Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease where substantial heterogeneity in clinical presentation urgently requires a better stratification of patients for the development of drug trials and clinical care. In this study we explored stratification through a crowdsourcing approach, the DREAM Prize4Life ALS Stratification Challenge. Using data from >10,000 patients from ALS clinical trials and 1479 patients from community-based patient registers, more than 30 teams developed new approaches for machine learning and clustering, outperforming the best current predictions of disease outcome. We propose a new method to integrate and analyze patient clusters across methods, showing a clear pattern of consistent and clinically relevant sub-groups of patients that also enabled the reliable classification of new patients. Our analyses reveal novel insights in ALS and describe for the first time the potential of a crowdsourcing to uncover hidden patient sub-populations, and to accelerate disease understanding and therapeutic development. ispartof: SCIENTIFIC REPORTS vol:9 issue:1 ispartof: location:England status: published

Neonates born very preterm (before 32 weeks’ gestational age), are a significant public health concern because of their high-risk of mortality and life-long disability. In addition, caring for very preterm neonates can be expensive, both during their initial hospitalization and their long-term cost of permanent impairments. To address these issues, national and regional neonatal networks around the world collect and analyse data from their constituents to identify trends in outcomes, and conduct benchmarking, audit and research. Improving neonatal outcomes and reducing health care costs is a global problem that can be addressed using collaborative approaches to assess practice variation between countries, conduct research and implement evidence-based practices. The International Network for Evaluating Outcomes (iNeo) of neonates was established in 2013 with the goal of improving outcomes for very preterm neonates through international collaboration and comparisons. To date, 10 national or regional population-based neonatal networks/datasets participate in iNeo collaboration. The initiative now includes data on >200,000 very preterm neonates and has conducted important epidemiological studies evaluating outcomes, variations and trends. The collaboration has also surveyed >320 neonatal units worldwide to learn about variations in practices, healthcare service delivery, and physical, environmental and manpower related factors and support services for parents. The iNeo collaboration serves as a strong international platform for Neonatal-Perinatal health services research that facilitates international data sharing, capacity building, and global efforts to improve very preterm neonate care.

IMPORTANCE Early exposure to complex dietary proteins may increase the risk of type 1 diabetes in children with genetic disease susceptibility. There are no intact proteins in extensively hydrolyzed formulas. OBJECTIVE To test the hypothesis that weaning to an extensively hydrolyzed formula decreases the cumulative incidence of type 1 diabetes in young children. DESIGN, SETTING, AND PARTICIPANTS An international double-blind randomized clinical trial of 2159 infants with human leukocyte antigen-conferred disease susceptibility and a first-degree relative with type 1 diabetes recruited from May 2002 to January 2007 in 78 study centers in 15 countries; 1081 were randomized to be weaned to the extensively hydrolyzed casein formula and 1078 to a conventional formula. The follow-up of the participants ended on February 28, 2017. INTERVENTIONS The participants received either a casein hydrolysate or a conventional adapted cows milk formula supplemented with 20% of the casein hydrolysate. The minimum duration of study formula exposure was 60 days by 6 to 8 months of age. MAIN OUTCOMES AND MEASURES Primary outcome was type 1 diabetes diagnosed according to World Health Organization criteria. Secondary outcomes included age at diabetes diagnosis and safety (adverse events). RESULTS Among 2159 newborn infants (1021 female [47.3%]) who were randomized, 1744 (80.8%) completed the trial. The participants were observed for a median of 11.5 years (quartile [Q] 1-Q3, 10.2-12.8). The absolute risk of type 1 diabetes was 8.4% among those randomized to the casein hydrolysate (n = 91) vs 7.6% among those randomized to the conventional formula (n = 82) (difference, 0.8%[95% CI, -1.6% to 3.2%]). The hazard ratio for type 1 diabetes adjusted for human leukocyte antigen risk group, duration of breastfeeding, duration of study formula consumption, sex, and region while treating study center as a random effect was 1.1 (95% CI, 0.8 to 1.5; P = .46). The median age at diagnosis of type 1 diabetes was similar in the 2 groups (6.0 years [Q1-Q3, 3.1-8.9] vs 5.8 years [Q1-Q3, 2.6-9.1]; difference, 0.2 years [95% CI, -0.9 to 1.2]). Upper respiratory infections were the most common adverse event reported (frequency, 0.48 events/year in the hydrolysate group and 0.50 events/year in the control group). CONCLUSIONS AND RELEVANCE Among infants at risk for type 1 diabetes, weaning to a hydrolyzed formula compared with a conventional formula did not reduce the cumulative incidence of type 1 diabetes after median follow-up for 11.5 years. These findings do not support a need to revise the dietary recommendations for infants at risk for type 1 diabetes. Background: 96-well microtiter plate assay are becoming popular analytical procedures in laboratory and clinical practices generating a demand for microtiter plate readers. The present colorimetric and fluorimetric based microtiter plate reader are efficient in general applications such as measuring reflectance, absorbance, and optical density of reaction mixtures. However, a dedicated system incorporating automation and computational techniques in specific microtiter plate readers for Drug discovery and Microbial viability, provides portability and compatibility with highthroughput analysis at affordable cost. Objective: To develop image-based reader system for semi-quantitative measurements of transport assay performed in microtiter plate. In this work, we focus on deduction of illumination source and classification of fluorescence emitted by transport assay using this microtiter plate reader. Method: We have taken the images of 96-well fluorescence efflux assay and developed an appropriate image analysis system to read the individual well images and categorize the results as fast efflux and slow efflux by the classifier. The images exhibited non-uniform illumination due to UV transilluminator, which then replaced by the circuit of blue LED after performing a study on its classification performance. Statistical descriptive features extracted from the image are classified using k-means clustering technique. The proposed system can be adapted to read any kind of microtiter plate assay. Results: For uniform illumination of the source, of both UV transilluminator and blue LED, the color profile varied in accord to reaction mixture concentration. For classification performance of the source, out of four different images at various intensities of illumination from blue LED circuit controlled by potentiometer, the classification of efflux assay for image with maximum intensity gave high accuracy than the rest. For classification performance of the classifier, out of 1920-sub-images, the classifier misclassifies none. Conclusion: For the microtiter plate reader, which classifies the liquid transport assay as fast efflux or slow efflux, the usage of UV transilluminator as illumination source is limited due to its non-uniform illumination and blue LED is apt for this application due its uniformity. The k-means clustering technique had been effective for this application when fed with statistical features describing the clusters.

To examine the metabolic and antiarrhythmic effects of glucose infusion during myocardial ischemia, an electrode catheter was placed in the left anterior descending coronary artery for thrombus production in intact anesthetized dogs. 85 Krypton was injected distal to the thrombus for serial coronary blood flow measurements. Arterial-great cardiac vein concentrations of potassium ion across the ischemic site indicated that a net loss of K + occurred before ventricular ectopic beats. A four hour infusion of 1 L. of 10 per cent glucose containing K + and insulin was begun after the onset of ischemia; it was associated with enhanced glucose uptake, diminished K + loss and a reduced incidence of ventricular fibrillation. Reduction of coronary blood flow and S-T segment elevation persisted. Infusions of solutions made hypertonic without glucose as well as solutions of isotonic saline had a qualitatively similar effect on K + egress and the arrhythmia, without enhancing glucose uptake. Left ventricular end-diastolic pressures were significantly higher than in untreated control animals. It is postulated that a neurohumoral mechanism, perhaps adrenergic, may be elicited by substantial acute increments of extracellular volume affecting K + transport in the myocardium. The lower survival rates obtained with lesser volumes of glucose solution support this view.

Lidocaine at low doses was unable to correct arrhythmia or myocardial K+ loss. A high dose of lidocaine was effective in reducing mortality and arrhythmia, and an associated improvement in the electrolyte abnormality was seen.

For obvious reasons it is important to recognize alcoholic cardiomyopathy early in its natural history. Hemodynamic studies were performed in nine patients with normal ECG's and normal cardiothoracic ratios on chest x-rays. Findings were elevated mean LV end-diastolic pressure, low mean cardiac output, and depressed myocardial contractility. Thus, true functional cardiac impairment may exist in these patients even prior to the development of abnormal clinical parameters such as cardiomegaly.

The electronic properties of hydrogenated silicene and germanene, so called silicane and germanane, respectively, are investigated using first-principles calculations based on density functional theory. Two different atomic configurations are found to be stable and energetically degenerate. Upon the adsorption of hydrogen, an energy gap opens in silicene and germanene. Their energy gaps are next computed using the HSE hybrid functional as well as the G(0)W(0) many-body perturbation method. These materials are found to be wide band-gap semiconductors, the type of gap in silicane (direct or indirect) depending on its atomic configuration. Germanane is predicted to be a direct-gap material, independent of its atomic configuration, with an average energy gap of about 3.2 eV, this material thus being potentially interesting for optoelectronic applications in the blue/violet spectral range.
