Background: Upregulation of mitochondrial E3 ubiquitin ligase 1 (Mul1) contributes to brain injury in ischemic stroke due to disturbance of mitochondrial dynamics, and bioinformatics analysis predicts that Mul1 is a potential target of Dipsacoside B. Objective: The aim of the study was to explore whether Dipsacoside B can exert a beneficial effect on brain injury in the ischemic stroke rat via targeting Mul1. Methods: The SD rat brains or PC12 cells were subjected to 2 h-ischemia or 8 h-hypoxia plus 24 h-reperfusion or 24 h-reoxygenation to establish the ischemic stroke rat model in vivo or in vitro, which were treated with Dipsacoside B at different dosages. The brain or PC12 cell injury, relevant protein levels and mitochondrial functions were measured by methods of biochemistry, flow cytometry or Western blot. Results: The neurological dysfunction and brain injury (such as infarction and apoptosis) observed in the ischemic stroke rats were accompanied by increases in Mul1 and dynamin-related protein 1 (Drp1) levels along with decreases in mitofusin 2 (Mfn2) level and ATP production. These effects were attenuated by Dipsacoside B. Consistently, cell injury (necroptosis and apoptosis) occurred in the PC12 cells exposed to hypoxia concomitant with the upregulation of Mul1 and Drp1 along with downregulation of Mfn2 and mitochondrial functions (such as increases in reactive oxygen species production and mitochondrial fission and decreases in mitochondrial membrane potential and ATP production).These phenomena were reversed in the presence of Dipsacoside B. Conclusion: Dipsacoside B can protect the rat brain against ischemic injury via inhibition of Mul1 due to the improvement of mitochondrial function.

Due to the scarcity of indium (In) in the earth and its potential harm to individuals, the development of In-free transparent conductive film is considered crucial. In this work, In-free SnO2:Sb/Au/SnO2:Sb (ATO/Au/ATO, SAS) tri-layer films with high transparency and conductivity were successfully prepared on polycarbonate (PC) substrates by RF and DC magnetron sputtering at room temperature. The influence of the Au layer thickness on microstructure, electrical and optical performances was systematically studied after fixing the ATO thickness to 50 nm. It was indicated by X-ray diffraction patterns that ATO is amorphous and Au is oriented along (111). The trend of increasing and then decreasing light transmission with Au layer thickness was observed in both experimental and simulation results. The improved figure of merit (FoM, 1.89 × 10−2 Ω−1) was achieved in SAS tri-layer film, the resistivity and average transmittance of which was lowered to 7.50 × 10−5 Ω cm and 81.4%, respectively, when Au layer thickness is 11 nm. Moreover, the mechanism of the variation of optical and electrical properties at different Au layer thickness was proposed. Particularly, the SAS tri-layer films also exhibit superior flexibility, durability and adhesion. These results demonstrate SAS tri-layer films are promising alternative to ITO in flexible electronics applications.

Purpose: To investigate the clinical value of urine interleukin-18 (IL-8), neutrophil gelatinase-associated lipocalin (NGAL) and kidney injury molecule-1 (KIM-1) for the early diagnosis of acute kidney injury (AKI) in patients with ureteroscopic lithotripsy (URL) related urosepsis. Methods: A retrospective study was carried out in 157 patients with urosepsis after URL. The patients were divided into AKI group and non-AKI group according to the Kidigo guideline and urine IL-8, NGAL and KIM-1 levels were detected by enzyme-linked immunosorbent assay at 0, 4, 12, 24 and 48 h after the surgery. Introduction Recently, Nrf2/HO-1 has received extensive attention as the main regulatory pathway of intracellular defense against oxidative stress and is considered an ideal target for alleviating endothelial cell (EC) injury. Objectives This paper aimed to summarized the natural monomers/extracts that potentially exert protective effects against oxidative stress in ECs. Methods A literature search was carried out regarding our topic with the keywords of “atherosclerosis” or “Nrf2/HO-1” or “vascular endothelial cells” or “oxidative stress” or “Herbal medicine” or “natural products” or “natural extracts” or “natural compounds” or “traditional Chinese medicines” based on classic books of herbal medicine and scientific databases including Pubmed, SciFinder, Scopus, the Web of Science, GoogleScholar, BaiduScholar, and others. Then, we analyzed the possible molecular mechanisms for different types of natural compounds in the treatment of atherosclerosis via the protection of vascular endothelial cells from oxidative stress. In addition, perspectives for possible future studies are discussed. Results These agents with protective effects against oxidative stress in ECs mainly include phenylpropanoids, flavonoids, terpenoids, and alkaloids. Most of these agents alleviate cell apoptosis in ECs due to oxidative stress, and the mechanisms are related to Nrf2/HO-1 signaling activation. However, despite continued progress in research on various aspects of natural agents exerting protective effects against EC injury by activating Nrf2/HO-1 signaling, the development of new drugs for the treatment of atherosclerosis (AS) and other CVDs based on these agents will require more detailed preclinical and clinical studies. Conclusion Our present paper provides updated information of natural agents with protective activities on ECs against oxidative stress by activating Nrf2/HO-1. We hope this review will provide some directions for the further development of novel candidate drugs from natural agents for the treatment of AS and other CVDs.

Background Antiviral therapy should reduce the recurrence of hepatitis B virus‐related hepatocellular carcinoma (HBV‐related HCC) after surgical resection. However, there is little research on whether various antiviral drugs have different prognostic effects in patients with HBV‐related HCC after curative liver resection. The present study compared the effects of nucleotide analog (NtA) and nucleoside analog (NsA) antiviral therapies after surgical resection on the prognosis of HBV‐related HCC. Methods A total of 1303 patients with HBV‐related HCC who received curative hepatectomy at five institutes between April 2014 and April 2019 were retrospectively enrolled and analyzed. Propensity matching analysis was used to compare the outcomes of HCC patients given NsA versus NtA therapy. Subgroup analysis of patients treated with entecavir (ETV) and tenofovir disoproxil fumarate (TDF) was also performed. Results Among 1303 patients, 759 (58.2%) patients developed recurrence, and 460 (35.3%) patients died. Multivariable analyses revealed that NtA therapy significantly decreased the risk of HCC recurrence (hazard ratio [HR], 0.64; 95% confidence interval [CI], 0.51–0.80; p, The aim of this study was to investigate the effect of antiviral drug on the prognosis of hepatitis B virus‐related hepatocellular carcinoma (HCC) after curative hepatectomy. We found that nucleotide analog treatment, rather than nucleoside analog treatment, significantly reduced HCC recurrence and improved overall survival after curative hepatic resection.

Precision agriculture involves applying artificial intelligence, computers, sensors, and automation to improve crop field productivity while monitoring environmental conditions to conserve soil, water, and other natural resources focusing on agricultural sustainability. Despite many applications in agriculture, data monitoring and recording technologies have limited use due to the price. Low-cost open-source systems, like the ones available with the Internet of things (IoT) world, can potentially be developed as a universal-fit and cloud-connected technology for multiple applications. We designed and built a basic data collecting system using a commercial standalone embedded computer with Python programming language, serial data interface (SDI)-12/analog sensor adaptor, and digital sensors to monitor soil moisture and transmit the data remotely. SDI-12 is a standard communication protocol that transfers digital sensor measurements to a data recorder. We set up a pilot study that automatically collected and uploaded the data into the Internet to allow remote data transfer and access. The system performed reliably over 1 week with potting soil under field conditions without maintenance and successfully recorded data in real-time. The volumetric water content ranged from 0.03 to 0.23 m3·m−3, dielectric permittivity from 3.3 to 18.9 (unitless), EC from 0.0 to 0.3 dS·m−1, and soil temperature from 20.7 to 44.8 °C. All the data were successfully collected and uploaded to the cloud every 20 min, allowing users to remotely monitor the data using a free online application. However, heavy rainfall and high insolation could damage the system through excessive moisture or overheating, requiring a waterproof and heavy-duty protection case. The ThingSpeak channel allows customizing to suit a user’s specific requirements or adding more features for further development, such as automated irrigation, which can improve irrigation and fertilization efficiency by applying water and fertilizers at the right time based on sensor readings.

Purpose Abiraterone acetate, prescribed for metastatic prostate cancer, has enhanced absorption with food. This effect was exploited in a randomized trial which showed noninferiority of PSA decline for 250 mg abiraterone with a low-fat meal (LOW) compared to 1,000 mg abiraterone fasting (STD). Drug was obtained via patient insurance. Patient out-of-pocket costs and adherence were surveyed. Methods Trial participants were randomized to STD or LOW, and surveys of adherence and out-of-pocket costs were administered at baseline and just before coming off study (follow-up). Results Out-of-pocket costs were available from 20 of 36 STD and 21 of 36 LOW patients. Median out-of-pocket costs for a month of drug were $0 (LOW) and $5 (STD); mean costs were $43.61 (LOW) and $393.83 (STD). The two groups did not differ significantly (p = 0.421). Maximum out-of-pocket cost was $1,000 (LOW) and $4,000 (STD). Monthly out-of-pocket costs > $500 were found in 1 LOW and 5 STD patients. For adherence, only 11 STD and 19 LOW patients had questionnaires completed at both baseline and follow-up. STD adherence was 98.18% at baseline and 91.69% at follow-up, differing significantly (p = 0.0078). LOW adherence was 96.52% at baseline and 97.86% at follow-up, not differing significantly (p = 0.3511). Adherence did not correlate with demographics. At follow-up, increasing adherence correlated significantly with decreasing dose (p = 0.013; rho = - 0.458). Conclusions Out-of-pocket costs did not differ significantly in this limited analysis. Adherence was significantly different in STD as the trial progressed, which was not found in LOW. Trial registration ClinicalTrials.gov NCT01543776; registered March 5, 2012.

OBJECTIVE: This study was undertaken to investigate the COVID-19 vaccine uptake rate and possible postvaccination effects in adults with epilepsy. METHODS: We invited adults with epilepsy attending three centers in China from July 24 to August 31, 2021 to participate in this study. We also asked age- and sex-matched controls among people attending for other chronic neuropsychiatric conditions and healthy controls accompanying people with illness attending the hospitals to participate. We excluded people who, under the national guidelines, had evident contradictions to vaccination. Participants were interviewed face-to-face using questionnaires. Vaccine uptake and postvaccine adverse events among the people with epilepsy were compared with those with neuropsychiatric conditions and controls. We also compared the willingness and reasons for hesitancy among unvaccinated participants. RESULTS: We enrolled 981 people, of whom 491 had epilepsy, 217 had other neuropsychiatric conditions, and 273 were controls. In recent years, machine learning (ML) algorithms have been approved effective in the intrusion detection. However, as the ML algorithms are mainly applied to evaluate the anomaly of the network, the detection accuracy for cyberattacks with multiple types cannot be fully guaranteed. The existing algorithms for network intrusion detection based on ML or feature selection are on the basis of spurious correlation between features and cyberattacks, causing several wrong classifications. In order to tackle the abovementioned problems, this research aimed to establish a novel network intrusion detection system (NIDS) based on causal ML. The proposed system started with the identification of noisy features by causal intervention, while only the features that had a causality with cyberattacks were preserved. Then, the ML algorithm was used to make a preliminary classification to select the most relevant types of cyberattacks. As a result, the unique labeled cyberattack could be detected by the counterfactual detection algorithm. In addition to a relatively stable accuracy, the complexity of cyberattack detection could also be effectively reduced, with a maximum reduction to 94% on the size of training features. Moreover, in case of the availability of several types of cyberattacks, the detection accuracy was significantly improved compared with the previous ML algorithms.

The polarization of macrophages plays a critical role in the physiological and pathological progression of rheumatoid arthritis (RA). Activated M1 macrophages overexpress folate receptors in arthritic joints. Hence, we developed folic acid (FA)-modified liposomes (FA-Lips) to encapsulate triptolide (TP) (FA-Lips/TP) for the targeted therapy of RA. FA-Lips exhibited significantly higher internalization efficiency in lipopolysaccharide (LPS)-stimulated RAW 264.7 cells than liposomes (Lips) in the absence of folate. Next, an adjuvant-induced arthritis (AIA) rat model was established to explore the biodistribution profiles of FA-Lips which showed markedly selective accumulation in inflammatory paws. Moreover, FA-Lips/TP exhibited greatly improved therapeutic efficacy and low toxicity in AIA rats by targeting M1 macrophages and repolarizing macrophages from M1 to M2 subtypes. Overall, a safe FA-modified liposomal delivery system encapsulating TP was shown to achieve inflammation-targeted therapy against RA via macrophage repolarization.

Inspired by the success of classical neural networks, there has been tremendous effort to develop classical effective neural networks into quantum concept. In this paper, a novel hybrid quantum–classical neural network with deep residual learning (Res-HQCNN) is proposed. We firstly analyse how to connect residual block structure with a quantum neural network, and give the corresponding training algorithm. At the same time, the advantages and disadvantages of transforming deep residual learning into quantum concept are provided. As a result, the model can be trained in an end-to-end fashion, analogue to the backpropagation in classical neural networks. To explore the effectiveness of Res-HQCNN , we perform extensive experiments for quantum data with or without noisy on classical computer. The experimental results show the Res-HQCNN performs better to learn an unknown unitary transformation and has stronger robustness for noisy data, when compared to state of the arts. Moreover, the possible methods of combining residual learning with quantum neural networks are also discussed.

Roberts, SSH, Aisbett, B, Teo, W-P, and Warmington, S. Monitoring effects of sleep extension and restriction on endurance performance using heart rate indices. J Strength Cond Res 36(12): 3381-3389, 2022-Heart rate (HR) indices are useful for monitoring athlete fatigue or "readiness to perform." This study examined whether HR indices are sensitive to changes in readiness following sleep restriction (SR) and sleep extension (SE). Nine athletes completed a crossover study with 3 conditions: SR, normal sleep (NS), and SE. Each condition required completion of an endurance time trial (TT) on 4 consecutive days (D1-D4). Athletes slept habitually before D1; however, time in bed was reduced by 30% (SR), remained normal (NS), or extended by 30% (SE), on subsequent nights (D1-D3). Daily resting HR and HR variability were recorded. The maximal rate of HR increase and HR recovery was determined from a constant-load test before TTs. Exercise intensity ratios incorporating mean HR, mean power (W), and perceived exertion (RPE) were recorded at steady state during constant-load tests (W:HR SS ) and during TTs (W:HR TT , RPE:HR TT ). Compared with D4 of NS, RPE:HR TT was lower on D4 of SE ( p = 0.008)-when TT performances were faster. Compared with D1 of SR, RPE:HR TT was higher on D3 and D4 of SR ( p0.02). Moderate correlations were found between percentage changes in W:HR TT and changes in TT finishing time in SR ( r = -0.67, p = 0.049) and SE ( r = -0.69, p = 0.038) conditions. Intensity ratios incorporating mean HR seem sensitive to effects of sleep duration on athlete readiness to perform. When interpreting intensity ratios, practitioners should consider potential effects of prior sleep duration to determine whether sleep-promoting interventions are required (e.g., SE).

OBJECTIVES Vascular rings are rare anomalies of congenital heart disease that cause respiratory and gastrointestinal symptoms. This study assessed the long-term outcomes of patients with vascular ring division. METHODS A multi-institution retrospective review of 371 patients with vascular rings undergoing surgical division at 3 paediatric cardiac institutions between November 2007 and October 2019 was performed. RESULTS The complete vascular rings consisted of a double aortic arch (24.5%), right aortic arch with left ligamentum arteriosum (36.7%) and left aortic arch, with right ligamentum arteriosum (0.5%). The incomplete vascular rings consisted of a pulmonary artery sling (22.9%), left aortic arch with aberrant right subclavian artery (15.1%) and innominate artery compression syndrome (0.3%). Respiratory symptoms included stridor (71.4%), wheezing (49.1%), coughing (31.5%), gastrointestinal symptoms included choking (12.4%), dysphagia (3.2%) and emesis (1.9%). Only one patient died after discharge, yielding a late mortality rate of 0.3% (1/360). The 10-year overall survival rate was 96.8%. Postoperative complications were reported in 51 patients, 15 of whom required reoperation. The 10-year freedom from reoperation rate was 95.9%. Follow-up was completed in 95.4% (354/371) of patients, with a mean follow-up time of 4.3 ± 2.9 years (range from 1 to 13 years). Twenty patients (5.6%) experienced residual symptoms during long-term follow-up. CONCLUSIONS The outcomes of vascular ring division are excellent. A Kommerell diverticulum >1.5 times the aberrant left subclavian artery origin is an operative indication for primary resection. Tracheomalacia is a risk factor for reoperation and residual symptoms, and preoperative fibrobronchoscopy is important for evaluation.

Machine-learning (ML) can be employed to enhance the positioning accuracy of visible-light-positioning (VLP) system. To diminish the training time and complexity, the whole area is usually divided into several positioning unit cells. Most literatures only focus on the positioning performance within an unit cell, and assume the unit cell can be repeatedly duplicated to cover the whole area. In this work, we propose and demonstrate a positioning unit cell model duplication scheme, named as spatial sequence adaptation (SSA) process. We also propose and demonstrate a residual concatenation neural network (RCNN) and transfer learning (TL) to refine the model of the target positioning unit cell. A practical test-bed with vertical distance of 2.8 m consisting of two unit cells with dimensions of about 1.55 m × 2 m per cell is constructed. The client side is an autonomous mobile robot (AMR) for acquiring continuous training and testing data. Our experimental results reveal that high precision positioning in the duplicated unit cell duplication can be achieved.

Background Falls are a major health burden for older adults with Parkinson’s disease (PD), but there is currently no reliable questionnaire to capture the circumstances and consequences of falls in older adults with PD. This study aimed to develop a PD-specific falls questionnaire and to evaluate its test-retest reliability in older adults with PD. Methods A novel PD-specific falls questionnaire (PDF-Q) was developed in two modes (online and paper-based version) and used to assess falls and near-falls events over the past 12-months. Questions were agreed upon by an expert group, with the domains based on previous falls-related questionnaires. The questions included the number and circumstances (activities, location and direction) of falls and near-falls, and consequences (injuries and medical treatment) of falls. The PDF-Q was distributed to 46 older adults with PD (online n = 30, paper n = 16), who completed the questionnaire twice, 4 weeks apart. Kappa (κ) statistics were used to establish test-retest reliability of the questionnaire items. Results Pooled results from both questionnaires for all participants were used to assess the overall test-retest reliability of the questionnaire. Questions assessing the number of falls (κ = 0.41) and the number of near-falls (κ = 0.51) in the previous 12-months demonstrated weak agreement, while questions on the location of falls (κ = 0.89) and near-falls (κ = 1.0) demonstrated strong to almost perfect agreement. Questions on the number of indoor (κ = 0.86) and outdoor (κ = 0.75) falls demonstrated moderate to strong agreement, though questions related to the number of indoor (κ = 0.47) and outdoor (κ = 0.56) near-falls demonstrated weak agreement. Moderate to strong agreement scores were observed for the most recent fall and near-fall in terms of the direction (indoor fall κ = 0.80; outdoor fall κ = 0.81; near-fall κ = 0.54), activity (indoor fall κ = 0.70; outdoor fall κ = 0.82; near-fall κ = 0.65) and cause (indoor fall κ = 0.75; outdoor fall κ = 0.62; near-fall κ = 0.56). Conclusions The new PDF-Q developed in this study was found to be reliable for capturing the circumstances and consequences of recent falls and near-falls in older adults with PD.

PURPOSE This phase I/II study evaluated tremelimumab (anticytotoxic T-lymphocyte–associated antigen-4 monoclonal antibody) and durvalumab (antiprogrammed death ligand-1 monoclonal antibody) as monotherapies and in combination for patients with unresectable hepatocellular carcinoma (HCC), including a novel regimen featuring a single, priming dose of tremelimumab (ClinicalTrials.gov identifier: NCT02519348 ). PATIENTS AND METHODS Patients with HCC who had progressed on, were intolerant to, or refused sorafenib were randomly assigned to receive T300 + D (tremelimumab 300 mg plus durvalumab 1,500 mg [one dose each during the first cycle] followed by durvalumab 1,500 mg once every 4 weeks), durvalumab monotherapy (1,500 mg once every 4 weeks), tremelimumab monotherapy (750 mg once every 4 weeks [seven doses] and then once every 12 weeks), or T75 + D (tremelimumab 75 mg once every 4 weeks plus durvalumab 1,500 mg once every 4 weeks [four doses] followed by durvalumab 1,500 mg once every 4 weeks). Safety was the primary end point. Secondary end points included objective response rate (ORR) by Response Evaluation Criteria in Solid Tumors v1.1 and overall survival; exploratory end points included circulating lymphocyte profiles. RESULTS A total of 332 patients were enrolled (T300 + D, n = 75; durvalumab, n = 104; tremelimumab, n = 69; and T75 + D, n = 84). Tolerability was acceptable across arms, with grade ≥ 3 treatment-related adverse events occurring in 37.8%, 20.8%, 43.5%, and 24.4%, respectively. Confirmed ORRs (95% CI) were 24.0% (14.9 to 35.3), 10.6% (5.4 to 18.1), 7.2% (2.4 to 16.1), and 9.5% (4.2 to 17.9), respectively. An early expansion of CD8+ lymphocytes was associated with response across arms, with highest proliferating CD8+ lymphocyte levels occurring in the T300 + D arm. The median (95% CI) overall survival was 18.7 (10.8 to 27.3), 13.6 (8.7 to 17.6), 15.1 (11.3 to 20.5), and 11.3 (8.4 to 15.0) months in the T300 + D, durvalumab, tremelimumab, and T75 + D arms, respectively. CONCLUSION All regimens were found to be tolerable and clinically active; however, the T300 + D regimen demonstrated the most encouraging benefit-risk profile. The unique pharmacodynamic activity and association with ORR of the T300 + D regimen further support its continued evaluation in HCC.

A silicon cantilever-based fiber-optic acoustic sensor (FOAS) is presented in this work. A rectangular cantilever is fabricated upon a silicon-on-insulator (SOI) wafer using a micro-electro-mechanical system (MEMS). The length, width and thickness of the silicon cantilever are $530~\mu \text{m}$ , $200~\mu \text{m}$ and $3~\mu \text{m}$ , respectively. The resonant frequency of the silicon cantilever is 14820 Hz with a sensitivity of 950 nm/Pa. An ultra-high speed absolute cavity length demodulation method is adopted using a complementary metal oxide semiconductor (CMOS) spectrometer and an 850 nm superluminescent light emitting diode (SLED). A modified Buneman frequency estimation and total phase demodulation algorithm is adopted. The frequency response curve shows a relatively flat trend from 20 Hz to 13 kHz. The sensor exhibits good linearities at different frequencies while the applied acoustic pressure is increased from 0 Pa to 2.5 Pa. Experimental results indicate that the minimum detectable pressure (MDP) of the proposed FOAS is calculated to be $25.68~\mu $ Pa/Hz1/2 at the frequency of 13 kHz. The silicon cantilever-based FOAS achieves broad operating bandwidth, high sensitivity, small size, and good stability.

Grant-free media access is vital for applications in Industrial IoTs (IIoTs), where stringent delays are required. Recently, due to the capability of supporting parallel receptions, Non-Orthogonal Multiple Access (NOMA) has gained research interests in IIoTs. Obviously, combining them organically is beneficial for enhancing the delay performances. In this paper, for a typical convergecast wireless network where its data sink is NOMA-based, we propose a grant-free MAC (Media Access Contention) scheme based on Compressive Sensing in Busy Tone Channel (CSiBTC), by exploiting the transmission sparsity in IIoTs. First, a to-be transmitter acquires the identities of active transmitters with the proposed CSiBTC scheme completely by itself. Two construction methods for CSiBTC are proposed for two distinct application scenarios respectively. Then, given the locations of all wireless sensors and the data sink in the network, the to-be transmitter can find out if it is eligible for starting its transmission without impairing the on-going transmissions. The scheme is grant-free and makes the most use of the parallel reception capability of NOMA, and therefore both the delay performance and throughput performance can be improved with respect to the general CS-based MAC. Performance evaluations also strongly support the above conclusions.

Background Presently, whether X-ray repair cross complementing group 3 (XRCC3) Thr241Met polymorphism is correlated to leukemia risk remains controversial. Because of this reason, the objective of current study is to explore whether XRCC3 Thr241Met polymorphism confers risk to leukemia. Methods Two independent authors systematically and comprehensively searched Pubmed, Embase, the Cochrane library, Google academic, China National Knowledge Infrastructure (CNKI). Search time is from database foundation to March 2021. Results Overall, significant associations between leukemia risk and XRCC3 Thr241Met polymorphism were found in Caucasian population by allele contrast (T vs. C: OR 1.20, 95% CI 1.02–1.40), homozygote comparison (TT vs. CC: OR 1.35, 95% CI 1.05–1.73), and recessive genetic model (TT vs. TC/CC: OR 1.31, 95% CI 1.04–1.64). Conclusions The present meta-analysis suggests that the XRCC3 Thr241Met polymorphism may be a risk factor for leukemia in Caucasian population.

Background The controlling nutritional status (CONUT) score has been widely used to evaluate the nutritional and immunological status. Clinical value of postoperative CONUT (PoCONUT) score in hepatocellular carcinoma (HCC) remains unknown. This study assessed whether PoCONUT score could serve as a useful predictor of survival for patients with small HCC. Methods 547 consecutive patients with small HCC who underwent liver resection between February 2007 and December 2015 were included in this retrospective case-control study. Patients were categorized into two groups: low PoCONUT group (PoCONUT score ≤ 2, n = 382) and high PoCONUT group (PoCONUT score ≥ 3, n = 165). Propensity score matching (PSM) analysis was applied to balance the bias in baseline characteristics. A cumulative survival curve was established by the Kaplan–Meier method, and differences in OS and RFS among CONUT score groups were determined by the log rank test. Cox proportional hazard regression analysis was used to evaluate the association of PoCONUT score and overall survival (OS) and recurrence-free survival (RFS), with calculation of hazard ratios (HRs) and 95 % confidence intervals (95 % CIs). Results Cox proportional hazard regression analysis suggested that the PoCONUT score was an independent risk factor for both OS and RFS in patients with small HCC before and after PSM. Conclusions High PoCONUT score helps to predict worse OS and RFS in patients with small HCC who underwent liver resection.

While strong inhibitory control is critical for health and wellbeing, there are no broadly applicable effective behavioural interventions that enhance it. This meta-analysis examined the neurocognitive rationale for combined physical and cognitive training and synthesised the rapidly growing body of evidence examining combined paradigms to enhance inhibitory control. Across the research to date, there was a small positive effect (n studies = 16, n participants = 832) of combined training on improving inhibitory control. Sub-group analyses showed small-moderate positive effects when the physical component of the combined training was moderately intense, as opposed to low or vigorous intensities; moderate positive effects were found in older adults, as compared to adolescents and adults; and healthy individuals and those with vascular cognitive impairment, as compared to ADHD, ASD, mild cognitive impairment and cancer survivors. This is the first meta-analysis to provide evidence that combined physical, specifically when moderately intense, and cognitive training has the capacity to improve inhibitory control, particularly when delivered to healthy individuals and those experiencing age-related decline.

The Regional Greenhouse Gas Initiative (RGGI) is a cap-and-trade system targeting CO2 emissions from the electricity sector in the northeastern United States. As a major power producer and carbon emitter, Pennsylvania plans to join RGGI in 2022, which will affect both the carbon market (i.e., RGGI) and the regional electricity market (i.e., PJM). Combining a PJM power system model with a reduced-form model of CO2 emissions abatement from RGGI states that are not in PJM, we find the annual average emissions from power plants in Pennsylvania can be reduced by 40%, 79%, 68%, and 76% for CO2, SO2, NOx, and PM2.5, respectively, during 2022-2030. Then, based on a range of source-specific marginal damage estimates, we find the cumulative monetized health cobenefits to be 17.7 to 40.8 billion USD. However, the reduced emissions and health damages in Pennsylvania are slightly offset by increases in the other states in PJM that do not participate in RGGI. Our study hence highlights the potential cross-state leakage issue that warrants careful consideration in the policy design and implementation process.

Traditionally, analysis of the costs of cutting greenhouse gas emissions has assumed that governments would implement idealized, optimal policies such as uniform economy-wide carbon taxes. Yet actual policies in the real world, especially in large federal governments, are often highly heterogeneous and vary in political support and administrative capabilities within a country. While the benefits of heterogeneous action have been discussed widely for experimentation and leadership, little is known about its costs. Focusing on the United States, we represent plausible variation (by more than a factor of 3) in the stringency of state-led climate policy in a process-based integrated assessment model (GCAM-USA). For a wide array of national decarbonization targets, we find that the nationwide cost from heterogeneous subnational policies is only one-tenth higher than nationally uniform policies. Such results hinge on two critical technologies (advanced biofuels and electricity) for which inter-state trade ameliorates the economic efficiencies that might arise with heterogeneous action. The nationwide cost of cutting emissions can be affected by local policies. This study considers the differences across the US states, with integrated assessment model results showing that varying state policies only increases nationwide costs by about 10%.

Immunotherapy has been a revolutionary innovation in cancer therapy in recent years, but it is accompanied by various unique immune-related adverse events (irAEs). Among these irAEs, anaphylactic shock is very rare. Here, we report a case of a patient who developed anaphylactic shock after receiving one dose of atezolizumab. A 74-year-old male patient with small cell lung cancer experienced recurrence 10 years after surgery. After one cycle of treatment, the patient developed a grade 2 rash and recovered after receiving oral methylprednisolone tablets. In the second cycle, atezolizumab was discontinued. Then, the patient was scheduled to receive atezolizumab plus carboplatin and etoposide again after three weeks, but approximately three minutes after an intravenous infusion of atezolizumab, the patient developed signs and symptoms of anaphylactic shock, such as dyspnea, cold limbs, and loss of consciousness. At this point, the infusion was immediately stopped, and a normal saline infusion was administered. Meanwhile, ECG monitoring, supplemental humidified high-flow supplemental 100% oxygen, epinephrine, dopamine, hormone treatment with methylprednisolone, and other anti-shock treatments were carried out. For better recuperation, this patient was transferred to the intensive care unit for further treatment and was discharged two days later. Anaphylactic shock develops rapidly and is also a very severe complication. Prompt detection, diagnosis, and therapeutic intervention are the basics for survival.

We have developed a 6-kA/cm² superconducting integrated circuit fabrication process toward large-scale single flux quantum digital circuits. To evaluate the process reliability and controllability, a set of process control monitors (PCMs) for monitoring fabrication and design parameters was designed and tested. Fabrication parameters, for example, film thicknesses and etched depths of the layers, were measured by testing specific patterns during fabrication. Design parameters such as the critical current density J c, normal-state resistance R n and actual sizes of Josephson junctions, the sheet resistance of Mo resistor R sh, and the penetration depth of Nb films were evaluated by electrically characterizing PCM circuits after fabrication. We also monitored superconducting critical currents for vias and Nb lines, and insulation properties between the metal layers systematically.

We have developed a fabrication technology for superconducting integrated circuits with Nb-based Josephson junctions. The standard fabrication process with 10 mask levels includes 3 Nb superconducting layers and a Mo resistor layer. The influence of deposition parameters on film stress, electrical properties, and surface roughness were studied systematically. High quality Nb, Al, Mo, and SiO2 films were successfully deposited for the subsequent fabrication of circuits. The circuit fabrication started with the fabrication of Mo resistors with a target sheet resistance R sh of 2 Ω, followed by the deposition of Nb/Al-AlO x /Nb trilayer Josephson-junction. The target critical current density J c was set at 6 kA/cm2. Small-scale circuits such as our standard library cells have been successfully fabricated and tested, confirming the capability of our fabrication technology for superconducting integrated circuits.

Dielectric tunable devices with improved overall tunability properties are in urgent demand for tunable applications. Hence, a series of Pb-free dielectric tunable ceramics based on (Ba1−xCax) (Zr0·18Ti0.82)O3 (abbreviated as BCZTx, x = 0.05–0.21, corresponding to BCZT05 to BCZT21) were carefully prepared using the traditional solid-state route in this work. The crystal structure, surface morphology, dielectric properties, and tunable performance of different BCZTx ceramics at room temperature (RT) were systematically studied. The temperature-dependent dielectric tunable performance was further investigated. The phase evolutions were co-confirmed by XRD and d e r d T -T curves. Interesting, high tunability (86.09%), together with a relatively low dielectric loss (~0.19% @7.42 kV/cm) were obtained in BCZT09 ceramics at 1 kHz, resulting in a giant figure-of-merit (FOM) of 448, which implied these ceramics are promising matrix for dielectric tunable applications that operated at RT. From the Tunability−T and FOM−T curves, it can be found that Ca2+ incorporation can improve the temperature stability of dielectric tunable performance to a certain extent. In addition, the FOM−T curve of BCZT09 showed advantages over other compositions in the temperature range of −20 °C–85 °C, and its maximum FOM (~886) was reached at 40 °C. These observations suggest the BCZT09 ceramic as a promising matrix for application in dielectric tunable devices operating at RT. This work may guide the design of novel high-performance tunable ceramic materials.

In the process of building electrical load data collection, it is inevitable to introduce different kinds of noises, which makes the observation values deviate from the actual values, thus resulting in high levels of uncertainties. And such uncertainties make it difficult to achieve accurate point prediction of the short-term building electrical load. To improve the rationality of the prediction results and offer more effective information for decision makers, this paper proposes a novel multi-objective algorithm optimized modular fuzzy method which can accomplish the interval prediction for the short-term electrical load. First, one novel single-input-rule-modules (SIRMs)-based distributed interval fuzzy model (SIRM-DIFM) is proposed by replacing the original functional weights of the traditional SIRMs-based fuzzy inference system (SIRM-FIS) with the interval functional weights. Then, a data-driven learning scheme is presented for constructing the SIRM-DIFM. This learning sheme includes two main steps. The first step utilizes the iterative least square method to generate fuzzy rules for the SIRMs and determine the centers of the interval functional weights, while in the second step, the genetic algorithm (GA)-based multi-objective optimization algorithm is adopted to determine the widths of the interval functional weights. Through these two steps, accurate point estimation and reasonable interval prediction results can be achieved. Finally, two building electrical load prediction experiments are conducted to verify the effectiveness of the presented SIRM-DIFM. Simulation results indicate that the proposed SIRM-DIFM can compensate the shortcomings of the low accuracy of the point estimation and the predicted interval can effectively cover the observed data, providing the decision-makers more reliable and useful information.

In this article, we propose a hybrid time and wavelength division multiplexed (TWDM) active quadrature demodulation technique for fiber-optic Fabry–Perot acoustic sensor network. A modulated grating Y-branch (MG-Y) laser is utilized to achieve fast and programmable wavelength tuning. For each sensing channel, two operating wavelengths with quadrature phase difference are determined by the initial cavity length of the sensor. According to the DC voltages of the detected signal, the operating wavelength is feedback-controlled to locate on the spectral slope, maintaining the high sensitivity of the sensing system. Compared with traditional passive quadrature demodulation and quadrature point intensity demodulation methods, the feedback control logic is simple and flexible. Moreover, the proposed active quadrature demodulation scheme is easily applicable to sensors with different cavity lengths. Combined with a hybrid TWDM technology, simultaneous multi-point acoustic detection can be achieved, requiring only one tunable laser and one photodetector. A two-point sound velocity sensing system and a four-point sound source localization system are constructed. Experimental results show that different sensing channels can work effectively without crosstalk.

In order to simultaneously detect high-frequency dynamic parameters and low-frequency quasi-static parameters through spectrum acquisition, a novel frequency-scanned parallel phase-shifting interferometry is proposed in this work. A Michelson interferometer (MI) composed of a 3 × 3 coupler is used for high-frequency vibration measurement. Different from conventional 3 × 3 fiber coupler based demodulation methods, a frequency swept laser source with flat intensity output is used. The time-varying vibrations will be encoded on the phase shifted spectra measured at two return ports of the coupler. Using a modified phase shifting algorithm, the optical path difference (OPD) changes along optical frequency can be retrieved, thereby significantly improving the sampling rate of the dynamic MI. Thanks to the full-spectrum scanning, the asymmetric parameters of non-ideal 3 × 3 couplers can be directly measured. Moreover, it provides the possibility of spectral demodulation of multiple quasi-static parameters. Another sensing channel containing multiple fiber Bragg grating (FBG) sensors is used to achieve quasi-distributed temperature measurement. Experimental results demonstrate its effectiveness in dynamic/static multi-parameter measurement.

A sandwich type titled fiber Bragg grating (TFBG) plasmonic sensor based on self-assembled p-mercaptophenylboronic acid (PMBA) monolayer and Au nanoparticles (AuNPs), has been demonstrated and successfully applied for sensitive and stable low-concentration glucose detection. The sensor was immobilized with PMBA monolayer acting as the highly sensitive and stable glucose recognition molecule instead of a traditional unstable enzyme. In the experiments, the high sensitivity for glucose detection is achieved, establishing a competitive limit of detection (LOD) of 295 pM and large dynamic measurement range from 1 nM to 10 mM simultaneously. Moreover, due to the unique two-set diol configurations of glucose, the combination of TFBG-SPR with AuNPs and PMBA exhibit a good selectivity over fructose and galactose, which both possess only one set of diol configurations. Non-invasive saliva glucose detection, as a better alternative to blood, has emerged and attracted many researchers’ attention. Therefore, the practicality of this method was successfully validated by detecting glucose in a simulated human saliva sample. We believe that this method represents an appealing solution for flexible, temperature-insensitive, high-sensitivity, and high-stability detection of ultra-low concentration glucose in modern non-invasive point-of-care testing as well as tumor micro-environment (TME) related cancer research.

When the cavity changes during spectrum acquisition process, Doppler phenomena will occur and cause conventional scanning white-light interferometry (WLI) to fail. To absolutely demodulate the cavity length of the dynamic low-finesse Fabry–Perot sensor, a novel laser frequency modulated phase-shifting WLI (PS-WLI) is proposed. Two phase-shifted spectra can be obtained by introducing a fixed phase-shift after each scanning frequency. The phase results containing modulations of frequency scanning and dynamic signals are retrieved based on quadrature demodulation. Although the full-spectrum acquisition frequency of the proposed PS-WLI is only 50 Hz, 0.5∼20 kHz vibration signals can be demodulated. It combines the advantages of frequency-switched two-step phase shifting interferometry (PSI) and scanning WLI to achieve absolute measurement with high sampling rate. Numerical simulations and experiments verify its effectiveness.

Optical fiber hot-wire anemometer is an emerging technique for wind speed measurement. In many practical applications, both the magnitude and the direction of the flow need to be obtained. However, most of previously reported fiber optic anemometers show omnidirectional response, rendering difficulty in recognizing the flow direction. Here, we propose a simple and effective fiber-optic flow sensor with directional response based on an asymmetric coating of single-wall carbon nanotubes (SWCNTs) on the surface of tilted fiber Bragg grating (TFBG), paving the way for the vector analysis of wind flow field. FLUENT software was employed to analyze the wind speed field, temperature distribution field, and vortex street. The temperature field shows asymmetrical distribution directly related to the wind directions, causing directional response of the sensor to the wind flow. In the experiments, with optimized 12˚ TFBG with 1.3 μm thick asymmetric SWCNTs coating, three wind directions at different angle (0˚, 180˚, 270˚) were clearly distinguished. Moreover, high sensitivity and efficiency were also achieved by investigating and optimizing the system parameters like pumping power, coating thickness. In addition to directional response, the proposed sensor exhibits many advantages such as simple structure, low power consumption, small size, and high sensitivity, which has good potential application for infield static wind field and slowly varying wind (SVW) measurement.

Herein, we report the simultaneously application of surface plasmon resonance (SPR) spectroscopy and electrochemistry (EC) technology for the real-time quantitative investigation of heavy metal ions, with high selectivity and sensitivity. A surface of optical fiber coated over with nanometric gold film and then modified with a dithiol self-assembled monolayer of 1,6-hexanedithiol (HDT) was developed as the dual mode electrochemical-surface plasmon resonance (EC-SPR) optical fiber sensor. The limit of detection of the proposed sensor exposed in Hg2+ ions solution without and with the multiple cyclic voltammetry (CV) scans to activate the HDT monolayer can be improved from 0.47 μM to 0.22 μM. More importantly, the CV scans can activate the redox reaction of Hg2+ ions, and then easily strip the Hg2+ ions from the HDT monolayer to restore the active sites, and endow the designed sensor the characteristics of repeated use. Under simultaneous SPR monitoring, the differential pulse voltammetry (DPV) currents of HDT-functionalized optical fiber can also act as a microelectrode to develop electrochemical sensor platform for monitoring Cu2+, with a limit of detection of 94 nM and a dynamic range up to 90 μM. As a comparison, the EC-SPR sensor still has good analysis performance for both Hg2+ and Cu2+ ions in presence of Ni2+, Fe2+, Co2+, Pb2+, K+, among others. We therefore demonstrated that the proposed fiber-based EC-SPR sensor has the advantages of real-time, miniaturized, and cost-effective sensing, paving the way for environmental monitoring applications for on site inspection.

Introduction Home noninvasive positive pressure ventilation (NIPPV) has become a well-established treatment for stable hypercapnic chronic obstructive pulmonary disease (COPD) patients. There are still other challenges including appropriate titration of ventilator parameters, adequacy of follow-up, monitoring and management at home to ensure effectiveness and security, and to improve quality of life. The Internet of Things (IoT) is the name given to the network of devices and other "things" with built-in sensors, software, electronics, and network connectivity, which can communicate these objects over wireless networks and then send data to a cloud platform. Reliable tele-monitoring and transmission of clinical parameters from home to hospitals has prompted the development of IoT-based home NIPPV. Objectives This review provides an overview of titration and follow-up of home NIPPV and focus on different technologies, modalities, management, and cost-effectiveness used in IoT-based tele-monitoring of home mechanical ventilation. Data source Literature search of Web of Science, PubMed and EMBASE was made to find relevant articles about tele-monitoring and the Internet of things in home mechanical ventilation over the last 15 years. We used the following search terms: NIPPV, COPD, home mechanical ventilation, telemedicine, tele-monitoring and management. Conclusion IoT-based management of home NIPPV, such as home titration and follow-up with the use of tele-monitoring, are emerging and yielding positive findings. However, clear conclusions based on RCT of tele-monitoring in COPD patients with NIPPV at home are only a few and large-scale multicenter studies are required for replication and further validation.

Optical wireless communication (OWC) technology has attracted many attentions recently. Lighting luminaries equipped with red-green-blue (RGB) based light-emitting-diodes (LEDs) or laser-diode (LDs) can modify the perceived color of illumination. These highly directional light sources with adjustable perceived colors makes them very suitable for high speed and high security data transmissions. Here, we propose and demonstrate a two-level LD based color-shift-keying orthogonal-frequency- division-multiplexing (CSK OFDM) signal for the OWC systems. This scheme cannot only provide non-flickering effect to minimize human health concern, but also can enhance the transmission data rate of the CSK system. Here, the system model and the implementation of bit and power loading algorithm on the LD-CSK-OFDM are discussed. Experimentally results show that the demonstrated LD-CSK-OFDM signal can achieve data rate of 28.4 Gbit/s with 1.25 m free-space transmission distance satisfying the forward-error-correction (FEC) threshold.

Introduction Peritoneal carcinomatosis is difficult to treat. Pressurized Intra-Peritoneal Aerosolised Chemotherapy (PIPAC) is a novel method of delivering chemotherapy to the peritoneal cavity, aiming for homogenous and deeper drug distribution. To date, limited chemotherapeutics have been used with promising results. Here, we evaluate the pharmacokinetics, peritoneal tissue drug concentration, penetration, and short-term safety of PIPAC using solvent-based paclitaxel in swine to guide clinical trials. Materials and methods PIPAC solvent-based paclitaxel was administered at 60, 30, and 15mg/m2 for 3 cohorts. Each PIPAC procedure was followed by intravenous (IV) administration of the same dose of solvent-based paclitaxel on Day 7, serving as control for pharmacokinetic comparison in the same pig. Safety and toxicity were evaluated by clinical assessment, blood counts and biochemistry. Blood samples were taken for pharmacokinetic analysis. Peritoneal biopsies were taken to measure tissue paclitaxel concentrations and distribution. Results 12 Yorkshire x Landrace pigs underwent trial procedures. With PIPAC, there was linear pharmacokinetics and lower systemic exposure to paclitaxel compared to IV administration. MALDI-MSI demonstrated concentration of paclitaxel at the peritoneal surface, with estimated 2 mm penetration. PIPAC paclitaxel had favorable toxicity profile. The most significant adverse event was neutropenia which was dose dependent, with absolute neutrophil count Conclusion PIPAC paclitaxel at 15mg/m2 may be considered for a Phase I study.

Recent advancements in cancer biology, microbiology, and bioengineering have spurred the development of engineered live biotherapeutics for targeted cancer therapy. In particular, natural tumor-targeting and probiotic bacteria have been engineered for controlled and sustained delivery of anticancer agents into the tumor microenvironment (TME). Here, we review the latest advancements in the development of engineered bacteria for cancer therapy and additional engineering strategies to potentiate the delivery of therapeutic payloads. We also explore the use of combination therapies comprising both engineered bacteria and conventional anticancer therapies for addressing intratumor heterogeneity. Finally, we discuss prospects for the development and clinical translation of engineered bacteria for cancer prevention and treatment.

Multimodal sentiment analysis has increasingly attracted attention since with the arrival of complementary data streams, it has great potential to improve and go beyond unimodal sentiment analysis. In this article, we present an efficient separable multimodal learning method to deal with the tasks with modality missing issue. In this method, the multimodal tensor is utilized to guide the evolution of each separated modality representation. To save the computational expense, Tucker decomposition is introduced, which leads to a general extension of the low-rank tensor fusion method with more modality interactions. The method, in turn, enhances our modality distillation processing. Comprehensive experiments on three popular multimodal sentiment analysis datasets, CMU-MOSI, POM, and IEMOCAP, show a superior performance especially when only partial modalities are available.

Large benign and malignant tumors in the scalp cannot be sutured directly after resection. Instead, skin grafting or skin flap repair is the most commonly used techniques. Local tissue depression and lack of hair growth are some of the drawbacks associated with these techniques. The use of a modified local flap (the O-Z flap) may effectively overcome these issues.To explore the application of O-Z flap in wound repair after excision of benign and malignant tumors of the scalp.Between April 2016 and November 2017, the authors treated 6 patients with scalp tumors. They underwent round or oval radical tumor resection with negative margins. Tumor specimens were diagnosed by cryosection during operation. According to the wound defect size and location, surrounding scalp looseness, and hair distribution, 2 rotating flaps in opposite directions were formed on the left and right sides or front and back of the wound. Subsequently, the skin flaps were rotated in opposite directions to repair the wound.The scalp tumors comprised 2 cases of basal cell carcinoma, 2 cases of squamous cell carcinoma, 1 case of hair sheath carcinoma, and 1 case of epidermoid cyst. After complete tumor resection, the wound defect area was between 3.0 cm × 3.5 cm and 5.0 cm × 6.0 cm. After operation, approximately 6% of the tip of the skin flap was necrotized. The wounds healed after 4 weeks of dressing treatment. All skin flaps survived in stage I and no complications occurred. All patients were followed up for 3 to 12 months; the scalps were in good condition and no tumor recurrence was found.The use of the O-Z flap to repair scalp wounds offers flexible design, good blood circulation, uniform tension, and good hair growth after operation; thus, this technique is suitable for wound repair following scalp tumor resection.

The silicon-controlled rectifier (SCR) has been reported to protect CMOS integrated circuits (ICs), due to high ESD robustness within a small silicon area. However, the holding voltage ( ${V}_{h}{)}$ of the SCR device was too low to suffer the latch-up issue. Thus, the ${V}_{h}$ value of the SCR device must be improved to be greater than the circuit operating voltage for safe applications. In this work, the Schottky-embedded modified lateral SCR (SMLSCR) with high holding voltage for ESD protection was proposed and verified in a 0.18- $\boldsymbol \mu \text{m}$ 1.8-V/3.3-V CMOS process. By using the Schottky barrier junction, the ${V}_{h}$ value of the SCR device can be improved by the reverse-bias Schottky barrier diode (SBD) that is embedded into the SCR device structure. Among those experimental results on the SMLSCR devices with split layout parameters in the silicon test chip, the SMLSCR device without $\text{P}^{+}$ guard ring has the best second breakdown current ( ${I}_{{t{2}}}{)}$ of 3.1 A and a high ${V}_{h}$ value of 9.7 V.

Spin-orbit torques (SOTs) from transition metal dichalcogenides systems (TMDs) in conjunction with ferromagnetic materials are recently attractive in spintronics for their versatile features. However, most of the previously studied crystalline TMDs are prepared by mechanical exfoliation, which limits their potentials for industrial applications. Here we show that amorphous WTe2 heterostructures deposited by magnetron sputtering possess a sizable damping-like SOT efficiency {\xi}_DL^WTe2 ~ 0.20 and low damping constant {\alpha} = 0.009/pm0.001. Only an extremely low critical switching current density J_c ~ 7.05\times10^9 A/m^2 is required to achieve SOT-driven magnetization switching. The SOT efficiency is further proved to depend on the W and Te relative compositions in the co-sputtered W_100-xTe_x samples, from which a sign change of {\xi}_DL^WTe2 is observed. Besides, the electronic transport in amorphous WTe2 is found to be semiconducting and is governed by a hopping mechanism. With the above advantages and rich tunability, amorphous and semiconducting WTe2 serves as a unique SOT source for future spintronics applications.

Background Immunotherapy targeting PD-1 provides a limited survival benefit in patients with unresectable advanced or recurrent gastric cancer (GC). Beside PD-L1, the expression of inhibitory ligands such as CEACAM-1 and LSECtin on GC cells account for this limitation. Here we assessed their expression and immune suppressive effect in GC patients. Methods Using multiplexed immunohistochemistry staining, we evaluated the distribution of different inhibitory ligands, including PD-L1, CEACAM-1, LSECtin, and MHC class II, in 365 GC patients. We analyzed their correlations and overall survival (OS) based on the expression of each inhibitory ligand and the independent prognostic factors that affect OS. Subsequently, we evaluated the additive effect of anti-PD-1 mAb or anti-PD-L1 mAb with/without anti-Lag-3 mAb with/without anti-Tim-3 mAb in cytotoxic assay using tumor-antigen specific CTL clones against GC cell lines. Results Co-expression of the inhibitory ligands for PD-1, Tim-3, and Lag-3 was observed in the largest proportion (34.7%). CEACAM-1, LSECtin, and MHC class II expression showed significant correlation with PD-L1 expression and OS. Multivariable analysis demonstrated that CEACAM-1 low is an independent prognostic factor. Furthermore, combining dual and triple ICIs yielded additive effect on cytotoxicity of CTL clones against each immune inhibitory ligand positive GC cell lines. Conclusions Our findings suggested that the expression of inhibitory ligands for Tim-3 and Lag-3 on GC cells serve as potential biomarkers to predict the response to anti-PD-1 therapy and the combinatorial immunotherapy with ICIs targeting for PD-1, Tim-3, and Lag-3 has a therapeutic potential for GC patients.

To study the efficacy of breast nodule lysis (BNL) for the treatment of mammary gland hyperplasia (MGH). A total of 304 patients with MGH and breast pain were randomly divided into a BNL group and an oral tamoxifen (OT) group. In the BNL group (n = 156), patients were treated with BNL technique once a day for 7 consecutive days. BNL is a novel technique developed by our department, by which breast nodules can be released and breast duct can be cleared via external force and manipulation. In the OT group (n = 148), patients took oral tamoxifen 10 mg once a day for 15 consecutive days beginning 5 days after termination of the menstrual period for 3 menstrual cycles. The McGill Pain Questionnaire (MPQ) score, number and size of breast nodules pre- and post-treatment, efficacy, time to response, total treatment expenses, recurrence rate, and side effects were compared across groups. The MPQ score was significantly better in the BNL than the OT on day 7, and the total effective rate (86.54%) was significantly higher in the BNL than the OT (43.92%) on day 15. The number and size of breast nodules were decreased in the BNL compared with the OT 15 days, 3 months, and 6 months after treatment. The total treatment expenses were higher in the BNL than the OT. The recurrence rate within 6 months was lower in the BNL than the OT. There was no difference in side effects (nausea, vomiting, diarrhea, or dizziness) across groups. BNL can effectively alleviate pain and reduce the number and size of nodules with quick effect and a low recurrence rate, making it an enticing treatment for MGH.

In this article we demonstrated a novel interference behavior excited by visible light in the capillary waveguide (CWG). We investigated the sensing principle of CWG with filling liquid and optical characteristics of interference systematically. This CWG-based sensing element is theoretically verified to possess continuously tunable thermal sensitivity by changing the thermo-optic coefficient of the filling liquid. As expected, experimental results show that the proposed sensor possesses sharp interference fringes and the tunable sensitivity of temperature. Unlike most optical fiber interferometers, the optical path difference of our sensing element is generated in the wall of CWG instead of air cavity. Besides, interference occurs in the visible light band, which greatly improves dynamic range and reduces the cost of system setup. The novel optical behavior we investigated offers a way to the utility of the waveguide as an optical interferometer.

Gastric cancer cases are often diagnosed at an advanced stage with poor prognosis. Platinum-based chemotherapy has been internationally accepted as first-line therapy for inoperable or metastatic gastric cancer. To achieve greater benefits, selection of patients eligible for this treatment is critical. Although gene expression profiling has been widely used as a genomic classifier to identify molecular subtypes of gastric cancer and to stratify patients for different chemotherapy regimens, its prediction accuracy can be improved. Adenosine-to-inosine (A-to-I) RNA editing has emerged as a new player contributing to gastric cancer development and progression, offering potential clinical utility for diagnosis and treatment. Using a systematic computational approach followed by both in vitro validations and in silico validations in The Cancer Genome Atlas (TCGA), we conducted a transcriptome-wide RNA editing analysis of a cohort of 104 patients with advanced gastric cancer and identified an RNA editing (GCRE) signature to guide gastric cancer chemotherapy. RNA editing events stood as a prognostic and predictive biomarker in advanced gastric cancer. A GCRE score based on the GCRE signature consisted of 50 editing sites associated with 29 genes, predicting response to chemotherapy with a high accuracy (84%). Of note, patients demonstrating higher editing levels of this panel of sites presented a better overall response. Consistently, gastric cancer cell lines with higher editing levels showed higher chemosensitivity. Applying the GCRE score on TCGA dataset confirmed that responders had significantly higher levels of editing in advanced gastric cancer. Overall, this newly defined GCRE signature reliably stratifies patients with advanced gastric cancer and predicts response from chemotherapy. Significance: This study describes a novel A-to-I RNA editing signature as a prognostic and predictive biomarker in advanced gastric cancer, providing a new tool to improve patient stratification and response to therapy.
