Introduction Circulating intact parathyroid hormone (iPTH) levels include full-length (1-84) PTH and long C-PTH fragments, but primarily (7-84) PTH, which have been reported to have antagonistic effects on the bones and kidneys. However, their effects on the cardiovascular system remain unclear. In this study, the relationships between the plasma PTH fragments levels and heart rate variability (HRV) in stage 5 chronic kidney disease (CKD5) patients are explored. Furthermore, the effects of parathyroidectomy (PTX) on the above indices are investigated. Methods In this cross-sectional study, 164 healthy controls and 354 CKD5 patients, including 208 secondary hyperparathyroidism (SHPT) subgroup with PTX, were enrolled. Circulating (7-84) PTH levels were calculated by subtracting plasma (1-84) PTH levels from iPTH levels. The HRV parameters were measured using a 24-hour Holter. Results The baseline levels of plasma iPTH, (1-84) PTH, and (7-84) PTH in the CKD5 patients were 930.40 (160.65, 1792.50) pg/mL, 448.60 (99.62, 850.45) pg/mL, and 468.20 (54.22, 922.55) pg/mL, respectively. In the CKD5 patients, plasma (1-84) PTH levels were independently correlated with the standard deviation of the normal-to-normal R-R intervals (SDNN) and the standard deviation of the five-minute average of the normal R-R intervals (SDANN). With a median follow up time of 6.50 months after PTX in the SHPT patients (n = 30), improved SDNN and SDANN markers were related with decreased (1-84) PTH levels. Furthermore, an improved SDNN was related with decreased (7-84) PTH levels. Conclusions The CKD5 patients’ baseline (1-84) PTH levels were correlated with the SDNN and SDANN. After PTX, an improved SDNN was related with decreased (1-84) PTH and (7-84) PTH levels, while improved SDANN was related with decreased (1-84) PTH levels. No antagonistic effects of (1-84) PTH and (7-84) PTH on HRV were found in the CKD5 patients.

Based on the original text information, this paper converts the users' theme preferences and text sentiment features into attention information and combines different forms with the LSTM (Long Short-Term Memory) model to predict the personality characteristics of social network users. Finally, the experimental results of multiple groups’ show that the Attention-based LSTM model proposed in the paper can achieve better results than the currently popular methods in the recognition of user personality traits and that the model has good generalization ability.

This paper presents a novel dual-band circularly polarized patch antenna for precise satellite navigation. The radiation elements are composed of the inner cross-shaped patch and the outer annular patch which are printed on the same surface of one substrate. Two patches work in different bands, respectively, and emit dual-band circularly polarized radiation. In order to obtain a more compact antenna to meet the application of precise satellite navigation, we vertically place four metal cylinders under the ends of the cross-shaped patch to form four capacitive loadings to lower the resonant frequency of the inner cross-shaped patch. A capacitive coupling feed structure is used to match the input impedance of a patch antenna and make the antenna compact enough. The simulated and measured results show that the proposed antenna can produce appropriate dual-band circularly polarized radiation patterns for precise satellite navigation. The measured results of the antenna illustrate that maximum RHCP gain of the antenna is 4.72 dBi in the low band and 3.98 dBi in the high band, the 3 dB gain bandwidth is 70 MHz in the low band and 65 MHz in the high band.

Calciphylaxis is a rare disease characterized histologically by microvessel calcification and microthrombosis, with high mortality and no proven therapy. We reported a severe uremic calciphylaxis patient with progressive skin ischemia, large areas of painful malodorous ulcers and mummified legs. Because of her rapid progression and refractory to conventional therapy, human amnion-derived mesenchymal stem cells (hAMSCs) treatment was approved. Establishment and release inspection of hAMSCs, efficacy and safety assessment including cytokines secretory ability, immunocompetence, tumorigenicity and genetics analysisin vitrowere introduced. We further performed acute and long-term hAMSC toxity evaluations in C57BL/6 mice/rats, abnormal immune response tests in C57BL/6 mice and tumorigenic tests in the neonatal NU nude mice. After pre-clinical research, she was treated by hAMSCs with intravenous and local intramuscular injection and external supernatants application to her ulcers. When followed up to 15 months, her blood-based markers of bone and mineral metabolism were improved, with regeneration of skin soft tissue and a more favorable profile of peripheral blood mononuclear cells. Skin biopsy after 1 month treatment showed vascular regeneration with mature non-calcified vessels within dermis and 20 months later re-epithelialization restored the integrity of damaged site. No infusion or local treatment related adverse events occurred. To the best of our knowledge, this is the first evidence for the clinical use of hAMSCs. These findings suggest hAMSCs warrant further investigation as a potential regenerative treatment for uremic calciphylaxis with effects of inhibiting vascular calcification, stimulating angiogenesis and myogenesis, anti-inflammatory and immune modulation, multi-differentiation, re-epithelialization and restorage of integrity.

Purpose The purpose of this paper is to examine the impact of carbon permit allocation rules (grandfathering mechanism and benchmarking mechanism) on incentive contracts provided by the retailer to encourage the manufacturer to invest more in reducing carbon emissions. Design/methodology/approach The authors consider a two-echelon supply chain in which the retailer offers three contracts (wholesale price contract, cost-sharing contract and revenue-sharing contract) to the manufacturer. Based on the two carbon permit allocation rules, i.e. grandfathering mechanism and benchmarking mechanism, six scenarios are examined. The optimal price and carbon emission reduction decisions and members’ equilibrium profits under six scenarios are analyzed and compared. Findings The results suggest that the revenue-sharing contract can more effectively stimulate the manufacturer to reduce carbon emissions compared to the cost-sharing contract. The cost-sharing contract can help to achieve the highest environmental performance, whereas the implementation of revenue-sharing contract can attain the highest social welfare. The benchmarking mechanism is more effective for the government to prompt the manufacturer to produce low-carbon products than the grandfathering mechanism. Although a loose carbon policy can expand the total emissions, it can improve the social welfare. Practical implications These results can provide operational insights for the retailer in how to use incentive contract to encourage the manufacturer to curb carbon emissions and offer managerial insights for the government to make policy decisions on carbon permit allocation rules. Originality/value This paper contributes to the literature regarding to firm’s carbon emissions reduction decisions under cap-and-trade policy and highlights the importance of carbon permit allocation methods in curbing carbon emissions.

Objective Persistent secondary hyperparathyroidism (SHPT) may occur because of residual cervicothoracic parathyroids in parathyroidectomy (PTX) patients with chronic kidney disease. We prospectively compared the predictive values of intraoperative plasma (1-84) parathyroid hormone (PTH) and intact PTH (iPTH) levels to improve the safety and efficacy of PTX. Methods We included 100 healthy controls, 162 stage 5 chronic kidney disease patients without SHPT, and 214 patients who underwent PTX because of SHPT. Plasma iPTH and (1-84) PTH levels were measured before incision (io-iPTH0 and io-[1-84]PTH0, respectively) and 10 minutes (io-iPTH10 and io-[1-84]PTH10, respectively) and 20 minutes (io-iPTH20 and io-[1-84]PTH20, respectively) after removing all parathyroids. The percentage reduction of iPTH and (1-84) PTH at 10 minutes (io-iPTH10% and io-[1-84]PTH10%, respectively) and 20 minutes (io-iPTH20%, and io-[1-84]PTH20%, respectively) was calculated. iPTH and (1-84) PTH were measured using second- and third-generation PTH assays, respectively. Results Compared with the controls and non-PTX patients, the PTX group had more obvious mineral metabolism disorders. There were 187 successful PTXs, 19 patients with persistent SHPT, and 8 patients lost to follow-up. The receiver operating characteristic curves revealed that io-(1-84)PTH10% >86.6% and io-(1-84)PTH20% >87.5% suggested successful PTX. The sensitivity of io-iPTH20% and io-(1-84)PTH20% were higher than those at the timepoint of 10 minutes. Moreover, the specificity and sensitivity of the (1-84) PTH reduction percentage were superior to that of iPTH. Conclusion Intraoperative reduction percentages of plasma (1-84) PTH levels are superior to iPTH for accurately predicting successful PTX, especially at 20 minutes after all cervicothoracic parathyroids had been resected.

With the strengthening of environmental awareness, the government pays much more attention to environmental protection and thus implements carbon trading schemes to promote the reduction of global carbon dioxide emissions. The carbon Generalized System of Preferences (GSP) is an incentive mechanism for citizens to value their energy conservation and carbon reduction. Individual travel needs to rely on various means of transportation, resulting in energy consumption. Carbon tax or subsidy can only be carried out after carbon GSP accurately measures individual carbon emissions. The big data acquired from the smart cards of passengers&rsquo, travels provide the possibility for carbon emission accounting of individual travel. This research proposes a carbon emission measurement of individual travel. Through establishing the network model of the Nanjing metro with a complex method, the shortest path of the passengers&rsquo, travels is obtained. Combined with the origination&ndash, destination (OD) records of the smart cards, the total distance of the passengers&rsquo, travels is obtained. By selecting the operation table to estimate the carbon emissions generated by the daily operation of the subway system, the carbon emissions per kilometer or per time of passenger travel are finally obtained. With the accurate tracking of carbon emissions for individual travel, the government may establish a comprehensive monitoring system so as to establish a carbon tax and carbon supplement mechanism for citizens.

Background Without adequate reporting of research, valuable time and resources are wasted. In the same vein, adequate reporting of practice guidelines to optimise patient care is equally important. Our study examines the quality of reporting of published WHO guidelines, over time, using the RIGHT (Reporting Items for Practice Guidelines in HealThcare) reporting checklist. Methods We examined English-language guidelines approved by the WHO Guidelines Review Committee from inception of the committee in 2007 until 31 December 2017. Pairs of independent, trained reviewers assessed the reporting quality of these guidelines. Descriptive data were summarised with frequencies and percentages. Results We included 182 eligible guidelines. Overall, 25 out of the 34 RIGHT items were reported in 75% or more of the WHO guidelines. The reporting rates improved over time. Further, 90% of the guidelines reported document type in the title. The identification of evidence, the rationale for recommendations and the review process were reported in more than 80% of guidelines. The certainty of the evidence using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) system was assessed in 81% of the guidelines assessed. While 82% of guidelines reported funding sources, only 25% mentioned the role of funders. Conclusions WHO guidelines provide adequate reporting of many of the RIGHT items and reporting has improved over time. WHO guidelines compare favourably to guidelines produced by other organisations. However, reporting can be further improved in a number of areas.

Purpose: RNA polymerase I subunit D (POLR1D) is involved in the synthesis of ribosomal RNA precursors and small RNAs, but its mechanism in the development and progression of colorectal cancer (CRC) remains ambiguous. Thus, this research aimed to investigate POLR1D’s expression and significance in human CRC patients and evaluate its association with clinicopathological characteristics. Methods: Matched fresh-frozen cancerous and non-cancerous tissues were collected from 100 patients diagnosed with CRC. Immunohistochemical, Western blot, and quantitative real-time polymerase chain reaction analyses were adopted to validate the correlation between POLR1D expression and clinicopathological parameters in CRC tissues and adjacent normal tissues (ANTs). Results: POLR1D expression in CRC tissues was significantly higher than in the ANTs. χ2 test and Spearman’s correlative analysis showed that a high POLR1D expression is significantly associated with clinical stage, Dukes stage, tumor differentiation, depth of invasion, and metastasis (p < 0.05). It is not correlated with gender, age, and tumor location and size (p > 0.05). Kaplan-Meier survival curves show that the overall survival (OS) time for the low expression group is remarkably longer than for the high expression group (p < 0.0015). Univariate and multivariate analyses indicate that a high POLR1D expression is an independent prognostic factor for poor OS (p = 0.000). Conclusion: The findings of this study strongly indicate that POLR1D plays a pivotal role in the occurrence and progression of CRC. It might be an independent adverse prognostic factor for CRC patients and could serve as a potential therapeutic target for clinical diagnosis in CRC and anticancer drug development.

An effective degradation indicator created from the general features is still a hotspot for the condition monitoring of bearing. To cover the shortage of the general features based indicator, some new indicators are built using multiple general features extracted from the original vibration signal without considering the internal relevancy among the features. To address that problem, a new indicator is proposed using the Orthogonal Neighborhood Preserving Projections (ONPP) and 2-Dimensional Hidden Markov Model (2-D HMM). With the ability of keeping the local structure of data set, Orthogonal Neighborhood Preserving Projections is used to obtain the low dimensional features with the main information remained. Unlike 1-Dimensional data-processing algorithm that commonly converts the multiple features into a vector to deal with the high-dimensional data with the integral property of the multiple features considered only, 2-Dimensional Hidden Markov Model not only take the relevance between the individuals of fault features into consideration but also capture the global characteristics of the multiple features. Then a likelihood probability based health assessment indication can be constructed by combing 2-D HMM with the data pre-processed by ONPP. The experiment results indicate that the proposed indicator show great abilities to make degradation performance of the bearing and is sensitive to incipient defects.

Introduction In recent years, assisted reproductive technology (ART) has developed rapidly, leading to an increasing number of clinical practice guidelines in this field. However, the reporting quality of current clinical practice guidelines in ART is still unknown. Objective To evaluate the reporting quality of clinical practice guidelines in the field of ART using the RIGHT checklist. Method Relevant guidelines were identified by electronic search of PubMed, Chinese Biomedical Literature Database (CBM), Wan Fang Database and Chinese National Knowledge Infrastructure (CNKI) from the beginning of the database to October, 2017. We also searched the websites of the guideline development organizations, including Guidelines International Network (GIN), National Guideline Clearinghouse (NGC) and the National Institute for Health and Care Excellence (NICE), as well as from two medical associations, including the European Society of Human Reproduction and Embryology (ESHRE) and the American Society for Reproductive Medicine (ASRM). We used Google Scholar to find additional clinical practice guidelines (CPGs) as well. Two investigators searched the database, selected guidelines independently based on the inclusion criteria, and extracted the relevant information. Result Fifteen guidelines (i.e. six developed by individual institutions and 9 by associations) were included. On average, 12.7 out of 35 items in the RIGHT standard (36.3%) were reported in each guideline. Five items were not reported by any of these guidelines. The reporting proportion of the seven domains (i.e. Basic information; Background; Evidence; Recommendations; Review and quality assurance; Funding and declaration and management of interests; Other information) were 46.7%, 40.8%, 45.3%, 29.5%, 53.3%, 10.0%, 26.7%, respectively. Conclusion At present, the reporting quality of guidelines for ART is poor, especially regarding the funding. In the future guideline development, more consideration should be given to reporting, dissemination and implementation.

Retailers in supply chains usually deal with the problem of capital shortage by obtaining financing from the Chinese financial market. Examining the financing requirement of capital-constrained retailers, this paper studies the optimal decisions of the supplier and retailer in the supply chain, who are both risk-neutral decision-makers under wholesale price contracts. We assume that the retailer may obtain financing from a bank, the supplier and investors. Thus, the retailer can adopt two mixed financing modes: mixed financing of a bank and equity credit and mixed financing of trade and equity credit. We obtain equilibrium decisions under the two financing modes. By comparing the equilibrium results, we find that the mixed financing of trade and equity credit exhibits the only financing equilibrium. A Pareto interval of the wholesale price exists such that both the supplier and retailer can obtain higher profits with the mixed financing of trade and equity credit than the mixed financing of bank and equity credit. Finally, we present a numerical analysis to support our results.

Background Hypertension is one of the common diseases, which brings heavy burden to human race. Currently, western medication is in absolutely dominant position in the treatment of hypertension, but it maintains for a short time and there are various side effects and drug tolerance. Therefore, Chinese medicine has attracted great attention in the treatment field of hypertension, and angelica is one of the most frequently used herbs. Objectives In order to give some inspiration to researches in these fields, this article presents the current research status of angelica and its compound formulas treating hypertension and its complications with evidence mapping. Methods Main databases were systematically searched, and researches about angelica or its compound formulas containing angelica treating hypertension or its complications were included. EXCEL 2013 was used to integrate and process the data, and the result is showed intuitively with the bubble diagram. Results 49 RCTs were included after screening. The articles recruited were published with a rising trend along with time. Of the 49 RCTs, there is the outcome measure of general the efficacy in the result part in 34 RCTs (69.4%), and all the clinical effective rate in the angelica intervention group is significantly higher than the control group. There is the outcome measure of reduction of MAP in the result part in 28 RCTs, and 27 RCTs (96.4%) showed that the angelica intervention group is significantly improved than the control group while 1 (3.6%) showed no significant differences. There is the outcome measure of efficacy of target organ protection in the result part in 26 RCTs, and 25 RCTs (96.2%) showed that there is significant difference between the angelica intervention group and the control group. Of the 49 RCTs, there is the outcome measure of adverse effects in the result part in 17 RCTs. 14 RCTs (82.4%) reported no adverse effects, 2 RCTs (11.8%) reported adverse effects rate as lower than 10%, and 1 RCT (7.1%) reported adverse effects rate as higher than 40%. Conclusion Current research with low quality has revealed that angelica is effective in reduction of MAP and target organ protection and the adverse effects rate is low, and the effectiveness and safety of angelica needs to be proved by further researches with high quality. Researches of high quality are needed to provide scientific evidence for angelica in treating hypertension and its complications.

The facility layout problem (FLP) is to arrange facilities within a given area so as to minimise the total material-handling cost. An efficient arrangement of facilities plays an important part in the achievement of high productivity in a manufacturing system. This paper focuses on the layout design of unequal-sized facilities with fixed shapes and input/output (I/O) points. A mixed-integer programming (MIP) model is developed to obtain optimal solutions. Since the MIP model is not applicable to large-sized problems, a two-step heuristic algorithm is developed to solve the FLPs. In the first step, a layout solution with moderate quality is generated by using an interconnected zone algorithm and a simulated annealing (SA) algorithm. With the intensification of global warming and the levy of energy tax, more industries are paying attention to energy saving and reduction of carbon footprint. For the food industry, energy cost in the supply chain of perishable food is quite high because of cold-chain transport and storage. Therefore, the efficacies of cold chain management and inventory control are the key factors that increase the efficiency of food supply chain and make it more ecological. This research analyzes the degradation process of perishable food and determines the optimal temperature of the cold chain as well as the optimal price to maximize the channel profit. We prove that there is an optimal price with a certain temperature and develop an efficient search algorithm to find the optimal temperature. We also perform sensitivity analyses to test which parameters affect the channel profit significantly. Numerical experiments are conducted to illustrate the proposed models.

The PWM motor drive system has well running characteristics and it has been widely used in the industrial and civilian equipment field， but the high speed switching action of the power device may cause serious problem of EMI by voltage and current transients. This paper based on the research of the differential mode interference and common mode interference of the system while multiple converters parallel has been accessed to the power grid. First we establish a system of the frequency domain model which shows the two converter parallel system of differential-mode voltage and common-mode voltage expression. Then we set up the experiment with the three power of size 1.5kw, 5.5kw and7.5kw converters parallel accessed grid. Finally we draw the conclusion from the data of the experiment：in the low frequency,the system of differential-mode current and common-mode current is relative to the phase difference between the multiple converters of differential-mode voltage source and common-mode voltage source; the probability of the offset phenomenon between the differential mode current common-mode current is increased by the number of the converters. In the high frequency, ,the system of EMI does not effect to the numble of converters.
