmonary embolism (PE) outcomes has not been well studied. Furthermore, optimal cutoffs to identify low-risk and intermediate- to high-risk patients are not well known. Research Question: Does an association exist between baseline HR and PE outcome across the continuum of HR values? Study Design and Methods: The current study included 44,331 consecutive nonhypotensive patients with symptomatic PE from the Registro Informatizado de la Enfermedad TromboEmbólica registry between 2001 and 2021. Outcomes included 30-day all-cause and PE-specific mortality. We used hierarchical logistic regression to assess the association between admission HR and outcomes. Results: A positive relationship was found between admission HR and 30-day all-cause and PE-related mortality. Considering an HR of 80 to 99 beats/min as a reference, patients in the higher HR strata showed higher rates of all-cause death (adjusted OR, 1.5 for HR of 100-109 beats/min; adjusted OR, 1.7 for HR of 110-119 beats/min; adjusted OR, 1.9 for HR of 120-139 beats/min; and adjusted OR, 2.4 for HR of ≥ 140 beats/min). Patients in the lower strata of HR showed significantly lower rates of 30-day all-cause mortality compared with the same reference group (adjusted OR, 0.6 for HR of 60-79 beats/min; and adjusted OR, 0.5 for HR of < 60 beats/min). The findings for 30-day PE-related mortality were similar. For identification of low-risk patients, a cutoff value of 80 beats/min (vs 110 beats/min) increased the sensitivity of the simplified Pulmonary Embolism Severity Index (sPESI) from 93.4% to 98.8%. For identification of intermediate- to high-risk patients, a cutoff value of 140 beats/min (vs 110 beats/min) increased the specificity of the Bova score from 93.2% to 98.0%. Interpretation: In nonhypotensive patients with acute symptomatic PE, a high HR portends an increased risk of all-cause and PE-related mortality. Modifying the HR cutoff in the sPESI and the Bova score improves prognostication of patients with PE.

Abstract: Aims Little is known about the prognosis of patients with massive pulmonary embolism (PE) and its risk of recurrent venous thromboembolism (VTE) compared with non-massive PE, which may inform clinical decisions. Our aim was to compare the risk of recurrent VTE, bleeding, and mortality after massive and non-massive PE during anticoagulation and after its discontinuation. Abstract: Registro Informatizado de Enfermedad Tromboembólica Investigators., [Objective] Overlap exists between the risk factors for coronary artery disease and venous thromboembolism (VTE). However, a paucity of data is available on the incidence of major acute cardiovascular events (MACE) and major adverse limb events (MALE) among patients presenting with VTE. Moreover, it is unknown whether the rate of cardiovascular outcomes differs among patients with unprovoked vs provoked VTE., [Methods] We analyzed the data from 2009 to 2017 in the Registro Informatizado de Enfermedad Tromboembólica registry, an ongoing, multicenter, international registry of consecutive patients with a diagnosis of objectively confirmed VTE. The query was restricted it to patients with data entry for the arterial outcomes. The baseline prevalence of coronary artery disease risk factors was compared between patients with provoked (ie, immobility, cancer, surgery, travel >6 hours, hormonal causes) and unprovoked VTE. After the initial VTE event, we followed up patients for the composite primary outcome of incident MACE (ie, stroke, myocardial infarction, unstable angina) and/or MALE (ie, major limb events). We used the χ2 test for baseline associations and a Cox proportional hazard for multivariate analysis. We used IBM SPSS, version 24 (IBM Corp, Armonk, NY) for statistical analysis. A P value of, [Results] We analyzed the data from 41,259 patients with VTE, of whom 22,633 (55.6%) had experienced a provoked VTE. During follow-up, the patients with provoked VTE were more likely to develop MACE or MALE than were patients with unprovoked VTE (hazard ratio [HR], 1.3; 95% confidence interval [CI], 1.1-1.5). The association of arterial events with recent immobility (HR, 1.4; 95% CI, 1.5-12.1) and cancer (HR, 1.7; 95% CI, 1.4-1.9) was strong. After adjusting for multiple conventional cardiovascular risk factors, provoked VTE, compared with unprovoked VTE, was significantly associated with an increased hazard for MACE (HR, 1.4; 95% CI, 1.1-1.7). Cancer remained a significant adjusted predictor for both MACE (HR, 1.7; 95% CI, 1.4-2.1) and MALE (HR, 2.1; 95% CI 1.01-4.6) in those with provoked VTE., [Conclusions] Among patients with VTE, provoked cases, specifically those with cancer-associated VTE, have an increased risk of major arterial events.

Abstract: BACKGROUND: Limited data exist about the clinical presentation, ideal therapy and outcomes of patients with hereditary hemorrhagic telangiectasia (HHT) who develop venous thromboembolism (VTE). METHODS: We used the data in the RIETE Registry to assess the clinical characteristics, therapeutic approaches and clinical outcomes during the course of anticoagulant therapy in patients with HHT according to initial presentation as pulmonary embolism (PE) or deep venous thrombosis (DVT). RESULTS: Of 51,375 patients with acute VTE enrolled in RIETE from February 2009 to January 2019, 23 (0.04%) had HHT: 14 (61%) initially presented with PE and 9 (39%) with DVT alone. Almost half (47.8%) of the patients with VTE had a risk factor for VTE. Most PE and DVT patients received low-molecular-weight heparin for initial (71 and 100%, respectively) and long-term therapy (54 and 67%, respectively). During anticoagulation for VTE, the rate of bleeding events (major 2, non-major 6) far outweighed the rate of VTE recurrences (recurrent DVT 1): 50.1 bleeds per 100 patient-years (95%CI: 21.6-98.7) vs. 6.26 recurrences (95%CI: 0.31-30.9; p = 0.020). One major and three non-major bleeding were epistaxis. No patient died of bleeding. One patient died shortly after being diagnosed with acute PE. CONCLUSIONS: During anticoagulation for VTE in HHT patients, there were more bleeding events than VTE recurrences.
