Despite firms' extensive usage of loyalty programs (LPs) and decades-long academic research on their effectiveness, LPs' effects on customer loyalty are still heavily debated. We perform a comprehensive meta-analysis of loyalty programs across various LP designs and industries and spanning different performance metrics to identify moderators of LP effectiveness. Based on a data set with 429 effect sizes, published or available between 1990 and 2020, we find strong evidence that LPs enhance customer loyalty. However, while LPs particularly enhance behavioral loyalty, shifting consumers' attitudinal loyalty is more challenging. Further, LP effectiveness differs systematically depending on LP design characteristics (LP structure, reward content and delivery) and industry characteristics. These effects are enabled by both cognitive and affective drivers, acting sequentially, as underlying mechanisms. Despite a wide range of methodologies investigating LPs' effectiveness, methodological choices have little impact on the substantive results. The circular economy, which minimises the level of environmental pollution, creates a promising background for sustainable development. The application of bioeconomics principles provides the use of by-products of agriculture, forestry, food waste, and wastewater as raw materials for electricity and heat. The objective is to analyse the prerequisites for bioenergy development as one of the circular economy areas. FGLS estimations are performed over the period of 2007–2018. The leading position in terms of bioenergy sphere growth is occupied by the European Union, followed by the countries of Asia and North America. The division of biomass into “traditional” and “modern” is discussed, where traditional biomass is mostly used for cooking and space heating in developing countries, while the EU is focusing on the development of second-generation (2G) biofuels when the biofuel is produced from non-food raw materials. It is estimated that the annual production of energy from renewable sources is about 225 thousand tons of o. e. in Ukraine. It is found that the legal field and the strategic directions of the bioeconomy in the European Union and Ukraine coincide, but the pace of development of bioenergy in Ukraine can be described as very slow. To increase the bioenergy potential, it is recommended to apply green tariffs, renewable energy certificates, and subsidies to energy producers.

The article describes the specific details of local communities functioning in Ukraine and the Czech Republic. It has been examined that Ukraine and the Czech Republic have similar, but not identical systems of local governance. We conducted a comparative analysis of the financial state of local communities in both countries by five indicators. Indicator 1 (total income per capita) characterises the community’s financial potential and reveals that Ukraine’s local communities have fewer financial resources to use. Indicator 2 (total expenditures per capita) describes the ability to provide residents with the resources generated in their community and Czech communities have a higher value of this indicator. Indicator 3 (share of the administrative expenditures) shows the effectiveness of money spent, and local communities in both Ukraine and the Czech Republic spend particularly the same part of their total expenditures on administrative needs. Indicator 4 (capital expenditures per capita) demonstrate how the money generated is spent on urgent capital investments and Ukraine’s communities have much lower capital expenditures per capita than Czech ones. Indicator 5 (the share of capital expenditures in total expenditures) reflects how local communities perceive the importance of investments in capital projects and Ukraine’s communities spend fewer financial resources for capital needs than Czech ones.

Basic research focuses on assessment the state of agricultural producers, analysing the financial and accounting system, logistics activities, ensuring competitiveness, and environmental pollution. The research results have been implemented in the different decision-making models during martial law, international logistics management, digital audit, agricultural lending, integrated marketing system, risk management and diversification of Ukrainian agricultural exports. The results of the study can be used in the developing of food security policies, programmes and strategies, agricultural production development, decision-making at the level of ministries and agencies that regulate food security management processes. The results can also be used by students and young scientists in the educational process and conducting scientific research on food security during martial law and Ukraine’s recovery economy after the victory.

Актуальність. У структурі патології органів травлення на частку виразкової хвороби припадає 1,7–16 %. Поширеність виразкової хвороби серед дитячого населення України становить 0,4–4,3 %. Пік захворюваності припадає на 9–11 років у дівчаток і 12–14 років у хлопчиків. Спадкова схильність при виразковій хворобі відмічається в 45–75 % хворих. Метою дослідження є визначення потреби реалізації стратегії зменшення захворюваності з урахуванням факторів ризику виникнення та несприятливого перебігу виразкової хвороби дванадцятипалої кишки в дітей. Матеріали та методи. Для розробки цільової програми профілактики виникнення виразкової хвороби дванадцятипалої кишки в дітей шкільного віку на первинному рівні використано методи системного аналізу, порівняння, організаційного експерименту, а також математичного та описового моделювання. Результати. Під час дослідження були вивчені основні фактори ризику виникнення виразкової хвороби дванадцятипалої кишки в дітей шкільного віку: стать, вік, група крові, наявність Helicobacter рylori, наявність супутньої патології, психоемоційні розлади, нераціональне харчування з порушенням його режиму, обтяжена спадковість, тютюнопаління, самолікування тощо. Інноваційним елементом удосконаленої первинної профілактики виразкової хвороби дванадцятипалої кишки в дітей шкільного віку насамперед стало залучення лікарів загальної практики — сімейних лікарів до своєчасного виявлення факторів ризику та несприятливого перебігу захворювання (зокрема, діагностика Helicobacter pylori). Участь лікарів загальної практики — сімейних лікарів набуває особливого значення в системі спостереження за категорією населення з факторами ризику. Саме тут проводиться стратифікація на групи динамічного спостереження за системою ризиків та їх моніторинг, тобто передбачаються розширення та удосконалення завдань профілактики, забезпечуються сучасні тенденції системного підходу щодо запобігання виникненню виразкової хвороби дванадцятипалої кишки та її негативних наслідків. Важливими на всіх рівнях є розробка, удосконалення та дотримання локальних протоколів ведення хворих на виразкову хворобу дванадцятипалої кишки в дітей шкільного віку. При цьому приділяється окрема увага комплексу заходів первинної профілактики, що включають широкий спектр медико-соціального впливу на популяційному рівні. Висновки. Концептуально запропонована модель профілактики виразкової хвороби дванадцятипалої кишки в дітей шкільного віку орієнтована насамперед на виконання лікарем загальної практики — сімейним лікарем при тісній взаємодії з лікарем-гастроентерологом первинної медичної допомоги на засадах сімейної медицини, що забезпечує всебічне, холістичне, безперервне та доступне медичне обслуговування дітей і включає пріоритет профілактичних принципів та координацію зусиль усіх зацікавлених сторін у системі охорони здоров’я.

The 2019 coronavirus disease (COVID-19) has infected people in many countries around the world. The discovery of an effective treatment for this disease, especially in severe cases, has been the subject of intensive scientific research. Therefore, the aim of this study was to evaluate the effectiveness of intravenous immunoglobulin (IVIG) administration in patients with severe COVID-19 infection. Patients with acute respiratory disease COVID-19 who received intravenous immunoglobulin in the intensive care unit were examined. All patients underwent non-invasive ventilation and all patients had concomitant pathology. Twenty patients participated in the study, 10 patients received an infusion of intravenous immunoglobulin. Patients who received IVIG were on invasive pulmonary ventilation for an average of 7 days, the condition of patients improved by days 2–3. At the time of transfer to adjacent departments (on average, day 14), the dynamics of the disease improved. Respiration rate was 23 per 1 min. SpO2 without O2 was 92 %. All patients presented with lymphopenia and increased transaminases level, the values of which normalized by days 2–3. When ultrasound examination of the lungs, there were no pulmonary consolidations in patients. Mortality in the group of patients who received IVIG was lower compared to patients who did not receive IVIG.

Purpose. The purpose of the article is to study the degree of disclosure of information about the risks of economic activity of enterprises in non-financial statements and to find ways to improve the organization and methods of analysis of such risks based on the financial statements of companies. Methodology / approach. The methodological basis of the study is a systematic approach, methods of generalization, comparison, abstraction, analysis, synthesis, induction and deduction, bibliometric analysis, cluster analysis, as well as methods of integrated economic analysis. The publications from the Scopus database, for 1988–01.04.2021, processed by VOSviewer software were the source of data for bibliographic and cluster analysis. The materials of the research on the state of disclosure of information about the risks of domestic enterprises of the food industry are based on their financial statements and management report. Results. The article analyzes the state and degree of disclosure of information about the risks of economic activity in non-financial reports of domestic food industry enterprises. It is established that the enterprises of the studied industry most often provide information about the following risks: economic, currency, financial, political, legal, judicial, interest, personnel, price, commercial, as well as liquidity risk and market risk. Originality / scientific novelty. Theoretical, methodological and organizational principles of risk analysis of companies based on non-financial and financial reports have been further developed. For the first time, we proposed our own approach to the methodology of analysis of such risks, based on financial reporting indicators. In particular, such analytical indicators include: financial risk – solvency, financial leverage; credit risk – investment coverage ratio, return on equity, return on assets; liquidity risk – coverage ratio, quick liquidity ratio, absolute liquidity ratio. Practical value / implications. The practical value of the research is that the results obtained by the authors will contribute to the disclosure of information about the risks of economic activity of enterprises in non-financial statements. Analysis of the status and level of disclosure of information about the risks of domestic food industry enterprises in non-financial reports showed that most companies provided information about the following risks: economic, currency, financial, political, legal, judicial, interest, personnel, price, commercial, and risk liquidity and the risk of changes in market conditions. As a result, the author's methodological approach to the analysis of internal risks of the studied industry (credit, financial risks, liquidity risk) is proposed. The proposed methodology is based on the indicators of financial reporting and is part of a comprehensive risk analysis of the enterprise for the purposes of the risk management system.

Objective: To assess faculty and staff beliefs about returning to campus as well as their mental health and healthy lifestyle behaviors. Participants: Faculty and staff at a large Mid-west public land-grant university. Methods: A survey was emailed to a random sample of faculty and staff (N = 6,000 faculty; N = 6,000 staff). Results: Response rate was 35.6% for faculty (n = 2,138) and 37.8% (n = 2,226) for staff. Participants who were working from home (>60%) had less confidence about returning to campus safely than those who had been working on campus. Eighteen to 27% of faculty and 25%-31% of staff met the cutoff for clinical anxiety and the rates of depression were 4.4%-8.3% and 9.7%-10.0% respectively. Conclusion: Institutions of higher education must build wellness cultures and accelerate access to mental health services, which should be evidence-based and include a focus on promoting and maintaining overall wellness.

Background Bone marrow consists of connective tissue and stem cells, which generate blood cells. This includes erythropoiesis, leukopoiesis and thrombopoiesis. Thus, hematologic disorders first affect the bone marrow and secondarily the blood. Methods Bone marrow changes can be sensitively detected using magnetic resonance imaging (MRI) and often represent the initial manifestation of the underlying disease. With longer duration of disease, changes can also be found on X‑ray or computed tomography (CT). Results The findings on MRI and X‑ray/CT are often nonspecific and can only be interpreted in the context of clinical information. Conclusion In the following article, we provide a brief overview of the clinical manifestations and imaging changes to be expected in leukemia, anemia, and chronic myeloproliferative disorders.

The proposed algorithm is based on the theory of electron drift in the field of a focusing magnetic lens and is designed using the methods of discrete mathematics and the minimax analysis. The obtained simulation results made it possible to establish the influence of the magnetic lens current on the focal diameter of a hollow conical electron beam and on its focal ring thickness. It is shown that the change in the focal parameters of a hollow conical electron beam can be effectively provided through the regulation of the magnetic lens current.

Context.— With multiple therapeutic options available for patients with advanced non–small cell lung cancer, the timely ordering and return of results to determine therapy are of critical importance. Objective.— To assess factors impacting anaplastic lymphoma kinase (ALK) test ordering and time to result delivery. Design.— A retrospective study using a de-identified electronic health record database was performed. Postdiagnosis ALK tests (n = 14 657) were analyzed from 14 197 patients with advanced non–small cell lung cancer diagnosed between January 2015 and May 2019. Time from non–small cell lung cancer diagnosis to ALK sample receipt in the laboratory was a surrogate for test order time. Test ordering was considered delayed if order time was more than 20 days. Turnaround time from sample received to test result was calculated and considered delayed if more than 10 days. Multivariable logistic regression was used to assess factors associated with order time and turnaround time delays. Results.— Median ALK test order time was 15 days, and 36.4% (5342) of all 14 657 orders were delayed. Factors associated with delays were non–fluorescence in situ hybridization testing, send-out laboratories, testing prior to 2018, nonadenocarcinoma histology, and smoking history. Median turnaround time was 9 days, and 40.3% (5906) of all 14 657 test results were delayed. Non–fluorescence in situ hybridization testing, tissue sample, and orders combining ALK with other biomarkers were associated with delayed ALK result reporting. Conclusions.— This study provides a snapshot of real-world ALK test ordering and reporting time in US community practices. Multiple factors impacted both test ordering time and return of results, revealing opportunities for improvement. It is imperative that patients eligible for targeted therapy be identified in a timely fashion.

Modern telecommunications companies operate in conditions of risk and uncertainty due to both external and internal factors. Due to the influence of various factors, companies may find themselves in a state of crisis due to the lack of a management mechanism, the main purpose of which is to ensure economic stability and efficiency based on sustainable capacity building. The purpose of this article is to identify the factors that affect the economic stability of telecommunications enterprises in Ukraine, and study their impact on the integrated indicator in order to form an organizational and economic mechanism for managing economic stability. The study was carried out using the following methods: analysis, synthesis, generalization, systematization, correlation and regression analysis. The main tendencies of Ukrainian telecommunication networks development are defined in this research. It is established that the crisis conditions in the world and national economy have led to the development of organizational and economic mechanism for managing the economic stability of telecommunications enterprises. The key structural element of this mechanism is proposed to determine the assessment of economic stability based on the use of a set of balanced indicators. Based on correlation and regression analysis, a significant dependence of the overall integrated indicator of economic stability on the subscriber component, moderate - on the component of internal business processes and staff training and development and a weak inverse dependence on the financial component. It was proved an expediency of using multicollinearity to check the tightness of the relationship between the analyzed variables in the regression model.

Background The interest in and demand for healthcare innovation has heightened amid the COVID-19 pandemic. Organizations are challenged to balance the goals of daily operations with innovation to stay relevant and compete in the marketplace. Innovation is critical for not only the success and sustainability of organizations, but the well-being of the faculty, staff, and clients they serve. Purpose In this article, we present an overview of several Nursing Innovation Centers in the United States as well as examples of colleges without formal innovation centers but who are addressing innovation in their programs. Methods We examined the subjective experience of nursing innovation in seven colleges of nursing using semi-structured intervieweds and thematic analysis. FIndings We discuss four themes for creating an innovation center or innovation focus and six themes important for sustainability and impact. In addition, we provide a working model for these themes and provide lessons learned along with trends and recommendations for the future. Discussion This information provides guidance and a framework for academic and practice organizations aspiring to create opportunities for innovation to flourish in their institutions. We also encourage leadership to critically evaluate and address biases in faculty hiring, research evaluation, publication practices, educational opportunities and mentoring to overcome the diversity innovation paradox.

Summary. Slipped capital femoral epiphysis (SCFE) is a multifactorial disease caused by a number of factors, and each of them can be triggered in its occurrence. Modern research is increasingly pointing to the leading role of epigenetic structures in the origin and course of this group of diseases. In recent years, the most promising area is the study of the role of microRNAs as an epigenetic factor that plays a leading role in the pathogenesis of multifactorial diseases. Objective: to develop a methodology for genetic genealogical examination of patients with juvenile epiphysiolysis of the femoral head. Materials and Methods. The study material was 26 patients with SCFE (15 girls and 11 boys) in different periods of sexual development: prepuberty – 7 patients, induction of puberty – 12 patients, and puberty and postpuberty – 7 patients. A questionnaire developed by us was used for clinical and genealogical research of the patients. The material for the epigenetic study, namely the study of microRNA-21 expression in patients with SCFE, was venous blood. Results. The expression of microRNA-21 in the blood of patients with SCFE is different (average expression is 0.86 conventional units) from those of healthy donors (average 0.393 conventional units) of the same age (p0.05) was revealed. The article brings light to the study of the nature reserve fund (NRF) of Zakarpatska oblast as a component of the national heritage of the country. Representativeness is considered to be one of the main principles of spatial formation of the NRF object network. Therefore, to determine the current state and ensure further effective development of the region’s nature reserve fund, it is necessary to analyze its territorial and internal structural indicators. The purpose of the study is to analyze the current status of the region’s nature reserve fund with the reference to the possibility of introduction of the further new management practices. The article is based on information and statistical materials, the provisions of regional programs for the formation of the ecological network and environmental legislation of Ukraine. To solve the assigned tasks, there has been done a comprehensive analysis of statistical reporting data and materials of the NRF Register in Zakarpatska oblast of the Department of Ecology of Natural Resources of Zakarpattia Regional State Administration within the period of 2019, and reporting documents of environmental institutions for the same period. The considered indicator is the territorial distribution of nature reserve areas and region areas by administrative districts (before the formation and implementation of changes to the administrative-territorial structure of the Zakarpatska oblast), which is presented in the form of the division into four groups. The spatial distribution of the territory and the NFR objects do not sufficiently meet the criteria of local representativeness, so their spatial structure needs significant improvement, be specific – the creation of nature reserves, especially in the lowlands of the region. The average density of nature protection objects in the region (36 units / 1000 km2) is almost three times higher than the corresponding indicator in the neighboring Lviv region. The average value of the reserve factor in the region makes 14.17 %. It has been found that the reserve ratio demonstrates geographically the largest disproportion in Zakarpatska oblast. It is the highest indicator in turns of administrative entities in the districts of Mizhhiria, Velyky Bereznyi and Rakhiv. The lowest one is in Svaliava and Berehove districts, respectively. In addition, the reserve ratio compared to the national average indicator and in other European countries has been carried out. The quality of the nature reserve network is determined by the insularity coefficient, which indicates the size of the NRF objects and their stability. Further expansion of the network of nature protection objects in Zakarpatska oblast is possible due to the creation of Latorytsa, Shaian and Uzhok landscape parks. A big assumption can be made that by 2020 it will be possible to create new and expand existing areas of NRF in the region up to 100–120 thousand hectares, which will increase the protected area up to 20–22 %. The key issues of nature reserves to be developed in Zakarpatska oblast are primarily related to the imperfection of the management system. To handle this problem, there is an urgent need to develop a strategy, which will envisage the perception of the NRF as a holistic anthropogenic and natural unit in order to put into practice the environmental, scientific, educational and recreational functions.

Background Oncology nurses are at increased risk for developing burnout. Although various interventions have been researched, mindfulness has been proven to be beneficial in mitigating burnout while improving well-being. Objectives The aim was to evaluate whether the use of a mindfulness mobile application (app), Headspace®, increases perceptions of well-being and decreases perceptions of burnout among inpatient bone marrow transplantation (BMT) staff nurses and nurse practitioners (NPs). Methods This evidence-based practice quality improvement initiative introduced the Headspace app to BMT nurses and evaluated its impact on burnout and well-being at baseline and every 30 days for 90 days. Findings There were significant improvements in burnout and well-being in staff nurses and NPs from baseline to each time point. Sleep hygiene meditations were the most widely used programs within the Headspace app for both nursing groups.

Annotation. Modern therapy of psoriasis is to contain the manifestations of the disease, the choice of treatment tactics depends on the severity of the course. However, the effectiveness of standardized complex therapy remains rather low and the results of treatment are unpredictable, which requires further study of ways to increase the effectiveness of psoriasis therapy. Aim – to evaluate the effectiveness of standard psoriasis therapy. An analysis of the effectiveness of standard therapy for psoriasis was carried out in 30 patients who were under the supervision of a dermatologist at the Vinnitsa Regional Clinical Dermato-Venereal Dispensary in 2019–2020. In addition to general clinical research methods, the patients were evaluated for the skin lesion index and the severity of the psoriatic process (PASI) and the quality of life was assessed – the dermatological index of quality of life (DLQI). ВІЛ-інфекція приводить до постійного зростання кількості осіб із опортуністичними захворюваннями та летальності від них. Частота розвитку захворювань органів дихання – більше 60 %, при цьому найчастіше діагностується туберкульоз. Діагностичні можливості стосовно опортуністичних хвороб залишаються обмеженими. Окрім того, наразі більш ніж у 50 % пацієнтів має місце поєднання різних вторинних захворювань, що призводить до зростання тяжкості їх перебігу, проблем діагностики та лікування. Мета – встановити діагностичну ефективність комплексного застосування різних методів гістологічного дослідження для верифікації етіології інфекційних уражень органів грудної порожнини на тлі ВІЛ-інфекції. Матеріал і методи. Групу дослідження склав 31 випадок вторинних інфекційних уражень органів грудної порожнини у ВІЛ-інфікованих хворих. Перш за все проводили традиційне гістологічне дослідження. Додатково застосовували гістохімічні методики: Ціль – Нільсен, флуоресцентний метод, метод Гоморі – Гроккота та ШЙК-реакцію з альціановим синім, а також імуногістохімічне дослідження для виявлення мікобактеріальних антигенів з двома поліклональними антитілами до M. tuberculosis. Результати. За результатами комплексного гістологічного дослідження діагностовано 15 (48,4 %) випадків туберкульозного процесу. Зіставлення попереднього та заключного клінічних діагнозів усіх випадків нашого дослідження показало, що лише у 3 випадках (9,7 %) отримано підтвердження попереднього клінічного діагнозу, у 9 випадках (29,0 %) клінічний діагноз був уточнений та у 12 випадках (38,7 %) встановлено діагноз. Ще в 5 випадках (16,1 %) попередній клінічний діагноз виявився хибним та був змінений. І лише у 2 (6,5 %) випадках остаточний діагноз не було встановлено. Таким чином, в 25 випадках (80,6 %) встановлено розвиток грибкової інфекції та у 15 випадках (48,4 %) діагностовано туберкульоз. Загалом у 16 (51,6 %) випадках виявлено асоціацію різних вторинних інфекцій. Висновок. При проведенні морфологічної діагностики у ВІЛ-інфікованих пацієнтів із вторинними інфекційними ураженнями органів грудної порожнини доцільно застосовувати низку спеціальних гістохімічних методів дослідження уражених тканин. У 67,7 % випадків при розширеному морфологічному дослідженні початковий патогістологічний висновок був уточнений або повністю змінений. Загальна ефективність комплексного морфологічного дослідження склала 93,5 %.

The theoretical foundations of the functioning of critical infrastructure, which focuses on critical analysis of the content of the concepts of critical infrastructure and critical infrastructure security are revealed in the article. The definition of a number of concepts in the field of critical infrastructure and its safety, which should be used in the relevant legal documents, is proposed and substantiated. The key factors to consider when organizing the security of critical infrastructure are suggested and grounded. It is determined that for the proper organization of the safety of critical infrastructure, the availability of official relevant legal documents and organizational structure is crucial, within which they should be divided into groups with clear content characteristics. Since, the structure of the security system of critical infrastructure facilities in Ukraine, the scope of security powers and responsibilities of their owners and the state will depend on their content. An expert assessment of the feasibility of standardization of basic security concepts and the organization of the security system of critical infrastructure is conducted. It is found that the formation of public-private partnership in the field of critical infrastructure security is important to increase the efficiency of this system. It has been established that security activity in the field of critical infrastructure is a rather complex process, which, in addition to security mechanisms, will include coordination, as well as activities that require appropriate management. All these factors require appropriate specialists, whose training and retraining should begin in higher education institutions that have educational and safety technologies, some experience with the involvement of practitioners. It is determined that the certification and maintenance of registers should be an important and obligatory element in the organization of security of critical infrastructure facilities.

BACKGROUND. Among countries of Europe in Ukraine the incidence and mortality rate from tuberculosis (TB), including the incidence of the relapses in patients with pulmonary tuberculosis (RPT), remain high. Absence of the national program of fight from TB, lack of financing of anti-TB measures, unsatisfactory statistical information confirm the necessity of study of situation from relapses in patients with RPT and development of measures on their warning. OBJECTIVE. To estimate the features of dynamics of the incidence of RPT in the last few years and consisting of epidemic situation of TB in a country. MATERIALS AND METHODS. An analysis of official statistical reports of anti-TB establishments of administrative territories of Ukraine, information of SI “Center of Public Health of the Ministry of Health of Ukraine” and SI “Center of Medical Statistics of the Ministry of Health of Ukraine” in the last few years. RESULTS AND DISCUSSION. By the features of dynamics of the incidence of RPT in the last few years in a country are high the incidence in patients with RPT with sputum positive smears with comparison in the incidence in patients with RPT with sputum negative smears; decline of index on 46,0 %; high part of RPT with sputum positive smears among patients with TB sputum positive smears (the TB new cases + relapses); diminishing of amount of cases of TB which the diagnosis of multidrug-resistant TB is confirmed, from them growth of percent of patients with extended resistant of Mycobacterium tuberculosis. The signs of epidemic situation are: decline of the TB incidence of lights (the new cases + relapses); high percent of patients sputum positive smears; high TB incidence among children by age 15-17; diminishing of the TB mortality rate and his prevalence; absence of changes of index prevalence of TB among children by age 0-17 and 0-14. CONCLUSIONS. The analysis of statistical data testifies to the presence of failings in an exposure and treatment of patients with the relapses of TB, on the necessity of introduction of complex anti-TB measures for warning of relapses within the framework of the national program of fight from TB.

The aim of the work is to establish effective state instruments of development of entrepreneurial activity in the agrarian sector of Ukraine. For the realization of the purpose was used a number of economic-mathematical methods of analysis by panel data of EU and OECD countries for the period 1986-2020: regression modeling of relationships between the total value of agrarian production and total support, rendered to agrarian sector, types and structural volumes of such support; regression dependence statistics, methods of comparative analysis and graphics analysis. The results allowed us to establish that the change in the total production value of the agrarian sector by 96,79% is determined by the types and structural volumes of state support provided to the agrarian sector. For the development of entrepreneurial activity in the agrarian sector of Ukraine it is advisable to activate state support of knowledge and innovation system, infrastructure of agrarian sphere and optimize measures of inspection and control, significantly prevail among the tools of domestic regulation of the agrarian sphere.

For the network enterprises under research, the acquisition, organization of application of technologies, programmes, equipment which will improve economic, market, technical indicators, will provide the possibility to increase the general level of innovation has been suggested. To ensure the optimal selection, the use of innovative platforms for the selection of technologies, programmes, equipment has been proposed. Its application provides the possibility to identify the needs of innovative development of each of the enterprises under research and to develop individualized areas of innovation. The purpose of the article is to identify key problems of innovative development of Ukrainian network enterprises and to develop ways to improve its management on the basis of promising technologies. The methodology of assessing the state of innovative development of enterprises on the basis of four groups of indicators, namely the financial ones, the impact of innovative capacity on the level of international business development, staff composition, and logistics system, which has been implemented for five network structures on their primary information base, is presented. The expediency of adapting the Polish experience regarding the use of an innovative selection platform, which provides simulation modelling of the implementation of technologies and programmes, to the conditions and features of Ukrainian networks has been proved. Only network enterprises and their management system and innovative development were considered. The use of algorithms for commercialization of technologies, software that will allow the staff of research units of network structures to identify individualized needs of innovative development and develop individualized areas of innovation of the network structure, thus ensuring greater efficiency of innovation has been proposed. The results of the assessment made it possible to identify the main problems of innovation in network enterprises. In various violations of kidney function, there is a change in the hemostatic system, which can manifest itself in the form of thrombosis or bleedings. In these processes, vascular endothelium, platelets, plasma proteins of clotting and anti-clotting systems are involved. For correction of hemostatic disorders in uremic patients, the antithrombotic therapy (anticoagulants, antiplatelets agents) is used in hypercoagulation, and desmopressin, cryoprecipitate, conjugated estrogens, recombinant human erythropoietin, transfusions of red blood cells — in bleedings. The algorithm for the study of hemostatic system is recommended, in its use it is possible to prescribe adequate therapy depending on the thromboembolic or hemorrhagic complications.

Metabolic syndrome (MS) is a combination of a disorder of carbohydrate metabolism, abdominal obesity, dyslipidemia and arterial hypertension. Studies show that there is a close relationship between MS and chronic kidney disease (CKD). Patients with MS have a 2.5-fold higher risk of CKD. Factors of CKD development in the metabolic syndrome are an increased level of insulin and insulin resistance, active forms of oxygen, inflammatory mediators, biologically active substances, hormones, inflammatory cytokines, an increase in the activity of coagulation factors, inhibition of the fibrinolytic system. Pathological abnormalities in the kidneys in patients with MS are manifested by microvascular tubular atrophy, interstitial fibrosis and global or segmental sclerosis. Microalbumin is an early marker for MS.

Ревматические заболевания (РЗ) являются одними из самых распространенных хронических заболеваний опорно-двигательного аппарата и соединительной ткани, которые встречаются как у взрослых, так и у детей. Причиной этих патологических состояний могут быть изменения в функционировании иммунной системы, воспаление, инфекции и другие факторы. В некоторых случаях они приводят к инвалидности и оказывают значительное влияние на качество и продолжительность жизни. Существуют нефармакологические и фармакологические методы лечения РЗ. Первая линия (нефармакологическая) лечения большинства РЗ состоит в изменении образа жизни (физические упражнения, правильное питание, отказ от курения). К фармакологическому стандартному лечению относятся нестероидные противовоспалительные препараты (НПВП), глюкокортикоиды, противоревматические препараты, которые подавляют реакцию острой фазы, способствуют снижению уровня аутоантител. К сожалению, хотя НПВП и помогают контролировать симптомы РЗ, уменьшая боль, отек и воспаление, тем не менее они не замедляют прогрессирование заболевания. Это относится и к глюкокортикоидам, которые являются противовоспалительными гормонами, связанными с кортизолом — стероидом, вырабатываемым естественным путем в организме. Несмотря на свои преимущества, глюкокортикоиды дают значительные побочные эффекты, включающие диабет, остеопороз, гипертонию, катаракту и подверженность инфекциям. За последние годы получены новые данные о патогенезе ревматических заболеваний, что способствовало возможности создания инновационных иммунобиологических препаратов. Разработка таких лекарственных средств произвела революцию в терапевтическом подходе при хронических ревматических заболеваниях, особенно у пациентов, устойчивых к стандартному лечению. Механизм действия этих иммунобиологических препаратов основан на целенаправленном ингибировании специфических или молекулярных мишеней, непосредственно вовлеченных в патогенез заболевания. К ним относятся блокаторы фактора некроза опухоли, антагонисты интерлейкинов-1, -6, -17А, блокаторы CD20, CD80/CD86, ингибитор Т-клеточной костимуляции. Их использование позволяет улучшить прогноз и достичь клинической ремиссии у значительного количества пациентов.

Автосомно-домінантний полікістоз нирок (АДПКП) є спадковим захворюванням, що характеризується прогресуванням зростання кісти і збільшенням загального обсягу нирок, що призводить до ниркової недостатності. Основною причиною АДПКП є мутації в генах PKD1 і PKD2, які кодують утворення білків поліцистину-1 і поліцистину-2. Є зв’язок структурних і функціональних дефектів в первинній війці з АДПКП. Найбільш перспективними лікарськими препаратами для лікування АДПКП на сьогодні є антагоністи рецептора вазопресину-2, інгібітори mTOR і цАМФ.

Терапія КРС включає призначення діуретиків, вазодилататорів, інгібіторів ангіотензинперетворюючого ферменту, антагоністів рецепторів ангіотензину II, нітратів і серцевих глікозидів. Одним з ефективних методів лікування КРС є застосування методів замісної ниркової терапії.

In many diseases of the kidneys, N-acetyl-β-D-glucosaminidase (NAG) in the urine is excreted in large quantities. A significant increase in NAG activity is observed at the primary and secondary kidney damage, heavy metal poisoning, kidney transplantation, kidney tumor, hypertension, preeclampsia in pregnant women, diabetic nephropathy, polycystic kidneys, glomerulonephritis. Commercial kits have been developed to measure the activity of NAG c using a colorimetric (biochemical) and enzyme-linked immunosorbent (ELISA) methods of research.

При фокально-сегментарному гломерулосклерозі (ФСГС) відбувається початкове пошкодження подоцитів. Генетичні дослідження людини за останні кілька років показали, що ФСГС — це насамперед подоцитопатія з більш ніж 20 мутованими генами, що беруть участь у патогенезі даного захворювання. Нефрин (ген NPHS1) разом із подоцином (ген NPHS2) є основними білками щілинної діафрагми подоцитів. Аутосомно-рецесивні мутації NPHS1 та NPHS2 пов’язані з більш тяжким станом пацієнтів, що проявляється ранньою протеїнурією і термінальною нирковою недостатністю, ніж аутосомно-домінуючі мутації INF2, TRPC6 і ACTN4. Для ініціального лікування ФСГС Kidney Disease Improving Global Outcomes 2012 р. рекомендує використовувати кортикостероїдну й імуносупресивну терапію.

Исследования в области кислородзависимой регуляции эритропоэза предоставили новые данные о патогенезе анемии, связанной с хронической болезнью почек, что привело к разработке терапевтических средств для лечения. Новым классом агентов для лечения анемии при хронической болезни почек являются ингибиторы фермента пролилгидроксилазы, которые стабилизируют гипоксией индуцируемый фактор, являющийся ключевым регулятором эритропоэза и метаболизма железа. В настоящее время клинические исследования III фазы проходят такие препараты, как Roxadustat (FG-4592), Vadadustat (AKB-6548), Daprodustat (GSK1278863) и II фазы — Molidustat (BAY 85-3934).

При різних захворюваннях нирок успішно застосовується імунобіологічна терапія на основі моноклональних антитіл із різним механізмом дії. До таких лікарських засобів належать: ритуксимаб (взаємодія з рецептором CD20 і елімінування В-клітин), екулізумаб (інгібування розщеплення С5 комплементу), буросумаб (блокування фактора росту фібробластів FGF23), адалімумаб (анти-TNF-α агент), абатацепт (блокує костимуляцію Т-клітин шляхом зв’язування з CD80), фрезолімумаб (нейтралізація TGF-β). Ці лікарські засоби застосовуються при системних захворюваннях нирок, фокально-сегментарному гломерулосклерозі, вовчаковому нефриті, мембранозній нефропатії, при трансплатації нирки.

Метаболічна терапія — це один із небагатьох способів відновлення нормальної роботи всіх життєво важливих органів і систем. Метою роботи було дослідити ефективність курсового лікування метаболічним препаратом Вазонат (мельдонію дигідрат) щодо корекції прооксидантно-антиоксидантних та трофологічних порушень у хворих із коморбідним перебігом хронічного панкреатиту (ХП) та стабільної ішемічної хвороби серця (СІХС). У дослідження було включено 90 пацієнтів із ХП у поєднанні із СІХС, яких було поділено на дві групи (залежно від програм лікування): І група (45 хворих) отримувала загальноприйняте лікування; ІІ група (45 хворих) окрім загальноприйнятого лікування додатково отримувала препарат Вазонат за такою схемою: по 5 мл в/в струминно 1 раз на день протягом 10 днів з подальшим прийомом по 1 капсулі (250 мг) 2 рази на день протягом одного місяця. Було доведено, що додавання до комплексної терапії хворих із коморбідністю ХП та СІХС препарату Вазонат більшою мірою сприяє покращенню показників трофологічного та прооксидантно-антиоксидантного статусу, ніж стандартна базисна терапія.

Актуальность. В настоящее время важное значение в патогенезе ожирения и неалкогольной жировой болезни печени (НАЖБП) придается полиморфизму rs1801282 (Pro12Ala) гена PPAR-γ2 (гамма-рецептор, активируемый пролифераторами пероксисом). Однако в украинской популяции больных с сочетанной патологией не изучены клинико-лабораторные особенности пациентов с этим полиморфизмом. Цель — исследовать клинико-лабораторные особенности больных с ожирением и НАЖБП в зависимости от rs1801282 полиморфизма гена PPAR-γ2. Материалы и методы. В исследовании приняли участие 123 пациента с 1–3 степенью ожирения и НАЖБП. Всем пациентам проведено молекулярно-генетическое исследование rs1801282 полиморфизма гена PPAR-γ2, а также антропометрические измерения, лабораторное и инструментальное обследование (в том числе ультразвуковая стеатометрия). Результаты. У больных с ожирением и НАЖБП с генотипами CG и GG rs1801282 полиморфизма гена PPAR-γ2 выявлены достоверно более высокие показатели индекса массы тела (p = 0,03), глюкозы (p = 0,02), инсулина (p < 0,0001) и С-пептида натощак (p = 0,02), АЛТ (p = 0,04), гамма-глутамилтранспептидазы (p < 0,0001), общего холестерина (p = 0,03), триглицеридов (p = 0,002), холестерина липопротеидов низкой плотности (p = 0,004), холестерина липопротеидов очень низкой плотности (p = 0,001), коэффициента атерогенности (p < 0,0001), а показатели контролируемого параметра затухания (p = 0,002), стимулированного инсулина (p < 0,0001), мочевой кислоты (p = 0,008) и холестерина липопротеидов высокой плотности (p < 0,0001) оказались достоверно ниже по сравнению с CC-генотипом. Выводы. В украинской популяции больных с сочетанием ожирения и НАЖБП у носителей аллели G rs1801282 полиморфизма гена PPAR-γ2 обнаружены достоверные метаболические различия, которые могут быть связаны с защитой от развития стеатоза и сахарного диабета 2 типа, но в то же время повышают сердечно-сосудистый риск.
