1. Prevalence and screening of active tuberculosis in a prison in the South of Brazil

Abstract

SETTING Tuberculosis (TB) remains a challenge in Brazil, particularly among prison inmates. OBJECTIVE To assess TB prevalence by active case finding in a public prison in southern Brazil. DESIGN Prison inmates were screened for TB using the presence of cough and chest X-ray (CXR) from October 2014 to August 2016. Presence of cough, irrespective of duration, and abnormal CXRs were further investigated using laboratory tests. RESULTS Of 10 326 inmates screened, 196 had confirmed TB (1898/100 000 inmates screened). At the first screening, 1759 inmates presented with cough only, 16 of whom had TB; among those with only abnormal CXR (n = 1273), 92 had TB. Xpert was positive in 155 patients, and negative in 15; these results were confirmed using culture. The remaining 26 patients did not undergo Xpert testing and were confirmed using microscopy (27%), culture (42%) or both (31%). CONCLUSION The combined use of symptom screening (cough) and CXR was much more effective in maximising TB yield than using either method alone. If patients presenting with cough alone had not been investigated, 10% of TB patients would have been missed; if those with abnormal CXR but no cough had not been investigated, 51% of TB patients would have been missed. We detected high TB prevalence in this prison by using active case finding.

Abstract

Objectives Several plaque and lesion characteristics have been associated with an increased risk for procedural stroke during or shortly after carotid artery stenting (CAS). While technical advancements in stent design and cerebral protection devices (CPD) may help reduce the procedural stroke risk, and anatomy remains important, tailoring stenting procedures according to plaque and lesion characteristics might be a useful strategy in reducing stroke associated with CAS. In this descriptive report of the ongoing Asymptomatic Carotid Surgery Trial-2 (ACST-2), it was assessed whether choice for stent and use or type of CPD was influenced by plaque and lesion characteristics. Materials and methods Trial patients who underwent CAS between 2008 and 2015 were included in this study. Chi-square statistics were used to study the effects of plaque echolucency, ipsilateral preocclusive disease (90–99%), and contralateral high-grade stenosis (>50%) or occlusion of the carotid artery on interventionalists' choice for stent and CPD. Differences in treatment preference between specialties were also analysed. Results In this study, 831 patients from 88 ACST-2 centres were included. Almost all procedures were performed by either interventional radiologists (50%) or vascular surgeons (45%). Plaque echolucency, ipsilateral preocclusive disease (90–99%), and significant contralateral stenosis (>50%) or occlusion did not affect the choice of stent or either the use of cerebral protection and type of CPD employed (i.e., filter/flow reversal). Vascular surgeons used a CPD significantly more often than interventional radiologists (98.6% vs. 76.3%; p < .001), but this choice did not appear to be dependent on patient characteristics. Conclusions In ACST-2, plaque characteristics and severity of stenosis did not primarily determine interventionalists' choice of stent or use or type of CPD, suggesting that other factors, such as vascular anatomy or personal and centre preference, may be more important. Stent and CPD use was highly heterogeneous among participating European centres.

Abstract

Since the discovery of titanium silicalite more than 30 years ago, framework metal-containing zeotype materials have become an important class of catalyst, finding application in several industrial processes. Incorporation of cations of titanium, tin, iron, and other elements into zeotype frameworks (and also into ordered mesoporous materials) has led to both scientific progress and engineering innovations in catalysis. As a result of these developments, framework metal-containing zeotype materials have been implemented in the preceding decade in new commercial, by-product-free green processes, which have improved sustainability in the chemical industry. Based on a comprehensive analysis of the recent literature including patents, this review is a summary of the current knowledge of the science and technology of framework metal-containing zeotype materials. The synthesis of these materials is summarized, followed by an account of state-of-the-art characterization results. The key catalytic chemistries, which can be classified into oxidation reactions such as olefin epoxidation, aromatic hydroxylation and ammoximation, and weak Lewis acid-catalyzed reactions, are discussed. Mechanisms proposed for these transformations are reviewed, together with the theoretical and modeling tools applied in this context. An overview of the technologies associated with the use of framework metal-containing zeotype materials demonstrates how these processes are linked with each other through key chemicals involved.

Abstract

We describe an improved interconnect technology for the fabrication of multiturn flux transformers from YBa2Cu3O7-x-SrTiO3-YBa2Cu3O7-x multilayers. The essential improvements are reductions in the thicknesses of the trilayer films, typically to 100 nm, 250 nm and 250 nm respectively, and in the deposition rate, to 0.07 nm/laser pulse. This process yields crossovers in which the critical current density in the upper YBa2Cu3O7-x film at 77 K is (2-3)*106 cm-2 In situ trilayers exhibited 1/f flux noise levels at 1 Hz below the measurement sensitivity of 15 mu Phi 0 Hz-1/2, Where Phi 0 is the flux quantum. However the flux noise of trilayers in which each layer had been patterned was significantly higher. The best flip-chip magnetometer had a white noise of 40 fT Hz-1/2, increasing to 340 fT Hz-1/2 at 1 Hz; the corresponding flux noise levels were 9 mu Phi 0 Hz-1/2 and 75 mu Phi 0 Hz-1/2.

Abstract

Objective:To describe the first published case of superficial angiomyxoma with an epithelial component occurring in the retropharynx.Method:Case report of a patient with swallowing difficulties caused by a rare case of superficial angiomyxoma in the retropharynx.Results:Superficial angiomyxoma is a distinct entity among the dermal myxomatous lesions. Superficial angiomyxoma is poorly circumscribed, and local recurrence is common unless the tumour is excised with clear margins. Distinctive histological features include a myxoid mass composed of spindle and stellate-shaped cells and occasional multinucleated cells. There is prominent vasculature and a mixed inflammatory infiltrate in the stroma, particularly by neutrophil polymorphs. Epithelial structures are seen in about one-third of cases. A case of retropharyngeal tumour with morphological features of superficial angiomyxoma is reported. The tumour cells, including multinucleated ones, were negative for soft tissue differentiation markers. The inflammatory cells included lymphocytes, histiocytes and neutrophil polymorphs.Conclusion:This case demonstrates that a cutaneous type of angiomyxoma with epithelial-lined structures can occur in deep soft tissue, such as the retropharynx.

10. Long-term results of subtotal colectomy for acquired hypertrophic megacolon in eight dogs

Abstract

Objectives: To evaluate the long-term results of subtotal colectomy for acquired hypertrophic megacolon in the dog. Methods: Eight dogs with acquired hypertrophic megacolon underwent subtotal colectomy with preservation of the ileocolic junction. Long-term follow-up was obtained by clinical records and telephone interviews with the owners. Results: Eight large-breed dogs (age range: 6 to 12 years; mean age: 10·75 years) were enrolled. The use of bone meal, low levels of exercise, chronic constipation with dyschesia and tenesmus refractory to medical management were factors predisposing dogs to acquired hypertrophic megacolon. The diagnosis was confirmed in all animals by abdominal palpation, plain radiography and postoperative histopathological findings. There were no intraoperative complications. One dog died as a result of septic peritonitis. The clinical conditions (that is, resolution of obstipation and stool consistency) of the remaining seven dogs were improved at discharge; all animals returned to normal defecation in five to 10 weeks (mean: 7·3 weeks) and were alive 11 to 48 months (mean: 40·5 months) after surgery. Clinical Significance: Predominantly bony diet and/or low levels of physical activity may predispose dogs to acquired hypertrophic megacolon. Our results emphasise the long-term effectiveness of subtotal colectomy with preservation of the ileocolic junction in this condition.

Abstract

The authors describe the case of a fifteen-year-old girl with progressive paraparesis of the lower limbs that was caused by an intraspinal extradural dorsal arachnoid cyst at the level of Th 3-6. Diagnosis was established with MRI and MRI myelography. The latter revealed the CSF-like content of the cyst. The patient underwent laminotomy and en bloc resection of the cyst. Ligation of the pedicle of the cyst was done with laminoplasty. Quick and complete recovery was observed after surgery.

Abstract

We have fabricated YBa2Cu3O7−x–SrTiO3–YBa2Cu3O7−x (YBCO–STO–YBCO) trilayers, in which each layer is patterned photolithographically, capping the first YBCO film with an in situ STO film. Atomic force microscopy demonstrates that the capping process dramatically improves the quality of the surface of the second layer, allowing the growth of an upper YBCO film with a substantially reduced level of low‐frequency flux noise. A magnetometer with a multiturn flux transformer coupled to a dc superconducting quantum interference device achieved a magnetic field noise of 74 fT Hz−1/2 at 1 Hz, improving to 31 fT Hz−1/2 at 1 kHz.

Abstract

We present results on a near‐field scanning probe microwave microscope which is based on the nonlinear behavior of a YBa2Cu3O7 direct‐current superconducting quantum interference device (SQUID). Our system has a spatial resolution of about 30 μm and derives its probing field from circulating currents in the SQUID loop. The frequency is set by the Josephson relation and is continuously tunable up to about 200 GHz. By imaging small patterned normal metal thin‐film samples of Cu and Nb at 77 K, we have confirmed the operating bandwidth of the system and investigated the nature of the imaging process.

Abstract

We have used a scanning magnetic flux microscope based on a high transition temperature YBa2Cu3O7 superconducting quantum interference device (SQUID) to produce magnetic images of eddy currents in patterned Cu thin films and 11–30‐μm‐thick Cu on printed circuit boards. The fields produced by the eddy currents are imaged with a spatial resolution of about 80 μm over a 100‐mm2 sample area. With the sample and SQUID at 77 K, the microscope uses typical probing fields of 80 nT and can obtain simultaneously eddy current and static magnetic field images. At probing frequencies of 26–100 kHz, the system achieves a field sensitivity of about 7 pT Hz−1/2.

Abstract

We have fabricated magnetometers patterned in a single layer of YBa2Cu3O7−x involving dc superconducting quantum interference devices (SQUIDs) with bicrystalline grain boundary junctions. The magnetometers consist of either a SQUID with a large area square washer or a single turn pickup loop coupled directly to the body of a small area SQUID. We found that the transfer function falls off with increasing SQUID inductance much more rapidly than predicted; implications for magnetometer performance are discussed. When operated at 77 K with a bias reversal technique a directly coupled magnetometer had a noise of (105±10) fT Hz−1/2 at 1 kHz, increasing to (145±10) fT Hz−1/2 at 1 Hz.

Abstract

We have developed a magnetic flux microscope which uses a liquid nitrogen cooled thin‐film YBa2Cu3O7 dc superconducting quantum interference device (SQUID) to produce two‐dimensional images of magnetic fields. The instrument operates as a scanning probe microscope with a spatial resolution of about 80 μm and a field of view of about 100 mm2. The equivalent magnetic field noise in the SQUID ranges from 80 pT Hz−1/2 at 1 Hz to 20 pT Hz−1/2 at 1 kHz, yielding a field resolution of about 200 pT in our images. We have used the microscope to image magnetic domains in ferromagnetic samples, small currents flowing in fine wires, and trapped flux and diamagnetic susceptibility in superconducting thin films.

Abstract

We have developed scanning probe magnetic microscopes which use dc Superconducting QUantum Interference Devices (SQUIDs) to obtain images of the magnetic field above a sample surface. In our instruments, the SQUID is held fixed and the sample is scanned in a raster pattern by means of a computer-controlled cryogenic positioning mechanism. We record the output of the SQUID as a function of sample position and use this to construct gray-scale or false color images of the magnetic field above the sample. We presently have two microscopes which can scan in 3D; one microscope uses a high transition temperature ((Tau) c) YBa2Cu3O7 SQUID while the other microscope uses a low-(Tau) c Nb-PbIn SQUID. Our high-(Tau) c microscope operates with the sample and SQUID in liquid nitrogen and typically achieves a spatial resolution of about 20-80 micrometers and a magnetic field resolution of about 20-200 pT for a 1 second average. Our low-(Tau) c microsope operates with the sample and SQUID in a liquid-helium cooled vacuum space and is designed to allow the imaging of samples at different temperatures. Abstract

Purpose: Valganciclovir dosing recommendations for the prevention and treatment of CMV infections in children are based on patient renal function and either body surface area or weight. Infants and young children have a different pharmacokinetic profile than their adult counterparts and high-risk patients may benefit from individualized dosing guided by serum drug levels to achieve Area Under the Curve (AUC) goals that have been established as effective in the treatment and prevention of CMV disease in adults. Methods and Materials: A pharmacokinetic study was performed in pediatric solid organ transplant recipients between the ages of 6 months and 3 years in whom AUCs had been measured. Results: There was a strong corrlation between a single 2-hour plasma level and AUC (R 0.6) suggesting that this one measurement might suffice in clinical practice. No patients became neutropenic or experienced significant renal dysfunction secondary to valganciclovir therapy. AUC modeling suggests that the current package insert recommendations may not result in reliably appropriate valganciclovir blood levels for young patients.

Abstract

This survey was conducted on young dental students, with the aim of evaluating the influence of scientific information dispensed during their studies on their own oral state and on their behavior towards it. Therefore, various parameters were considered at the very beginning of their university studies and at the end of their studies. A questionnaire was used to collect information on the socioeconomic origin and oral hygiene habits of the students. In order to assess actual improvement in oral health standardized clinical examinations were utilized. The comparison of parameters measured at the beginning and at the end of the clinical studies of the students revealed that the OHI-S and the GI scores had decreased highly significantly (P less than 0.01), and that the DMFT score had increased significantly (P less than 0.01). The increase in the DMFT score was mainly due to a high number of fillings. The answers to the questionnaire were indicative of a favorable modification in hygiene habits. At the end of their clinical studies the students were on an average more aware of their oral health, the quality and the duration of their toothbrushing had improved, and they more often sought early treatment and preventive care than at the beginning of their studies.

Abstract

A methodology and a tool to support the early stages of model development is presented. The model development basis is conservation of extensive thermodynamical properties, called quantities. The conceptual basis for the graphical editor layout is description of dynamic interactions between process quantities and interactions from the environment in a so called DYnamic QUantity Interaction Diagram (DYQUID). The DYQUID model representation forms a superset of a large number of dynamic process model representations, such as locally linear model structures, qualitative differential equations and conventional nonlinear state space models. The prototype version of the editor is object oriented, and is written in C++. The editor works with objects which fit engineering notions. Each object has its graphical representation. The editor is equipped with generic function types for transfer and reaction mechanisms. The editor may be expanded with additional function types suitable for a pertinent application area. The editor is interactive, driven by menus and questionnaires. One purpose of this experimental version of a DYQUID editor is to obtain experience with the efficiency of the user interface for facilitating development of different types of mutually consistent model representations.
