This article aims to compare distinct metrics of the value at risk (VaR), differing from prior studies with respect about compare three asset categories belonging to seven countries. Since VaR inception, several approaches were developed to improve the loss estimation accuracy. However, there is hardly a universal consensus on which approach is the most appropriate, since VaR depends on statistical properties of the target asset and the market in which it is traded. It is relevant to compare the results obtained not only among the assets, but also among the markets in which they are traded, considering their specifics properties to verify if there is any pattern of the methods for the data. Considering the three asset categories, the semiparametric and non-parametric models obtained the lowest rejections number. It was also found that the models tested were not effective for the estimation of exchange rate VaR, which may be due to more relevant risks than the market in it asset price formation. Five models belonging to the parametric, semiparametric, and non-parametric approaches were tested. The analyses were divided in two, aiming to test the VaRs performances in distinct economic cycles; the first analyses considered a 1,000 days estimation window, while the second one considered a 252 days estimation window. To validated the results statistically, were applied the Kupiec and the Christoffersen tests. The results show that the conditional VaR and historical simulation have the best performance to estimate VaR. Comparing the markets, Chinese assets were the ones with the highest average number of tests rejections, which can be a consequence of its closed economy. Finally, it was found that shorter estimation window tends to perform better for high volatility assets, while longer window tends for lower volatility assets. RESUMO O objetivo deste artigo é comparar diferentes métricas de valor em risco (VaR), distinguindo-se de estudos anteriores na medida em que compara três categorias de ativos pertencentes a sete países. Desde a concepção do VaR, foram desenvolvidas várias abordagens para melhorar a precisão da estimativa de perdas. Entretanto, praticamente inexiste um consenso universal sobre qual abordagem é a mais apropriada, uma vez que o VaR depende das propriedades estatísticas do ativo alvo e do mercado no qual este é negociado. É importante comparar os resultados obtidos não apenas entre os ativos, mas também entre os mercados em que são negociados, considerando suas propriedades específicas para verificar se existe algum padrão dos métodos para os dados. Considerando as três categorias de ativos, os modelos semiparamétrico e não paramétrico obtiveram o menor número de rejeições. Verificou-se também que os modelos testados não foram eficazes para a estimação do VaR da taxa de câmbio, o que pode ser devido a riscos mais relevantes do que o mercado na formação do preço do ativo. Foram testados cinco modelos pertencentes às abordagens paramétrica, semiparamétrica e não paramétrica. As análises foram divididas em duas, com o intuito de testar os desempenhos dos VaRs em diferentes ciclos econômicos; as primeiras análises consideraram uma janela de estimação de 1.000 dias, enquanto as segundas consideraram uma janela de estimativa de 252 dias. A fim de validar estatisticamente os resultados, foram aplicados os testes de Kupiec e Christoffersen. Os resultados mostram que o VaR condicional e a simulação histórica apresentam o melhor desempenho para estimar o VaR. Comparando-se os mercados, os ativos chineses foram os que apresentaram o maior número médio de rejeições de testes, o que pode ser consequência de sua economia fechada. Por fim, constatou-se que a janela de estimação mais curta tende a apresentar um melhor desempenho para ativos de alta volatilidade, enquanto a janela mais longa tende a ter um melhor desempenho para ativos com menor volatilidade.

Pesquisa sem auxílio de agências de fomento Técnicas de identificação de sistemas são essenciais para o conhecimento de fenômenos naturais e processos de diferentes origens. O principal intuito da presente pesquisa tem sido construir, teoricamente e computacionalmente, uma abordagem para lidar com sistemas de dados com várias entradas e uma saída. Duas etapas fuzzy distintas e bem definidas estão presentes no processo de identificação utilizado neste estudo: clusterização dos dados e uma inferência do tipo Takagi-Sugeno-Kang. A principal contribuição desta pesquisa é a construção dos antecedentes e dos consequentes do sistema de inferência nos quais são utilizados métodos inéditos. Uma forma de validar esta nova metodologia tem sido através de simulações com diferentes bases de dados. Os experimentos envolveram comparações com outras técnicas consolidadas como: Nonlinear Auto-Regressive with eXogenous inputs, Hammerstein-Wiener e Multilayer-Perceptron (redes neurais artificiais). A codificação tem sido feita sem a utilização de toolboxes, exceto para os testes que têm sido feitos com a rotina Compare do software Matlab, a qual utiliza medida de acurácia denominada raiz média quadrática do erro normalizada (Normalized Root Mean Square em inglês). Ao final, e como resultado das comparações, tem havido ganho de precisão na resposta e queda no esforço computacional. Como aplicação da nova técnica, têm sido realizados testes de predição de dados, com resultados promissores, além de estruturar uma nova metodologia para a interpretação de sinais provenientes de exames médicos como, por exemplo, eletroencefalogramas. Futuros trabalhos incluem a introdução da teoria dos conjuntos fuzzy do tipo 2, extensão da teoria dos conjuntos fuzzy, na dinâmica da inferência, com o objetivo de tornar ainda melhor, o desempenho da metodologia proposta. System identification techniques are essential for the knowledge of natural phenomena and processes from different sources. The main purpose of this research has been to build, theoretically and computationally, an approach to deal with data systems with multiple inputs and an output. Two distinct and well-defined steps are present in the identification process used in this study: data clustering and a Takagi-Sugeno-Kang type inference. The main contribution of this research is the construction of the antecedents and the consequences of the inference system in which new methods are used. One way to validate this new methodology has been through simulations with different databases. The experiments involved comparisons with other consolidated techniques such as: Nonlinear Auto-Regressive with eXogenous inputs, Hammerstein-Wiener and Multilayer-Perceptron (artificial neural networks). The coding has been done without the use of toolboxes, except for the tests, it has been done with the Compare routine of the Matlab software, which uses an accuracy measure called Normalized Root Mean Square (English). As a result of the comparisons, there has been a gain in precision in the response and a decrease in the computational effort. As an application of the new technique, data prediction tests have been carried out, with promising results, in addition to structuring a new methodology for the interpretation of signals coming from medical examinations, such as electroencephalograms. Future works include the introduction of the type-2 fuzzy set theory, an extension of the fuzzy set theory, to the inference dynamic with the objective of making the performance of the proposed methodology even better. Multiple scoring systems have been designed to calculate the risk of major adverse cardiovascular events (MACE) in patients with chest pain. There is no data on whether the HEART score outperforms TIMI and GRACE in the prediction of MACE, especially in the era of high-sensitivity troponin assay and in an exclusively Latin-American population.To compare the performance of the HEART, TIMI, and GRACE scores for predicting major cardiovascular events at 30 days of follow-up, in patients who consult for chest pain in the emergency department.HEART, TIMI, and GRACE scores were analyzed in 519 patients with chest pain at the emergency department. The primary endpoint was the occurrence of MACE within 30 days. The performance of the HEART score was compared with the TIMI and GRACE scores using the DeLong test with p values of 0.05 considered statistically significant.A total of 224 patients (43%) had MACE at 30 days. The C statistic for the HEART, TIMI, and GRACE score was 0.937, 0.844, and 0.797 respectively (p0.0001). A HEART score of 3 or less had a sensitivity of 99.5% and a negative predictive value of 99% to classify low risk patients correctly; both values were higher than those obtained by the other scores.The HEART score more effectively predicts cardiovascular events at 30 days of follow-up compared to the other scores. High-sensitivity troponins maintain this score's previously demonstrated superiority. This score offers more precise identification of low-risk patients. Não há dados que avaliem se o escore HEART tem um desempenho superior a TIMI e GRACE para a predição de MACE, especialmente na era de troponina I de alta sensibilidade e em uma população exclusivamente latino-americana.Comparar o desempenho dos escores HEART, TIMI e GRACE para a predição de MACE em 30 dias de acompanhamento, em pacientes atendidos com dor no peito no departamento de emergência.Os escores HEART, TIMI e GRACE foram analisados em 519 pacientes com dor no peito no departamento de emergência. O desfecho primário foi a ocorrência de MACE no período de 30 dias. O desempenho do escore HEART foi comparado com o dos escores TIMI e GRACE utilizando o teste de DeLong, considerando estatisticamente significativos os valores de p de 0,05.Um total de 224 pacientes (43%) apresentaram MACE no período de 30 dias. A estatística C para os escores HEART, TIMI e GRACE foi de 0,937, 0,844 e 0,797 respectivamente (p0,0001). Uma pontuação de 3 ou menos no escore HEART apresentou uma sensibilidade de 99,5% e um valor preditivo negativo de 99% para classificar pacientes de baixo risco de maneira correta; ambos os valores foram mais elevados do que aqueles obtidos pelos outros escores.O escore HEART, em um período de 30 dias, prediz eventos cardiovasculares, mais eficazmente, em comparação com os outros escores. Objectives: To describe fidgety movements (FMs), i.e., the spontaneous movement pattern that typically occurs at 3–5 months after term age, and discuss its clinical relevance. Sources: A comprehensive literature search was performed using the following databases: MEDLINE/PubMed, CINAHL, The Cochrane Library, Science Direct, PsycINFO, and EMBASE. The search strategy included the MeSH terms and search strings (‘fidgety movement*’) OR [(‘general movement*’) AND (‘three month*’) OR (‘3 month*’)], as well as studies published on the General Movements Trust website (www.general-movements-trust.info). Summary of the data: Virtually all infants develop normally if FMs are present and normal, even if their brain ultrasound findings and/or clinical histories indicate a disposition to later neurological deficits. Conversely, almost all infants who never develop FMs have a high risk for neurological deficits such as cerebral palsy, and for genetic disorders with a late onset. If FMs are normal but concurrent postural patterns are not age-adequate or the overall movement character is monotonous, cognitive and/or language skills at school age will be suboptimal. Abnormal FMs are unspecific and have a low predictive power, but occur exceedingly in infants later diagnosed with autism. Conclusions: Abnormal, absent, or sporadic FMs indicate an increased risk for later neurological dysfunction, whereas normal FMs are highly predictive of normal development, especially if they co-occur with other smooth and fluent movements. Early recognition of neurological signs facilitates early intervention. It is important to re-assure parents of infants with clinical risk factors that the neurological outcome will be adequate if FMs develop normally. Resumo: Objetivos: Descrever os movimentos irregulares (FMs), ou seja, o padrão de movimentos espontâneos que normalmente ocorrem entre 3 e 5 meses após o nascimento e discutir sua relevância clínica. Fontes: Uma pesquisa abrangente na literatura foi realizada nas seguintes bases de dados: MEDLINE/PubMed, CINAHL, The Cochrane Library, Science Direct, PsycINFO e EMBASE. Resumo dos dados: Praticamente todos os neonatos se desenvolveram normalmente se os FMs estiveram presentes e foram normais, mesmo se seus resultados do ultrassom do cérebro e/ou históricos clínicos indicassem tendência a déficits neurológicos posteriores. Por outro lado, quase todos os neonatos que nunca desenvolveram FMs apresentaram maior risco de déficits neurológicos, como paralisia cerebral, e doenças genéticas de início tardio. Caso os FMs fossem normais, porém simultâneos a padrões posturais não adequados para a idade, ou o caráter geral dos movimentos fosse monótono, as capacidades cognitivas e/ou de linguagem na idade escolar seriam abaixo do ideal. Os FMs anormais não são específicos e têm baixo poder preditivo, porém ocorrem em grande parte em neonatos posteriormente diagnosticados com autismo. Conclusões: FMs anormais, ausentes ou esporádicos indicam um risco maior de disfunções neurológicas posteriores, ao passo que FMs normais são altamente preditivos de desenvolvimento normal, principalmente se forem simultâneos a outros movimentos suaves e fluentes. O reconhecimento precoce de sinais neurológicos facilita a intervenção antecipada. É importante garantir aos pais de neonatos com fatores de risco clínicos que o resultado neurológico será adequado se os FMs se desenvolverem normalmente. O futsal é uma modalidade intermitente, na qual se exige esforços de alta intensidade de diferentes demandas, como potência aeróbia e potência muscular. Dessa forma, a busca pelo aprimoramento dessas exigências a partir de relações entre si pode ser ferramenta relevante para o esporte. Buscou-se correlacionar e calcular equações preditivas para diferentes variáveis de aptidão física em protocolos de esforços intermitentes. Dezoito jogadores de futsal (15 ± 1,3 anos, 75,4 ± 20,2 kg, 167 ± 0,8 cm) foram avaliados quanto à capacidade de repetir corridas (a partir do Forward-Backward test), potência aeróbia máxima (PAM, com o Yo-Yo Intermitent Recovery Test) e potência de membros inferiores (salto horizontal triplo, SHT). O teste de Pearson foi utilizado para as correlações e o método stepwise para as regressões lineares. Além de diferentes correlações significantes, considerando o desempenho no SHT e de PAM, foram observados valores de determinação de 44% a 64% na capacidade de repetir corridas. The objective of this work was to evaluate the use of artificial neural networks in comparison with Bayesian generalized linear regression to predict leaf rust resistance in Arabica coffee (Coffea arabica). This study used 245 individuals of a F2 population derived from the self-fertilization of the F1 H511-1 hybrid, resulting from a crossing between the susceptible cultivar Catuaí Amarelo IAC 64 (UFV 2148-57) and the resistant parent Híbrido de Timor (UFV 443-03). The 245 individuals were genotyped with 137 markers. Artificial neural networks and Bayesian generalized linear regression analyses were performed. The artificial neural networks were able to identify four important markers belonging to linkage groups that have been recently mapped, while the Bayesian generalized model identified only two markers belonging to these groups. Lower prediction error rates (1.60%) were observed for predicting leaf rust resistance in Arabica coffee when artificial neural networks were used instead of Bayesian generalized linear regression (2.4%). The results showed that artificial neural networks are a promising approach for predicting leaf rust resistance in Arabica coffee. Resumo: O objetivo deste trabalho foi avaliar o uso de redes neurais artificiais em comparação à modelagem por meio de modelos lineares generalizados na predição de resistência à ferrugem em café arábica (Coffea arabica). Foram utilizados 245 indivíduos provenientes de uma população F2, oriundos da autofecundação do híbrido F1 H511-1, resultante do cruzamento da cultivar suscetível Catuaí Amarelo IAC 64 (UFV 2148-57) e do genitor resistente Híbrido de Timor (UFV 443-03). Os 245 indivíduos foram genotipados com 137 marcadores. Realizaram-se análises com redes neurais artificiais e com modelos lineares generalizados sob o enfoque bayesiano. As redes neurais identificaram quatro marcadores importantes pertencentes a grupos de ligação que foram recentemente mapeados, enquanto o modelo generalizado bayesiano identificou somente dois marcadores pertencentes a esses grupos. Foram observadas taxas de erro de predição inferiores (1,60%) para predizer a resistência à ferrugem em café arábica, quando foram utilizadas as redes neurais artificiais em vez de modelos lineares generalizados sob o enfoque bayesiano (2,4%). Os resultados mostraram que as redes neurais artificiais são uma abordagem promissora para predizer a resistência à ferrugem em café arábica.

Objective To evaluate the association of Doppler of uterine artery and flow-mediated dilation of brachial artery (FMD) in the assessment of placental perfusion and endothelial function to predict preeclampsia. Materials and Methods A total of 91 patients considered as at risk for developing preeclampsia were recruited at the prenatal unit of the authors' institution. All the patients underwent FMD and Doppler of uterine arteries between their 24th and 28th gestational weeks. Calculations of sensitivity and specificity for both isolated and associated methods were performed. Results Nineteen out of the 91 patients developed preeclampsia, while the rest remained normotensive. Doppler flowmetry of uterine arteries with presence of bilateral protodiastolic notch had sensitivity of 63.1% and specificity of 87.5% for the prediction of preeclampsia. Considering a cutoff value of 6.5%, FMD showed sensitivity of 84.2% and specificity of 73.6%. In a parallel analysis, as the two methods were associated, sensitivity was 94.2% and specificity, 64.4%. Conclusion The association of Doppler study of uterine arteries and FMD has proved to be an interesting clinical strategy for the prediction of preeclampsia, which may represent a positive impact on prenatal care of patients considered as at high-risk for developing such a condition.

Objetivo:Avaliar a capacidade de predição das formas precoce e tardia da pré-eclâmpsia pela dilatação fluxo-mediada da artéria braquial (FMD), marcador biofísico de disfunção endotelial.Materiais e Métodos:Um total de 91 pacientes de alto risco para desenvolvimento de pré-eclâmpsia foi submetido a FMD entre 24 e 28 semanas de gestação.Resultados:Das pacientes selecionadas, 19 desenvolveram pré-eclâmpsia, sendo 8 em sua forma precoce e 11 em sua forma tardia. Usando-se um valor de corte de 6,5%, a sensibilidade (S) da FMD para predição de pré-eclâmpsia precoce foi 75,0%, com especificidade (E) de 73,3%, valor preditivo positivo (VPP) de 32,4% e valor preditivo negativo (VPN) de 91,9%. Para predição de pré-eclâmpsia tardia, encontrou-se valor de S de 83,3%, E de 73,2%, VPP de 34,4% e VPN de 96,2%. Para a predição de todas as formas associadas de pré-eclâmpsia, encontrou-se valor de S de 84,2%, E de 73,6%, VPP de 45,7% e VPN de 94,6%.Conclusão:A FMD se mostrou uma ferramenta com boa capacidade de predição de pré-eclâmpsia, nas suas formas tardia e precoce, o que pode representar um impacto positivo no acompanhamento de gestantes de alto risco para desenvolvimento dessa síndrome. Objective:To assess the accuracy in the prediction of both early and late preeclampsia by flow-mediated dilation of the brachial artery (FMD), a biophysical marker for endothelial dysfunction.Materials and Methods:A total of 91 patients, considered at high risk for development of preeclampsia were submitted to brachial artery FMD between 24 and 28 weeks of gestation.Results:Nineteen out of the selected patients developed preeclampsia, 8 in its early form and 11 in the late form. With a cut-off value of 6.5%, the FMD sensitivity for early preeclampsia prediction was 75.0%, with specificity of 73.3%, positive predictive value (PPV) of 32.4% and negative predictive value (NPV) of 91.9%. For the prediction of late preeclampsia, sensitivity = 83.3%, specificity = 73.2%, PPV = 34.4% and NPV = 96.2% were observed. And for the prediction of all associated forms of preeclampsia, sensitivity = 84.2%, specificity = 73.6%, PPV = 45.7% and NPV = 94.6% were observed.Conclusion:FMD of the brachial artery is a test with good accuracy in the prediction of both early and late preeclampsia, which may represent a positive impact on the follow-up of pregnant women at high risk for developing this syndrome.

Many conditions are associated with hyperglycemia in preterm neonates because they are very susceptible to changes in carbohydrate homeostasis. The purpose of this study was to evaluate the occurrence of hyperglycemia in preterm infants undergoing glucose infusion during the first week of life, and to enumerate the main variables predictive of hyperglycemia. This prospective study (during 1994) included 40 preterm neonates (gestational age 31 weeks), birth weight (1500 g), glucose infusion rate (6 mg/kg/min) and clinical status (stable vs. unstable). Multivariate analysis by logistic regression gave the following mathematical model for predicting the probability of hyperglycemia: 1/exp{-3.1437 + 0.5819(GA) + 0.9234(GIR) + 1.0978(Clinical status)} The main predictive variables in our study, in increasing order of importance, were gestational age, glucose infusion rate and, the clinical status (stable or unstable) of the preterm newborn infant. The probability of hyperglycemia ranged from 4.1% to 36.9%. A multiplicidade de condições que provocam ou se associam à hiperglicemia em recém-nascidos pré-termo justificam a susceptibilidade destes a desequilíbrios na homeostase dos carboidratos. O objetivo deste é rastrear hiperglicemia em recém-nascidos pré-termo submetidos à infusão de glicose e relacionar as principais variáveis preditoras de hiperglicemia. Foi realizado um estudo prospectivo, durante 1994, com recém-nascidos pré-termo recebendo glicose parenteral na primeira semana de vida. No momento da determinação da glicemia eram conhecidos: idade gestacional, peso de nascimento, velocidade de infusão de glicose e condição clínica do recém-nascido (segundo critérios clínicos - hemodinâmicos e respiratórios - e laboratoriais). Estes "momentos clínicos" foram divididos em "instáveis" ou "estáveis", pela presença ou não de estabilidade na homeostase da glicose no momento da determinação da glicemia. Realizou-se um estudo caso-controle (caso-hiperglicemia; controle-normoglicemia) para o modelo multivariável (regressão logística), preditor de hiperglicemia. Os fatores de risco foram dicotomizados em: idade gestacional31sem, peso de nascimento 1500g, velocidade de infusão de glicose 6mg/kg/min e "momento clínico" (estável e instável). Realizaram-se 511 determinações de glicemia, obtendo-se 59 (11,5%) episódios de hiperglicemia em 40 recém-nascidos pré-termo. Pela análise multivariada e regressão logística, o modelo matemático da probabilidade da ocorrência de hiperglicemia foi: 1/expon{-3,1437+0,5819(IG)+0,9234(VIG)+1,0978 (Momento clínico)} e a probabilidade de hiperglicemia variou de 4,1 a 36,9%. Neste estudo, as principais variáveis preditoras de hiperglicemia, em ordem crescente de importância foram: idade gestacional, velocidade de infusão de glicose e a condição clínica instável do recém-nascido.

FUNDAMENTO: A hipertensão arterial está relacionada ao incremento da gordura corporal, a qual pode ser avaliada por meio de indicadores antropométricos. OBJETIVO: Determinar o poder preditivo de indicadores antropométricos e estabelecer seus pontos de corte como discriminadores de pressão arterial elevada. MÉTODOS: Estudo transversal realizado com uma amostra de 660 adolescentes de 14 a 19 anos sendo 51,9% moças. Foram considerados os seguintes indicadores antropométricos: índice de massa corporal (IMC), circunferência da cintura, razão cintura/estatura e índice de conicidade. A pressão arterial elevada foi caracterizada por valores acima do percentil 90 para pressão arterial sistólica e/ou pressão arterial diastólica. Para identificação dos preditores de pressão arterial elevada, foi adotada a análise das curvas Receiver Operating Characteristic (ROC), com intervalo de confiança de 95%. Posteriormente, identificaram-se os pontos de corte com suas respectivas sensibilidades e especificidades. RESULTADOS: As áreas sob as curvas ROC com os intervalos de confiança foram: rapazes - circunferência de cintura = 0,80 (0,72-0,89); IMC = 0,79 (0,68-0,89); razão cintura/estatura = 0,77 (0,66-0,88); índice de conicidade = 0,69 (0,56-0,81) e para as moças - circunferência de cintura = 0,96 (0,92-1,00); IMC = 0,95 (0,87-1,00); razão cintura/estatura = 0,93 (0,85-1,00); índice de conicidade = 0,74 (0,50-0,98). Os diversos pontos de corte dos indicadores antropométricos com melhores poderes preditivos e suas respectivas sensibilidades e especificidades foram identificados. CONCLUSÃO: Apesar de a razão cintura/estatura e de o IMC terem apresentado boas áreas sob a curva ROC, sugere-se a utilização da circunferência de cintura para a predição da pressão arterial elevada.

OBJETIVO: Predizer a resistência à insulina em crianças a partir de indicadores antropométricos e metabólicos por análise de sensibilidade e especificidade dos pontos de corte. MÉTODOS: Estudo transversal foi realizado em 109 crianças de 7 a 11 anos, sendo 55 obesas, 23 sobrepesadas e 31 eutróficas, classificadas pelo índice de massa corporal (IMC) para idade. Foram medidos IMC, circunferências da cintura e quadril, razão circunferência da cintura/circunferência do quadril, índice de conicidade e percentual de gordura (absortometria de raio X de dupla energia). Na identificação dos preditores de homeostase glicêmica, foi adotada a análise das curvas receiver operating characteristic com intervalo de confiança de 95%, calculando-se posteriormente a sensibilidade e especificidade. RESULTADOS: Os indicadores com poder de predição da resistência à insulina analisando a área sob a curva receiver operating characteristic (intervalo de confiança), com respectivos pontos de corte, foram, nesta ordem: insulinemia = 0,99 (0,99-1,00), 18,7 µU×mL-1; percentual de gordura = 0,88 (0,81-0,95), 41,3%; IMC = 0,90 (0,83-0,97), 23,69 kg×m²-¹; circunferência da cintura = 0,88 (0,79-0,96), 78,0 cm; glicemia = 0,71 (0,54-0,88), 88,0 mg×dL-1; trigliceridemia = 0,78 (0,66-0,90), 116,0 mg×dL-1 e índice de conicidade = 0,69 (0,50-0,87), 1,23 para amostra total; e insulinemia = 0,99 (0,98-1,00), 19,54 µU×mL-1; percentual de gordura = 0,76 (0,64-0,89), 42,2%; IMC = 0,78 (0,64-0,92), 24,53 kg×m²-¹; circunferência da cintura = 0,77 (0,61-0,92), 79,0 cm e trigliceridemia = 0,72 (0,56-0,87), 127,0 mg×dL-1 para os obesos. CONCLUSÕES: Indicadores antropométricos e metabólicos apresentaram bom poder de predição da resistência à insulina em crianças entre 7 e 11 anos, utilizando-se pontos de corte com melhor equilíbrio entre sensibilidade e especificidade da técnica preditora. Measurements were taken to determine BMI, waist and hips circumferences, waist circumference/hip circumference ratio, conicity index and body fat percentage (dual emission X-ray absorptiometry). Fasting blood samples were taken to measure triglyceridemia, glycemia and insulinemia. Insulin resistance was evaluated by the glycemic homeostasis method, taking the 90th percentile as the cutoff point. Receiver operating characteristic curves were analyzed to a 95% confidence interval in order to identify predictors of glycemic homeostasis, and sensitivity and specificity were then calculated. OBJECTIVE: To evaluate the association between anthropometric indexes - body mass index (BMI) and waist circumference (WC) - and hypertension, and to evaluate the predictive value of these indexes in detecting hypertension. METHODS: Cross-sectional population study conducted in the city of Goiânia (GO) with a sample of 1,238 adults aged twenty to 64 years, in 2001. Total obesity was defined as BMI > 30 kg/m2; abdominal obesity was defined as level 2 WC > 88 cm for women and > 102 cm for men, and hypertension was defined as systolic pressure > 140 mmHg, or diastolic pressure > 90 mmHg, or utilization of hypotensive drugs). Multiple logistic regression analysis was used to evaluate the associations between anthropometric indexes and hypertension. The Receiver Operating Characteristic (ROC) curve analysis was used to evaluate sensitivity and specificity of BMI (> 30) and level 2 WC as predictive factors of hypertension, and to determine the best predictive cut-off points for hypertension. RESULTS: WC was associated with hypertension in both genders. Level 2 WC and BMI >30 kg/m2 showed a low sensitivity in identifying hypertension. The best predictive cut-off points for hypertension coincided with level 1 WC (> 80 cm) and with BMI >25 kg/m2 (overweight) for women, and were lower than the values of level 1 WC and of overweight for men. CONCLUSION: Level 2 WC and BMI > 30 kg/m2 are not adequate to identify the groups at the highest risk of hypertension, since this risk rises with small increases in adiposity.

Objective: To determine the prevalence of Chronic Low Back Pain and predictors of Back Muscle Strength in patients with Systemic Lupus Erythematosus. Methods: Cross-sectional study. Ninety-six ambulatory patients with lupus were selected by non-probability sampling and interviewed and tested during medical consultation. The outcomes measurements were: Point prevalence of chronic low back pain, Oswestry Disability Index, Tampa Scale of Kinesiophobia, Fatigue Severity Scale and maximal voluntary isometric contractions of handgrip and of the back muscles. Correlation coefficient and multiple linear regression were used in statistical analysis. Results: Of the 96 individuals interviewed, 25 had chronic low back pain, indicating a point prevalence of 26% (92% women). The correlation between the Oswestry Index and maximal voluntary isometric contraction of the back muscles was r = −0.4, 95% CI [−0.68; −0.01] and between the maximal voluntary isometric contraction of handgrip and of the back muscles was r = 0.72, 95% CI [0.51; 0.88]. The regression model presented the highest value of R 2 being observed when maximal voluntary isometric contraction of the back muscles was tested with five independent variables (63%). In this model handgrip strength was the only predictive variable (β = 0.61, p = 0.001). Conclusions: The prevalence of chronic low back pain in individuals with systemic lupus erythematosus was 26%. The maximal voluntary isometric contraction of the back muscles was 63% predicted by five variables of interest, however, only the handgrip strength was a statistically significant predictive variable. The maximal voluntary isometric contraction of the back muscles presented a linear relation directly proportional to handgrip and inversely proportional to Oswestry Index i.e. stronger back muscles are associated with lower disability scores. Resumo Objetivo: Determinar a prevalência de lombalgia crônica (LBC) e os preditores de força muscular nas costas (FMC) em pacientes com lúpus eritematoso sistêmico (LES). Métodos: Estudo transversal. Selecionaram-se 96 pacientes ambulatoriais com LES por amostragem não probabilística, entrevistados e testados durante consultas médicas. As medidas de desfecho foram: prevalência ocasional de LBC, Índice de Incapacidade de Oswestry, Escala Tampa para Cinesiofobia, Escala de Gravidade da Fadiga e contrações isométricas voluntárias máximas (CIVM) de preensão manual e dos músculos das costas. Usaram-se o coeficiente de correlação e a regressão linear múltipla na análise estatística. Resultados: Dos 96 indivíduos entrevistados, 25 apresentavam LBC, o que indicou uma prevalência circunstancial de 26% (92% mulheres). A correlação entre o Índice de Incapacidade de Oswestry e a contração isométrica voluntária máxima dos músculos das costas foi de r = -0,4, IC 95% [-0,68; -0,01] e entre a CIVM de preensão manual e dos músculos das costas foi de r = 0,72, IC 95% [0,51; 0,88]. O modelo de regressão apresentou o maior valor de R2 observado quando a CIVM dos músculos das costas foi testada com cinco variáveis independentes (63%). Nesse modelo, a força de preensão manual foi a única variável preditiva (ß = 0,61, p = 0,001). Conclusões: A prevalência de LBC em indivíduos com LES foi de 26%. A CIVM dos músculos das costas foi 63% prevista por cinco variáveis de interesse. No entanto, apenas a força de preensão manual foi uma variável preditiva estatisticamente significativa. A CIVM dos músculos das costas apresentou uma relação linear diretamente proporcional à força de preensão manual e inversamente proporcional ao Índice de Incapacidade de Oswestry (ou seja, músculos das costas mais fortes estão associados a menores pontuações de incapacidade).

Objectives: The goal of this article is to provide an account of language development in the brain using the new information about brain function gleaned from cognitive neuroscience. By addressing the evidence obtained from non-invasive brain imaging in the light of prediction, this account goes beyond describing the association between language and specific brain areas to advocate the importance and possibility of predicting language outcomes using brain-imaging data. The goal is to address the current evidence about language development in the brain and the possibility of prediction of language outcomes. Sources: Recent studies will be discussed in the light of the evidence generated for predicting language outcomes and using new methods of analysis of brain data. Summary of the data: The present account of brain behavior will address: (1) the development of a hardwired brain circuit for spoken language; (2) the neural adaptation that follows reading instruction and fosters the “grafting” of visual processing areas of the brain onto the hardwired circuit of spoken language; and (3) the prediction of language development and the possibility of translational neuroscience. Conclusions: Brain imaging has allowed for the identification of neural indices (neuromarkers) that reflect typical and atypical language development; the possibility of predicting risk for language disorders has emerged. A mandate to develop a bridge between neuroscience and health and cognition-related outcomes may pave the way for translational neuroscience. RESUMO Objetivos: Apresentar um relato sobre o desenvolvimento da linguagem no cérebro com as novas informações sobre função cerebral obtidas na neurociência cognitiva. Com o uso das evidências obtidas de imagens cerebrais não invasivas em face da predição, o relato vai além da descrição da associação entre linguagem e áreas específicas do cérebro e defende a importância e a possibilidade de predizer os resultados de linguagem por meio de dados de imagens cerebrais. E tratar das evidências atuais sobre desenvolvimento da linguagem no cérebro e abordar a possibilidade de predição de resultados de linguagem. Fontes: Estudos recentes serão discutidos em face das evidências geradas pela predição de resultados de linguagem e pelo uso de novos métodos de análise de dados cerebrais. Resumo dos dados: Este relato de comportamento cerebral abordará: (1) o desenvolvimento de um circuito cerebral de linguagem falada; (2) a adaptação neural que segue a instrução da leitura e incentiva a “inserção” de áreas de processamento visual do cérebro no circuito de linguagem falada; e (3) a predição do desenvolvimento da linguagem e a possibilidade de uma neurociência translacional. Conclusões: As imagens cerebrais permitiram a identificação de índices neurais (neuromarcadores) que refletem o desenvolvimento da linguagem típico e atípico; surge a possibilidade de prever o risco de disfunções de linguagem. A responsabilidade de desenvolver uma ligação entre neurociência e resultados relacionados a saúde e cognição pode abrir o caminho para a neurociência translacional.
