Abstract

Local contents in broadcasting institutions, both of local broadcasting stations (SPL) and network station systems (SSJ), still face a problem. An effort to encourage the existence of local content in broadcasting institutions can be done by regional legislative institution (DPRD). The implementation of regional legislative function in encouraging optimization of local content existence in broadcasting institutions is the main focus of this study. This study uses a qualitative descriptive method with an empirical juridical approach with study documentation subject of regional legislative functions in encouraging the existence of local content. The results of study show that optimizing the function of regional legislative is significant in encouraging local content existence since they have access to KPID (The Regional Indonesian Broadcasting Commission) and Local Public Broadcasting Institution (LPPL). At least, there are implementation model of DPRD functions in encouraging the optimization of local content actualization in Broadcasting Institutions, namely: Accessibility Model through KPID and LPPL & LPS Broadcast Network Models

Abstract

CARTSMART is a human following trolley capable of automatic bill generation. In today’s world where everything is automated and smart, shopping is one of the areas which still follows the traditional path. So, this paper is capable of creating a drastic change in the traditional shopping system. As we know even though shopping is simple and enjoyable what makes it troublesome is the time spent at the billing counter carrying the heavily loaded trolley. Here the human following trolley came as a solution for carrying the heavily loaded trolley whereas the automatic billing system replaces the billing at the counter concept. The Radio Frequency Identification (RFID) system which consists of an RFID reader and tags, forms the automatic billing system, whereas the pi camera along with the ultrasonic sensor contributes to the human following concept as well as the obstacle detection. When the customer places a product in the Cartsmart the Liquid Crystal Display displays the product details along with the price, apart from that data is also given to the main server. The pi camera-based system is capable of the human following mechanism. For obstacle detection purposes the Cartsmart is provided with ultrasonic sensors. At the end of the shopping, the total amount is displayed on the LCD screen. Apart from that, the status of the purchased products is changed automatically in the main server so that the retailer can renew the stock. This whole system is controlled by a Raspberry pi module. By modifying this system, it can be applied in various domains such as industry and even in libraries.

Abstract

International audience; Objectives Data are scarce regarding the potential clinical differences between non-late onset schizophrenia (NLOS, i.e., disorder occurring before 40 years of age), late-onset schizophrenia (LOS, occurring between ages 40 and 60 years) and very-late-onset schizophrenia-like psychosis (VLOSLP, occurring after 60 years of age). Furthermore, previous research compared LOS patients with non-age matched NLOS patients. In this study, we sought to examine potential clinical differences between patients of similar age with LOS and NLOS. Methods/Design This is a cross-sectional multicentre study that recruited in- and outpatients older adults (aged >= 55 years) with an ICD-10 diagnosis of schizophrenia or schizoaffective disorder with NLOS and LOS. Sociodemographic and clinical characteristics, comorbidity, psychotropic medications, quality of life, functioning, and mental health care utilization were drawn for comparison. Results Two hundred seventy-two participants (79.8%) had NLOS, 61 (17.9%) LOS, and 8 (2.3%) VLOSLP. LOS was significantly and independently associated with greater severity of emotional withdrawal and lower severity of depression (all p < 0.05). However, the magnitude of these associations was modest, with significant adjusted odds ratios ranging from 0.71 to 1.24, and there were no significant between-group differences in other characteristics. Conclusion In an age-matched multicenter sample of elderly patients with schizophrenia, older adults with LOS were largely similar to older adults with NLOS in terms of clinical characteristics. The few differences observed may be at least partially related to symptom fluctuation with time. Implications of these findings for pharmacological and nonpharmacological management is yet to be determined.

Abstract

Diabetes mellitus (DM) is a global epidemic and affects populations in both developing and developed countries, with differing health care and resource levels. Diabetic retinopathy (DR) is a major complication of DM and a leading cause of vision loss in working middle-aged adults. Vision loss from DR can be prevented with broad-level public health strategies, but these need to be tailored to a country's and population's resource setting. Designing DR screening programs, with appropriate and timely referral to facilities with trained eye care professionals, and using cost-effective treatment for vision-threatening levels of DR can prevent vision loss. The International Council of Ophthalmology Guidelines for Diabetic Eye Care 2017 summarize and offer a comprehensive guide for DR screening, referral and follow-up schedules for DR, and appropriate management of vision-threatening DR, including diabetic macular edema (DME) and proliferative DR, for countries with high- and low- or intermediate-resource settings. The guidelines include updated evidence on screening and referral criteria, the minimum requirements for a screening vision and retinal examination, follow-up care, and management of DR and DME, including laser photocoagulation and appropriate use of intravitreal anti-vascular endothelial growth factor inhibitors and, in specific situations, intravitreal corticosteroids. Recommendations for management of DR in patients during pregnancy and with concomitant cataract also are included. The guidelines offer suggestions for monitoring outcomes and indicators of success at a population level.

Abstract

Law No. 14 of 2008 on public information transparency (KIP), must implement the information transparency. One of the Government Public Councils whose accessibility of information transparency to public is Regional House of Representatives (DPRD). DPRD has two different functional structures, consisting of leadership with DPRD complementary organs and secretariat. DPRD should have strategy in the implementation of KIP and it was in the study entitled The Policy Implementation of Regional House of Representatives in Public Information Transparency. Inductive data analysis was conducted through of the data simplification processes into a more readable and interpretable form. This study employed the statutory approach and descriptive qualitative method to reveal the strategies applied by DPRD in implementing KIP. This normative legal research took data from regulation of law relating to KIP. The result of the study shows that the law of KIP provides an obligation to DPRD as an "object" or a "subject". DPRD is “Dual of Public Council", leadership—complementary organs of DPRD and secretariat who are authorized to determine the status of information. The strategies implemented by DPRD consisting of strengthening the implementation of KIP, asserting the differentiation of DPRD and secretariat position, and supporting KIP with the main function of DPRD.

Abstract

In resource-poor countries, there is a reported lack of\ud quality and structured retinal laser training in residency\ud and hospital eye programs. This unmet training need has\ud been validated in current international diabetic retinopathy\ud screening centers.1 In our own experience, there is\ud a lack of specialist laser expertise that is a barrier to highquality\ud and competent laser treatment delivery for patients\ud in the developing world. Furthermore, with the introduction\ud of new pattern scan laser technology, clinicians in the\ud developing world have variable insights into new contemporary\ud laser settings. One of the critical issues for\ud a diabetic eye screening programs in the developing\ud world is the significant risk of loss of vision from inadequate\ud laser treatment despite successful eye screening\ud and detection of sight-threatening diabetic retinopathy.

To provide data on visual acuity (VA) outcomes and prognostic factors of microincision (23-gauge) vitrectomy surgery (MIVS) for retained lens fragments after complicated cataract surgery.Retrospective, interventional case series from 2012 to 2017.Precataract surgery and intraoperative (vitrectomy) parameters, postvitrectomy complications, and best-corrected visual acuities (BCVAs) were identified. Vitrectomy was performed as early as corneal clarity permitted. Univariate and multivariate logistic regression were used to characterize factors associated with achieving VA better than 20/40, or worse than 20/200 at 6 months.This study included 291 consecutive eyes (291 patients). LogMAR BCVA improved from 0.73 ± 0.70 before cataract surgery to 0.46 ± 0.63 (P.001) after vitrectomy. The previtrectomy VA was 1.43 ± 0.79. At 6 months, 183 (62.9%) and 45 patients (15.5%) achieved BCVAs better than 20/40 and worse than 20/200, respectively. Most frequent complications were de novo ocular hypertension (29 eyes, 10%) and transient cystoid macular edema (25 eyes, 8.6%). Postvitrectomy retinal detachment occurred in 9 eyes (3.1%). Final VA of 20/40 or better was independently associated only with better precataract surgery VA, age75 years, absence of preexisting diabetic macular edema (DME) or postvitrectomy persistent cystoid macular edema (P.05). Only poorer precataract surgery VA, delaying vitrectomy to later than 2 weeks, and final aphakic status were independently predictive of 20/200 or worse VA (P.05).Contemporary VA outcomes of 23-gauge vitrectomy for retained lens fragments are comparable with that of prior predominantly non-MIVS cohorts, but fall short of benchmarks for uncomplicated cataract surgery. IOL type or timing of placement do not impact final VA.

Abstract

Residents in rural communities across Canada collect potable water from aquifers. Fecal contaminants from sewage and agricultural runoffs can penetrate aquifers, posing a public health risk. Standard methods for detecting fecal contamination test for fecal indicator bacteria (FIB), but the presence of these do not identify sources of contamination. In contrast, DNA-based diagnostic tools can achieve this important objective. We employed quantitative polymerase chain reaction (qPCR) and high-throughput DNA sequencing to trace fecal contamination sources in Wainfleet, a rural Ontario township that has been under the longest active boil water advisory in Canada due to FIB contamination in groundwater wells. Using traditional methods, we identified FIBs indicating persistent fecal pollution in well waters. We used 16S rRNA sequencing to profile groundwater microbial communities and identified Campylobacteraceae as a fecal contamination DNA marker in septic tank effluents (STEs). We also identified Turicibacter and Gallicola as a potential cow and chicken fecal contamination marker, respectively. Using human specific Bacteroidales markers, we identified leaking septic tanks as the likely primary fecal contamination source in some of Wainfleet's groundwater. Overall, the results support the use of sequencing-based methods to augment traditional water quality testing methods and help end-users assess fecal contamination levels and identify point and non-point pollution sources. Abstract

The severity of psychopathology has a strong negative impact on quality of life (QoL) among older adults with schizophrenia spectrum disorder. However, because these subjects generally experience multiple psychiatric symptoms, it remains unclear whether decreased QoL in this population is due to specific symptoms (e.g., hallucinations), specific dimensions of psychopathology (e.g., negative symptoms), a general psychopathology dimension representing the shared effect across all psychiatric symptoms, or a combination of these explanations. Data were derived from the Cohort of individuals with Schizophrenia Aged 55 years or more (CSA) study, a large (N = 353) multicenter sample of older adults with schizophrenia spectrum disorder recruited from French public-sector psychiatric departments. We used structural equation modeling to examine the shared and specific effects of psychiatric symptoms on QoL, while adjusting for sociodemographic characteristics, general medical conditions, global cognitive functioning and psychotropic medications. Psychiatric symptoms and QoL were assessed face-to-face by psychiatrists using the Brief Psychiatric Rating Scale (BPRS) and the Quality of Life Scale (QLS). Abstract

Purpose To describe the characteristics and outcomes of patients presenting with rhegmatogenous retinal detachment (RRD) after ocriplasmin (OCP) injection. Methods Retrospective, multi-centre, observational case series with case note review. Results Eight patients with symptomatic vitreomacular traction (six with concomitant macular hole) were diagnosed with RRD after a median of 16 days (range 3–131 days) post-OCP injection. Presentation was within 3 weeks of the OCP injection in six of the cases. Five patients presented with symptoms post-OCP, and three were diagnosed asymptomatically on planned visits. Seven cases were phakic, one had high myopia (>8 dioptres), and two cases had lattice degeneration. Following RRD surgery, hole closure was achieved in 5/6 MH cases. The final median BCVA at 7 months was 20/80 (range 20/40–20/1200) similar to the baseline BCVA 20/80, with four patients gaining ≥1 line of vision compared to baseline but three losing ≥3 lines. Conclusions RRD is a non-negligible risk associated with intravitreal OCP, and it should be used with caution in eyes with high myopia and peripheral retinal pathology predisposing to RRD. Detailed peripheral retinal examination is recommended pre- and postoperatively at all visits. Patients should be advised to seek attention if symptoms recur after initial presentation.

Then propose and develop the computational approach to contribute to solve this problem is the reasonable and acceptable idea. Here we propose and develop a model to classify the enzymes based on their sequence alignment to compute the pair wise alignment between any two sequences namely, local and global alignment using different score matrices, BLOSUM30 and BLOSUM62 (default score matrix), through calculate the pair wise alignment between testing sequence and each sequence in training sequences. The results we have obtained were accept-able to some extent compared to previous studies that we surveyed.

Abstract

Background:The coronavirus disease 2019 (COVID-19) pandemic has impacted people with substance use disorders (SUDs) worldwide, and healthcare systems have reorganized their services in response to the pandemic.Methods:One week after the announcement of the COVID-19 as a pandemic, in a global survey, 177 addiction medicine professionals described COVID-19-related health responses in their own 77 countries in terms of SUD treatment and harm reduction services. The health responses were categorized around (1) managerial measures and systems, (2) logistics, (3) service providers, and (4) vulnerable groups.Results:Respondents from over 88% of countries reported that core medical and psychiatric care for SUDs had continued; however, only 56% of countries reported having had any business continuity plan, and 37.5% of countries reported shortages of methadone or buprenorphine supplies. Participants of 41% of countries reported partial discontinuation of harm-reduction services such as needle and syringe programs and condom distribution. Fifty-seven percent of overdose prevention interventions and 81% of outreach services were also negatively impacted.Conclusions:Participants reported that SUD treatment and harm-reduction services had been significantly impacted globally early during the COVID-19 pandemic. Based on our findings, we highlight several issues and complications resulting from the pandemic concerning people with SUDs that should be tackled more efficiently during the future waves or similar pandemics. The issues and potential strategies comprise the following: (1) helping policymakers to generate business continuity plans, (2) maintaining the use of evidence-based interventions for people with SUDs, (3) being prepared for adequate medication supplies, (4) integrating harm reduction programs with other treatment modalities, and (5) having specific considerations for vulnerable groups such as immigrants and refugees.
