Objectives The objective of this systematic review was to evaluate information on the levels of MMP-8 in patients diagnosed with prediabetes or type 2 diabetes mellitus with periodontal disease, analyzing its validity as a possible biomarker for the diagnosis and progression of periodontal disease (PD). Methods A systematic search of the following databases was performed: PubMed/Medline, CENTRAL (The Cochrane Library), EMBASE and Web of Science. Studies involving the evaluation of MMP-8 in patients with prediabetes or patients presenting type 2 diabetes mellitus concomitantly with PD were selected. The evaluation of the methodological quality of the selected studies was based on the methodological bias risk analysis (QUADAS-2). Results Eight of the initially identified 2683 articles were selected. In all the selected studies, evaluator calibration and the use of clear methods for patient diagnosis with periodontal disease were present. Studies have demonstrated significantly higher MMP-8 concentrations in PD patients compared to controls, as well as in patients presenting more advanced stages of PD. However, controversies regarding MMP-8 levels in prediabetes/diabetes type 2 patients with PD. Conclusions Higher MMP-8 levels in patients with PD compared to controls imply the potential use of MMP-8 in the diagnosis of PD. The influence of patient glycemic state, as well as medications these patients make use of, are factors that possibly contribute to the modulation of MMP-8 concentrations in patients with diabetes and should be analyzed, aiming at a better understanding of the relationship between glycemic state and MMP-8 levels in patients with PD.

Objective To aim of this systematic review was to explore the relationship between Human papillomavirus (HPV) and epithelial-mesenchymal transition (EMT) related to the prognosis of oropharyngeal squamous cell carcinoma (OPSCC). Design For this systematic review, searches were performed in PubMed, Web of Science, Scopus, Science Direct, and Cochrane, and a random-effects model was used for meta-analysis. The presence of EMT was confirmed by the loss of E-cadherin immunoexpression and overexpression of vimentin. Results In summary, EMT-related proteins were expressed regardless of HPV status; however, overall survival was better in HPV-positive OPSCC cases, with a 5.88 times lower death risk compared to HPV-negative patients (OR = 0.17; 95%CI = 0.10–0.30). Likewise, the maintenance of E-cadherin in OPSCC was associated with an 11.11 times lower risk of death due to the disease (OR = 0.09; 95%CI = 0.01–0.88). Conclusions More advanced clinical stages (III/IV) and the presence of lymph node metastases (N1–3) were common in OPSCC but were not significantly associated with HPV status.

Periodontal disease is characterized as a disorder of the oral microbiota resulting in an immune response which, in turn, leads to the destruction of periodontal tissue. Matrix metalloproteinase-8 (MMP-8) has been reported as the major metalloproteinase involved in periodontal disease, being present at high levels in gingival crevicular fluid and salivary fluid (SF). The aim of this systematic review was to evaluate the scientific literature regarding the expression of MMP-8 in gingival crevicular fluid and SF in patients with periodontal disease, analyzing its validity as a possible biomarker in the diagnosis of periodontal disease. A systematic review of the literature was performed using the PubMed/Medline, CENTRAL and Science Direct databases. Studies concerning the use of MMP-8 in the diagnosis of periodontal disease that evaluated its effectiveness as a biomarker for periodontal disease were selected. The search strategy provided a total of 6483 studies. After selection, six articles met all the inclusion criteria and were included in the present systematic review. The studies demonstrated significantly higher concentrations of MMP-8 in patients with periodontal disease compared with controls, as well as in patients presenting more advanced stages of periodontal disease. The findings on higher MMP-8 concentrations in patients with periodontal disease compared with controls imply the potential adjunctive use of MMP-8 in the diagnosis of periodontal disease.

Objectives The aim of this study was to investigate the prevalence of oral and maxillofacial lesions among older adults (≥60 years) from representative regions in Brazil. Design Retrospective descriptive cross-sectional study. Setting Biopsy records were obtained from the archives of four Brazilian referral centers of oral diagnosis between 2000 and 2016. Participants A total of 45,506 biopsy records of all patients were analyzed, of these 7,259 persons aged 60 and older were selected. Measurements Data such as gender, age, race, anatomical location, and histopathological diagnosis were collected and categorized. Pearson's chi-square test (P < .005) was used to evaluate differences in the frequency of the several groups of oral lesions. Results Oral and maxillofacial lesions were diagnosed in 7,259 older people, including 59.4% women (P < .001) and 61.3% white patients (P = .07). The most commonly affected sites were the cheek mucosa (20.3%) and mandible (8.9%) (P < .001). Reactive and inflammatory lesions were the most common lesions, followed by neoplasms. Oral squamous cell carcinoma was the most prevalent neoplasm (83.4%) (P < .001). Conclusion Knowledge of oral diseases obtained from biopsy records provides more accurate data about the diagnosis and oral health of elderly patients. These indicators thus support the development of specific health policies for the prevention and treatment of oral and maxillofacial lesions that affect this population.

Objective: To identify and describe clinical and epidemiologic profile of oral squamous cell carcinoma (OSCC) in young adults. Study Design: A systematic literature review was carried out in electronic databases Lilacs, SciELO, PubMed, and Periodico Capes, whose descriptors were: oral cancer, oral squamous cell carcinoma, epidemiology and young patient, in English, Spanish, and Portuguese, between January 2003 and January 2016. The review identified 348 articles, 8 of them had as main objective to evaluate clinical and epidemiologic aspects of OSCC in young adults between 20 and 45 years. Results: Tobacco use and/or alcoholism stand out among the etiologic factors related to OSCC. The tongue was the most affected region in oral cavity, followed by the lower lip and buccal floor. Regarding clinical tumor stage, it was observed, in 4 studies, that type III and IV prevailed, and regional-type metastasis was the most common. The predominant treatment associated surgery with radiotherapy, followed by isolated surgery. Conclusion: Further studies are required to define the clinical and epidemiologic characteristics of the tumor in this population and also to standardize the age group referring to the term "young adult." This information may enable the development of a primary prevention program for patients with OSCC.

Lichen planus is a chronic inflammatory disease involving the skin and mucosa, which often affects the oral cavity. The objective of this study is to report a case of oral lichen planus (OLP) with cutaneous manifestations and to discuss the clinical, histopathological aspects and the established treatment. A 61-years-old female white patient was referred for evaluation of white lesions in the oral mucosa. In the intraoral examination, multiple white lesions with striated appearance were observed in the jugal mucosa, tongue and border. The extraoral examination revealed scaly lesions on the arm, white spots on the legs, and nail dystrophy on feet. Based on biopsy of the oral lesions and the histopathological analysis, the diagnosis of OLP was confirmed. The patient underwent treatment with clobetasol propionate topical cream (0.5 mg), and was instructed to apply it to affected area, once or twice a day for four weeks. In the clinical follow-up after one month and 15 days, the improvement of the lesions could be analyzed. As OLP is a disease with an etiopathogenesis that is still poorly recognized, several factors may enable the development of this condition. Therefore, the dentist's clinical view is essential for the most effective treatment.

Odontogenic keratocyst (OKC) classified by the World Health Organization as a cystic lesion of benign development stands out among the other odontogenic lesions due to potentially aggressive biological behavior, with a tendency to recurrence. The present study aims to report an OKC case in the mandible, as well as discuss aspects related to the radiographic findings and types of treatments of this lesion. A male patient exhibited a circumscribed lesion, and a hard consistency, lasting for 6 months, measuring approximately 1.0 cm in extension in the posterior region of the left mandible. Under the clinical hypothesis of radicular cyst, incisional biopsy was performed. Based on the definitive diagnosis of odontogenic keratocystic after histopathological examination, tooth removal was performed and an intraosseous device was placed to allow OKC decompression. After 1 year of observation, no evidence of relapse was found, and total lesion regression was observed. The findings of the present case highlight the importance of the correct diagnosis and emphasize the importance of a thorough clinical examination of the oral cavity.

This study reports a case of oral lichen planus (OLP) with cutaneous manifestations and thereafter discusses the clinical, histopathologic, and established treatment aspects. A 61-year-old leucoderm female patient was referred for evaluation of white lesions in the oral mucosa. On intraoral examination white striated lesions in jugal mucosa, tongue, and collar were observed. Extraoral examination revealed scaly lesions on the arm, white spots on the legs, and dystrophic nail feet. A biopsy of the oral lesions was performed, and the histopathologic analysis confirmed the clinical diagnosis of OLP. The treatment was clobetasol propionate in cream (0.5 mg) applied to the affected area once or twice a day for 4 weeks. In the clinical follow-up after 1 month and 15 days, the improvement of the lesions could be analyzed. OLP is a disease with etiopathogenesis still little recognized, and several factors may allow the development of this condition.

The aim of this study was to report a case of a male patient with persistent burning and red stains on the lingual dorsum. Laboratory tests, such as blood count, VDRL, anti–human immunodeficiency virus (HIV), and vitamin B12 levels, were requested to rule out the diagnostic hypothesis of candidiasis, syphilis, and HIV as the cause of the injuries. The results of the tests were within normal ranges, except for vitamin B12, which presented values far below expectations, indicating hypovitaminosis. The patient was referred to a specialist, a treatment with vitamin B12 supplementation was performed, and symptoms in the oral cavity disappeared. We conclude that the request of laboratory exams and a multiprofessional approach is extremely important to give the patient a better prognosis.

Analysis of GLUT-1, GLUT-3, and angiogenic index in syndromic and non-syndromic keratocystic odontogenic tumors The aim of this study was to evaluate the immunoexpression of glucose transporters 1 (GLUT-1) and 3 (GLUT-3) in keratocystic odontogenic tumors associated with Gorlin syndrome (SKOTs) and non-syndromic keratocystic odontogenic tumors (NSKOTs), and to establish correlations with the angiogenic index. Seventeen primary NSKOTs, seven recurrent NSKOTs, and 17 SKOTs were selected for the study. The percentage of immunopositive cells for GLUT-1 and GLUT-3 in the epithelial component of the tumors was assessed. The angiogenic index was determined by microvessel count. The results were analyzed statistically using the nonparametric Kruskal-Wallis test and Spearman’s correlation test. High epithelial immunoexpression of GLUT-1 was observed in most tumors (p = 0.360). There was a higher frequency of negative cases for GLUT-3 in all groups. The few GLUT-3-positive tumors exhibited low expression of this protein in epithelial cells. No significant difference in the angiogenic index was observed between groups (p= 0.778). GLUT-1 expression did not correlate significantly with the angiogenic index (p > 0.05). The results suggest that the more aggressive biological behavior of SKOTs when compared to NSKOTs may not be related to GLUT-1 or GLUT-3 expression. GLUT-1 may play an important role in glucose uptake by epithelial cells of KOTs and this process is unlikely related to the angiogenic index. GLUT-1 could be a potential target for future development of therapeutic strategies for KOTs.Keywords: Odontogenic Cysts; Odontogenic Tumors; Glucose Transporter Proteins, Facilitative.
