This paper discusses the analysis of a Quasi Z source inverter with a trouble-free boost control tactic for renewable energy applications. The non-availability of uninterrupted renewable energy sources in atmosphere weather conditions raises variation in output voltage and currents. This may affects the performance of conventional inverters with potential source (VSI), current source connected (CSI) and impedance source inverter (ZSI). To overcome the drawback of VSI, CSI and ZSI a quasi-Z source inverters (QZSI) are used. The QZSI performs multiple operations such as DC-to-DC, AC-to-AC, AC-to-DC and DC- to-AC conversions. Also, it may be used for each buck and boost operation, using the shoot through state. The QZSI is derived from the ZSI topology, with a minor modification of the impedance network and overcomes the troubles facing in ZSI. The design construction and simulation study of quasi-Z source inverter is performed using MATLAB/Simulink environment.

The condition of hyperlipidemia is found to be a great establisher for all the negative health consequences which may lead to cardiac complications. Continuous usage of medication alone is not permanent remedy but also need physical exercise. The same situation was established in animals to assess the performance of pharmacological and non-pharmacological profile by standardized screening using In-vivo methods. The high-cholesterol diet caused a significant increase in total lipids, total cholesterol (TC), total triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), and the atherogenic index, whereas the level of high-density lipoprotein cholesterol (HDL-C) was significantly decreased for the treatment groups intended for physical exercise. This article enables the importance of health benefited by physical performance by depicting the biochemical parameters. Health is a fundamental force that decides the overall quality of human life. The health of the population is a vital and essential issue for any country. The new National Health Policy of India (2017) emphasizes the growing burden of non- communicable diseases and corresponding growing expenditure. Indian Government announced the world's most massive health scheme, the Ayushman Bharat Yojana, in 2018, which is a significant step towards attaining universal health coverage in India. This scheme provides a cover of Rs.5 lakh per family per year for various medical procedures. Although India's Government is taking many such initiatives to improve public health, a lot more needs to be done to attain universal coverage. Out of pocket expenditure paid by individuals towards health care in India ranges at a high percentage over the years. Although health insurance premium has shown an increase over the years, there is no drop in this expenditure. This study shows the phenomenal growth achieved by health insurance, specifically after privatization, premium growth, and improvement in claims ratios. It is suggested that the insurance companies should modify the coverage of health insurance policies so that the uncovered expenses are reimbursed during hospitalization. Insurance companies also should shift their focus towards wellness instead of covering sickness to keep their businesses sustainable in the long run.

Beeswax is the substance that is obtained from the structure of a honey comb. The bees secrete wax to build the honey combs to store honey. It has rich hydrophobic property, the beeswax is in fact present within cosmetics and body products. Also, beeswax is used in food industry. Beeswax is a complex mixture of hydrocarbons, free fatty acid, esters of fatty acid. Beeswax plays an important role in Ayurvedic medicine, the traditional medicine name with Madhushistha (Beewax). The Madhushistha is used for the care of wound from abrasion or even from burns with topical application. Some references in Ayurvedic granth highlight the use of Madhushistha in combination with other herbal and mineral mixture, like Sikta Taila, Mruddaharshringa, Jatyadi Taila, etc. Beeswax is mainly of two types, natural beeswax and processed beeswax. Natural beeswax is clarified further heating in water with petroleum waxes. Because of high demand and shortage in market, adulteration of beeswax with cheaper materials are common like animal fat, plant oil and petroleum spirits (Paraffin wax). Hence authenticity of beeswax is major concern and that can be determined by using physico-chemical parameters such as- organoleptic test, solubility, Melting point, Spacific gravity, Refractive index, Acid value, Ester value, Saponification value, volatile matter. With this justification the present study is expected to determine the quality status of four different beeswax samples procured from Mumbai market, western ghats (Natural habitat). The physico-chemical analysis revealed that volatile matter is detected in market sample (2) whereas market sample (1) shows higher melting point as compare to beeswax collected from the Natural habitat. Market sample (1,2) of beeswax shows slight adulteration hence natural habitat samples (1,2) of beeswax should be used in medicine preparation.

We report one year (2013–2014) of biomonitoring an insect community in a tropical old-growth rain forest, during construction of an industrial-level geothermal electricity project. This is the first-year reaction by the species-rich insect biodiversity; six subsequent years are being analyzed now. The site is on the margin of a UNESCO Natural World Heritage Site, Área de Conservación Guanacaste (ACG), in northwestern Costa Rica. This biomonitoring is part of Costa Rica’s ongoing efforts to sustainably retain its wild biodiversity through biodevelopmental integration with its societies. Essential tools are geothermal engineering needs, entomological knowledge, insect species-rich forest, government–NGO integration, common sense, DNA barcoding for species-level identification, and Malaise traps. This research is tailored for integration with its society at the product level. We combine an academic view with on-site engineering decisions. This biomonitoring requires alpha-level DNA barcoding combined with centuries of morphology-based entomological taxonomy and ecology. Not all desired insect community analyses are performed; they are for data from subsequent years combined with this year. We provide enough analysis to be used by both guilds now. This biomonitoring has shown, for the first year, that the geothermal project impacts only the biodiversity within a zone less than 50 m from the project margin.

The present article examines the trend in growth of workforce and the change in its structure in India during the last three decades using census data. The analysis indicates that there was a fast decelerating rate of growth in overall workforce, particularly females, between 2001 and 2011. But the work participation rate did not decline, if not increased, as the rate of growth in workforce was not less than that in population. Secondly, incremental workforce, especially the male, reduced to marginal workers category, whereas the high concentration of female in the category of marginal workers slightly reduced. Occupational distribution of workforce shows that cultivators were declining. Such a decline in agriculture was replaced by increasing agricultural labour. The study further shows that growth of workforce in non-agriculture was higher than that in agriculture. Growth of female workers engaged in non-agriculture was higher than their male counterparts. The most common soft tissue sarcomas of hand are epitheloid sarcomas, synovial sarcomas and malignant fibrous histiocytomas which are high grade, aggressive tumors. Liposarcomas of the hand are extremely rare and to the best of our knowledge less than 20 cases have been reported so far in the literature. Well differentiated spindle cell liposarcoma is an extremely rare subtype of well differentiated liposarcoma/atypical lipomatous tumor which is different from the other subtypes clinicopathologically, genetically and prognostically. Palmar localization of this rare variant of liposarcoma at this site makes ours only the second case reported in the world literature so far. This recently described entity is low grade lesion and the hand surgeons need to adopt a less aggressive approach in the treatment of this subtype.

Background: Gallstones are common in Indian population and its treatment has shown a decisive shift from open to minimally invasive route. There is no doubt that laparoscopy require longer and steeper learning curve and higher cost, especially in the absence of health insurance to majority of suburban and rural Indian population. However, preferences of patients are changing rapidly due to better level of awareness and availability of healthcare facility. Aims was to study safety and efficacy of laparoscopic cholecystectomy in patients of cholelithiasis by comparing withresults of mini laparotomy cholecystectomy by comparing use of post-operative analgesia, operative Time, post-operative hospital stay, morbidity and mortality.Methods: It is a prospective randomized study of 100 Patients of cholelithiasis aged between 25 years to 65 years operated during 2016-2017 at Dr. D Y Patil medical college Pune. They were divided into mini laparotomy and laparoscopic cholecystectomy group by draw a lot method. Patient’s written valid informed consent for the particular procedure was taken and the pros and cons of both the procedure were explained in detail to the patient. We report four cases who were referred to Dr. D. Y. Patil Medical Hospital, Pune suffering from abdominal pain and gastrointestinal manifestations. The patients' history was unremarkable, except that they had contact with dogs and live in rural communities. Laboratory findings showed peripheral blood eosinophilia, leukocytosis, and elevated liver enzymes. Ultrasonography showed well-circumscribed cystic masses in the liver. Diagnosis of hydatid cysts was confirmed by computed tomography (CT). Surgical treatment was performed and all patients recovered well. The results of these cases support the nation that CT scan can led to increased clarity, regarding surgical management, because of discordance between radiographic and laboratory findings.

Entomophthoromycosis is a rare entity. We hereby report a case of entomophthoromycosis in a three-year-old Asian child who presented with a painless, nontender, rapidly increasing large swelling on the thigh of six months duration, which was initially misdiagnosed as a soft tissue tumor and resected. The cause of misleading diagnosis was rapid growth of the lesion in a short duration of time, indicating the possibility of a tumor. Histopathological examination revealed an inflammatory lesion with aseptate fungal hyphae and the characteristic Splendore-Hoeppli phenomenon. Microbiological examination identified the fungus as Basidiobolus ranarum. Complete excision of the lesion followed by antifungal therapy was associated with complete recovery. Entomophthoromycosis should be considered early when children from endemic areas present with unusual, rapid-growing lesions of the subcutaneous region. In order to emphasize tumor-like presentation of zygomycosis, we are presenting this case.

Between January 1980 and May 1988, 1363 patients underwent first time coronary artery bypass grafting (CABG) and 49 (3.6%) were reoperated for recurrent angina by the same surgeon in this unit. There were 42 males and five females with a mean age of 53.6 years. Mean time since first operation was 68.9 (37 SD) months. Prior to surgery only eight (16.3%) were still working. There were three hospital deaths and two late deaths at 2.5 and 3.5 years after surgery and one patient underwent cardiac transplantation at 1.5 years. Follow-up has been completed up to March 1989, mean 3.7 years, range 10–108 months. Questionnaires were sent to all patients concerning the financial and functional situation. Hospital expenses were calculated (a) prospectively for 15 consecutive first time CABGs and for five second time CABGs, (b) retrospectively from patients' records with the help of a simple computer programme. The final figures were corrected for inflation rates. The 1- and5-year actuarial probability of angina-free interval was 87.5% and 40% respectively. Eight patients (12.2%) had no benefit, six (13.6.) had benefit but were not able to work, 16 (36.4%) were able to work but unemployed and 14 (13.8%) were fully employed producing a total salary of £69 072 per annum. Hospital expenses were. first time CABG £3645–£4049, second time CABG £6290–£7235. The total for the entire group was £308 166. Repeated coronary surgery can be accomplished with reasonable mortality and has good late results. However financially, in our region it is not as cost effective, being 1.8 times more expensive and half as good as a first operation at restoring patients to work.

Background: Ovarian cancer is the third leading site of cancer among women, trailing behind cervix and breast cancer. Aim: This study was undertaken to analyze the immunohistochemical (IHC) profile of estrogen receptors (ER), progesterone receptors (PR), Ki-67, and p53 in various ovarian epithelial tumors and attempt correlation with clinical and histopathological findings. Materials and Methods: The present study was conducted over a period of 4 years. A technique of manual tissue array was employed for cases subjected for IHC. The primary antibodies used were ER, PR, p53, and Ki-67. A correlation was attempted between histopathological and IHC findings. Results were subjected to statistical analysis. Software program "the primer of biostatistics 5.0" was used for calculation of interrelationships between the analyzed ER, PR, p53, and Ki-67 expression and histological factors by Pearson′s Chi-square test. The results were considered to be significant when the P < 0.05. Results: There were 110 cases of surface epithelial ovarian tumors (SEOT) encountered over the period of 4 years. The expression of ER was more in malignant tumors (13/16, 81.25%) than borderline (9/12, 75%) and benign (20/82, 24.39%). As compared to ER, the expression of PR was more in benign (51/82, 62.19%) than borderline (8/12, 66.67%) and malignant tumors (9/16, 56.25%). The expression of PR was more in benign tumors than borderline and malignant tumors. However, this was not statistically significant (Chi-square = 0.335 with 2 degrees of freedom; P = 0.846). The expression of p53 was less in benign (5/82, 6.1%) than borderline (9/12, 75%) and malignant tumors (13/16, 81.25%). The expression of Ki-67 was more in malignant (4/82, 4.88%) than borderline (10/12, 83.33%) and benign tumors (15/16, 93.75%). In all the above cases, the difference was statistically significant (P < 0.05). There was statistically significant difference in the expression of ER, PR, p53, and Ki-67 in the patients with age

Benign nontraumatic inflammatory stricture of Common bile duct (CBD) may result in obstructive jaundice, which can be misdiagnosed as a malignant tumor of the CBD preoperatively. A case with stricture of the common hepatic duct presenting with obstructive jaundice is reported herein. Preoperative radiological studies prompted us to confidently make the diagnosis of cholangiocarcinoma. However, the postoperative diagnosis on histological examination of the resected lesions was chronic inflammation and fibrosis.

Primary localized congenital sacrococcygeal neuroblastomas (SCNs) are rare. Diagnosis is based on histological and immunohistochemical evaluation, which is indispensable not only for determining tumor type but also for predicting biological behavior. We report a rare case of congenital SCN in a 9-month-old baby girl. Based on clinical and radiological findings, a provisional diagnosis of solid variant of sacrococcygeal teratoma (SCT) was made. The swelling was entirely excised. On histopathological examination, diagnosis of neuroblastoma, differentiating type in the sacrococcygeal region was considered. On immunohistochemistry, the tumor cells showed immunoreactivity for markers such as neuronspecific enolase, chromogranin-A, synaptophysin, and cyclin D1. S-100 showed positive cytoplasmic immunoreactivity. CD99, leucocyte common antigen, PanCK, and epidermal growth factor receptor were nonreactive. Cyclin D1 showed strong nuclear immunoreactivity. p53 was negative and Ki67 labelling index was less than 1%. The immunohistochemical markers studied, confirmed the histopathological diagnosis, and the cell proliferative index markers indicated it to be a very low grade lesion. Postoperatively, the child is disease-free and has achieved normal milestones for age for period of 6 months.
