Background Azithromycin and trimethoprim-sulfamethoxazole (SXT) are widely used to treat undifferentiated febrile illness (UFI). We hypothesized that azithromycin is superior to SXT for UFI treatment, but the drugs are noninferior to each other for culture-confirmed enteric fever treatment. Methods We conducted a double-blind, randomized, placebo-controlled trial of azithromycin (20 mg/kg/day) or SXT (trimethoprim 10 mg/kg/day plus sulfamethoxazole 50 mg/kg/day) orally for 7 days for UFI treatment in Nepal. We enrolled patients >2 years and, We compared azithromycin with trimethoprim-sulfamethoxazole for 7 days in the treatment of undifferentiated febrile illness in Nepal. Despite similar fever clearance time and treatment failure in the 2 arms, significantly fewer complications and relapses were noted in the azithromycin arm.

P2A (anterior) segment aneurysm of posterior cerebral artery is one of the rare aneurysm of posterior circulation. We report a case of 30 years old right handed young man who presented with features of SAH in Emergency Department and cerebral angiogram confirmed left posterior cerebral artery P2A segment thrombosedfusi form aneurysm. Postoperative recovery was good except left sided ptosis after microsurgical trapping via left temporal approach. In this paper, an efficient speed control technique is proposed for Direct Torque Control(DTC) of Permanent Magnet Synchronous Machine(PMSM) drive in order to reduce the ripple content in torque and flux. Though conventional DTC gives fast dynamic response, it has noticeably high ripple content due to the variable operating frequency of the inverter especially at lower rpm. To enervate this issue, SVPWM method is employed in this paper thereby making the inverter operating frequency independent of rotational speed. In order to further reduce the ripple content, a 3-level cascaded inverter is used. SVM is implemented using instantaneous voltage control technique for mathematical simplicity. The proposed method is implemented in Matlab/Simulink and the results obtained are compared to those of conventional DTC. It is observed from the results that there is a significant reduction in ripple content in the proposed technique when compared to the conventional method.

Case RepoR t A 10-year-old boy was reported to the Paediatric Dental Department for the treatment of fractured upper front tooth with esthetic concern. Patient gave history of trauma 6 months back due to fall from a bicycle. Clinical examination revealed Ellis class II (uncomplicated) fracture irt 21 [Table/Fig-1a]. The tooth was asymptomatic without any associated soft or hard tissue injuries to the supporting tissues and responded to EPT. Intraoral periapical radiograph confirms the absence of pulpal or periapical pathosis [Table/Fig-1b]. So, it was planned to restore the fractured segment using direct composite restoration technique. A 45°bevel was given to remove the unsupported enamel and increase the surface area. Preliminary impression of the upper and lower arches were made using fast setting alginate (Algitex; DPI, Mumbai, India) and dental stone study models were achieved. Mock preparation of the lost tooth structure was done using modelling wax and further checked for occlusal interference. After crown build up, the cast was duplicated by using Polyvinyl siloxane putty impression material (AFFINIS, COLTENE ADHESIVE AC) by using a disposable custom tray [Table/Fig-2]. Labial surface of the putty template was removed up to middle third of the crown, to aid in the reconstruction of the lost tooth structure. A clinical try- in of the template was done to ensure adequate fit [Table/Fig-3]. After appropriate shade selection of the composite material, this crown former was used to restore the fractured tooth quickly with minimal post-restoration finishing [Table/Fig-4].
