Abstract

Journal of Instrumentation 15(06), P06032 (2020). doi:10.1088/1748-0221/15/06/P06032, We describe an improved in-situ calibration of the single-photoelectron charge distributions for each of the in-ice Hamamatsu Photonics R7081-02[MOD] photomultiplier tubes in the IceCube Neutrino Observatory. The characterization of the individual PMT charge distributions is important for PMT calibration, data and Monte Carlo simulation agreement, and understanding the effect of hardware differences within the detector. We discuss the single photoelectron identification procedure and how we extract the single-photoelectron charge distribution using a deconvolution of the multiple-photoelectron charge distribution., Published by Inst. of Physics, London

Abstract

Chronic pulmonary aspergillosis (CPA) is infrequent respiratory disease which is difficult for diagnosis and can complicate other respiratory diseases and conditions. Currently, there are about 240,000 CPA patients in Europe. The most prevalent variant of CPA is chronic cavitary pulmonary aspergillosis (CCPA) which could progress to chronic fibrosing pulmonary aspergillosis (CFPA) when is untreated. Aspergillus nodules and single aspergilloma are less frequent clinical variants of this disease. All clinical variants of aspergillosis could be found in nonimmunocompromised patients with different underlying pulmonary disorders. Subacute invasive pulmonary aspergillosis, which was referred to as chronic necrotising pulmonary aspergillosis, is the most rapidly progressive clinical variant of the infection (< 3 months); it is typically diagnosed in moderately immunocompromised patients and should be treated as invasive aspergillosis. Diagnosis and management of CPA patients were previously described in sporadic guidelines. Due to this, expert group have been convened to develop clinical, radiological and microbiological guidelines. Diagnosis of CPA requires a combination of the following criteria existing for 3 months or more: one or more cavities in the lungs with or without fungal mass inside or nodules found on radiological examination; direct evidence of Aspergillus infection (using microscopic examination or biopsy culturing) or immunological response to Aspergillus spp. and exclusion of alternative diseases. Antibodies against Aspergillus spp. (precipitins) are increased in > 90% of patients. Aspergilloma should be resected if technically possible; videoassisted thoracic surgery is preferable. Chronic cavitary pulmonary aspergillosis requires longterm oral antifungal therapy to improve the patients' health and respiratory symptoms, to arrest haemoptysis and prevent the disease progression. Azole serum concentration and drug interaction should be thoroughly monitored to avoid toxic effects. Haemoptysis could be arrested using therapy with tranexamic acid or bronchial artery embolization; surgical resection of the lung is rarely required. Haemoptysis could indicate therapeutic failure and / or resistance to antifungals. Patients with a single Aspergillus nodule need antifungal therapy only if surgical resection is impossible. Antifungal therapy could be beneficial in patients with multiple Aspergillusis nodules; these patients need careful followup.

Abstract

The BiPo-3 detector is a low radioactive detector dedicated to measuring ultra-low natural contaminations of 208Tl and 214Bi in thin materials, initially developed to measure the radiopurity of the double β decay source foils of the SuperNEMO experiment at the μBq/kg level. The BiPo-3 technique consists in installing the foil of interest between two thin ultra-radiopure scintillators coupled to low radioactive photomultipliers. The design and performances of the detector are presented. In this paper, the final results of the 208Tl and 214Bi activity measurements of the first enriched 82Se foils are reported for the first time, showing the capability of the detector to reach sensitivities in the range of some μBq/kg.

Abstract

The Golden Retriever Muscular Dystrophy (GRMD) dog is the closest animal counterpart of Duchenne muscular dystrophy in humans and has, for this reason, increasingly been used in preclinical therapeutic trials for this disease. The aim of this study was to describe the abnormalities in canine dystrophic muscle non-invasively, quantitatively, thoroughly and serially by means of NMR imaging. Thoracic and pelvic limbs of five healthy and five GRMD dogs were imaged in a 3T NMR scanner at 2, 4, 6 and 9months of age. Standard and fat-saturated T 1 -, T 2 - and proton-density-weighted images were acquired. A measurement of T 1 and a two-hour kinetic study of muscle enhancement after gadolinium-chelate injection were also performed. Ten out of the 15 indices evaluated differed between healthy and GRMD dogs. The maximal relative enhancement after gadolinium injection and the proton-density-weighted/ T 2 -weighted signal ratio were the most discriminating indices. Inter-muscle heterogeneity was found to vary significantly for most of the indices. The body of data that has been acquired here will help in designing and interpreting preclinical trials using dystrophin-deficient dogs.

Abstract

Answer: Imported cutaneous diphtheria. The suspected colonies were rapidly identified as Corynebacterium diphtheriae by use of a Microflex LT matrix-assisted laser desorption ionization–time of flight (MALDI-TOF) mass spectrometer (Bruker Daltonics, Bremen, Germany) and subsequently characterized as nontoxigenic Corynebacterium diphtheriae biotype mitis by the National Reference Center for Corynebacterium of the diphtheriae Complex (Institut Pasteur, Paris, France). Treatment with antibiotics was carried on for a total duration of 14 days and allowed for a complete recovery. Nasopharyngeal swabs were taken for all members of the household (i.e., the patient's sister, father, and mother) in order to assess the presence of asymptomatic carriers. All samples were negative for C. diphtheriae growth. Multilocus sequence typing of the isolate generated a new allelic profile, assigned as sequence type 251 (1). Diphtheria is usually considered a life-threatening pharyngitis or laryngitis, and C. diphtheriae is generally not regarded as a skin pathogen. In vaccinated countries, cutaneous diphtheria is discovered as imported cases in travelers coming from areas where the disease is endemic or as autochthonous cases in socioeconomically disadvantaged populations (2,–4). Clinical presentation of skin diphtheria is nonspecific, and most commonly, the organs involved are the lower and upper limbs. Clinical symptoms typically begin as a vesicle, which rapidly progresses to form a punch-out ulcer. At first, these skin lesions may be painful, but they rapidly become insensitive to pain. Diphtheria ulcers generally evolve chronically, and spontaneous recovery can take several weeks. Interestingly, in the present case, the lesions were pruriginous, and scratching them could therefore enhance the diffusion of the bacterium. Definitive diagnosis is generally provided by culture of lesion swabs, but the specific Tinsdale medium is generally not available, as it is expensive and has a shelf life of 1 week. C. diphtheriae colonies are morphologically indistinguishable from those of other Corynebacterium species and thus are likely to be confused with skin flora. Furthermore, most ulcers yield growth of associated pathogens such as Staphylococcus aureus and Streptococcus pyogenes, therefore implying the existence of a causal relationship between the latter pathogens and the skin lesions (5, 6). In fact, these pathogens are usually involved in acute skin infections (e.g., erysipelas or impetigo), while cutaneous diphtheria is chronic and painless. C. diphtheriae identification is challenging and time-consuming with the use of conventional methods, but as it proved to be of valuable assistance in this investigation, MALDI-TOF mass spectrometry would probably simplify and expedite the identification of this pathogen (7, 8). Recommended treatment of skin diphtheria includes thorough cleaning of the lesion, penicillin or macrolide therapy, and, in cases of toxigenic symptoms, antitoxin (2, 9, 10). In conclusion, chronic cutaneous ulcers in specific epidemiological situations should be tested for diphtheria, independent of vaccination status. Their singular clinical and microbiological manifestations probably explain why cutaneous lesions evolve chronically and are more contagious than respiratory infections (3). Abstract

The Muon Ionization Cooling Experiment (MICE) will demonstrate the principle of muon beam phase-space reduction via ionization cooling. Muon beam cooling will be required for the proposed Neutrino Factory or Muon Collider. The phase-space before and after the cooling cell must be measured precisely. This is achieved using two scintillating-fibre trackers, each placed in a solenoidal magnetic field. This paper describes the software reconstruction for the fibre trackers: the GEANT4 based simulation; the implementation of the geometry; digitisation; space-point reconstruction; pattern recognition; and the final track fit based on a Kalman filter. The performance of the software is evaluated by means of Monte Carlo studies and the precision of the final track reconstruction is evaluated.

Abstract

International audience; Little is known about the vascular function and expression of endothelial and neuronal nitric oxide synthases (eNOS and nNOS) in Duchenne muscular dystrophy (DMD). Bradykinin is involved in the regulation of eNOS expression induced by angiotensin-converting enzyme inhibitors. We characterized the vascular function and eNOS and nNOS expression in a canine model of DMD and evaluated the effects of chronic bradykinin treatment. Vascular function was examined in conscious golden retriever muscular dystrophy (GRMD) dogs with left ventricular dysfunction (measured by echocardiography) and in isolated coronary arteries. eNOS and nNOS proteins in carotid arteries were measured by western blot and cyclic guanosine monophosphate (cGMP) content was analyzed by radioimmunoassay. Compared with controls, GRMD dogs had an impaired vasodilator response to acetylcholine. In isolated coronary artery, acetylcholine-elicited relaxation was nearly absent in placebo-treated GRMD dogs. This was explained by reduced nNOS and eNOS proteins and cGMP content in arterial tissues. Chronic bradykinin infusion (1 μg/min, 4 weeks) restored in vivo and in vitro vascular response to acetylcholine to the level of control dogs. This effect was NO-mediated through upregulation of eNOS and nNOS expression. In conclusion, this study is the first to demonstrate that DMD is associated with NO-mediated vascular endothelial dysfunction linked to an altered expression of eNOS and nNOS, which can be overcome by bradykinin.

Oral health has long been recognized as an essential component of general health. Poor oral hygiene can negatively affect basic needs, such as swallowing, eating, drinking and communicating, and, thereby, the quality of life. Moreover, oropharyngeal colonization is associated with a variety of oral diseases as well as systemic conditions, including cardiovascular disease and stroke, chronic obstructive pulmonary disease (COPD), endocarditis, bacteremia, and a higher risk of preterm low birth-weight babies [1], [2]. In addition to the adverse clinical effects of poor oral hygiene, a substantial health-economic burden has been documented [3].

Abstract

Background Europe is a continent with strong public healthcare systems, but diverging antibiotic policies and resistance patterns. Aims To describe the performance and methodological approach in a retrospective data collection effort (1997–2001), through an international network of surveillance systems, aiming to collect publicly available, comparable and reliable data on antibiotic use in Europe. Methods A central multidisciplinary management team co-ordinated a network of national representatives, liasing with national data providers and bodies responsible for antibiotic policy. The data collected were screened for bias, using a checklist. We focused on detection bias in sample and census data; errors in assigning medicinal product packages to the Anatomical Therapeutic Chemical Classification (ATC); errors in calculations of defined daily doses (DDD) per package; bias by over-the-counter sales and parallel trade; and bias in ambulatory care (AC)/hospital care (HC) mix. Datasets were corrected after national feedback, and classified as valid; valid but with minor bias; not valid. Results Of the 31 participating countries, 21 countries delivered AC data suitable for cross-national comparison (14 for all 5 years). Of these, 17 countries provided data on a quarterly basis for at least 1 year. For HC, 14 countries were able to deliver valid data (nine for all 5 years). A valid estimate of the total exposure of national populations to human antibiotic consumption could be made in 17 countries. Conclusion In cross-national comparisons of antibiotic consumption in Europe, methodological rigour in correcting for various sources of bias and checking the validity of ATC/DDD assignment is needed.

Abstract

Introduction Burn patients with inhalation injury requiring mechanical ventilation (MV) are at particular risk for ventilator-associated pneumonia (VAP), which is associated with increased morbidity and mortality. Routine endotracheal surveillance cultures (SC) may provide information about the causative pathogen in subsequent VAP, facilitating early appropriate antibiotic therapy. Objectives To assess the value of routine endotracheal SC to predict multidrug resistant (MDR) aetiology of VAP in burn patients with inhalation injury. Methods Historical cohort (N=46) study including all burn patients with inhalation injury who developed VAP during admission to the burn unit at Ghent University Hospital (2002–2009). Results Overall, 70 episodes of VAP occurred. Median age and total burned surface area were 43.5 y (IQR 38.0 to 54.3) and 32.5% (18.0 to 45.8) respectively. The median Belgian Outcome in Burn Injury score was 5 (4–6), reflecting a predicted mortality of 30% (20–50%). 1 Median duration of MV prior to onset VAP was 7d (4–9d). The incidence of VAP was 55 episodes/1000 MV days and 112 episodes/1000 MV days “at risk.” In 23 episodes (32.9%) at least one MDR causative pathogen was involved (24 MDR pathogens), mostly Pseudomonas aeruginosa (10/23) and Enterobacter spp. (7/23). The sensitivity and specificity of SC to predict MDR pathogens was respectively 83.0% and 96.2%, corresponding with a positive predictive value of 87.0% and a negative predictive value of 95.0%. Subgroup analyses showed higher predictive values for second VAP episodes, and when P aeruginosa was involved. Conclusions In this cohort routine SC appear to have excellent operating characteristics to predict MDR involvement in VAP.

Abstract

Chronic hypertension is a major predisposing factor for stroke in humans. It has recently been shown that long-term inhibition of nitric oxide synthase activity causes a gradual time-dependent increase in arterial blood pressure in rats. We used this new animal model of chronic hypertension to study the occurrence and spatial features of infarcts in the central nervous system. Rats were treated with a nitric oxide synthase inhibitor, NG-nitro-L-arginine methyl ester, dissolved in the drinking water at 50 mg/kg per day for 11 weeks. The brains and spinal cords of hypertensive rats with and without motion disturbances were processed for standard microscopic examination. Seventy-nine percent of the hypertensive rats showed motion dysfunctions, especially front leg paralysis, and/or died suddenly when their systolic blood pressure reached approximately 215 mm Hg after approximately 7 weeks of treatment. All of the hypertensive rats with stroke had spinal cord infarcts (90% at the cervical or cervicothoracic level) either alone or combined with brain lesions (30%). These structural alterations ranged from focal areas of pale, spongy tissue to large necrotic sites with vascular alterations, including thickened or fibrinoid degenerated vessel wall, macrophage invasion, and reactive astrocytes. Infarcts occurred in the central nervous system with a high incidence in the spinal cord of hypertensive rats in which nitric oxide synthase was chronically blocked. This location of the hypertensive neuropathologic sequelae contrasts with the model of stroke-prone spontaneously hypertensive rats. The results suggest that nitric oxide is a key factor in spinal cord arteriolar vasomotion and structure in rats.

Abstract

La dystrophie musculaire de Duchenne est causée par des mutations du gène de la dystrophine. Elle est caractérisée par la dégénérescence des fibres musculaires striées conduisant aux défaillances locomotrice, respiratoire et cardiaque. Le chien Golden Retriever est un modèle spontané de dystrophie musculaire (GRMD) où la mutation intéresse l’intron 6 du gène de la dystrophine. Alors que les altérations fonctionnelles et moléculaires des muscles striés (squelettiques et caediaques) ont été bien décrites, celles qui concernent la fonction endothéliale vasculaire le sont moins. Ainsi, nous avons examiné s’il existait une dysfonction endothéliale vasculaire et étudié l’effet d’une perfusion chronique de bradykinine, dont nous avons démontré qu’elle exerçait des effets protecteurs vasculaires dans un modèle d’insuffisance cardiaque indépendamment de tout effet presseur. Contains fulltext : 229471.pdf (Publisher’s version ) (Closed access) OBJECTIVES: To summarize the available evidence on the diagnostic performance for invasive aspergillosis (IA) in non-hematological, non-solid organ transplantation critically ill patients of the following: (i) existing definitions of IA (developed either for classical immunocompromised populations or for non-immunocompromised critically ill patients); (ii) laboratory tests; (iii) radiology tests. METHODS: A systematic review was performed by evaluating studies assessing the diagnostic performance for IA of a definition/s and/or laboratory/radiology test/s vs. a reference standard (histology) or a reference definition. RESULTS: Sufficient data for evaluating the performance of existing definitions and laboratory tests for the diagnosis of IA in critically ill patients is available only for invasive pulmonary aspergillosis. Against histology/autopsy as reference, the AspICU definition showed a promising diagnostic performance but based on small samples and applicable only to patients with positive respiratory cultures. Studies on laboratory tests consistently indicated a better diagnostic performance of bronchoalveolar lavage fluid (BALF) galactomannan (GM) than serum GM, and a suboptimal specificity of BALF and serum (1,3)-β-D-glucan. CONCLUSIONS: Evidence stemming from this systematic review will guide the discussion for defining invasive aspergillosis within the FUNDICU project. The project aims to develop a standard set of definitions for invasive fungal diseases in critically ill, adult patients.

Risk factors for mortality were assessed by logistic regression analysis.ResultsThe cohort included 2621 patients. Setting of infection acquisition was community-acquired in 31.6%, early onset hospital-acquired in 25%, and late-onset hospital-acquired in 43.4% of patients. Overall prevalence of antimicrobial resistance was 26.3% and difficult-to-treat resistant Gram-negative bacteria 4.3%, with great variation according to geographic region. No difference in prevalence of antimicrobial resistance was observed according to setting of infection acquisition. Overall mortality was 29.1%. Independent risk factors for mortality included late-onset hospital-acquired infection, diffuse peritonitis, sepsis, septic shock, older age, malnutrition, liver failure, congestive heart failure, antimicrobial resistance (either methicillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, extended-spectrum beta-lactamase-producing Gram-negative bacteria, or carbapenem-resistant Gram-negative bacteria) and source control failure evidenced by either the need for surgical revision or persistent inflammation.ConclusionThis multinational, heterogeneous cohort of ICU patients with intra-abdominal infection revealed that setting of infection acquisition, anatomical disruption, and severity of disease expression are disease-specific phenotypic characteristics associated with outcome, irrespective of the type of infection. Antimicrobial resistance is equally common in community-acquired as in hospital-acquired infection. Contains fulltext : 193204.pdf (Publisher’s version ) (Closed access) The European Society for Clinical Microbiology and Infectious Diseases, the European Confederation of Medical Mycology and the European Respiratory Society Joint Clinical Guidelines focus on diagnosis and management of aspergillosis. Of the numerous recommendations, a few are summarized here. Chest computed tomography as well as bronchoscopy with bronchoalveolar lavage (BAL) in patients with suspicion of pulmonary invasive aspergillosis (IA) are strongly recommended. For diagnosis, direct microscopy, preferably using optical brighteners, histopathology and culture are strongly recommended. Serum and BAL galactomannan measures are recommended as markers for the diagnosis of IA. PCR should be considered in conjunction with other diagnostic tests. Pathogen identification to species complex level is strongly recommended for all clinically relevant Aspergillus isolates; antifungal susceptibility testing should be performed in patients with invasive disease in regions with resistance found in contemporary surveillance programmes. Isavuconazole and voriconazole are the preferred agents for first-line treatment of pulmonary IA, whereas liposomal amphotericin B is moderately supported. Combinations of antifungals as primary treatment options are not recommended. Therapeutic drug monitoring is strongly recommended for patients receiving posaconazole suspension or any form of voriconazole for IA treatment, and in refractory disease, where a personalized approach considering reversal of predisposing factors, switching drug class and surgical intervention is also strongly recommended. Primary prophylaxis with posaconazole is strongly recommended in patients with acute myelogenous leukaemia or myelodysplastic syndrome receiving induction chemotherapy. Secondary prophylaxis is strongly recommended in high-risk patients. We strongly recommend treatment duration based on clinical improvement, degree of immunosuppression and response on imaging.
