1. Post-traumatic Hemothorax: Management in a Limited-medium Structure

Abstract

Post-traumatic hemothorax is common in young adults in the context of road accidents. Explorations of the lesions may be limited by the technical platform. The objective of our study is to discuss the difficulty of managing post-traumatic hemothorax in a limited-medium structure. This was a 20-month retrospective study (01 May 2018-31 December 2019), involving 38 patients treated for post-traumatic hemothorax, whether or not associated with pneumothorax, in the general surgery department of the CHU Analankinina Toamasina. The hospital frequency was 1.66%, with a mean age of the patients of 31.89 years, and a male predominance (73.68%). The main provider of hemothorax was the civil liability accident (52.63%). Chest X-rays were performed in almost all patients (94.74%). Complications had been found in 4 patients with secondary surgery, with favorable outcomes. The frequency of hemothorax is constantly increasing in Madagascar, where the civil liability accident is the main cause. Chest X-ray remains the most accessible examination to be carried out in emergency. An early and efficient thoracic drainage generally ensures a favorable evolution without after-effects.

2. Digestive Surgical Emergencies at Analankininina Toamasina Madagascar University Hospital during the Covid-19 Pandemic

Abstract

During the period of total containment during the Covid-19 pandemic; our department became the sole treatment center for all surgical emergencies. The influx of many patients made the task very difficult for the surgeon. This is a 3-month descriptive prospective study in the visceral surgery department at the Analankininina Toamasina University Hospital Center. Of the 81 patients hospitalized during the study, 32 patients were admitted for digestive surgery emergency, i.e. 39.50%. The mean age was 34.87 years with a sex ratio of 1.66, predominantly male. Abdominal pain occupied 43.75% of the reason for consultation, the most frequent pathologies were acute intestinal obstruction (18.75%) and acute appendicitis (18.75%). An exploratory laparotomy is performed on 40.62% of patients. Many patients were operated on for proctologic emergency (18.75%). The average length of hospitalization is 4.18 days. The visceral emergencies are present despite the presence of the Covid-19 pandemic and the activities of the health personnel are uninterrupted.

Abstract

Acute appendicitis was often diagnosed at the complication stage during the Covid19 pandemic at CHU Analankininina Toamasina. Our objective was to discuss the particularity of the management of appendicitis during the pandemic. Our study was prospective, descriptive for 6 months and took place in the general surgery department of the Analankininina Toamasina University Hospital Center, Madagascar from March 2020 to August 2020. We had collected 28 patients. The sex ratio was 3.66. The average age was 27.57 years. Appendicular abscess represented 64.29% (n=18) of the appendicular patients operated in emergency. Appendicular plastron occupied 14.29% (n=2) and generalized acute peritonitis was 7.14% (n=2). Post-operative follow-up was simple in 71.43% (n=20). The average length of hospitalization was 4 days. Acute appendicitis is often complicated during the Covid 19 pandemic in our hospital center. Surgery was systematic in our hospital, contrary to some literature which advocated antibiotic therapy at an early stage in front of the health emergency.

4. It's not just poverty: unregulated global market and bad governance explain unceasing deforestation in Western Madagascar

Abstract

Madagascar is recognized both for its unparalleled biodiversity and the high level of threat suffered by this biodiversity, associated in particular with anthropogenic deforestation. Despite sustained efforts to fight poverty and curb deforestation, forest cover in Madagascar is rapidly decreasing. To try to explain why it is so difficult to stop deforestation in Madagascar, we analyzed the recent deforestation process in Western Madagascar through satellite image analysis and field surveys. We show that deforestation has increased from less than 0.9%/yr on 2000-2010 to more than 2%/yr on 2010-2017. We identified two major causes of deforestation, which were not associated with subsistence agriculture: slash-and-burn agriculture for the cultivation of cash crops (maize and peanut), and uncontrolled fires to create open pasture. Maize production is mainly at the destination of the domestic market and is used in particular for livestock feeding. Peanut production has boomed since 2013 and more than half of it is now exported towards Asiatic countries. The money earned by farmers is principally invested into zebu herd acquisition. Trade of agricultural commodities benefits several intermediaries, some of whom have political responsibilities thus creating conflicts of interest. On the other hand, agents from institutions in charge of the management of the protected areas have no means to enforce laws against deforestation. In the absence of an efficient strategy to stop deforestation, we predicted that 38-93% of the forest present in 2000 will have disappeared in 2050. Forest loss, apart from biodiversity loss and climate-change global issues, will be at the expense of local population. In order to stop deforestation, international aid should be used to improve local governance to enforce environmental laws and pressure should be put on trading companies to buy certified agricultural commodities that are not derived from deforestation.

Abstract

Land degradation and regeneration are complex processes that greatly impact climate regulation, ecosystem service provision, and population well-being and require an urgent and appropriate response through land use planning and interventions. Spatially explicit land change models can greatly help decision makers, but traditional regression approaches fail to capture the nonlinearity and complex interactions of the underlying drivers. Our objective was to use a machine learning algorithm combined with high-resolution data sets to provide simultaneous and spatial forecasts of deforestation, land degradation, and regeneration for the next two decades. A 17,000-km(2) region in the south of Madagascar was taken as the study area. First, an empirical analysis of drivers of change was conducted, and then, an ensemble model was calibrated to predict and map potential changes based on 12 potential explanatory variables. These potential change maps were used to draw three scenarios of land change while considering past trends in intensity of change and expert knowledge. Historical observations displayed clear patterns of land degradation and relatively low regeneration. Amongst the 12 potential explanatory variables, distance to forest edge and elevation were the most important for the three land transitions studied. Random forest showed slightly better prediction ability compared with maximum entropy and generalized linear model. Business-as-usual scenarios highlighted the large areas under deforestation and degradation threat, and an alternative scenario enabled the location of suitable areas for regeneration. The approach developed herein and the spatial outputs provided can help stakeholders target their interventions or develop large-scale sustainable land management strategies.

Abstract

Summary Background Maternal and neonatal mortality is high in Africa, but few large, prospective studies have been done to investigate the risk factors associated with these poor maternal and neonatal outcomes. Methods A 7-day, international, prospective, observational cohort study was done in patients having caesarean delivery in 183 hospitals across 22 countries in Africa. The inclusion criteria were all consecutive patients (aged ≥18 years) admitted to participating centres having elective and non-elective caesarean delivery during the 7-day study cohort period. To ensure a representative sample, each hospital had to provide data for 90% of the eligible patients during the recruitment week. The primary outcome was in-hospital maternal mortality and complications, which were assessed by local investigators. The study was registered on the South African National Health Research Database, number KZ_2015RP7_22, and on ClinicalTrials.gov, number NCT03044899. Findings Between February, 2016, and May, 2016, 3792 patients were recruited from hospitals across Africa. 3685 were included in the postoperative complications analysis (107 missing data) and 3684 were included in the maternal mortality analysis (108 missing data). These hospitals had a combined number of specialist surgeons, obstetricians, and anaesthetists totalling 0·7 per 100 000 population (IQR 0·2–2·0). Maternal mortality was 20 (0·5%) of 3684 patients (95% CI 0·3–0·8). Complications occurred in 633 (17·4%) of 3636 mothers (16·2–18·6), which were predominantly severe intraoperative and postoperative bleeding (136 [3·8%] of 3612 mothers). Maternal mortality was independently associated with a preoperative presentation of placenta praevia, placental abruption, ruptured uterus, antepartum haemorrhage (odds ratio 4·47 [95% CI 1·46–13·65]), and perioperative severe obstetric haemorrhage (5·87 [1·99–17·34]) or anaesthesia complications (11·47 (1·20–109·20]). Neonatal mortality was 153 (4·4%) of 3506 infants (95% CI 3·7–5·0). Interpretation Maternal mortality after caesarean delivery in Africa is 50 times higher than that of high-income countries and is driven by peripartum haemorrhage and anaesthesia complications. Neonatal mortality is double the global average. Early identification and appropriate management of mothers at risk of peripartum haemorrhage might improve maternal and neonatal outcomes in Africa. Abstract

Background The African Surgical Outcomes Study (ASOS) showed that surgical patients in Africa have a mortality twice the global average. Existing risk assessment tools are not valid for use in this population because the pattern of risk for poor outcomes differs from high-income countries. The objective of this study was to derive and validate a simple, preoperative risk stratification tool to identify African surgical patients at risk for in-hospital postoperative mortality and severe complications. Methods ASOS was a 7-day prospective cohort study of adult patients undergoing surgery in Africa. The ASOS Surgical Risk Calculator was constructed with a multivariable logistic regression model for the outcome of in-hospital mortality and severe postoperative complications. The following preoperative risk factors were entered into the model; age, sex, smoking status, ASA physical status, preoperative chronic comorbid conditions, indication for surgery, urgency, severity, and type of surgery. Results The model was derived from 8799 patients from 168 African hospitals. The composite outcome of severe postoperative complications and death occurred in 423/8799 (4.8%) patients. The ASOS Surgical Risk Calculator includes the following risk factors: age, ASA physical status, indication for surgery, urgency, severity, and type of surgery. The model showed good discrimination with an area under the receiver operating characteristic curve of 0.805 and good calibration with c-statistic corrected for optimism of 0.784. Conclusions This simple preoperative risk calculator could be used to identify high-risk surgical patients in African hospitals and facilitate increased postoperative surveillance. Abstract

Summary Background There is a need to increase access to surgical treatments in African countries, but perioperative complications represent a major global health-care burden. There are few studies describing surgical outcomes in Africa. Methods We did a 7-day, international, prospective, observational cohort study of patients aged 18 years and older undergoing any inpatient surgery in 25 countries in Africa (the African Surgical Outcomes Study). We aimed to recruit as many hospitals as possible using a convenience sampling survey, and required data from at least ten hospitals per country (or half the surgical centres if there were fewer than ten hospitals) and data for at least 90% of eligible patients from each site. Each country selected one recruitment week between February and May, 2016. The primary outcome was in-hospital postoperative complications, assessed according to predefined criteria and graded as mild, moderate, or severe. Data were presented as median (IQR), mean (SD), or n (%), and compared using t tests. This study is registered on the South African National Health Research Database (KZ_2015RP7_22) and ClinicalTrials.gov (NCT03044899). Findings We recruited 11 422 patients (median 29 [IQR 10–70]) from 247 hospitals during the national cohort weeks. Hospitals served a median population of 810 000 people (IQR 200 000–2 000 000), with a combined number of specialist surgeons, obstetricians, and anaesthetists totalling 0·7 (0·2–1·9) per 100 000 population. Hospitals did a median of 212 (IQR 65–578) surgical procedures per 100 000 population each year. Patients were younger (mean age 38·5 years [SD 16·1]), with a lower risk profile (American Society of Anesthesiologists median score 1 [IQR 1–2]) than reported in high-income countries. 1253 (11%) patients were infected with HIV, 6504 procedures (57%) were urgent or emergent, and the most common procedure was caesarean delivery (3792 patients, 33%). Postoperative complications occurred in 1977 (18·2%, 95% CI 17·4–18·9]) of 10 885 patients. 239 (2·1%) of 11 193 patients died, 225 (94·1%) after the day of surgery. Infection was the most common complication (1156 [10·2%] of 10 970 patients), of whom 112 (9·7%) died. Interpretation Despite a low-risk profile and few postoperative complications, patients in Africa were twice as likely to die after surgery when compared with the global average for postoperative deaths. Initiatives to increase access to surgical treatments in Africa therefore should be coupled with improved surveillance for deteriorating physiology in patients who develop postoperative complications, and the resources necessary to achieve this objective. Abstract

Background: In developing countries, knowledge of antimicrobial resistance patterns is essential to define empirical therapy. Methodology: All the surgery and intensive care wards of two hospitals in Antananarivo were included to study the antimicrobial susceptibility of the pathogenic bacteria causing nosocomial infections. A repeated cross-sectional survey was conducted between September 2006 and March 2008, one day per week. Isolates were identified using classical methods, and resistance to antibiotics was assessed according to the recommendations of the Antibiogram Committee of the French Microbiology Society. Results: Clinical specimens from 706 from 651 patients were collected. Of the 533 bacterial pathogens, 46.7% were Enterobacteriaceae , 19.3% were Staphylococcus aureus , and 19.1% were pathogens from the hospital environment ( Pseudomonas aeruginosa and Acinetobacter baumannii ).Frequencies of resistance were high, particularly in Enterobacteriaceae ; however, the rate of Staphylococcus aureus isolates resistant to oxacillin (13.6 %) was moderate and all these isolates were susceptible to glycopeptids. The percentages of isolates susceptible to ceftazidim were 81.8% for E. coli , 60.9% for Klebsiella , and 52.5% for Enterobacter spp. Resistance to third-generation cephalosporins was due to extended spectrum betalactamases (ESBL). Multivariate analysis showed that diabetes (adjusted OR: 3.9) and use of an invasive procedures (adjusted OR: 3.5) were independent risk factors for resistance to third-generation cephalosporins. Conclusion: A nationwide surveillance programme is needed to monitor the microbial trends and antimicrobial resistance in Madagascar.
