Introduction Les patients drépanocytaires ont très tôt été considérés comme personnes “fragiles” vis-à-vis de la COVID-19 car, indépendamment du risque propre de l’infection, il est bien établi que toute infection peut déclencher une crise vaso-occlusive (CVO). Il y a cependant peu d’études descriptives sur larges cohortes décrivant les caractéristiques des patients drépanocytaires avec COVID-19. Nous avons voulu comparer les caractéristiques des patients drépanocytaires infectés par le SARS-Cov2 suivant qu’ils aient été ou non hospitalisés afin d’établir un profil des patients ayant nécessité une hospitalisation. Matériels et méthodes Étude de cohorte prospective, multicentrique et observationnelle Française. Dès le 13 mars 2020, nous avons établi en France grâce au réseau des centres de référence/compétence et de la filière de santé MCGRE (pédiatrie et médecine adulte), un appel à déclaration de toutes infections à SARS-CoV-2 consécutives confirmées par test RT-PCR sur écouvillon naso-pharyngé. Les caractéristiques cliniques simples concernant la drépanocytose et son traitement et les signes cliniques de l’infection ont été recueillis sur une fiche de recueil standardisée. Un modèle univarié a été utilisé pour comparer les caractéristiques des patients, par génotypes de drépanocytose et selon qu’ils aient été pris en charge en ambulatoire ou hospitalisés (passage au minimum d’une nuit aux urgences). Résultats 536 patients drépanocytaires infectés par le SARS-CoV-2 ont été inclus : 319 patients hospitalisés et 217 non hospitalisés. Il s’agissait de 431 drépanocytaires de génotypes S/S ou S/b0-thalassémie (âge moyen 27 ± 12,7 ans), 82 de génotype SC (âge moyen de 33,6 ± 15 ans) et 23 de génotype S/b ± halassémie (29,7 ± 15,4 ans). Nous détaillons dans ce résumé les données concernant les patients majoritaires, de génotype S/S ou S/b0-thalassémie. Chez ces patients, l’âge moyen, le sexe, l’index de masse corporelle, les antécédents de syndrome thoracique aigu (STA), d’HTA ou de diabète, le traitement par hydroxyurée ou immunosuppresseurs étaient identiques entre patients infectés hospitalisés et infectés ambulatoires. Leur fragilité vis-à-vis d’une infection virale n’a jamais bien été établie si ce n’est que celle-ci augmente le risque de crise vaso-occlusive (CVO) et donc d’hospitalisation. Les patients drépanocytaires ont très tôt été considérés comme personnes “fragiles” vis-à-vis de la COVID-19 mais cela reste controversé et les facteurs de gravité dans cette population sont mal connus. Nous avons voulu identifier les facteurs de risque associés à des formes sévères de COVID-19 dans une cohorte de patients drépanocytaires infectés par le SARS-Cov2 et hospitalisés. Matériels et méthodes Cohorte prospective, multicentrique et observationnelle Française incluant des patients drépanocytaires hospitalisés avec une infection SARS-CoV-2 confirmée (par test RT-PCR sur écouvillon naso-pharyngé) entre mars 2020 et mai 2021. Les données cliniques et le devenir durant l’hospitalisation ont été recueillis. Un modèle de régression logistique multivarié a été utilisé pour identifier les facteurs associés aux formes sévères de COVID-19 définies par le recours à une ventilation mécanique ou un décès intra-hospitalier. Nous avons effectué des comparaisons en fonction des génotypes de drépanocytose. Résultats 319 patients drépanocytaires, d’âge moyen 27,4 ans (de 2 mois à 85,5 ans), ont été hospitalisés concomitamment à la découverte d’une infection par le SARS-CoV-2. International audience; Myelodysplastic syndromes are a heterogeneous group of clonal myeloid disorders characterized by peripheral cytopenias and an increased risk of progression to acute myeloid leukemia. Inflammatory, auto-immune or syndromic symptoms can make the diagnosis difficult. Diagnosis is currently based on bone marrow cytology but cytogenetics and molecular features are currently overpassing their initial prognostic function (allowing early diagnosis and prediction of therapeutic response). The prognostic classification is based on the Revised International Prognostic Scoring System, which also provides guidance for therapeutic management. The treatment of low-risk myelodysplastic syndromes is based on the correction of cytopenias (erythropoiesis stimulating agents, transfusions, lenalidomide, etc.), whereas in high-risk group, the goal is the control of the leukemic clone (hypomethylating agents, allograft of hematopoietic stem cell transplantation). Other molecules are used to manage complications of cytopenias or transfusion (anti-infectious prophylaxis and treatments, martial chelation). New molecules are being studied with some interesting results (luspatercept, venetoclax). This article aims to provide an update on the knowledge that an internist should know for the practical management of myelodysplastic syndromes in 2019.

International audience; Essentials Risk factors of bleeding in adult immune thrombocytopenia are not known. This multicenter study assessed risk factors of bleeding at immune thrombocytopenia onset. Platelet count thresholds associated with bleeding were < 20 × 109 L-1 and < 10 × 109 L-1 . Exposure to anticoagulants was a major risk factor of severe bleeding.Summary: Background The aim of this cross-sectional study was to assess risk factors for bleeding in immune thrombocytopenia (ITP) adults, including the determination of platelet count thresholds. Methods We selected all newly diagnosed ITP adults included in the Cytopénies Auto-immunes Registre Midi-PyrénéEN (CARMEN) register and at the French referral center for autoimmune cytopenias. The frequencies of any bleeding, mucosal bleeding and severe bleeding (gastrointestinal, intracranial, or macroscopic hematuria) at ITP onset were assessed. Platelet count thresholds were assessed by the use of receiver operating characteristic curves. All potential risk factors were included in logistic regression models. Results Among the 302 patients, the frequencies of any, mucosal and severe bleeding were 57.9%, 30.1%, and 6.6%, respectively. The best discriminant threshold of platelet count for any bleeding was 20 × 109 L-1 . In multivariate analysis, factors associated with any bleeding were platelet count (< 10 × 109 L-1 versus ≥ 20 × 109 L-1 , odds ratio [OR] 48.2, 95% confidence interval [CI] 20.0-116.3; between 10 × 109 L-1 and 19 × 109 L-1 versus ≥ 20 × 109 L-1 , OR 5.2, 95% CI 2.3-11.6), female sex (OR 2.6, 95% CI 1.3-5.0), and exposure to non-steroidal anti-inflammatory drugs (NSAIDs) (OR 4.8, 95% CI 1.1-20.7). A low platelet count was also the main risk factor for mucosal bleeding. Exposure to anticoagulant drugs was associated with severe bleeding (OR 4.3, 95% CI 1.3-14.1). Conclusions Platelet counts of < 20 × 109 L-1 and < 10 × 109 L-1 were thresholds for major increased risks of any and mucosal bleeding. Platelet count, female sex and exposure to NSAIDs should be considered for assessment of the risk of any bleeding. Exposure to anticoagulant drugs was a major risk factor for severe bleeding.
