A 76-year-old man developed repeated fulminant myocarditis in a short period, and immunosuppressive therapy was remarkably effective. A pathologic evaluation showed that inflammatory cells had infiltrated the myocardium. Not only invasion of inflammatory cells but also the formation of lymphoid follicle was noted. Chronic myocardial inflammation was proven, but cardiac sarcoidosis was negative according to the results of various examinations. This is the first report of recurrent autoimmune myocarditis with a lymphoid follicle in the myocardium. These findings may suggest a novel pathogenesis of myocarditis.

Objective Rectal lymphoid follicular aphthous (LFA) lesions are related to ulcerative colitis (UC) and can be initial lesions of UC. We investigated the clinical course and prognosis of rectal LFA lesions. Methods This is a retrospective analysis of the clinical records at a single center. Patients Thirteen consecutive cases with LFA lesions treated at Hiroshima University Hospital between 1998 and 2015 were evaluated. Another 49 consecutive cases with ulcerative proctitis treated in the same period were enrolled as the control group. The clinical course and prognosis of both groups were evaluated. Results The group with LFA lesions included 9 women and 4 men with a median age of 39.9 years (range, 21-70 years). A total of 11 cases progressed to typical UC at 5-51 months. Proximal extension of these typical UC lesions was observed in 7 (53.8%) cases, which was significantly higher than in the control group (10 cases, 20.8%). Three cases (5-year accumulation incidence rate, 27.3%) progressed to steroid-intractable UC, a significantly higher incidence than that of the control group (3 cases; 5-year accumulation incidence rate, 6.9%). Conclusion Rectal LFA lesions frequently progress to typical UC with proximal extension, some of which become intractable to corticosteroid treatment.

BACKGROUND Tumor infiltrating lymphocytes (TILs) are known to correlate with the prognosis of patients affected by a variety of cancer types. We evaluated TILs in patients who underwent surgery for lung squamous cell carcinoma (SCC). METHODS Specimens obtained from patients during resection of lung SCC were examined for TIL density, lymphoid follicle formation, PD-L1 expression, and the appearance of regulatory T cells (Tregs). RESULTS We enrolled 72 patients who underwent surgery for SCC (TIL grades 0, 1, and 2: 29, 18, and 25, respectively). Lymphoid follicles were observed in 13 (18.1%) patients and 8 were positive for Tregs, which were always observed in association with lymphoid follicles (P < 0.001). Multivariate analysis revealed that lymphoid follicle formation, the appearance of Tregs, pathological stage, and pleural invasion were independent prognostic factors related to overall survival, whereas TIL density and PD-L1 expression were not. CONCLUSION SCC patients with lymphoid follicle formation accompanied by Tregs show poor survival following lung resection surgery.

Lupus erythematosus panniculitis is a rare type of chronic cutaneous lupus erythematosus which is histopathologically characterized by a lobular panniculitis. We present a 43-year-old woman with an indurated painful plaque on her right flank from 3 years ago. She mentioned a previous blunt trauma to this site. Microscopic examination revealed a predominant lobular panniculitis with lymphoid follicle formation, hyaline fat necrosis, membranocystic changes and mucin deposition. She was treated with oral hydroxychloroquine with moderate improvement. An indurated subcutaneous painful plaque could be the presenting feature of lupus panniculitis and we should evaluate systemic involvement in these patients.

Cutaneous plasmacytosis is a rare disorder of uncertain etiology, described mainly in patients of Japanese descent. Clinically, it is characterized by multiple pigmented papules and plaques distributed primarily on the trunk. Histopathologically, it is marked by a dense dermal plasma cell infiltrate. Here, we describe a case of cutaneous plasmacytosis in a 55-year-old Indian male who presented with hyperpigmented plaques on the body. Histopathological examination revealed dense superficial and deep perivascular and periappendageal infiltrate composed mainly of plasma cells, lymphoid follicles with reactive germinal centres, perineural distribution of plasma cells, mast cell infiltration and increased dermal small blood vessels. Immunohistochemical analysis confirmed the polyclonal nature of the plasma cells. Laboratory investigations were within normal limits, except for the presence of polyclonal hypergammaglobulinemia without any M band. There was no evidence of autoimmune disease or any infection. There was no systemic involvement in this patient. The patient was diagnosed as cutaneous plasmacytosis and advised long-term follow-up. Peculiar histopathological finding in this case of cutaneous plasmacytosis was the presence of abundant mast cells in the dermis.

The treatment of choice and final diagnosis were by splenectomy and exploratory laparotomy followed by biopsy and histopathological examination, respectively. Nylon tie bands are devices made of polyamide 6.6, same material of surgical nylon wires, and has been used in different surgical procedures in animals. The present report case aimed to describe the use of nylon tie bands polyamide 6.6 in a 2.3-year-old Golden retriever dog diagnosed with splenic hematoma and lymphoid hyperplasia; and the clinical presentation, diagnostic evaluation, histological diagnosis of splenic hematoma and lymphoid hyperplasia. Case: A 2.3-year-old intact male Golden retriever dog was presented with history of anorexia, apathy, regurgitation and weight loss. During the physical examination was identified a mass in the left side of abdomen. The complete blood count (CBC), serum biochemistry, urinalysis, and thoracic radiographs were no alteration. Abdominal ultrasound revealed splenomegaly and heterogeneous hypoechoic structure. Splenectomy using an adjustable nylon ties polyamide 6.6 for hemostasis technique was performed during an exploratory laparotomy. The spleen presented hematoma and lymphoid hyperplasia at the histopathological examination, as well as the presence of siderocalcinotic plate in the trabecular spleen. The dog presented with no regurgitation, apathy or anorexia after 10 days of surgery. CBC, serum biochemistry analysis, thoracic radiographs and abdominal ultrasound revealed no abnormalities. Video laparoscopy did not showed any sign of adherence on the local where used the nylon tie bands. One year and three months after the surgery, the owner was contacted by telephone, since he had changed to other city, and he reported that the dog was healthy without any clinical signs. Discussion: The present case report describes the using of nylon tie polyamide 6.6 bands in a 2.3-year-old Golden retriever dog diagnosed with splenic hematoma and lymphoid hyperplasia. According to the author’s knowledge, the present case is the first report regarding to using nylon tie bands in splenectomy, and clinical characteristics of hematoma and lymphoid hyperplasia in young dog. The adjustable nylon tie bands allowed a secure and faster ligature when compared with traditional nylon surgical suture wires in different surgical procedures in animals and humans patients, and was considered well tolerated and non-toxic. Reports concerning to complications to the use of nylon tie in veterinary surgery were present in the literature. Otherwise, these complications were related to surgical technique failure and inadequate aseptic technique; inadequate use of nylon tie bands before assigning it as a hemostatic material. Partial splenectomy is recommended for treatment of splenic hematoma and lymphoid hyperplasia in dogs whenever possible; however, the macroscopic findings concerning splenic disorders are not enough to set the distinction between benign and malignant disorders. Clinical sign associated with splenic disorders in the present case was abdominal distension due to hematoma and nodular lymphoid hyperplasia. An adjustable nylon tie polyamide 6.6 band can be used to clamp the spleen vessels, gastroepiploic artery, gastric artery and omento veins. Plasmacytoid dendritic cells (pDCs) are professional antigen-presenting cells with antiviral and tolerogenic capabilities. Viral infections and autoimmunity are proposed to be important mechanisms in the pathogenesis of chronic obstructive pulmonary disease (COPD).The study aimed to quantify blood dendritic cell antigen 2-positive pDCs in lungs of subjects with or without COPD by immunohistochemistry and flow cytometry, combined with the investigation of the influence of cigarette smoke extract (CSE) on the function of pDCs in vitro.pDCs were mainly located in lymphoid follicles, a finding compatible with their expression of lymphoid homing chemokine receptors CXCR3 and CXCR4. pDC accumulated in the lymphoid follicles and in lung digests of patients with mild to moderate COPD, compared with smokers without airflow limitation and patients with COPD Global Initiative for Chronic Obstructive Lung disease (GOLD) stage III–IV. Exposing maturing pDC of healthy subjects to CSE in vitro revealed an attenuation of the expression of co-stimulatory molecules and impaired interferon-α production. Maturing pDC from patients with COPD produced higher levels of tumour necrosis factor (TNF)-α and interleukin (IL)-8 compared to pDC from healthy subjects.CSE significantly impairs the antiviral function of pDCs. In COPD, a GOLD stage dependent accumulation of pDC in lymphoid follicles is present, combined with an enhanced production of TNF-α and IL-8 by maturing pDCs.

Diffuse nodular lymphoid hyperplasia (DNLH) of the alimentary tract is regarded as a very uncommon finding in the adult population. It is characterized by the presence of numerous visible tiny polypoid masses uniformly distributed through the involved segment, representing an exaggeration of the normal gut-associated lymphoid follicle pattern. The DNLH usually occurs in the small bowel. We report an unusual case of DNLH of the colon in an adult woman who presented with constipation. The DNLH mimics a variety of polyposis syndromes and this may cause difficulties in diagnosis. Streszczenie Rozlany guzkowy przerost limfoidalny (diffuse nodular lymphoid hyperplasia – DNLH) jest rzadką zmianą przewodu pokarmowego u doroslych. Charakteryzuje sie wystepowaniem licznych drobnych polipow rownomiernie rozproszonych w zajetym segmencie jelita, bedących pobudzonymi, powiekszonymi grudkami chlonnymi. Zwykle jest on rozpoznawany w jelicie cienkim. W pracy przedstawiono rzadko diagnozowany DNLH w jelicie grubym (obejmujący jelito od wstepnicy do zstepnicy) u kobiety cierpiącej z powodu zaparcia stolca. Moze on imitowac zespoly polipowatości jelita grubego i dlatego sprawia trudności diagnostyczne.

Objective: To investigate the clinical and morphologic spectrum of early adult–onset dermatomyositis (DM), an inflammatory disease that affects small vessels of the muscle and the skin. Methods: Histologic evaluation of frozen muscle samples was employed to visualize the cellular organization of ectopic lymphoid structures in muscle biopsies obtained from 2 patients diagnosed with DM. Clinical presentation and morphologic features, as well as treatment and follow-up, were assessed and documented. Electron microscopy was used to confirm the light microscopic diagnosis of DM. Clonality analysis of B-cell populations using PCR was performed. Results: Muscle biopsy of both patients fulfilled the morphologic European Neuromuscular Centre criteria of DM. Analyses of muscle biopsy samples revealed ectopic lymphoid follicle-like structures that showed a remarkable similarity to secondary lymphoid organs (SLOs) with distinct T- and B-cell compartmentalization. Our 2 patients exhibited an atypical and mild clinical presentation and responded favorably to therapy. Conclusions: The clinical and histopathologic features of DM can be atypical, and the presence of SLOs is not inevitably linked to an unfavorable prognosis.

To elucidate the differences in the progression of liver cirrhosis of chronic hepatitis B and C, the laparoscopic appearance of the liver surface and the histological findings were carefully assessed in 65 patients with chronic hepatitis B and 58 patients with chronic hepatitis C. There was a difference noted in the distribution of periportal reddish markings between the two types of chronic hepatitis. Localized and irregular markings were found to be common in hepatitis C. While the markings were seen to be diffusely distributed over the liver surface in hepatitis B. In addition, localized and longitudinal depressions on the liver surface were observbed more frequently in the hepatitis C patients than in those with hepatitis B. Biopsy specimens obtained from the hepatitis C patients with the above-mentioned laparoscopic findings showed more advanced histological activity. These laparoscopic changes developed in accordance with the progression of histological activity in chronic hepatitis C. Lymphoid follicles or aggregates within the portal areas were observed more frequently in the patients with chronic hepatitis C than in the chronic hepatitis B patients. Although it is not yet clear how the laparoscopic and histological differences between these two types of chronic hepatitis come about, it appears that the progression of hepatocellular necrosis in chronic hepatitis C is different from that of chronic hepatitis B.

cells with a naive phenotype decreased from baseline to Month 1 (36.7% to 2.3%), whereas the percentage of CD4 memory cells increased (62.5% to 97.4%). The percentages of both cell types then gradually returned toward baseline levels, but did not reach baseline levels by Month 12. The proportion of CD4 T cells with a regulatory phenotype increased from baseline to Month 1 (3.8% to 12.5%), and remained elevated at Month 12. A similar pattern was observed for CD8 T-cell subsets. The proportion of B cells with a mature naive phenotype decreased from 56.4% to 5.4% from baseline to Month 1, whereas the immature cell fraction increased from 4.7% to 36.8%; relative proportions then approached baseline levels by Month 6. Conclusions:Alemtuzumab-induced lymphocytedepletionwasselective andresulted inanincreasedproportionofmemoryandregulatoryTcells. A distinctive pattern of lymphocyte repopulation was observed within weeks. These effects may explain alemtuzumab's sustained benefit despite infrequent (yearly) administration. Objective: To determine the accuracy of transrectal ultrasonographic (TRUS) staging of rectal villous tumors when used to select a surgical procedure. Design: Case series. Setting: College hospital, Hyogo, Japan. Patients: From 1988 to 1995, 24 patients were treated for villous tumors after diagnosis by TRUS, and were followed up for 0.5 to 7.5 years. Main Outcome Measures: Ultrasonographic tumor stage (u-T), pathologic tumor stage (p-T), biopsy findings, and surgical procedures were studied. Results: Overall, 71% of the tumors were accurately staged by TRUS. However, 4 u-T1 and 3 u-T2 tumors were overstaged, with pathological examination showing 2 adenomas and 5 p-Tis tumors. Three of these 7 patients were overtreated surgically. The results of a biopsy predicted adenoma in 2 of the u-T1 tumors and 2 of the u-T2 tumors that were overstaged by TRUS. Pathological examination revealed that an enlarged lymphoid follicle in the mucosa had deformed the thin muscle layer of the rectal wall and abolished its continuity on TRUS imaging. The weakness of TRUS would seem to be in the accuracy of p-Tis staging. Conclusions: Transrectal ultrasonography has the advantage of predicting malignant infiltration in rectal villous tumors. The surgeon should know the limitations of TRUS diagnosis to select the most appropriate operation for each patient. Arch Surg. 1996;131:714-717

Bacterial lipopolysaccharide (LPS) exhibits an adjuvant action on antibody response to various antigens in mice. The adjuvant action induced by subcutaneous (s.c.) injection of LPS with antigen is different from that by intraperitoneal (i.p.) or intravenous (i.v.) injection (2). In the present study, we investigated histological changes at the regional s.c. tissues after injection with LPS and the tissue distribution of the LPS. It was suggested that there were two distinct mechanisms of adjuvant action which operated at the early and late stages after s.c. immunization, respectively.

Recently, some studies revealed that gastric cancer and precancer, were more frequent in individuals with H. pylori antibodies. Histological studies, found that the relationship between H. pylori infection and gastric cancer were in wide range (19% - 80%). In our department, from 1999 Jan 1st to 1999 Dec 31th No biopsy, showed severe atrophic/dysplasia, or lymphoid follicle. Age, ethnic and location were also studied, correlated to malignant and premalignant lesions.

A prospective study of arthritis was performed in 47 patients with Behçet's disease followed up over a 47-month period (mean 19.25 months, SD 14.09). These patients had a total of 80 episodes of arthritis, which were analysed for joint distribution and symmetry, in 56 of which the duration could also be determined. Attacks were oligoarticular, affecting up to 4 joints per patient, 54 (68%) being monoarticular. Knees, ankles, and wrists were the most commonly involved joints. Involvement of spinal, shoulder, hip, and sacroiliac joints was rare. The arthritis was usually not deforming and subacute; 82% (46/56) of the attacks lasted for 2 months or less and 18% (10/56) for between 3 months and 4 years. The ESR was moderately elevated during the attacks. In 32 specimens the synovial fluid was inflammatory (cell count 14.7 +/- 10.1 x 10(9)/l), but in 19 (59%) a good mucin clot formed. Synovial biopsy in 12 patients revealed superficial ulceration, paucity of plasma cells, and in 5 instances lymphoid follicle formation.

A method for examining the microvasculature of the dog spleen by angiography is described and the findings are related to morphological studies. The marginal sinus of the lymphoid follicle has been shown to be an important part of the vascular pathway in the spleen. It allows intimate mixing of blood elements and spleen cells and it is suggested that this plays an important immunological role. The control of blood flow to the lymphoid follicle is discussed but requires further elucidation.

Context: The vermiform appendix is an abdominal organ performing the immunological function. Histological characteristics of this organ show its specialized immunological function. Vermiform appendix is involved in different disease processes such as appendicitis, carcinoma and diverticulitis. Appendicitis is the most important clinical condition. An appropriate anatomical knowledge about vermiform appendix is important for surgeons, pathologists and other physicians for proper diagnosis and management of appendicitis and carcinoma. Study design: Descriptive type of study. Place and period of the study : Department of Anatomy, Sir Salimullah Medical College, Dhaka from January 2006 to June 2007. Materials: Sixty (60) human postmortem vermiform appendix, age ranging from 0 to 65 years. Samples were collected from unclaimed dead bodies within 24 hours of death from the morgue of Sir Salimullah Medical College and Dhaka Medical College, Dhaka. Two cases of retroperitoneal fibrosis are discussed. The first patient was a known hypertensive and diabetic and had been on Aldomet and Daonil for a long time. The second patient was a tuberculotic and is improving on antitubercular therapy. Retroperitoneal fibrosis due to the above-mentioned drugs and tuberculosis are relatively unknown.

One way to consider this matter is to think about managing patients with Crohn’s disease from the point of view of the initial diagnostic evaluation, then from the point of view of routine examinations during the course of the patient’s illness, and finally the special indications for doing x-ray studies during the course of the patient’s illness.

Basic reactions of the avian respiratory system to injury include catarrhal inflammation, deciliation, necrosis, metaplasia, hyperplasia, congestion, edema, fibrin, exudation, cellular infiltrations, and lymphoid follicle formation. Specific diseases provide examples for illustration of these responses. The effects of such changes on basic defense mechanisms of the respiratory system are discussed.

Various results have been obtained in intact animals receiving antilymphoid sera. Woodruff [1] and Waksman et al. [2], using antilymphoid serum to prolong graft survival observed a developed resistance after 10–14 days; this was followed by progressive reestablishment of the normal state, in spite of continuous administration of antilymphoid serum.

A review of inflammatory subcutaneous disease revealed that lymphoid follicles are observed frequently in lupus panniculitis. They are found rarely in other lymphocytic inflammatory diseases involving the panniculus, such as morphea, erythema nodosum, and erythema induratum. The lymphoid follicle may be an indication of immunoreactive and connective tissue disease, and cases should be evaluated with this in mind. ( Arch Dermatol 115:442-443, 1979)
