Real-time dense SLAM techniques aim to reconstruct the dense three-dimensional geometry of a scene in real time with an RGB or RGB-D sensor. An indoor scene is an important type of working environment for these techniques. The planar prior can be used in this scenario to improve the reconstruction quality, especially for large low-texture regions that commonly occur in an indoor scene. This article fully explores the planar prior in a dense SLAM pipeline. First, we propose a novel plane detection and segmentation method that runs at 200 Hz on a modern graphics processing unit. Our algorithm for constructing global plane constraints is very efficient; hence, we use it in the process of each input frame for the camera pose estimation while maintaining the real-time performance. Second, we propose herein a plane-based map representation that greatly reduces the memory footprint of plane regions while keeping the geometric details on planes. The experiments reveal that our system yields superior reconstruction results with planar information running at more than 30 fps. Aside from speed and storage improvements, our technique also handles the low-texture problem in plane regions.

In addition to 3D geometry, accurate representation of texture is important when digitizing real objects in virtual worlds. Based on a single consumer RGBD sensor, accurate texture representation for static objects can be realized by fusing multi-frame information; however, extending the process to dynamic objects, which typically have time-varying textures, is difficult. Thus, to address this problem, we propose a compact keyframe-based representation that decouples a dynamic texture into a basic static texture and a set of multiplicative changing maps. With this representation, the proposed method first aligns textures recorded from multiple keyframes with the reconstructed dynamic geometry of the object. Errors in the alignment and geometry are then compensated in an innovative iterative linear optimization framework. With the reconstructed texture, we then employ a scheme to synthesize the dynamic object from arbitrary viewpoints. By considering temporal and local pose similarities jointly, dynamic textures in all keyframes are fused to guarantee high-quality image generation. Experimental results demonstrate that the proposed method handles various dynamic objects, including faces, bodies, cloth, and toys. In addition, qualitative and quantitative comparisons demonstrate that the proposed method outperforms state-of-the-art solutions.

BackgroundPrevious studies have shown that leukocyte cell-derived chemotaxin 2 (LECT2), a hepatokine, is associated with obesity and non-alcoholic fatty liver disease (NAFLD). It is well known that hepatokines play important roles in mediating interactions among bone, adipose tissue, and liver. We sought to examine serum LECT2 levels in subjects with osteoporosis (OP) to confirm its association with OP.MethodsFrom March 2019 to March 2020, a total of 96 adult subjects (52 OP patients and 44 controls) visiting the 2nd Spine Department of the Affiliated Hospital of School of Medicine of Ningbo University were recruited. The bone mineral density (BMD) of all subjects were assessed by dual-energy X-ray (DXA). Blood samples were collected for measurements of high-sensitivity C-reactive protein (hs-CRP), plasma glucose (PG), alanine aminotransferase (ALT), aspartate aminotransferase (AST), total cholesterol (TC), triglycerides (TG), creatinine and uric acid. Serum LECT2 levels of total 96 participants were measured by enzyme linked immunesorbent assay (ELISA). The relationships between serum LECT2 levels and biomedical parameters were analyzed using the Spearman correlation coefficient.ResultsSerum LECT2 levels in OP patients were significantly higher than that of healthy controls (29.57 ng/mL VS 19.82 ng/mL, P < 0.01). To confirm the role LECT2 played in OP, we found a significantly negative correlation in all subjects between serum levels of LECT2 and lumbar BMD, as well as femoralneck BMD. A significantly positive correlation in all was observed between serum levels of LECT2 and TC, whereas there was a significantly negative correlation between serum levels of LECT2 and creatinine. Meanwhile, serum LECT2 levels were measured to diagnose OP patient by plotting receiver-operating characteristic (ROC) curve, the area under the ROC curve was 0.729(P < 0.01). The optimal cutoff point for LECT2 concentration to diagnose OP patient was 16.44 ng/mL.ConclusionsWe showed that serum LECT2 levels were significantly up-regulated in OP patients, and LECT2 levels were significant positively associated with total cholesterol and negatively associated with creatinine. It could be a potential biomarker for OP diagnosis.

Introduction Although capsaicin has long been used as food additive and medication worldwide, its actions on gastrointestinal tract as its most delivery pathway have not been well addressed. Objectives In the present study, we aimed to study GI actions of capsaicin on mesenteric arterioles in normal and colitis mice and to elucidate the underlying mechanisms. Methods Vasorelaxation of human submucosal arterioles and the mesenteric arterioles from wide-type (WT) mice, TRPV1−/− and TRPV4−/− (KO) mice were measured. The expression and function of TRPV channels in endothelial cells were examined by q-PCR, immunostaining, Ca2+ imaging and membrane potential measurements. Results Capsaicin dose-dependently induced vasorelaxation of human submucosal arterioles and mouse mesenteric arterioles in vitro and in vivo through endothelium-dependent hyperpolarization (EDH), nitric oxide (NO), and prostacyclin (PGI2). Using TRPV1 and TRPV4 KO mice, we found that capsaicin-induced vasorelaxation was predominately through TRPV4/EDH, but marginally through TRPV1/NO/PGI2. Capsaicin induced hyperpolarization through activation of endothelial TRPV4 channels and intermediate-conductance of Ca2+-activated K+ channels to finally stimulate vasorelaxation. Importantly, capsaicin exerted anti-colitis action by rescuing the impaired ACh-induced vasorelaxation in WT colitis mice but not in TRPV4 KO colitis mice. Conclusions Capsaicin increases intestinal mucosal blood perfusion to potentially prevent/treat colitis through a novel TRPV4/EDH-dependent vasorelaxation of submucosal arterioles in health and colitis. This study further supports our previous notion that TRPV4/EDH in mesenteric circulation plays a critical role in the pathogenesis of colitis.

In this paper, a unique one-dimensional double anti-PT-symmetric ring optical waveguide network (1D DAPTSROWN) is designed whose refractive index satisfies n ( x ) = − n ∗ ( − x ) . The extraordinary optical characteristics of the network are studied by means of the generalized Floquet-Bloch theorem, three-material network equation and generalized eigenfunction method. Due to the physical difference between the anti-PT-symmetric and PT-symmetric optical waveguide networks, the previous methond of the PT-symmetric optical waveguide network is not applicable to the anti-PT-symmetric optical waveguide network designed in this paper. Thus, based on the previous method and the network equation, we propose a connection function to identify the extremum mode points in the anti-PT-symmetric optical waveguide network. At the extremum mode point, the optical waveguide network can produce extraordinary ultra-strong transmission, ultra-strong reflection, and ultra-strong photonic localizations. The corresponding calculated values in this paper arrives at 1 0 28 , which is much larger than 1 0 12 (the best result reported previously). This shows that the anti-PT-symmetric network may have potential in applications in designing optical filters, optical amplifiers and so on.

Pipeline inner inspection technology based on Pipeline Inspection Gauge (PIG), is the primary means for ensuring the safety of submarine pipelines. The dynamic characteristics of a PIG can change abruptly with the excitation of obstacles such as girth welds inside the pipeline, which would result in failure or inaccuracy of the inspection results. This study establishes a dynamic model of the PIG sealing disc based on Kelvin spring damping in the circumferentially confined space. The axial vibration differential equations of the PIG is examined in detail. MSC/ADAMS is used to conduct the dynamic simulation of the PIG at different motion velocities and center of mass positions while passing through the girth weld process. Results indicate that the axial vibration caused by the girth weld intensifies substantially as the speed of the PIG increases, while the pitch and vertical vibrations exhibit a significant decline with an increase in the motion velocity. The change in the PIG’s center of mass positions has little effect on its axial vibration, while the pitch and vertical vibration conditions are significantly different in the same circumstances.

Compared to visual sensors that have light dependence and privacy intrusion issues, non-intrusion millimeter-wave (mmw) radars are more suitable for the daily person identification. In a realistic home scenario, there are new challenges that are not taken into account in existing research. This paper attempts to address these issues such as multipath interference, complex walking process and recognition robustness in smart home scenarios, and designs a lightweight multi-branch convolutional neural network with an Inception-Pool module and a Residual-Pool module to learn and classify gait Doppler features. The experimental results in a home living room scenario indicate that the designed mmw radar person identification system can achieve accurate and robust real-time identification performance.

In this study, a novel coupling structure based on half‐mode substrate‐integrated waveguide (HMSIW) is proposed with a small size and broad bandwidth. The HMSIW coupler is realised by connecting their equivalent magnetic walls with metal strips, which avoids exposing the semi‐enclosed magnetic walls outside. Hence, it has greater anti‐interference capability than the existing HMSIW couplers. The directional coupling principle is introduced by analysing the proposed single connecting strip coupler. In order to lower return loss and higher isolation, a 3 dB directional coupler working from 10.8 to 16.8 GHz is designed with two connecting strips. Finally, a six‐port circuit is achieved by utilising this compact 3 dB directional coupler. Furthermore, a broadband HMSIW power divider is optimised to satisfy the performance of this six‐port circuit. The proposed coupler with metal strips helps to increase bandwidth and reduce size, which makes it more applicable to demanding systems with complex requirements such as detecting and transceiver systems. The 3 dB coupler and six‐port circuit are fabricated and measured. The measured results are in accordance with the simulated data.

Microwave imaging with large rotating angle and sparse sampling is an attractive approach to obtain the high-resolution target image with reduced radar resource. However, the popular imaging methods, e.g., Range-Doppler (RD), back projection (BP), and sparse recovery (SR), are difficult to deal with large rotating angle and sparse sampling simultaneously. In recent years, deep learning (DL) has been widely studied and been successfully used to handle the problems in computer vision. However, since most existing DL networks are put forward for the real visual image and a large amount of data is essential for network training, DL cannot be directly used to process the complex and sparse target echo for microwave imaging. In this article, a new learning imaging framework is proposed and a model-driven learning imaging network (MDLI-Net) is built for high-resolution microwave imaging with large rotating angle and sparse sampling. In the proposed framework, the electromagnetic scattering model is used to generate the training data efficiently, and the sparse microwave imaging theory is applied to guide the design of the deep imaging network. By inputting the 2-D sparse complex-valued target echo, the trained MDLI-Net can output the high-resolution and focused target image efficiently. The effectiveness of the proposed learning imaging method is validated by experiment results with both simulated and real data.

We conducted a prospective randomized controlled trial to compare the radiological and clinical outcomes of Sanders type III calcaneal fractures treated with percutaneous prodding reduction and K-wire fixation via a sinus tarsi approach (PPRKF) versus open reduction and internal fixation (ORIF). Fifty-one patients with closed, unilateral, Sanders type III calcaneal fractures were randomly assigned to the PPRKF group (n = 26) or the ORIF group (n = 25). The clinical outcomes evaluated were time to surgery, blood loss, operative time, hospital stay, wound healing time, wound complications, and Maryland foot score. Radiological results were evaluated on lateral and axial X-rays and computed tomography images and included Böhler's angle, Gissane's angle, and calcaneal width. Compared with the ORIF group, the PPRKF group had shorter time to surgery, shorter operative time, less blood loss, shorter hospital stay, shorter wound healing time, and fewer wound complications (p.001). The postoperative Böhler's angle, Gissane's angle, and calcaneal width in both groups were significantly better than those measured preoperatively (p.001) and did not differ between the PPRKF group and ORIF group (p.05). Regarding clinical results, there was no significant difference in Maryland foot score between the two groups at 12 months after surgery (p.05). Both PPRKF and ORIF can result in satisfactory clinical function. PPRKF is superior to ORIF in reducing the time to surgery, operative time, blood loss, hospital stay, wound healing time, and wound complications.

3-D computerized ionospheric tomography (CIT) is an ill-posed problem due to the insufficient amount of observations, it remains challenging for practical applications. In this article, we proposed an ionospheric tomography method that combined data-driven methods with compressed sensing (CS) to deal with the ill-posed problem. First, slant total electron content (STEC) data were extracted by undifferenced and uncombined precise point positioning (UCPPP) with known fixed station coordinates. Second, data-driven methods were adopted to construct the projection matrix from the ionospheric model. Third, compressed sensing was used to derive the sparse solution based on L₁ norm. The ionospheric tomography can be achieved well by using observations during the shorter time interval and in a sparse receiver distribution based on the property of compressed sensing. Results of experiment based on real Global Positioning System (GPS) observation data verified the effectiveness of the proposed methods. By comparing with the colocated ionosonde, it is found that the CS methods are more consistent with the actual ionospheric fluctuation than the modified constrained algebraic reconstruction technique (CART). In terms of the differential STEC (dSTEC) analysis, the error of the tomography model by Compressed Sensing-Principal Component Analysis (CS-PCA) is less than 0.2 TEC unit (TECU), and the time resolution is 5 min. The UCPPP with constraint by CS-PCA shows the best performance of 12.2%, 40.9% and 0.31% improvement in positioning accuracy, convergence time, and fixed rate over the UCPPP with constraint by modified CART. The proposed data-driven methods may be important for high-resolution 4-D ionospheric tomography in the future.

Purpose Studies concerning do-not-resuscitate (DNR) orders in mainland China are rather scarce. We explored the prevalence and related factors of DNR orders among in-hospital cardiac arrest (IHCA) patients at a general tertiary hospital in mainland China. Materials and Methods We identified all IHCA patients hospital-wide between July 2019 and September 2020. Data regarding DNR status were collected from medical records. We investigated the frequency of DNR orders and explored the determinant factors of DNR establishment using logistic regression. Results A total of 1154 IHCA patients were included, 535 (46.4%) of whom established DNR orders. The following variables were independently associated with a higher DNR rate: female (OR 1.491; 95% CI 1.130–1.965), older age (OR 1.016; 95% CI 1.008–1.024), being a local resident (OR 1.790; 95% CI 1.344–2.383), pulmonary infection (OR 1.398; 95% CI 1052–1.859), respiratory insufficiency (OR 1.356; 95% CI 1.009–1.823), shock (OR 1.735; 95% CI 1.301–2.313), acute stroke (OR 1.821; 95% CI 1.235–2.686),neurological dysfunction (OR 1.527; 95% CI 1.149–2.028) and cancer (OR 3.316; 95% CI 2.461–4.468). Counterintuitively, patients with new-onset coronary artery disease (OR 0.592; 95% CI 0.419–0.837) were less likely to create DNR orders. Conclusion In mainland China, the DNR order signing rate is low, and the establishment of a DNR order is associated with demographics and comorbidity characteristics.

Systematic testing of existing drugs and their combinations is an attractive strategy to exploit approved drugs for repurposing and identifying the best actionable treatment options. To expedite the search among many possible drug combinations, we designed a combinatorial CRISPR–Cas9 screen to inhibit druggable targets. Coblockade of the N-methyl-d-aspartate receptor (NMDAR) with targets of first-line kinase inhibitors reduced hepatocellular carcinoma (HCC) cell growth. Clinically, HCC patients with low NMDAR1 expression showed better survival. The clinically approved NMDAR antagonist ifenprodil synergized with sorafenib to induce the unfolded protein response, trigger cell-cycle arrest, downregulate genes associated with WNT signaling and stemness, and reduce self-renewal ability of HCC cells. In multiple HCC patient-derived organoids and human tumor xenograft models, the drug combination, but neither single drug alone, markedly reduced tumor-initiating cancer cell frequency. Because ifenprodil has an established safety history for its use as a vasodilator in humans, our findings support the repurposing of this drug as an adjunct for HCC treatment to improve clinical outcome and reduce tumor recurrence. These results also validate an approach for readily discovering actionable combinations for cancer therapy. Significance: Combinatorial CRISPR–Cas9 screening identifies actionable targets for HCC therapy, uncovering the potential of combining the clinically approved drugs ifenprodil and sorafenib as a new effective treatment regimen.

Background Despite its popularity in recent years, segmentectomy still faces a challenge: the accurate delineation of the intersegmental plane, especially in complex segmentectomy. In this study, we describe a method using collateral ventilation to create an inflation-deflation line for video-assisted thoracoscopic surgery (VATS) segmentectomy and evaluated its efficacy in complex segmentectomy by comparing it with simple segmentectomy. Methods Enrolled in the study were 264 patients who underwent VATS segmentectomy from January 2017 to September 2018. We classified the clarity of the inflation-deflation line into 4 grades, and the procedures of grade 3 or 4 were considered successful. Meanwhile, we performed a propensity score matching analysis to compare complex and simple segmentectomy. Results Complete resection with free margins was achieved in all patients. In inflation-deflation line clarity evaluation, 245 patients were classified as grade 4 (92.8%), 10 as grade 3 (3.8%), 8 as grade 2 (3.0%), and 1 as grade 1 (0.4%). Procedural success (grade 3 or 4) was achieved in 255 patients (96.6%). Prolonged air leak (>5 days) was observed in 11 patients (4.2%). Propensity matching generated 83 pairs of well-matched patients. The proportion of procedural success and the incidence of prolonged air leak (>5 days) were similar in both groups. However, compared with simple segmentectomy, complex segmentectomy was associated with a longer median operative time (159 versus 135 minutes; P Conclusions Collateral ventilation method is simple, safe, and effective in VATS segmentectomy to identify the intersegmental plane, and also well-adapted for complex segmentectomy.

In this letter, two compact and high-performance filters based on multilayer slow-wave empty substrate-integrated waveguide (SW-ESIW) are presented. SW-ESIW, which contains the air cavity and a bottom layer including several rows of internal metallized via holes, provides a compromise between insertion loss and footprint. It is the first time that SW-ESIW is used to implement compact filters with multilayer air cavity. The proposed filters are simulated, fabricated, and measured. The measurements are consistent with simulations, indicating that the proposed filters can be reduced more than 35% in dimension compared to the same filters in ESIW. Based on the SW-ESIW technology, the proposed multilayer filters realize the advantages of low insertion loss and miniaturization, and express the tremendous potential on applications to complex and high-performance microwave circuits and systems.

Aim: We investigated the efficacy and safety of hepatic artery infusion chemotherapy (HAIC) combined with anti-PD-1 immunotherapy and tyrosine kinase inhibitors (TKIs) for advanced hepatocellular carcinoma (HCC). Method: This retrospective study included HCC patients treated with HAIC, TKIs and anti-PD-1 antibodies between May 2019 and November 2020 in our hospital. Primary end points were progression-free survival and safety. Results: Twenty-seven advanced HCC patients were analyzed. The median follow-up was 12.9 months (range: 4.0–24.0 months) and the median progression-free survival was 10.6 months. The objective response rate and disease control rate were 63.0 and 92.6%, respectively. No treatment-related deaths occurred. Conclusion: In patients with advanced HCC, treatment with HAIC, anti-PD-1 antibodies and oral TKIs was effective and safe.

Functional defecatory disorders (FDDs) are highly prevalent around the world. Biofeedback is an effective treatment for FDDs. Traditionally, this treatment is performed by clinicians in a limited number of hospitals because of procedure-related expenses and a need for a dedicated procedure room. To make the biofeedback therapy more widely available, we have designed a novel wireless, smartphone-based biofeedback device, with the ultimate goal of performing the therapy at home. The aim of this pilot study was to investigate whether the developed device can be employed to treat patients with FDDs in a clinical setting, prior to employing it in a home setting. From March 2018 to July 2018, we performed the biofeedback therapy using the newly developed wireless, smartphone-based device in patients with FDDs 30 min daily during weekdays for 2 weeks. A Visual Analogue Scale (VAS) for bowel satisfaction, Patient Assessment of Constipation‐Symptoms (PAC‐SYM), Patient Assessment of Constipation Quality of Life (PAC-QOL), and a balloon expulsion test were assessed at baseline and after the therapy. Thirteen patients were recruited and ten of them (mean age 70.3 ± 8.9 years, female:male ratio: 3:7) completed the study. Spontaneous bowel movements, complete spontaneous bowel movements, and VAS were all significantly increased after the therapy compared with baseline, respectively (6.5 ± 4.3 vs. 1.5 ± 2.2, p = 0.006; 3.1 ± 2.2 vs. 0.2 ± 0.6, p = 0.002; 49.5 ± 31.0 vs. 12.0 ± 9.2, p = 0.003). There was a significant decrease in the PAC-SYM and PAC-QoL after the biofeedback therapy (0.7 ± 0.6 vs. 1.4 ± 0.3, p = 0.001; 0.7 ± 0. 6 vs. 1.5 ± 0.4, p = 0.001). The therapy reduced the balloon expulsion time significantly (83.9 ± 68.8 s vs. 160.0 ± 36.7 s, p = 0.002). No obvious adverse events related to the procedure itself occurred. Biofeedback training using the newly developed wireless, smartphone-based device is feasible in the clinic setting, and it seems to be a promising method for improving constipation and related symptoms in patients with FDDs. These findings could be used to develop a much-needed, home-based, suitably powered, randomized, controlled clinical trial.

BACKGROUND Colorectal cancer (CRC) is one of the most common malignant tumors of the digestive tract. Lymphatic metastases of this tumor are mostly confined to the regional lymph nodes, and distant supraclavicular lymph node metastases are very rare. CASE SUMMARY In this report, we describe a patient with sigmoid carcinoma and isolated synchronous supraclavicular lymph node metastases. A 56-year-old male presented with a left cervical mass that was confirmed as a lymph node metastasis from sigmoid cancer by several auxiliary examinations. After 6 cycles of chemotherapy with the 5-fluorouracil, leucovorin and oxaliplatin + cetuximab regimen, the sigmoid colon tumor and Virchow’s lymph node metastasis were significantly smaller than before treatment, and no new metastatic sites were observed. Considering the effects of chemotherapy on quality of life, resection of the primary tumor was performed followed by 4 cycles of chemotherapy with the original chemotherapy regimen. Virchow’s lymph node dissection was selected by mutual consultation between the patient and us. After the second surgery, the patient received capecitabine and cetuximab chemotherapy and did not experience recurrence or metastasis during follow-up. CONCLUSION In conclusion, supraclavicular lymph node metastasis without any other solid organ metastasis is a potential metastatic pathway for CRC. In addition, after resection of the primary lesion, postoperative chemotherapy combined with supraclavicular lymph node dissection is feasible for the treatment of patients with CRC and isolated synchronous Virchow’s lymph node metastases.

The mixing morphology control plays a crucial role in photovoltaic power generation, yet this specific effect on device performances remains elusive. Here, we employed computational approaches to delineate the photovoltaic properties of layered heterojunction polymer solar cells with tunable mixing morphologies. One-step quench and two-step quench strategies were proposed to adjust the mixing morphology by thermodynamic and kinetic effects. The computation for the one-step quench revealed that modulating interfacial widths and interfacial roughness could significantly promote the photovoltaic performance of layered heterojunction polymer solar cells. The two-step quench can provide a buffer at a lower temperature before the kinetic quenching, leading to the formation of small-length-scale islands connected to the interface and a further increase in photovoltaic performance. Our discoveries are supported by recent experimental evidence and are anticipated to guide the design of photovoltaic materials with optimal performance.

Objective To evaluate five examination techniques in the diagnosis and treatment of osteoarticular tuberculosis (TB). Methods Microbiological samples were collected from a total of 284 patients during the period August 2017 to December 2019 in Wuhan Pulmonary Hospital. The specimens were examined by acid-fast bacillus (AFB) smear microscopy, mycobacterial culture, PCR, T-SPOT.TB, and X-pert MTB/RIF rapid molecular detection. Results The diagnostic sensitivity of the Xpert technology was 96.8% (116/120), specificity was 96.8% (58/60), the Youden index was 0.936, and the area under the receiver operating characteristic (ROC) curve was 0.967. The sensitivity and specificity of PCR were 84.2% (104/128) and 95.2% (76/80), respectively; the area under the ROC curve was 0.881. T-SPOT.TB had a detection sensitivity of 75.0% (12/16) and specificity of 85.0% (17/20). AFB smear microscopy had a sensitivity of 60.0% (75/125) and specificity of 95.8% (152/159). TB culture sensitivity was 58.1% (72/124) and specificity was 96.2% (73/76). The sensitivity and specificity of Xpert MTB/RIF for detecting rifampicin resistance were 100% (2/2) and 97.3% (73/75), respectively. Conclusions The Xpert MTB/RIF technique was found to have a good diagnostic value. With an additional diagnosis of Rifampicin resistance, it was also useful in tuberculosis therapy.

BACKGROUND Adenoid cystic carcinoma (ACC) occurs mainly in the head and neck. Tracheal ACC (TACC) is uncommon. Primary resection is recommended as the main treatment of choice, and radiotherapy is considered for residual disease in the postoperative setting. Definitive radiotherapy is an alternative approach to cure unresectable TACC. As the status of radiosensitivity in TACC is uncertain, the evidence for radiotherapy in unresectable TACC is not well established, especially in terms of the optimal dosage and its response evaluation. Herein, we report a case of locally advanced TACC. CASE SUMMARY A 49-year-old woman was diagnosed with TACC, which included a range of lesions arising in the upper trachea extending caudally 2 cm to 7 cm of the glottis. She was treated with definitive radiotherapy, given the low likelihood of complete resection of the disease. Due to the indolent growth and the propensity for infiltration along the airways, the scheduled radiation dose of 76 Gy in 38 fractions with 6-MV X-ray delivered by intensity-modulated radiotherapy was conducted to the primary tumor volume. After irradiation of 40 Gy, the patient’s dyspnea on exertion was dramatically relieved and bronchoscopy revealed that the previous large polypoid intra-luminal mass was significantly eliminated, with near-complete response. The patient completed two phases of scheduled radiotherapy, and acute reactions to treatment included subjective chest tightness and grade 2 esophagitis, managed medically. After 5 years of treatment, the patient is alive without recurrent disease, and there were no serious late radiation esophagus and lung damage, with only slight dysphagia without perforation and fistula. CONCLUSION Taken together, TACC is uncommon and the treatment of unresectable TACC is challenging. This case indicated that patients with unresectable TACC who rapidly respond to radiation may benefit from primary radical radiotherapy. Radiotherapy may be considered an effective alternative treatment modality.

Chemotherapy-induced side effects affect the quality of life and efficacy of treatment of cancer patients. Current approaches for treating the side effects of chemotherapy are poorly effective and may cause numerous harmful side effects. Therefore, developing new and effective drugs derived from natural non-toxic compounds for the treatment of chemotherapy-induced side effects is necessary. Experiments in vivo and in vitro indicate that Panax ginseng (PG) and its ginsenosides are undoubtedly non-toxic and effective options for the treatment of chemotherapy-induced side effects, such as nephrotoxicity, hepatotoxicity, cardiotoxicity, immunotoxicity, and hematopoietic inhibition. The mechanism focus on anti-oxidation, anti-inflammation, and anti-apoptosis, as well as the modulation of signaling pathways, such as nuclear factor erythroid-2 related factor 2 (Nrf2)/heme oxygenase-1 (HO-1), P62/keap1/Nrf2, c-jun N-terminal kinase (JNK)/P53/caspase 3, mitogen-activated protein kinase (MEK)/extracellular signal-regulated kinases (ERK), AMP-activated protein kinase (AMPK)/mammalian target of rapamycin (mTOR), mitogen-activated protein kinase kinase 4 (MKK4)/JNK, and phosphatidylinositol 3-kinase (PI3K)/AKT. Rib fracture is the most common injury in chest trauma. Most of patients with rib fractures were treated conservatively, but up to 50% of patients, especially those with combined injury such as flail chest, presented chronic pain or chest wall deformities, and more than 30% had long-term disabilities, unable to retain a full-time job. In the past two decades, surgery for rib fractures has achieving good outcomes. However, in clinic, there are still some problems including inconsistency in surgical indications and quality control in medical services. Before the year of 2018, there were 3 guidelines on the management of regional traumatic rib fractures were published at home and abroad, focusing on the guidance of the overall treatment decisions and plans; another clinical guideline about the surgical treatment of rib fractures lacks recent related progress in surgical treatment of rib fractures. The Chinese Society of Traumatology, Chinese Medical Association, and the Chinese College of Trauma Surgeons, Chinese Medical Doctor Association organized experts from cardiothoracic surgery, trauma surgery, acute care surgery, orthopedics and other disciplines to participate together, following the principle of evidence-based medicine and in line with the scientific nature and practicality, formulated the Chinese consensus for surgical treatment of traumatic rib fractures (STTRF 2021). This expert consensus put forward some clear, applicable, and graded recommendations from seven aspects: preoperative imaging evaluation, surgical indications, timing of surgery, surgical methods, rib fracture sites for surgical fixation, internal fixation method and material selection, treatment of combined injuries in rib fractures, in order to provide guidance and reference for surgical treatment of traumatic rib fractures.

The authors sought to explore whether hypertension classification was risk factor for lobar and non‐lobar hypertensive intracerebral hemorrhage (HICH) and the prognosis in patients with hematoma. This retrospective cohort study was conducted on HICH patients admitted at the First Affiliated Hospital of Soochow University. Observations with first‐ever intracerebral hemorrhage (ICH) were recruited. The authors divided the brain image into three groups according to the location of ICH to predict whether there were significant differences between lobar and non‐lobar ICH. A Mann‐Whitney U test was used and this retrospective trial also compared the operation and mortality rates. Our cohort included 209 patients (73.7% male; median age:60.5±16.7). The overall incidence of lobar HICH was less than non‐lobar HICH (24.4% vs. 68.4%), 7.2% cases of mixed HICH was included in this analysis. Digital polymerase chain reaction (dPCR) has found widespread applications in molecular diagnosis of various diseases owing to its sensitive single-molecule detection capability. However, the existing dPCR platforms rely on the auxiliary procedure to disperse DNA samples, which needs complicated operation, expensive apparatus, and consumables. Besides, the complex and costly dPCR readers also impede the applications of dPCR for point-of-care testing (POCT). Herein, we developed a portable digital loop-mediated isothermal amplification (dLAMP) platform, integrating a microscale hydrogel (microgel) array chip for sample partition, a miniaturized heater for DNA amplification, and a hand-held reader for digital readout. In the platform, the chip with thousands of isolated microgels holds the capability of self-absorption and partition of DNA samples, thus avoiding auxiliary equipment and professional personnel operations. Using the integrated dLAMP platform, λDNA templates have been quantified with a good linear detection range of 2-1000 copies/μL and a detection limit of 1 copy/μL. As a demonstration, the epidermal growth factor receptor L858R gene mutation, a crucial factor for the susceptibility of the tyrosine kinase inhibitor in non-small-cell lung cancer treatment, has been accurately identified by the dLAMP platform with a spiked plasma sample. This work shows that the developed dLAMP platform provides a low-cost, facile, and user-friendly solution for the absolute quantification of DNA, showing great potential for the POCT of nucleic acids.

BACKGROUND Atopic dermatitis (AD) is a chronic skin inflammation with a heterogeneous immunological profile. Leukocyte cell-derived chemotaxin 2 (LECT2) is a liver-derived multifunctional cytokine that has been studied due to its important role in inflammatory and autoimmune diseases. However, the relationship between AD and LECT2 has not been defined. This study was performed to investigate the levels of LECT2 in patients with AD and to determine whether it was associated with the severity and clinical characteristics of AD. METHODS The study included 42 AD patients and 30 healthy controls from the Affiliated Hospital of Medical School of Ningbo University. Participants' serum levels of LECT2 were measured using enzyme-linked immunosorbent assay kits. The values in the patient group and control group were statistically compared. The relationships between the different markers were evaluated by correlation analysis. RESULTS The serum levels of LECT2 were significantly higher in AD patients than those in the controls. Background Adenosine deaminase (ADA) is essential for the differentiation and maturation of lymphocytes, while lymphocytes infiltration in thyroid tissue is a vital pathological feature of Graves’ disease (GD). The aim of the present study was to compare the concentration of ADA between healthy controls (HC) and patients with GD, and evaluate the association between ADA and GD. Methods A total of 112 GD patients and 77 matched HC were enrolled in this study. Each participant was examined for thyroid hormones and autoantibodies, ADA concentration, and thyroid ultrasonography. Results Serum ADA levels in GD patients were significantly higher than that in HC subgroup (P < 0.001). In GD patients, serum ADA levels were positively associated with serum-free triiodothyronine (FT3), free thyroxine (FT4), thyroid peroxidase antibody (TPOAb), thyroid-stimulating hormone receptor antibody (TRAb) levels, and total thyroid gland volume (thyroid VolT) and negatively associated with serum thyroid-stimulating hormone receptor (TSH) levels (all P < 0.05). There were no similar correlations in the HC subgroup. Multiple linear regression analysis suggested that serum TSH, FT3, and ADA levels played an important role in serum TRAb levels. Conclusions Our results demonstrated that serum ADA levels were closely associated with GD.

Background Artificial intelligence (AI) assistance has been considered as a promising way to improve colonoscopic polyp detection, but there are limited prospective studies on real‐time use of AI systems. Methods We conducted a prospective, multicenter, randomized controlled trial of patients undergoing colonoscopy at six centers. Eligible patients were randomly assigned to conventional colonoscopy (control group) or AI‐assisted colonoscopy (AI group). AI assistance was our newly developed AI system for real‐time colonoscopic polyp detection. Primary outcome is polyp detection rate (PDR). Secondary outcomes include polyps per positive patient (PPP), polyps per colonoscopy (PPC), and non‐first polyps per colonoscopy (PPC‐Plus). Results A total of 2352 patients were included in the final analysis. Compared with the control, AI group did not show significant increment in PDR (38.8% vs. 36.2%, p = 0.183), but its PPC‐Plus was significantly higher (0.5 vs. 0.4, p, Artificial intelligence (AI) assistance plays a limited role in overall polyp detection. AI system increases detection of easily missed polyps, including non‐first polyps, diminutive and flat polyps. The impact is greater for male operators, longer withdrawal time, and elderly obese patients.

BACKGROUND AND PURPOSE The spinal cord central echo complex (SCCEC) is a special ultrasonography-based intramedullary structure, but its clinical significance in degenerative cervical myelopathy (DCM) is undefined. This study aimed to explore the potential of the SCCEC in predicting postoperative neurological recovery in DCM. METHODS Thirty-two DCM patients who underwent intraoperative ultrasonography-guided French-door laminoplasty were prospectively enrolled. The modified Japanese Orthopaedic Association (mJOA) score was evaluated preoperatively and 12 months postoperatively. SCCEC width (SCCEC-W), and anteroposterior diameter (APD) and transverse diameter (TD) of the spinal cord were measured on transverse ultrasonographic images, while the tissue widths from anterior and posterior borders of the spinal cord to the SCCEC were measured on sagittal ultrasonographic images. The APD of the spinal cord and occupying rate of the spinal canal were measured on preoperative magnetic resonance imaging (MRI). RESULTS All patients achieved improvements in mJOA scores, with an average recovery rate (RR) of 68.69 ± 20.22%. Spearman correlation analysis revealed that SCCEC-W, and ratios between the SCCEC-W and APD/TD based on ultrasonography, correlated moderately with mJOA score RR, with coefficients of -0.527, -0.605 and -0.514, respectively. The ratio between SCCEC-W and ultrasonographic TD correlated moderately with preoperative APD of the spinal cord. The MRI measurements and ultrasonography-based tissue widths showed no significant correlation with mJOA score RR. CONCLUSIONS The SCCEC may have predictive potential as an intraoperative indicator of neurological recovery in treating DCM. SCCEC-W may be related to spinal cord compression in DCM.

Multifunctional antimicrobial strategies are urgently needed to treat methicillin-resistant Staphylococcus aureus (MRSA) caused pneumonia due to its increasing resistance, enhanced virulence, and high pathogenicity. Here, we report that lysostaphin, a bacteriolytic enzyme, encapsulated within poly(lactic-co-glycolic acid) microspheres (LyIR@MS) specially treats planktonic MRSA bacteria, mature biofilms, and related pneumonia. Optimized LyIR@MS with suitable diameters could deliver a sufficient amount of lysostaphin to the lung without a decrease in survival rate after intravenous injection. Furthermore, the degradable properties of the carrier make it safe for targeted release of lysostaphin to eliminate MRSA, repressing the expression of virulence genes and improving the sensitivity of biofilms to host neutrophils. In the MRSA pneumonia mouse model, treatment or prophylaxis with LyIR@MS significantly improved survival rate and relieved inflammatory injury without introducing adverse events. These findings suggest the clinical translational potential of LyIR@MS for the treatment of MRSA-infected lung diseases.

Artificial intelligence including deep learning and 3D reconstruction methods is changing the daily life of people. Now, an unmanned aerial vehicle that can move freely in the air and avoid harsh ground conditions has been commonly adopted as a suitable tool for 3D reconstruction. The traditional 3D reconstruction mission based on drones usually consists of two steps: image collection and offline post-processing. But there are two problems: one is the uncertainty of whether all parts of the target object are covered, and another is the tedious post-processing time. Inspired by modern deep learning methods, we build a telexistence drone system with an onboard deep learning computation module and a wireless data transmission module that perform incremental real-time dense reconstruction of urban cities by itself. Two technical contributions are proposed to solve the preceding issues. First, based on the popular depth fusion surface reconstruction framework, we combine it with a visual-inertial odometry estimator that integrates the inertial measurement unit and allows for robust camera tracking as well as high-accuracy online 3D scan. Second, the capability of real-time 3D reconstruction enables a new rendering technique that can visualize the reconstructed geometry of the target as navigation guidance in the HMD. Therefore, it turns the traditional path-planning-based modeling process into an interactive one, leading to a higher level of scan completeness. The experiments in the simulation system and our real prototype demonstrate an improved quality of the 3D model using our artificial intelligence leveraged drone system.

This prospective non-randomized, multicenter clinical trial was performed to investigate efficacy and safety of 131I-labeled metuximab in adjuvant treatment of unresectable hepatocellular carcinoma. Methods: Patients were assigned to treatment with transcatheter arterial chemoembolization (TACE) combined with 131I-metuximab or TACE alone. The primary outcome was overall tumor recurrence. The secondary outcomes were safety and overall survival. Results: The median time to tumor recurrence was 6 months in the TACE+131I-metuximab group (n = 160) and 3 months in the TACE group (n = 160) (hazard ratio, 0.55; 95% confidence interval, 0.43 to 0.70; P < 0.001). The median overall survival was 28 months in the TACE+131I-metuximab group and 19 months in the TACE group (hazard ratio, 0.62; 95% confidence interval, 0.47 to 0.82; P = 0.001). Conclusion: TACE+131I-metuximab showed a greater anti-recurrence benefit, significantly improved the 5-year survival of patients with advanced hepatocellular carcinoma, and was well tolerated by patients.

The recent discovery of immune checkpoint inhibitors is a significant milestone in cancer immunotherapy research. However, some patients with primary or adaptive drug resistance might not benefit from the overall therapeutic potential of immunotherapy in oncology. Thus, it is becoming increasingly critical for oncologists to explore the availability of new immune checkpoint inhibitors. An emerging co-inhibitory receptor, CD112R (also called PVRIG), is most commonly expressed on natural killer (NK) and T cells. It binds to its ligand (CD112 or PVRL2/nectin-2) and inhibits the strength with which T cells and NK cells respond to cancer. Therefore, CD112R is being presented as a new immune checkpoint inhibitor with high potential in cancer immunotherapy. CD112 is easily detectable on antigen-presenting or tumor cells, and its high level of expression has been linked with tumor progression and poor outcomes in most cancer patients. This review explores the molecular and functional relationship between CD112R, TIGIT, CD96, and CD226 in T cell responses. In addition, this review comprehensively discusses the recent developments of CD112R/CD112 immune checkpoints in cancer immunotherapy and prognosis.

Objective To investigate the MRI features and clinical outcomes of unexpected uterine sarcomas in patients after high-intensity focused ultrasound (HIFU) ablation for presumed uterine fibroids. Materials and methods 15,759 consecutive patients who came for HIFU treatment, from November 2008 to September 2019, for presumed uterine fibroids were retrospectively reviewed. All the patients had completed a pre-HIFU MRI. All MRI images were independently analyzed and interpreted by two radiologists in every center. Results According to the T2WI MRI features of hyperintensity, accompanied by irregular margins, necrosis or cystic degeneration, multi-lobulated lesion with internal septation, 46 patients were suspected to be uterine sarcomas before HIFU. Eleven patients were histologically diagnosed as uterine sarcomas after laparotomy. Among the 15713 patients who received HIFU treatment for presumed uterine fibroids, 8 patients were found to have occult recurrence during the follow-up period, and 6 were confirmed histologically as uterine sarcomas after laparotomy. The incidence rate of uterine sarcomas was 0.108% (17/15759). Among them, 12 cases were low-grade endometrial stromal sarcoma (LG-ESS) and 5 cases were uterine leiomyosarcoma (LMS). No histological dissemination of the sarcoma was detected in patients with unexpected uterine sarcomas. Conclusion Although some MRI features of uterine sarcomas and uterine fibroids overlapped, MRI is valuable in distinguishing between uterine fibroids and uterine sarcomas. HIFU does not seem to cause histological dissemination of the sarcoma, but follow-up visits should be strictly adhered to in order to detect unexpected uterine sarcomas at an early stage and to treat them in a timely manner.

The aim of the present study was to evaluate the association between adenosine deaminase (ADA) levels and diabetic kidney disease (DKD) in patients with type 2 diabetes (T2D). In this study, patients with T2D who had been screened for DKD were recruited. Patients with an estimated glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2 or a urinary albumin-to-creatinine ratio (UACR) ≥ 30 mg/g for 3 months were identified as having DKD. The prevalence of DKD was 13.3%, and the range of serum ADA levels was 4–37 U/L. Serum ADA levels were positively associated with cystatin C levels and UACR (r = 0.295 and r = 0.302, respectively, both P < 0.05) and negatively associated with eGFR (r = −0.342, P < 0.05). The proportion of participants with DKD increased significantly from 3.8% in the first tertile (T1) to 13.6% in the second tertile (T2) and 25.9% in the third tertile (T3) of ADA (P for trend < 0.001). After adjusting for clinical risk factors for DKD via multiple logistic regression, the corresponding odds ratios (ORs) of DKD for the participants in T2 and T3 vs those in T1 of ADA were 5.123 (1.282–20.474) and 10.098 (1.660–61.431), respectively. Receiver operating characteristic (ROC) analysis revealed that the optimal cutoff value of ADA to indicate DKD was 10 U/L. Its corresponding sensitivity and specificity were 75.5 and 56.4%, respectively. Our results demonstrated that serum ADA levels were closely associated with DKD and partly reflect the risk of DKD in patients with T2D.

This study aimed to identify key common genes in OSA and lung cancer and explore their prognostic value in lung cancer.Materials and Methods: Transcriptome data of OSA and lung cancer were obtained from the Gene Expression Omnibus (GEO) and The Cancer Genome Atlas (TCGA) database, respectively. Genes associated with OSA and lung cancer were screened by weighted gene co-expression network analysis (WGCNA). Univariate and multivariate Cox regression algorithms were applied to identify key genes and construct the risk score model. Receiver operating characteristic (ROC) curves and a nomogram were performed to evaluate the prognostic value of the risk score. The screened key genes and their roles in prognosis were validated by GEO (GSE30219) analysis.Results: A total of 104 common genes were screened in OSA and lung cancer by WGCNA. Modulator of apoptosis 1 (MOAP1), chromobox 7 (CBX7), platelet-derived growth factor subunit B (PDGFB), and mitogen-activated protein kinase 3 (MAP2K3) were identified as key genes by univariate and then multivariate Cox regression analyses. The risk score model was constructed on the basis of four gene signatures. ROC curves and the nomogram showed that the risk score had a high accuracy in predicting the survival of patients with lung cancer. In addition, the result of multivariate Cox regression analysis indicated that the risk score was an independent prognostic factor in lung cancer.Conclusion: This study constructed a unique model for predicting the prognosis of lung cancer patients on the basis of four genes common to OSA and lung cancer. Background High-frequency rTMS over the dorsolateral prefrontal cortex (DLPFC) has demonstrated mixed effects on chronic and provoked pain. Objectives/Methods In this study, a meta-analysis was conducted to characterise the potential analgesic effects of high-frequency rTMS over the DLPFC on both chronic and provoked pain. Results A total of 626 studies were identified in a systematic search. Twenty-six eligible studies were included for the quantitative review, among which 17 modulated chronic pain and the remaining investigated the influence on provoked pain. The left side DLPFC was uniformly targeted in the chronic pain studies. While our data identified no overall effect of TMS across chronic pain conditions, there was a significant short-term analgesia in neuropathic pain conditions only (SMD = −0.87). In terms of long-lasting analgesia, there was an overall pain reduction in the midterm (SMD = −0.53, 24.6 days average) and long term (SMD = −0.63, 3 months average) post DLPFC stimulation, although these effects were not observed within specific chronic pain conditions. Surprisingly, the number of sessions was demonstrated to have no impact on rTMS analgesia. In the analysis of provoked pain, our data also indicated a significant analgesic effect following HF-rTMS over the DLPFC (SMD = −0.73). Importantly, we identified a publication bias in the studies of provoked pain but not for chronic pain conditions. Conclusions Overall, our findings support that HF-DLPFC stimulation is able to induce an analgesic effect in chronic pain and in response to provoked pain. These results highlight the potential of DLPFC-rTMS in the management of certain chronic pain conditions and future directions are discussed to enhance the potential long-term analgesic effects.

Background Deterioration of sleep quality has been reported to contribute to the incidence of diabetes and may be responsible for glycemic status in diabetes. The present study explored the relationship between sleep quality and glycemic variability in patients with type 2 diabetes (T2D). Methods We recruited 111 patients with T2D for this cross-sectional study. Each patient underwent flash glucose monitoring for 14 days to obtain glycemic variability parameters, such as standard deviation of glucose (SD), coefficient of variation of glucose (CV), mean amplitude of glycemic excursions (MAGE), mean of daily differences (MODD), and time in glucose range of 3.9–10 mmol/L (TIR3.9–10). After 14 days of flash glucose monitoring, each patient received a questionnaire on the Pittsburgh Sleep Quality Index (PSQI) to evaluate subjective sleep quality. HbA1c was also collected to assess average glucose. Results HbA1c was comparable among the subgroups of PSQI score tertiles. Across ascending tertiles of PSQI scores, SD, CV and MAGE were increased, while TIR3.9–10 was decreased (p for trend p for trend = 0.090). Moreover, PSQI scores were positively correlated with SD, CV, MODD and MAGE (r = 0.322, 0.361, 0.308 and 0.354, respectively, p < 0.001) and were inversely correlated with TIR3.9–10 (r = − 0.386, p < 0.001). After adjusting for other relevant data by multivariate linear regression analyses, PSQI scores were independently responsible for SD (β = 0.251, t = 2.112, p = 0.041), CV (β = 0.286, t = 2.207, p = 0.033), MAGE (β = 0.323, t = 2.489, p = 0.018), and TIR3.9–10 (β = − 0.401, t = − 3.930, p < 0.001) but not for MODD (β = 0.188, t = 1.374, p = 0.177). Conclusions Increased glycemic variability assessed by flash glucose monitoring was closely associated with poor subjective sleep quality evaluated by the PSQI in patients with T2D.

Background The high mortality rate of comatose patients with traumatic brain injury is a prominent public health issue that negatively impacts patients and their families. Objective, reliable tools are needed to guide treatment decisions and prioritize resources. Objective This study aimed to evaluate the prognostic value of the bispectral index (BIS) in comatose patients with severe brain injury. Methods This was a retrospective cohort study of 84 patients with severe brain injury and Glasgow Coma Scale (GCS) scores of 8 and less treated from January 2015 to June 2017. Sedatives were withheld at least 24 hr before BIS scoring. The BIS value, GCS scores, and Full Outline of UnResponsiveness (FOUR) were monitored hourly for 48 hr. Based on the Glasgow Outcome Scale (GOS) score, the patients were divided into poor (GOS score: 1-2) and good prognosis groups (GOS score: 3-5). The correlation between BIS and prognosis was analyzed by logistic regression, and the receiver operating characteristic curves were plotted. Results The mean (SD) of the BIS value: 54.63 (11.76), p = .000; and GCS score: 5.76 (1.87), p = .000, were higher in the good prognosis group than in the poor prognosis group. Lower BIS values and GCS scores were correlated with poorer prognosis. Based on the area under the curve of receiver operating characteristic curves, the optimal diagnostic cutoff value of the BIS was 43.6, and the associated sensitivity and specificity were 85.4% and 74.4%, respectively. Conclusion Taken together, our study indicates that BIS had good predictive value on prognosis. These findings suggested that BIS could be used to evaluate the severity and prognosis of severe brain injury.

Aircraft detection in synthetic aperture radar (SAR) images is a challenging task because of the discreteness, variability, and interference of aircraft scattering features. This article proposes a new hybrid approach of scattering information enhancement (SIE) and an attention pyramid network (APN). It first extracts strong scattering points (SSPs) of aircraft via an adapted Harris–Laplace detector. These SSPs are then clustered into candidate scattering regions by density-based spatial clustering of applications with noise (DBSCAN) and are then modeled with a Gaussian mixture model (GMM). Target scattering clusters are discriminated from background clutter by measuring the Kullback–Leibler divergence (KLD) to the known target templates. These target scattering clusters are enhanced in the preprocessing stage. All the SIE-preprocessed images are then fed into the APN for training and testing. It is composed of the multiscale feature pyramid network (FPN) and the modified convolutional block attention module (CBAM) to cope with the discreteness and variability of aircraft. In addition, focal loss (FL) is adopted to deal with the issue of unbalanced sample distribution and the interference from hard samples. Experiments conducted on the Gaofen-3 and TerraSAR-X data sets demonstrate the effectiveness of the proposed method with an average precision (AP) of 83.25%.

The localized surface plasmon resonance (LSPR) absorption peaks of semiconductor nanocrystals are mainly concentrated in the infrared band, and the absorption characteristics can be controlled by the amount of element doping. The coupling of upconversion nanocrystals (UCNPs) and semiconductor nanocrystals can improve the upconversion luminescence (UCL) of rare-earth ions. Here, the LSPR absorption and morphology of the semiconductor nanocrystalline TiO2: W were adjusted by using ammonium fluoride during synthesis. Significant absorption enhancement of TiO2: W in the near-infrared region was obtained to enhance the UCL of NaYF4: Yb3+, Er3+. The Glass/NaYF4: Yb3+, Er3+/TiO2: W@SiO2 layered structure films were fabricated through spin coating. Compared with Glass/NaYF4: Yb3+, Er3+, the green and red lights of the Glass/NaYF4: Yb3+, Er3+/TiO2: W@SiO2 films were enhanced by 15.9 and 17.8 times, respectively. The UCL enhancement of Glass/NaYF4: Yb3+, Er3+/TiO2: W@SiO2 was derived from the LSPR property of TiO2: W through the enhancement of the excitation. The present work is important for possible applications of these layered structures as biomarkers, photocatalysts, flexible materials, and photoluminescence display panels.

Background Hereditary hemorrhagic telangiectasia is an autosomal dominant hereditary hemorrhagic disease. Its main feature is an abnormal structure of the blood vessel wall. Cirrhosis of the liver is a common chronic progressive disease with one or more causes in which diffuse liver damage occurs after long-term or repeated injury. Liver cirrhosis can cause dilation of gastrointestinal capillaries. Many patients with hereditary hemorrhagic telangiectasia accompanied by gastrointestinal vascular malformations and liver cirrhosis may be diagnosed only with liver cirrhosis if the clinician does not pay attention to physical examination findings and family history. Moreover, general treatment measures, such as blood transfusion, iron supplementation, and application of hemostatic drugs, are less effective for bleeding in patients with hereditary hemorrhagic telangiectasia than in those with liver cirrhosis alone. Case presentation Here, we report the rare case of a 75-year-old Chinese man who was admitted to the hospital with repeated melena and epistaxis. He was diagnosed with unexplained liver cirrhosis, which was later confirmed as hereditary hemorrhagic telangiectasia. Subsequently, we implemented the treatment intervention of oral thalidomide combined with gastrointestinal argon plasma coagulation. A follow-up of more than 8 months showed that the treatment effect was excellent. Conclusions If patients with liver cirrhosis and gastrointestinal vascular malformations also have a family history of epistaxis, special attention should be paid to targeted physical examination results, and the possibility of hereditary hemorrhagic telangiectasia should be considered. Moreover, for patients with hereditary hemorrhagic telangiectasia and both gastrointestinal bleeding caused by gastrointestinal capillaries and repeated epistaxis, when other general treatment measures are ineffective, thalidomide combined with gastrointestinal argon plasma coagulation may be an effective intervention.

A compact slow‐wave empty substrate integrated waveguide (SW‐ESIW) structure is proposed in this article. This structure is composed of an empty substrate with a bottom layer including several rows of internal metallised via holes. In order to decrease both the lateral and longitudinal dimensions, the slow wave effect, caused by physical separation of electric and magnetic fields, is introduced while retaining the advantages of ESIW, such as high quality factor, small insertion loss, and the advantages of SIW, such as low cost and easy processing. Compared to the classical ESIW, the proposed SW‐ESIW structure achieves a size reduction of 37% and retains the triple‐layer structure. At the same time, in contrast with general SW‐SIW structure, which needs double layer with dielectric material, the novel waveguide obtains a significant performance improvement in insertion loss and quality factor. In the article, a fourth‐order filter is designed to verify the idea that the SW‐ESIW technology provides the low loss, high quality factor and compact structure. By combining the advantages of SW‐SIW and ESIW technology, the proposed SW‐ESIW structure can be widely applied to the microwave integrated circuits with miniaturisation and high quality factor.

In recent years, point-of-care testing (POCT) has attracted more and more attention because it is cheap, fast and easy-to-operate. However, detection accuracy and reliability remain challenging for POCT. Machine learning methods are powerful for data processing and analysis, which is the potential to improve the accuracy and reliability of POCT greatly. Additionally, it is also possible to bring breakthroughs in remote medicine and data sharing fields for POCT. In this review, we describe the basic principles of machine learning algorithms and explain their advantages in POCT; then introduce the applications of machine learning in POCT, including paper-based assays, microfluidic lab-on-chip technologies, and wearable devices; after that, we also put forward suggestions on the selection of machine learning algorithms based on the data type and detection targets of POCT tasks; finally, we propose several directions of the future development of machine learning algorithms in POCT.

Neurofibromatosis (NF) is a genetic disease in which the lungs are rarely involved. However, in NF cases with lung involvement, chest computed tomography may show bilateral basal reticulations, apical bullae, and cysts without bronchiectasis. Herein, we report a patient diagnosed with NF on the basis of the results of genetic testing who presented with early-onset wet cough and bronchiectasis. Considering the differential diagnosis of bronchiectasis combined with his early-onset wet cough, sinusitis, and sperm quality decline, we considered the possibility of primary ciliary dyskinesia (PCD). Further electron microscopy analysis of cilia and identification of homozygous mutations in the RSPH4A gene confirmed the diagnosis of PCD. Therefore, for patients with NF, when an image change exists in the lungs that does not correspond to NF, the possibility of other diagnoses, including PCD, must be considered.
