Background and Aim High rates of inflammatory bowel disease (IBD) have been documented in New Zealand (NZ) children. The objectives of this study were to describe the outcomes and disease course of childhood IBD in the first 3 years following diagnosis. Methods All children diagnosed with IBD in 2015 in NZ were included. Clinical data obtained during routine care for 3 years following diagnosis were analyzed. Growth parameters, disease activity scores, and blood parameters were compared at diagnosis and follow up. Results Three‐year outcome data were available for 48 of 51 children. At follow up, median age was 15.1 years, and 34 had Crohn's disease (CD), 11 had ulcerative colitis (UC), and three had IBD‐unclassified (IBDU). Although disease progression including development of perianal disease occurred in 13 (38%) of 34 children with CD, the majority (n = 30) had inflammatory disease at follow up. Disease extension occurred in 25% (2/8) of children initially diagnosed with UC. Of all IBD patients, the mean body mass index z‐score increased from −0.40 to +0.10 (P = 0.01). Disease activity scores reduced from diagnosis to follow up in both CD (mean pediatric Crohn's disease activity index 35–6, P, This manuscript aimed, for the first time, to assess and describe the outcomes of a cohort of New Zealand children diagnosed with inflammatory bowel disease. Three years after diagnosis, this group of children was predominantly in remission with satisfactory improvements in growth parameters.

The COVID-19 crisis has revealed the limitations in the education system that could have been resolved by the Classroom 2.0 programme of blended and networked learning. This chapter presents the Emotivate Project that took place before this crisis and showed how it is possible to deliver cross-community education to provide evidence in support of the big society and free schools agenda. Cross-community education is called School 3.0, and lessons from this chapter could be learned so that it is possible to deliver education in such a way that pandemics do not bring the education system down in the way they have during COVID-19.

Project management is systematically conducted in order to achieve a deliverable outcome. In the case of information technology projects, project failure is very common – this is also the case with those IT projects that involve the development of e-learning systems, whether they involve minimal or intensive use of ICTs. The aim of this study, therefore, is to propose an approach to project management that involves the creation of a toolkit so that people without a background in project management, or e-learning, can more successfully run projects within their organisation. The toolkit enhances through approach to knowledge engineering by tailoring project management methodologies to an organisation and its partner organisations.

A chronic and gradual increase in pulse pressure (PP) is associated with cognitive decline and dementia in older individuals, but the mechanisms remain ill-defined. We hypothesized that a chronic elevation of PP would cause brain microvascular endothelial mechanical stress, damage the neurovascular unit, and ultimately induce cognitive impairment in mice, potentially contributing to the progression of vascular dementia and Alzheimer disease. To test our hypothesis, male control wild-type mice and Alzheimer disease model APP/PS1 (amyloid precursor protein/presenilin 1) mice were exposed to a transverse aortic constriction for 6 weeks, creating a PP overload in the right carotid (ipsilateral). We show that the transverse aortic constriction procedure associated with high PP induces a cascade of vascular damages in the ipsilateral parenchymal microcirculation: in wild-type mice, it impairs endothelial dilatory and blood brain barrier functions and causes microbleeds, a reduction in microvascular density, microvascular cell death by apoptosis, leading to severe hypoperfusion and parenchymal cell senescence. These damages were associated with brain inflammation and a significant reduction in learning and spatial memories. In APP/PS1 mice, that endogenously display severe cerebral vascular dysfunctions, microbleeds, parenchymal inflammation and cognitive dysfunction, transverse aortic constriction–induced high PP further aggravates cerebrovascular damage, Aβ (beta-amyloid) accumulation, and prevents learning. Our study, therefore, demonstrates that brain microvessels are vulnerable to a high PP and mechanical stress associated with transverse aortic constriction, promoting severe vascular dysfunction, disruption of the neurovascular unit, and cognitive decline. Hence, chronic elevated amplitude of the PP could contribute to the development and progression of vascular dementia including Alzheimer disease.

Cystic fibrosis (CF) is the most common, life-shortening, genetic illness affecting children in Australia and New Zealand. The genetic abnormality results in abnormal anion transport across the apical membrane of epithelial cells in a number of organs, including the lungs, gastrointestinal tract, liver and genito-urinary tract. Thus, CF is a multi-system disorder that requires a multi-disciplinary approach. Respiratory disease is the predominant cause of both morbidity and mortality in patients with CF. However, there are significant and clinically relevant gastrointestinal, liver, pancreatic and nutritional manifestations that must be detected and managed in a timely and structured manner. The aim of this review is to provide evidence-based information and clinical algorithms to guide the nutritional and gastrointestinal management of patients with CF.

The global incidence of paediatric inflammatory bowel disease (IBD) is increasing. Much of the evidence attesting to this has arisen from North America and Europe. There is a relative paucity of information on the epidemiology of paediatric IBD in the Southern Hemisphere. The present study aimed to document the prospectively collected incidence of paediatric IBD in New Zealand in 2015.All patients younger than 16 years of age and diagnosed with IBD in New Zealand between 1 January 2015 and 31 December 2015 were identified. Demographic and disease phenotypic details were collected and entered into a secure database. Age-specific population data for New Zealand were obtained and national incidence rates for IBD and its subtypes were calculated.The prospectively calculated incidence of paediatric IBD, Crohn disease, ulcerative colitis (UC), and IBD unclassified in New Zealand in 2015 were 5.2 (95% confidence interval 3.9-6.8), 3.5 (2.4-4.8), 1.0 (0.5-1.8), and 0.7 (0.3-1.4) per 100,000 children, respectively.Incidence rates of paediatric IBD in New Zealand are comparable to the highest rates published in the literature from Western Europe and North America. Ongoing prospective ascertainment of the incidence of paediatric IBD is required to better understand the environmental factors, which are accounting for this increase in disease burden.

Research on digital addiction has been increasing significantly since the start of the 2010s. What is not currently available is a measurement scale to assess the extent to which adolescents are at risk of abuse on the Internet that might lead them to develop digital addiction. This chapter sets out to develop a check-list that can be used to risk assess those youths who might be at risk of digital addiction. Through using data from a study into 1,828 young people aged 9-16, the study devised a 6-point check-list based on using a t-test to determine those at high risk and those at low risk. The check-list can be seen as a reliable way for screening those adolescents for whom concerns are raised over their online activities. The chapter concludes that further research will be needed to test the scale with people in older age ranges.

Sexual harassment has been a problem within organisations for some time. Its manifestation in electronic communication networks can be seen to amount to cyber-bullying or cyber-stalking. Through looking at records relating to an instance of sexual harassment at a higher education institution, including from that member of staff's workplace, and those created by referrals to the police, the court service, and their workplace, this chapter shows how a piece of assistive technology called the ‘Protective Technology for Ensuring Guardianship of Environmental Resources' (PROTEGER) can automatically detect sexual harassment narratives. In this context, ‘environmental resources' should refer to both humans and documents. Human resource managers would be better equipped to deal with disputes between staff if PROTEGER was running on their local area network as it might not be a matter of one member of staff's word against another's.

However, there is a scarcity of data on the epidemiology of pediatric IBD in the Southern Hemisphere. This study aimed to document the point prevalence of pediatric IBD in New Zealand on June 30, 2015. Methods All patients in New Zealand, under 16 years of age, with a diagnosis of IBD on June 30, 2015 were identified. Demographic and disease phenotypic details were collected and entered into a secure database. Age-specific population data for New Zealand were obtained and national and regional prevalence rates were calculated. Results The point prevalence of pediatric IBD, Crohn's disease, ulcerative colitis, and inflammatory bowel disease unclassified in New Zealand on June 30, 2015 was (95% confidence intervals) 21.7 (18.9-24.8), 16.5 (14.0-19.2), 3.3 (2.2-4.6), and 1.9 (1.2-3.0) per 100,000 children, respectively. There was a striking disparity between the prevalence rates in the North and South Islands. Conclusions The point prevalence of pediatric IBD in New Zealand represents the first-ever national, population-based prevalence rates of pediatric IBD published. Results from the Paediatric IBD in New Zealand (PINZ) study are also the first to show markedly higher prevalence rates of IBD in the southern part of a country compared with its northern counterpart. Ongoing prospective ascertainment of the incidence of pediatric IBD is required.

Background\ud Standardised mortality ratios do not provide accurate measures of preventable mortality. This has generated interest in using case-notes to assess the preventable component of mortality. But different methods of measurement have not been compared. We compared the reliability of two scales for assessing preventability and the correspondence between them.\ud Methods\ud Medical specialists reviewed case-notes of patients who had died in hospital using two instruments: a five-point Likert scale and a continuous (0-100) scale of preventability. To enhance generalisability we used two different hospital datasets with different types of acute medical patients across different epochs and in two jurisdictions (UK and US). We investigated the reliability of measurement and correspondence of preventability estimates across the two scales. Ordinal mixed effects regression methods were used to analyse the Likert scale and to calibrate it against the continuous scale. We report the estimates of the probability a death could have been prevented accounting for reviewer inconsistency.\ud Results \ud Correspondence between the two scales was strong; the Likert categories explained most of the variation (76% UK, 73% US) in the continuous scale. Measurement reliability was low but similar across the two instruments in each dataset (intra-class correlation 0.27 UK, 0.23 US). Adjusting for the inconsistency of reviewer judgements reduced the proportion of cases with high preventability, such that the proportion of all deaths judged probably or definitely preventable on the balance of probability was under 1%.\ud Conclusions \ud The correspondence is high between a Likert and continuous scale, although the low reliability of both would suggest careful measurement design would be needed to use either scale. Few to no cases are above the threshold when using a balance of probability approach to determining a preventable death, and in any case there is little evidence supporting anything more than an ordinal correspondence between these reviewer estimates of probability and the true probability. Thus, it would be more defensible to use them as an ordinal measure of the quality of care received by patients who died in the hospital.\ud

Proximal femoral fractures are a major public health concern with estimated annual direct and social costs amounting to £2 billion and average 30-day mortality risk of 7.5%. In response to the recent debate over out-of-hours hospital provision we investigated the ‘weekend effect’ at a major trauma centre, caring for acute injuries. A single centre, multi-surgeon review of 2060 patients performed. The distribution of patient and treatment variables compared in patients admitted on a weekday or the weekend. Fewer patients met performance indicators during weekend admission, time to surgery (63 vs. 71%) and time to geriatric review (86 vs. 91%). Weekend admission 30-day mortality was marginally lower than weekday (9.7% vs. 10.2%, OR 0.94, 95% CI 0.67 to 1.32, p = 0.7383). Increasing age, female gender, co-morbidities and confusion increased mortality risk. Binary regression analysis including these variables found no significant ‘weekend effect’. Despite the unit observing an increasing workload in the last five years, with meticulous workforce planning, senior doctor provisions and careful use of resources, it is possible to provide a seven-day fracture neck of femur service with no variation in thirty-day mortality by the day of admission.

Retaliatory feedback is a significant problem on the Internet, which is not just confined to online auction websites, but other online environments dependent on reputation systems. Explored in this paper are the acts of an Inter- net troller who spread malicious and false allegations that the series of confer- ences called WORLDCOMP are " fake. " This paper interviews one of the organisers of this conference to ask how they went about dealing with the retaliatory feed- back, and in particular their engagement with law enforcement agencies, such as the FBI, which now forms part of the US Department of Homeland Security. To reform the UK Home Office to learn lessons from this, the paper proposes making greater use of National Crime Agency and Police and Crime Commission- ers to provide a better strategic set-up for law enforcement under the UK Home Office. It also suggest using publicly funded solicitors and community wardens, as opposed to the current set-up of police constables, to deal with community policing. The paper concludes that in order for the promise made by the New Labour Government to be " tough on crime and tough on the causes of crime " to become a reality, encourages peaceful discourses between community members and former offenders is essential.

This is different from the original meaning online in the 1990s, which referred to the posting of provocative messages for humourous effect. Those systems operators (sysops) who run online communities are finding they are being targeted because of abuse posted on their platforms. Political discussion groups are some of the most prone to trolling, whether consensual or unwanted. Many such websites ara open for anyone to join, meaning when some members post messages they know are offensive but legal, others might find grossly offensive, meaning these messages could be illegal. This paper develops a questionnaire called the This Is Why We Can't Have Nice Things Scale (TIWWCHNT-20), which aims to help sysops better plan the development of online communities to take account of different users' capacity to be offended, and for users to self-assess whether they will be suited to an online community. The scale is discussed in relation to different Internet posting techniques where different users will act differently.

BackgroundIn children, paracetamol overdose due to deliberate self-poisoning, accidental exposure or medication errors can lead to paediatric acute liver failure and death. In Australia and New Zealand, the nature of ingestion and outcomes of paracetamol-associated paediatric acute liver failure have not been described.ObjectiveTo describe the nature and outcomes of paracetamol-associated paediatric acute liver failure.DesignRetrospective analysis of paracetamol-associated paediatric acute liver failure cases presenting 2002–2012.SettingNew Zealand and Queensland Paediatric Liver Transplant Services.Results14 of 54 cases of paediatric acute liver failure were attributed to paracetamol, the majority were secondary to medication errors. 12 of the 14 children were under the age of 5 years. Seven children received doses in excess of 120 mg/kg/day. Many of the other children received either a double dose, too frequent administration, coadministration of other medicines containing paracetamol or regular paracetamol for up to 24 days. Three children underwent transplant. One of these and one other child died.ConclusionsIn Australia and New Zealand, paracetamol overdose secondary to medication errors is the leading cause of paediatric acute liver failure. A review of regional safety practices surrounding paracetamol use in children is indicated.

A concern shared among nearly all generations of adults is that they must do something to tackle the problems in society caused by young people. They often forget that they were once young, and all too often blame young people for all of problems in their community. This paper challenges this view and shows how the blaming of Internet trolling on today's young people – called digital teens – is probably inaccurate. What might otherwise be called Troublesome Online Youth Groups (TOYGs), this paper looks at data collected from subjects in three UK regions (n=150 to 161), which includes young people who are not in education, employment or training (NEETs). Unlike might be typically thought, the data shows that far from these NEETs being the causes of Internet trolling it is in fact the areas with high levels of productivity, higher education and higher intelligence that report lower perceptions of quality of life that these electronic message faults (EMFts) most occur in.

The advancement of information and communications technology often results in early adoption, followed by concern over a digital divide, followed by mass adoption and then, inevitably, abuse and misuse of that platform. The most recent of these technologies is social networking services. The early adopters used Friendster and MySpace, and the masses now use Facebook and Twitter. The abuse of people on these platforms was called Cyberbullying in the case of the first two in the 2000s, and Internet trolling in the case of the second two in the 2010s. This paper reviews the legislation enacted in the UK parliament between 1981 and 2012 to deal with these offences, called ‘flame trolling’, for those based on transgress humour, or electronic message faults more generally. The paper presents a framework that includes a ‘Trolling Magnitude Scale’ based on established trolling culture, in order to link the legislative offences to the severities of those faults, as well as to the ability of specific Internet users to tolerate them or otherwise. The paper concludes that by using this framework law enforcement agencies such as the police can apply the laws more fairly and proportionally to protect free speech and at the same time be tough on the causes of electronic message faults in the form of Internet abuse and data misuse.

When one thinks of barriers to setting up a news corporation, one might think in terms of the costs of machinery and staffing. This case study of a start-up news corporation called Crocels News shows that the biggest cost can be in resolving legal disputes, most significantly from news articles scrutinising public bodies and their staff. This chapter investigates the difficulties faced by Crocels News in providing news content. By considering the legal correspondence received, the chapter provides insights into some of the problems all news services are likely to experience if they do not have access to the huge legal budgets of the established news corporations. The findings are particularly worrying for emerging forms of news reporting, such as citizen journalism. The chapter therefore proposes changes in statute so that case law that protects free speech is more easily enforced.

Educator–learner interaction has been a key factor in the advantage traditional learning environments have had over electronic ones. This chapter explores four projects that have challenged this premise. The Young Enterprise Project showed that computers can play an important part of youth entrepreneurship, and subsequently the Emotivate Project showed how they can be an essential part of blending electronic learning with community activism and arts. The Digital Classroom of Tomorrow Project showed that it is possible to use eTwinning of the schools through networked learning. Finally, the Free Digital Project is discussed, which is an initiative that brought all the other projects together to show how through the Cloud young and disabled people can become Internet entrepreneurs.

This paper presents an introduction to the use of affective computing in at-risk educational environments, such as those that schools located in areas where there is armed conflict and for the safeguarding of children and at-risk adults more generally. This paper has discussed the improvement of the EigenFace based facial emotion recognition by continually streamlining the facial dataset used and its application in at-risk educational environments. One of these mimics the authors' EigenFaces library and appears to have better performance in poor lighting and poor camera situations, making it possibly better for drone use. It is therefore paramount that as the authors develop the system further that they keep each component separate, in case it is decided to utilise commercial libraries (e.g. Microsoft Project Oxford) for certain aspects. A structure such as VoisOver that allows for third party technologies to be plugged in would mean the authors' code can remain separate from that of third party plug-ins, namely specific algorithms for identifying the core 12 emotion sets the authors have devised in contexts that might not even have been considered yet. Such algorithms could work with the system described in this paper to make its operation in at-risk educational environments even more possible.

It is unclear whether recent therapeutic advances have improved the growth of children with Crohn's disease (CD).To assess the frequency of short stature and poor growth and their relationship to disease course and therapy in children with CD. WHAT IS ALREADY KNOWN ON THIS TOPIC: Growth retardation may occur in children with Crohn's disease (CD). Current therapy for CD in the UK is less likely than previously to involve the use of long-term glucocorticoids.Despite advances in therapy, short stature and slow growth continue to be encountered in children with CD. There is a need for simple and consistent definitions of growth that can identify poor growth in children with chronic disease.The anthropometric and treatment details of 116 children (68 male) with a mean (range) age at diagnosis of 10.8 years (4.9-15.5) and a mean age at maximum follow-up (MF) of 15.4 years (9.4-19.3) were studied retrospectively at diagnosis (T0), at 1 (T1), 2 (T2) and 3 years (T3) after diagnosis and at MF.At T0, mean height SD score (HtSDS) was -0.5 (-3.3 to 2.6) compared to a mid-parental HtSDS of 0.2 (-2.0 to 01.4) (p=0.002). At T1, T2, T3 and MF, mean HtSDS was -0.6 (-4.8 to 7.8), -0.6 (-2.9 to 2.2), -0.7 (-3.6 to 2.5) and -0.5 (-3.5 to 2.9), respectively. Mean Ht velocity (HV) SDS at T1, T2, T3 and MF was -1.4 (-7.4 to 7.4), -0.6 (-7.5 to 6.1), -0.1 (-6.6 to 7.6) and 0.6 (-4.8 to 7.8), respectively (p0.05). In final models, HtSDS was associated negatively with the use of prednisolone (p=0.0001), azathioprine (p=0.0001), methotrexate (p=0.0001) and weight SDS (WtSDS) (p=0.0001). HVSDS was associated positively with age (p=0.0001) and WtSDS (p=0.01). ΔHtSDS was associated negatively with use of prednisolone (p0.02).Although current therapy for CD is associated with improved rate of growth for the first few years, a substantial proportion of children remain short. This study also highlights the need for consistency in describing growth in children with chronic diseases.

New Labour was a description of a particular approach to government of the British Labour Party, which was in power in the United Kingdom between 1997 and 2010. While this government initially envisaged an end to the social causes of misdemeanours, its actions led to a greater number of laws on the statute books creating thousands of statutory offences. A small number of these had direct effects on the number of computer related offences that were able to be prosecuted. This paper reviews these laws, and the role of legal systems in responding to the increasing number of misdemeanours that are occurring in computer environments for which New Labour's approach of creating more statutory offences has not addressed.

We report the first two pediatric patients with CF who underwent successful combined liver-pancreas transplantation in Australia and New Zealand for CF liver disease and CF-related diabetes mellitus.

Hyperammonemia is a rare and important complication post-liver transplantation. We review a case of a 5-month-old boy with biliary atresia who received a split liver transplant following a variceal bleed. The transplant was complicated by recurrent portal vein thrombosis. Colonized with Serratia marcescens pretransplant, he developed disseminated infection associated with very high levels of ammonia that led to his death. It is important to be aware of serum ammonia levels in patients with portal vein thrombosis, particularly in the setting of gastrointestinal bleeding and sepsis.

This chapter looks at how suitable the current equality policies of Wales's universities are to compete in the current economic climate and the changes needed to deliver best value to people with disabilities and all other taxpayers. The chapter makes the finding that universities are too bloated, by carrying out functions, which in Wales could be better handled by the public sector that is under direct control of the Welsh Government's education minister. This would involve learning from how the telecoms and energy companies work UK wide, so that HEFCfW becomes an infrastructure provider, Estyn would become responsible for ensuring the equality of access to higher education and ensuring the standards of university education. Universities would thus consist mainly of teaching and research staff, optimising how they use the infrastructure to attract the most students to their degrees, which are homogenised. The chapter makes clear, however, that whilst this policy would likely work in Wales, it would be unlikely to in England, perhaps allowing “clear red water” between governments.

Designing an effective organisational architecture for an undertaking can be considered essential to its success. The way an organisation is designed – or otherwise appears to its workers – will affect the extent to which those workers associated with it can be effective at their jobs. This chapter undertakes a case study into an organisation that is based around contingent working and inter-professionalism. Important things drawn from the study include the importance of the Cloud to distance working, such as teleworking; the identity of the organisation and how workers relate to it; as well as what factors assist on inhibit worker motivation. The study concludes that the organisational structure of the organisation investigated – where different firms perform different tasks, could be seen as best practice in supporting inter-professional environments.

The current phenomenon of Big Data – the use of datasets that are too big for traditional business analysis tools used in industry – is driving a shift in how social and economic problems are understood and analysed. This chapter explores the role Big Data can play in analysing the effectiveness of crowd-funding projects, using the data from such a project, which aimed to fund the development of a software plug-in called ‘QPress'. Data analysed included the website metrics of impressions, clicks and average position, which were found to be significantly connected with geographical factors using an ANOVA. These were combined with other country data to perform t-tests in order to form a geo-demographic understanding of those who are displayed advertisements inviting participation in crowd-funding. The chapter concludes that there are a number of interacting variables and that for Big Data studies to be effective, their amalgamation with other data sources, including linked data, is essential to providing an overall picture of the social phenomenon being studied.

Gastroesophageal reflux occurs in the majority of infants, with severity ranging from asymptomatic to severe esophagitis and failure to thrive. Omeprazole is recognized as a safe and effective treatment of gastroesophageal reflux in older children, at an initial dosage of 0.7 mg x kg(-1) x day(-1). To our knowledge, no dose-finding studies have been carried out in children under 2 years of age. The aim of the present study was to prospectively determine the dosage of omeprazole required to treat symptomatic gastroesophageal reflux in children younger than 2 years.Children under 2 years with clinical suspicion of gastroesophageal reflux underwent 24-hour dual-channel intraesophageal/gastric pH monitoring. A reflux index above 10% in children under 1 year and above 6% in children older than 1 year was deemed significant. Treatment with omeprazole at an initial dosage of 0.7 mg x kg(-1) x day(-1) (in 2 divided doses) was followed by dual-channel pH study after 14 days. The dosage was increased in increments of 0.7 mg x kg(-1) x day(-1), and pH studies were repeated until the gastroesophageal reflux was controlled.Ten children (5 male, 5 female), mean age 7.75 months (range, 1.25-20 months), were investigated. The initial median reflux index was 18.5% (range, 6.5%-56.3%). Follow-up median reflux index was improved at 1.6% (0.1%-8.1%) (P0.05). The median dosage required was 1.05 mg x kg(-1) x day(-1). Four children required 1.4 mg x kg(-1) x day(-1), and 1 required 2.8 mg x kg(-1) x day(-1). Corrected reflux index improved from 34.8% (16.8%-90.8%) to 20.1% (0.4%-100%) but did not achieve statistical significance. There were no serious complications or side effects.Omeprazole is an effective treatment for gastroesophageal reflux in children younger than 2 years. The majority respond to a dosage of 0.7 mg x kg(-1) x day(-1), but increased dosages up to 2.8 mg x kg(-1) x day(-1) may be required.

Online communities are becoming an accepted part of the lives of Internet users, although participation in these communities is dependent on the types of people that form them. Some of the online community's members do not participate, people referred to as lurkers, whereas others who have been in the community for a long time, referred to as elders, participate regularly and support others. Understanding what drives these individuals and how they chose whether or not to participate will lead to online communities that thrive. This paper proposes a conceptual framework to describe what drives such individuals to carry out actions such as posting messages and adding content (level 1), the cognitions they use to determine whether or not to take such actions (level 2) and the means by which they go about carrying out the action in the environment (level 3). Finally, the framework is applied to the problem of encouraging members to participate by discussing the methods by which people can be persuaded to participate by changing the way they interpret their desires and their environment.

The small bowel is anatomically difficult to examine and was investigated by invasive, indirect modalities, such as push enteroscopy and small bowel follow-through. The aim of this study was to assess the efficacy and clinical impact of wireless capsule endoscopy (WCE) in children.Over the last 3 years (2002-2005), 37 patients with suspected small-bowel disease were investigated with WCE at a median age of 11 years (range, 16 months-16 years). The indications for WCE was suspected Crohn's disease (CD) (18), obscure or occult gastrointestinal bleeding (7), polyposis syndromes (5), protein losing enteropathy (4), recurrent abdominal pain (2), and malabsorption syndrome (1). All patients had preceding upper gastrointestinal endoscopy (OGD), ileocolonoscopy, and 26 cases had a small bowel follow-through (SBFT). These results were compared with the findings on WCE.Thirty-three (33) cases successfully completed the WCE through the small bowel. Four (4) patients were unable to swallow the capsule, 3 of which had to be placed in the duodenum endoscopically. In 3 patients, the capsule remained in the stomach and no small bowel images were obtained. The overall diagnostic yield was 85% (28/33 patients). The diagnostic findings included CD (13), source of gastrointestinal bleeding (7), polyposis syndromes (3), erosive enteropathy and patchy lymphangiectasia (4), and intussusception (1). WCE was found to be more sensitive for small bowel pathology than SBFT (20 vs. 6 [30% sensitivity, compared to WCE]) and endoscopic investigations (28 vs. 12 [43% sensitivity compared to WCE]). As a result of WCE findings, there was a positive alteration in the management in 28 of 33 (85%) cases.WCE is a novel, noninvasive, and useful tool for the investigation of the small intestine in children. It is superior and more sensitive than other conventional endoscopic and radiologic investigations in the assessment of the small bowel. It can help in guiding surgical decisions and should be routinely integrated as a part of the diagnostic work-up of small bowel pathology.

With the introduction of mouse models for the study of cardiac morphogenesis, there arises a need for new imaging protocols that can capture both morphological and functional information. High-resolution 2D cardiac cine MRI has often been used to quantify left and right ventricular function. In this study we propose a 3D isotropic cardiac cine MRI protocol with a voxel size of 200 microm(3) as a means of studying cardiac multi-chamber morphology and function. A black blood sequence was used to enhance blood myocardium contrast. Manual segmentation of the ventricles was used to measure ventricular volumes at end diastole and end systole. This method is demonstrated on an Irx4-deficient mouse model. We have been able to identify the volumes of both ventricles dynamically and to show differences in ejection fraction in the mutant. We have also identified an abnormality of the papillary muscle in the mutant that had been missed in previous phenotyping with ultrasound and histology.

Cardiac MR imaging in small animals presents some difficulties due to shorter cardiac cycles and smaller dimensions than in human beings, but prospectively gated techniques have been successfully applied. As with human imaging, there may be certain applications in animal imaging for which retrospective gating is preferable to prospective gating. For example, cardiac imaging in multiple mice simultaneously is one such application. In this work we investigate the use of retrospective gating for cardiac imaging in a mouse. Using a three-dimensional imaging protocol, we show that image quality with retrospective gating is comparable to prospectively gated imaging. We conclude that retrospective gating is applicable for small animal cardiac MRI and show how it can be applied to the problem of cardiac MRI in multiple mice.

Tbx5 del/+ mice provide a model of human Holt-Oram syndrome. In this study, the cardiac functional phenotypes of this mouse model were investigated with 30-MHz ultrasound by comparing 12 Tbx5 del/+ mice with 12 wild-type littermates at 1, 2, 4, and 8 wk of age. Cardiac dimensions were measured with two-dimensional and M-mode imaging. The flow patterns in the left and right ventricular inflow channels were evaluated with Doppler flow sampling. Compared with wild-type littermates, Tbx5 del/+ mice showed significant changes in the mitral flow pattern, including decreased peak velocity of the left ventricular (LV) early filling wave (E wave), increased peak velocity of the late filling wave (A wave), and decreased or even reversed peak E-to-A ratio. The prolongation of LV isovolumic relaxation time was detected in Tbx5 del/+ neonates as early as 1 wk of age. In Tbx5 del/+ mice, LV wall thickness appeared normal but LV chamber dimension was significantly reduced. LV systolic function did not differ from that in wild-type littermates. In contrast, the Doppler flow spectrum in the enlarged tricuspid orifice of Tbx5 del/+ mice demonstrated increased peak velocities of both E and A waves and increased total time-velocity integral but unchanged peak E/A. In another 13 mice (7 Tbx5 del/+, 6 wild-type) at 2 wk of age, significant correlation was found between Tbx5 gene expression level in ventricular myocardium and LV filling parameters. In conclusion, the LV diastolic function of Tbx5 del/+ mice is significantly deteriorated, whereas the systolic function remains normal.

In order to realize the full potential in an MR receiver, the digitizer must capture a signal magnitude range from the central k-space peak to the thermal noise floor of the system. This dynamic range can exceed the performance of standard 16-bit data converters. For example, a whole-body mouse scan in a 7 Tesla magnet requires 20 bits of dynamic range. A 3D high-resolution mouse scan at 75 μm isotropic voxel resolution using a 16-bit spectrometer shows an eightfold improvement in image SNR by gain stepping the receiver prior to digitization to cover the full magnitude dynamic range compared to a standard fixed gain approach. Social impairments materialise in a number of forms, from developmental disabilities such as autistic spectrum disorder, to psychiatric conditions such as social phobia. The individuals diagnosed with these problems find it difficult to deal with social situations through either the inability to perform in these situations or the fear of not being able to do so. The study investigated the social and practical implications of using Mobile Internet technology to deliver information relating to a social situation in real-time to participants with Autistic Spectrum Disorders (n = 10) and General Social Phobia (n = 3) diagnosed using DSM-IV. The participants used the agent on their mobile phone to convert phrases they found offensive or confusing into more concise and understandable definitions. Analysing their attitudes revealed that the technology enables socially impaired individuals to learn the meaning of emotions and understand more about how they communicate with their peers. However, the study concludes that governmental organisations, education providers and society as a whole need to adopt a cohesive approach to communication to ensure socially impaired individuals are fully included in society

The quantitative-qualitative and subjectivity-objectivity debates plague research methods textbooks, divide academic departments, and confuse post-modernists as to their existence. Those from the objective-quantitative camps will usually demand methods assume parametric principles from the start, such as homogeneity and normal distribution. Many of the subjective-qualitative camps will insist on looking and the individual meanings behind what someone is saying through their narratives and other discourses. The objective-quantitative camps on the other hand think anything that does not involve systematic acquisition and analysis or data cannot be valid. This chapter presents an approach to derive a parametric user model for understanding users that makes use of the premises and ideals of both these camps.

This chapter explores how a learning organisation differs from a teaching organisation, such as that each person holds responsibility for their own learning, yet are supported and guided by those who wish to help them further their personal development. This chapter aims to develop a software project management methodology, based on existing approaches, which can accommodate all people, regardless of ability. The model developed, called the C2-Tech-S2 approach, is specifically designed for projects that use crowd-funding and agile development, particularly in environments based around the Cloud. A pilot study is carried out to demonstrate the ‘technology' stage of this model for assessment using the ‘support' stage. This finds that all stages of the model need to be applied in a project, because on their own the stages may not produce the most effective outcomes in terms of increased participation.

This chapter using an empirical data-driven approach to investigate crime recording logs of South Wales Police relating to Internet trolling byand towards different sexes. The chapter finds more favourable attitudes towards women as victims in even the most trivial of cases. It finds male victims of trolling are only treated as victims when a form of unwanted face-to-face encounter is needed for action. The chapter shows transferring police powers to local authorities, can cut the cost of community policing by 50% across the board and eliminate sexist attitudes also. The chapter finds that the way social media platforms are exercising ‘sysop prerogative' where they have no right to – such as not passing over account information on alleged defamers – puts a huge burden on police resources. Using local authorities, which have many of the same powers as the police and indeed more, can resolve problems without the need to criminalise offenders.

This chapter seeks to explore the role media content ratings play in the age of “Internet trolling” and other electronic media issues like “sexting.” Using ANOVA to validate a four-factor approach to media ratings based on maturity, the chapter finds the ability of a person to withstand various media content, measured in “knol,” which is the brain's capacity to process information, can be used to calculate media ratings. The study concludes it is feasible to have brain-computer interfaces for PCs and kiosks to test the maturity of vulnerable persons and recommend to parents/guardians or cinema managers whether or not to allow someone access to the content they wish to consume. This could mean that computer software could be programmed to automatically censor content that person is likely to be distressed or grossly offended by. Public policy issues relating to these supply-side interventions are discussed.

Unmanned aerial vehicles (UAVs), commonly known as drones, are a robotic form of military aircraft that are remotely operated by humans. Due to lack of situation awareness, such technology has led to the deaths of civilians through the inaccurate targeting of missile or gun attacks. This chapter presents the case for how a patented invention can be used to reduce civilian casualties through attaching an affect recognition sensor to a UAV that uses a database of strategies, tactics and commands to better instruct fighter pilots on how to respond while in combat so as to avoid misinterpreting civilians as combatants. The chapter discusses how this system, called VoisJet, can reduce many of the difficulties that come about for UAV pilots, including reducing cognitive load and opportunity for missing data. The chapter concludes that using UAVs fitted with VoisJet could allow for the reduction of the size of standing armies so that defence budgets are not overstretched outside of peacetime.

Computer jargon is something that can either unite people, or draw them apart. This chapter looks at definitions of the terms, ‘trolling,' ‘flame,' ‘flame-war' and ‘lurking,' as presented in specialist dictionaries, newspapers and through a survey of laypersons. The aim of the chapter was to see whether it was possible to objectively define terms using a quantitative analysis of qualitative data. The study finds that objectively determining a definition of a term requires a bigger dataset than is used for qualitative studies. It further notes that whilst there is a lot in common with expert definitions, the problem with drawing definitions from others is that whilst it might produce objective definitions they might not be accurate ones.

Since time immemorial, the legal systems of Great Britain have often been spoken of highly as pinnacles of democracy. However, the split between criminal law and tort law have often caused problems where the police has often focused on the prosecution of people in poverty and where only the wealthy can afford to use the system. This chapter discusses the extent and limitations of existing measures to tackle computer-related crime, particularly with regards to the abusive kind of Internet Trolling, namely “flame trolling.” The chapter recommends further research to establish whether it should be the case that in a society based on dualism that criminal and civil cases should be held at the same time, and that in both instances those being accused of an offence or tort should be allowed to bring a counter-claim. It is discussed that in such a system the cases that would be brought are where there is a clear victim who had no part in the offence against them, such as murder, rape, theft and burglary, which are usually carefully planned and orchestrated acts.

In quasi-static magnetic resonance elastography, strain images of a tissue or material undergoing deformation are produced. In this paper, the signal/noise (S/N) ratio [SNR] of elastographic strain images, as measured by a phase-contrast technique, is analyzed. Experiments are conducted to illustrate how diffusion-mediated signal attenuation limits maximum strain SNR in small displacement cases, while the imaging point-spread function limits large displacement cases. A simple theoretical treatment agrees well with experiments and shows how an optimal displacement encoding moment can be predicted for a given experimental set of parameters to achieve a maximum strain SNR. A further experiment demonstrates how the limitation on strain SNR posed by the imaging point-spread function may potentially be overcome.

C | Abstract: In 1997 I was asked to organize humanities outreach activities at the University of California, Irvine. The result was the formation of Humanities Out There (HOT). In our workshops, faculty members and graduate students supervise teams of undergraduates in order to take the methods and materials of the university into the larger community.I believe that programs like these will become increasingly important in the next century, as economic, cultural, and educational divisions deepen in the wake of the demise of affirmative action and as the humanities fight to define their missions in a world driven by technology and its discontents. In this brave new world, what I call the new outreach may have a role to play in responding to social crises as they are visited on the life of the university. The new outreach will be driven by intellectual content, not public relations.It will take its orientation from the faculty rather than administrators. It will engage all the research disciplines rather than remain the purview of education departments. It will be integrated into the professional lives of its participants rather than rely on the spirit of volunteerism alone.

This chapter presents a location-based affective computing system, which can assist growing emerging markets by helping them reduce crime and increase public safety when used in conjunction with CCTV. Internet systems based on location-based services have increased in availability. Social platforms such as Twitter and Facebook now employ the information on user locations to provide context to their posts, and services such as Foursquare rely on people checking into different places, often to compete with their friends and others. Location-based information, when combined with other records, such as CCTV, promotes the opportunity for a better society. People normally abused by corrupt state officials for crimes they did not commit will now have alibis, shops will be able to more effectively build trust and procure new customers through “social proof,” and other forms of corruption will be tackled such as benefit fraud and tax evasion. Trust that everyone is paying his or her fair share can develop.

While the concept of Classroom 2.0 has been around for over a decade, the concept of electronic and distance learning as a mode to improve education outcomes has existed ever since the first broadcast of television programs carrying educational content. The governments in the United Kingdom have always sought to intervene in education, whether this has been allowing schools to opt-out of local authority control with grant-maintained schools under Margaret Thatcher, co-operative trusts under Tony Blair, or free schools under David Cameron. Not all government interventions are as successful. Homogenized one-size-fits-all education based on catchment areas such as Comprehensive Schools and state-run projects like the UK e-University have been shown to lack the return on investment of Specialist and Independent Schools and the Open University. This chapter reviews some of the microeconomic models used by governments to intervene in the market for instructional services, including e-participation in education, namely Classroom 2.0. It also looks at some of the possibilities of Classroom 2.0 in education systems that have been affected by UK and respective devolved government's education policy.

Wales is the “land of the poets so soothing to me,” according to its national anthem. The political and economic landscape does not on the whole provide for the many creative people that are in Welsh communities. Social media Websites like MySpace and YouTube as well as Websites like MTV.com, eJay, and PeopleSound, whilst providing space for artists to share their works, but do not usually consider the needs of local markets, such as in relation to Welsh language provision through to acknowledgement of Welsh place names and Wales's status as a country. The chapter finds that there are distinct issues in relation to presenting information via the Web- or Tablet-based devises and suggests some of the considerations needed when designing multi-platform environments.
