ZusammenfassungDas Ziel der vorliegenden Übersichtsarbeit ist die systematische Darstellung der aktuellen Evidenz zur Wirksamkeit von Acamprosat, Naltrexon, Disulfiram und Nalmefen für die Aufrechterhaltung der Abstinenz oder Trinkmengenreduktion bei alkoholabhängigen Patient:innen. Bezogen auf den Zeitraum 2005 bis 2020 wurden insgesamt 27 Studien identifiziert, die alle Einschlusskriterien (u. a. RCT, mind. 8 Wochen Behandlungsdauer) erfüllten. Es wurden überwiegend Hinweise zur Wirksamkeit von Nalmefen gefunden. Acamprosat war nicht (zusätzlich) wirksam. Naltrexon trug in kombinierten Interventionen einen zusätzlichen Nutzen bei, dies galt jedoch nicht für die Mehrzahl der Studien. Disulfiram war nicht wirksam darin, einen zusätzlichen Nutzen zur Erhaltung der Abstinenz beizutragen, im Vergleich zu Topiramat, einem Antikonvulsivum, jedoch effektiver. Die Ergebnisse stehen nicht im Einklang mit dem bisher bekannten Forschungsstand, der Acamprosat, Naltrexon und Nalmefen als überwiegend effektiv und sicher einstuft und Disulfiram als mäßig wirksam. Der pharmakologische Interventionsbedarf sollte bei Bestehen alkoholbezogener Probleme exploriert und ggf. erwogen werden, damit diesbezüglich Vorbehalte abgebaut und ein umfassendes und zugleich individuelles Behandlungsangebot geschaffen wird. Hinsichtlich der Trinkmengenreduktion sollten Aspekte der Schadensminderung als Behandlungsziel berücksichtigt werden.

Zusammenfassung Ziel der Studie Für die erfolgreiche Implementierung von Leitlinien sind kontextspezifische Umsetzungsstrategien, welche die spezifische Versorgungssituation sowie mögliche Umsetzungsbarrieren berücksichtigen, vielversprechend. Ziel dieser Untersuchung war es, mögliche Barrieren und präferierte Umsetzungsstrategien für die Implementierung der S3-Leitlinie „Screening, Diagnose und Behandlung alkoholbezogener Störungen“ aus der Perspektive von Behandlern und Betroffenen zu erfassen. Methodik Als Teil des vom Bundesministerium für Gesundheit geförderten Projekts Implementierung und Evaluation der S3-Leitlinie zu Screening, Diagnose und Behandlung alkoholbezogener Störungen (IMPELA) wurden Barrieren und mögliche Strategien für die Umsetzung der Leitlinienempfehlungen mithilfe eines selbst entwickelten Fragebogens querschnittlich erhoben. Sowohl Gesundheitsfachpersonal aus Primärversorgung und Suchthilfe (Behandler) als auch Betroffene von alkoholbezogenen Störungen und deren Angehörige in der Modellregion Bundesland Bremen wurden mithilfe unterschiedlicher Rekrutierungsstrategien (postalisch, persönlich, über ausgelegte Flyer) zur Teilnahme eingeladen. Der Fragebogen konnte in Papierversion oder online bearbeitet werden. Zusätzlich wurde der Fragebogen online über relevante Fachgesellschaften und Patientenorganisationen mithilfe von E-Mailverteilern bundesweit verbreitet. Ergebnisse Datensätze von 263 Behandlern wurden in die Auswertung eingeschlossen, davon 163 (62%) Behandler aus Bremen und 100 (38%) aus anderen Bundesländern. Die Behandler waren im Mittel zwischen 51,7 (Bremen) und 50,9 (Bundesweit) Jahre alt und hatten 22,8 (Bremen) bzw. 23,7 (Bundesweit) Jahre Berufserfahrung. Die Mehrheit der Befragten waren Ärzte (62,6% Bremen; 50% Bundesweit). Als Hauptbarrieren empfanden die Behandler das mangelnde Wissen über die Existenz der Leitlinie (65,6% Bremen; 44% Bundesweit) und die geringen zeitlichen Ressourcen im Behandlungsalltag (68,7% Bremen; 50% Bundesweit). Als mögliche Umsetzungsstrategien wurden u. a. die stärkere Information der Fachöffentlichkeit (33,7% Bremen; 39% Bundesweit) sowie Fact-Sheets (34,9% Bremen; 38% Bundesweit) genannt. Zusätzlich konnten Daten von 94 Betroffenen/Angehörigen ausgewertet werden (57,7% männlich; Alter M=44,7 Jahre). Von diesen gaben 51 Personen (54,3%) an, bereits selbst eine Suchtbehandlung in Anspruch genommen zu haben. Als Schwierigkeiten für die Versorgung benannten die Betroffenen/Angehörigen am häufigsten, dass Betroffene aus Angst vor Konsequenzen keine Hilfe suchen (n=51; 54%) oder ihre Probleme nicht wahrhaben wollen (n=49, 52,1%), sowie zu lange Wartezeiten auf Behandlungsplätze (n=45; 47,8%). Verbesserungspotential sahen die Betroffenen v. a. in den Bereichen Screening („Ansprechen des Alkoholkonsums“; n=44; 46,8%) und einem nahtlosen Übergang zwischen verschiedenen Behandlungssettings (n=42; 44,6%). Schlussfolgerungen Zusätzlich zu den bisherigen Bemühungen zur Disseminierung der S3-Leitlinie Screening, Diagnose und Behandlung alkoholbezogener Störungen legen die Ergebnisse dieser Studie die Notwendigkeit einer kontextspezifischen Förderung der Leitlinienumsetzung nahe.

Zusammenfassung Ziel der Studie Die weltweit effektivste Behandlungsform der Opioidabhängigkeit ist die Substitutionsbehandlung mit Opioiden (Opioidsubstitutionstherapie – OST). Diese Therapieform ist auch in Deutschland etabliert. Jedoch bestehen Versorgungslücken, v. a. in ländlichen Gebieten und einzelnen Bundesländern, was auf eine abnehmende Anzahl substituierender ÄrztInnen zurückzuführen ist. Mit der 3. Verordnung zur Änderung der Betäubungsmittel-Verschreibungsverordnung (3. BtMVVÄndV) in 2017 wurden ärztlich-therapeutische Aufgaben der OST in die Richtlinienkompetenz der Bundesärztekammer überführt. Die umfassende Reform der Rahmenbedingungen der OST führt zu einer stärkeren Rechtssicherheit dieser Behandlungsform. Die vorliegende Studie zielt darauf ab, die Auswirkungen der 3. BtMVVÄndV aus der Behandelndenperspektive zu analysieren. Methode Ein Fragebogen zu individuellen Erfahrungen mit den Veränderungen durch die 3. BtMVVÄndV wurde zwischen August und Dezember 2019 an 2503 substituierende ÄrztInnen in Deutschland sowie an 563 nicht oder länger nicht mehr substituierende ÄrztInnen in Hamburg, Bayern, Nordrhein-Westfalen sowie Sachsen verschickt. Die Versendung erfolgte über die Bundesopiumstelle sowie die Kassenärztlichen Vereinigungen der ausgewählten Bundesländer. Die Auswertung differenziert zwischen substituierenden ÄrztInnen mit und ohne suchtmedizinische Weiterbildung sowie zwischen städtischen und ländlichen Kreisen. Ergebnisse Die Rücklaufquote substituierender ÄrztInnen lag bei 34,1%. Das Durchschnittsalter betrug 57,9 (± 8,7) Jahre und 64,5% waren männlich. Aus substitutionsärztlicher Sicht waren die relevantesten Änderungen der 3. BtMVVÄndV keine zeitliche Vorgabe für das Erreichen einer Opioidabstinenz (85,3%), die neue Bewertung und Behandlung des Konsums weiterer psychotroper Substanzen (71,0%), die Möglichkeit, die Take-Home-Verordnung auf 30 Tage auszudehnen (70,0%) sowie die größere Rechtssicherheit (66,2%). Die Ausweitung der Konsiliarbehandlung auf 10 PatientInnen erfuhr mit 14,8% wenig Zustimmung. 36,7% sah die Versorgung substituierter PatientInnen weder aktuell noch zukünftig gesichert. Schlussfolgerung Die Neuerungen durch die 3. BtMVVÄndV wurden positiv aufgenommen und hinsichtlich der erhöhten Rechtssicherheit und therapeutischen Freiheiten als relevant eingestuft. Informationsbedarf besteht in ländlichen Gebieten, bei substituierenden ÄrztInnen ohne suchtmedizinische Weiterbildung sowie bei ÄrztInnen, die in der Vergangenheit substituierten, es aktuell jedoch nicht mehr tun.

Zusammenfassung Ziel Erfassung der Häufigkeit, mit der zentral in die Gesundheitsversorgung der Zielgruppe eingebundene Berufsgruppen auf problematischen Alkoholkonsum screenen. Die S3-Leitlinie „Screening, Diagnose und Behandlung alkoholbezogener Störungen“ empfiehlt „allen Patient/innen in allen Settings“ ein fragebogenbasiertes Screening anzubieten. Methodik Querschnittsstudie zur Umsetzungshäufigkeit von Screenings auf problematischen Alkoholkonsum durch Fachärzt/innen (Allgemein- und Innere Medizin, Gynäkologie, Psychiatrie und Psychotherapie, Pädiatrie, Kinder- und Jugend- Psychiatrie), Psychotherapeut/innen, Sozialarbeiter/innen und Hebammen/Geburtshelfer in der Modellregion Bremen. Mittels logistischer Regression wurden behandler/innenseitige Einflussfaktoren auf die Umsetzung von Screenings exploriert. Ergebnisse Mit Rücklaufquoten von etwa 20% berichteten die verschiedenen Berufsgruppen, durchschnittlich bei 6,9% aller behandelten Patient/innen innerhalb der zurückliegenden 4 Wochen ein fragebogenbasiertes Alkoholscreening durchgeführt zu haben. Meist wurden eigene Formulierungen anstatt empfohlener Instrumente (AUDIT, AUDIT-C) genutzt. Höhere Screeningraten wurden für Patient/innen mit neu diagnostiziertem Bluthochdruck (21,2%), alkoholassoziierten Folgeerkrankungen (43,3%) und psychischen Störungen (39,3%) berichtet. Die Kenntnis der S3-Leitlinie war mit einer häufigeren Umsetzung von Screenings verbunden (OR=4,67; 95% KI 1,94–11,25, p Schlussfolgerungen Das von der Leitlinie empfohlene flächendeckende, fragebogenbasierte Screening auf problematischen Alkoholkonsum ist von einer praktischen Umsetzung weit entfernt. Maßnahmen zu einer breiteren Bekanntmachung der S3-Leitlinie erscheinen sinnvoll, um eine häufigere Anwendung von Alkoholscreenings im Gesundheitswesen zu fördern.

Since 2015 slow-release oral morphine (SROM) is approved for opioid substitution treatment (OST) in Germany. The SROMOS study (efficacy and tolerability of slow-release oral morphine in opioid substitution treatment) evaluates the efficacy and safety of SROM in routine care. This article describes the switching process from racemic methadone, levomethadone and buprenorphine to SROM. Between July 2016 and November 2017 180 patients in 23 study centers in Germany were included in the prospective, non-interventional, naturalistic observational study. Patients were already in OST and switched from a previous medication to SROM. The switching process was analyzed during a period of fourteen days. Data were available for 169 participants. The switching process had a different progression depending on premedication and pre dosage. On the fourteenth day of SROM treatment patients switched from racemic methadone took an average dosage of 922.2 mg/day, from levomethadone 801.0 mg/day and from buprenorphine 626.7 mg/day. Average conversion ratio racemic methadone to SROM was 1:11.8, levomethadone to SROM 1:17.4 and buprenorphine to SROM 1:58.0. This study provides the first data on the switching process from buprenorphine to SROM. Average dose ratio racemic methadone to SROM on the fourteenth day of treatment was considerably higher than recommended in the prescribing information.

Introduction: Due to their euphoric and sedative effects, opioid analgesics have high potential for abuse and dependence. In the last decade in the USA and many Western European Countries the prescription rates of opioid analgesics have steadily increased. Objective: This study describes 5-year trends in the prescription of opioid analgesics and risk indicators such as duration, dose and ‘doctor shopping’ in Northern Germany. Methods: The annual rates of opioid analgesic prescriptions have been analysed for patients with statutory health insurance processed by the North German Pharmacy Data Center for the years 2011–2015. A distinction was made between non-cancer patients and cancer patients, and different groups according to prescription duration and dose level. Results: Between 2011 and 2015, the annual number of patients with opioid analgesic prescriptions increased from 500,000 to 550,000. About half of non-cancer-patients (85% of the total sample) and cancer patients received opioid analgesics for 90 days or less. The rates for long-term prescriptions (⩾9 months) ranged between 6–7% for non-cancer patients and 7–8% for cancer patients. Between 1.2–1.8% received opioid analgesics in doses of more than two defined daily doses. The majority of non-cancer patients with opioid analgesic prescriptions were female. The average age of non-cancer patients was 66 years. About 80% of non-cancer patients with first opioid analgesic prescription received World Health Organization step II medication. Conclusion: For the first time, this study provides comprehensive patient-related analyses of opioid analgesic prescriptions in Germany over a 5-year period. Despite a slight increase in the overall number of opioid analgesic prescriptions, an epidemic spread of opioid analgesics cannot be observed.

Background Identifying and addressing heavy drinking represents a major public health priority worldwide. Whilst the majority of alcohol screening and brief intervention (ASBI) research has been conducted in western, high-income countries, evidence is growing that ASBI can also impact positively on heavy drinkers in low- and middle-income country populations. This mixed methods study aims to assess the feasibility of conducting a fully randomised controlled trial of the effectiveness of ASBI in primary care in Kazakhstan and explore the feasibility and acceptability of implementing ASBI in this setting from patients’ and physicians’ perspectives. Methods Six primary health care units in the region of Pavlodar will be cluster randomised to either an intervention (WHO manualised 5 min alcohol brief intervention plus alcohol leaflet) or control group (simple feedback plus alcohol leaflet). Primary feasibility measures will be rates of participation at baseline and retention of eligible patients at the 3-month follow-up point. Patient/physician questionnaires and physician focus groups will assess additional dimensions of feasibility, as well as acceptability, according to the RE-AIM framework: Reach (rates of eligible patients screened/received advice); Effectiveness (change in AUDIT-C score); Adoption (rate/representativeness of participating physicians); Implementation (quality of ASBI/barriers and facilitators to delivery); and Maintenance (potential sustainability of intervention). Discussion This is the first trial of the feasibility and acceptability of ASBI in Kazakhstan. As the planning and assessment of implementation determinants is based on the RE-AIM framework, the project outcomes will be relevant for the future development, tailoring and implementation of ASBI in Kazakhstan. Riskanter Alkoholkonsum und schwere Alkoholkonsumstorungen sollten wegen gesundheitlichen Folgeschaden rechtzeitig erkannt werden. Die S3-Leitlinie „Screening, Diagnose und Behandlung alkoholbezogener Storungen“ empfiehlt daher die routinemasige Anwendung von Screeningverfahren, z. B. in Allgemeinarztpraxen. Wird ein riskanter Konsum festgestellt, soll eine Kurzintervention erfolgen. Bei schwerer Alkoholkonsumstorung ist eine Entzugsbehandlung angezeigt, bei Komplikationsrisiken in stationarer Form. Schatzung des Anteils von Personen mit riskantem Konsum und schwerer Alkoholkonsumstorung in leitlinienkonformer Versorgung im Bundesland Bremen. Die Pravalenz riskanten Konsums (weibl.: ≥12 g/Tag; mannl.: ≥24 g/Tag) und schwerer Alkoholkonsumstorungen (weibl.: ≥60 g/Tag; mannl.: ≥90 g/Tag) wurde uber den Pro-Kopf-Konsum von Reinalkohol geschatzt. Behandlungsraten wurden auf Grundlage von Umfragedaten (fur riskanten Konsum) sowie mittels Sekundardaten vollstationarer Krankenhausaufenthalte (fur schwere Alkoholkonsumstorung) geschatzt. Die Schatzungen beziehen sich auf die Bevolkerung ab 15 Jahre im Bundesland Bremen im Jahr 2016. Nur 2,9 % aller Personen mit riskantem Alkoholkonsum wurden durch ihre Hausarzt*innen gescreent; die Halfte davon (1,4 %) erhielt eine Kurzintervention. 7,1 % der Personen mit einer schweren Alkoholkonsumstorung wurden vollstationar behandelt. Stationare Entzugsbehandlungen wurden bei 14,1 % der Personen mit einer schweren Alkoholkonsumstorung durchgefuhrt, bei denen ein stationarer Entzug erforderlich ist. Unterdurchschnittliche Behandlungsraten wurden bei 21- bis 39-Jahrigen registriert. Riskanter Konsum und schwere Alkoholkonsumstorungen sind in Bremen unzureichend versorgt, insbesondere bei 21- bis 39-Jahrigen.

Abstract. Background: The German Guideline on Screening, Diagnosis and Treatment of Alcohol Use Disorders aims to increase the uptake of evidence-based interventions for the early identification, diagnosis, prevention and treatment of alcohol-related disorders in relevant healthcare settings. To date, dissemination has not been accompanied by a guideline implementation strategy. The aim of this study is to develop tailored guideline implementation strategies and to field-test these in relevant medical and psycho-social settings in the city of Bremen, Germany. Methods: The study will conduct an impact and needs assessment of healthcare provision for alcohol use orders in Bremen, drawing on a range of secondary and primary data to: evaluate existing healthcare services; model the potential impact of improved care on public health outcomes; and identify potential barriers and facilitators to implementing evidence-based guidelines. Community advisory boards will be established for the selection of single-component or multi-faceted guideline implementation strategies. The tailoring approach considers guideline, provider and organizational factors shaping implementation. In field tests quality outcome indicators of the delivery of evidence-based interventions will be evaluated accompanied by a process evaluation to examine patient, provider and organizational factors. Outlook: This project will support the translation of guideline recommendations for the identification, prevention and treatment of AUD in routine practice and therefore contributes to the reduction of alcohol-related burden in Germany. The project is running since October 2017 and will provide its main outcomes by end of 2020. Project results will be published in scientific journals and presented at national and international conferences.

Zusammenfassung Hintergrund Opioidabhängige sind häufig von psychischen Komorbiditäten, insbesondere affektiven Störungen wie Depressionen betroffen. Die Diagnostik und Behandlung von depressiven Störungen in der Substitutionsbehandlung Opioidabhängiger stellen eine besondere Herausforderung dar. Dabei besteht die Gefahr, dass ein Teil der Patienten mit depressiven Symptomen unterversorgt bleibt. Ziel Die Studie untersucht das Ausmaß depressiver Symptome bei opioidabhängigen Patienten in einer Substitutionsbehandlung und prüft die Übereinstimmung von Selbst- und Fremdbeurteilungen durch den Patienten und den jeweiligen substituierenden Arzt. Methoden Zeitgleiche Fragebogenerhebung in 6 substituierenden Einrichtungen in Hamburg, Bremen und Kassel. Die Selbstbeurteilung zum Ausmaß von depressiven Symptomen erfolgte bei den Patienten mittels Beck Depressionsinventar (BDI-II). Zeitgleich wurden die substituierenden Ärzte anhand der Hamilton Depressions-Skala (HAMD) zur Beurteilung der depressiven Symptombelastung ihrer Patienten befragt. Ergebnisse Opioidabhängige Patienten in Substitutionstherapie sind häufig in einem klinisch relevanten Ausmaß von depressiven Symptomen betroffen. Es konnte ein positiver Zusammenhang zwischen den Instrumenten in 4 kategorialen Ausprägungen bestätigt werden. Im Vergleich zur Selbstbeurteilung schätzten die Ärzte die depressive Belastung ihrer Patienten als weniger schwer ein. Schlussfolgerung Die beiden verwendeten Instrumente sind geeignet, die Diagnostik von depressiven Symptomen bei Opioidabhängigen in Substitutionstherapie zu unterstützen, wobei die Fremdbeurteilung durch die substituierenden Ärzte zurückhaltender ausfällt. Die Studienergebnisse verweisen auf die hohe Relevanz der Diagnostik depressiver Symptomatik unter Opioid-Substituierten, um ggf. geeignete Behandlungsmaßnahmen einzuleiten.

ZusammenfassungEs wird untersucht, wie sich die kassenärztlichen Verschreibungen von Benzodiazepinen (BZD) und Z-Substanzen (ZS) über einen 10-Jahres-Zeitraum entwickelt haben. GKV-Rezepte des Norddeutschen Apotheken-Rechenzentrums (NARZ) wurden für Schleswig-Holstein, Hamburg, Niedersachsen und Bremen von 2006 – 2015 ausgewertet. 1,64 Mio. Patienten erhielten Verschreibungen von BZD und/oder ZS im 10-Jahres-Zeitraum. 65,3 % sind Frauen, über die Hälfte sind 60 Jahre oder älter. Der Anteil an Patienten mit leitliniengerechten Verschreibungen steigt von 51,7 % in 2006 auf 60,2 % in 2015. Problematische Verschreibungen gehen von 34,8 % auf 27,1 % zurück. Der Anteil leitlinienabweichender Verschreibungen von BZD und ZS, die auf Missbrauch oder Abhängigkeit deuten, ist unter GKV-Patienten seit vielen Jahren rückläufig.

Introduction: Amphetamine-type stimulants (ATSs) are the second most commonly used class of illegal substances in Europe. Although concurrent substance use has been subject to research, little is known about associations between concurrent use of cocaine, alcohol, or cannabis and ATS dependence. We expect that the concurrent use of any of the substance, especially cannabis and cocaine, is associated with ATS dependence. Methods: Cross-sectional data were gathered within the European ATTUNE study in 2018/2019. Participants (N = 721) were asked about their consumption patterns and social, psychological, and economic situation. Multivariate logistic regressions were carried out for associations between ATS dependence and use combinations of frequent cocaine, alcohol, or cannabis, with the reference group of no frequent concurrent use (model 1). Model 2 calculated associations for ATS dependence with lifetime methamphetamine use for respective use combinations. Results: The study population was on average 28.9 years old (SD = 7.7), with the majority being male (63.5%). In model 1, the adjusted odds ratio (aOR) for frequent alcohol use was 0.70 (confidence interval [CI] 0.41–1.20). Similar results were shown for model 2 (aOR 0.82, CI 0.42–1.62). Frequent cannabis use significantly reduced the chance for ATS dependence by 50% in adjusted model 1 (aOR 0.50, CI 0.28–0.89) and by 62% in model 2 (aOR 0.38, CI 0.18–0.82). For frequent cocaine use, models 1 and 2 report an aOR at 1.37 (CI 0.58–3.25) and 2.39 (CI 0.77–7.43), although not statistically significant. Frequent users of all 3 substances had a significant 3-fold chance for ATS dependence (model 1: aOR 2.98, CI 1.16–7.63; model 2: aOR 2.95, CI 1.02–8.58). Discussion: Against initial hypotheses, frequent concurrent use of alcohol or cannabis generally decreased chances for ATS dependence. An explanation could be the study population, which consists of many irregular users of ATS, who mainly consume alcohol or cannabis. Cocaine generally increased chances, although results were not significant. The frequent use of all 3 substances together with ATS in the last year was significantly associated with dependence, thus reporting important information for treatment services. Further research is needed for disentangling causal relationships underlying these associations and for pinpointing consequences for relapse prevention and retention success.

Background Patient-reported outcomes (PROs) can help to reduce uncertainties about hepatitis C virus (HCV) treatment with direct-acting antivirals (DAAs) among people who inject drugs and increase treatment uptake in this high-risk group. Besides clinical data, this study analyzed for the first time PROs in a real-world sample of patients on opioid agonist treatment (OAT) and HCV treatment with DAAs. Methods HCV treatment data including virological response, adherence, safety, and PROs of 328 German patients on OAT were analyzed in a pragmatic prospective cohort study conducted from 2016 to 2018. Clinical effectiveness was defined as sustained virological response (SVR) at week 12 after end of treatment and calculated in per-protocol (PP) and intention-to-treat (ITT) analyses. Changes over time in PROs on health-related quality of life, physical and mental health, functioning, medication tolerability, fatigue, concentration, and memory were analyzed by repeated-measures analyses of variances (ANOVAs). Results We found high adherence and treatment completion rates, a low number of mainly mild adverse events, and high SVR rates (PP: 97.5% [n = 285]; ITT: 84.5% [n = 328]). Missing SVR data in the ITT sample were mainly caused by patients lost to follow-up after treatment completion. Most PROs showed statistically significant but modest improvements over time, with more pronounced improvements in highly impaired patients. Conclusions This real-world study confirms that DAA treatment among OAT patients is feasible, safe, and effective. PROs show that all patients, but particularly those with higher somatic, mental, and social burden, benefit from DAA treatment.

Background Research has described subtypes with more internalizing and more externalizing symptoms in samples of patients with post-traumatic stress disorder (PTSD) and samples of patients with substance use disorders (SUD). Objective This study sought to examine the respective subtypes in female PTSD-SUD patients and potential relationships with substance use characteristics. Methods We performed a latent class analysis (LCA) in 343 adult female participants of a multisite therapy trial on PTSD and SUD. We derived externalizing symptoms from the Assessment of DSM-IV Personality Disorders (ADP-IV) questionnaire. We assessed internalization using the Symptom Checklist-27 (SCL-27) and the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-I). We collected substance use–related information using the Addiction Severity Index-Lite (ASI-lite). Results LCA suggested four latent classes including an externalizing (10.8%), a moderately internalizing class (31.0%), and a highly internalizing class (22.0%), as well as a class with a low severity of psychological symptoms (36.3%). Externalizing participants used more substances than any other class, while the duration of substance use did not differ between groups. Regular use started at a significantly younger age among the members of the highly internalizing subgroup compared to the moderately internalizing and low severity participants, but at an older age compared to the externalizing group members. Conclusions The finding of two internalizing subgroups along with an externalizing and a low severity class emphasizes the heterogeneity and complexity of populations with PTSD and SUD. This heterogeneity bears implications for research among this group of patients, but also for their treatment, especially considering our results on differences in substance use.

ObjectiveThere is a dearth of high-quality systematic evidence on the impact of opioid substitution medications on mental health. We compared mental health outcomes between opioid medications and placebo/waitlist, and between different opioids.MethodsThis systematic review and meta-analysis of randomized clinical trials (RCTs) was pre-registered at PROSPERO (CRD42018109375). Embase, MEDLINE, PsychInfo, CINAHL Complete, and Web of Science Core Collection were searched from inception to May 2020. RCTs were included if they compared opioid agonists with each other or with a placebo/waitlist in substitution treatment of patients with opioid use disorder, and reported at least one mental health outcome on a span of more than 1-month post baseline. Studies with psychiatric care, adjunct psychotropic medications, or unbalanced psychosocial services were excluded. Primary outcomes were comparison of depressive symptoms and overall mental health between opioids and placebo/waitlist. Random effects model was used for all the meta-analysis.ResultsNineteen studies were included in the narrative synthesis and 15 in the quantitative synthesis. Hydromorphone, diacetylmorphine (DAM), methadone, slow-release oral morphine, buprenorphine, and placebo/waitlist were among the included interventions. Based on network meta-analysis for primary outcomes, buprenorphine (SMD (CI95%)= −0.61 (−1.20, −0.11)), DAM (−1.40 (−2.70, −0.23)), and methadone (−1.20 (−2.30, −0.11)) were superior to waitlist/placebo on overall mental health. Further direct pairwise meta-analysis indicated that overall mental health improved more in DAM compared to methadone (−0.23 (−0.34, −0.13)).ConclusionsIt appears that opioid medications improve mental health independent of psychosocial services. Potential contribution of other factors needs to be further investigated.

Background Knowledge of health-related quality of life (HRQOL) of patients receiving opioid substitution treatment (OST) is limited and fragmented. The present study examines the HRQOL of a large national sample of OST patients in Germany and sociodemographic and clinical correlates. Methods Cross-sectional data on the HRQOL of 2176 OST patients was compared with German general population norms. Patients were recruited from 63 OST practices across Germany. To identify correlates of HRQOL, as measured with the SF-12, we performed bi- and multivariate analyses with sociodemographic and clinical variables, including patient- and clinician-reported outcomes on physical and mental health. Results Patients’ HRQOL was significantly poorer than in the general population, especially their mental HRQOL. Factors associated with lower physical HRQOL were older age, longer duration of opioid dependence, hepatitis C virus infection, and HIV infection. Benzodiazepine use was associated with lower mental HRQOL, and amphetamine use with higher physical HRQOL, compared to non-use of these substances. For both mental and physical HRQOL, the factor with the strongest positive association was employment and the factors with the strongest negative associations were physical and mental health symptom severity, psychiatric diagnosis, and psychopharmacological medication. Conclusions Compared to the general population, we found substantially lower HRQOL in OST patients, especially in their mental HRQOL. OST programs can benefit from further improvement, particularly with regard to mental health services, in order to better serve their patients’ needs. Clinicians may consider the use of patient-reported outcome measures to identify patients’ subjective physical and psychological needs. Background: New approaches are required to slow down or reverse increasing trends of levels of delta-9-tetrahydrocannabinol (THC) and cannabis-attributable hospitalizations in Germany. Legal access to cannabis may constitute one viable effective policy response; however, available evidence does not suffice to inform a regulation model for Germany. The proposed study aims to reduce harm for cannabis users through legal access to herbal cannabis through pharmacies. Protocol: A quasi-experimental study comparing cannabis users with legal access to herbal cannabis (Berlin, intervention group) to those without legal access (Hamburg, control group) (total N=698). As the primary outcome, we hypothesize that: 1) illegal THC consumption will reduce by at least 50% in the intervention group and 2) total THC exposure in the intervention group will be reduced by at least 10% lower than that of the control group, taking into account baseline values. Secondary outcomes comprise measures of frequency of use, THC-impaired driving, and mode of administration. Paired t-tests and multilevel regression models will be performed for statistical analyses. Discussion: This study proposal is currently being reviewed by the ‘Federal Institute for Drugs and Medical Devices’ – the body responsible for approving research studies on classified substances, including cannabis. Upon approval and prior to the start of the study, a full ethical review will be undertaken. Results may inform a regulation model for Germany and other jurisdictions and are expected to deepen the understanding of the effects of legal access to cannabis. Zusammenfassung Hintergrund Die Langzeitverordnung von Benzodiazepinen (BZD) und Z-Substanzen ist mit dem Risiko einer Abhängigkeitsentwicklung verbunden. Bei vielen älteren Patienten kommt es zu einer Langzeiteinnahme, häufig verbunden mit einer Niedrigdosisabhängigkeit ohne Dosissteigerung. Studienziel Es sollen die Einnahmegründe, Beschwerden bzw. Indikationen und Nebenwirkungen sowie die Verschreibungsumstände unter älteren Patienten, denen innerhalb der letzten 12 Monate BZD oder Z-Substanzen verschrieben wurden, untersucht werden. Methode 4000 Patienten der AOK-NordWest, stratifiziert nach Altersgruppe (50–65 Jahre;>65 Jahre) und Einnahmeverhalten (lang bzw. leitlinienabweichend; kurz bzw. leitliniengerecht), wurde ein 9-seitiger Fragebogen zugeschickt. Der Fragebogen enthielt eine ausführliche Liste von BZD und Z-Substanzen, für die angegeben werden sollte, ob man sie früher und/oder aktuell eingenommen hat. Zudem wurden die Gründe und zugrunde liegenden Symptome der Medikamenteneinnahme sowie mögliche Nebenwirkungen und Versuche, die Medikamente auszusetzen, erhoben. Ergebnisse 466 AOK-Versicherte sendeten den Fragebogen ausgefüllt zurück (Rücklaufquote 11,7%). Weitere 43 Privatversicherte konnten über Apotheken erreicht werden. 169 Personen, vornehmlich aus der Gruppe mit leitliniengerechten Verschreibungen, gaben an, keine der vorgegeben Medikamente eingenommen zu haben. Die verbliebenen 340 Patienten sind zu 68,5% weiblich. Das Durchschnittsalter beträgt 72,1 Jahre. Im Vergleich zwischen Patienten mit leitlinienabweichenden und leitliniengerechten Verschreibungen nehmen Erstere zu einem deutlich größeren Anteil Z-Substanzen ein. Die Tagesdosis nach DDD ist signifikant höher als bei jenen mit leitliniengerechten Verschreibungen, wobei in beiden Patientengruppen die Medikamente durchschnittlich in niedriger Dosis ( Schlussfolgerungen Vor dem Hintergrund der Empfehlungen, BZD und Z-Substanzen nur kurzzeitig bei schweren Schlafstörungen einzusetzen, ist das Ausmaß an Insomnie-Indikationen hoch. Die geringe Dosierung lässt für beide Gruppen auf ein vorsichtiges und kontrolliert durchgeführtes Verordnungsverhalten schließen, wobei sich bei der Mehrheit der Langzeiteinnehmer eine Niedrigdosisabhängigkeit entwickelt haben dürfte. Dies ist offensichtlich mit nur geringem Interventionsbedarf verbunden, da nur sehr wenige Medikamentenabhängige die Angebote der Suchtkrankenhilfe in Anspruch nehmen.

Traumatic experiences and posttraumatic stress disorder (PTSD) are prevalent in patients with alcohol use disorders (AUDs) and affect its course and outcome. Prior prevalence reports are limited by the inclusion of individuals with additional substance use disorders (SUDs), a focus on childhood events only and the use of self-ratings only.To examine the prevalence of traumatic experiences, revictimization and PTSD in inpatients treated for alcohol dependence without other SUD, emphasizing interpersonal violence across the whole lifespan.For this cross-sectional study alcohol-dependent patients without additional SUD (N = 230, 73% male, mean age 43 years) were recruited in an inpatient detoxification unit and were administered the Structured Trauma Interview, the Posttraumatic Diagnostic Scale, and the Structured Clinical Interview for DSM-IV. Data analysis comprised descriptive statistics and appropriate significance tests.36.2% reported severe childhood physical or sexual abuse and 45.6% reported at least one of these types of abuse in adulthood. The lifetime rate of interpersonal violence was 61.1%. The prevalence of current PTSD was 13.2%. Women with a history of childhood abuse were about seven times as likely to be victimized in adulthood as women without these experiences, while in men revictimization was not significant.Even in patients with alcohol dependence without additional SUD experiences of interpersonal violence and PTSD are frequent. In order to adequately respond to the needs of this population, trauma and PTSD should routinely be assessed in alcohol-dependence treatment and considered in treatment planning if necessary.

In Deutschland sind zwischen 1,4 und 2,6 Mio. Personen von Medikamenten abhangig. Die Mehrheit hat ein Problem mit Benzodiazepinderivaten oder Opioid-Analgetika. Trotz dieser hohen Zahl an Betroffenen liegen bisher kaum Studien zum Verordnungsverhalten und zur Einnahme vor. Insbesondere zu Personen uber 65 Jahren, welche uberdurchschnittlich haufig von einem missbrauchlichen Medikamentenkonsum betroffen sind, gibt es bisher nur wenige, empirisch belastbare Befunde. Auf Basis der Daten des Norddeutschen Apothekenrechenzentrums (NARZ), welches die Rezeptdaten von rund 11 Mio. Bundesburgern erfasst und eine Abdeckungsquote von uber 80 % aller Apotheken in Norddeutschland erreicht, werden die auf Kassenrezepten dokumentierten Verschreibungen von Benzodiazepinen (BZD), Z‑Substanzen und Opioid-Analgetika personenbezogen ausgewertet. Mittels Hochrechnungen wird die Pravalenz des Gebrauchs sowie der Langzeiteinnahme dieser Medikamente bestimmt, und entsprechende Trends werden dargestellt. Zudem wird die pro Tag durchschnittlich eingenommene Wirkstoffmenge (in DDDs) berichtet. Im Jahr 2006 haben exakt 10,6 % der GKV-Mitglieder mindestens einen der hier untersuchten Wirkstoffe verschrieben bekommen. Dieser Anteil andert sich in den funf nachfolgenden Jahren kaum. Der Anteil der Patienten mit BZD-Verschreibungen ging zwischen 2006 (5,5 %) und 2010 (5,1 %) kontinuierlich zuruck – insbesondere bei alteren Personengruppen. Wahrend bei den Z‑Substanzen die Pravalenz uber den gesamten Zeitraum stabil (ca. 1 %) bleibt, ist bei den Opioid-Analgetika ein leichter Anstieg – von 5,4 % (2006) auf 5,7 % (2010) – zu verzeichnen. Die Anteile von Patienten mit Langzeitverschreibungen sinken bei den BZD-Patienten stetig von 17,0 % (2006) auf 12,8 % (2010). Wahrend bei den Z‑Substanzen dieser Abfall geringer ausfallt – von 24,3 % (2006) auf 21,2 % (2010) –, ist bei den Opioid-Analgetika eine Steigerung von 19,2 % (2006) auf 21,2 % (2010) festzustellen. Das in diesem Beitrag vorgestellte Analyseverfahren stellt einen innovativen epidemiologischen Ansatz der Auswertung personenbezogener Registerdaten uber mehrere Jahre dar. Mit dem Aufbau eines darauf basierenden Monitorings der Verschreibungen von Medikamenten mit Abhangigkeitspotenzial konnten zukunftig problematische Trends schneller erkannt und entsprechende Gegenmasnahmen initiiert werden.

Background An estimated 1.4 to 2.6 million people in German suffer from drug dependence. Most of them are long-term users of benzodiazepines (BZD), Z drugs (ZD), or opioid analgesics (OA). Methods This analysis is based on prescription data from patients of the national statutory health insurance system in the German federal states of Schleswig-Holstein, Hamburg, Bremen, and Lower Saxony. Drug-taking trends, duration, dosage, and long-term use of BZD, ZD, and OA in the years 2006 to 2015 are analyzed; prevalences are estimated for the years 2006 to 2016. Results In 2006, 7.7% of patients received at least one prescription for a BZD, ZD, or OA; in 2016, 7.0% did. Over the period of analysis, a marked drop was seen in prescriptions of BZD and a slight fall in prescriptions of ZD (2006: BZD 3.5%, ZS 1.1%; 2016: BZD 2.0%, ZS 0.8%), but there was also an increase in prescriptions of OA, from 4.2% to 4.9%. The number of defined daily doses (DDD) prescribed per year fell for both BZD and ZD. For OA, the number of DDD prescribed per year rose from 2006 to 2009 and decreased by a small amount in subsequent years. The proportions of BZD and ZD patients who had long-term prescriptions fell over time, while the corresponding percentage of OA patients rose. Conclusion Nearly one-fifth of all prescriptions for BZD were long-term prescriptions for an entire year, in violation of the relevant guidelines. The rising prevalence of OA use was in the expected range in view of the aging population, but the number of prescriptions rose among younger patients as well. This trend toward more common treatment with opioid analgesics should be critically examined.

IntroductionAmphetamine-type stimulants (ATS) including amphetamine, methylenedioxymethamphetamine/‘ecstasy’, methamphetamine, synthetic cathinones and ‘Ritalin’ are the second most commonly used illicit drugs globally. Yet, there is little evidence on which factors are associated with the development of different patterns of ATS use over the life course. This study aims to examine which individual, social and environmental factors shape different pathways and trajectories of ATS consumption. The study will be conducted in five European countries: Germany, the Netherlands, Poland, Czech Republic and the UK.Methods and analysisWe will use a sequential mixed-methods study design to investigate the multiple factors (familial, social and occupational situation, critical life events, general risk behaviour, mental and physical health, satisfaction with life) that shape individual ATS use pathways. A systematic literature review will be performed to provide an overview of the current academic literature on the topic. In module 1, qualitative semistructured interviews (n=ATS users and non-users) will be conducted to explore individual experiences of, and perspectives on, dynamics of change in stimulant consumption patterns. In module 2, structured questionnaires (n=2000 ATS users and non-users) will be administered via tablet computers to validate and enhance the generalisability of the interview findings. Data integration will take place at two key points. First, during the study, where the findings from the first qualitative interviews will inform the design of the structured questionnaire. Second, at the end of the study, where mixed methods data will be brought together to generate an in-depth, contextualised understanding of the research topic.Ethics and disseminationThe study has been approved by the respective responsible ethics committee in each participating country. Data will be treated confidentially to ensure participants’ anonymity. Findings will be disseminated in peer-reviewed scientific journals, national and international conferences, and in briefings for policy and practice.

Background Hepatitis C virus (HCV) infection is highly prevalent among people who inject drugs (PWID). Accurate data on HCV prevalence and incidence rates among patients receiving opioid substitution treatment (OST) are needed to estimate the current and future burden of HCV infections in this high-risk population. Methods Baseline data from routine care were collected between October 2014 and June 2016 from randomly selected OST facilities in Germany. The primary outcome measure was the HCV status (antibody and RNA prevalence). Patients who were HCV antibody–negative at baseline were followed up after 12 months to calculate the HCV incidence rate. Results Sixty-three facilities from 14 German Federal States provided clinical data for a total of 2466 OST patients. HCV antibody and HCV RNA prevalence were 58.8% (95% confidence interval [CI], 56.8%–60.8%) and 27.3% (95% CI, 25.5%–29.2%), respectively. At baseline, a total of 528 patients (21.4%) had previously undergone antiviral treatment. Moreover, lower HCV RNA prevalence was associated with female gender, employment, younger age, and shorter duration of OST and opioid dependence. The HCV incidence rate was 2.5 cases per 100 person-years. Conclusions The low HCV RNA prevalence and HCV incidence rates confirm that OST in Germany is an effective setting both for treating chronic HCV infections and for preventing new infections among PWID. Scaling up the provision of OST, HCV testing, and HCV treatment among OST patients are important public health strategies for reducing HCV infections in this high-risk population.

Background: Opioid addiction is one of the most common substance-related disorders worldwide, and morbidity and mortality due to opioid addiction place a heavy burden on society. Knowing the size of the population that is addicted to opioids is a prerequisite for the development and implementation of appropriate health-policy measures.Methods: Our estimate for Germany for 2016 is based on an enumeration of opioid-addicted persons who were entered in a registry of persons receiving substitution therapy, an enumeration of persons receiving outpatient and inpatient care for addiction without substitution therapy, an extrapolation to all addiction care facilities, and an estimation of the number of opioid-addicted persons who were not accounted for either in the substitution registry or in addiction care.Results: The overall estimate of the number of opioid-addicted persons in Germany in 2016 was 166 294 persons (lower and upper bounds: 164 794 and 167 794), including 123 988 men (122 968 to 125 007) and 42 307 women (41 826 to 42 787). The estimates for each German federal state per 1000 inhabitants ranged from 0.1 in Brandenburg to 3.0 in North Rhine-Westphalia and 5.5 in Bremen. The average value across Germany was 3.1 per 1000 inhabitants.Conclusion: Comparisons with earlier estimates suggest that the number of persons addicted to opioids in Germany has hardly changed over the past 20 years. Despite methodological limitations, this estimate can be considered highly valid. Nearly all persons who are addicted to opioids are in contact with the addiction care system. Den Umfang der opioidabhängigen Population zu kennen, ist eine Voraussetzung für gesundheitspolitische Maßnahmen.Methode: Die Schätzung für das Jahr 2016 basiert auf einer Zählung von Personen in Deutschland mit einer Opioidabhängigkeit, die im Substitutionsregister gemeldet waren, einer Zählung der in der ambulanten und stationären Suchthilfe gemeldeten Personen ohne Substitutionsbehandlung und einer Hochrechnung auf alle Einrichtungen sowie einer Schätzung der Anzahl von Personen mit einer Opioidabhängigkeit, die weder im Substitutionsregister noch in der Suchthilfe erfasst waren.Ergebnisse: Für Deutschland ergibt sich eine Gesamtschätzung von 166 294 Personen (untere und obere Grenzen: 164 794; 167 794) mit einer Opioidabhängigkeit, wobei 123 988 Männer (122 968; 125 007) und 42 307 Frauen (41 826; 42 787) betroffen sind. Die Schätzungen für die Bundesländer pro 1 000 Einwohner schwanken zwischen 0,1 in Brandenburg, 3,0 in Nordrhein-Westfalen bis 5,5 Bremen. Ziel der Studie: Ermittlung und Vergleich von Konsummotiven bei Personen, die Amphetamin bzw. Methamphetamin konsumieren. Methoden: Anhand einer Literaturrecherche und einer zusatzlichen Expertenbefragung wurde ein Fragebogen entwickelt, der 145 Patienten in ambulanter und stationarer Behandlung sowie 247 Personen uber das Internet vorgelegt wurde. Diese wiesen in den zuruckliegenden 12 Monaten einen Amphetaminkonsum auf (N=205) oder konsumierten (teilweise zusatzlich zu Amphetamin) auch Methamphetamin (N=187). Ergebnisse: Wahrend freizeit- und genussbezogene Motive bei allen Befragten eine grose Rolle spielten, wurden von der Methamphetamingruppe auffallend haufig weitere Motive berichtet, etwa der Konsum zur Tagesstrukturierung (39,0%), zur Unterdruckung von Albtraumen (14,4%), zur Selbstbehandlung von Storungen (36,9%) sowie um Sexualitat uberhaupt moglich zu machen (18,7%). Schlussfolgerung: Bei Personen, die Methamphetamin konsumieren, scheint im Vergleich zu Amphetamin ein breiteres Spektrum von Konsummotiven eine Rolle zu spielen. Hierbei handelt es sich teilweise um Motive, die mit dem Funktionsniveau im Alltag sowie mit psychischen Belastungen in Zusammenhang zu stehen scheinen. Eine sorgfaltige Analyse der vielfaltigen und komplexen Konsummotive bei Stimulanzienkonsumenten konnte dazu beitragen, therapeutische und praventive Interventionen optimal auf deren individuelle Bedurfnisse abzustimmen.

BACKGROUND AND AIMS: Despite ambiguous evidence for the effectiveness of alcohol screening with brief interventions (BI) in emergency departments (ED), ambition for their widespread implementation continues to grow. To clarify whether such an application of BI is justifiable, we conducted a systematic review and meta-analysis on studies testing the impact of BI on alcohol consumption. METHODS: We included peer-reviewed,randomized controlled studies investigating the effects of BI on alcohol consumption in injured and/or intoxicated patients, published January 2002 - September 2015. Changes from baseline in consumption quantity, intensity and number of heavy drinking episodes were assessed at 3, 6 and 12 month follow-up, resulting in 9 separate random-effects meta-analyses of standardized mean differences (SMD). Moderation effects of intervention mode, length, type of interventionist, intensity of control intervention and study quality were assessed using subgroup comparisons and meta-regression. RESULTS: We considered 33 publications (28 separate studies) including 14,456 patients. Six out of nine comparisons revealed small significant effects in favour of BI, with the highest SMD at 0.19 (95% CI: 0.08-0.31). No significant moderators could be identified, and statistical heterogeneity (I(2) ) was below 40%. CONCLUSIONS: In a large meta-analysis of randomized controlled trials in emergency care settings, there was evidence for very small effects of brief interventions on alcohol consumption reductions. More intensive interventions showed no benefit over shorter approaches. Non-face-to-face interventions appear comparably effective, but this finding remains tentative due to the low number of non-face-to-face studies. This article is protected by copyright. Introduction The long-term use of benzodiazepines (BZD) and z-drugs in older populations is associated with a variety of sociodemographic and health-related factors. Recent studies reported that long-term BZD and z-drugs use is associated with increased age, female sex, and severe negative psychological (e.g., depression) and somatic (e.g., chronic disease) factors. The current study explores the sociodemographic and health-related factors associated with long-term BZD and z-drugs use in the elderly. Methods We conducted a cross-sectional survey among randomly selected patients of one health insurance plan (“AOK North-West”) with BZD and z-drugs prescriptions in the past 12 months. The sample was stratified by appropriate German prescription guidelines (yes vs. no) and age (50–65 vs. >65 years). To examine the association of selected sociodemographic and psychological variables (e.g., sex, employment status, quality of life, depression) with long-term use, a binary logistic regression analysis was conducted. Results In total, data from 340 patients were analyzed. The mean age was 72.1 (SD = 14.5) years, and the most commonly used substances were zopiclon (38.1%), oxazepam (18.1%), and lorazepam (13.8%). The mean defined daily dose (DDD) was 0.73 (SD = 0.47). Insomnia was the main reason for prescribing BZD and z-drugs. The long-term use of BZD and z-drugs was significantly associated with unemployment (OR = 2.9, 95% CI [1.2–7.1]) and generally problematic medication use (OR = 0.5, 95% CI [0.2–1.0]). Discussion Unemployment status and problematic medication use had a significant association with the patient-reported, long-term use of BZD and z-drugs. Divergent prescription patterns might suggest problematic patterns of BZD and z-drugs use. The causal connection between the identified factors and problematic BZD and z-drugs prescription is not discussed in this paper. Nevertheless, employment status and possible evidence of general problematic drug use may be a warning signal to the prescribers of BZD and z-drugs.

Purpose Opioid dependence is a chronic relapsing disorder. Despite increasing research on quality of life (QOL) in people with opioid dependence, little attention has been paid to the instruments used. This systematic review examines the suitability of QOL instruments for use in opioid-dependent populations and the instruments’ quality. Methods A systematic search was performed in the databases Medline, PsycInfo, The Cochrane Library, and CINAHL. Articles were eligible if they assessed QOL of opioid-dependent populations using a validated QOL instrument. Item content relevance to opioid-dependent people was evaluated by means of content analysis, and instrument properties were assessed using minimum standards for patient-reported outcome measures. Results Eighty-nine articles were retrieved, yielding sixteen QOL instruments, of which ten were assessed in this review. Of the ten instruments, six were disease specific, but none for opioid dependence. Two instruments had good item content relevance. The conceptual and measurement model were described in seven instruments. Four instruments were developed with input from the respective target population. Eight instruments had low respondent and administrator burden. Psychometric properties were either not assessed in opioid-dependent populations or were inconclusive or moderate. Conclusions No instrument scored perfectly on both the content and properties. The limited suitability of instruments for opioid-dependent people hinders accurate and sensitive measurement of QOL in this population. Future research is in need of an opioid dependence-specific QOL instrument to measure the true impact of the disease on people’s lives and to evaluate treatment-related services. A history of sexual violence has been related to more complex treatment needs in patients with substance use disorders (SUD). Most of the existing studies, however, included patients with various types of SUD, did not examine gender differences and focused on a small range of clinical domains. Our sample consisted of opioid-dependent outpatients treated during a three-year period in a German metropolitan region. The analysis was based on a local case register and included all patients for whom information on lifetime sexual violence was available (N = 3531; 68.3% males). In a case–control design, patients with a history of sexual violence were compared to patients without these experiences regarding a wide range of clinical and social factors indicative of potential needs. Almost two thirds (65.6%) of the female patients and 10.9% of the males reported experiences of sexual violence. Victims differed from non-victims across a variety of domains, including more psychiatric symptoms and suicide attempts, more legal problems, financial and family problems, as well as a higher use of services. In contrast to a previous study among alcohol-dependent patients, no gender differences became apparent. Our findings suggest that experiences of sexual violence are an indicator for more complex needs in opioid-dependent patients of both genders. In addition to integrated trauma-informed approaches, an effort needs to be made to link addiction facilities to further institutions to meet these complex needs.

Background: Opioid maintenance treatment is the option of choice to stabilize opioid-dependent patients. Whilst efficacy of methadone and buprenorphine has been studied extensively, fewer data on slow-release oral morphine are available. Aims: This study analyzes the effects of slow-release oral morphine compared to methadone with regard to self-reported mental symptoms, drug use and satisfaction with treatment. Methods: The study was carried out as an open-label randomized crossover trial in 14 treatment sites in Switzerland and Germany. It comprised 2 crossover periods of 11 weeks each. For measuring mental symptoms, the Symptom Checklist-27 (SCL-27) was used. Drug and alcohol use was assessed by the number of consumption days, and treatment satisfaction by a visual analogue scale. Results: A total of 157 patients were included for the analyses (per-protocol sample). Statistically significantly better outcomes for morphine as compared to methadone treatment were found for overall severity of mental symptoms (SCL-27 Global Severity Index), as well as 5 of the 6 syndrome groups of the SCL-27, and for treatment satisfaction. There were no statistically significant differences with regard to drug or alcohol use between groups. Conclusions: This study supports positive effects of slow-release oral morphine compared to methadone on patient-reported outcomes such as mental symptoms and treatment satisfaction with comparable effects on concomitant drug use. Slow-release oral morphine represents a meaningful alternative to methadone for treatment of opioid dependence.

Opioid substitution treatment (OST) for opioid dependence may be limited by adverse events (AEs). Increasing the range of therapeutic options optimizes outcomes and facilitates patient management. An international, multi-center, two-phase study investigated the efficacy and safety of slow-release oral morphine (SROM) versus methadone in patients receiving methadone therapy for opioid dependence. In phase 1 (two way cross-over, 11 weeks each period) patients were randomized to SROM or methadone oral solution. In phase 2 (25 weeks), patients continued treatment with SROM (group A) or switched from methadone to SROM (group B). In total, 211 out of 276 completed phase 1 and 198 entered phase 2 (n=95 group A, n=103 group B). Treatment with both SROM and methadone was well tolerated. However, the mean QTc-interval associated with methadone was significantly longer than that under SROM. Higher treatment satisfaction, fewer cravings for heroin, and lower mental stress were reported with SROM. This study adds a significant further weight of evidence that SROM is an effective and well tolerated long-term maintenance treatment for opioid dependence with a beneficial risk profile compared to methadone regarding cardiac effects and supports its clinical utility.

Aims To compare the efficacy of slow-release oral morphine (SROM) and methadone as maintenance medication for opioid dependence in patients previously treated with methadone. Design Prospective, multiple-dose, open label, randomized, non-inferiority, cross-over study over two 11-week periods. Methadone treatment was switched to SROM with flexible dosing and vice versa according to period and sequence of treatment. Setting Fourteen out-patient addiction treatment centres in Switzerland and Germany. Participants Adults with opioid dependence in methadone maintenance programmes (dose ≥50 mg/day) for ≥26 weeks. Measurements The efficacy end-point was the proportion of heroin-positive urine samples per patient and period of treatment. Each week, two urine samples were collected, randomly selected and analysed for 6-monoacetyl-morphine and 6-acetylcodeine. Non-inferiority was concluded if the two-sided 95% confidence interval (CI) in the difference of proportions of positive urine samples was below the predefined boundary of 10%. Findings One hundred and fifty-seven patients fulfilled criteria to form the per protocol population. The proportion of heroin-positive urine samples under SROM treatment (0.20) was non-inferior to the proportion under methadone treatment (0.15) (least-squares mean difference 0.05; 95% CI = 0.02, 0.08; P > 0.01). The 95% CI fell within the 10% non-inferiority margin, confirming the non-inferiority of SROM to methadone. A dose-dependent effect was shown for SROM (i.e. decreasing proportions of heroin-positive urine samples with increasing SROM doses). Retention in treatment showed no significant differences between treatments (period 1/period 2: SROM: 88.7%/82.1%, methadone: 91.1%/88.0%; period 1: P = 0.50, period 2: P = 0.19). Overall, safety outcomes were similar between the two groups. Conclusions Slow-release oral morphine appears to be at least as effective as methadone in treating people with opioid use disorder.

Buprenorphin ist in Deutschland seit dem Jahr 2000 fur die Substitutionsbehandlung Opiatabhangiger zugelassen und wird derzeit bei knapp einem Funftel aller substituierten Patienten eingesetzt. Studien belegen die Wirksamkeit von Buprenorphin in der Opiatsubstitution. Ahnlich anderer Substitute fuhren hohere Dosierungen mit Buprenorphin vermehrt zu einer erhohten Retention und reduziertem Beigebrauch weiterer psychotroper Substanzen, wohingegen ein Teil der Patienten bereits von niedrigen bis mittleren Dosen profitiert. Entscheidend ist somit nicht per se die Hohe der Dosierung, sondern deren Angemessenheit, ausgerichtet an den individuellen Bedurfnissen des Patienten. Zu diesem Zweck wurde die „Opiate Dosage Adequacy Scale“ (ODAS) entwickelt, die bislang nur fur das Substitut Methadon vorlag. Erstellung einer deutschsprachigen Ver­sion der „Opiate Dosage Adequacy Scale“ (ODAS) fur Buprenorphin. Die Anpassung des ODAS-Fragebogens durch eine Expertengruppe erfolgte sprachlich auf Basis der deutschen Skala fur Methadon. Gleichzeitig wurde der Fragenalgorithmus gepruft und zur besseren Verstandlichkeit und Anwendbarkeit angepasst. Nach der sprachlichen Anpassung liegt die „Opiate Dosage Adequacy Scale“ (ODAS) nun erstmals auch fur Buprenorphin vor. Nach erfolgreicher Validierung in einer Anwendungsstudie konnte sich der ODAS-Fragebogen als ein ausgezeichnetes Instrument fur die Qualitatssicherung in der Opiatsubstitu­tion mit Buprenorphin erweisen.

BACKGROUND: Beyond unquestioned merits of opioid substitution treatment (OST), non-prescribed use of substitution medication (NPU) and its availability on the black market still constitute critical issues. METHODS: This cross-sectional study was conducted in low-threshold drug services in 13 German cities. Clients were interviewed face-to-face, using a structured questionnaire on drug use patterns, prevalence and motives for NPU, and availability and prices of substitution medication on the black market. RESULTS: Our sample of 753 drug users (74.5% male) had a mean age of 36.4 years, 42.4% were currently in OST and 78.4% reported heroin use in the past 30 days. NPU prevalence for lifetime, past 30 days, and past 24h was 66.7%, 25.8%, and 9.3%. Access to substitution medication was considered 'easy'. Injection of substitution medication was reported by 9.5% of our sample (i.e. 14.4% of injectors). Motives for NPU show large heterogeneity, with "lack of other drugs" and "insufficient OST dosage" as the reasons stated most frequently. CONCLUSIONS: NPU represents a relevant source of opiates among German drug users. While OST is associated with a reduction of illicit drug use and injection use, motives for NPU suggest that the treatment services do not sufficiently attract opioid-addicted persons. Language: en

Studien aus mehreren Landern konnten mittlerweile die grosere Wirksamkeit der Diamorphin- im Vergleich zur Methadonbehandlung bei Opiatabhangigen belegen. Jedoch sind nur wenige langfristige Ergebnisse sowie insbesondere jene zur sozialen Integration der Patienten bekannt. Es wird der Frage nachgegangen, wie sich der soziale Integrationsprozess unter den Patienten des bundesdeutschen Modellprojekts nach 4 Jahren Behandlungsdauer entwickelt. Bei Patienten, die mindestens 4 Jahre mit Diamorphin behandelt wurden (n=156), werden die sich im Verlauf ergebenden Veranderungen in den Bereichen Gesundheit, Drogenkonsum sowie schwerpunktmasig der sozialen Integration anhand von Messwiederholungsanalysen auf Signifikanz gepruft. Dabei wird auf die Instrumente der OTI-Gesundheitsskala und der Symptom-Checkliste SCL-90-R, auf arztliche Beurteilungen, Urinkontrollen zum Drogenkonsum sowie auf Variablen des European Addiction Severity Index und dessen Composite-Scores zuruckgegriffen. In allen Bereichen ergeben sich signifikante Verbesserungen im Zeitverlauf. Der Anteil an Patienten, die aktuell arbeiten, hat sich mit knapp 40% nach 4 Jahren Diamorphinbehandlung mehr als verdreifacht. Auch in der Wohnsituation sowie im Freizeitverhalten zeigen sich Verbesserungen, und die Delinquenz geht erheblich zuruck. Haupteinflussfaktor auf eine gelungene soziale Integration nach 4 Jahren Behandlung ist die Arbeitsfahigkeit. Die Diamorphinbehandlung erweist sich hinsichtlich der Verbesserung des Gesundheitszustands, der Verringerung des Konsums harter Drogen sowie der verbesserten sozialen Integration langfristig als ausgesprochen erfolgreiche Therapie schwerstabhangiger Heroinkonsumenten. Die Ergebnisse zeigen ferner, dass soziale (Re-)Integrationsprozesse bei Drogenabhangigen Zeit benotigen.

Schatzungen der Deutschen Hauptstelle fur Suchtfragen zufolge gibt es in Deutschland zwischen 1,4 und 1,9 Millionen Medikamentenabhangige. Nach den Zahlen der Deutschen Suchthilfestatistik findet sich nur ein Bruchteil der Betroffenen in Einrichtungen der Suchtkrankenhilfe. Bei der Mehrheit der Medikamentenabhangigen spielen Benzodiazepine eine Rolle. Diese wiederum werden in der Regel arztlich verordnet und sollen nach den gangigen Leitlinien nur kurzzeitig und in niedrigen Dosen eingenommen werden. Bei der vorliegenden Studie geht es um die systematische Erfassung problematischer und ggf. missbrauchlicher Versorgung von Patienten mit Benzodiazepinen und Non-Benzodiazepinen. Innerhalb eines 2-jahrigen Zeitraums (2005–2007) wurden die Daten des Norddeutschen Apothekenrechenzentrums (NARZ) fur die Bundeslander Hamburg, Bremen und Schleswig-Holstein personenbezogen fur 12 Monate prospektiv ausgewertet. Im Zeitraum Juli 2005 bis Juni 2006 erhielten 78 456 Hamburger auf insgesamt 294 143 Rezepten kassenarztliche Verschreibungen von Benzodiazepinen und Non-Benzodiazepinen. Im Verlauf eines beobachteten Jahres konnten gemas DDD bei 51,1% die kassenarztlichen Verordnungen als leitliniengerecht zugeordnet werden. Bei 15,6% der Hamburger „Benzodiazepin-Patienten“ liegt eine tendenzielle Dauerversorgung von Benzodiazepinen mit relevant hoher Dosierung (0,5–1 DDD uber mind. 2 Monate) vor. Frauen und alteren Personen uber 70 Jahre werden Benzodiazepine uberproportional haufig verschrieben. Im Vergleich zu den Bundeslandern Bremen und Schleswig-Holstein nimmt Hamburg keine Sonderstellung ein. Die nicht Leitlinien gerechte (Langzeit-)Verschreibung von Benzodiazepinen ist weit verbreitet und fordert zur Aufklarung und Diskussion dieser Problematik in der Arzte- und Apothekerschaft heraus. Auch die Suchthilfe sollte sich den Problemen und Hilfebedarfen der Betroffenen nicht verschliesen, da davon auszugehen ist, dass sich die Zahl an Medikamentenabhangigen in der alternden Gesellschaft zukunftig noch erhohen durfte.

Fragestellung: Die Arbeit untersucht, ob sich alkoholabhängige Klienten, die zusätzlich zur ambulanten Betreuung in der professionellen Suchthilfe Selbsthilfegruppen aufsuchen, von solchen Klienten unterscheiden, die das nicht tun. Methodik: Es wurde ein Datensatz der Hamburger Basisdatendokumentation der ambulanten Suchthilfe (BADO) – die eine zeitliche Verlaufsbetrachtung ermöglicht – analysiert. Ergebnisse: Klienten, die neben der ambulanten Suchthilfe an Selbsthilfegruppen teilnehmen, sind sowohl zu Beginn als auch zu Ende der Betreuungsepisode häufiger abstinent. Obwohl sie über günstigere psychosoziale Ressourcen als solche Klienten verfügen, die das nicht tun, berichten sie häufiger über traumatische sexuelle Gewalterfahrungen. An Selbsthilfegruppen teilnehmende Klienten leiden seltener unter Schlafstörungen. Alkoholabhängige in der ambulanten Suchthilfe, die über einen längeren Zeitraum hinweg Selbsthilfegruppen aufgesucht haben, leiden schließlich seltener unter Leberschäden. Schlussfolgerung: Alkoholklienten, die neben ihrer ambulanten Betreuung Selbsthilfegruppen aufsuchen, weisen mehr Ressourcen und einen besseren Verlauf auf. Dies mag auf Wirkungen der Selbsthilfegruppe als ergänzende Hilfe oder auf Selektionseffekte zurückgehen.

Aims To evaluate physical and mental health and compare treatment outcomes in opiate-dependent patients substituted either with heroin or methadone. Design Twelve-month open-label randomized controlled trial. Setting Out-patient substitution clinics in seven German cities. Participants A total of 1015 opiate-dependent individuals. Introduction: This evaluation is part of a Twinning Project between Cyprus and Germany, aimed at evaluating the governmental drug services in Cyprus and promoting the improvement and introduction of new drug treatment services, in order to assist the new Member State in the implementation and harmonization with the European Community's legislation.Methods: A field investigation studied parameters as population in need, treatment demand and coverage, as well as high-risk patterns and their trend over the past years.Results: The main findings were a high level of problematic drug use, mainly high-risk intravenous heroin and cocaine use, and an increase treatment demand for women, adolescents and migrants. Expert interviews named gaps in treatment options, including a previous lack of maintenance treatment, insufficient harm reduction measures and inadequate treatment concepts to meet specific needs of problematic drug users.Discussion: Recommendations for improvement can be integrated into present services....

BackgroundAlthough long-term use of benzodiazepines (BZDs) and Z drugs is associated with various side effects, they remain popular among the older population. Possible reasons for this phenomenon could be ineffective ways of transmitting information on the health risks associated with long-term use, and communication gaps between patients and healthcare professionals.AimThe aim was to investigate the views of patients, physicians, nurses, and pharmacists regarding long-term BZD and Z drug use.Design & settingThe qualitative study design used focus group interviews with physicians, pharmacists, and nurses in Hamburg. Patient interviews were conducted in Lippstadt, Germany.MethodThe interviews were audiotaped with each participant’s permission, transcribed, and thematically analysed using a software program for qualitative research (MAXQDA).ResultsThe data from the four focus groups consisting of 28 participants were analysed. Patients indicated lack of knowledge about risks and side effects, difficult access to alternatives, and fears of ceasing drug use without professional support. Although the physicians were reported to be cautious about prescribing BZDs and Z drugs, the psychosocial problems of older patients are often considered to be complex and treatment knowledge, experience, and resources are frequently unsatisfactory. Nurses described that when BZDs were prescribed, they did not feel it was their responsibility to evaluate their effects. Pharmacists were reported to be strongly ambivalent in informing patients about the risks, which may contradict the prescription advice provided by the physician.ConclusionPatients, physicians, nurses, and pharmacists reported differences in the perception of long-term BZD and Z drug use. Nevertheless, all of the participants described lack of information and expressed the need for greater communication exchange.

The main objective of this follow-up study is to explore factors influencing the success or failure of women in reintegrating after their release from prison. Female drug users in five European cities were tracked after being released from prison. Out of 234 female prisoners contacted in prisons, 59 were included in the follow-up study. Structured questionnaires were given out and statistical group comparisons were conducted. The authors found that receiving preparation for release significantly supports a successful reintegration. On the other hand, living with a current drug user and initiating regular heroin use at an early age of less than 18 years are major barriers to successful reintegration.

Introduction: Studies indicate that different areas of mental, physical, social and daily life functioning need to be considered in order to improve intervention outcomes in substance user patients. The aim of the study was to assess health-related quality of life (HRQOL) in patients diagnosed with opioid dependence as compared to healthy controls and patients diagnosed with depression and schizophrenia. Methods: A total of 1,015 outpatients diagnosed with opioid dependence were investigated during 12 months of maintenance treatment. HRQOL (MSQoL), addiction (EUROP-ASI), and sociodemographic characteristics were assessed. Results: HRQOL in opioid dependence improved significantly (p < .001), but was lower as compared to that of healthy controls and patients diagnosed with schizophrenia. Severely opioid-dependent patients are at high risk of both acquiring and spreading the hepatitis C virus (HCV). It is uncertain, however, whether these patients are possible candidates for HCV treatment. We therefore explored treatment retention and adherence as well as sustained viral response in co-morbid severely opioid-dependent subjects under heroin maintenance, who previously failed in conventional substitution treatment or were not in any drug treatment.All patients in heroin maintenance in the German heroin trial, who received standard antiviral HCV therapy with pegylated interferon and ribavirin, were included. Co-consumption of licit and illicit drugs was tolerated as long as it did not interfere with treatment.Twenty-six patients in heroin maintenance were treated for chronic HCV infection. Both the Global Severity Index of the Symptom Checklist 90-R (average score 65.9) and the Opiate Treatment Index (average score 16.6) indicated relevant co-morbidity. Twenty-one patients (81%) were retained in treatment; the adherence rate was 92%. Eighteen patients (69%) achieved a sustained viral response, with a 100% response rate for genotype 2, 90% for genotype 3, and 42% for genotype 1.This is the first study that investigates the feasibility of antiviral HCV treatment in a well-defined sample of co-morbid severely opioid-dependent subjects in heroin maintenance treatment. Viral response rates are comparable to non-drug-user populations. Within a need-adapted treatment setting, HCV treatment may even be extended to difficult-to-treat opioid-dependent patients.

Objectives: Owing to the limited knowledge of the effect of cocaine use on the outcome of opioid dependence, a long-term follow-up study was conducted to assess the importance of cocaine use in the long-term course. Methods: A representative sample of 350 opioid-dependent patients in treatment was studied over a 4-year period using the European version of the Addiction Severity Index. The sample was split into 2 groups according to cocaine use at baseline: 1 group with regular cocaine use and 1 group with irregular or no cocaine use. Results: The group of regular cocaine using opioid-dependent patients had a significantly higher severity of dependence and a higher need for treatment at baseline compared to those with irregular or no cocaine use. At the 4-year follow-up examination no differences were found between the 2 groups. Conclusions: The lack of evidence of long-term effects weakens the predictive value of the negative effect of cocaine use on outcome.

This study aims to empirically explore the connection between parenthood and addiction in terms of the best interest and welfare of the children. Three groups of opioid addicts are compared with regard to fifteen risk indicators (concerning drug use, traumatic live experiences, social circumstances and health): parents who do have care of their children, parents not living with their children and drug users without children. The data set is derived from the documentary system of the Hamburg outpatient drug treatment services (n=4971). 30.3% of the patients have children. Only 35.9% of the parents live together with their children and exercise parenthood. Parents with children at home showed the lowest, parents with children living elsewhere the highest prevalence for almost all of the risk indicators. More appropriate help should be offered to drug using parents in order to empower them to keep their children. However, top priority must be given to the best interest and welfare of the children.

Fragestellung: Ziel dieser Studie war eine Bestätigung der Wirksamkeit einer diamorphingestützten Behandlung an einer größeren Stichprobe von Non-Responder in der Methadonsubstitution und unter Einbeziehungen einer zweiten Zielgruppe derzeit nicht in Behandlung befindlicher Opiatabhängiger (»Nicht-Erreichte«). Methodik: In einer randomisierten Kontrollgruppenstudie wurden 1.015 Schwerstopiatabhängige – 487 Non-Responder aus der Methadonsubstitution (MS) und 528 Nicht-Erreichte (NE) – entweder mit Diamorphin (N=515) oder mit Methadon (N=500) über 12 Monate behandelt. Ergebnisse: Die Haltequote war in der Diamorphingruppe (67,2 %) signifikant höher als in der Methadongruppe (40,0 %). In beiden Hauptzielkriterien erzielten Patienten in der Diamorphingruppe eine höhere Response als diejenigen in der Methadongruppe, ohne signifikanter Wechselwirkung mit den MS- und NE-Stratum. Schlussfolgerungen: Die Ergebnisse dieser Studie bestätigen die Effektivität der diamorphingestützten Behandlung bei Non-Respondern der Methadonsubstitution. Die höhere Wirksamkeit bei nicht in Behandlung befindlichen Schwerstopiatabhängigen könnte genutzt werden, um diese Patienten in das Suchthilfesystem besser einzubinden.

Fragestellung: Ziel der vorliegenden Studie war es zu untersuchen, ob und inwieweit Art und Setting, Inanspruchnahme und Intensität der psychosozialen Behandlung (PSB) einen Einfluss auf die Wirksamkeit der Behandlung hat. Methodik: Es wurde eine randomisierte und kontrollierte Therapiestudie mit einer Heroin- und Methadonvergabe und den psychosozialen Interventionen Gruppen-Psychoedukation/Drogenberatung (PE/DB) sowie Case Management mit Motivierender Gesprächsführung (CM/MI) durchgeführt. Untersuchungsgruppe: Insgesamt wurden 1.015 Patienten in die Studie aufgenommen. Von ihnen nahmen 394 Patienten an PE/DB und 404 Patienten an CM/MI teil. Ergebnisse: Die psychosozialen Interventionen unterschieden sich hinsichtlich der Wirkungen auf die Verbesserung des Gesundheitszustandes und die Verringerung des illegalen Drogenkonsums nicht signifikant voneinander. Außerhalb der randomisierten Zuordnung deutet ein Vergleich der Patienten, die sich in einem selbstselektiven Prozess für eine kombinierte Behandlung entschieden haben (medikamentöse Behandlung und PSB) mit den Patienten, die eine Substitutionsbehandlung ohne PSB wählten, auf einen Mehrwert der kombinierten Behandlung hin. Schlussfolgerungen: Sowohl Case Management mit Motivierender Gesprächsführung als auch ein psychoedukatives Gruppenprogramm wurden in der Substitutionsbehandlung mit Heroin oder Methadon erfolgreich eingesetzt und unterschieden sich nicht in ihrem Einfluss auf die primären Effekte der Studienbehandlung.
