WOS: 000408863600003 Objectives: The aim of the present study was to evaluate demographic and clinical findings of patients who underwent follow-up at the newly established epilepsy outpatient clinic of the Mugla University Faculty of Medicine. Methods: Included were 208 patients who underwent follow-up at the clinic between March 2014 and September 2015. Results: Mean age was 29.42 +/- 15.86, with a range of 5-84 years. Eighty-three patients were men (39.9%); 125 were women (60.1%). The most common risk factors included head trauma, febrile convulsion, and family history of epilepsy. Seizures of 155 patients (75.5%) were partialonset, those of 22 (10.6%) were generalized, and those of 9 (4.3%) were unclassified. Sixty-seven patients (37.2%) had normal electroencephalogram, 96 patients (53.3%) had partial, and 10 patients (5.6%) had generalized epileptiform abnormalities. Of the 172 patients (28.5%) to whom antiepileptic drugs had been administered, 49 were resistant to medical treatment. Aims: Clinical detection of inflammatory markers is useful to assess systemic inflammation in patients with obstructive sleep apnea syndrome (OSAS). The aim of the study was to evaluate whether the lymphocyte-to-C-reactive protein (CRP) ratio (LCR) was a predictive marker in diagnosing and determining the severity of OSAS. Settings and Design: This was a retrospective clinical study. Subjects and Methods: One hundred and forty-one patients who had undergone polysomnography were included in the study. The sex, age, Apnea-Hypopnea Index (AHI), body mass index, and complete blood count parameters of the patients were recorded. AHI scores were used to classify the severity of OSAS. Statistical Analysis Used: Differences among the groups for each parameter were analyzed using Student's t-test and one-way analysis of variance with Tukey correction for normally distributed parameters and the Mann-Whitney U test and Kruskal-Wallis test for nonnormally distributed parameters. The correlation between LCR, neutrophil-to-lymphocyte ratio, platelet-to-lymphocyte ratio, CRP, and AHI scores was assessed using Pearson's correlation coefficient. Results: First, the patients were divided into four groups according to their AHI results, as control group, mild, moderate, and severe OSAS groups. The median LCR levels were 2.57 (1.59, 3.51) in the control group, 1.28 (0.74, 3.27) in Group 2, 1.63 (0.86, 2.6) in Group 3, and 1.05 (0.62, 2.31) in Group 4. In the comparison of all patients with OSAS and the control group, the median LCR level was 1.27 (0.7, 2.74) in patients with OSAS and 2.57 (1.59, 3.51) in the control group (P = 0.002). Conclusions: This study demonstrated that LCR is an important marker for systemic inflammation in patients with OSAS. LCR may be a new predictive marker in the diagnosis and prognosis of patients with OSAS.

In this study, we aimed to determine the blood levels of nesfatin-1, GLP-1 and oxidative stress status in patients with PD.Material and methodForty patients with PD, followed-up at the Department of Neurology of Mugla Sitki Kocman University Training and Research Hospital, were enrolled, as well as 40 age- and sex-matched participants as a control group. We determined and compared nesfatin-1, GLP-1, total antioxidant status (TAS), and total oxidant status (TOS) levels in patients with PD and control group.ResultsThe mean GLP-1 and nesfatin-1 values of patients with PD were lower than those of the control group, whereas their mean TOS value was higher. The mean TAS values, on the other hand, did not reveal any significant difference between the patient and the control groups.ConclusionThe lower nesfatin-1 and GLP-1 levels, in addition to higher TOS levels, in patients with PD compared to those of control group suggest that the neuroprotective effects of these molecules might be related to the oxidative processes. Further studies are required to search for the impact of abovenamed molecules on the treatment option and the likelihood that they may slow down disease progression. Transient global amnesia (TGA) is a clinical syndrome characterized by sudden-onset anterograde amnesia, accompanied by repetitive questioning, sometimes with a retrograde component, lasting up to 24 hours, and without compromise of other neurologic functions. Typically, it occurs in individuals aged 50-80 years, with a decreased incidence in younger and older populations. There may be many causes of TGA. Hippocampal ischemia also contributes to the cause of TGA. In this case report, a 67-year-old woman who presented with TGA clinical features accompanied by right hippocampal diffusion-weighted imaging hyperintensity is presented. In this study, our aim was to investigate the correlation of waist-to-height ratio, an indicator of central obesity, with presence and severity of OSAS; to compare the use of this ratio with the use of waist circumference and body mass index (BMI); and to determine OSAS-related cutoff values. Methods The patient records were retrospectively analyzed for whom a polysomnography was conducted at our sleep. Sex, age, Apnea-Hypopnea Index (AHI), waist circumference, height, and BMI values of those patients were recorded. AHI scores were used to classify severity of OSAS. Results The study included 437 OSAS patients and 72 control cases. Out of the patient group, OSAS was severe in 208 (47%) patients, moderate in 124 (28%), and mild in 105 (24%) of them. In the group of OSAS patients, waist-to-height ratio, waist circumference, and BMI were higher compared to the control group with a further difference of all three parameters among severe, moderate, mild OSAS, and controls both in males and females. Cutoff values for OSAS of females were 95.5cm for waist circumference, 0.595 for waist-to-height ratio, and 27.75 for BMI whereas the cutoff values of males were 100.5cm, 0.575, and 27.75, respectively. Conclusions A high value of waist circumference, waist-to-height ratio, and BMI is associated with the presence and severity of OSAS. We have determined the cutoff values of the studied anthropometric measurements in both sexes for OSAS and severe OSAS.

WOS: 000435398900004 Objective: Restless Legs syndrome (RLS) is a frequent neurological disease. Levetiracetam (LEV) is an effective and broad-spectrum anticonvulsant drug. The aim of this study is to investigate the frequency of RLS in patients diagnosed with epilepsy who took LEV monotherapy. Materials and Methods: Two neurologists were reviewed the files of 1680 patients, who were followed in epilepsy outpatient clinic. One hundred seven patients under LEV monotherapy for at least six months and 120 healthy controls were included in the study. The criteria for the International Restless Legs Syndrome Study Group were taken into consideration for the assessment of RLS. Results: The mean age of patient group was 38.26 +/- 17.39 years, while the mean age of healthy controls was 39.17 +/- 16.12 years. There were no statistically significant differences in terms of age and sex between each two groups. The mean duration of LEV usage was 28.34 months. The LEV usage dose was between 500-3000 mg/day. RLS was seen in 7 (5.83%) healthy controls, whereas 4 patients (3.73%) had RLS in patient group before LEV treatment. The symptom of RLS in two patients disappeared after LEV treatment started and this rate decreased to 1.87%. The biochemical parameters were normal in all patient groups, after LEV treatment, peripheral neuropathy was detected in one patient whose symptoms were not relieved. Conclusion: LEV may have therapeutic effect on RLS. It may be preferred especially in comorbid situations where epilepsy and RLS are seen together. Longitudinal and prospective studies are needed to manifest better the therapeutic effects of LEV on RLS.

WOS: 000427323200003 Objective: Obstructive Sleep Apnea syndrome (OSAS) is a disease commonly seen in the community, causing labor loss and neuropsychiatric symptoms. The aim of the study is to compare the symptoms of depression and anxiety with those of healthy controls in patients with OSAS and to investigate the relationship between OSAS severity with anxiety and depression. Materials and Methods: Two hundred and thirteen patients with OSAS who underwent polysomnographic examination and 116 healthy controls were included in the study. The patient group was classified as mild, moderate and severe OSAS. Beck Anxiety and Beck Depression Scales were administered to the patients and healthy controls. Results: There was a significant difference between the patient group and the control group in the mean scores of Beck Depression and Beck Anxiety Scales. The mean scores of depression of the OSAS group were 11.16 +/- 8.87 (mild depression); 6.85 +/- 6.15 of the control group (p0.05). Conclusion: In our study; although there was a significant difference between depression and anxiety mean scores in patient and control group; there was no significant difference in anxiety and depression mean scores among mild, moderate and severe OSAS patients. Patients with OSAS were found to have mild anxiety and depressive symptoms. In these patients, there was no correlation between AHI values and depression and anxiety scores.

WOS: 000465507100013 PubMed ID: 30759289 The etiology of juvenile myoclonic epilepsy (JME) is still unknown and the process of elaboration of multiple genetic mechanisms is ongoing. The aim of this study was to investigate the potential role of NKCC1 (SCL12A2) and KCC2 (SCL12A5) in JME by comparing their DNA methylation status in patients with JME versus healthy controls. Forty-nine patients with JME and 39 healthy individuals were compared for DNA methylation at the 5CpG islands. A total of 71 (81%) samples were found to have methylation in the NKCC1 gene, 36 (73%) from patients and 35 (90%) from healthy individuals. Out of the KCC2 samples, 50 (57%) were found to have methylation, 33 (67%) from patients and 17 (44%) from healthy individuals. In patients with JME, methylation of NKCC1 (73%) was lower than its methylation in the controls (90%) (p=0.047). On the other hand, methylation of KCC2 in patients with JME (67%) was greater than the methylation in the controls (44%) (p=0.022). Twenty-eight patients were treated with VPA and ongoing medications were not found to be associated with methylation (p>0.05). In the present study, we determined significantly lower NKCC1 DNA methylation and significantly higher KCC2 DNA methylation levels in patients with JME compared with the healthy controls. This implies that NKCC1 expression can be higher and KCC2 expression can be reduced in affected people. Further studies that investigate the potential effect of DNA methylation mechanisms regulating gene expression on seizure activity and how they change JME network activity will be helpful.

WOS: 000452714700010 Aim: To explore if patients with juvenile myoclonic epilepsy (JME) have differences in heart rate variability (HRV) associated with risk of arrhythmias and in systolic and diastolic functions. Patients and methods: This single-centre prospective study included 50 patients with JME followed up at the epilepsy outpatient clinic within the Antalya Training and Research Hospital's Neurology Department (34 women, mean age 26 +/- 7.58 years) and 45 healthy controls (30 women, mean age 26.71 +/- 5.14 years). Two patients were excluded since they had arterial hypertension, one patient was excluded due to diabetes mellitus and one patient was excluded due to rheumatic mitral stenosis. Finally, 46 patients were included in the study. All patients and controls were evaluated by conventional echocardiography and tissue Doppler imaging for systolic and diastolic functions (e.g. left ventricular ejection fraction, left ventricle diameters and volumes, deceleration time, pulmonary forward flows) and performing 24-h Holter monitoring to explore time domain (e.g. standard deviation of the normal-to-normal interval, the square root of the mean squared differences of successive normal-to-normal intervals) and frequency domain parameters of HRV. Results: There were no significant differences between the Holter parameters of the two groups with respect to HRV. Echocardiographic investigation did not reveal any significant differences except for the ratio of pulmonary venous systolic and diastolic (PVS/PVD) forward flows to one another (p = 0.008). Conclusion: In this study, we did not find anything else about cardiac involvement other than increased ratio of PVS/PVD and we did not find autonomic dysfunction in patients with JME. This may be due to good seizure control.

WOS: 000353003800007 PubMed ID: 25799899 Purpose: Behcet disease (BD) is a chronic relapsing inflammatory disorder. Neuro BD (NBD) is seen in approximately 5% of all patients. The aim of this study is to investigate the frequency, type and prognosis of epileptic seizures in different forms of NBD. Methods: All files of 42 patients with NBD were evaluated between 2006 and 2012, retrospectively. The demographic data, the presentation of NBD, clinical findings including seizures, EEG and neuroimaging findings were reviewed. Results: The mean age of patients was 35.02 +/- 8.43 years. Thirty (71.4%) patients were male; the remaining 12 of them were female. Twenty-four patients had brainstem lesions; 16 patients had cerebral venous thrombosis. Spinal cord involvement was seen in two patients. Seven patients had epileptic seizures (six partial onset seizures with or without secondary generalization). Six of them had cerebral sinus thrombosis (CVT). Four patients had a seizure as the first symptom of the thrombosis. One patient had late onset seizure due to chronic venous infarct. The other patient with seizure had brainstem involvement. The remaining was diagnosed as epilepsy before the determination of NBD. Conclusion: CVT seen in BD seems to be the main risk factor for epileptic seizures in patients with NBD. The prognosis is usually good especially in patients with CVT. Epileptic seizures in patients with brainstem involvement may be an indicator for poor prognosis. Superior sagittal thrombosis or cortical infarct would be predictor of seizures occurrence because of the high ratio in patients with seizures. (C) 2015 British Epilepsy Association. As the age of onset rises, the risk increases. Patients with refractory epilepsy are particularly at risk. Likewise, male gender, early age of onset, polytherapy, nocturnal seizures, generalized tonic-clonic seizures (GTCS) are also among the risk factors. Respiratory, cardiac, autonomic and brain stem related pathologies are held to be responsible for its pathophysiology.

Respiratory weakness and bulbar involvement are the major complications and early diagnosis of these complications is very important for the planning of treatment. Method and Patients: Twenty-one patients with myasthenia gravis and 21 healthy controls were included in this study. Repetitive nerve stimulation (3Hz) tests were performed on facial, ulnar and long thoracic nerves. The difference between the first and fourth compound muscle action potential (CMAP) amplitudes was recorded. Respiratory functions were evaluated using forced vital capacity (FVC) and the ratio of forced expiratory volume in 1s (FEV1) to FVC. Results: 15 male and 6 female patients were included in the patient group. The sensitivity and specificity of long thoracic nerve compared to facial nerve were 0.86 and 0.66, respectively. There was a significant difference between the decrements recorded from serratus anterior (SA). A strong correlation was observed between the percentage of the decrement recorded from SA and FEV1/FVC ratio. Conclusions: The use of repetitive stimulation of long thoracic nerve compared to respiratory function tests might be a well-tolerated and simple way for the early diagnosis of pulmonary dysfunction in MG. Bu çalışmada MG hastalarında gelişebilecek respiratuar yetmezliğin tespitinde uzun torasik sinir repetetif stimulasyonunun (RNS) solunum fonksiyon testi ile korelasyonu ve güvenilirliğini araştırmayı amaçladık. Metod: Yirmibir MG hastası ve 21 sağlıklı kontrol grubu çalışmaya dahil edildi. Fasiyal, ulnar, uzun torasik sinirlerde repetetif sinir uyarımı (3 Hz) yapıldı. Birinci ve dördüncü birleşik kas aksiyon potansiyel (BKAP) amplitüdleri arasındaki fark kaydedildi. Solunum fonksiyonları, zorlu vital kapasite (FVC) ve birinci saniyedeki zorlu ekspirasyon hacminin (FEV1), FVC'ye oranı ölçülerek değerlendirildi. Bulgular: Hasta grubunda 15 erkek ve 6 kadın vardı. Uzun torasik sinirin fasiyal sinire göre duyarlılık ve özgüllüğü, 0.86 ve 0.66 idi. Hasta ve kontrol grubu arasında serratus anterior kasından (SA) kaydedilen dekrement yanıtlar arasında anlamlı fark saptandı. SA kasından kaydedilen dekrement yanıtların yüzdesi ile FEV1/FVC oranı arasında güçlü bir korelasyon gözlendi. Sonuç: Uzun torasik sinirin repetetif uyarımı; solunum fonksiyon testi ile kıyaslandığında iyi tolere edilebilen ve MG'e bağlı solunum disfonksiyonunun erken teşhisinde basit bir yöntemdir.

Soluble urokinase plasminogen activator receptor (suPAR) is a potential novel inflammatory marker. We aim to determine the association between serum values of suPAR, procalcitonin, fibrinogen, and high-sensitivity C-reactive protein (hs-CRP) and migraine disease characteristics. The study involved a total of 60 migraine patients (33 patients in the interictal period, 27 patients in the attack period) and 30 healthy individuals. The serum values of suPAR were found to be significantly higher in migraine patients in the attack period than in migraine patients in the interictal period, and in healthy individuals (p < .01 for both). In addition, levels of suPAR were determined to be higher in migraine with aura patients than in migraine without aura patients. When we subdivided migraine patients according to frequency of attack (attacks/month), significant differences were found between the suPAR and procalcitonin levels (measured during the attack period) of those in the frequent-attack group (4-5 or more) versus those in the less frequent attack group (less than 4). Serum levels of procalcitonin were shown to be significantly higher in migraine patients during the attack period compared with migraine patients in the interictal period and in control subjects (p = .001 for both). Significant differences were found between plasma levels of fibrinogen in migraine patients versus control subjects (p < .01). No statistically significant difference was found between levels of hs-CRP in migraine patients versus the control group. These findings may show that presenting a high level of suPAR in migraine patients with attack and aura results to predisposition to occurring on the symptoms and that high levels of suPAR, procalcitonin and fibrinogen in patients with migraine result in neurogenic inflammation during migraine headaches. We investigated epicardial fat thickness in patients who had sustained an acute ischemic stroke, and we evaluated the relationship of epicardial fat thickness with other prognostic factors. We enrolled 61 consecutive patients (age, >= 18 yr) who had sustained a first acute ischemic stroke and had been admitted to our hospital within 24 hours of the onset of stroke symptoms. The control group comprised 82 consecutive sex-and age-matched patients free of past or current stroke who had been admitted to our cardiology clinics. Blood samples were taken for measurement of N-terminal pro-brain natriuretic peptide (NT-proBNP) levels at admission. Aortic stiffness indices and epicardial fat thickness were measured by means of transthoracic echocardiography within the first 48 hours. In comparison with the control group, the patients with acute ischemic stroke had significantly higher epicardial fat thickness (4.8 +/- 0.9 vs 3.8 +/- 0.7 mm; P < 0.001), lower aortic distensibility (2.5 +/- 0.8 vs 3.4 +/- 0.9 cm(2).dyn(-1); P < 0.001) and lower aortic strain (5.5% +/- 1.9% vs 6.4% +/- 1.8%; P=0.003). We found a significant association between epicardial fat thickness, NT-proBNP levels, and arterial dysfunction in patients who had sustained acute ischemic stroke. WOS: 000389053000033 PubMed ID: 27966319 Background/aim: This study aimed to define the frequency of a primitive reflex, the buccopalpebral reflex (BPR), and its association with the clinical situation in patients with Parkinson disease. Materials and methods: Between May 2010 and May 2011, 222 patients, 115 with Parkinson disease and 107 patients without any sign of neurodegenerative disease, were included in the study. All included patients were examined for BPR and snout reflex and were also evaluated with the Mini Mental State Examination. All patients with Parkinson disease were classified with the Unified Parkinson's Disease Rating Scale (UPDRS) and the Hoehn and Yahr Score to determine their clinical severity. Results: Sixteen patients with Parkinson disease (13.9%) had a BPR (+) and 4 patients in the control group (3.7%) (P < 0.001). The UPDRS score, UPDRS daily life activities score, and UPDRS motor system score were all higher in the group with BPR (+). All patients with a BPR also had a positive snout reflex. Conclusion: BPR is more frequent in patients with Parkinson disease than in patients without a neurodegenerative disease.

Rufinamid, dört yaşından büyük hastalarda Lennox-Gastaut Sendromu ile ilişkili nöbetlerin ek tedavisi için Amerikan Gıda ve İlaç Dairesi tarafından Kasım 2008 yılında onaylanan bir ilaçtır. Lennox-Gastaut Sendromu ise tedaviye en dirençli epilepsilerin başında gelmekte, nöbetlerin çeşitliliği politerapiyi zorunlu kılmaktadır. Son yıllarda çocukluk çağı epileptik ensefalopati ve epileptik spazmlar, tonik nöbetler, miyoklonikastatik epilepsi, Dravet sendromu ve migrasyon anormalliklerine bağlı dirençli parsiyel nöbetlerde Rufinamid’in etkinliğini bildiren yayınlar dikkat çekmektedir. Bu yazıda, Ağustos 2011 tarihinde kurulan Antalya Eğitim ve Araştırma Hastanesi Epilepsi Polikliniği’nde Rufinamid’in, dirençli epileptik beş olgudaki etkinliğini değerlendirdik. The drug rufinamide was approved in November 2008 by the US Food and Drug Administration for use in the adjunctive treatment of seizures associated with Lennox-Gastaut syndrome in patients older than 4 years. Lennox-Gastaut Syndrome is one of the most treatment-resistant epilepsies, and seizures require a diversity of polytherapy. In recent years, a remarkable number of reports have been published on the effectiveness of rufinamide in childhood epileptic encephalopathy and epileptic spasms, tonic seizures, myoclonic-astatic epilepsy, Dravet syndrome, and refractory partial seizures dependent on migration abnormalities. This article discusses the efficiency of rufinamide as used in the treatment of five refractor epileptic cases at the Epilepsy Clinic established in Antalya Education and Research Hospital in August 2011.

Both stroke patients and their family can therefore experience increased traumatic stress level. Methods The participants are close relatives of patients (n=65) who had a first time stroke (CRPWS) hospitalized. A control group (CG) (n=61), who had no history of chronic illness in their family and had at least one traumatic life event experience. The National Institutes of Health Stroke Scale, Modified Rankin Scale, Personal Information Form, Life Events Checklist, Traumatic Stress Symptom Scale, and Multidimensional Scale of Perceived Social Support, were used in the study. Results We found no significant association between NIHSS and MRS of patients and traumatic stress level of the family member. The traumatic stress level was significantly higher in the CRPWS group than in the CG group. Traumatic stress level was higher in women than men and was not associated with perceived social support in the CRPWS group. Conclusion The traumatic stress level of the relatives was not associated with the clinical features of the stroke patients. In the early phase, after the diagnosis of stroke, psychological support may be important to prevent CRPWS from PTSD.
