Background Characteristics, associated factors, and survival probability of toxoplasmic encephalitis (TE) in the era of advanced highly active antiretroviral therapy (HAART) have not been fully clarified. Methods Data for 205 individuals with acquired immunodeficiency syndrome (AIDS)-related TE were derived from the Italian Registry Investigative NeuroAIDS database, and the cases were studied longitudinally to evaluate prevalence, clinical characteristics, and survival. Moreover, the relationship between the occurrence of TE and exposure to antiretroviral therapy and to TE prophylaxis was evaluated. Results With an overall prevalence of 26%, TE represented the most frequent neurological disorder in the cohort. Female sex, severe immunodeficiency, and absence of primary TE prophylaxis significantly increased the risk of TE, and previous exposure to antiretroviral therapy reduced the probability of disease occurrence. Thirty-six percent of patients who had received antiretroviral therapy developed TE, although in most of these cases, the patient experienced failure of antiretroviral therapy. Of note, 66% of patients who had experienced antiretroviral therapy did not receive prophylaxis for TE at TE diagnosis. The 1-year probability of that infection with human immunodeficiency virus (HIV) would progress or that death would occur after TE was 40% and 23%, respectively. Cognitive symptoms, low CD4(+) cell count, not receiving HAART after TE, and initiating HAART >2 months after TE diagnosis were all significantly associated with an increased probability of progression of HIV infection. Not receiving HAART after diagnosis negatively affected survival. Conclusions TE remains a highly prevalent disorder of the central nervous system, even in the late HAART era, particularly among severely immunosuppressed patients and in absence of prophylaxis. Considering that persons with TE have a high probability of early death, prophylaxis should be maintained in immunosuppressed patients who experience failure of antiretroviral therapy, and HAART should be initiated as soon as possible after TE diagnosis.

Introduction: congenital intrahepatic portosystemic shunts are abnormal communications between hepatic and portal vessels. We present a case of antenatally diagnosed portohepatic shunt in intrauterine growth restricted (IUGr) fetus with cardiomegaly and transient fetal anemia. case report: A 37-yearold primiparous woman was referred in a late second trimester for fetal growth assessment. Ultrasound revealed IUGr fetus at 9th centile with brain spearing and forward end-diastolic flow in umbilical artery. Middle cerebral artery (McA) peak systolic velocity (PsV) Doppler was high for the gestational age, possibly representing fetal anemia. Fetal liver was enlarged with abnormal circulation consisting of tortuous dilated intrahepatic vessels originating from the umbilical vein and giving off several branches with no clear end point communication sites. In psychiatry, individual‐based registries have provided key information on risks and benefits associated with the use of psychotropic drugs but they have rarely been employed for monitoring and evaluating the everyday prescribing of psychopharmacological treatments. This article describes the cultural background that gave impetus to the idea of registering all prescriptions of psychotropic drugs dispensed by physicians working in the South Verona community mental health service, and presents the methodology employed to develop such a registry in a community psychiatric service where a psychiatric case register (PCR) has been operating since 1978. We developed a registry including every patient receiving psychotropic medications in ordinary practice. This registry is linked to the PCR in order to obtain data on social and demographic characteristics, clinical symptoms, diagnosis, use of services, and outcomes. No exclusion criteria are allowed — anyone receiving treatment is automatically included. This system, which can link drug and service‐use data with hard outcome indicators, can generate information on the proportion of subjects discontinuing treatment, switching medication because of side‐effects, recovery or inefficacy, as well as on the proportion of subjects failing to return to the physician, and the proportion of patients who improve. The innovative aspect of this approach is that this registry is developed, organized and used by physicians interested in monitoring their clinical practice and in providing patients, relatives and the public with accurate information on drug use in their specific context of care. Aim: To develop an instrument to investigate knowledge and predictive factors of needlestick and sharps injuries (NSIs) in nursing students during clinical placements. Design: Instrument development and cross-sectional study for psychometric testing. Methods: A self-administered instrument including demographic data, injury epidemiology and predictive factors of NSIs was developed between October 2018–January 2019. Content validity was assessed by a panel of experts. The instrument's factor structure and discriminant validity were explored using principal components analysis. The STROBE guidelines were followed. Results: Evidence of content validity was found (S-CVI 0.75; I-CVI 0.50–1.00). A three-factor structure was shown by exploratory factor analysis. Of the 238 participants, 39% had been injured at least once, of which 67.3% in the second year. Higher perceptions of “personal exposure” (4.06, SD 3.78) were reported by third-year students. Higher scores for “perceived benefits” of preventive behaviours (13.6, SD 1.46) were reported by second-year students.

In this paper we show a hybrid modeling approach which combines Artificial Neural Networks and a simple statistical approach in order to provide a one hour forecast of urban traffic flow rates. Experimentation has been carried out on three different classes of real streets and results show that the proposed approach outperforms the best of the methods it puts together. © 2015 Elsevier B.V.

The COVID-19 pandemic has had a severe psychosocial impact on healthcare workers (HCWs). This systematic review and meta-analysis aimed at evaluating the association between individual features and depressive symptoms reported by HCWs during the pandemic. We searched Medline, Embase, and PsycInfo up to 23 June 2020. We included cross-sectional studies testing the association between individual correlates and depressive symptoms in HCWs during the SARS-CoV-2 outbreak. Fourteen studies met inclusion criteria, involving 14,173 HCWs (3,070 with depressive symptoms). Women (OR = 1.50; 95 %CI: 1.28–1.76; I2 = 40.0 %), individuals with suspected/confirmed COVID-19 (OR = 2.10; 95 %CI: 1.64–2.69; I2 = 0 %), and those with an infected family member or friend (OR = 1.67; 95 %CI: 1.37–2.04; I2 = 0%) were more likely to report depressive features, which, instead, were less frequent among doctors (compared with nurses) (OR = 0.80; 95 %CI: 0.66–0.98; I2 = 48.2 %) and HCWs who felt adequately protected (OR = 0.48; 95 %CI: 0.32–0.72; I2 = 36.3 %). Our study provided timely evidence on the correlates of depressive symptoms among HCWs during the pandemic. Early screening is crucial to develop tailored health interventions, redesigning the response to COVID-19.

In this paper, an innovative and automated fault detection and diagnosis (FDD) approach based on high-level correlation rules in order to improve reliability, safety and efficiency of a supervised building is presented. The proposed method is based on the data fusion of different measurements, using their fuzzification and aggregation through suitable operators, in order to get dimensionless severity indicators able to diagnose faults and to identify the possible causes (ranked according their severity) generating them. Thus, a set of possible anomalies that can occur in a building and the correlation with measured physical quantities were identified. Experimentation of this FDD technique was applied to indoor lighting of a real office building. The proposed method was validated over a onemonth period with the aim of detecting anomalous consumption events, considering when and in which circumstances they occurred. Applications of Big Data to smart cities are nearly limitless. However, challenges emerged with handling data with new magnitude of dimensionality, heterogeneity, required processing timeliness, and the lack of reported development experiences on how to turn Big Data into Smart Data may hinder the adoption of Big Data technologies in delivering smarter city services. This paper reports our experience in developing a Smart Big Data-centric software platform for tracking city performance. Here we examine how to design such a platform by exploiting architectural styles and open source technology for Big data management. We describe the actual development of an instance of it, the PELL Smart City Platform, for processing and managing urban data in the domain of public lighting. In particular, we focus on the formulation and evaluation of key performance indicators related to energy consumption to derive Smart Data from Big Data in the context of public street lighting.

International audience; The carrier trapping and recombination mechanisms occurring in Ce-doped silica fibers, produced by a sol-gel technique, are investigated by combining temperature-dependent steady-state X-ray-excited luminescence, wavelength- and time-resolved scintillation measurements, and wavelength-resolved thermally stimulated luminescence, focusing especially on the temperature range from 10 to 320 K. The scintillation decay features a decay time of the order of tens of nanoseconds, characteristic of the parity- and spin-allowed 5d-4f radiative transition of Ce3+ ions. In addition, a slow and complex decay contribution in the microsecond timescale is detected. We interpret these features as due to the radiative recombination at Ce centers of carriers freed from a continuous distribution of trapping sites in the forbidden gap as well as to the occurrence of an athermal tunneling recombination process between traps and Ce3+ ions. This interpretation is reinforced by good agreement between independent evaluations of trap depths and lifetimes obtained by both the numerical analysis of scintillation time decays and thermally stimulated luminescence experiments.

Several drugs previously tested in clinical trials and approved for different indications have been repurposed for bipolar disorder. We carried out a systematic meta-review of meta-analyses of randomized placebo-controlled trials investigating repurposed drugs as adjunctive treatments for mania and bipolar depression. We performed a critical appraisal using ‘A MeaSurement Tool to Assess systematic Reviews’ Version 2 (AMSTAR 2). We synthesized results on efficacy, tolerability, and safety, assessing evidence quality according to the ‘Grading of Recommendations, Assessment, Development and Evaluations’ (GRADE) approach. Our systematic search identified nine eligible studies investigating 12 drugs, four for mania and eight for bipolar depression. The quality of reporting was heterogeneous according to AMSTAR 2. In mania, allopurinol (for symptoms reduction and remission at 4–8 weeks) and tamoxifen (for response and symptoms reduction at 4–6 weeks) showed higher efficacy than placebo, with low and very low quality of evidence, respectively. Concerning bipolar depression, modafinil/armodafinil (for response, remission, and symptoms reduction at 6–8 weeks) and pramipexole (for response and symptoms reduction at 6 weeks) were superior to placebo, despite the low quality of evidence. Results on the efficacy of celecoxib and N-acetylcysteine were of low quality and limited to certain outcomes. Overall, the lack of evidence of high and moderate quality does not allow us to draw firm conclusions on the clinical utility of repurposed drugs as adjunctive treatments for mania and bipolar depression, highlighting the need for additional research.

Although cannabis’ major psychoactive component, Δ-9-tetrahydrocannabinol (THC), has been linked to both earlier onset and poorer outcomes of psychotic disorders, Cannabidiol (CBD) seems to have different pharmacological mechanisms and potential therapeutic properties. However, no clinical study has investigated CBD for the treatment of co-occurring psychotic and cannabis use disorders so far, even though its utility seems grounded in a plausible biological basis. The aim of this work is thus to provide an overview of available clinical studies evaluating the efficacy of CBD for psychotic symptoms induced by THC, schizophrenia, and cannabis use disorders. After searching for relevant studies in PubMed, Cochrane Library, and ClinicalTrials.gov, we included 10 clinical studies. Available evidence suggests that CBD may attenuate both psychotic-like symptoms induced by THC in healthy volunteers and positive symptoms in individuals with schizophrenia. In addition, preliminary data on the efficacy of CBD for cannabis use disorders show mixed findings. Evidence from ongoing clinical studies will provide insight into the possible role of CBD for treating psychotic and cannabis use disorders.

Background: Although preventive strategies have been proposed against catheter-associated urinary tract infections (CAUTIs) in intensive care units (ICUs), more efforts are needed to control the incidence rate. Aim: To distinguish patients according to their characteristics at ICU admission, and to identify clusters of patients at higher risk for CAUTIs. Methods: A two-step cluster analysis was conducted on 9656 patients from the Italian Nosocomial Infections Surveillance in Intensive Care Units project. Findings: Three clusters of patients were identified. Type of admission, patient origin and administration of antibiotics had the greatest weight on the clustering model. Cluster 1 comprised more patients with a medical type of ICU admission who came from the community. Cluster 2 comprised patients who were more likely to come from other wards/hospitals, and to report administration of antibiotics 48 h before or after ICU admission. Cluster 3 was similar to Cluster 2 but was characterized by a lower percentage of patients with administration of antibiotics 48 h before or after ICU admission. Patients in Clusters 1 and 2 had a longer duration of urinary catheterization [median 7 days, interquartile range (IQR) 12 days for Cluster 1; median 7 days, IQR 11 days for Cluster 2] than patients in Cluster 3 (median 6 days, IQR 8 days; P

Background and objectives: Readmissions of youths hospitalized for a severe mental disorder are common events and bear a remarkable human, social, and economic burden. The current study aimed at evaluating predictors of 1-year rehospitalization in a sample of adolescents and young adults with severe mental disorders. Materials and Methods: Data for &le, 25-year-old inpatients with a severe mental disorder and consecutively admitted between 1 January 2016 and 30 June 2019 were collected. Subjects were retrospectively assessed over a follow-up period of one year after the index discharge to track readmissions&mdash, i.e., the primary outcome variable. Standard descriptive statistics were used. The association between variables and 1-year rehospitalization was estimated using the univariate Cox proportional hazards regression model. We then carried out a multivariable Cox regression model, also estimating the covariate-adjusted survivor function. Hazard ratios (HRs) with related 95% confidence intervals (95% CIs) were provided. Results: The final sample included 125 individuals. The multivariable Cox regression model estimated that co-occurring substance use disorders (HR = 2.14, 95% CI: 1.08 to 4.26, p = 0.029) and being admitted for a suicide attempt (HR = 2.49, 95% CI: 1.13 to 5.49, p = 0.024) were both significant predictors of 1-year rehospitalization. Conclusions: Our study showed that comorbid substance use disorders and being admitted for a suicide attempt were predictors of early readmission in youths with severe mental disorders. Although their generalizability is limited, our findings could contribute to improve the quality of young patients&rsquo, mental health care by identifying vulnerable subjects who may benefit from tailored interventions to prevent rehospitalizations.

Public lighting is one of higher expenditure for municipal administrations, in fact it is an energy-intensive service because of its inefficient functioning and consumption. One of the major problems is the lack of information about the actual state of public lighting systems, which would allow us to know its consumption, costs and performance, safety, compliance with standards and potential for improvement. Lunched by ENEA, PELL project has been developed as operative instrument of the Lumiere project, to improve the development of a new efficient and effective management model for public lighting to help turn urban centers into smart cities. This work describes the main goals of the project and the underlying infrastructure: big data platform, portal and related services provided to end users joining the PELL project directives.

The results of irradiation tests on Ce-doped sol-gel silica using X- and g-rays up to 10 kGy are reported, in order to investigate the radiation hardness of this material for high energy physics applications. Sol-gel silica fibers with Ce concentrations of 0.0125 mol% and 0.05 mol% are characterized by means of optical absorption and attenuation length measurements before and after irradiation. The two different techniques give comparable results, evidencing the formation of a main broad radiation-induced absorption band, peaking at about 2.2 eV, related to radiation-induced color centers. The results are compared with those obtained on bulk silica. This study reveals that an improvement of the radiation hardness of Ce-doped silica fibers can be achieved by reducing Ce content inside the fiber core, paving the way for further material development.

Background Several devices have been proposed to assess arterial stiffness in clinical daily use over the past few years, by estimating aortic pulse wave velocity (PWV) from a single measurement of brachial oscillometric blood pressure, using patented algorithms. It is uncertain if these systems are able to provide additional elements, beyond the contribution carried by age and blood pressure levels, in the definition of early vascular damage expressed by the stiffening of the arterial wall. Methods and Results The aim of our study was to compare the estimated algorithm‐based PWV values, provided by the Mobil‐O‐Graph system, with the standard noninvasive assessment of aortic PWV in patients with Marfan syndrome (ie, in subjects characterized by premature aortic stiffening and low blood pressure values). Aortic stiffness was simultaneously evaluated by carotid‐femoral PWV with a validated arterial tonometer and estimated with an arm cuff–based ambulatory blood pressure monitoring Mobil‐O‐Graph device on 103 patients with Marfan syndrome (50 men; mean± SD age, 38±15 years). Aortic PWV, estimated by the Mobil‐O‐Graph, was significantly ( P SD, 6.1±1.3 m/s) than carotid‐femoral PWV provided by arterial tonometry (mean± SD , 8.8±3.1 m/s). The average of differences between PWV values provided by the 2 methods (±1.96×SD) was −2.7±5.7 m/s. Conclusions The Mobil‐O‐Graph provides PWV values related to an ideal subject for a given age and blood pressure, but it is not able to evaluate early vascular aging expressed by high PWV in the individual patient. This is well shown in patients with Marfan syndrome.

BACKGROUND Aortic pulse wave velocity (PWV) is an indirect index of arterial stiffness and an independent cardiovascular risk factor. Consistency of PWV assessment over time is thus an essential feature for its clinical application. However, studies providing a comparative estimate of the reproducibility of PWV across different noninvasive devices are lacking, especially in the elderly and in individuals at high cardiovascular risk. METHODS Aimed at filling this gap, short-term repeatability of PWV, estimated with 6 different devices (Complior Analyse, PulsePen-ETT, PulsePen-ET, SphygmoCor Px/Vx, BPLab, and Mobil-O-Graph), was evaluated in 102 high cardiovascular risk patients hospitalized for suspected coronary artery disease (72 males, 65 ± 13 years). PWV was measured in a single session twice, at 15-minute interval, and its reproducibility was assessed though coefficient of variation (CV), coefficient of repeatability, and intraclass correlation coefficient. RESULTS The CV of PWV, measured with any of these devices, was CONCLUSIONS Our study shows that the short-term repeatability of PWV measures is good but not homogenous across different devices and at different PWV values. These findings, obtained in patients at high cardiovascular risk, may be relevant when evaluating the prognostic importance of PWV.

The use of aliovalent sintering aids, especially TEOS, for scintillation ceramics preparation caused serious problems due to the need for charge compensation. Isovalent ions, on the other hand, do not cause such a problem and remain a new field to be explored. By using La 2 O 3 and Sc 2 O 3 isovalent sintering aids, we succeeded to produce Pr:LuAG ceramics with pure LuAG phase, good micro-structure, and characterized by high optical quality with 80% transmittance in the visible range. Luminescence and scintillation properties were also investigated and turned out to be similar to those of previously studied transparent ceramics obtained with aliovalent sintering agents. Thermoluminescence data show the presence of several trapping states experiencing a disordered surrounding. It can be concluded that sintering aids, regardless of their valence, introduce local disorder due to their different radii with respect to that of the lattice ions resulting in the appearance of a UV emission and decrease of Pr 3+ fast 5d–4f luminescence intensity. The aspects of energy transfer and trapping due to sintering aids in LuAG lattices are discussed.

OBJECTIVE: The aim of this literature review was to assess the effects of Animal-Assisted Interventions (AAI) on elderly patients with dementia or various psychiatric disorders. METHODS: We conducted a comprehensive literature search using the online PubMed network of the US National Library of Medicine & National Institutes of Health, Embase, PsycINFO, with the purpose of investigating AAI effects on cognitive functions, mood, and behaviour. RESULTS: A total of 18 articles on dementia and 5 on psychiatric disorders were included in the present review. AAI were found to have positive influences on demented patients by reducing degree of agitation and by improving degree and quality of social interaction. Few studies have assessed the effects of AAI on mood, and even fewer have assessed its consequences on cognitive functions. The results that are available indicate a positive effect on communication and coping ability, but none on cognitive performance. A substitute pet robot yielded encouraging results, but its use requires further investigation. The few studies conducted for elderly patients presenting a variety of psychiatric diagnoses produced controversial findings. CONCLUSIONS: In spite of the encouraging results of AAI, much more research examining the issue of optimal AAI duration, frequency of sessions, and suitable target group is needed.

A radioluminescent (RL) dosimeter based on cerium-doped silica optical fibre was recently developed by means of the sol-gel and powder-in-tube techniques. The detector, already tested with both soft X-rays and more energetic photon beams used in conventional radiation therapy and in stereotactic radiosurgery, showed interesting and promising properties. A feasibility study for the use of the doped fibre in proton therapy is here presented. The measurements were performed at the proton therapy facility of the Paul Scherrer Institute (PSI) using a 138 MeV scanned proton pencil beam. The preliminary results showed a good sensitivity of the RL dosimeter to proton beams and a satisfactory reproducibility of the response (better than 0.5%) following repeated irradiations under the same experimental conditions. A linear dose response of the signal was observed in the investigated range (0.1-10 Gy) and, moreover, the stem effect proved to be negligible. The characteristic of the system to enable a real-time measurement of the dose rate was exploited for time-resolving different spot scanning sequences. Finally, the detector was used for assessing the depth dose curve in water: by comparing the curve obtained with the doped fibre with the corresponding one measured with a standard ionization chamber, a quenching effect in the order of 12% in the region of the Bragg peak was observed, as consequence of the decrease of the scintillation efficiency with increasing the Linear Energy Transfer (LET) of protons. © 2009 Elsevier Ltd. The effect of sintering atmosphere on the optical and structural properties of Cerium doped sol-gel silica was investigated. Two sets of xerogels with Ce concentration from 0 to 10 mol% were prepared and densified in different conditions. The effect of a post-densification rapid thermal treatment was also considered. Optical absorption measurements evidenced that in oxidizing conditions Cerium is preferably incorporated inside the silica matrix as Ce4+ while the trivalent state is favoured by reducing conditions. Transmission electron microscopy images show rare-earth clusters formation whose nature was investigated by means of Raman, EDS and microdiffraction measurements. In order to evaluate the efficiency of the glasses as scintillating materials radio-luminescence measurements were carried out. A numerical fit of RL spectra showed the presence of two components peaking at 2.7 and 3.1 eV. Photon beams produced by a cobalt-60 Unit and a 6 MV linear accelerator equipped with stereotactic collimators were used to investigate some dosimetric properties of a radioluminescent dosimeter based on a Ce-doped optical fibre. The stability of the signal with increasing the irradiation time, enabled a reliable and real-time measurement of the dose rate. The methods, recently developed, of optical coupling, light collection and signal processing allowed to achieve a satisfactory signal to noise ratio, a good reproducibility and a linear dose-response. The punctual dimension of the doped fibre diameter was exploited in the characterization of stereotactic fields, providing results in agreement with those obtained using a micro-ionization chamber. © 2008 Elsevier Ltd. A set of terbium-doped silicate glasses densified up to 1050 °C was prepared by the sol-gel method. A broad range of dopant concentrations (0.001-10 mol%) was considered together with an undoped sample as a reference. The effects of a high-temperature (1600-1800 °C) post-densification rapid thermal treatment (RTT) on the luminescence properties of the samples were investigated. RTT enhances the scintillating efficiency of the samples increasing the radio-luminescence intensity up to two orders of magnitude. The relevant effects induced by RTT on radio- and photo-luminescence spectra (RL, PL) and on PL decay curves indicate the occurrence of modifications of the surroundings of Tb3 + luminescence centres. © 2007 Elsevier Ltd. The Active Demand Response (ADR), integrated with the distributed energy generation and storage systems, is the most common strategy for the optimization of energy consumption and indoor comfort in buildings, considering the energy availability and the balancing of the energy production from renewable sources. In the paper an overview of basic requirements and applications of ADR management is presented. Specifically, the model predictive control (MPC) adopted in several applications as optimal control strategy in the ADR buildings context is analysed. International audience; Point defects in the host lattice of a scintillator material can trap carriers, slowing down their migration or even preventing their transfer to luminescent centers. Such competition schemes between defects and luminescent centers may explain also the hysteresis effect, which consists of a progressive enhancement of scintillation efficiency with accumulated dose. We propose a comparison between the scintillation hysteresis effect of Yb-doped sol–gel silica glasses in bulk and fiber forms, and we correlate them with traps monitored by wavelength-resolved thermally stimulated luminescence in both materials. The results demonstrate that the fiber-drawing process is responsible for modifications of the defectiveness of the glass network, with a change of the local distribution of the traps surrounding the luminescent center. The consequence of such modifications is the removal, in the fiber samples, of the thermally stimulated luminescence peak ascribed to traps closer to Yb ions and unstable at room temperature. We highlight that suitable postdensification thermal treatments can significantly modify the concentration and spatial distribution of defects around a luminescent center and can therefore be used as a tool for the engineering of scintillating glasses.

A feasibility study for the use of a Ce3+-doped optical fibre as a radio luminescent dosimeter in radiotherapy was carried out. The prototype showed a satisfactory reproducibility (1.7%) and a good linearity over a clinically relevant dose range (from few cGy to approximately 10 Gy). Moreover, the device enabled a reliable evaluation of the absorbed dose, independently of the dose rate and of the orientation of the incoming radiation. A slight energy dependence of the response was observed when the system was irradiated with electron beams of different energies. The results indicate that the employment of this new system might be very promising both for quality control measurements and "in vivo" dosimetry. Some improvements are nevertheless still required in order to allow a real-time evaluation of the Cherenkov radiation produced by the irradiated portion of the passive optical fibre, which influences some of the dosimetric properties of the system. In the building energy efficiency field, developing automatic and accurate fault detection and diagnosis methods is necessary in order to ensure optimal operations of systems and to save energy. In this paper first, fault detection analysis based on statistical methods where anomalies are detected through a comparison with neighborhood and averaged fault-free values and through a clustering technique is performed. Following the fault detection step, a fault diagnosis analysis based on fuzzy sets and fuzzy logic is implemented. Experimentation is carried out over a one day monitoring data set in December 2013 for the fan coil electric consumption of an actual office building located at ENEA ‘Casaccia’ Research Centre. Results show the effectiveness of proposed approaches in automatic detection and diagnosis of abnormal building fan coil electric consumption.

Objective Anxiety among community-dwelling older adults has not been studied sufficiently. The aims of this cross-sectional population-based study were to estimate the point prevalence of clinically relevant anxiety symptoms and to describe their socio-demographic and clinical features, with particular focus on the association with somatic illnesses. Methods Three-hundred-sixty-six non-demented older adults (mean age 83.7±6.2, range 74–99 years) from the Faenza Project (Northern Italy) were assessed using the Cambridge Mental Disorders of the Elderly Examination-Revised (CAMDEX-R) and the Geriatric Anxiety Inventory short form (GAI-sf). Multi-adjusted regression analyses were used to estimate Odds Ratio (OR) and 95% Confidence Intervals (95% CI). Results Clinically relevant anxiety symptoms occurred in one out of five participants (point prevalence 21.0%) and were significantly associated with depression (OR 5.6 per rank; 95% CI: 3.1–10.1), physical morbidity (OR 3.5 per illness; 95% CI: 1.0–11.9) and female gender (OR 2.8; 95% CI: 1.4–5.5). Further, there were significant associations with a consumption of alcohol exceeding 1 alcoholic unit/day. Conclusions Anxiety symptoms are very common in older subjects, especially when medically ill. Depression and alcohol consumption often co-occur with late-life anxiety symptoms, thus requiring special attention in daily clinical practice.

International audience; For the next generation of calorimeters, designed to improve the energy resolution of hadrons and jets measurements, there is a need for highly granular detectors requiring peculiar geometries. Heavy inorganic scintillators allow compact homogeneous calorimeter designs with excellent energy resolution and dual-readout abilities. These scintillators are however not usually suited for geometries with a high aspect ratio because of the important losses observed during the light propagation. Elongated single crystals (fibers) of Lutetium Aluminium garnet (LuAG, Lu3Al5O12) were successfully grown with the micropulling-down technique. We present here the results obtained with the recent fiber production and we discuss how the light propagation could be enhanced to reach attenuation lengths in the fibers better than 0.5 m.

The depth-dose-profile and the transversal beam profile of a 6 MV photon beam are measured by means of Eu-doped silica optical fibres. In both cases, a non-negligible influence of the stem effect was observed as consequence of the change in the relative contribution of the Cerenkov light while varying the vertical and lateral position of the fibre. The approach for removing the spurious luminescence signal based on the radioluminescence (RL) spectral analysis proved to be effective and independent of the irradiation configurations.

In this study, the spectral emission of Eu3+-doped silica optical fibres is investigated under irradiation with photons and electrons of different energies, field sizes and orientations, in order to discover the origin of stem effect and evaluate its influence on the radioluminescence (RL) spectral shape under experimental conditions. A comparison with previously investigated Ce-doped silica fibres is also made. Below the Cerenkov radiation energy threshold, a slight contribution to the total RL signal is observed. This is due to fluorescence effects occurring in the passive fibre. Above the energy threshold, the stem effect is mainly caused by Cerenkov radiation. A method for removing the stem effect, based on RL spectral analysis, is proposed and validated by measuring the output factors of extended photon fields and by studying the angular dependence of the dosimeter.

In this work, we propose a new street lighting energy management system in order to reduce energy consumption. The key idea we want to accomplish is that of "energy on demand" meaning that energy, in this case light, is provided only when needed. In order to achieve this goal, it is critical to have a reliable demand model, which in the case of street lighting turns out to be a traffic flow rate forecasting model. In order to achieve this goal, several methods on the 1-h prediction have been compared and the one providing the best results is based on artificial neural networks. Moreover, several control strategies have been tested and the one which gave the best energy savings is the adaptive one we carried out. Experimentation has been carried out on real data and the study shows that with the proposed approach, it is possible to save up to 50 % of energy compared to no regulation systems. © 2013 Springer Science+Business Media Dordrecht.

We investigate scintillation properties and energy trapping processes in Pr-doped Lu 3Al 5O 12(Pr:LuAG) optical ceramics, specifically focusing on the effects of sintering aids. Ceramics with good transmittance and scintillation properties comparable with those of single crystals were obtained without the use of sintering aids. Their introduction caused a transparency improvement, accompanied, however, by a deterioration of scintillation properties in terms of light yield and time decay. We interpret the results, discussing the role of aliovalent sintering agents on the formation of point defects acting as traps affecting the transport of charge carriers to the luminescence centers.

We study defect states in Pr3+-doped Lu2Si2O7 crystal by wavelength resolved thermally stimulated luminescence both below and above room temperature. We identify the TSL peaks, analyze them by the initial rise technique and calculate the characteristic parameters of the corresponding traps. The role of tunnelling of charge carriers between traps and recombination centers up to RT is observed and discussed. The TSL spectra show that the charge carrier recombination predominantly occurs at Pr3+ centers. We also provide some comparison with the LPS:Ce3+ crystal prepared by the same method. OBJECTIVE: The objective of this study was to assess whether a minimally invasive protocol can be effective in the long-term control of necrotic areas and pain in patients suffering osteonecrosis of the jaw associated with the use of bisphosphonate drugs (BRONJ). STUDY DESIGN: Thirty-four consecutive patients (14 male, 20 female) with BRONJ under treatment with zoledronate, pamidronate, or alendronate were enrolled. All of the patients received professional oral hygiene treatment and antiseptic oral rinses, and if in pain they assumed an antibiotic therapy with amoxicillin/clavulanate potassium and metronidazole for ten days. At the baseline visit, as well as at each 3-month recall, the size of the osteonecrotic lesions were measured and the pain level assessed with a visual analog scale. RESULTS: The results from the general linear model showed a statistically significant (F = 16.1; P < .01; r(2) = 0.95) time-related decrease in the size of exposed bone areas during the nonsurgical therapy (from 12.5 ± 12.0 mm to 8.8 ± 10.3 mm). CONCLUSIONS: This conservative protocol seems to provide successful treatment in the vast majority of patients.

Effect of Ce content on the γ-ray response of LiYF4 single crystals with four different Ce concentrations is reported. The LiYF 4 crystals were grown by the micro-pulling down method (μ-PD) with Ce content ranged from 0.1 to 10 mol%. The emission spectra of all samples under α-ray (241Am) excitation are due to the Ce3 5d4f transition and are characterized by a band at about 320 nm. The pulse height spectra under γ-quanta (137Cs) were successfully observed and light yields were evaluated to be about 2800±280 (2 mol%), 3000±300 (5 mol%), and 3700±370 (10 mol%) phs/MeV, respectively. The energy resolution decreased with increasing of Ce concentration as follows: 12±3 (2 mol%), 19±3 (5 mol%), and 30±22 (10 mol%) %, respectively. The α/γ ratio were evaluated to be about 0.18 (2 mol%), 0.175 (5 mol%), and 0.15 (10 mol%), respectively. The non-proportionality curve of specimen with Ce 5 mol% doping exhibited good proportionality. Furthermore, the decay time examined with pulsed X-ray irradiation, detected using a streak camera, decreased with increasing of Ce concentration. © 2010 Elsevier B.V. All rights reserved.

Various fundamental characteristics of Y2 O3 ceramics, such as optical transmittance, reflectivity, absorption coefficient, refractive index, and dielectric constant, were investigated. Furthermore, in order to evaluate the possible application of this material as γ -ray scintillator, other radiation responses, like the α -ray excited emission spectrum, the γ -ray excited pulse height spectrum, the light yield, the nonproportionality, the energy resolution, and the γ -ray excited scintillation decay curves were obtained. The light yield of Y2 O3 ceramics was evaluated in about 1.13 times higher than that of Bi4Ge3 O12 (BGO):Ce single crystals and was estimated in 9300 photon/MeV. The nonproportionality measurements showed a good proportionality. The scintillation decay curve could be fitted by an exponential function characterized by a fast component of about 34 ns. © 2010 American Institute of Physics.

The radioluminescence (RL) properties of Ce-doped silica glasses prepared by sol-gel method were investigated as a function of the dopant amount in a wide range of concentrations (Ce:Si molar ratios from 1 x 10(-5) to 5 x 10(-2)). The effects of xerogel densification temperature and of a postdensification thermal treatment were also considered. In order to understand the microstructural features governing Ce3 luminescence efficiency, optical absorption measurements in the ultraviolet, visible, and infrared regions and time-resolved photoluminescence experiments were performed. The complex dependence of RL intensity upon rare-earth (RE) concentration and thermal treatment was attributed to the role of OH vibrations as nonradiative recombination channels, as well as to the formation of RE aggregates in the silica matrix. Specifically, the segregation of CeO2 nanoparticles, in which Ce4 does not supply any radiative emission, has been revealed by infrared absorption measurements, in agreement with previous Raman, transmission electron microscopy, and X-ray diffraction data. Moreover, an optical absorption band, centered at 2.4 eV and whose intensity increases with the square of cerium concentration, was observed and tentatively assigned to an intervalence electron-transfer transition involving Ce3+Ce4 dimers. Postdensification thermal treatments markedly reduce the intensity of this band and increase RL intensity. The relationship between the 2.4 eV band and the RL properties will be outlined. In particular, the increase in RL intensity will be discussed, mainly as a consequence of microscopic modifications leading to an improvement of the charge-transfer efficiency toward emitting centers.

Background. It is not well defined whether concentration-controlled intervention (CCI) and rules-based human immunodeficiency virus (HIV) type 1 genotype drug-resistance interpretation (GI) or virtual phenotype drug-resistance interpretation (VPI) may improve the outcome of HIV salvage therapy. Methods. In a prospective, randomized, controlled trial, patients were randomized (on a factorial basis) to change treatment after either GI or VPI, and they then were further randomized to the control arm (no CCI) or the CCI arm. Protease inhibitor (PI) and nonnucleoside reverse-transcriptase inhibitor (NNRTI) trough concentration (C trough ) values were determined at weeks 1, 4, 12, and 24 of the study. Results. A radioluminescent (RL) dosimetric system, based on a SiO2 optical fiber with the core doped by Ce3+ ions as luminescent activators has been investigated. Structural and optical properties of the luminescent fiber have been studied by Raman, refractive index, RL and scintillation time decay measurements, and compared to those obtained on bulk material. The RL response of a composite fiber made of a short portion of active Ce-doped fiber coupled to a long commercial one has been investigated by x-ray irradiation. A linear RL intensity response has been found in the dose rate interval 6x10(-3)-40 mGy/s together with a good radiation hardness, suggesting possible application in low-dose monitoring. Background Treatment strategies for patients in whom HIV replication is not suppressed after exposure to several drug classes remain unclear. We aimed to assess the inter-relations between viral load, CD4-cell count, and clinical outcome in patients who had experienced three-class virological failure.Methods We undertook collaborative joint analysis of 13 HIV cohorts from Europe, North America, and Australia, involving patients who had had three-class virological failure (viral load >1000 copies per mL for >4 months). Regression analyses were used to quantify the associations between CD4-cell-count slope, HIV-1 RNA concentration, treatment information, and demographic characteristics. Predictors of death were analysed by Cox's proportional-hazards models.Findings 2488 patients were included. 2118 (85%) had started antiretroviral therapy with single or dual therapy. During 5015 person-years of follow-up, 276 patients died (mortality rate 5.5 per 100 person-years; 3-year mortality risk 15.3% (95% Cl 13.5-17.3). Risk of death was strongly influenced by the latest CD4-cell count with a relative hazard of 15.8 (95% CI 9.28-27.0) for counts below 50 cells per muL versus above 200 cells per muL. The latest viral load did not independently predict death. For any given viral load, patients on treatment had more favourable CD4-cell-count slopes than those off treatment. For patients on treatment and with stable viral load, CD4-cell counts tended to be increasing at times when the current viral load was below 10 000 copies per mL or 1.5 log(10) copies per mL below off-treatment values.Interpretation In patients for whom viral-load suppression to below the level of detection is not possible, achievement and maintenance of a CD4-cell count above 200 per muL becomes the primary aim. Treatment regimens that maintain the viral load below 10 000 copies per mL or at least provide 1.5 log(10) copies per mL suppression below the off-treatment value do not seem to be associated with appreciable CD4-cell-count decline.

Human herpesvirus oesophagitis in human immunodeficiency virus positive patients is caused by cytomegalovirus and herpes simplex virus; no cases of oesophagitis and oesophagobrochial fistula as a result of varicella zoster virus (VZV) have been reported to date. This report describes the case of a patient with a 2–3 mm deep oesophageal ulcer whose viral culture was positive for VZV. The patient was treated with acyclovir with resolution of the symptomatology. After the end of the induction treatment, because of the onset of fever and fits of coughing during eating, the patient underwent oesophagography, which showed an ulcer with an oesophagobronchial fistula in the middle and lower third of the oesophagus. This case report stresses the role of VZV infection as a possible cause of oesophagobronchial fistula, a rare but benign condition in patients with AIDS.

Single crystals of CsI, CsI:Tl, and CsI:Tl, Bi were grown by the vertical Bridgman method. We studied the effect of Bi3+ codoping on the CsI:Tl X-ray-induced afterglow. Thermally stimulated luminescence measurements were carried out in order to investigate the role of traps. The results showed a remarkable reduction of afterglow, of about an order of magnitude, and of trap concentration by Bi codoping. These improvements could be related to a possible role of Bi in the compensation of intrinsic nonstoichiometry defects. (c) 2012 The Japan Society of Applied Physics

The optical emission from the 5d – 4f allowed transition of Pr3+ ions embedded in sol-gel silica is investigated for High Energy Physics applications requiring fast scintillating materials. A complete and detailed characterization of the optical, scintillation and radiation hardness properties of Pr-doped silica is carried out employing different experimental techniques including steady-state and time-resolved photo-luminescence, radio- and thermo-luminescence, scintillation and optical absorption. Optical absorption measurements, performed after X-ray irradiation sequences up to 1 kGy, evidence the formation of radiation-induced absorption bands related to point defects acting as color centers. Spontaneous partial recovery of the radiation-induced defects at room temperature, as well as after thermal treatments, is also disclosed. Particular attention is paid to the comparison between bulk silica, both before and after a melting process, and fibers. The results reveal the presence of a lower concentration of optically active defects in melted glass. Such comparison highlights a role of the fiber drawing in modifying the glass defectiveness, consisting in the occurrence of a structural reorganization of the amorphous network during the process.
