Background To determine the feasibility of conducting an RCT on the potential effectiveness of memantine hydrochloride in prolonging safe driving in mild AD. Methods A placebo-controlled, double blind randomized trial was conducted. Forty-three individuals ≥60 with mild AD met screening criteria and were randomized. Driving ability was measured by a standardized on-road driving test. Outcomes were driving capacity at 6 and 12 months and completion of the 12-month intervention. Results Of 43 participants randomized, 59% of the memantine group and 52% of the placebo group completed the on-road test at 12 months (p = .66). All 13 memantine group participants maintained their driving status at 12 months, whereas only 8 of the 11 placebo group participants did (p = .040, OR = 4.45). Conclusions Results provide the framework for designing a rigorous multisite clinical trial of memantine effect on maintaining driving capacity in mild AD.

The paper aims to review the available data about the main mechanisms enabling improvement or accelerating vascular ageing due to food choices, considering recent experimental and clinical data, and emphasising potential implications for clinical practice and therapy. The main food choices which will be discussed are diets rich in fruits and vegetables, the Mediterranean diet, polyunsaturated fatty acids, cocoa, caffeine, tea, meat, dairy products, sodium, and potassium intake.

This material summarizes the results regarding the analysis of the mechanical stress states for a piping system subjected to impeded thermal expansion. It were considered different load cases corresponding to the primary loads (own weight + internal pressure) and the secondary loads (induced by thermal expansion, including the thermal displacements of the equipment connection points). The stresses values determined for each load cases were compared with the allowable limits evaluated according to ASME B31.3 code, resulting that the analyzed piping system is safe.

The objective of the study was to determine if the pathology depicted on a mammogram is either benign or malignant (ductal or non-ductal carcinoma) using deep learning and artificial intelligence techniques. A total of 559 patients underwent breast ultrasound, mammography, and ultrasound-guided breast biopsy. Based on the histopathological results, the patients were divided into three categories: benign, ductal carcinomas, and non-ductal carcinomas. The mammograms in the cranio-caudal view underwent pre-processing and segmentation. Given the large variability of the areola, an algorithm was used to remove it and the adjacent skin. Therefore, patients with breast lesions close to the skin were removed. The remaining breast image was resized on the Y axis to a square image and then resized to 512 × 512 pixels. A variable square of 322,622 pixels was searched inside every image to identify the lesion. Each image was rotated with no information loss. For data augmentation, each image was rotated 360 times and a crop of 227 × 227 pixels was saved, resulting in a total of 201,240 images. The reason why our images were cropped at this size is because the deep learning algorithm transfer learning used from AlexNet network has an input image size of 227 × 227. The mean accuracy was 95.8344% ± 6.3720% and mean AUC 0.9910% ± 0.0366%, computed on 100 runs of the algorithm. Based on the results, the proposed solution can be used as a non-invasive and highly accurate computer-aided system based on deep learning that can classify breast lesions based on changes identified on mammograms in the cranio-caudal view.

Microservices have become an essential architectural enabler in cloud-based software development and delivery.1 Many organizations, from large tech companies (e.g., Google, Amazon, Netflix) to small start-ups, have adopted microservices as a best practice.2 This has served them well in some cases and, as you will see here, not so well in others.

The recent push to adopt building information modeling (BIM) for bridges and structures in the transportation industry has encountered major barriers owing to the lack of standardization. Unlike the building industry that has the National BIM Standard®–United States (NBIMS-US™) as a formal open platform standard and guide for the development of interoperable BIM software, the transportation industry does not currently have a similar open platform standard to enable the creation of interoperable BIM software to serve the needs of transportation stakeholders. The purpose of this paper is to present the findings of the research for one of the first use cases and development of data exchange requirements and model view definitions in adopting the open platform NBIMS-US applied to bridges and structures for the U.S. transportation industry. A subcommittee of the American Association of State Highway and Transportation Officials and the National Steel Bridge Alliance Steel Bridge Collaboration was formed to conduct a pilot study into the creation of information delivery manuals (IDMs) for steel bridges. This study served as pilot for the development of future IDMs in the transportation industry. As a result, the current IDM for Steel Bridge Detailing and Fabrication serves as the starting point of TPF-5(372) BIM for Bridges and Structures development of the Design to Fabrication model view definition. Finally, this study provided the outcomes and recommendations needed to expedite the development of IDMs for other use cases in the bridge and transportation industry.

Objectives. The study aimed to establish correlations between implant geometry, bone density and survival rate of implant therapy. Material and method. The study material consisted of medical documents (clinical observation and treatment sheets, which included both the results of recommended laboratory tests, photographs and imaging investigations performed pre / intra / postoperatively) of patients who were addressed between October 1st, 2014 and October 1st, 2018 to the Oral Rehabilitation Clinic „Prof. Dr. Barbu“. The study included patients wearing mobilizable prostheses, with advanced periodontal diseases that required tooth extraction, patients who accepted dental implants as a treatment solution and presented at all scheduled sessions, including long-term post-therapeutic dispensary of at least 1 year. Statistical processing was performed with Microsoft Excel 2016. Results. The study group included 73 patients of both sexes (28 men and 45 women), old carriers of mobilizable prostheses, with severe periodontal diseases, which required dental extractions. 618 implants were inserted in the study group in order to prosthetic the edentulous gaps. Discussions. The study highlighted the high interest of females in the application of modern dental therapies. At the age group, the 50-59 age group was best represented, closely followed by the 40-49 age group. Most implants were inserted at the upper arch, at a right angle and were conical. Conclusions. Due to the lower bone density in the upper arch, 81.4% of failures occurred in the group studied in the jaw. Implants with small steps between turns have a better prognosis compared to implants with large steps between turns. From the point of view of the type of implant, the cylindrical-conical ones had the best prognosis, and the zygomatic ones registered the weakest prognosis. Bone additions were correlated with a higher failure rate (55.81% of cases). Implants inserted at 10˚ had the best survival rate, and those inserted at 35˚ and 55˚ had the lowest survival rate.

Introduction Performing a colonoscopy allows the examination of the entire colon and the assessment of polyps. Patients, materials and methods We performed a retrospective analysis of prospectively collected data from January 2018 until February 2020 (two years), in which we enrolled a number of 210 patients performing colonoscopy in the Research Center of Gastroenterology and Hepatology, University of Medicine and Pharmacy of Craiova, Romania. We performed 326 polypectomies. Results We classified the polyps into diminutive (n=169), small (n=103) and large polyps (n=54). Regarding the polypectomy technique, our results indicated that 40 out of 48 (83.3%) polypectomies with the biopsy forceps were complete, as well as 27 out of 31 (87.1%) cold snare polypectomies and 12 out of 14 (85.7%) hot snare polypectomies. The differences were not statistically significant (p=0.116). Regarding the number of incomplete polypectomies, our data suggests that the high expertise endoscopist had two incomplete resections (5.1% of total), the medium expertise endoscopist 1 had also two incomplete resections (11.1% of total), the medium expertise endoscopist 2 had three incomplete resections (15% of total), the limited expertise endoscopist 1 had three incomplete resections (27.27% of total) and the limited expertise endoscopist 2 had four incomplete resections (30.76% of total). Analyzing the data, the differences were statistically significant (p=0.006). Conclusions Our study is able to suggest that high-definition white-light endoscopy (HD-WLE) macroscopic visualization of the polyp resection site is not enough to assess complete polyp resection and follow-up colonoscopy should be performed for cases with incomplete margins of resection.

Objectives. The study aimed to assess the periodontal status of a group of patients and to highlight the correlations with factors favoring the occurrence and worsening of periodontal disease. Material and method. The study was carried out within the „Dr. Vodiță“ Dental Clinic for a period of 2 years. The study group initially consisted of 266 patients who approached the Clinic for various dental reasons. We applied inclusion and exclusion criteria from the study to the initial group of patients. After applying the inclusion and exclusion criteria from the study, the final group of patients consisted of 205 patients of both sexes, aged between 18 and 76 years. The data were analyzed and statistically processed with the Microsoft Excel 2016 program. Results. The study group consisted of 138 male patients and 67 female patients, including 5 bruxomaniacs, 12 oral respirators, 36 patients with malocclusions and 54 patients with reduced or extended edentulous partially prosthetic. The Loe and Silness gingival index and the papillary bleeding index had the majority value 1. Discussions. In the present study, young patients were best represented. Cardiovascular diseases have an increased incidence among the group analyzed, 17.07% of patients said they live in stressful conditions and 47.32% of patients said they smoke. Conclusions. Most patients in the present study were young adults with generalized forms of periodontal disease. The main reasons for presenting to the doctor were gingival bleeding and halitosis, which are signs of periodontal disease. Smoking, daily stress and cardiovascular diseases are factors involved in the appearance and evolution of periodontal disease.

Pleuroparenchymal fibroelastosis (PPFE) is a progressive and frequently fatal interstitial lung disease that involves the upper lobes. Although its cause remains unknown, the histopathologic evidence underlying PPFE bears striking resemblance to that of the pulmonary apical cap (PAC), a relatively common and benign entity. We describe the case of a patient with PAC that evolved into distinctly asymmetric PPFE over 6 years after unilateral surgical lung injury. Given the histologic similarity between these two conditions, we propose that these two entities underlie common biologic pathways of abnormal response to lung injury, with the presence of a PAC increasing susceptibility to the development of PPFE in the face of ongoing inflammatory insults. This case describes the histopathologic evolution of PAC to PPFE before and after an inciting injury.

The latest decades are characterized by an enormous progression in the field of human genetics. In consequences, for various phenotypic manifestations, genetic testing could identify a specific underlying cause. An estimated incidence for all types of 18q deletions is one in 55 000 births predominant on females. About 94% of cases with 18q deletion syndrome appearance are de novo, and the remaining 6% are the inherited from a parent carrying a balanced chromosomal translocation. We present the case of a 35-year-old female who was admitted in our Unit for a second ultrasound opinion after being diagnosed at the second trimester scan at gestational age of 21 weeks of pregnancy with multiple brain and heart malformations, having the recommendation for fetal magnetic resonance imaging (MRI). Further investigations included genetic analysis and pathological examination. Major malformations diagnosed and confirmed were agenesis of the corpus callosum, ventriculomegaly with dilated fourth ventricle, partial agenesis of vermis, bilateral anophthalmia with wide nasal base and left cleft lip. Additional, cardiac malformation, with an important ventricular septal defect and overriding aorta were noted. The results of the microarray analysis showed an abnormal fetal karyotype with a loss of 30.5 basis identified in the long arm of chromosome 18. Although most of the cases of 18q deletion are sporadically or de novo, could be cases where the possible existing syndromes can be inherited from a healthy or mild affected parent. Therefore, in order to establish the recurrence risk, parental karyotypes are recommended.

Systemic candidiasis is a frequent complication in neonatal units, but congenital systemic candidiasis is an unusual diagnosis, observed in both full-term and preterm infants, with less than 50 cases reported to date. Congenital candidiasis presents with a wide spectrum of symptoms, ranging from diffuse skin eruptions to severe systemic disease, resulting in fetal demise or early neonatal death. Although management guidelines have been published almost two decades ago, due to the rarity of this type of infection, conclusive recommendations are difficult to establish, since they are based on anecdotal experience. In this paper, we present a comprehensive meta-analysis of the current scientific knowledge regarding congenital candidiasis, which spans 54 years and includes a total of 44 cases.

Introduction Amongst all malignant tumors, cancers of the digestive tract rank first in terms of yearly deaths. Patients above 60 years of age are the most affected, as the diagnosis is frequently made in advanced stages of the disease when therapy is less effective. Our study aimed to evaluate the efficiency of narrow-band imaging (NBI) endoscopy and probe-based confocal laser endomicroscopy (pCLE) in the correct diagnosis of preneoplastic lesions in the upper and lower digestive tract. Patients, materials and methods We included 46 patients with digestive preneoplastic lesions, who underwent either upper or lower digestive endoscopy, followed by NBI and pCLE. We recorded 5-10 frames per each lesion, from different angles and distances during white-light endoscopy and selected frames from full recordings of NBI and pCLE. Usual preparation was used for the endoscopic procedures; pCLE required in vivo administration of 10% Sodium Fluorescein as a contrast agent. Pathology was performed in case of solid tumors. Three medical professionals with different levels of training, blinded to the results, interpreted the data. Results The experienced physician correlated very well the NBI findings with pathology (0.93, p=0.05), while the resident physician and the experienced nurse obtain lower, albeit still statistically significant, values (0.73 and 0.62, respectively). For pCLE, the experienced physician obtained near-perfect correlation with pathology (0.96), followed closely by the resident physician (0.93). The nurse obtained a modest correlation (0.42). All examiners obtained approximately equal performances in discerning between malignant and benign lesions. Conclusions Digestive endoscopy in NBI mode proved its effectiveness. Even less experienced endoscopists can achieve good results, while an experienced nurse can positively influence the diagnosis. In the case of pCLE, when available, it can greatly reduce diagnostic times, while requiring higher expertise and specialty training.

Acrylic acid is a high-value monomer produced today only from petrochemical resources. This paper investigates the conceptual design of an eco-efficient process using renewable low-cost glycerol. The key step is glycerol dehydration to acrolein, performed in a fluidized bed reactor employing fast nanosheet zeolite catalyst. The investigation makes use of hydrodynamic modelling and computer simulation in Aspen Plus. The feasibility of four alternatives is studied: conventional bubble (BFB) and turbulent (TFB) fluidized bed, as well as two types of circulating fluidized bed (CFB) reactors. Working with smaller bubble size, in the range 2.5–10 cm, a BFB reactor may achieve a productivity of 0.45 kg/kg-cat h for 99.7% conversion, which is only 20% of the performance of an ideal plug flow reactor. Changing to TFB regime raises the productivity to 0.9 kg/kg-cat h. Implementing CFB reactors leads to a spectacular improvement. High productivity of 3 and 4 kg/kg-cat h is achieved with turbulent (CTFB) and high-density (HD-CFB) reactors, respectively. The TFB reactor is preferred when catalyst coking is low or moderate, while CTFB is favoured when continuous regeneration is needed. Employing m-xylene driven azeotropic distillation leads to the lowest energy consumption reported so far by acrylic acid purification. The integration of reaction and separation sections by combined heat and power system leads to a positive energy process. Dynamic simulation demonstrates flexibility and robustness of the separation section faced with disturbances occurring in the reaction system. The sustainability indices show substantial improvement in eco-performance with respect to the propylene-based process.

MicroRNAs (miRNAs) are a group of non-coding RNA molecules that have an important role in modulating the expression of genes involved in regulating cellular functions. A growing number of studies suggest the abnormal expression of microRNAs in different types of cancer cells. MiRNA-124 is a microRNA that is down-regulated in many types of cancer cells, including bladder cancer. Our objective is to provide a review of the key publications that studied the effect of miR-124 on bladder cancer. This review focus on the targets and different pathways of miR-124 that were identified in various studies and differences between their expressions in normal urothelium and tumor tissues. We also include data regarding urinary methylations levels of miR-124 and their role in bladder cancer diagnosis and prognosis. Subsequently, we establish future perspectives of miR-124 research and its promising role in bladder cancer.

Introduction While the visual inspection of histopathology images by expert pathologists remains the golden standard method for grading of prostate cancer the quest for developing automated algorithms for the job is set and deep-learning techniques have emerged on top of other approaches. Methods Two pre-trained deep-learning networks, obtained with transfer learning from two general purpose classification networks - AlexNet and GoogleNet, originally trained on a proprietary dataset of prostate cancer were used to classify 6000 cropped images from Gleason2019 Challenge. Results The average agreement between the two networks and the six pathologists was found to be substantial for AlexNet and moderate for GoogleNet. When tested against the majority vote of the six pathologists the agreement was perfect and moderate for AlexNet, and GoogleNet, respectively. Despite our expectations, the average inter-pathologist agreement was moderate, while between the two networks it was substantial. Resulted accuracy for AlexNet and GoogleNet when tested against the majority vote as ground truth was of 85.51% and 74.75%, respectively. This result was higher than the score obtained on the dataset that they were trained on, showing their generalization capabilities. Conclusions Both the agreement and the accuracy indicate a better performance of AlexNet over GoogleNet, making it suitable for clinical deployment thus could potentially contribute to faster, more accurate and with higher reproducibility prostate cancer diagnosis.

In this paper, we propose a method of learning representation layers with squashing activation functions within a deep artificial neural network which directly addresses the vanishing gradients problem. The proposed solution is derived from solving the maximum likelihood estimator for components of the posterior representation, which are approximately Beta-distributed, formulated in the context of variational inference. This approach not only improves the performance of deep neural networks with squashing activation functions on some of the hidden layers - including in discriminative learning - but can be employed towards producing sparse codes.

Left ventricular assist device (LVAD) implantation may be an attractive alternative therapeutic option for elderly patients with heart failure who are ineligible for heart transplantation.We aimed to describe the characteristics and outcomes of elderly patients (i.e. aged≥70 years) receiving an LVAD.This observational study was conducted in 19 centres between 2006 and 2016. Patients were divided into two groups-younger (aged70 years) and elderly (aged≥70 years), based on age at time of LVAD implantation.A total of 652 patients were included in the final analysis, and 74 patients (11.3%) were aged≥70 years at the time of LVAD implantation (maximal age 77.6 years). The proportion of elderly patients receiving an LVAD each year was constant, with a median of 10.6% (interquartile range 8.0-15.4%) per year, and all were implanted as destination therapy. Elderly and younger patients had similar durations of hospitalization in intensive care units and total lengths of hospital stays. Both age groups experienced similar rates of LVAD-related complications (i.e. stroke, bleeding, driveline infection and LVAD exchange), and the occurrence of LVAD complications did not impact survival in the elderly group compared with the younger group. Lastly, when compared with younger patients implanted as destination therapy, the elderly group also exhibited similar mid-term survival.This work strongly suggests that selected elderly adults can be scheduled for LVAD implantation.

Fighting fake news is a difficult and challenging task. With an increasing impact on the social and political environment, fake news exert an unprecedently dramatic influence on people’s lives. In response to this phenomenon, initiatives addressing automated fake news detection have gained popularity, generating widespread research interest. However, most approaches targeting English and low-resource languages experience problems when devising such solutions. This study focuses on the progress of such investigations, while highlighting existing solutions, challenges, and observations shared by various research groups. In addition, given the limited amount of automated analyses performed on Romanian fake news, we inspect the applicability of the available approaches in the Romanian context, while identifying future research paths.

Public opinion is a valuable source for evaluating the performance of transportation services, which can guide management and policy adjustment. In contrast to traditional surveying methods, which are often inefficient and high-cost, social media has become a popular channel to collect public opinion due to its large data volume and accessibility characteristics. However, existing research lack efficient methods to extract and interpret Twitter data for transportation services evaluation. Therefore, this paper presents a comprehensive framework that enables high-efficient extraction and analysis of public opinions on transportation services from Twitter. The transportation system of Miami-Dade County is chosen as the case study to describe the framework development and validation process. First, Twitter data in a defined area over a certain period are collected and preprocessed to clean erroneous and redundant information. Then, the Twitter data that relate to personal opinions on transportation services (POTS) are filtered hieratically using text classification models trained by manually labeled dataset. Next, timeline analysis using Post Intensity (PI) and Average Sentiment Value (ASV) is implemented on the selected Twitter data to explore the public perceptions towards the transportation-related events. Topic modeling and tokenization techniques are also adopted to extract relevant semantic content needed for data analysis. Significantly, the developed framework can improve the efficiency of Twitter data extraction and analysis. Many manual steps are involved in the development process of the framework while can be avoided when generalized to other applications. The framework can be used as a tool for stakeholders to enable a holistic understanding of public opinions on transportation services and increase the degree of public participation in transportation management.

Gestational diabetes mellitus (GDM) and gestational hypertension (GH) are some of the most common medical conditions associated with pregnancy. These can be correlated with placental morphopathological changes and implicitly can influence good fetal development. The age and weight of the mother can be correlated directly proportionally with those of the fetus but also with histoarchitecture and placental vascularization. The placental appearance associated with GDM and GH reveals macroscopic features, such as calcifications, fibrin deposits and placental infarcts, but the most relevant pathological features are the microscopic ones, highlighted by the classical staining techniques: Hematoxylin-Eosin (HE), Periodic Acid-Schiff (PAS)-Hematoxylin and Masson's trichrome (MT), but also by immunohistochemical technique with the help of the anti-cluster of differentiation 34 (CD34) antibody that labeled the capital endothelium in the structure of the placental terminal villi and thus we were able to quantify the vascular density according to the associated medical pathology. The microscopic changes identified were represented by intravillous and extravillous fibrin depositions, massive placental infarctions caused by vascular suppression due to various causes, such as thrombosis, but also placental calcifications. All these macroscopic and microscopic morphopathological changes, together with the clinical data of the mother and the newborn, we have demonstrated that they are interconnected and that they can vary depending on the pathology, GH or GDM.

Background Antifungals act on fungal sterols structurally similar to human cholesterol. Ketoconazole reversibly suppresses steroidogenesis by inhibiting cytochrome P450 enzymes and interferes with dihydrotestosterone (DHT) activity by binding to the androgen receptor. Hypospadias was reported in infants exposed to nystatin in utero. Case presentation A male infant exposed to antepartum nystatin presented with severe under-undervirilization and transient adrenal corticosteroid abnormalities. He was born in USA at 31 weeks gestation to a mother treated with vaginal Polygynax capsules (nystatin-100,000 international units, neomycin sulphate-35,000 international units and polymyxin B-35,000 international units) for vaginal discharge in the Ivory Coast. She used approximately 60 capsules between the first trimester until delivery. The infant was born with micropenis, chordee, perineo-scrotal hypospadias and bifid scrotum with bilaterally palpable gonads. The karyotype was 46,XY. No Mullerian structures were seen on ultrasound. Serum 17-hydroxyprogesterone (17 OHP) on newborn screening was high (304 ng/ml, normal Conclusions We report severe undervirilization in a 46,XY infant born to a mother treated with prolonged and high dose nystatin during pregnancy. This presentation suggests that prolonged antepartum use of high dose nystatin could lead to severe but transient defects in androgen synthesis and/or action possibly by acting as an endocrine disruptor. Further studies are warranted to confirm this finding. Thus, endocrine disruptors should be considered in male newborns with atypical genitalia not explained by common pathologies.

Techniques of balneo-physio-kinetotherapy and complex oral rehabilitation are general measures approach, which are addressed to all patients for maintaining the homeostasis of the whole body and also of the cephalic extremity, as well as specific measures, targeted in our case at the level of the stomatognathic system. These kind of appoach treatments are in accordance with an individualized therapeutical scheme and gradually complying with the principles and concepts of complex oral rehabilitation. Infrared radiation and masotheraphy is a form of balneo-physiotheraphy medicine in adjuvant and incresed results of benefits of clasic treatment in temporo-mandibular disorders. The association of these treatments, is favorited above monotherapy.

BACKGROUND: Multiple sulfatase deficiency (MSD) is an ultra-rare neurodegenerative disorder caused by pathogenic variants in SUMF1. This gene encodes formylglycine-generating enzyme (FGE), a protein required for sulfatase activation. The clinical course of MSD results from additive effect of each sulfatase deficiency, including metachromatic leukodystrophy (MLD), several mucopolysaccharidoses (MPS II, IIIA, IIID, IIIE, IVA, VI), chondrodysplasia punctata, and X-linked ichthyosis. While it is known that affected individuals demonstrate a complex and severe phenotype, the genotype-phenotype relationship and detailed clinical course is unknown.; METHODS: We report on 35 cases enrolled in our retrospective natural history study, n=32 with detailed histories. Neurologic function was longitudinally assessed with retrospective scales. Biochemical and computational modeling of novel SUMF1 variants was performed. Genotypes were classified based on predicted functional change, and each individual was assigned a genotype severity score.; RESULTS: The median age at symptom onset was 0.25years; median age at diagnosis was 2.7years; and median age at death was 13years. All individuals demonstrated developmental delay, and only a subset of individuals attained ambulation and verbal communication. All subjects experienced an accumulating systemic symptom burden. Earlier age at symptom onset and severe variant pathogenicity correlated with poor neurologic outcomes.; CONCLUSIONS: Using retrospective deep phenotyping and detailed variant analysis, we defined the natural history of MSD. We found that attenuated cases can be distinguished from severe cases by age of onset, attainment of ambulation, and genotype. Pedestrians are the most vulnerable road user and represent about 23% of the road traffic deaths in the world. A finite element (FE) model corresponding to a 5th percentile female pedestrian (F05-PS) was developed by morphing the Global Human Body Models Consortium (GHBMC) 50th percentile male pedestrian (M50-PS) model to the reconstructed geometry of a recruited small female subject. The material properties of the pedestrian model were assigned based on GHBMC M50-PS model. In model validation, the knee lateral stiffness and force time histories of F05-PS upper body showed similar trends, but softer responses than the corresponding data recorded in post mortem human surrogate (PMHS) tests and linearly scaled to average male anthropometry. Finally, the pedestrian model was verified in a Car-to-Pedestrian Collison (CPC) simulation. The marker trajectories recorded in simulation were close to the data recorded on small PMHS in testing and the model predicted typical knee ligament ruptures. Therefore, we believe the F05-PS model, the first FE model developed based on a female reconstructed geometry, could be used to improve vehicle front-end design for pedestrian protection and/or to investigate various pedestrian accidents.

Real-time fighting games are challenging for computer agents in that actions must be decided within a relatively short cycle of time, usually in milliseconds or less. That is only achievable by either very powerful machines or state-of-the-art algorithms. The former is usually a costly option while the latter remains an ongoing research topic despite countless research. This paper describes our algorithmic approach towards real-time fighting games via the fighting game AI challenge. The focus of our research is the LUD division, the most challenging category of the competition where action data is hidden to prevent methods that are dependent on prior training. In this paper, we propose several generic heuristics that can be used in combination with Monte-Carlo tree search. Our experimental results show that such an approach would provide an excellent AI outperforming pure Monte-Carlo tree search and classic algorithms such as evolutionary algorithms or deep reinforcement learning. Nonetheless, we believe that our proposed heuristics should be able to generalize to other domains beyond the scope of the fighting game AI challenge.

The Exchange of knowledge between universities and SMEs became a source of sustainable competitive advantages. The innovation capacity of SMEs is under pressure for various reasons since knowledge management is more costly and innovation readiness more difficult to be achieved. Higher Education Institutions (HEIs) became in this context critical players in the knowledge-based economy to interact with small companies. The traditional functions of universities are the generation of knowledge (research) and its transmission (teaching) are reconsidered in order to better serve the managers. This paper investigate conditions SMEs and universities may better work together, assuming that a pro university-SMEs cooperation behaviour at the level of the SMEs managers and universities is an essential factor of an efficient transfer of knowledge. Evidence of this interaction from development regions of Romania are shown in order to identify emerging cooperation patterns. Another area this paper investigates is the interaction of universities and SMEs when they are bound together in a longer cooperation alliances, in clusters. Actions to develop clusters include universities as central resource. The research intends to review the role of the university in cluster development for Romanian development regions on their struggle to occupy a role in European smart specialization processes. As a main conclusion, the article argues that smart specialization and communities of practice are interconnected concepts enabling innovation processes in university-industry interaction. Knowledge transfer may be more efficient when clusters and universities are interacting on a long-term basis.

Objectives. To assess the level of adherence to combined antiretroviral therapy (cART) among pregnant women infected with HIV-1 in early childhood, registered in Craiova Regional Center for Monitoring and Evaluation of HIV / AIDS within „Victor Babes” Clinical Hospital of Infectious Diseases and Pneumoftiziology of Craiova (CRC) and to identify the main factors associated with non-adherence. Material and method. Retrospective study, during January 1st 2014 and December 31st 2019, with the analysis of the data mentioned in the medical records for the HIV-positive pregnant women registered within CRC. The level of adherence was assessed after applying an adherence questionnaire: 18-22 points (adherence ≥ 80%) and under 18 points (non-adherence < 80%). Results. The studied group included 71 patients and it was divided into two batches, depending on the route of HIV transmission: group A which included 48 pregnant women parenterally infected with HIV in early childhood and 23 pregnant women sexually infected with HIV. We found that the proportion of non-adherent patients was higher in sexually infected mothers (77.7%) compared to those parenterally infected, in early childhood (14.8%), with statistically significant differences (p = 0.001). The HIV viral load level was correlated with the level of adherence to cART (p = 0.005). Conclusions. Pregnant women infected with HIV-1 in early childhood had a higher level of adherence to cART compared to those sexually infected, directly correlated with HIV viremia.

The usage of Facebook among university students is more and more widespread. Students are using Facebook anywhere and anytime, sometimes too much. Several researchers questioned the usefulness and the motives for its use. This paper aims to analyze the relationship between the motives for Facebook use and its perceived usefulness from the perspective of consumption values theory. The motives for Facebook use have been conceptualized as a formatively meas-ured construct that impacts two drivers of the intention to use: the perceived usefulness and the perceived ease of use. This approach has the advantage of using a small set of indicators corre-sponding to the main categories of motives. The model has been tested on a sample of 182 uni-versity students from a technical faculty. The results show that the perceived usefulness is rela-tively low, a little bit over the neutral value. As regards the motives for using Facebook, the es-timation results revealed four motives: keeping in touch with known people, finding information and resources, socialization, and entertainment.

HIV infection is still associated with high maternal, feto-neonatal and infant morbidity. Early detection of HIV infection during pregnancy and early initiation of antiretroviral therapy are correlated with obtaining an HIV viral load of less than 50 copies/ml, in which case the risk of maternal-fetal transmission of the infection is less than 0.5%. Objectives. To establish the maternal-fetal transmission rate of HIV infection among HIV-positive pregnant women registered in Craiova Regional Center for Monitoring and Evaluation of HIV/AIDS within „Victor Babeş” Clinical Hospital of Infectious Diseases and Pneumoftiziology of Craiova (CRC) and to identify some correlations between it and certain risk factors. Material and method. Retrospective study, during January 1st, 2014-December 31st, 2019, on 73 HIV-positive pregnant women, registered by Craiova Regional Center (CRC). For the patients included in the study group we analysed: demographic data, epidemiological data, clinical data, biological data, data provided by gynaecological and obstetrical examination, data on antiretroviral treatment (ART). The children were biologically evaluated until the age of 18 months. Results. We found a maternal-fetal transmission rate of 6.3%. For patients with complete prophylaxis measures, the maternal-fetal transmission rate was 3%. The application of complete prophylaxis measures was correlated with a low risk of vertical transmission of HIV infection (p = 0.006). The low adherence to ART was correlated with a high risk of maternal-fetal transmission of HIV infection (p = 0.02), detectable viral-HIV load (VL-HIV) (p = 0.0004) and advanced immunosuppression (p = 0.02). The disease stage of HIV-positive pregnant women was not correlated with the risk of vertical transmission of the infection (p = 1). The level of HIV viral load of mothers who gave birth to HIV-positive children was higher than in the general group and correlated with the maternal-fetal transmission (p = 0.04). Conclusions. The maternal-fetal transmission rate in the studied period was relatively high (6.3%), due to the incomplete application of prevention measures, 12 of the pregnant women being diagnosed during pregnancy or childbirth. The vertical transmission of HIV infection was correlated with detectable HIV viremia and low ART adherence in pregnant women.

A learning health system (LHS) seeks to establish a closer connection between clinical care and research and establishes new responsibilities for healthcare providers as well as patients. A new set of technological approaches in medication adherence monitoring can potentially yield valuable data within an LHS, and raises the question of the scope and limitations of patients’ responsibilities to use them. We argue here that, in principle, it is plausible to suggest that patients have a prima facie obligation to use novel adherence monitors. However, the strength of the obligations depends considerably on the extent to which data that adherence monitors generate are, in fact, used to further the goals of LHSs. The way in which data ownership is structured in the USA poses a considerable challenge here, while the European Union framework offers a more promising alternative.

Vascular stress at the level of the uterus-placental unit, with chronic placental ischemia, results in intrauterine growth restriction. Expectation management can be used, when the situation allows, in cases of compensated intrauterine growth restriction. The aim of the present study was to evaluate the neonatal prognosis of preterm births with and without growth restriction and term births with growth restriction in order to improve decisional accuracy regarding the termination of pregnancy. The frequency of term birth infants with low birth weight for gestational age was ~2%. The male sex, predominated only in the group of premature infants with normal weight for the gestational age. The highest frequency of neonatal complications studied occurred in the group of preterm neonates small for gestational age (SGA) with statistical significance obtained for cardiovascular arrest acute respiratory failure, ulcer-necrotic enterocolitis, respiratory distress, cerebral edema, intraventricular hemorrhage, cerebral hemorrhage, pulmonary hemorrhage, neonatal infection, hypoglycemia, retinopathy, anemia, hemorrhagic disease, disseminated intravascular coagulation, disease of hyaline membranes, neonatal sepsis, need for intensive neonatal therapy and death. In conclusion, immediate neonatal adaptation of SGA preterm neonates is more deficient than for preterm neonates with appropriate weight for gestational age; the adaptation of preterm neonates, in turn, is more deficient than term newborns with intrauterine growth restriction. The term newborns with intrauterine growth restriction have a neonatal adaptation comparable to that of the term newborns with weight corresponding to the gestational age.

Objectives: We sought to compare outcomes between intravascular ultrasound– (IVUS) versus angiography (AO)-guided peripheral vascular interventions (PVIs). Introduction: Intravascular ultrasound facilitates plaque visualization and angioplasty during PVIs for peripheral arterial disease. It is unclear whether IVUS may improve the durability of PVIs and lead to improved clinical outcomes. Methods: This is a study-level meta-analysis of observational studies. The primary end points of this study were rates of primary patency and reintervention. Secondary end points included rates of vascular complications, periprocedural adverse events, amputations, technical success, all-cause mortality, and myocardial infarction. Results: Eight observational studies were included in this analysis with 93 551 patients. Mean follow-up was 24.2 ± 15 months. Intravascular ultrasound–guided PVIs had similar patency rates when compared with AO-guided PVIs (relative risk [RR]: 1.30, 95% confidence interval [CI]: 0.99-1.71, P = .062). There was no difference in rates of reintervention in IVUS-guided PVIs when compared to non-IVUS-guided PVIs (RR: 0.41, 95% CI: 0.15-1.13, P = .085). There is a lower risk of periprocedural adverse events (RR: 0.81, 95% CI: 0.70-0.94, P = .006) and vascular complications (RR: 0.81, 95% CI: 0.68-0.96, P = .013) in the IVUS group. All-cause mortality (RR: 0.76, 95% CI: 0.56-1.04, P = .084), amputation rates (RR 0.83, 95% CI: 0.32-2.15, P = .705), myocardial infarctions (RR: 1.19, 95% CI: 0.58-2.41, P = .637), and technical success (RR: 1.01, 95% CI: 0.86-1.19, P = .886) were similar between the groups. Conclusions: Intravascular ultrasound–guided PVIs had similar primary patency and reintervention when compared with AO-guided PVIs with significantly lower rates of periprocedural adverse events and vascular complications in the IVUS-guided group.

OBJECTIVES Our goal was to provide a picture of left ventricular assist device (LVAD) activity in France between 2007 and 2016 based on the multicentric ASSIST-ICD registry. METHODS We retrospectively collected 136 variables including in-hospital data, follow-up survival rates and adverse events from 671 LVAD recipients at 20 out of 24 LVAD implant centres in France. The average follow-up time was 1.2 years (standard deviation: 1.4); the total follow-up time was 807.5 patient-years. RESULTS The included devices were the HeartMate II®, HeartWare LVAS® or Jarvik 2000®. The overall likelihood of being alive while on LVAD support or having a transplant (primary end point) at 1, 2, 3 and 5 years postimplantation was 65.2%, 59.7%, 55.9% and 47.7%, respectively, given a cumulative incidence of 29.2% of receiving a transplant at year 5. At implantation, 21.5% of patients were on extracorporeal life support. The overall rate of cardiogenic shock at implantation was 53%. The major complications were driveline infection (26.1%), pump pocket or cannula infection (12.6%), LVAD thrombosis (12.2%), ischaemic (12.8%) or haemorrhagic stroke (5.4%; all strokes 18.2%), non-cerebral haemorrhage (9.1%) and LVAD exchange (5.2%). The primary end point (survival) was stratified by age at surgery and by the type of device used, with inference from baseline profiles. The primary end point combined with an absence of complications (secondary end point) was also stratified by device type. CONCLUSIONS The ASSIST-ICD registry provides a real-life picture of LVAD use in 20 of the 24 implant centres in France. Despite older average age and a higher proportion of patients chosen for destination therapy, survival rates improved compared to those in previous national registry results. This LVAD registry contrasts with other international registries because patients with implants have more severe disease, and the national policy for graft attribution is distinct. We recommend referring patients for LVAD earlier and suggest a discussion of the optimal timing of a transplant for bridged patients (more dismal results after the second year of support?).

Introduction. Hemolytic uremic syndrome (HUS) is a hemorrhagic systemic microangiopathy, which primarily affects the kidney and is defined by the triad: acute renal failure, microangiopathic hemolytic anemia, and thrombocytopenia. We report two cases of HUS with neurological manifestations and different evolution. Cases presentation. We report the cases of two young children, without significant medical history, who were diagnosed with acute renal failure, hemolytic anemia and thrombocytopenia. The onset of the disease was a few days prior to admission, with fever and diarrhea, for which they received symptomatic treatment at home, with worsening of the general condition. The particularity of these cases is that even if the patients had the featured triad of HUS and they were both diagnosed with this disease, the neurological complications evolved differently, with a finally favorable outcome. Conclusions. Hemolytic uremic syndrome (HUS) is described as a multisystemic disease that affects mainly the kidney and is a major cause of morbidity and mortality in children less than 5 years old. The kidney and the brain are the main target organs in patients with HUS. In these two cases, the patients diagnosed with HUS and neurological complications, presented a favorable outcome with specific treatment.

The postoperative digestive fistula is the most feared complication of gastrointestinal surgeries. We performed a retrospective study over a period of 6 years, in which we included 28 patients who developed postoperative esophageal, gastric or duodenal fistulae in the General Surgery Department of “Bagdasar–Arseni” Emergency Hospital. We assessed the risk factors for this complication, its management and its results. Most patients were males and the mean age was 61.1 years. For 15 patients, the surgeries were required for benign lesions while the rest of 13 patients had malignant disease. Regarding the type of the fistula, 14 were duodenal stump fistulas, 13 were anastomotic leakages and one was a dehiscence of a sutured duodenal ulcer. Six cases required emergency relaparotomy for sepsis and peritonitis. Sixteen patients had a favourable outcome while death occurred in 12 cases. The mortality was higher for patients with sepsis and peritonitis as the first clinical manifestations of fistula and with the need for an emergency reoperation.

Publisher's version (útgefin grein), Carriers of large recurrent copy number variants (CNVs) have a higher risk of developing neurodevelopmental disorders. The 16p11.2 distal CNV predisposes carriers to e.g., autism spectrum disorder and schizophrenia. We compared subcortical brain volumes of 12 16p11.2 distal deletion and 12 duplication carriers to 6882 non-carriers from the large-scale brain Magnetic Resonance Imaging collaboration, ENIGMA-CNV. After stringent CNV calling procedures, and standardized FreeSurfer image analysis, we found negative dose-response associations with copy number on intracranial volume and on regional caudate, pallidum and putamen volumes (β = −0.71 to −1.37; P < 0.0005). In an independent sample, consistent results were obtained, with significant effects in the pallidum (β = −0.95, P = 0.0042). The two data sets combined showed significant negative dose-response for the accumbens, caudate, pallidum, putamen and ICV (P = 0.0032, 8.9 × 10−6, 1.7 × 10− 9, 3.5 × 10−12 and 1.0 × 10−4, respectively). Full scale IQ was lower in both deletion and duplication carriers compared to non-carriers. This article describes rehabilitation of one case, complex psychiatric treatment, facial asimetry, with mandibular and maxilla missing teeth and dental disharmony, with a fixed and also removable hybrid prosthesis.Rehabilitation with fixed or removable prosthesis is even more challenging when the edentulous span is long and the ridge is irregular deformities and unfavorable biomechanics encountered at the prosthetic field for complex rehabilitation. In such situation, a fixed-removable prosthesis allows favorable biomechanical stress distribution along with restoration of esthetics, phonetics, comfort, hygiene, and better postoperative care and maintenance.

The clinical picture of the behavioural variant of frontotemporal dementia can be very similar to a manic episode. Paying a careful attention to memory and attention problems and assessing the cognitive status of a patient with mania-like symptoms and no previous psychiatric history should be a standard procedure in order to distinguish mania from behavioural variant of frontotemporal dementia. Sometimes, imaging techniques are revealing important aspects that are essential for a correct diagnosis and treatment plan. Here, we present a case of frontotemporal dementia with early onset that had a manic like clinical presentation. Also, we shortly discuss the available pharmacological approaches and outline the importance of thorough differential diagnosis.

The disruptive potentials of the Internet of Things (IoT) entails multifaceted requirements and development issues (large scale deployments, heterogeneity, cyberphysicality, interoperability, distributed smartness, self-management, etc.). To adequately tackle them and to comprehensively support the development of the IoT ecosystem, the Agent-Based Computing (ABC) represents a proper and solid modeling, programming and simulation paradigm. Indeed, abstractions, design methods, technology and frameworks related to the ABC have been widely exploited, possibly jointly with other wellestablished/emerging computing paradigms, to actually develop advanced IoT ecosystem. This survey, an extension of our previous work, reports most relevant contemporary contributions in the field, aiming at assessing suitability of the ABC paradigm for the (current and future) IoT development. By accumulating ever greater amounts of genomic data, scientists have identified >100 genes associated with Mendelian forms of epilepsy and neurodevelopmental disorders with seizures. For most of the identified genes a wide range of genetic variants have been identified and affected patients are clinically heterogeneous. It is not clear to which degree the clinical heterogeneity can be attributed to the disease causing variant alone. We need to improve our current understanding of biophysical effects of variants on protein function and the role of polygenic background in modifying the clinical representation. In addition, longitudinal clinical data need to be recorded using standardized methods and shared across research centers to build large virtual cohorts for each single gene disorder. Without large, comprehensive, longitudinal datasets, studying the interplay of environmental factors and genetic factors will be challenging. As a community, we must work together to set the foundation for biorepositories and the collection and sharing of ‘big data’ in order to allow genetic-phenotypic characterization of the epilepsies and to fully utilize the potential for drug discovery, and patient-specific tailored management.

Summary Literature exploration in PubMed on a large number of biomedical entities (e.g. genes, diseases or experiments) can be time-consuming and challenging, especially when assessing associations between entities. Here, we describe SimText, a user-friendly toolset that provides customizable and systematic workflows for the analysis of similarities among a set of entities based on text. SimText can be used for (i) text collection from PubMed and extraction of words with different text mining approaches, and (ii) interactive analysis and visualization of data using unsupervised learning techniques in an interactive app. Availability and implementation We developed SimText as an open-source R software and integrated it into Galaxy (https://usegalaxy.eu), an online data analysis platform with supporting self-learning training material available at https://training.galaxyproject.org. A command-line version of the toolset is available for download from GitHub (https://github.com/dlal-group/simtext) or as Docker image (https://hub.docker.com/r/dlalgroup/simtext/tags.).
