People with type 2 diabetes mellitus experience an increased prevalence of non-alcoholic fatty liver disease (NAFLD) compared with the general population and often with worse outcomes. As part of the ABCD Liraglutide Nationwide Audit Programme, we obtained and analysed data from 2009 to 2018 to assess the impact of liraglutide on alanine aminotransferase (ALT) levels as a marker of liver inflammation (often used in clinical trials as a marker of NAFLD). After excluding those with insufficient or incomplete data, we analysed the results from 1,759 patients treated in the real-world clinical setting. Rationale: Poor adherence is common in difficult-to-control asthma. Distinguishing patients with difficult-to-control asthma who respond to inhaled corticosteroids (ICS) from refractory asthma is an important clinical challenge. Objectives: Suppression of fractional exhaled nitric oxide (FE NO ) with directly observed ICS therapy over 7 days can identify nonadherence to ICS treatment in difficult-to-control asthma. We examined the feasibility and utility of FE NO suppression testing in routine clinical care within UK severe asthma centers using remote monitoring technologies. Methods: A web-based interface with integrated remote monitoring technology was developed to deliver FE NO suppression testing. We examined the utility of FE NO suppression testing to demonstrate ICS responsiveness and clinical benefit on electronically monitored treatment with standard high-dose ICS and long-acting b 2 -agonist treatment. Measurements and Main Results: Clinical response was assessed using the Asthma Control Questionnaire-5, spirometry, and biomarker measurements (FE NO and peripheral blood eosinophil count). Of 250 subjects, 201 completed the test with 130 positive suppression tests. Compared with a negative suppression test, a positive test identified a FE NO -low population when adherent with ICS/long-acting b 2 -agonist (median, 26 ppb [interquartile range, 16–36 ppb] vs. 43 ppb [interquartile range, 38–73 ppb]) with significantly greater FEV 1 % (mean, 88.2 6 16.4 vs. 74.1 6 20.9; P, 0.01). Asthma Control Questionnaire-5 improved significantly in both groups (positive test: mean difference, 21.2; 95% confidence interval, 20.9 to 21.5; negative test: mean difference, 20.9; 95% confidence interval, 20.4 to 21.3). Conclusions: Remote FE NO suppression testing is an effective means of identifying nonadherence to ICS in subjects with difficult-to-control asthma and the substantial population of subjects who derive important clinical benefits from optimized ICS/long-acting b 2 -agonist treatment.

Ischemic colitis and proctitis is a rare manifestation of systemic lupus erythematosus (SLE) and results from mesenteric vasculitis. Owing to diverse blood supply and presence of multiple collaterals, rectum is the least effected site in SLE enteritis. Ischemic proctocolitis as the presenting feature of SLE is exceedingly rare, with only three cases reported in the published scientific literature. We present the first case of SLE presenting as ischemic proctitis, leading to intraperitoneal hemorrhage and abdominal compartment syndrome. A young lady presented with ischemic proctitis and a hematoma masquerading as a pelvic mass, with subsequent development of massive intraperitoneal hemorrhage, shock, and rectal perforation. The patient required urgent surgery and was initiated on high-dose steroids.

The 13th GCC Closed Forum for Bioanalysis was held in New Orleans, Louisiana, USA on April 5th, 2019. This GCC meeting was organized to discuss the contents of the 2019 ICH M10 Bioanalytical Method Validation Draft Guideline published in February 2019 and consolidate the feedback of the GCC members. In attendance were 63 senior-level participants from eight countries representing 44 bioanalytical CRO companies/sites. This event represented a unique opportunity for CRO bioanalytical experts to share their opinions and concerns regarding the ICH M10 Bioanalytical Method Validation Draft Guideline and to build unified comments to be provided to the ICH.

Representatives from international bioanalytical Contract Research Organizations were in attendance in order to discuss scientific and regulatory issues specific to bioanalysis. The issues discussed at the meeting included: critical reagents; oligonucleotides; certificates of analysis; method transfer; high resolution mass spectrometry; flow cytometry; recent regulatory findings and case studies involving stability and nonclinical immunogenicity. Conclusions and consensus from discussions of these topics are included in this article.

Over the last decade, the use of biomarker data has become integral to drug development. Biomarkers are not only utilized for internal decision-making by sponsors; they are increasingly utilized to make critical decisions for drug safety and efficacy. As the regulatory agencies are routinely making decisions based on biomarker data, there has been significant scrutiny on the validation of biomarker methods. Contract research organizations regularly use commercially available immunoassay kits to validate biomarker methods. However, adaptation of such kits in a regulated environment presents significant challenges and was one of the key topics discussed during the 12th Global Contract Research Organization Council for Bioanalysis (GCC) meeting. This White Paper reports the GCC members’ opinion on the challenges facing the industry and the GCC recommendations on the classification of commercial kits that can be a win-win for commercial kit vendors and end users.

Digital, Background: Two glucagon-like peptide-1 (GLP-1) receptor agonists reduced renal outcomes in people with type 2 diabetes at risk for cardiovascular disease. We assessed the long-term effect of the GLP-1 receptor agonist dulaglutide on renal outcomes in an exploratory analysis of the REWIND trial of the effect of dulaglutide on cardiovascular disease. Methods: REWIND was a multicentre, randomised, double-blind, placebo-controlled trial at 371 sites in 24 countries. Men and women aged at least 50 years with type 2 diabetes who had either a previous cardiovascular event or cardiovascular risk factors were randomly assigned (1:1) to either weekly subcutaneous injection of dulaglutide (1·5 mg) or placebo and followed up at least every 6 months for outcomes. Urinary albumin-to-creatinine ratios (UACRs) and estimated glomerular filtration rates (eGFRs) were estimated from urine and serum values measured in local laboratories every 12 months. The primary outcome (first occurrence of the composite endpoint of non-fatal myocardial infarction, non-fatal stroke, or death from cardiovascular causes), secondary outcomes (including a composite microvascular outcome), and safety outcomes of this trial have been reported elsewhere. In this exploratory analysis, we investigate the renal component of the composite microvascular outcome, defined as the first occurrence of new macroalbuminuria (UACR >33·9 mg/mmol), a sustained decline in eGFR of 30% or more from baseline, or chronic renal replacement therapy. Analyses were by intention to treat. This trial is registered with ClinicalTrials.gov, number NCT01394952. Findings: Between Aug 18, 2011, and Aug 14, 2013, 9901 participants were enrolled and randomly assigned to receive dulaglutide (n=4949) or placebo (n=4952). At baseline, 791 (7·9%) had macroalbuminuria and mean eGFR was 76·9 mL/min per 1·73 m2 (SD 22·7). During a median follow-up of 5·4 years (IQR 5·1-5·9) comprising 51 820 person-years, the renal outcome developed in 848 (17·1%) participants at an incidence rate of 3·5 per 100 person-years in the dulaglutide group and in 970 (19·6%) participants at an incidence rate of 4·1 per 100 person-years in the placebo group (hazard ratio [HR] 0·85, 95% CI 0·77-0·93; p=0·0004). Digital, Background Three different glucagon-like peptide-1 (GLP-1) receptor agonists reduce cardiovascular outcomes in people with type 2 diabetes at high cardiovascular risk with high glycated haemoglobin A1c (HbA1c) concentrations. We assessed the effect of the GLP-1 receptor agonist dulaglutide on major adverse cardiovascular events when added to the existing antihyperglycaemic regimens of individuals with type 2 diabetes with and without previous cardiovascular disease and a wide range of glycaemic control. Methods This multicentre, randomised, double-blind, placebo-controlled trial was done at 371 sites in 24 countries. Men and women aged at least 50 years with type 2 diabetes who had either a previous cardiovascular event or cardiovascular risk factors were randomly assigned (1:1) to either weekly subcutaneous injection of dulaglutide (1·5 mg) or placebo. Randomisation was done by a computer-generated random code with stratification by site. All investigators and participants were masked to treatment assignment. Participants were followed up at least every 6 months for incident cardiovascular and other serious clinical outcomes. The primary outcome was the first occurrence of the composite endpoint of non-fatal myocardial infarction, non-fatal stroke, or death from cardiovascular causes (including unknown causes), which was assessed in the intention-to-treat population. This study is registered with ClinicalTrials.gov, number NCT01394952. Findings Between Aug 18, 2011, and Aug 14, 2013, 9901 participants (mean age 66·2 years [SD 6·5], median HbA1c 7·2% [IQR 6·6–8·1], 4589 [46·3%] women) were enrolled and randomly assigned to receive dulaglutide (n=4949) or placebo (n=4952). During a median follow-up of 5·4 years (IQR 5·1–5·9), the primary composite outcome occurred in 594 (12·0%) participants at an incidence rate of 2·4 per 100 person-years in the dulaglutide group and in 663 (13·4%) participants at an incidence rate of 2·7 per 100 person-years in the placebo group (hazard ratio [HR] 0·88, 95% CI 0·79–0·99; p=0·026). Device failure analysis typically requires multiple systems for fault identification, preparation and analysis. In this paper we discuss the practicalities and limits of using a single FIBSEM system for a complete failure analysis workflow. The theoretical requirements of using a nanomanipulator for both lamella lift out and electrical testing are discussed and the current capabilities of windowless X-rays detectors for chemical analysis demonstrated. When the required resolution for failure analysis exceed the limits of a FIBSEM and TEM is required, the combination of the nanomanipulator and X-ray detector for advanced lift out and thickness controlled thinning techniques are demonstrated to prepare exceptional quality lamellae.

The Underwater Radiation Spectral Identification System (URSIS) is a portable spectrometer used for the in situ detection of radioactivity in the marine environment. This paper reports on the first time application of this technology to assess, in a preliminary manner, the potential radiation threat to the public and environment at an aquatic disposal site – the Massachusetts Bay Industrial Waste Site (IWS). Utilizing the meneuvering capabilities of ROV and manned submersible vehicles, the URSIS was successfully positioned close (5–10 cm) to waste containers for a period sufficient to detect, in real time, the presence of radioactive materials. Spectral data from 45 individual targets indicated that the radionuclides present in sediments which draped or partially buried waste containers were consistent with natural background concentrations. No man-made radionuclides were detected at any of the target or background measurement locations. These data support the conclusion that low-level radiation does not pose an imminent and widespread human health or ecological threat in Massachusetts Bay.

BACKGROUND: Studies regarding the immuno-histological expression and relevance of Beta-Human Chorionic Gonadotrophin (beta-HCG) in urothelial carcinoma are few.. There is also no clear cut way of predicting exactly which superficial urothelial carcinomas would subsequently recur or progress and which muscle-invasive urothelial tumours would progress. The objective of the study was to study the immunohistological expression of beta-HCG in urothelial carcinoma with regards to grade, category and outcome following treatment. METHODS: The expression of beta-HCG in urothelial carcinomas of 86 patients was studied with regards to grade, stage and outcome using an immunohistological (ABC) method and formalin fixed/paraffin embedded tumours. RESULTS: Of the 86 tumours (55 superficial and 31 muscle-invasive) studied 45, 16 and 26 were graded as G1, G2, and G3 respectively. Thirteen of the 55 superficial tumours were positively stained for beta-HCG and 42 negatively stained. Twenty of the 31 muscle-invasive tumours studied were positively stained for beta-HCG and 11 were negative. Of the 13 beta-HCG positive superficial tumours only one did not recur at follow up and 12 subsequently recurred, of the 42 beta-HCG negative superficial tumours 19 did not recur and 23 recurred. Only one of twenty patients with beta-HCG positive muscle-invasive tumours survived; 6 of 11 patients with beta-HCG negative muscle-invasive tumours survived. The results indicate that positive staining of the tumours was more commonly associated with tumours of higher grade, higher stage and inferior outcome. CONCLUSION: The Immunohistological expression of beta-HCG would likely predict superficial tumours that would recur and muscle-invasive tumours with inferior outcome

The incidence of human tuberculosis (TB) is increasing worldwide, mainly because of the spread of human immunodeficiency virus (HIV)/acquired immune deficiency syndrome (AIDS) and the emergence of drug resistance. Pulmonary TB occurs more commonly than genitourinary TB (GUTB). Symptoms of GUTB are vague and are frequently go undetected. This review serves as a reminder that GUTB exists and, if undiagnosed, may cause unnecessary morbidity. GUTB occurs in 4–20% of patients with pulmonary TB. Onset of GUTB is insidious, and with pyuria and microscopic haematuria in 90% of patients. Diagnosis is achieved by the demonstration of tubercle bacilli in urine. Imaging features include calcifications, calyceal distortion and infundibular and ureteral strictures. Early changes include erosion and blunting of the calyces, narrowing of collecting system infundibula, overt papillary necrosis and parenchymal scarring/calcification. Prostatic involvement is nodular and non-tender. Genital TB presents with superficial ulcers in both males and females. Obstruction of the vas deferens causes infertility. Tuberculous orchitis may mimic other testicular mass lesions. Treatment is with antituberculous therapy. Early endourological decompression of ureteric strictures improves the salvage rate of the kidney.

Leprosy, or Hansen ’ s disease, is one of the world ’ s oldest infectious diseases. It affects the skin and nerves and, if left untreated, leads to deformity. Every 2 minutes someone is diagnosed with leprosy, but, because of the lack of education and the stigma surrounding the disease, some people are diagnosed too late and develop life-changing disabilities. Through a simple course of medication, leprosy is an entirely curable disease. Literature, such as reported cases and review papers, on leprosy in pregnancy was obtained using various internet search engines. Leprosy in pregnancy is a rarely reported event, but nine countries in Africa, Asia and Latin America have considered it a public health problem. An effective cure for leprosy is available in the form of multidrug therapy, taken for some months, but if treatment is delayed until a later stage, there is a high risk of disability. Pregnancy is hazardous for women with leprosy. It is most dangerous during the third trimester when infection can lead to adverse obstetric and fetal outcomes, such as low birthweight, prematurity, exfoliative dermatitis in the newborn and erythema nodosum in the pregnant mother. Documentation on leprosy in pregnancy indicates that pregnancy is not only a trigger for leprosy but also an ideal in vivo model for research. Leprosy in pregnancy can be treated safely and effectively by combined drug therapy. However, early detection, well-planned health education for leprosy patients and the highest standard of clinical supervision during pregnancy are key strategies in reducing problems associated with the disease and are also the best way to prevent disability.

There are three really fundamental principles of speaking after dinner. The first is to be humorous, the second is to be brief, I am not going to do that, and the third is not to say anything which might offend your audience. So I thought it would be suitable to start off by introducing you to a tabloid journal that you might not all come across, called Capital Gay. Because in the last year, something very interesting has happened on pages 18 and 19. These are adverts for telephone dating services. One of them according to a very small study has generated one and a half million telephone calls since it started. Their capital costs consist of putting the advert in Capital Gay. There is a difference between peak time charges of 38p a minute and off peak charges of 25p a minute. The only difference, I can imagine, is that the 13p in those differences has gone to British Telecom. Those of you who are nasty and competitive will be pleased to know that not a single one of them uses ‘you know who's’ telephone lines. The only other thing they have to do, is employ somebody called Arthur, who doesn't appear to be a computer, but if you use a few words which he can recognise then he beeps in and disconnects you because either ‘Nanny Telecom’ or somebody outside British Telecom in ‘Nanny Government’ decided the last thing that was to happen in Britain, was that people were to use naughty words over the telephones because after all it's illegal to use naughty words through the mail.
