The article provides a practical example of the positive experience of the students of the medical academy in the international social project on combating arterial hypertension in order to improve the level of general competence and effective intellectual and professional socialization in getting higher medical education. The phenomenon of professional socialization should be considered along with the concepts of professional orientation, professional development, professional adaptation and professional becoming. Professional socialization is the ultimate link in the professional way of the individual. The algorithm of professional socialization of students in higher education consists of the following stages: professional orientation → professional development → professional adaptation → professional becoming → professional socialization. It should be noted that a personality who successfully socializes, has a well-defined social orientation, actively strives to self-determination and self-realization in society and is able of productive adaptation to the surrounding society on the basis of knowledge about social reality. In this case, the success of socialization is connected not only with the presence of specified qualities in the person, but, first of all, with the real actions in which these qualities are realized. By mastering the roles system, students acquire socially significant qualities, develop their worldview, their goals, motives, interests, feelings, personal and socially important needs, develop different types of competencies, including social competence, which will allow them to become what they want to become. Professional development of young talented individuals is possible only in open communication and interaction systems, it is in the international social projects of medical societies that constantly exchange energy and information with the environment, and at the same time, being a source of development and prospects for preserving the life of nation. Such a model of education supposes changing the role of teacher: the transition to joint actions with the student in new situations in an open, changing, modern world.

The article considers risk factors for the development of cognitive disorders, the most powerful of which is age. The role of the cardiovascular system diseases in the formation of cognitive impairments, in particular, the development of changes in the cognitive sphere in arterial hypertension, is reflected. The main causes of the appearance and clinical variants of the manifestation of cognitive impairment in patients with arterial hypertension are highlighted. Attention is paid to morpho-functional changes in the brain, large and small cerebral vessels in the increased blood pressure and their relationship to cognitive impairments. The indications to the study of the cognitive sphere and the variants of diagnosis of cognitive impairments in patients with arterial hypertension are reflected.

The article deals with topical issues of arterial hypertension, as an interdisciplinary problem, maintaining the quality of life and the level of cognitive functions in such patients. The program of care for patients with hypertension has gained interdisciplinary importance. Of primary importanceis the problem of the lack of real knowledge and practicals kills for early diagnosis of CI in the stage of primary care of general practitioners – family medicine, therapists – those who see the patient first and have the opportunity to observe the dynamics of the cardiovascular disease. The necessity of early detection of cognitive impairment in patients with arterial hypertension is grounded, the option of correcting cognitive impairment using the combined nootropic drug Cognifen is proposed.All patients with hypertension should pay close attention to the state of higher mental functions. The management of patients with hypertension in all age groups with CI should be based on achieving adequate control of blood pressure and as soon as possible include adequate neuroprotective therapy.

By the WHO data,Ukraineis country number1 inEuropeand number2 inthe world according to the cardiovascular mortality rate. In the structure of the prevalence of cardiovascular diseases in Ukraine, the percentage of persons of working age with arterial hypertension is 36.7%. The improvement of the prognosis of patients with arterial hypertension depends not only on long-term effective control of blood pressure, but the diagnosis and initiation of therapy at the initial stage of the development of the disease, when the target organs have not yet undergone hypertensive damage is also an important condition. The article presents the results of neuropsychological examination of 509 outpatients with a controlled second stage arterial hypertension, with an integrated assessment of cognitive functions using the Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MCA) methods. It has been established that even in patients with controlled arterial hypertension stage II with a low cardiovascular risk, by SCORE scale cognitive disorders were revealed in 32,2%. According to the survey of 68 patients with arterial hypertension stage II, 1-2 degrees, controlled on the background of antihypertensive therapy , cardiovascular risk, severity of cognitive impairment were analyzed. Instrumental examination of ambulatory blood pressure monitoring; electrocardiography , ultrasound investigation of the main arteries of the head and neck was performed . Neuropsychological examination: an integrated assessment of cognitive functions by MMSE scale; m emory evaluation using the procedure of memorizing "Saving 10 words" by Luria; rating of attention and speed of sensorimotor reactions by Schulte and Rybakov procedure was made . Total cardiovascular risk was defined on a scale «SCORE». T ypical cognitive disorders in patients with hypertension of working age, even i n the controlled blood pressure level were found. Presence of verbal memory disorders associated with hypertension was revealed . The importance of determining the overall cardiovascular risk, diagnosis of cognitive impairment and measur ing thickness of the intima-media of the arterial wall as factors of brain lesion which is the target organ in arterial hypertension of 1-2 degrees , was proved .

The activity of gelatinases A and B in blood plasma of rats with doxorubicin-induced cardiopathy at the application of cyclooxygenases inhibitors – ketorolak, lornoxycam and celecoxib was investigated. Significant increase of activity of both gelatinases in rats with cardiopathy was identified. Under ketorolak effect gelatinase B activity increases, and gelatinase A is inhibi­ted. The opposite reaction was observed when celecoxib was applied: gelatinase B acti­vity decreases, and gelatinase A increases. Combination of bioflavonoid quercetin and COX inhibitors decreased the activity of both latent forms of gelatinases in blood plasma of rats with cardiopathy. It is concluded that the stabilization of gelatinases activity is an indicator of myocardial metabolism and additional indicator of fibrosis development.
