Abstract

This study aims to understanding how the practice of collective recycling work is learned, produced and reproduced, based on a case study in front a recycling association. The construction of empirical data was based on the zoom in and zoom out method, operated through ethnographic techniques. The analysis was developed through textual analysis and the use of templates. The data reveals that practitioners give new meaning to the practices of collective recycling work through negotiation and reflexivity, suspending the flow of the previous action and legitimizing a new repertoire of normative constructions, recognized together. It is concluded that the way of organizing and learning of the recyclers provides the construction and reconstruction of practices over time, produced in the interactions between subjects, knowledge and materiality, making the practice of collective recycling work remain in constant refinement. The contributions of these findings, especially in regard to collective work, reveals nuances about the organizing and learning process, supporting managers to overcome challenges facing social projects based on learning through practice.

Abstract

This study aimed to analyze how a recycling project, as an association of waste pickers, can promote the reintegration of individuals into society. The method was qualitative, descriptive research developed through a single intrinsic case study, and the data were treated through interpretative textual analysis. It was evidenced that the social development project studied, directed to the association of recycling Alfa, has been positively influencing the life of the collectors since it appears as an opportunity for reallocation in society, in which the value obtained with the work has been satisfactory. There is growing individual and collective, as well as receive more support and a more organized structure to develop their activities. As a contribution, it is pointed to a working relationship and projects, and its importance for society is highlighted, being the agents influencing economic and environmental development, the search for spaces of recognition, and social valorization.

Abstract

O presente trabalho tem por objetivo compreender a transformação do papel social do contador dentro das organizações privadas. Com o advento dos processos internos mais automatizados, o acesso à informação nas organizações é feito de forma cada vez mais rápida, visto que existem rotinas parametrizadas. Em função disso, a figura do profissional contábil hoje se encontra na ponta final de um processo produtivo, responsável pela análise conclusiva dos dados, enquanto a estrutura base das informações iniciais procede de sistemas desenvolvidos por departamentos de tecnologia da informação. Para isso, foram entrevistados oito profissionais experientes de Contabilidade, abordando questões relacionadas ao seu inicio de carreira, inserção no mercado contábil, habilidades necessárias para o exercício da profissão e mudanças a partir da tecnologia nas rotinas contábeis. Com base nas análises dos resultados obtidos, foi possível inferir uma tendência de transformação do profissional contábil para um perfil mais analítico, reestruturando sua responsabilidade em diagnósticos relacionados à tomada de decisão na instituição e, consequentemente, seu papel como profissional incumbido de contribuir com informações para tomada de decisão. Na percepção dos entrevistados, a tecnologia e o processo de convergência contábil foram os impulsionadores de tais mudanças.

A Governança de Tecnologia da Informação (TI) tem-se tornado importante nas organizações, pois facilita a tomada de decisões por meio do alinhamento da área de TI com a área de negócio (foco) da organização. No entanto, a governança de TI é um processo lento e caro em termos de infraestrutura e recursos. Neste contexto, este artigo apresenta um estudo de caso, utilizando a metodologia de pesquisa qualitativa-quantitativa, sobre a governança de TI em um campus avançado da Universidade Federal de Santa Maria (UFSM-FW) e propõe uma reorganização dos serviços de TI com suporte em ferramentas de código aberto. O estudo demonstrou a necessidade de uma reestruturação do setor de TI e que o apoio de ferramentas para a automação pode tornar a UFSM mais gerenciável.

Abstract

A sustainability-oriented logic is required in order to change the consumerist logic of the society. This is possible by changing the beliefs and values shared among stakeholders regarding their behaviors. The institutional logic of sustainability (ILS) concept emerges in response to this need. In the context of the sharing economy, this research aims to understand the formation of ILS with the implementation of shared bikes in the city of Fortaleza-CE. Therefore, an in-depth case study was developed with actors that developed the Bicicletar project. The results demonstrate that: (a) several events happened to enable the insertion of the project; (b) city hall played a prominent role as a stimulator of change; (c) the city's mobility structure has been modified; and (d) sustainability practices are observed in all dimensions, with an emphasis on the environmental one. With this discussion, we contribute to the understanding of how sustainability can be practiced in other contexts. There is also the contribution of theoretical elements that facilitate understanding of the sustainability concept.

Abstract

O objetivo deste artigo é analisar o impacto na utilização de diferentes estratégias de ensino-aprendizagem pelos professores, levando-se em conta a forma de estudo que se pretende que os alunos adotem em um curso de pós-graduação em Ciências Contábeis, entre elas as abordagens superficial e profunda de aprendizagem. Foram testadas duas modalidades didáticas durante duas aulas, sendo: uma aula tradicional, composta de aula expositiva e resolução de exercícios (aula 1); e uma aula prática, tendo sido desenvolvido material a ser utilizado em sala de aula com apoio da Taxonomia de Bloom e das teorias sobre as abordagens de aprendizagem (aula 2). Nessa proposta, entende-se que a aula tradicional estaria mais ligada a uma abordagem superficial de aprendizagem, enquanto o material produzido para a aula 2 levaria ao alcance de uma abordagem profunda de aprendizagem, dado que propicia maior participação e interação dos alunos com o conteúdo proposto. Como resultado, não foram percebidas diferenças significativas em relação às duas modalidades didáticas. No que tange à abordagem superficial e profunda, a modalidade proposta para a aula 2 foi mais efetiva no sentido de garantir a participação dos alunos, bem como seu envolvimento no sentido de buscar aplicações práticas e estabelecer relações entre a teoria e a prática. Já no que diz respeito às categorias inerentes ao domínio cognitivo da Taxonomia de Bloom, os dois métodos se mostraram eficazes no alcance das diferentes vertentes investigadas, a saber, aula expositiva e aula prática. Ressalta-se, no entanto, que a segunda modalidade, aula prática, permite desenvolver objetivos relacionados também ao domínio afetivo da taxonomia.

Abstract

A temática da sustentabilidade tem alterado o cenário estratégico das empresas diante de várias mudanças. Uma destas diz respeito às transformações nos critérios de decisão dos consumidores, que começam a predispor-se a tornar o verde a sua prioridade, atraídos por marcas que possuem princípios verdes. A partir disso, as empresas passaram a usar estratégias de comunicação dos seus princípios sustentáveis diretamente nas embalagens de seus produtos. Diante disso, o objetivo deste estudo foi verificar os impactos da comunicação verde na identidade corporativa e propensão de compra. Para tanto, realizou-se um estudo de caráter quase-experimental e com design fatorial da ordem de 2 (Embalagens e Selos Verdes) x 2 (Identidade Corporativa e Propensão de Compra), manipulando a presença ou ausência do tipo de apelo utilizado (embalagem ou selos verdes). Como principais resultados, o estudo mostra que a identidade corporativa é percebida positivamente mediante a presença tanto dos selos quanto pelas embalagens verdes – sendo os primeiros mais efetivos – o que também influencia, de maneira positiva, na propensão de compra. EFFECTS IN CORPORATE IDENTITY AND PURCHASE PROPENSITY ABSTRACT The sustainability issues have changed the strategic environment of companies facing many changes. One of these being related to changes in the consumer’s decision criteria that have begun to promising the green being acted to brands that have linked green principles. Since that the companies started to use communication strategies for their sustainable principles directly on the packaging of their products. Thus, the objective of this study was to verify the impact of green communication on corporate identity and propensity to purchase. Therefore, we carried out a quasi-experimental study and with the factorial design of the order 2 (Packaging and Green Stamps) x 2 (Corporate Identity and Willingness to Purchase), manipulating the presence or absence of the type of appeal used (packaging or green stamps). As the main results of the study shows that corporate identity is perceived positively by both, the presence of seals for packaging as green - the former being more effective - which also affects, in a positive way, the propensity to buy. RESUMO Como resultado do avanço tecnológico e da velocidade com que as informações são processadas, a cada momento a sociedade passa a dispor de novas tecnologias, desenvolvidas com o propósito de facilitar o trabalho dos indivíduos. Frente a este cenário, este estudo teve como objetivo identificar os fatores que explicam o uso de um sistema de informação, a partir dos modelos de aceitação à tecnologia, TAM e TTF, considerados consenso na literatura desse tema. Assim, realizou-se uma pesquisa, através de uma abordagem qualitativa e descritiva, por meio desses modelos que, na maioria dos casos, são aplicados em análises de natureza quantitativa. Com relação ao método para realização desta pesquisa, utilizou-se um estudo de caso em uma instituição pública. A análise dos resultados obtidos demonstrou que todos os fatores avaliados, ou seja, utilidade percebida, facilidade de uso, intenção de uso e ajuste da tecnologia à tarefa explicaram o uso do sistema estudado. Desta forma, percebe-se que o uso de um SI não está relacionado somente com o ajuste entre a tarefa e a tecnologia, ou com o comportamento e as atitudes dos indivíduos frente aos sistemas, mas também com a combinação desses fatores (Klopping e McKinney, 2004). Palavras-chave Sistema de informação, Uso, Modelos de aceitação ABSTRACT As a result of technological advances and the speed at which information is processed, every moment society starts to count on new technologies developed for the purpose of making the work of individuals easier. Concerning this scenario, this study had the objective to identify the factors which explain the use of a managerial information system from the technology acceptance models, TAM and TTF, considered a consensus in the literature about the theme. Thus, a research of qualitative and descriptive approach was conducted by means of these models which, in most cases, are applied in quantitative analysis. Relating to the method for carrying out this research, a case study was used in a public institution. The analysis of the results which were obtained showed that all the factors evaluated, that is, noticeable utility, facility of use, intention of use and adjustment of technology to task were relevant to explain the use of the system being studied. In this way, it is noticed that the use of an Information System is not only related to the adjustment between task and technology, or to the individuals’ behavior and attitude toward the systems, but also to the combination of these factors (Klopping & McKinney, 2004). 10. Differential impact of hospital and community factors on breadth and depth of hospital population health partnerships

Abstract

Objective: The aim was to identify hospital and county characteristics associated with variation in breadth and depth of hospital partnerships with a broad range of organizations to improve population health. Data Sources: The American Hospital Association Annual Survey provided data on hospital partnerships to improve population health for the years 2017-2019. Design: The study adopts the dimensional publicness theory and social capital framework to examine hospital and county characteristics that facilitate hospital population health partnerships. The two dependent variables were number of local community organizations that hospitals partner with (breadth) and level of engagement with the partners (depth) to improve population health. The independent variables include three dimensions of publicness: Regulative, Normative and Cultural-cognitive measured by various hospital factors and presence of social capital present at county level. Covariates in the multivariate analysis included hospital factors such as bed-size and system membership. Methods: We used hierarchical linear regression models to assess various hospital and county factors associated with breadth and depth of hospital-community partnerships, adjusting for covariates. Principal Findings: Nonprofit and public hospitals provided a greater breadth (coefficient, 1.61; SE, 0.11; p < 0.001 and coefficient, 0.95; SE, 0.14; p < 0.001) and depth (coefficient, 0.26, SE, 0.04; p < 0.001 & coefficient, 0.13; SE, 0.05; p < 0.05) of partnerships than their for-profit counterparts, partially supporting regulative dimension of publicness. At a county level, we found community social capital positively associated with breadth of partnerships (coefficient, 0.13; SE, 0.08; p < 0.001). Conclusions: An environment that promotes collaboration between hospitals and organizations to improve population health may impact the health of the community by identifying health needs of the community, targeting social determinants of health, or by addressing patient social needs. However, findings suggest that publicness dimensions at an organizational level, which involves a culture of public value, maybe more important than county factors to achieve community building through partnerships. Abstract

Higher education institutions are moving towards highlighting the importance of research. According to the Department of Higher Education and Training in South Africa, the status of higher education institutions will be determined by the extent to which they are engaged in research and research-related activities. Higher education institutions have a role to play in generating new knowledge as well as producing appropriately skilled professionals. The current study explored key stakeholders' perspectives of the role of the research units within a faculty of health sciences that is clinically driven, and how these units could contribute towards developing and strengthening interprofessional postgraduate research, collaboration and capacity development amongst staff. The study adopted a qualitative, exploratory descriptive approach. Data were gathered from individual face-to-face, in-depth semi-structured interviews with 15 participants. Five themes emerged from the thematic analysis, namely: i) 'There's been intentions and there is the reality'; ii) 'Driving the research agenda and pulling it together'; iii) 'The stronger your base, the stronger your output, the stronger your future: Creating a succession pipeline'; iv) 'It takes a specific kind of personality to run a Unit' and v) 'The climate has changed, views have changed'. The findings of the study clearly indicated that the stakeholders perceived the role of the Units differently. The establishment of a 'Faculty Research Centre' with a contextually relevant framework or model could contribute towards developing a clear understanding and consistent description of a research centre. 16. Measuring Financial Conditions using Equal Weights Combination

Abstract

In this paper, we assess the merits of financial condition indices (FCIs) constructed using equal weights averaging versus alternatives that use data reduction techniques, like principal components, or that allow for time-varying parameters. Our analysis is based on data for 18 advanced and emerging economies at the monthly frequency covering about 70% of the world's GDP. We study the performance of these indicators based on their ability to capture tail risk for economic activity and to predict banking and currency crises. We find that averaging with equal weights produces FCIs that are not inferior to, and often perform better than, those constructed with more sophisticated statistical methods. For the USA and for the euro area, based on the same evaluation criteria, they also work better than two popular alternatives that receive wide attention in policy discussions, namely the Chicago Fed National Financial Conditions Index and the Composite Index of Systemic Stress. O processo de difusão de novas tecnologias tem transformado o atual cenário empresarial, uma vez que ameaça a sobrevivência de empresas e promove novos mercados. Para enfrentar novos desafios, portanto, empresas apóiam-se na inovação tecnológica para capturar mercados e criar vantagens competitivas. Entretanto, a despeito desse esforço, é necessário o incentivo por parte do Poder Público nas empresas. O presente artigo trata de um estudo de caso realizado num município inovador (Santa Rita do Sapucaí, parte integrante do "Vale da Eletrônica"), objetivando compreender o papel do Poder Público na promoção da inovação tecnológica em uma incubadora de empresas. A pesquisa conclui que a parceria entre o Poder Público e a incubadora de empresas pode ser uma tentativa de amenizar o problema de desemprego estrutural vivenciado no Brasil, além de propiciar vantagens para as empresas abrigadas e para a economia da região, uma vez que produz pesquisa e desenvolvimento econômico e social.

Abstract

Given the specificities of the phenomenon of labor platformization in a country that occupies a position of dependence within the capitalist system, the objective of this article is to identify the alternative forms and dynamics of political organization and collective mobilization adopted by platform drivers and delivery workers in Brazil. The path chosen in the study reported here required not only an analysis of the national social context, in interaction with neoliberal rationality, but also an analysis of the literature evoking the difficulties of union organization among precarious workers. The analysis focused on current experiments, which are part of a search for alternative forms of collective organization for platform workers to traditional unionization. From a theoretical-methodological standpoint, the ergological perspective adopts an integrated approach at the macro and institutional levels and at the micro level of the collective dynamics of workers in their daily struggles. Involved in this research were the leaders, animators and members of two associations, two collectives opting for self-management and a network of self-organizing workers. The options and obstacles encountered in building organization and joint action were identified. We found that the cohesive strength of the collective entities analysed was based in the sharing of values that support the course of joint action. Thus, in the face of the heterogeneity of these groups of platform workers, and by adopting a less restrictive conception of what is covered by a professional or economic category, we raise the possibility of the formation of alternative movements and organizational forms that could prove to be conducive to their interests. Dans le cadre de la plateformisation du travail, l'article est consacre aux defis d'une construction de l'action collective de livreurs et de chauffeurs de centres urbains au Bresil. Le contexte sociopolitique et economique de dependance au sein du systeme capitaliste dans lequel agit ce groupe social, en interaction avec la rationalite neoliberale, contribue a expliquer les difficultes d'organisation syndicale de ces travailleurs. L'echantillon d'enquete reunit des dirigeants, des animateurs et des membres de collectifs en quete de meilleures remunerations et conditions de travail, ainsi que d'alternatives aux plateformes marchandes. Ils integrent deux associations, deux collectifs autogeres et un reseau autonome de travailleurs. En se fondant sur l'analyse des entretiens reunis, des hypotheses sont avancees concernant ces formes alternatives d'organisation, de representation et de mobilisation, en tenant compte de la composition heterogene de ces entites collectives. Compte tenu des specificites du phenomene de la plateformisation du travail dans un pays qui occupe une position de dependance au sein du systeme capitaliste, l'objectif de cet article est d'identifier les formes et les dynamiques d'organisation politique et de mobilisation collective adoptees par des chauffeurs et des livreurs de plateforme au Bresil. La recherche presentee s'appuie sur une analyse du contexte social national, en interaction avec la rationalite neoliberale, mais egalement sur une analyse de la litterature qui evoque les difficultes d'organisation syndicale chez les travailleurs precaires. L'analyse a porte sur des experiences en cours, qui s'inscrivent dans une recherche de formes d'organisation collective des travailleurs de plateforme alternatives a des modes traditionnels de syndicalisation. Sur le plan theorico-methodologique, adoptant la perspective ergologique, l'approche a integre les niveaux macro et institutionnels et le niveau micro de la dynamique collective des travailleurs dans leurs luttes quotidiennes. Ont ete associes a cette recherche, les dirigeants, animateurs et membres de deux associations, deux collectifs ayant opte pour l'autogestion du travail et un reseau autonome de travailleurs. Les options et les obstacles rencontres dans la construction de l'organisation et de l'action commune ont ete identifies. On observe que la force de cohesion des entites collectives analysees se fonde sur le partage de valeurs qui accompagnent le cours de l'action commune. Ainsi, face a l'heterogeneite de ces groupes de travailleurs de plateforme et en adoptant une conception moins restrictive de ce que recouvre une categorie professionnelle ou economique, on evoque la possibilite de formation de mouvements et de modes organisationnels alternatifs en mesure de representer leurs interets. Abstract

In spite of abundant generation potential, as of 2019 East Africa has an electricity access level of 36%, with over 140 million people without service. Here, a bottom-up geospatial model (OnSSET) is used to estimate least-cost pathways to universal access to electricity by 2030 for different consumption-tier objectives under three regional grid electricity generation mix scenarios. Results suggest median total required investments of $57 and $110 billion for guaranteeing basic (160 and 44 kWh/person/year in urban and rural areas) and moderate--i.e. including potential to enable some productive uses--(423 and 160 kWh/person/year) consumption for newly connected households by 2030, respectively. This corresponds to an average of $5.6 billion/year, and implies median capacity additions of 12.2 GW (59% on-grid, 37% mini-grids, and 4% standalone solutions). At least further $2.7 billion/year in generation capacity are required to satisfy the projected demand growth from already electrified consumers. A grid electricity scenario with 25% lower photovoltaic costs and a higher penetration of renewables reveals to be up to 10% cheaper and 46% less carbon-intensive, while also requiring less up-front investment. To achieve such objectives, investment must be channelled within an enabling policy environment, which we discuss. Abstract

Background and purpose Some studies have shown that air pollution, often assessed by thin particulate matter with diameter below 2.5 µg/m3 (PM2.5), may contribute to severe COVID‐19 courses, as well as play a role in the onset and evolution of multiple sclerosis (MS). However, the impact of air pollution on COVID‐19 has never been explored specifically amongst patients with MS (PwMS). This retrospective observational study aims to explore associations between PM2.5 and COVID‐19 severity amongst PwMS. Methods Data were retrieved from an Italian web‐based platform (MuSC‐19) which includes PwMS with COVID‐19. PM2.5 2016–2018 average concentrations were provided by the Copernicus Atmospheric Monitoring Service. Italian patients inserted in the platform from 15 January 2020 to 9 April 2021 with a COVID‐19 positive test were included. Ordered logistic regression models were used to study associations between PM2.5 and COVID‐19 severity. Results In all, 1087 patients, of whom 13% required hospitalization and 2% were admitted to an intensive care unit or died, were included. Based on the multivariate analysis, higher concentrations of PM2.5 increased the risk of worse COVID‐19 course (odds ratio 1.90; p = 0.009). Conclusions Even if several other factors explain the unfavourable course of COVID‐19 in PwMS, the role of air pollutants must be considered and further investigated., Air pollution, often assessed by particulate matter with diameter below 2.5 µg/m3, may contribute to severe COVID‐19 courses. 1087 patients were included, of whom 13% required hospitalization and 2% were admitted to an intensive care unit or died. Even if several other factors explain the unfavourable course of COVID‐19 in patients with multiple sclerosis, the role of air pollutants must be considered and further investigated.

Abstract

Aim: To evaluate the quality of YouTube™ videos on bladder pain syndrome (BPS) and to investigate whether they can be used as a reliable source of information. Methods: The search term "bladder pain syndrome" was used on YouTubeTM platform. The first 100 videos were selected. Patient Education Materials Assessment Tool for audio-visual content (PEMAT A/V), Global Quality Score (GQS), Misinformation tool, and DISCERN score were used to assess videos' quality content. Pearson's test was used to assess potential correlations between variables. Results: Seventy-nine videos were suitable for the analyses. The median PEMAT A/V Understandability score and PEMAT A/V Actionability score were 66.7% (interquartile range [IQR]: 46.2-100.0) and 75.0% (IQR: 37.5-100.0), respectively. According to GQS, 26 (32.9%), 32 (40.5%), 3 (3.8%), 15 (19.0%), and 3 (3.8%) videos were excellent, good, moderate, generally poor, and poor, respectively. According to Misinformation tool, of all videos, 81% (n = 64), 6.3% (n = 5), 5.1% (n = 4), 5.1% (n = 4), 2.5% (n = 2) had respectively no, very little, moderate, high, and extreme misinformation. The overall median DISCERN score ranged from 5.0 (IQR: 2.0-5.0) to 5.0 (IQR: 5.0-5.0). A positive statistically significant correlation was found between video length and PEMAT A/V Understandability (r = 0.27, p = 0.01), video length and PEMAT A/V Actionability (r = .26, p = 0.02), and video length and DISCERN Question 16 (r = 0.28, p = 0.01). Conclusions: Nowaday, the overall quality of YouTubeTM videos on BPS have been evaluated good according to PEMAT A/V, GQS, Misinformation tool, and DISCERN score. It is possible to assume that YouTubeTM may be considered as a reliable source of information on BPS.

Background: The management of melanoma patients with metastatic sentinel nodes (SN) is evolving based on the results of trials questioning the impact of completion lymph node dissection (CLND) and demonstrating the efficacy of new adjuvant treatments. In this landscape, new prognostic tools for fine risk stratification are eagerly sought to optimize the therapeutic path of these patients. Methods: A retrospective cohort of 2,086 patients treated with CLND after a positive SN biopsy in thirteen Italian Melanoma Centers was reviewed. Overall survival (OS) was the outcome of interest; included independent variables were the following: age, gender, primary melanoma site, Breslow thickness, ulceration, sentinel node tumor burden (SNTB), number of positive SN, non-sentinel lymph nodes (NSN) status. Univariate and multivariate survival analyses were performed using the Cox proportional hazard regression model. Results: The 3-year, 5-year and 10-year OS rates were 79 %, 70% and 54%, respectively. At univariate analysis, all variables, except for primary melanoma body site, were found to be statistically significant prognostic factors. Abstract

Introduction During pregnancy, women are at an increased risk of developing iron-deficiency anemia. Objective The objective of this study was to assess the diagnostic performance of the reticulocyte hemoglobin equivalent (RET-He) in the early detection of iron-deficiency anemia in a group of pregnant women and to establish a reference range for this parameter in a group of control individuals. Method: A total of 60 patients and 130 control subjects were included in the study. Blood samples collected from the subjects were submitted to a complete blood count and a serum ferritin test and the data were analyzed by comparing the groups and ROC curves. Results The reference range found for the RET-He was between 29.75pg and 38.24pg, with a median of 35pg. The receiver operating characteristic (ROC) curve analysis for the ferritin parameter showed an area under the curve of 0.732 for the RET-He, 0.586 for hemoglobin, 0.551 for the mean corpuscular hemoglobin concentration and 0.482 for the mean corpuscular volume. Conclusion Early diagnosis of iron deficiency anemia in pregnancy is essential to prevent damage to both maternal and fetal health. The RET-He presents an excellent potential as an auxiliary tool for the diagnosis of iron deficiency in pregnant women.

Abstract

BackgroundPsoriasis is a chronic immune-mediated inflammatory skin disease which can also involve joints. It is often associated with burdensome comorbidities which negatively impact prognosis and quality of life (QoL). Biologic agents have been shown to be effective in controlling disease progression, but their use is associated with higher costs compared with traditional systemic treatments. The economic analysis of the CANOVA (EffeCtiveness of biologic treAtmeNts for plaque psOriasis in Italy: an obserVAtional longitudinal study of real-life clinical practice) study aims to assess the costs and cost-effectiveness of biologics in a real-world context in Italy.MethodsThe annualised overall direct costs of moderate-to-severe plaque psoriasis management, the annualised cost of biologic drugs and the cost per responder in the Italian National Health System perspective were assessed. More specifically, the cost per response and cost per sustained response of the most prescribed biologic therapies for the treatment of moderate-to-severe plaque psoriasis within the CANOVA study were assessed using the Psoriasis Area Severity Index (PASI) at several score levels (75, 90 and 100%).ResultsThe most frequently used biologic therapies for plaque psoriasis were secukinumab, ustekinumab, adalimumab originator, and ixekizumab. Cost of biologics was the driver of expenditure, accounting for about 98% of total costs. Adalimumab originator was the biologic with the lowest cost per responder ratio (range: €7848 - €31,378), followed by secukinumab (range: €9015 - €33,419). Ustekinumab (range: €11,689 – €39,280) and ixekizumab (range: €11,092 – €34,289) ranked respectively third and fourth, in terms of cost-effectiveness ratio. As concerns the cost per sustained response analysis, secukinumab showed the lowest value observed (€21,375) over the other options, because of its high response rate (86% vs. 60–80%), which was achieved early in time.ConclusionBiologic therapy is a valuable asset for the treatment of moderate-to-severe plaque psoriasis. Concomitant assessment of treatment costs against the expected therapeutic response over time can provide physicians and payers additional insights which can complement the traditional risk-benefit profile assessment and drive treatment decisions.

Odontogenic choristomas are a rare and recently classified entity, defined as neoplasms containing tissues of odontogenic derivation, found in abnormal locations and usually diagnosed in the early age. The authors report a unique case of a 79-year-old patient who presented to our attention for a skin lesion in the right cheek with no history of trauma or recent dental procedures, that underneath presented an indolent tooth-like structure inside the soft tissue of the right cheek, which at the histological study resulted to be an odontogenic choristoma. This rare histopathological diagnosis should be considered when a hard lesion is present in the head and neck region.

Abstract

Most patients with psoriasis present with localized mild-to-moderate disease. In this case, the application of topical treatments in the first-line setting is recommended in most cases. Among different topical options, the fixed-dose combination of betamethasone dipropionate (BD) and vitamin D analogue (Cal) aerosol foam (Enstilar®, Leo Pharma) is approved as first-line topical therapy for the treatment of psoriasis in USA and the EU, due to its high efficacy and its favorable administration scheme. The PSO-LONG was the first trial to report on the long-term efficacy and safety of the Cal/DB foam treatment for the proactive management of psoriasis and now, the indications of Cal/BD foam included its use in the psoriasis maintenance treatment. However, the precise role of this treatment and the potential therapeutic schemes in the long-term management of psoriasis need further clarification. This Position Paper, authored by a group of Italian Expert Dermatologists, critically discusses the long-term management of psoriasis with Cal/BD foam in clinical practice. In particular, the biological rationale in the proactive treatment with Cal/BD foam and current evidence regarding this therapeutic approach are presented, along with its application also in patients with moderate-to-severe disease, difficult-to-treat lesions, or within combination regimens. In addition, strategies to improve adherence to long-term treatment of psoriasis are discussed,.

Abstract

Background and aims Although rectal cancer is predominantly a disease of older patients, current guidelines do not incorporate optimal treatment recommendations for the elderly and address only partially the associated specific challenges encountered in this population. This results in a wide variation and disparity in delivering a standard of care to this subset of patients. As the burden of rectal cancer in the elderly population continues to increase, it is crucial to assess whether current recommendations on treatment strategies for the general population can be adopted for the older adults, with the same beneficial oncological and functional outcomes. This multidisciplinary experts’ consensus aims to refine current rectal cancer-specific guidelines for the elderly population in order to help to maximize rectal cancer therapeutic strategies while minimizing adverse impacts on functional outcomes and quality of life for these patients. Methods The discussion among the steering group of clinical experts and methodologists from the societies’ expert panel involved clinicians practicing in general surgery, colorectal surgery, surgical oncology, geriatric oncology, geriatrics, gastroenterologists, radiologists, oncologists, radiation oncologists, and endoscopists. Research topics and questions were formulated, revised, and unanimously approved by all experts in two subsequent modified Delphi rounds in December 2020–January 2021. The steering committee was divided into nine teams following the main research field of members. Each conducted their literature search and drafted statements and recommendations on their research question. Literature search has been updated up to 2020 and statements and recommendations have been developed according to the GRADE methodology. A modified Delphi methodology was implemented to reach agreement among the experts on all statements and recommendations. Conclusions The 2021 SICG-SIFIPAC-SICE-WSES consensus for the multidisciplinary management of elderly patients with rectal cancer aims to provide updated evidence-based statements and recommendations on each of the following topics: epidemiology, pre-intervention strategies, diagnosis and staging, neoadjuvant chemoradiation, surgery, watch and wait strategy, adjuvant chemotherapy, synchronous liver metastases, and emergency presentation of rectal cancer.
