Background The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P Interpretation This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men.

In the context of a generative model, such as predictive coding, pain and heat perception can be construed as the integration of expectation and input with their difference denoted as a prediction error. In a previous neuroimaging study (Geuter et al., 2017) we observed an important role of the insula in such a model, but could not establish its temporal aspects. Here we employed electroencephalography to investigate neural representations of predictions and prediction errors in heat and pain processing. Our data show that alpha-to-beta activity was associated with stimulus intensity expectation, followed by a negative modulation of gamma band activity by absolute prediction errors. This is in contrast to prediction errors in visual and auditory perception, which are associated with increased gamma band activity, but is in agreement with observations in working memory and word matching, which show gamma band activity for correct, rather than violated predictions.

There is considerable interest in independent publishing models that operate at or on the periphery of our industry, beyond the ‘centre’ of commercial publishing—from ‘litmags’ (Edmonds 2015) to ‘prosumers’ (Stinson 2016); self-publishers, to digital-first and open access examples. There is less discussion of established ‘literary’ writers producing ‘peripheral’ works, which may be significant in the current context, when publishing faces the challenge of technological developments ‘orders of magnitude greater than the momentous evolution from monkish scriptoria to movable type’ (Epstein 2010). As an author and publisher—working in both roles across distinct categories—I am interested in creative projects that mis/use aspects of generic forms to re/make (a)new literary fiction. In this paper, I consider David Mitchell’s latest novel/la, Slade House (2015), in light of its first appearance as a Twitter ghost story released at midnight on All Hallows’ Eve: a tale-in-instalments that gothic readers and fanfic communities alike would be familiar with, but was and still is more novel for mainstream trade publishers. Mitchell’s avant-garde example may work to illustrate the direction and fluidity of literary fiction at a point in publishing history when traditional practices are being challenged by writers via alternative models of production, which may exhibit a capacity to move between genres that might, in earlier times, have seemed exclusive.

This collaborative paper explores how the ‘spec-fic’ category may be responding to contemporary political and environmental challenges. It presents two case studies, in the personal writing and professional publishing experiences of authors Rose Michael and Cat Sparks, to consider the ways speculative fiction engages with real-world concerns. The paper acknowledges the genre’s contested relationship to harder-to-categorise cross-genre or interstitial forms of non-realist fiction, as well as its obvious antecedents in science fiction and its arguable overlap with ‘big L’ literature. As creative practitioners and published authors who dis/identify with generic labels in different ways, the authors contend that the use, misuse, and abuse of genre conventions has been, and continues to be, personally and professionally productive – particularly in a contemporary publishing landscape impacted by changes to technology and platforms that have transformed traditional relationships and roles.

Claims for a growth of despondency in the British workforce in the 1990s, based on job satisfaction data, are questioned by an evaluation of: (i) the bases of comparison, (ii) features of job-satisfaction measures, (iii) the properties of key data sets and (iv) inferences drawn from the data. A more complex situation is presented showing significant falls in satisfaction with the job facets, the work itself, and hours worked; significant rises in satisfaction with total pay and security of job; a steep decline in overall job satisfaction among women and stable or slightly rising overall job satisfaction among men. Trends in job quality, workforce composition, the economic cycle and changing work values among women, rather than generalized despondency, are proposed as sources for hypotheses for future research. The latter should include a review of data requirements, and research on the performance of measures of job satisfaction.

I’ve only been here three years, so what would I know? I’ve only worked in four areas, how much have I seen? Enough, surely—if the average tenure is eighteen months, as they said when I joined, then I’m practically an old hand. Enough to tell my story like any traveller, write up my notes like a true anthropologist. Or so I thought, until I started. Then I began to wonder. You could spend your whole life here and never get your head around how it works. Indeed, if you spent your life here you’d have no hope of ever getting your head around it: the longer you’re inside the Matrix, the harder it is to see.
