1. THE INFLUENCE OF CUSTOMER INSPIRATION ON SOCIAL MEDIA MARKETING TOWARD ARCHITECTURAL SERVICE PURCHASE INTENTION

Abstract: Social media has been used as a marketing tool by architecture firms to generate purchase intention. However, inspiration as a motivating internal response is needed to achieve the marketing objective. This study aims to examine the effect of utilitarian content, hedonic content, and openness as antecedents of customer inspiration, the effect of customer inspiration on purchase intention, and the mediating role of customer inspiration between antecedents and purchase intention. A total of 395 respondents were taken as samples using simple random sampling, and the data was then analyzed with SmartPLS. Results show that utilitarian content, hedonic content, and openness positively affect customer inspiration, and customer inspiration positively affects purchase intentions. Additionally, customer inspiration also mediates the effect of utilitarian content, hedonic content, and openness on purchase intentions. This study contributes to the literature on customer inspiration from a customer-firm relationship perspective. Moreover, this study can give insight for architectural firms to effectively practice social media marketing by focusing on creating customer inspiration through the content provided and targeting the right potential customers in order to generate architectural design service purchase intention.

2. Integration of Technology Enhanced Learning within Business Organizations: Which Strategy to Choose?

Abstract: The article discusses a responsible and a responsive strategic organizational approach for a smooth integration of technology enhanced learning (TEL). A response to external and internal contingencies and an involvement of different stakeholders into the development and implementation of the so-called eLearning strategies is one of the approaches which may facilitate TEL integration within different types of organizations. A survey carried out in 2014 in sought to investigate how employees perceive and accept organizational strategies and organizational approaches to TEL integration. Research results helped to identify the most sensible areas for eLearning strategies developers and to highlight responsible and responsive approaches for TEL integration. The framework parameters for TEL integration discussed and analysed in the article may serve as a scientifically approved benchmarking tool for organisations which are in the process or planning to integrate TEL.

Abstract: Smartphone market is rapidly changing and facing a highly competitive environment, with constant product introductions. It is characterized by quickly evolving technology and designs, aggressive pricing, short product life cycles, and rapid imitation. Thus, the players in smartphone industry need to invent a major breakthrough in their marketing strategy. Consider a large company like Apple. Apple loyalists are some of the most recognized product evangelists in the market, sharing their experiences with emerging technology in enthusiastic ways. Apple as the pioneer of Brand Evangelism in 1984, the company relies on customers to communicate marketing messages to other potential customers. It can be an alternative marketing tool for organizations that want to achieve their sustainable competitiveness as brand evangelists will deliver positive information, ideas, and feelings toward a specific brand to others voluntarily in order to influence consumption behaviour. This study aims to examine the phenomenon of brand evangelism and understand the dimensions involved in a consumer becoming brand evangelist. The research method of this study is based on the implementation of quantitative survey research design. The data used in this study were obtained by administering online questionnaires to 468 respondents who have used Apple iPhone for at least 6 months in Indonesia. The data analysis method used in this study is multiple regression analysis. The findings show that brand satisfaction, consumer-brand identification, brand salience, brand trust and opinion leadership have positive influence towards brand evangelism.

4. Climate Policy and Strategic Operations in a Hydro-Thermal Power System

Abstract: Decarbonisation of the Nordic power sector entails substantial variable renewable energy (VRE) adoption. While Nordic hydropower reservoirs can mitigate VRE output's intermittency, strategic hydro producers may leverage increased flexibility requirements to exert market power. Using a Nash-Cournot model, we find that even the current Nordic power system could yield modest gains from strategic reservoir operations regardless of a prohibition on 'spilling' water to increase prices. Instead, strategic hydro producers could shift generation from peak to offpeak seasons. Such temporal arbitrage becomes more attractive under a climate package with a [euro] 100/t C[O.sub.2] price and doubled VRE capacity. Since the package increases generation variability, lowers average prices, and makes fossil-fuelled plants unprofitable, strategic hydro producers face lower opportunity costs in shifting output from peak to off-peak seasons and encounter muted responses from price-taking fossil-fuelled plants. Hence, a climate package that curtails C[O.sub.2] emissions may also bolster strategic hydro producers' leverage. Abstract: Background and Objectives Public health concerns surrounding social isolation and loneliness heightened during the coronavirus disease 2019 (COVID-19) pandemic, as infection prevention measures led to increased feelings of loneliness and depression. Our objective was to evaluate the implementation of the HOW-R-U? program, during the pandemic (March 2020–December 2021). HOW-R-U? is a weekly volunteer-delivered telephone program designed to facilitate social connection and ease feelings of social isolation, loneliness, and depression in older people. Research Design and Methods This pragmatic study used the Implementation Framework for Aged Care to adapt and implement HOW-R-U? in an Australian aged and community care organization and a tertiary health service in Melbourne. The evaluation involved analysis of program data, semistructured interviews, and surveys with program recipients, volunteers, and referrers. A process evaluation was conducted alongside an assessment of outcomes including pre- and post-symptoms of isolation, depression, and loneliness. Results The implementation evaluation indicated that codesigned systems and processes effectively supported the ongoing implementation of HOW-R-U? with transition into business as usual across both organizations. Recipients reported that telephone calls had a positive impact on their lives, whereas volunteers reported enjoyment of supporting others. Several challenges were identified, namely in program reach and fidelity. Discussion and Implications HOW-R-U? was well regarded by all involved, and evaluation learnings have informed implementation into business as usual by both organizations. [ABSTRACT FROM AUTHOR]

A pesar de ser una entidad benigna, tiene un gran impacto en el ambito emocional y psicosocial de los pacientes. En la actualidad, se dispone de una amplia variedad de tratamientos tanto topicos como orales. Se presenta el caso de una paciente de 24 anos, con alopecia areata recurrente severa, sin respuesta a multiples tratamientos previamente prescritos, en quien se descarto una causa secundaria y se confirmo diagnostico histologico con biopsia. Se inicio tratamiento con tofacitinib, un inhibidor de la JAK, con una excelente respuesta clinica al mes de iniciado el tratamiento.

Abstract: Manufacturing companies are currently undergoing a digitalization transformation in which digitally enabled, new, and innovative advanced service offerings are being launched. These so-called “digital services” represent a shift in the business logic of manufacturing firms, from up-front product sales to advanced service contracts. This business model shift has profound implications for cost structures, risk management, and revenue streams, providing manufacturing companies with the key challenge of rethinking how to capture value. Using a multiple case study of 11 companies, the purpose of this article is to enhance knowledge on how to design new revenue models for digital services. Results reveal a revenue model design framework of key phases and activities that carries implications for the emerging literature on digital servitization, as well as the business model innovation literature. The findings reveal a highly customer-centric, iterative, and agile process where close collaboration with key customers during the early stages guides the framing of revenue models for digital services. Abstract: Image deletion refers to removing images from a compressed image set in cloud servers, which has always received much attention. However, in some cases images are not successfully deleted, and coding performance still remains to rise. In this paper, we propose a low-complexity and high-coding-efficiency image deletion algorithm. First, all the images are classified into to-be-deleted images, images unneeded to be processed, and images needed to be processed further divided into images needed to be only decoded and images needed to be re-encoded. Then, we also propose a depth- and subtree-constrained minimum spanning tree (DSCMST) heuristics to produce the DSCMST of images needed to be processed. Third, every image unneeded to be processed is added to the just obtained DSCMST as the child of the vertex that is still its parent in the compressed image set. Finally, after the encoding of images needed to be re-encoded, a new compressed image set is constructed, implying the completion of image deletion. Experimental results show that under various circumstances our proposed algorithm can effectively remove any images, including root vertex, internal vertices, and leaf vertices. Moreover, compared with state-of-the-art methods, the proposed algorithm achieves higher coding efficiency while having the minimum complexity.

Abstract: Introduction A burn injury is a very painful experience, with subsequent emotional problems, which have been gaining relevance to the extent that survival from burns has improved. Among the alterations of the mental sphere in this population is Acute Stress Disorder (ASD) that has been described in up to one-third of patients with major burns. Methodology A nested case-control study was carried out in a cohort of hospitalized patients in a burn referral unit, in patients over 16 years of age. A total of 135 patients, 41 cases, and 94 controls were included. All of them underwent a psychiatric interview, a standardized form was filled out on sociodemographic and clinical information, and the PID-5-BF scale was applied to evaluate associated personality elements. The diagnosis of acute stress was made with the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) criteria. Results The incidence of acute stress was 23.4%. When the clinical and sociodemographic characteristics of both the cases and the controls were compared, the risk factors were subsidized insurance, flame burn, burn in a special area, third-degree burn, high pain, stay in the Intensive Care Unit (ICU), peritraumatic amnesia and life threat. Patients also had a higher percentage of burned body surface, higher average pulse, longer hospital length of stay, higher average in the negative affect variable and in the psychoticism variable of the PID-5-BF scale. But for the multivariate analysis using logistic regression, the model that best explains the presence of acute stress only includes the variables life threat (Odds Ratio adjusted (ORa): 117.0; Confidence Interval (CI): 10.9-1258.5), severe pain (ORa: 9.9; CI: 1.8-52.8), electrical burn (ORa: 20.8; CI: 17.2-250), burn in a special area (ORa: 8.9; CI: 1, 0-76.8), third-degree burn (ORa: 10.4; CI: 0.7-166.7). Conclusion Acute stress disorder is frequent in the hospitalised burn population, and is more frequent than in other types of trauma. Associated factors with the presentation of Acute Stress Disorder are the feeling of life threat at the time of the burn, having pain classified as strong (Visual Analog Scale (VAS) 5-10), electrical burn, and burns in special areas.

Abstract: Nephrotic syndrome in patients with cancer may be related to the primary malignancy or chemotherapeutic therapy. Solid organ cancers may cause membranous glomerulonephritis manifesting with nephrotic syndrome; other less common histologic presentations include focal and segmental glomerulosclerosis and minimal change disease. In addition, chemotherapy agents can cause renal toxicity by affecting the small blood vessels, glomeruli, tubules, and interstitium. Tyrosine kinase inhibitors such as sunitinib may cause endothelial and podocyte damage leading to renal limited thrombotic microangiopathy, manifested by proteinuria and hypertension. We report a case of an elderly man with gastrointestinal stromal tumor (GIST) on treatment with sunitinib who had as a complication of a thrombotic microangiopathy manifested with nephrotic syndrome and difficult-to-control hypertension, which was controlled by stopping this drug but with a fatal outcome due to its malignant neoplasm.

Abstract: Typically, through monitoring, collecting, integrating, and analyzing the sport scores of individuals, we can know about the sport physique of the individuals in a real-time manner, so as to further guarantee a high-quality health level of people. However, in the abovementioned sport scores-based sport physique monitoring scenario, two major challenges are raised. First, the collected sport scores for sport physique monitoring are often multi-dimensional, which makes it hard to evaluate the sport physique of individuals fairly and reasonably. Moreover, the multiple dimensions of sport scores are often of different weights or significances, which raise a challenging task to objectively evaluate the sport physique of an individual. Considering these challenges, a novel multi-dimensional sport physique evaluation and ranking method, named MSPER, is proposed in this research work for better healthcare for individuals, which is based on the TOPSIS theory. Abstract: Sweet syndrome (SS), also referred as acute febrile neutrophilic dermatosis, is an inflammatory process characterised by the abrupt appearance of erythematous papules or nodules with predominant neutrophilic infiltration in the dermis. Fever and neutrophilia are common presenting features. However, extracellular manifestations, including ocular and musculoskeletal, may occur. SS is divided into three subtypes: classical (or idiopathic), malignancy associated and drug induced. Medication-induced subtype accounts for up to 26% of cases. In recent years, emerging evidence has showed that SS may also occur in neutropenic patients who underwent induction for acute myeloid leukemia (AML). The identification of FMS-like tyrosine kinase 3 (FLT3) gene mutation in approximately 30% of patients with AML has promoted the targeted therapy with FLT3-internal tandem duplication (ITD) inhibitors. Midostaurin, a recently Food and Drug Administration-approved medication for FLT3-ITD-positive AML, was reported once as cause for SS. Abstract: Over the past decades, video salient object detection has achieved rapid development. However, the effect of edge information on spatial and temporal features extraction has not been explored. The method STEG-Net is proposed to solve the problem, we use the extracted edge information to guide the extrction of spatial and temporal features simultaneously. We combine deep texture information with shallow edge information, which can not only retain the edge of the object but also enhance the global information, leading to the more accurate location of the object. At the same time, the shallow edge information and the deep texture information are complementary. As a result, edge information and salient features could advance each other while extracting features. Finally, we evaluate the 16 state-of-the-art VSOD methods on ViSal and FBMS datasets. At the same time we also evaluate the 13 state-of-the-art VSOD methods on UVSD and MCL datasets. The experiment results indicate that the proposed method outperforms the state-of-the-art approaches.

Abstract: The primary objective was to compare the intake of important micronutrients provided from enteral nutrition to critically ill patients with the Australia and New Zealand recommended dietary intakes. A secondary objective was to compare the upper levels of intake and investigate prespecified subgroups.A systematic literature review was performed.MEDLINE, EMBASE, CINAHL, and CENTRAL were used.Databases were searched for randomised controlled trials that investigated an enteral nutrition intervention as the sole source of nutrition, were published in English between January 2000 and January 8th, 2021, and provided data to calculate micronutrient intake. The primary outcome was the % recommended dietary intake. The quality of individual trials was assessed using the Cochrane Risk of Bias Tool. Outcomes are presented as either mean ± standard deviation or median [interquartile range], with a p 0.05 considered statistically significant.Thirteen trials were included (n = 1538 patients). Trials investigating hypocaloric nutrition were excluded from the primary outcome assessment (conducted in nine trials (n = 1220)). All nine trials delivered ≥104% of the recommended dietary intakes and100% of the upper level of intakes of all micronutrients. In subgroup analyses, trials with ≥80% target energy delivered a higher % of the recommended dietary intake of vitamin B12, thiamine, zinc, and vitamin C. Acute Physiology and Chronic Health Evaluation scores ≥20 delivered a higher % of the recommended dietary intake of vitamin B12 and vitamin A. Antioxidant formulas compared with standard formulas delivered a higher % recommended dietary intake of vitamin C and thiamine. In the four trials that investigated hypocaloric feeding compared with control, there was no difference in micronutrient intake. Abstract: The pharmacy support workforce (PSW) is the mid-level cadre of the global pharmacy profession, referring to pharmacy technicians, assistants and other cadres that assist in the delivery of pharmaceutical services in a variety of practice contexts. The PSW undertake technical tasks delegated under the supervision of a pharmacist or performed collaboratively. The PSW are not intended to replace pharmacists, but rather work side-by-side with the pharmacist to achieve a shared goal. However, extensive variation in the PSW exists globally, ranging from an educated, regulated, and highly effective workforce in some countries to unrecognized or non-existent in others. Vast differences in education requirements, specific roles, regulatory oversight, and need for pharmacist supervision, inhibit the development and advancement of a global PSW. As clinical care providers, pharmacists worldwide need for a competent support workforce. Without the confidence to delegate technical responsibilities to a well-trained and capable PSW, pharmacists will be unable to fully deliver advanced clinical roles. A clear vision for the role of the PSW in the expanding scope of pharmacy practice is needed. One organization working to unite global efforts in this area is the International Pharmaceutical Federation (FIP). The FIP Workforce Development Hub Pharmacy Technicians & Support Workforce Strategic Platform was established to address the pharmacy workforce shortage in low and middle-income countries. Further developments were made in 2019, with the creation of a representative global PSW advisory panel, to provide guidance towards the development of the global PSW. Provision of frameworks and strategic input to support quality in education, development of legislative frameworks, guidelines for registration and licensure, and advice on appropriate role advancement are critical to move the PSW forward. In order to produce substantial advancement of roles and recognition of the PSW and advancement of pharmacists as patient care providers, global collaborative work is needed.

Abstract: The aim of steganography detection is to identify whether the multimedia data contain hidden information. Although many detection algorithms have been presented to solve tasks with inconsistent distributions between the source and target domains, effectively exploiting transferable correlation information across domains remains challenging. As a solution, we present a novel multiperspective progressive structure adaptation (MPSA) scheme based on active progressive learning (APL) for JPEG steganography detection across domains. First, the source and target data originating from unprocessed steganalysis features are clustered together to explore the structures in different domains, where the intradomain and interdomain structures can be captured to provide adequate information for cross-domain steganography detection. Second, the structure vectors containing the global and local modalities are exploited to reduce nonlinear distribution discrepancy based on APL in the latent representation space. In this way, the signal-to-noise ratio (SNR) of a weak stego signal can be improved by selecting suitable objects and adjusting the learning sequence. Third, the structure adaptation across multiple domains is achieved by the constraints for iterative optimization to promote the discrimination and transferability of structure knowledge. In addition, a unified framework for single-source domain adaptation (SSDA) and multiple-source domain adaptation (MSDA) in mismatched steganalysis can enhance the model's capability to avoid a potential negative transfer. Extensive experiments on various benchmark cross-domain steganography detection tasks show the superiority of the proposed approach over the state-of-the-art methods.

Abstract: Background: Immunotherapy drugs, known as immune checkpoint inhibitors (ICIs), work by blocking checkpoint proteins from binding with their partner proteins. The two main pathways that are specifically targeted in clinical practice are cytotoxic T-lymphocyte antigen-4 (CTLA- 4) and programmed cell death protein 1 (PD-1) that showed potent immune-modulatory effects through their function as negative regulators of T cell activation. Methods: In view of the rapid and extensive development of this research field, we conducted a comprehensive review of the literature and updated on the use of CTLA-4, PD-1, and PD-L1 targeted therapy in the treatment of several types of cancer, including melanoma, non-small-cell lung carcinoma, breast cancer, hepatocellular carcinoma, Hodgkin lymphoma, cervical cancer, and head and neck squamous cell carcinoma. Results: Based on the last updated list released on March 2019, seven ICIs are approved by the FDA, including ipilimumab, pembrolizumab, nivolumab, atezolizumab, avelumab, durvalumab, and cemiplimab. Conclusion: This review highlighted the most common adverse effects caused by ICIs which affect people in different ways.

Abstract: The underlying mechanisms of E2F5 upregulation and its pro-tumor functions have not been elucidated in gastric cancer (GC). Here, the expression, prognostic value, mutation status, and promoter methylation of E2F5 were evaluated. The effects of E2F5 depletion on cell proliferation and invasion in GC, were also assessed through in vitro experiments. Additionally, gene set enrichment analysis (GSEA) was applied to analyze the potential downstream regulator of E2F5. The study also assessed the correlation and transcription regulation between E2F5 and UBE2T. Finally, the roles of UBE2T in E2F5-related pro-tumor functions were examined. The findings revealed that E2F5 was upregulated and showed remarkable association with pathological variables and prognosis. Hypomethylation of the E2F5 promoter predicted poor prognosis and partially caused E2F5 upregulation in GC. E2F5 knockdown significantly inhibited the proliferation and invasion of GC cells. E2F5 had a significant positive correlation with UBE2T in GC. Mechanistically, E2F5 promoted UBE2T transcription and UBE2T overexpression reversed the effects of E2F5 depletion on the proliferation and invasion of cells in GC. Taken together, this study originally confirmed the upregulation of E2F5 in GC, revealed that E2F5 can directly upregulate UBE2T transcription, and subsequently promote the malignant progression, which highlights that the E2F5/UBE2T axis can potentially be used in the diagnosis and treatment of GC.

Abstract: OBJECTIVE To determine whether men and women with type 2 diabetes have different psychosocial, behavioural, and clinical characteristics at the time of their first visit to a diabetes education centre. DESIGN A questionnaire on psychosocial and behavioural characteristics was administered at participants’ first appointments. Clinical and disease-related data were collected from their medical records. Bivariate analyses (χ2 test, t test, and Mann-Whitney test) were conducted to examine differences between men and women on the various characteristics. SETTING Two diabetes education centres in the greater Toronto area in Ontario. PARTICIPANTS A total of 275 men and women with type 2 diabetes. RESULTS Women were more likely to have a family history of diabetes, previous diabetes education, and higher expectations of the benefits of self-management. Women reported higher levels of social support from their diabetes health care team than men did, and had more depressive symptoms, higher body mass, and higher levels of high-density lipoprotein cholesterol than men did. CONCLUSION The results of this study provide evidence that diabetes prevention, care, and education need to be targeted to men and women differently. Primary care providers should encourage men to attend diabetes self-management education sessions and emphasize the benefits of self-care. Primary care providers should promote regular diabetes screening and primary prevention to women, particularly women with a family history of diabetes or a high body mass index; emphasize the importance of weight management for those with and without diabetes; and screen diabetic women for depressive symptoms.

Abstract: Objective Iatrogenic laryngotracheal stenosis (iLTS) is the pathologic narrowing of the glottis, subglottis, and/or trachea secondary to intubation or tracheostomy related injury. Patients with type 2 diabetes mellitus (T2DM) are more likely to develop iLTS. To date, the metabolomics and phenotypic expression of cell markers in fibroblasts derived from patients with T2DM and iLTS are largely unknown. Study design Controlled in vitro cohort study. Setting Tertiary referral center (2017-2020). Methods This in vitro study assessed samples from 6 patients with iLTS who underwent surgery at a single institution. Fibroblasts were isolated from biopsy specimens of laryngotracheal scar and normal-appearing trachea and compared with controls obtained from the trachea of rapid autopsy specimens. Patients with iLTS were subcategorized into those with and without T2DM. Metabolic substrates were identified by mass spectrometry, and cell protein expression was measured by flow cytometry. Results T2DM iLTS-scar fibroblasts had a metabolically distinct profile and clustered tightly on a Pearson correlation heat map as compared with non-T2DM iLTS-scar fibroblasts. Levels of itaconate were elevated in T2DM iLTS-scar fibroblasts. Flow cytometry demonstrated that T2DM iLTS-scar fibroblasts were associated with higher CD90 expression (Thy-1; mean, 95%) when compared with non-T2DM iLTS-scar (mean, 83.6%; P = .0109) or normal tracheal fibroblasts (mean, 81.1%; P = .0042). Conclusions Scar-derived fibroblasts from patients with T2DM and iLTS have a metabolically distinct profile. These fibroblasts are characterized by an increase in itaconate, a metabolite related to immune-induced scar remodeling, and can be identified by elevated expression of CD90 (Thy-1) in vitro.

Abstract: We aimed to determine the impact of pre-operative isolation on postoperative pulmonary complications after elective surgery during the global SARS-CoV-2 pandemic. We performed an international prospective cohort study including patients undergoing elective surgery in October 2020. Isolation was defined as the period before surgery during which patients did not leave their house or receive visitors from outside their household. The primary outcome was postoperative pulmonary complications, adjusted in multivariable models for measured confounders. Pre-defined sub-group analyses were performed for the primary outcome. A total of 96,454 patients from 114 countries were included and overall, 26,948 (27.9%) patients isolated before surgery. Postoperative pulmonary complications were recorded in 1947 (2.0%) patients of which 227 (11.7%) were associated with SARS-CoV-2 infection. Patients who isolated pre-operatively were older, had more respiratory comorbidities and were more commonly from areas of high SARS-CoV-2 incidence and high-income countries. Although the overall rates of postoperative pulmonary complications were similar in those that isolated and those that did not (2.1% vs 2.0%, respectively), isolation was associated with higher rates of postoperative pulmonary complications after adjustment (adjusted OR 1.20, 95%CI 1.05–1.36, p=0.005). Sensitivity analyses revealed no further differences when patients were categorised by: pre-operative testing; use of COVID-19-free pathways; or community SARS-CoV-2 prevalence. The rate of postoperative pulmonary complications increased with periods of isolation longer than 3days, with an OR (95%CI) at 4–7days or ≥8days of 1.25 (1.04–1.48), p=0.015 and 1.31 (1.11–1.55), p=0.001, respectively. Isolation before elective surgery might be associated with a small but clinically important increased risk of postoperative pulmonary complications. Longer periods of isolation showed no reduction in the risk of postoperative pulmonary complications. These findings have significant implications for global provision of elective surgical care. Abstract: Interventional cardiologists are significantly exposed to X- rays and no dose of radiation may be considered well tolerated or harmless. Leaded aprons protect the trunk and the thyroid gland, leaded glasses protect the eyes. The operator's legs, arms, neck and head are, instead, not fully protected. In fact, the operator's brain remains the closest part to the primary X-ray beam and scatter in most interventional procedures and specifically the physician's front head is the most exposed region during device implantation performed at the patient's side. After the initial description of cases of brain and neck tumours, additional reports on head and neck malignancies have been published. Although a direct link between operator radiation exposure and brain cancer has not been established, these reports have heightened awareness of a potential association. The use of lead-based cranial dedicated shields may help reduce operator exposure but upward scattered radiation, weight and poor tolerability have raised concerns and hindered widespread acceptance. The purpose of this review is to describe current knowledge on occupational X-ray exposure of interventional cardiologists, with a special focus on the potential risks for the head and neck and efficacy of available protection devices.

Abstract: Background: MicroRNAs can regulates tumor metastasis either as an oncomiR or suppressor miRNA. Here, we investigate the role of miR-224 in lymphatic metastasis of non-small-cell lung cancer (NSCLC). Methods: The expression of miR-224 was demonstrated by a validation cohort of 156 lung cancer patients (77 cases with lymphatic metastasis) by q-PCR. In vitro and in vivo experiments were performed to study the malignant phenotype after upregulation and inhibition of miR-224 expression. Furthermore, the direct target genes of miR-224 were determined by a luciferase reporter assay. Results: miR-224 was identified as a high expression miRNA in the tumor tissues with lymphatic metastasis) with an area under the receiver operating characteristic curve (AUC) of 0.57. Forced expression of miR-224 in H1299 cells promoted not only the cell viability, plate clone formation, migration and invasion in vitro, but also tumor growth and lung metastasis in vivo. Consistently, inhibition of miR-224 suppressed the malignant characters both in vitro and in vivo. Molecular mechanism research suggested that miR-422a targeted the ANGPTL1 as a novel tumor suppressor.Conclusions: The present study demonstrates that miR-224 is a potential marker for the prediction of lymphatic metastasis of NSCLC. And application of miR-224 may help for prophylactic intervention of NSCLC in clinical practice.

Abstract: Understanding and forecasting mobility patterns and travel demand are fundamental and critical to efficient transport infrastructure planning and service operation. However, most existing studies focused on deterministic demand estimation/prediction/analytics. Differently, this study provides confidence interval based demand forecasting, which can help transport planning and operation authorities to better accommodate demand uncertainty/variability. The proposed Origin-Destination (OD) demand prediction approach well captures and utilizes the correlations among spatial and temporal information. In particular, the proposed Probabilistic Graph Convolution Model (PGCM) consists of two components: (i) a prediction module based on Graph Convolution Network and combined with the gated mechanism to predict OD demand by utilizing spatio-temporal relations; (ii) a Bayesian-based approximation module to measure the confidence interval of demand prediction by evaluating the graph-based model uncertainty. We use a large-scale real-world public transit dataset from the Greater Sydney area to test and evaluate the proposed approach. The experimental results demonstrate that the proposed method is capable of capturing the spatial-temporal correlations for more robust demand prediction against several established tools in the literature.

Abstract: We consider a simple long short-term memory (LSTM) neural network extension of the Poisson Lee-Carter model, with a particular focus on different procedures for how to use training data efficiently, combined with ensembling to stabilise the predictive performance. We compare the standard approach of withholding the last fraction of observations for validation, with two other approaches: sampling a fraction of observations randomly in time; and splitting the population into two parts by sampling individual life histories. We provide empirical and theoretical support for using these alternative approaches. Furthermore, to improve the stability of long-term predictions, we consider boosted versions of the Poisson Lee-Carter LSTM. In the numerical illustrations it is seen that even in situations where mortality rates are essentially log-linear as a function of calendar time, the boosted model does not perform significantly worse than a simple random walk with drift, and when non-linearities are present the predictive performance is improved. Moreover, boosting allows us to obtain reasonable model calibrations based on as few data points as 20 years.

Abstract: Early tumor size reduction (TSR) has been explored as a prognostic factor for survival in patients with advanced melanoma in clinical trials. The purpose of this analysis is to validate, in a routine clinical milieu, the predictive capacity of TSR by 10% for overall survival (OS) and progression-free survival (PFS) and to compare its predictive performance with the RECIST 1.1 criteria.This retrospective study was approved by the local ethics committee. A total of 152 patients with both CT before immunotherapy initiation and at first response evaluation after immunotherapy initiation were included. Prior to statistical analysis, treatment response was trichotomized as follows: Complete response and/or partial response, stable disease and progressive disease. Furthermore, response was dichotomized regarding TSR (TSR ≥ 10% and TSR10%). Kaplan-Meier survival estimates, Cox regression and Harrel's concordance index (C-index) were computed for prediction of overall survival and progression-free survival.Tumor size reduction by at least 10% significantly differentiated between patients with increased survival from the ones with decreased survival (median OS: TSR ≥ 10%: 2137 days vs. TSR10%: 263 days) (p0.001) (median PFS: TSR ≥ 10%: 590 days vs. TSR10%: 11 days) (p0.001). RECIST 1.1. criteria had a slightly higher C-index for overall survival reflecting a slight superior predictive capacity (RECIST: 0.69 vs TSR: 0.64) but a similar predictive capacity regarding progression-free survival (both: 0. 63).Early tumor size reduction serves as a simple-to-use metric which can be implemented on the first follow-up CT. Tumor size reduction by at least 10% can be considered an additional biomarker predictive of overall survival and progression-free survival in routine clinical care and not only in the context of clinical trials in patients with advanced melanoma undergoing immunotherapy. Nevertheless, RECIST-based criteria should remain the main tool of treatment response assessment until results of prospective studies validating the TSR method are available.

Abstract: Resumen La brucelosis es una zoonosis que genera una infeccion granulomatosa multiorganica. Tiene una clinica diversa e inespecifica que puede hacer dificil el diagnostico. Con frecuencia, el personal medico no la reconoce de forma temprana. El retraso en el tratamiento se asocia con una gran morbilidad e incluso mortalidad. Su diagnostico oportuno requiere un alto indice de sospecha. Se presenta el caso de un hombre de 35 anos, zootecnista, previamente sano, con un cuadro progresivo de dos meses de evolucion de dolor lumbar irradiado a cadera izquierda, diaforesis nocturna y perdida no intencional de peso. Se documento elevacion de reactantes de fase aguda y en la resonancia magnetica se encontraron signos de tendinitis del psoas y cambios inflamatorios en la articulacion sacroiliaca izquierda. Los anticuerpos IgG e IgM por inmunoensayo para Brucella fueron positivos, y luego de instaurar tratamiento antibiotico se evidencio marcada mejoria clinica con resolucion del proceso inflamatorio.

Abstract: Image-based patient-specific modelling of hemodynamics are gaining increased popularity as a diagnosis and outcome prediction solution for a variety of cardiovascular diseases. While their potential to improve diagnostic capabilities and thereby clinical outcome is widely recognized, these methods require considerable computational resources since they are mostly based on conventional numerical methods such as computational fluid dynamics (CFD). As an alternative to the numerical methods, we propose a machine learning (ML) based approach to calculate patient-specific hemodynamic parameters. Compared to CFD based methods, our approach holds the benefit of being able to calculate a patient-specific hemodynamic outcome instantly with little need for computational power. In this proof-of-concept study, we present a deep artificial neural network (ANN) capable of computing hemodynamics for patients with aortic coarctation in a centerline aggregated (i.e., locally averaged) form. Considering the complex relation between vessels shape and hemodynamics on the one hand and the limited availability of suitable clinical data on the other, a sufficient accuracy of the ANN may however not be achieved with available data only. Another key aspect of this study is therefore the successful augmentation of available clinical data. Using a statistical shape model, additional training data was generated which substantially increased the ANN's accuracy, showcasing the ability of ML based methods to perform in-silico modelling tasks previously requiring resource intensive CFD simulations.

Abstract: Background Bariatric surgery can influence the presentation, diagnosis, and management of gastrointestinal cancers. Oesophago-Gastric (OG) malignancies in patients who have had a prior bariatric procedure have not been fully characterised. Objective To characterise OG malignancies after bariatric procedures. Setting University Hospital, United Kingdom. Methods We performed a retrospective, multi-centre observational study of patients with OG malignancies after bariatric surgery to characterise this condition. Results This study includes 170 patients from 75 centres in 25 countries who underwent bariatric procedures between 1985 and 2020. At the time of the bariatric procedure, the mean age was 50.2 ± 10 years and the mean weight 128.8 ± 28.9 kg. Females comprised 57.3% (n=98) of the population. Most (n=64) patients underwent a Roux-en-Y Gastric Bypass (RYGB) followed by Adjustable Gastric Band (AGB) (n = 46) and Sleeve Gastrectomy (SG) (n = 43). Time to cancer diagnosis after bariatric surgery was 9.5 ± 7.4 years and mean weight at diagnosis was 87.4 ± 21.9 kg. The time lag was 5.9 ± 4.1 years after SG compared to 9.4 ± 7.1 years after RYGB and 10.5 ± 5.7 years after AGB. One-third of patients presented with metastatic disease. The majority of tumours were adenocarcinoma (82.9%). Approximately 1 in 5 patients underwent palliative treatment from the outset. Time from diagnosis to mortality was under one year for most patients who died over the intervening period. Conclusions The OGMOS (Oesophago-Gastric Malignancies after Obesity/ Bariatric Surgery) study presents the largest series to date of patients developing OG malignancies after bariatric surgery and attempts to characterise this condition.

Abstract: Rationale and Objectives Aortic stenosis (AS) is one of the most common valvular heart disease. Symptomatic AS is associated with a high mortality rate which prompts fast intervention. The introduction of transcatheter aortic valve replacement (TAVR) has drastically improved the outcome of high surgical risk for mortality patients with severe AS. However, this procedure requires the employment of multimodality imaging in the pre-procedural planning, intra-procedural optimization, and post-procedural follow-up stages. This also requires an accurate understanding of the indications, measurements, strength, and limitations of each imaging modality during the different TAVR stages. Conclusion In this review, we aim to outline to radiologists the evidence-based approach and indications of different imaging modalities through the pre, peri, and post TAVR stages.

Abstract: Resumen Introduccion Las enfermedades cronicas son un problema de salud publica; el 80% de ellas se relacionan con factores de riesgo modificables, como una dieta poco saludable, la inactividad fisica, el tabaquismo y el consumo riesgoso de alcohol. La intervencion en el tabaquismo y el consumo riesgoso de alcohol se ha demostrado efectiva en el cuidado primario, pero se desconoce si funciona de la misma manera en el contexto hospitalario. Objetivo Evaluar la efectividad de la consejeria breve para modificar el estadio de cambio en pacientes fumadores y bebedores en riesgo atendidos en un hospital de alta complejidad. Metodos Experimento clinico aleatorizado, que evalua la efectividad de 4 modalidades de consejeria breve para la cesacion de tabaquismo y el consumo riesgoso de alcohol en comparacion con el cuidado habitual, seleccionadas segun el estadio de cambio del sujeto. El resultado primario es la proporcion de pacientes en cada uno de los grupos (intervencion y control) en los cuales se identifica el avance en el estadio de cambio; ademas se analizara la reduccion de consumos. Protocolo registrado en ClinicalTrials.gov (NCT03521622). Resultados Los resultados se publicaran en revistas de literatura cientifica y su aplicacion pretende generar protocolos de intervenciones conductuales en factores de riesgo modificables en hospitales de alta complejidad. R01MH116269 and R01MH121455 [to DMR]), NIGMS of the National Institutes of Health (Grant No. T32GM007347 [to JK]), and the Brain & Behavior Research Foundation (NARSAD Young Investigator Award No. 29551 [to NM])., BACKGROUND: Suicide is a leading cause of death worldwide, and nonfatal suicide attempts, which occur far more frequently, are a major source of disability and social and economic burden. Both have substantial genetic etiology, which is partially shared and partially distinct from that of related psychiatric disorders. METHODS: We conducted a genome-wide association study (GWAS) of 29,782 suicide attempt (SA) cases and 519,961 controls in the International Suicide Genetics Consortium (ISGC). The GWAS of SA was conditioned on psychiatric disorders using GWAS summary statistics via multitrait-based conditional and joint analysis, to remove genetic effects on SA mediated by psychiatric disorders. We investigated the shared and divergent genetic architectures of SA, psychiatric disorders, and other known risk factors. RESULTS: Two loci reached genome-wide significance for SA: the major histocompatibility complex and an intergenic locus on chromosome 7, the latter of which remained associated with SA after conditioning on psychiatric disorders and replicated in an independent cohort from the Million Veteran Program. This locus has been implicated in risk-taking behavior, smoking, and insomnia. SA showed strong genetic correlation with psychiatric disorders, particularly major depression, and also with smoking, pain, risk-taking behavior, sleep disturbances, lower educational attainment, reproductive traits, lower socioeconomic status, and poorer general health. After conditioning on psychiatric disorders, the genetic correlations between SA and psychiatric disorders decreased, whereas those with nonpsychiatric traits remained largely unchanged. Abstract: Space-air-ground integrated network (SAGIN) has emerged as a paradigm shifting architecture that offers large-scale, flexible wireless coverage and seamless, high-rate connectivity to complement terrestrial communications. Nonetheless, unlocking the potentials of SAGIN is subject to addressing non-trivial challenges associated with their inherent time-variability, self-organization, and heterogeneity. Meanwhile, the concept of reconfigurable intelligent surfaces (RISs) is recognized as a disruptive technology that supports an unprecedented control of electromagnetic waves propagation and potentially offers significant enhancements in spectral efficiency, coverage expansion, and security, all achieved in a low-cost and energy-efficient manner. We anticipate that the integration of RISs into SAGIN will not only play a fundamental role in improving the quality of inter- and intra- layer communications, but will also provide complex interactions among the three network segments, and hence, opens the horizons for a new breed of applications across all industries. In this article, we explore the underlying opportunities and challenges of integrating RIS-enabled communications into SAGIN, and present a forward-looking overview of the cross-layer operational elements supported by RISs. Finally, we outline major enabling technologies and present a look ahead toward addressing open research issues.

Abstract: Predicting that a stimulus is painful facilitates action to avoid harm. But how distinct are the neural processes underlying the prediction of upcoming painful events, and those taking action to avoid or prevent them? In this fMRI experiment, we addressed this by investigating brain activity as a function of current and predicted painful or nonpainful thermal stimulation, and the ability of voluntary action to affect the duration of the upcoming stimulation. Participants (n = 30) performed a task which involved the administration of a painful or nonpainful stimulus (S1), which predicted an immediately subsequent very painful or nonpainful stimulus (S2). On action-effective trials, pressing a response button within a specified time window during S1 reduced the duration of the upcoming stimulation in S2. On action-ineffective trials, pressing the button had no effect on upcoming stimulation. Predicted pain increased activation in regions including anterior cingulate cortex (ACC), midcingulate cortex (MCC), and insula; however, activation in ACC and MCC depended on whether a meaningful action was performed, with MCC activation showing a direct relationship with motor output. Region-of-interest analyses revealed that insula’s responses for predicted pain were also modulated by potential action consequences, especially in the left hemisphere, albeit without a direct relationship with motor output. Taken together, these findings suggest that cortical pain processing is not specifically tied to the sensory stimulus, but instead depends on the consequences of that stimulus for sensorimotor control of behavior.Significance statementDuring acute pain, the processing of an acute sensory event likely occurs in parallel with predictive processing about its relevance for current and upcoming voluntary behavior. Here, we temporally separated the functional processes underlying current and predicted pain and found that activation in regions typically implicated in acute pain is modulated both by the noxious nature of upcoming events and by the possibility to affect those events via voluntary action (a button press). Our findings suggest that cortical pain processing is not specifically tied to the sensory stimulus, but instead is processed in “consequence-level” terms based on what the stimulus implies for sensorimotor control of behavior.

Abstract: Identity recognition plays an important role in ensuring security in our daily life. Biometric-based (especially activity-based) approaches are favored due to their fidelity, universality, and resilience. However, most existing machine learning-based approaches rely on a traditional workflow where models are usually trained once for all, with limited involvement from end-users in the process and neglecting the dynamic nature of the learning process. This makes the models static and can not be updated in time, which usually leads to high false positive or false negative. Thus, in practice, an expert is desired to assist with providing high-quality observations and interpretation of model outputs. It is expedient to combine both advantages of human experts and the computational capability of computers to create a tight-coupling incremental learning process for better performance. In this study, we develop RLTIR, an interactive identity recognition approach based on reinforcement learning, to adjust the identification model by human guidance. We first build a base tree-structured identity recognition model. And an expert is introduced in the model for giving feedback upon model outputs. Then, the model is updated according to strategies that are automatically learned under a designated reinforcement learning framework. To the best of our knowledge, it is the very first attempt to combine human expert knowledge with model learning in the area of identity recognition. The experimental results show that the reinforced interactive identity recognition framework outperforms baseline methods with regard to recognition accuracy and robustness.

Abstract: Light fidelity (LiFi), which is based on visible light communications (VLC), is celebrated as a cutting-edge technological paradigm that is envisioned to be an indispensable part of 6G systems. Nonetheless, LiFi performance is subject to efficiently overcoming the line-of-sight blockage, whose adverse effect on the reliability of wireless reception becomes even more pronounced in highly dynamic environments, such as vehicular applications. Meanwhile, reconfigurable intelligent surfaces (RISs) emerged recently as a revolutionary concept that transforms the physical propagation environment into a fully controllable and customisable space in a low-cost low-power fashion. We anticipate that the integration of RISs in LiFi enabled networks will not only support blockage mitigation but will also provision complex interactions among network entities, and is hence manifested as a promising platform that enables a plethora of technological trends and new applications. In this article, for the first time in the open literature, we set the scene for a holistic overview of RIS-assisted LiFi systems. Specifically, we explore the underlying RIS architecture from the perspective of physics and present a forward-looking vision that outlines potential operational elements supported by RIS-enabled transceivers and RIS-enabled environments. Finally, we highlight major associated challenges and offer a look ahead toward promising future directions.

Abstract: Background Clostridioides difficile infection (CDI) is a disease that is potentially preventable by vaccination. A good knowledge of its epidemiology, which can change over time, is warranted for prevention purposes and to help decision-making on the use of vaccines in public health programs. The objective of the research was to determine the epidemiology of healthcare-associated CDI (HA-CDI) and community-associated CDI (CA-CDI) in hospitalized patients in Spain using point prevalence data. Methods Point prevalence survey data on infections of hospitalized patients for years 2012–2019 were analyzed. HA-CDI and CA-CDI prevalence rates were calculated. Both HA-CDI and CA-CDI, as well as age group prevalence rates, were examined for trends. Patient comorbidities were tested for association to CDI. Results The prevalence of CDI in Spanish hospitals has grown exponentially from 14.1% in 2012 to 35.9% in 2019 (cases/10.000 hospitalized patients). Almost two thirds of the cases are of nosocomial onset. This increase was observed for HA-CDI and CA-CDI at an annual rate of 1.11% (CI 95% 1.08–1.15) and 1.09% (CI 95% 1.04–1.13), respectively. Patients 50 years old or older represent 87% of the total number of cases. Patients suffering from neoplasm (OR 1.39), immunodeficiency (OR 3.26), neutropenia (OR 3.70), cirrhosis (OR 1.92) and chronic renal failure (OR 1.91) have a significant increased risk of developing CDI, after adjusting for age. Conclusion In Spain, the prevalence rate of both HA-CDI and CA-CDI have been increasing. Burden of CDI as well as clinical and epidemiological characteristics of CDI patients will help to support public health decision-making.

Abstract: Steganalysis in real-world application often exhibit skewed sample distribution which poses a massive challenge for steganography detection. Conventional steganalysis algorithms are not effective when the training data distribution is imbalanced, and may fail in the scenario of imbalanced data distribution. To address imbalanced data distribution issue in steganalysis, a novel framework termed adaptive cost-sensitive feature learning via F-measure maximization is proposed, which is inspired by the fact that F-measure is a more suitable performance metric compared to accuracy for imbalanced data. We investigate the adaptive cost-sensitive strategy by generating and assigning different weight to each instance with misclassification occurrence. This scheme adaptively determines the weights according to the intra-class and inter-class costs from the imbalanced distribution. Features corresponding to the largest F-measure can be obtained by solving a series of adaptive cost-sensitive feature learning problems with optimization theory. In this way, the learned features are the most representative features between the cover and stego images so that imbalanced steganalysis can significantly alleviate. Extensive experiments on various imbalanced steganalysis tasks show the superiority of the proposed method over the state-of-the-art methods, and it can recognize more minority samples and has excellent classification performance.
