Perceptions of physical attractiveness vary across cultural groups, particularly for female body size and shape. It has been hypothesized that visual media propagates Western "thin ideals." However, because cross-cultural studies typically consider groups highly differentiated on a number of factors, identifying the causal factors has thus far been impossible. In the present research, we conducted "naturalistic" and controlled experiments to test the influence of media access on female body ideals in a remote region of Nicaragua by sampling from villages with and without regular TV access. We found that greater TV consumption remained a significant predictor of preferences for slimmer, curvier female figures after controlling for a range of other factors in an ethnically balanced sample of 299 individuals (150 female, aged 15-79) across 7 villages. Within-individual analyses in 1 village over 3 years also showed an association between increased TV consumption and preferences for slimmer figures among some participants. Finally, an experimental study in 2 low-media locations demonstrates that exposure to media images of fashion models can directly impact participants' body size ideals. We provide the first converging cross-sectional, longitudinal, and experimental evidence from field-based research, that media exposure can drive changes in perceptions of female attractiveness. To examine the effectiveness of antithrombotic medications to prevent venous stent malfunction for iliocaval occlusive disease. A retrospective analysis was performed on 62 patients who underwent technically successful endovascular iliocaval stent placement between May 2008 and April 2017. Clinical records were reviewed for demographic information, procedure details, post-stenting antithrombotic prophylaxis and stent patency on follow-up. Stent malfunction was defined as > 50% stenosis or occlusion at follow-up. Risk factors for stent malfunction were assessed with univariable and multiple Cox proportional hazard models. The median follow-up period was 11.6 months (range 0.1–76.4). Overall primary and secondary cumulative patency rates at 12 months were 70.0% and 92.4%, respectively. After stent placement, 97% of patients received anticoagulation with warfarin, enoxaparin or a factor Xa inhibitor. In addition, 61% received antiplatelet prophylaxis with aspirin, clopidogrel or a combination. In multiple Cox regression analysis, post-stenting antiplatelet use remained significantly associated with primary stent patency (HR = 0.28, P = 0.022). After iliocaval venous stenting, stent patency was best predicted by concomitant antiplatelet and anticoagulation therapy rather than anticoagulation alone. This novel finding warrants further research underlying mechanisms leading to venous stent thrombosis, and has implications for optimal medical management after venous stenting.

OBJECTIVE. Transjugular intrahepatic portosystemic shunt (TIPS) creation increases the risk of hepatic encephalopathy due to overshunting. Techniques exist to secondarily reduce the shunt for refractory encephalopathy. The purpose of this article is to describe a technique for primary TIPS restriction using a balloon-expandable stent within the transvenous hepatic track followed by deployment of a self-expanding polytetrafluoroethylene–lined stent-graft within the balloon-expandable stent to create the TIPS. CONCLUSION. This technique enables control over the degree of portosystemic shunting in elective TIPS creation.

Atomically thin materials such as graphene and transition metal dichalcogenides are being developed for a range of optoelectronic devices, but their applications are currently limited by low light absorption. Here, we describe a dielectric cavity design with chirped Bragg reflectors for broadband coherent absorption. The chirped cavity absorption is calculated by the transfer matrix method and optimized using the Nelder–Mead optimization protocol. We numerically demonstrate that with cavity enhancement, a monolayer MoS2 photodetector absorbs as much as 33% of incident visible light over a 300 nm bandwidth, and the external quantum efficiency of an atomically thin monolayer graphene/monolayer MoS2 solar cell can be enhanced 3.6 times to a predicted value of 7.09%. The proposed layered dielectric structures operate across a wide range of incident angles and could enable applications for atomically thin photodetectors or solar cells.

Recent decades have seen rapid development of new analytical methods to investigate patterns of interspecific variation. Yet these cutting-edge statistical analyses often rely on data of questionable origin, varying accuracy, and weak comparability, which seem to have reduced the reproducibility of studies. It is time to improve the transparency of comparative data while also making these improved data more widely available. We, the authors, met to discuss how transparency, usability, and reproducibility of comparative data can best be achieved. We propose four guiding principles: 1) data identification with explicit operational definitions and complete descriptions of methods; 2) inclusion of metadata that capture key characteristics of the data, such as sample size, geographic coordinates, and nutrient availability (for example, captive versus wild animals); 3) documentation of the original reference for each datum; and 4) facilitation of effective interactions with the data via user friendly and transparent interfaces. We urge reviewers, editors, publishers, database developers and users, funding agencies, researchers publishing their primary data, and those performing comparative analyses to embrace these standards to increase the transparency, usability, and reproducibility of comparative studies.

Phenotypic traits are products of two processes: evolution and development. But how do these processes combine to produce integrated phenotypes? Comparative studies identify consistent patterns of covariation, or allometries, between brain and body size, and between brain components, indicating the presence of significant constraints limiting independent evolution of separate parts. These constraints are poorly understood, but in principle could be either developmental or functional. The developmental constraints hypothesis suggests that individual components (brain and body size, or individual brain components) tend to evolve together because natural selection operates on relatively simple developmental mechanisms that affect the growth of all parts in a concerted manner. The functional constraints hypothesis suggests that correlated change reflects the action of selection on distributed functional systems connecting the different sub-components, predicting more complex patterns of mosaic change at the level of the functional systems and more complex genetic and developmental mechanisms. These hypotheses are not mutually exclusive but make different predictions. We review recent genetic and neurodevelopmental evidence, concluding that functional rather than developmental constraints are the main cause of the observed patterns.

Purpose To describe the use of intravascular ultrasound (US) guidance for creation of transjugular intrahepatic portosystemic shunts (TIPSs) in humans. Materials and Methods The initial 25 cases of intravascular US–guided TIPS were retrospectively compared versus the last 75 conventional TIPS cases during the same time period at the same institution in terms of the number of needle passes required to establish portal vein (PV) access, fluoroscopy time, and needle pass–related complications. Results Intravascular US–guided TIPS creation was successful in all cases, and there was no statistically significant difference in number of needle passes, fluoroscopy time, or needle pass–related complications between TIPS techniques. Intravascular US–guided TIPS creation was successful in cases in which conventional TIPS creation had failed as a result of PV thrombosis or distorted anatomy. Intravascular US guidance for TIPS creation was additionally useful in a patient with Budd–Chiari syndrome and in a patient with intrahepatic tumors. Conclusions Intravascular US is a safe and reproducible means of real-time image guidance for TIPS creation, equivalent in efficacy to conventional fluoroscopic guidance. Real-time sonographic guidance with intravascular US may prove advantageous for cases in which there is PV thrombus, distorted anatomy, Budd–Chiari syndrome, or hepatic tumors.

We demonstrate terahertz (THz) imaging and spectroscopy of single-layer graphene deposited on an intrinsic Si substrate using THz time-domain spectroscopy. A single-cycle THz pulse undergoes multiple internal reflections within the Si substrate, and the THz absorption by the graphene layer accumulates through the multiple interactions with the graphene/Si interface. We exploit the large absorption of the multiply reflected THz pulses to acquire high-contrast THz images of graphene. We obtain local sheet conductivity of the graphene layer analyzing the transmission data with thin-film Fresnel formula based on the Drude model.

The properties of the scattered acoustic vector fields generated by simple spheres illuminated by monotonic continuous wave (CW) plane waves are investigated. Analytical solutions are derived from general acoustic pressure scattering models and analyzed for wave numbers in the resonance region. Of particular interest is the understanding of the characteristics of the scattered acoustic vector field in the near-to-far-field transition region. The separable active and reactive components of the acoustic intensity are used to investigate the structural features of the scattered field components. Numerical results are presented for the near and transition regions for a rigid sphere. A method of mapping nulls in the scattered intensity field components is described. The analysis is then extended to include a simple fluid-filled boundary and finally the evacuated thin-walled shell. Near field acoustic intensity field structures are compared against mechanical material properties of vacuous shells. The ability to extract scattered field features is illustrated with measurements obtained from a recent in-air experiment using an anechoic chamber and acoustic vector sensor probes to measure the scattered acoustic vector field from rigid spheres.

We report on a method to produce free-standing graphene sheets from epitaxial graphene on silicon carbide (SiC) substrate. Doubly clamped nanomechanical resonators with lengths up to 20 microm were patterned using this technique and their resonant motion was actuated and detected optically. Resonance frequencies of the order of tens of megahertz were measured for most devices, indicating that the resonators are much stiffer than expected for beams under no tension. Raman spectroscopy suggests that the graphene is not chemically modified during the release of the devices, demonstrating that the technique is a robust means of fabricating large-area suspended graphene structures.

Purpose: To evaluate the safety of inferior vena cava (IVC) filter retrieval in therapeutically anticoagulated patients in comparison to prophylactically or not therapeutically anticoagulated patients with respect to retrieval-related hemorrhagic complications. Materials and methods: This was a retrospective study of 115 consecutive attempted IVC filter retrievals in 110 patients. Filter retrievals were stratified as performed in patients who were therapeutically anticoagulated (group 1), prophylactically anticoagulated (group 2), or not therapeutically anticoagulated (group 3). The collected data included anticoagulant and antiplatelet medications (type, form and duration of administration, dosage) at the time of retrieval. Phone interviews and chart review was performed for the international normalized ratio (INR), activated partial thromboplastin time, platelet count, infusion of blood products, and retrievalrelated hemorrhagic complications. Results: Group 1 included 65 attempted filter retrievals in 61 therapeutically anticoagulated patients by measured INR or dosing when receiving low-molecular-weight heparin (LMWH). Four retrievals were not successful. In patients receiving oral anticoagulation, the median INR was 2.35 (range, 2 to 8). Group 2 comprised 23 successful filter retrievals in 22 patients receiving a prophylactic dose of LMWH. Group 3 included 27 attempted filter retrievals in 27 patients not receiving therapeutic anticoagulation. Six retrievals were not successful. Five patients were receiving oral anticoagulation with a subtherapeutic INR (median, 1.49; range, 1.16 to 1.69). No anticoagulation medication was administered in 22 patients. In none of the groups were hemorrhagic complications related to the retrieval procedures identified. Conclusions: These results suggest that retrieval of vena cava filters in anticoagulated patients is safe. Interruption or reversal of anticoagulation for the retrieval of vena cava filters is not indicated. (CHEST 2007; 132:31–36)

Endoscopic surgical applications of erbium lasers have been limited due to the lack of a suitable mid-IR optical fiber delivery system. Germanium oxide fibers are used for noncontact tissue ablation, but are not recommended for contact tissue ablation applications typically required during endoscopic surgery. This study describes the assembly and characterization of hybrid mid-IR fibers consisting of germanium oxide trunk fibers and sapphire fiber tips, and side-firing germanium oxide fibers with either 45deg angled tips or micromirrors. Average powers up to 8.5 W (850 mJ at 10 Hz) and transmission up to 70% was demonstrated through 450-mum-core, 1.5-m-long fibers, sufficient for endoscopic laser ablation of soft and hard tissues in contact mode.

Here, we report organic thin film transistors based on vertical heterojunctions of graphene and organic semiconductors. In these thin heterostructure devices, current modulation is accomplished by tuning of the injection barriers at the semiconductor/graphene interface with the application of a gate voltage. N-channel devices fabricated with a thin layer of C60 show a room temperature on/off ratio >104 and current density of up to 44 mAcm–2. Because of the ultrashort channel intrinsic to the vertical structure, the device is fully operational at a driving voltage of 200 mV. A complementary p-channel device is also investigated, and a logic inverter based on two complementary transistors is demonstrated. The vertical integration of graphene with organic semiconductors via simple, scalable, and low-temperature fabrication processes opens up new opportunities to realize flexible, transparent organic electronic, and optoelectronic devices.

A previously healthy 27-year-old woman (gravida 2 para 1) presented in the 42nd week of an otherwise ordinary pregnancy with right flank pain radiating to the groin. Membranes were intact, the cervix was long and closed, and no uterine contractions were present. Fundal height was appropriate (40 cm) and fetal assessment was unremarkable. She was afebrile with mild tachycardia (117 bpm) and was extremely tender in the right flank. Routine laboratory tests were within normal limits except for the urinalysis, which revealed 31 RBC. She was admitted to the labor and delivery unit for intravenous hydration with a presumed diagnosis of right renal colic. Within a few hours of admission, the flank pain evolved into generalized abdominal pain and physical examination revealed marked abdominal tenderness. Simultaneously, fetal heart tracings began to show ominous decelerations and loss of variability, prompting an emergency delivery for suspected placental abruption. In the operating room, a live male fetus weighing 7 lb 14 oz. was delivered by cesarean section (APGAR, 7/9). Placental abruption was not confirmed. Rather, inspection of the maternal abdomen revealed a hemoperitoneum of approximately 200 cm which was clotted and dark in color. A retroperitoneal hematoma was identified inferior to the liver which appeared to be the source of the intraperitoneal blood. No active bleeding was observed, so the abdomen was closed. However, her postoperative hematocrit level of 16.8% was lower than anticipated for the combined operative blood loss and hemoperitoneum (her hematocrit level on admission was 32.8%). A total of six units of blood were transfused postoperatively as the patient’s hematocrit level decreased on two further occasions. Computed tomography (CT) of the abdomen was performed to confirm the presence of a retroperitoneal hemorrhage on the right side as seen at surgery.

An inadvertent case of pericardial placement of a central venous catheter was successfully treated by direct thrombin injection.

To prospectively evaluate the clinical efficacy of polyethylene-covered metallic Z-stents in treatment of dysphagia secondary to malignant esophageal obstruction and esophagorespiratory fistula.Thirty-five patients with dysphagia due to malignant esophageal obstruction (n = 32) and esophagorespiratory fistula (n = 3) were treated with polyethylene-covered Gianturco-Rösch Z-stents.Thirty-nine stents were placed in 35 patients. Stent placement was technically successful in all patients. Improvement in dysphagia was achieved in 34 of 35 patients. The average dysphagia score decreased from 3.1 (dysphagia to liquids) to 0.6 (essentially normal diet). An esophagorespiratory fistula was completely sealed in two of three patients. All 35 patients were followed up clinically at 1 day and 1 week and at 3-month intervals (range, 1 week to 18 1/2 months; mean, 4.8 months). Recurrent dysphagia or aspiration occurred in only three of 34 (9%) patients whose disease was initially palliated and was easily treated in all cases. Nine complications occurred in eight patients (23%) and included chest pain that required analgesia (n = 3), food impaction (n = 1), stent migration (n = 2), and upper gastrointestinal tract hemorrhage (n = 3).Polyethylene-covered stents are a relatively safe and effective means of long-term palliation in patients with severe malignant esophageal obstruction and esophagorespiratory fistula. These stents are easily deployed, and the rate of stent migration is relatively low.

We describe two cases of airway foreign bodies (FB) consisting of a dental crown. The shape and composition of dental crowns complicate their extraction from the tracheobronchial tree, sometimes necessitating thoracotomy. We describe the use of a multidisciplinary approach involving rigid and flexible bronchoscopy in concert with the use of wire snares under fluoroscopic guidance for extraction of these challenging FB. These cases illustrate that this multidisciplinary approach can allow successful extraction of the difficult FB from much of the tracheobronchial tree and the avoidance of thoracotomy.

Resonant mechanics of high quality factor (Q) graphene coated silicon nitride devices have been explored using optical and electrical transduction schemes. With the addition of the graphene layer, we retain the desirable mechanical properties of silicon nitride but utilize the electrical and optical properties of graphene to transduce and tune the resonant motion by both optical and electrical means. By positioning the graphene-on-silicon-nitride drums in a tunable optical cavity, we observe position dependent damping and resonant frequency control of the devices due to optical absorption by graphene.

High stress stoichiometric silicon nitride resonators, whose quality factors exceed one million, have shown promise for applications in sensing and signal processing. Yet, electrical integration of the insulating silicon nitride resonators has been challenging, as depositing even a thin layer of metal degrades the quality factor significantly. In this work, we show that graphene used as a conductive coating for Si3N4 membranes reduces the quality factor by less than 30 % on average, which is minimal when compared to the effect of conventional metallization layers such as chromium or aluminum. The electrical integration of Si3N4-Graphene (SiNG) heterostructure resonators is demonstrated with electrical readout and electro-static tuning of the frequency by up to 1 % per volt. Background: Mediastinal malignancies may involve the esophagus, leading to esophageal stenosis and dysphagia. Rigid and self-expanding esophageal stents have been used for effective palliation, but their use in extrinsic, compressive lesions is controversial. Methods: A retrospective review of self-expanding Gianturco-Rosch Z-stents that were successfully placed in 13 patients with malignant esophageal obstruction due to extrinsic lesions. Results: All patients had an improvement in dysphagia of at least two dysphagia grades. The mean dysphagia grade fell from 3.15 to 0.62. Mean survival was 2.2 months. Early (within 48 hours) procedure-related complications occurred in 4 of 13 patients and consisted of minor, transient chest pain that resolved within 6 hours (3 patients) and endoscopic stent dislodgment into the stomach (1 patient). Late complications (>48 hours) occurred in 2 patients and consisted of a partial proximal stent migration and the development of a benign stricture proximal to the stent. There was no procedural or stent related mortality. Conclusions: Esophageal obstruction and malignant dysphagia from extrinsic, compressive mediastinal malignancies can be effectively and safely palliated with self-expanding Gianturco-Rosch Z-stents. Purpose To evaluate expandable metallic Gianturco-Rosch Z (GRZ) stents for treatment of benign and malignant tracheobronchial stenoses. Patients and Methods Six patients, ages 45–73 years, were treated for severe dyspnea with placement of GRZ stents. Three patients had benign tracheal lesions (one tracheomalacia, two postoperative) and received uncovered GRZ stents. Three patients had malignant stenoses at the level of the carina; one received an uncovered stent and the other two received silicone-covered GRZ stents. Results Two patients with benign lesions responded well to stent placement. One was asymptomatic for a year and then was lost to follow-up; the other improved substantially but died of end-stage lung disease 5 months after stent placement. A third patient with a benign high tracheal lesion did poorly; symptoms recurred secondary to inferior migration of a stent, which was removed surgically at 4 months. All patients with malignant lesions improved symptomatically after stent placement and remained without significant dyspnea until death (from 1 to 6 months). Conclusion Expandable GRZ stents are promising devices for treatment of benign lesions and offer effective palliation of malignant tracheobronchial stenoses.

By virtue of their low mass and stiffness, atomically thin mechanical resonators are attractive candidates for use in optomechanics. Here, we demonstrate photothermal back-action in a graphene mechanical resonator comprising one end of a Fabry–Perot cavity. As a demonstration of the utility of this effect, we show that a continuous wave laser can be used to cool a graphene vibrational mode or to power a graphene-based tunable frequency oscillator. Owing to graphene’s high thermal conductivity and optical absorption, photothermal optomechanics is efficient in graphene and could ultimately enable laser cooling to the quantum ground state or applications such as photonic signal processing.

We demonstrate THz imaging and time-domain spectroscopy of a single-layer graphene film. The large-area graphene was grown by chemical vapor deposition on Cu-foil and subsequently transferred to a Si substrate. We took a transmission image of the graphene/Si sample measured by a Si:bolometer (pixel size is 0.4-mm). The graphene film (transmission: 36 - 41%) is clearly resolved against the background of the Si substrate (average transmission: 56.6%). The strong THz absorption by the graphene layer indicates that THz carrier dynamics are dominated by intraband transitions. A theoretical analysis based on the Fresnel coefficients for a metallic thin film shows that the local sheet resistance varies across the sample from 420 to 590 Ohm, consistent with electron mobility ~ 3,000 cm 2 V -1 s -1 . We also measured time-resolved THz waveforms through the Si substrate and the graphene/Si sample. The waveforms consist of a series of single-cycle THz pulses: a directly transmitted pulse, then subsequent "echos" corresponding to multiple reflections from the substrate. The amplitude difference between graphene/Si pulses and Si pulses becomes more pronounced as the pulses undergo more reflections. From these measurements, we obtained spectrally flat transmission spectra of the transmitted pulses and the average sheet resistance 480 Ohm, consistent with the results of the power transmission measurement. The flat spectral responses indicate that the carrier scattering time in our graphene sample is much shorter than the THz pulse duration.

THz imaging and spectroscopy using broadband THz pulses map out the THz carrier dynamics of large-area single-layer graphene. Non-contacting, non-destructive THz probing reveals the local sheet conductivity of the graphene samples.

Esophagogastric malignancies often are manifested with progressive dysphagia or esophagorespiratory fistulas. Palliative modalities currently available have significant limitations. A modified Gianturco-Rosch silicone-covered self-expanding metallic Z stent was used in 32 consecutive patients with malignant esophageal obstruction (n =24) or esophagorespiratory fistulas (n = 8). The stent was placed successfully in all patients. Dysphagia improved by at least two grades in 21 of the 24 patients (87.5%); the mean dysphagia grade fell from 3.21 to 1.08. Six of the 8 patients with fistulas were able to resume a normal diet, and the other 2 were able to eat solids without symptoms of aspiration. Complications occurred in 10/32 patients (31%) and included stent migration (4 patients), food impaction (2 patients), membrane disruption with tumor ingrowth (1 patient), tumor overgrowth (1 patient), early pressure necrosis with hemorrhage (1 patient), and late pressure necrosis with sepsis (1 patient). The latter 2 patients died, giving a mortality rate of 6.3%. Many complications were managed with endoscopic or interventional radiologic techniques. Although randomized prospective clinical trials are needed, the silicone-covered Gianturco-Rosch Z stent offers promise for the effective palliation of malignant esophageal obstruction and esophagorespiratory fistulas. Graphene's unparalleled strength, stiffness, and low mass per unit area make it an ideal material for nanomechanical resonators, but its relatively low quality factor is an important drawback that has been difficult to overcome. Here, we use a simple procedure to fabricate circular mechanical resonators of various diameters from graphene grown by chemical vapor deposition. In addition to highly reproducible resonance frequencies and mode shapes, we observe a striking improvement of the membrane quality factor with increasing size. At room temperature, we observe quality factors as high as 2400 ± 300 for a resonator 22.5 μm in diameter, about an order of magnitude greater than previously observed quality factors for monolayer graphene. Measurements of quality factor as a function of modal frequency reveal little dependence of Q on frequency. These measurements shed light on the mechanisms behind dissipation in monolayer graphene resonators and demonstrate that the quality factor of graphene resonators relative to their thickness is among the highest of any mechanical resonator demonstrated to date.

The single-shot interferometric imaging of normal mode dynamics in MEMS resonators, oscillating in the radio frequency (rf) regime, is demonstrated by synchronous imaging with a pulsed nanosecond laser. Profiles of mechanical modes in suspended silicon and graphene thin-film structures, and their extracted equilibrium profiles, are measured through nondestructive all-optical Fabry-Perot reflectance fits to the temporal traces. As a proof of principle, the modal patterns of a microdrum silicon resonator is analyzed, and its extracted vibration modes and equilibrium profile show good agreement with other characterization and numerical estimations.

We fabricated large arrays of suspended, single-layer graphene membrane resonators using chemical vapor deposition (CVD) growth followed by patterning and transfer. We measure the resonators using both optical and electrical actuation and detection techniques. We find that the resonators can be modeled as flat membranes under tension, and that clamping the membranes on all sides improves agreement with our model and reduces the variation in frequency between identical resonators. The resonance frequency is tunable with both electrostatic gate voltage and temperature, and quality factors improve dramatically with cooling, reaching values up to 9000 at 10 K. These measurements show that it is possible to produce large arrays of CVD-grown graphene resonators with reproducible properties and the same excellent electrical and mechanical properties previously reported for exfoliated graphene.

Nanomechanical resonators operating in vacuum are capable of detecting and weighing single biomolecules, but their application to the life sciences has been limited by viscous forces that impede their motion in liquid environments. A promising approach to avoid this problem, encapsulating the fluid within a mechanical resonator surrounded by vacuum, has not yet been tried with resonant sensors of mass less than approximately 100 ng, despite predictions that devices with smaller effective mass will have proportionally finer mass resolution. Here, we fabricate and evaluate the performance of doubly clamped beam resonators that contain filled nanofluidic channels and have masses of less than 100 pg. These nanochannel resonators operate at frequencies on the order of 25 MHz and when filled with fluid have quality factors as high as 800, 2 orders of magnitude higher than that of resonators of comparable size and frequency operating in fluid. Fluid density measurements reveal a mass responsivity of 100 Hz/fg and a noise equivalent mass of 2 fg. Our analysis suggests that realistic improvements in the quality factor and frequency stability of nanochannel resonators would render these devices capable of sensing attogram masses from liquid.

WHEN Ehrlich introduced arsphenamine (N. N. R.) in 1909 it was his sincere hope that syphilis could be cured with a single injection of the drug. It soon became apparent that one injection was woefully inadequate and, moreover, that three, six or even more injections failed to yield the desired result. With the introduction of bismuth as a powerful ally, the campaign against syphilis moved closer to its goal. Two decades after Ehrlich's discovery, following the admirable work of the Clinical Cooperative Group, 1 it was generally recognized that the use of alternating courses of the arsenicals and bismuth preparations, given continuously over a period of approximately twelve to eighteen months, in a large percentage of cases, gave the result which Ehrlich had sought in vain. TWO REPORTS have been published recently, 1 showing spectacularly favorable results in the prevention of prenatal syphilis with the use of penicillin in the treatment of syphilitic pregnant women. It is well known that mothers with syphilis which is beyond the infectious state may deliver normal nonsyphilitic children in a varying but large percentage of cases. Transmission of syphilis to the fetus occurs much more frequently when pregnant women have syphilis in the early infectious stages than in the later stages of the disease. The efficacy of any therapeutic procedure in preventing prenatal syphilis, therefore, will be evaluated most reliably by the effect on the offspring of mothers who during pregnancy had indisputable early infectious syphilis. When rigidly selected, the number of patients whom we saw who had proved infectious syphilis during pregnancy was limited. To obtain a satisfactory number of cases for evaluation it is necessary to compile the

Gianturco-Rösch expandable Z-stents were used in 22 patients with superior vena cava syndrome (SVCS). Stents were placed in all patients in the SVC and in 17 patients, also into the innominate veins. Stent placement resulted in complete relief of symptoms in all patients. Twenty-one patients had no SVCS recurrence from 1 to 16 months, to their death, or to the present time. SVCS recurred only in 1 patient 9 months after stent placement due to tumor ingrowth and secondary thrombosis. Based on ours and on other reported experiences, expandable metallic stents are effective devices for treatment of the SVCS which is difficult to manage by other means.

The colour of sportswear has been shown to influence the outcome of bouts for several different combat sports. The generality of these effects, and whether they extend to collaborative forms of contests (team sports), is uncertain. Since 1947, English football teams wearing red shirts have been champions more often than expected on the basis of the proportion of clubs playing in red. To investigate whether this indicates an enhancement of long-term performance in red-wearing teams, we analysed the relative league positions of teams wearing different hues. Across all league divisions, red teams had the best home record, with significant differences in both percentage of maximum points achieved and mean position in the home league table. The effects were not due simply to a difference between teams playing in a colour and those playing in a predominantly white uniform, as the latter performed better than teams in yellow hues. No significant differences were found for performance in matches away from home, when teams commonly do not wear their "home" colours. A matched-pairs analysis of red and non-red wearing teams in eight English cities shows significantly better performance of red teams over a 55-year period. These effects on long-term success have consequences for colour selection in team sports, confirm that wearing red enhances performance in a variety of competitive contexts, and provide further impetus for studies of the mechanisms underlying these effects.

Purpose To retrospectively review the indications and outcomes of transjugular liver biopsy (TJLB) with the left internal jugular vein (IJV) approach. Materials and methods A retrospective review of all TJLBs performed at one institution was performed. From August 1995 to October 2004, 293 liver biopsies were performed. Nineteen of the 293 procedures (6.5%) were performed from the left IJV in 17 patients. There were 11 men and six women aged 36–59 years (mean, 49.3 years). All patients were suspected of having diffuse hepatic parenchymal disease and had contraindications to conventional percutaneous liver biopsy. The stiff metal biopsy cannula was advanced across the mediastinum and through the heart over a stiff guide wire. Biopsy was performed from the right hepatic vein in 15 procedures, the middle hepatic vein in two, and the intrahepatic IVC in two. Results The indications for using the left IJV approach were nonusable right IJV in 12 procedures, difficult previous catheterization of the hepatic veins from the right IJV due to distorted hepatic anatomy in six procedures, and the operator’s predilection for this approach in one procedure (the patient had previously undergone liver transplantation). All biopsies were successful, with diagnostic tissue obtained in 100% of cases. Two patients had chest pain during the procedure associated with placement of the rigid cannula through the mediastinal veins. One patient had a vaso-vagal episode that necessitated the use of intravenous atropine but subsequently underwent successful biopsy. All episodes of chest pain resolved after the cannula was removed. There were no recorded hemorrhagic or cardiac complications. Conclusion TJLB can be safely and repeatedly performed from the left IJV when the right IJV is not available or the hepatic anatomy prevents catheterization of the hepatic veins from the right IJV.

A new study of contact calls in dolphins shows that individuals can recognize one another using information encoded in the frequency modulation pattern of these calls, in the absence of general voice characteristics.

Proper positioning of inferior vena cava (IVC) filters is necessary for effective protection from pulmonary embolism (PE). This report describes a case of an IVC filter malpositioned partially in the right common iliac vein that resulted in risk of recurrent PE caused by thrombus emanating from the left common iliac vein. A new filter was placed in the IVC above the first device, followed by retrieval of the malpositioned filter through the new filter.

In order to understand and address the environmental impact of sonar and acoustic systems testing, the Naval Undersea Warfare Center (NUWC) and its partners are developing new techniques and applications for the passive classification and tracking of marine mammals in designated Navy acoustic sensor testing areas. The combined transient and broadband nature of the whale clicks contains a richly unique structure that can be exploited for completely non-evasive tracking and localization. NUWC has developed unique harmonic classification tools and other advanced visual tactical decision aids for the US Navy's latest sonar systems, designed to maximize operator performance and awareness of the undersea environment. In this paper, we explore the use of harmonic frequency and temporal-patterned structures in the detection and tracking of whales on the Atlantic Undersea Test and Evaluation Center (AUTEC) acoustic range. We demonstrate and report the results of the analysis of actual at-sea recorded blue whale vocalizations utilizing advanced sonar visualization and classification tools, and new tracking applications being developed.

Introduction The primates comprise a diverse group of eutherian mammals, with between some 200 and 400 species, depending on the taxonomic authority consulted (e.g., Corbet & Hill, 1991; Wilson & Reeder, 2005). Most of these species dwell in tropical forests, but primates also thrive in many other habitats, including savannas, mountainous forests of China and Japan, and even some urban areas. Living primates are divided into two groups, the strepsirrhines (lemurs and lorises) and the haplorrhines (monkeys, apes, and tarsiers). Strepsirrhines include mostly arboreal species and retain several ancestral characteristics, including greater reliance on smell and (in most species) a dental comb that is used for grooming. Most are nocturnal, but some have, in parallel with most haplorrhines, evolved a diurnal niche. They are found only in the Old World tropics. Haplorrhines are more widely distributed geographically, being found in both the New and Old Worlds. They include two groups, the platyrrhines and the catarrhines. Platyrrhines are monkeys native to the New World. Catarrhines include both Old World monkeys and apes. With the exception of owl monkeys in the genus Aotus , all monkeys and apes are active during the day (i.e., diurnal), and most live in bisexual social groups that vary in size from 2 to well over 100 adults (Smuts, Cheney, Seyfarth, et al., 1987). Nonhuman primates are among the best-studied of mammals, in large part because of their close phylogenetic relatedness to humans.

The Virtual Table presents stereoscopic graphics to a user in a workbench-like setting. This device shares with other large- screen display technologies (such as data walls and surround- screen projection systems) the lack of human-centered unencumbered user interfaces and 3D interaction technologies. Such shortcomings present severe limitations to the application of virtual reality (VR) technology to time- critical applications as well as employment scenarios that involve heterogeneous groups of end-users without high levels of computer familiarity and expertise. Traditionally such employment scenarios are common in planning-related application areas such as mission rehearsal and command and control. For these applications, a high grade of flexibility with respect to the system requirements (display and I/O devices) as well as to the ability to seamlessly and intuitively switch between different interaction modalities and interaction are sought. Conventional VR techniques may be insufficient to meet this challenge. This paper presents novel approaches for human-centered interfaces to Virtual Environments focusing on the Virtual Table visual input device. It introduces new paradigms for 3D interaction in virtual environments (VE) for a variety of application areas based on pen-and-clipboard, mirror-in-hand, and magic-lens metaphors, and introduces new concepts for combining VR and augmented reality (AR) techniques. It finally describes approaches toward hybrid and distributed multi-user interaction environments and concludes by hypothesizing on possible use cases for defense applications.
