1. Tracking the Performance of Listed Shares: A Comparison Between JSE Single- and Dual-listed Shares

Abstract: The measurement of a stocks return over a time period is analysed for several reasons. The most obvious and most important one is to inform investors’ expectations regarding future earnings potential. Therefore, the study made a comparison using the financial performance of single-listed shares versus dual-listed shares that trade primarily on the South African stock market (JSE). The time-period for the comparison of financial performance of single- and dual-listed shares was from 2005 to 2020 to confirm or refute the general perception surrounding superior returns of dual-listed companies as opposed to single-listed companies. Utilising financial ratios can be imperative when making informed judgments about investment portfolios. Seven of the most important financial ratios were used to measure the performance of company shares within nine specified industry sectors in South Africa. These included the earnings per share ratio (EPS), price-earnings ratio (P/E), market to book value ratio (M/B), current ratio (CR), debt to equity ratio (DER), and the return on equity ratio (ROE). The nine identified industries included the transport, consumer staples, printing, pharmaceutical, mining and manufacturing, technology, luxury goods and services, financial services, and real estate industries. The results indicate that the dual-listed companies do indeed outperform single-listed companies on the JSE for the majority of the financial ratios over the specified period. This study contributes to portfolio management by informing equity allocation in the short-term and long-term.

2. The Impact of Covid-19 on the Performance of Hedge Funds Compared to Mutual Funds in South Africa

Abstract: Investors are constantly searching for methods to generate value above passive investment techniques. Therefore, analysing the performance of hedge funds as compared to mutual funds, particularly in the wake of Covid-19, can aid investors in their investment decision-making process. Those investors who desire above-average returns, particularly in volatile market conditions place an expectation on hedge funds to be able to achieve higher performance during economic downturns, given that they are designed to mitigate risk and to take advantage of harsh financial market conditions. Monthly, secondary data were collected from 30 September 2018 to 31 August 2021 to analyse and compare the risk-adjusted performance of five hedge funds and five mutual funds in South Africa. Both hedge and mutual funds indicated higher risk-adjusted returns from the pre-Covid-19 period compared to during the pandemic. Hedge funds were found to have higher risk-adjusted returns than mutual funds during the Covid-19 period. The novelty of these results indicated that hedge fund managers can achieve higher returns for investors during extreme market events.

3. Towards integrated reporting: The inclusion of content elements of an integrated annual report in the chairmen’s statements of JSE-listed companies

Abstract: A call for greater transparency following various corporate scandals in a volatile global economy has placed intense pressure on South African corporations to reform, not only their conduct but also their reporting. As investors and other stakeholders insisted on the assurance that company practices were transparent, accountable and fair to all stakeholders, the JSE changed listing requirements to oblige listed companies to integrate their sustainability reports with their annual reports. To understand how companies incorporate integrated reporting standards in voluntary narrative disclosures, this article reports on how chairpersons of 50 high-performing and 50 poor-performing companies listed on the JSE included content elements within their statements. Qualitative content analysis was used to analyse how content elements were presented and communicated in the 2012 chairpersons’ statements. Findings indicate that South African companies embraced new reporting requirements. Just as the King Reports have been praised as an internationally leading code of governances, annual corporate reports of JSE-listed companies can become an international benchmark for integrated reporting. Even though the chairperson’s statement does not present the platform to discuss all the content elements in detail, the content elements of integrated reporting seem to provide a useful framework for chairpersons on what to include in their chairperson’s statement.

Abstract: Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. Abstract: To document incidence rate and severity of specific sub-categories of respiratory tract illness (RTill) in rugby players during the Super Rugby tournament.Cross-sectional study.Team physicians completed daily illness logs in 537 professional male rugby players from South African teams participating in the Super Rugby Union tournaments (2013-2017) (1141 player-seasons, 102,738 player-days). The incidence rate (IR: illness episodes/1000 player-days) and severity [%RTill resulting in time-loss, illness burden (IB: days lost to illness/1000 player-days) and days until return-to-play (DRTP)/single illness (mean: 95% Confidence Intervals)] are reported for the following specific sub-categories of RTill: non-infective respiratory tract illness (RTnon-inf), respiratory tract infections (RTinf), influenza-like illness, infective sinusitis, upper respiratory tract infections (URTinf), lower respiratory tract infections (LRTinf).The overall IR of RTill was 2.9 (2.6-3.3). IR was higher for RTinf (2.5; 2.2-2.9) vs. RTnon-inf (0.4; 0.3-0.6) (p 0.001). For sub-categories the highest IR was in URTinf (1.9; 1.7-2.2), while the % illness causing time-loss was influenza-like illness (100%), LRTinf (91.7%), infective sinusitis (55.6%), and URTinf (49.0%). IB was highest for URTinf (2.0; 1.6-2.5), and the DRTP/single illness was highest for LRTinf (3.2; 2.3-4.4), and influenza-like illness (2.1; 1.6-2.8).RTinf accounted for57% of all illness during the Super Rugby tournament, and mostly URTinf. Influenza-like illness. LRTinf caused time-loss in90% cases. URTinf, LRTinf and influenza-like illness resulted in the highest burden of illness and LRTinf caused the highest DRTP. Prevention strategies should focus on mitigating the risk of RTinf, specifically URTinf, LRTinf and influenza-like illness.

Abstract: OBJECTIVES To determine the prevalence of self-reported pre-race chronic medical conditions and allergies in ultramarathon race entrants and to explore if these are associated with an increased risk of race-day medical encounters (MEs). METHODS Data from two voluntary open-ended pre-race medical screening questions (Q1 - history of allergies; Q2 - history of chronic medical conditions/prescription medication use) were collected in 133641 Comrades Marathon race entrants (2014-2019). Race-day ME data collected prospectively over 6 years are reported as incidence (per 1000 starters) and incidence ratios (IR: 95%CI's). RESULTS Pre-race medical screening questions identified race entrants with a history of chronic medical conditions and/or prescription medication use (6.9%) and allergies (7.4%). The % entrants with risk factors for cardiovascular disease (CVD) was 30% and being older (>45 years) or male (27.5%) were the most frequent CVD risk factors. 0.3% of entrants reported existing CVD. The overall incidence of MEs was 20/1000 race starters. MEs were significantly higher in race entrants reporting a 'yes' to Q1 (allergies) (IR = 1.3; 1.1-1.5) (p = 0.014) or Q2 (chronic medical conditions and/or prescription medication use) (IR = 1.3; 1.1-1.5) (p = 0.0006). CONCLUSIONS Voluntary completion of two open-ended questions identified chronic medical conditions and/or prescription medication use in 6.9% and allergies in 7.4% of ultramarathon race entrants. This is lower than that reported for other races that implemented compulsory completion of a more comprehensive pre-screening questionnaire. Despite potential under-reporting, a pre-race self-reported history of chronic medical conditions and allergies was associated with a higher risk of race-day MEs.

Abstract: Despite the importance attached to MI and other marketing information functions, surprisingly few studies have explicitly examined the relationship between MI and strategic marketing decision-making. This article reports on a study conducted with the aim of determining the relationship between marketing intelligence (MI) and strategic marketing in South African organisations. A quantitative survey was conducted among 166 South African marketing decision-makers. The findings suggest a substantial gap between the importance and availability of key types of MI. Marketing decision-makers found the traditional MI and marketing tools of great value in supporting marketing decision-making, but the value of several of the newer MI tools and technologies was less clear. An analysis of MI practices suggested that MI quality and particularly information and communication technology (ICT) support for MI are areas requiring further attention.

Abstract: Writing guidelines on procedural sedation and analgesia is a formidable and challenging task. There are many disciplines and societies at international level now involved in writing guidelines and we need to be aware of what they see as important in their guidelines. We always need to identify new trends in sedation; for that we need to be involved at international level. Our aim with these guidelines is to provide a guidance for safe sedation practice for all healthcare providers who are involved in sedation practice. In this issue we have added adverse events and record-keeping, accreditation and the importance of simulation training to our recommendations under clinical governance. We would like our sedation practitioners to keep records and report adverse events to our societies, e.g. SOSPOSA, so that we all can benefit from the information we get. It is also crucial that sedation practitioners keep their logbooks and update them regularly – “if it is not written down, it never happened”. All aspects of accreditation remain an important issue and we need to address this urgently. This forms an important part of safe sedation practice, also done at international level, and we need to follow this. The facilities where we work, especially outside the operating theatre, must meet the requirements for safe practice. It is our responsibility to see that this is the case. In the appendices there is a practice appraisal protocol that should be filled in by sedation practitioners doing sedation outside the operating theatre. We do not cover sedation techniques in the guidelines. We believe sedation practitioners must learn the techniques with supervised clinical training. Capnography for sedation outside the operating theatre will become a focus point in future. We are only supposed to be doing ASA I and II patients outside the operating theatre so there may be a feeling we do not need capnography. There is pressure to include capnography under monitoring as a minimum monitoring standard, like pulse oximetry and blood pressure monitoring. Our appendices have been revised with more information on what patients need to know about sedation e.g. an example of a cover letter to the patient. This will include information to the patient, as well as information from the patient. Sedation is today one of the fastest growing areas in anaesthesia care. Our research studies show a low incidence of side-effects, and a high incidence of patient satisfaction. To all our readers we wish you a safe sedation journey.

Abstract: Commercial reality demands long and lasting relationships that are beneficial to both advertising agencies and their clients. This article presents a conceptual model to illustrate the salient choice criteria employed by advertisers when renewing advertising agency contracts. The model was empirically tested, and data obtained from 116 respondents were subjected to factor and correlation analysis to explore the dimensionality of the retention construct. Research results support that retention is multidimensional and highlights the limitations of research that measure single factors related to retention. The model postulates that retention is the result of a good working relationship, service satisfaction, respect and support, clear terms of engagement; and affective conflict resolution. Insight into the nuances of each of these antecedents is also offered. In management practice, this model will provide valuable insight to agencies with regard to client retention management. Findings of this study may also be extended to other high-value professional service industries.

The objective is to identify factors that may predict GITill among 21.1km and 56km race starters. METHODS This is a cross-sectional analysis of data collected prospectively over 8 years at the Two Oceans 56km and 21.1km races with 153 208 race starters. GITill encounters requiring medical attention on race day were recorded by medical staff. Risk factors associated with GITill explored in univariate models included: race distance (21.1km; 56km), sex, age group, running experience, running speed, and environmental factors (wet-bulb temperature, wind speed and humidity). Abstract: Abalone is currently considered South Africa's most successfully produced aquacul‐ ture export product, with a 76% share of the total value generated by the aquaculture sector. A major risk factor for this sector is slow growth rates experienced during farming. Abalone feeds are often supplemented with amino acids in an attempt to enhance abalone growth. This is a first investigation of the effect of added proline to standard abalone feed, on the metabolite profile of slow‐growing abalone. A targeted liquid chromatography tandem mass spectrometry metabolomics research approach was followed to recognise the metabolic response of abalone showing slower growth performance. The addition of proline to the standard abalone diet was found to serve as a substrate for amino acid catabolism in slower growing abalone, by means of pro‐ line breakdown to assist with energy production via the tricarboxylic acid cycle. Other amino acids and urea cycle intermediates, that is, arginine, asparagine, ornith‐ ine and creatine further support energy production via the action of protein catabo‐ lism in slow‐growing abalone. Additionally, the importance of understanding how abalone respond metabolically to modified feed highlights the use of metabolomics to answer abalone aquaculture farming questions

Abstract: Background: Chronic exertional compartment syndrome (CECS) of the forearm is a rare but important cause of morbidity amongst athletes involved in strenuous upper limb activities. The diagnosis remains challenging due to the absence of objective, reproducible diagnostic studies. Objectives: To assess and quantify signal intensity (SI) changes of involved muscles in patients with CECS of the forearm compared to healthy control subjects competing in similar sporting disciplines. Also, to objectively measure MRI SIs within muscle compartments when using a pre- and post-exercise regime and calculating a signal intensity ratio (SIR) between post- and pre-exercise studies. Method: The study retrospectively examined MRI scans of patients treated for CECS of the forearm and compared these to the MRI scans of asymptomatic high-level rowers. A specific, reproducible pre- and post-exercise MRI scanning protocol was utilised in both patient and control subjects between 2011 and 2020. Signal intensities were evaluated pre- and post-exercise in involved muscle groups and ratios were calculated. Results: A total of 86 SIs were measured (43 pre- and 43 post-exercise) in nine study participants (five patients and four controls). After post:pre-exercise comparisons, a statistically significant difference was found between control and patient groups ( p = 0.0010). The extensor carpi radialis, flexor digitorum profundus and flexor digitorum superficialis muscles were most commonly involved. Conclusion: This study confirms that significant SI changes are apparent in patients with CECS of the forearm when making use of a standardised pre- and post-exercise MRI protocol. Furthermore, SIR may be used to accurately diagnose CECS of the forearm.

Abstract: Breast cancer is one of the most common cancers affecting women. Frequent metastatic sites include liver, lungs and bone. Gastrointestinal metastases are a rare entity. We report a case of ductal carcinoma metastasizing to the sigmoid colon. A 74-year female was found to have a mass at her sigmoid colon on surveillance colonoscopy. The initial histology showed poorly differentiated carcinoma. Further immunohistochemical staining revealed poorly differentiated metastatic ductal breast carcinoma 27 years after her primary breast cancer. She proceeded to resection and had an uneventful recovery. She remains alive with stable retroperitoneal lymph node and tail of pancreas metastatic disease.

Abstract: A mobile-based behavioural change program iloveLive.mobi was implemented to prevent HIV among young people (12-24 years) in South Africa. The mobile site offered access to sexual and reproductive health and psychosocial information through interactive learning. The site provided incentives for positive behaviour (such as HIV testing). The research aimed to evaluate the effectiveness of the iloveLife.mobi site in promoting protective behaviour of users. A mixed-methods approach was used, including document review, KAPB survey (n = 1882), group discussions (n = 68) and telephonic interviews (n = 175) with users and interviews with 46 project and community stakeholders. The SRH and psychosocial information on the site was age-appropriate and useful to young people (82.2%). The site reached young men who are generally difficult to reach with SRH information. High-frequency users reported more confidence related to condom use and HIV testing and more protective behaviour (condom use, VMMC) compared to low-frequency users. Users also reported more protective behaviour (HIV testing, VMMC, condom use) than a comparable national sample. iloveLife.mobi became a repository for learning to reach young people with health information. The research highlights key issues to consider when implementing an mHealth platform in a low- or middle-income country, where literacy levels and technical infrastructure may cause challenges.

Abstract: Bacillus based probiotics are becoming relevant as alternatives to antibiotics used in poultry production and in other animal husbandry. This study describes the isolation of 48 Bacillus spp. candidates, from chickens and chicken environments, for use as potential probiotics in poultry production. These isolates, plus a further 18, were tested in a comprehensive in vitro screening regime that was specifically designed to select the best isolates that satisfied multiple modes of action desirable for commercial poultry probiotics. This screening programme involved the evaluation of the ability of the isolates to survive and grow in the limiting conditions of the chicken gastrointestinal tract. Only 11 of the isolates fulfilled these criteria; hence, they were further evaluated for the ability to adhere to epithelial cells, produce extracellular enzymes, and to demonstrate antagonistic activity against selected pathogens of significant importance in poultry production. Of these, a total of 6 isolates were selected, due to their all-round probiotic capability. Identification by 16S RNA sequencing confirmed these isolates as B. subtilis and B. velezensis, identities which are generally regarded as safe. The Bacillus isolates reported in our study exhibit strong all-inclusive probiotic effects and can potentially be formulated as a probiotic preparation for poultry production.

Abstract: Given the high failure rate of small and medium enterprises (SMEs) globally, this study aimed to identify domains of SME wealth creation. The study used a developmental configuration approach with a derived mathematical model, which are powerful tools for building and relating detailed models consisting of several variables and domains, illustrating complicated and inter-related relationships between several variables. The basis for the developmental configuration applied in the present study was an in-depth review of academic literature on wealth creation. The paper concludes with the presentation of a theoretical conceptual model of wealth creation. The six major domains included in this model are human resources, technology, innovation and creativity, unit cost economies, organizational infrastructure and strategy. The new conceptual model provides an entirely new premise for viewing wealth creation at the firm level, particularly for SMEs. The study also provides different indicators for the identified domains.

Abstract: International audience; Today, 3D objects are an increasingly popular form of media. It has become necessary to secure them during their transmission or archiving. In this paper, we propose a two tier reversible data hiding method for 3D objects in the encrypted domain. Based on the homomorphic properties of the Paillier cryptosystem, our proposed method embeds a first tier message in the encrypted domain which can be extracted in either the encrypted domain or the clear domain. Indeed, our method produces a marked 3D object which is visually very similar to the original object. It seeks to be format compliant and to preserve the original size of the data, without the need for an auxiliary file. Moreover, large keys are used, rending our method secure for real life applications.

Abstract: Summary Modern broilers have been selected for rapid growth but demonstrate reduced heat tolerance toward market age. As the poultry industry expands globally, strategies must be developed to support broiler performance in challenging climates. The objective of this study was to evaluate the effect of embryonic thermal manipulation (TM) and dietary fat source during the finisher period on broiler performance during acute heat stress (AHS) close to market age. The cyclic exposure to high temperatures during mid-incubation used in TM has been demonstrated to improve broiler tolerance to heat stress. However, high incubation temperatures can be detrimental to embryonic development and impair posthatch broiler performance. Embryos were exposed to 39.5°C for 12 h daily from incubation day 7 to 16 to assess the impact of TM on hatching and broiler performance. Dietary fat is commonly added to poultry diets during heat stress and it was theorized that differences in fat source may further impact bird performance. Finisher diets were supplemented with soya oil, poultry fat, or olive oil at 4.5% each. Broilers were exposed to a period of AHS at 43 d. Embryo mortality was increased, and hatchability was reduced by TM. Broiler performance was also decreased for the TM birds, but mortality during AHS was markedly reduced. Dietary fat source did not influence bird performance but was shown to interact with incubation treatment. Overall, the present data suggest optimal performance in modern broiler strains may be at odds with improved heat tolerance.

Abstract: This paper develops a multi-dimensional model of retention to provide a more complete and integrated view of client retention and its determinants in service contexts. To uncover the antecedents of client retention, social and economic exchanges were reviewed under the fundamental ideas of the Social Exchange Theory. Findings from a survey of senior South African advertising executives suggest that client retention is the result of evaluative as well as relational factors that can influence client responses. Despite contractual obligations, advertisers are willing to pay the costs and make the sacrifices of switching should their expectations be unmet. An important contribution of this study is the use of multi-item scales to measure retention. The model developed provides valuable insight to agencies on client retention management and the optimal allocation of resources for maximum customer equity. This model may also be applied to other service organisations to provide insight to client retention.

Abstract: In the digital age travellers have more choices and are better informed. This conceptual paper considers the evolution of business models in the travel industry and evaluates the value propositions offered by traditional retail travel agencies. Conclusions suggest that services provided by travel agents are still relevant and valuable. It was also found that the digital age offers various opportunities to agencies to harness the benefits of information communication technologies. However, awareness of its functionality and resources is required to utilize this platform to achieve economies of scale and rationalise costs. Value propositions, offered by travel agencies, also need to be altered to meet the changing customers' buying habits of tourism products. It is recommended that ICT and the Internet should become part of an integrated, customer centric marketing plan which includes online and traditional marketing tools. Travel agencies also need to engage in clear customer segmentation based on established customer needs. Despite the many technological advances, many travellers still find the idea of traveling daunting and the Internet to be intimidating and are willing to pay a premium for human interaction.

Abstract: Purpose: To assess the impact of long-haul transmeridian travel on subjective sleep patterns and jet lag symptoms in youth athletes around an international tournament. Methods: An observational descriptive design was used. Subjective sleep diaries and perceived responses to jet lag were collected and analyzed for a national junior netball team competing in an international tournament. Sleep diaries and questionnaires were completed daily prior to and during travel, and throughout the tournament. Results were categorized into pretravel, travel, training, and match nights. Means were compared performing a paired Student t test with significance set at P Results: Athletes reported significantly greater time in bed on match days compared with training (P P = .002) days, and on pretravel days compared with travel (P P = .028) days. Sleep ratings were significantly better on pretravel days compared with match (P = .013) days. Perceived jet lag was worse on match (P = .043) days compared with pretravel days. Significant differences were also observed between a number of conditions for meals, mood, bowel activity, and fatigue. Conclusion: Youth athletes experience significantly less opportunity for sleep during long-haul transmeridian travel and face disruptions to daily routines during travel which impact food intake. Young athletes also experience disturbed sleep prior to and during competition. These results highlight the need for practices to alleviate jet lag symptoms and improve the sleep of young athletes traveling for tournaments in an effort to optimize recovery and performance.

Abstract: Traditional crops have historically provided accessible and affordable nutrition to millions of rural dwellers but have been neglected, with most modern agricultural systems over-reliant on a small number of internationally traded crops. Traditional crops are typically well-adapted to local agro-ecological conditions and many are nutrient-dense. They can play a vital role in local food systems through enhanced nutrition (particularly where diets are dominated by starch crops), food security and livelihoods for smallholder farmers, and a climate-resilient and biodiverse agriculture. Using short-read, long-read and phased sequencing technologies, we generated a high-quality chromosome-level genome assembly for Amaranthus cruentus, an under-researched crop with micronutrient- and protein-rich leaves and gluten-free seed, but lacking improved varieties, with respect to productivity and quality traits. The 370.9 Mb genome demonstrates a shared whole genome duplication with a related species, Amaranthus hypochondriacus. Comparative genome analysis indicates chromosomal loss and fusion events following genome duplication that are common to both species, as well as fission of chromosome 2 in A. cruentus alone, giving rise to a haploid chromosome number of 17 (versus 16 in A. hypochondriacus). Genomic features potentially underlying the nutritional value of this crop include two A. cruentus-specific genes with a likely role in phytic acid synthesis (an anti-nutrient), expansion of ion transporter gene families, and identification of biosynthetic gene clusters conserved within the amaranth lineage. The A. cruentus genome assembly will underpin much-needed research and global breeding efforts to develop improved varieties for economically viable cultivation and realization of the benefits to global nutrition security and agrobiodiversity.

Studies on the suitability of Bacillus probiotics are emerging and showing benefits, but information on the production technology is limited. We developed the production process for a novel probiotic product previously shown to be effective in field trials. All strains were cultivated to a spore concentration exceeding 1 × 1010 CFU. mL−1. The spores of each strain were harvested, processed into a powder intermediate and formulated into an end product with 100 % recoveries and a shelf life stability >1 year. The probiotic was shown to be incorporated into broiler feed exceeding the desired concentration of 1 × 106 CFU. g−1. Using efficient process technology and lower cost materials, this study presents a commercially relevant case for the potential adoption of probiotic products by the poultry industry.

Abstract: BackgroundStreptococcus pneumoniae, Haemophilus influenzae and Neisseria meningitidis are leading causes of invasive diseases including bacteraemic pneumonia and meningitis, and of secondary infections post-viral respiratory disease. They are typically transmitted via respiratory droplets. We investigated rates of invasive disease due to these pathogens during the early phase of the COVID-19 pandemic.MethodsLaboratories in 26 countries across six continents submitted data on cases of invasive disease due to S pneumoniae, H influenzae and N meningitidis from 1 January 2018 to 31 May 2020. Weekly cases in 2020 vs 2018-2019 were compared. Streptococcus agalactiae data were collected from nine laboratories for comparison to a non-respiratory pathogen. The stringency of COVID-19 containment measures was quantified by the Oxford COVID-19 Government Response Tracker. Changes in population movements were assessed by Google COVID-19 Community Mobility Reports. Interrupted time series modelling quantified changes in rates of invasive disease in 2020 relative to when containment measures were imposed.FindingsAll countries experienced a significant, sustained reduction in invasive diseases due to S pneumoniae, H influenzae and N meningitidis, but not S agalactiae, in early 2020, which coincided with the introduction of COVID-19 containment measures in each country. Similar impacts were observed across most countries despite differing stringency in COVID-19 control policies. There was no evidence of a specific effect due to enforced school closures.InterpretationThe introduction of COVID-19 containment policies and public information campaigns likely reduced transmission of these bacterial respiratory pathogens, leading to a significant reduction in life-threatening invasive diseases in many countries worldwide.

32. One in four trail running race entrants sustained an injury in the 12 months training preceding the 2019 SkyRun race

Abstract: Objective To determine the epidemiology and clinical characteristics of RRIs among trail runners who entered the 2019 SkyRun races. Design Descriptive cross-sectional study. Setting 2019 SkyRun races. Participants Consent for data analysis was given by 305 of 412 (74%) race entrants. Main outcome measures Retrospective annual incidence (RRIs/1000 h), point prevalence (%), frequency (%), characteristics (anatomical region, body area, tissue type, pathology type) and injury severity (mean severity score; 95% CI) of RRIs. Results 28.2% of participants reported at least one RRI. The retrospective annual incidence was 49.5 RRIs per 1000h and the point prevalence was 1.3%. Most injuries occurred in the lower limb (87.3%), with the knee (26.5%), ankle (21.6%), and foot (16.7%) reported as the most frequently injured body areas. Muscle/tendon accounted for 44.1% of tissue type injuries. Tendinopathy (27.5%), joint sprain (19.6%), and muscle injury (15.7%) were the most common pathology types reported. The mean injury severity score was 31.6. Conclusions One in 4 trail runners reported at least one RRI in the 12 months leading up to a race. RRIs mostly affected the lower limb specifically the knee, ankle and foot. Future research should establish injury risk factors to ultimately develop specific injury prevention strategies.

Abstract: Introduction: Nuclear medicine technologists are experts in the acquisition of myocardial perfusion images, including many other images performed in nuclear medicine departments. They are expected to ensure that images are of optimum quality in order to facilitate accurate image interpretation by nuclear medicine physicians. However, optimum image quality assurance is a shared responsibility between nuclear medicine technologists and nuclear medicine physicians. The shared responsibilities resulted in inconsistences in the assessment of myocardial perfusion image quality among nuclear medicine technologists in the respective departments. Little is known about the perceptions and experiences of nuclear medicine technologists on the assessment of myocardial perfusion image quality. Therefore, the focus of this research study was on nuclear medicice technologists. Aim: The aim of this qualitative study was to explore and describe the perceptions and experiences of nuclear medicine technologists on the assessment of myocardial perfusion image quality. The research question was “how do nuclear medicine technologists perform the responsibility of myocardial perfusion image quality?” Methods: The study followed a qualitative explorative approach using focus groups as a means of collecting data. Nineteen nuclear medicine technologists from four academic hospitals were purposefully selected to participate. A semi- structured questionnaire was used to conduct the focus groups. The collected data was managed using ATLAS.ti version 7, a computer aided qualitative data analysis software to formulate codes, categories and themes. Results: Two overarching themes emerged from the data. (1) The management of the myocardial perfusion images (2) resources required to support nuclear medicine technologists. The nuclear medicine technologists differed in their management of myocardial perfusion images, due to the prevailing circumstances in their respective departments. In addition, the results suggested that nuclear medicine technologists’ level of involvement in the assessment of myocardial perfusion image quality was influenced by the availability of resources required for processing and assessing image quality. Conclusion: Despite the shared responsibility in the assessment of myocardial perfusion image quality with nuclear medicine physicians, the nuclear medicine technologists considered themselves playing a major role. However, resources to facilitate the assessment of image quality are considered necessary and should be available to support the nuclear medicine technologists to submit images of optimum quality for interpretation.

Abstract: Background: It is plausible that optimal cardiovascular disease (CVD) risk management differs in patients with rheumatoid arthritis (RA) from low or middle income compared to high income populations. This study aimed at producing evidence-based points to consider for CVD prevention in South African RA patients. Methods: Five rheumatologists, one cardiologist and one epidemiologist with experience in CVD risk management in RA patients, as well as two patient representatives, two health professionals and one radiologist, one rheumatology fellow and 11 rheumatologists that treat RA patients regularly contributed. Systematic literature searches were performed and the level of evidence was determined according to standard guidelines. Results: Eighteen points to consider were formulated. These were grouped into 6 categories that comprised overall CVD risk assessment and management (n = 4), and specific interventions aimed at reducing CVD risk including RA control with disease modifying anti-rheumatic drugs, glucocorticoids and non-steroidal anti-inflammatory drugs (n = 3), lipid lowering agents (n = 8), antihypertensive drugs (n = 1), low dose aspirin (n = 1) and lifestyle modification (n = 1). Each point to consider differs partially or completely from recommendations previously reported for CVD risk management in RA patients from high income populations. Currently recommended CVD risk calculators do not reliably identify South African black RA patients with very high-risk atherosclerosis as represented by carotid artery plaque presence on ultrasound. Conclusions: Our findings indicate that optimal cardiovascular risk management likely differs substantially in RA patients from low or middle income compared to high income populations. There is an urgent need for future multicentre longitudinal studies on CVD risk in black African patients with RA., The first meeting held amongst local Rheumatologists was funded by the South African Arthritis and Rheumatology Association. Abstract: Background: Given the minimal capacity and sometimes the failure of the mammalian nervous system to regenerate and repair itself after damage, strategies are required to help enhance this regenerative process. Adipose-derived Mesenchymal Stem Cells (ADMSCs) are likely candidates to assist in the recovery process due to their ability to differentiate into neural cells. Successful implementation of this intervention in a clinical setting would increase the rate of recovery following traumatic brain injury. Review: Various strategies have been attempted to differentiate ADMSCs into neural cells for clinical use. Such methods have not been entirely successful in the development of functioning specialized cells for subsequent practical use. Therefore, the implementations of this differentiation technique in the clinical trial have not been effective. In this article, the potential of differentiating ADMSCs into neural cells and the various methods employed, including biological induction, chemical induction and photobiomodulation (PBM) will be discussed, where the combined use of transducers and PBM for neural differentiation of ADMSCs is also deliberated. Conclusion: PBM shows promise as an avenue for effective ADMSCs differentiation into neural cells and their proliferation. Applying PBM with optimized biological factors and chemical inducers may prove to be an effective tool for clinical application.

Abstract: Skeletal muscle relaxants (SMRs) consist of a heterogeneous group of medications with a side effect profile of concern. The aim of this paper was to review the evidence of use of SMRs in the treatment of sports injuries. A literature search between 2005 – 2018 (Ovid MEDLINE, SPORTDiscus and SCOPUS) were conducted. In addition, citations within articles were searched, and the most commonly prescribed SMRs in South Africa were also used to explore the literature for additional publications. Relevant studies that met the inclusion criteria were selected. Clinical recommendations for general practitioners are given based on the Strength of Recommendation Taxonomy (SORT) level of evidence. Combination drugs rather than single agents are mostly used, however the effectiveness of SMR agents, single and in combination, as well as its significance as opposed to analgesics and non-steroidal anti-inflammatory drugs, still has to be evaluated. Evidence suggest SMRs to be probably effective for use in non-specific lower back pain (acute and chronic lower back muscle strains, ligament sprains, soft tissue contusions), as well as for whiplash associated disorder, mechanical neck disorders, piriformis syndrome, lateral epicondylosis, and plantar fasciitis. It does not appear if there is a role for SMRs as part of combination management for acute cervical strains, post-exercise muscle soreness or myofacial pain syndrome. However, substantial evidence to confirm the use of SMRs in the treatment of sports injuries have not been adequately investigated and is currently largely based on case reports and general reviews. Abstract: Exporters, in developing countries, often struggle to access accurate market information which will help them make informed decisions about which export markets and products to focus on. Despite the existence of several market selection techniques, a lack of market information has been identified as a major impediment to export activity in the Indian Ocean Rim region. This paper introduces the TRADE-DSM®, a scientific market selection methodology that screens large quantities of data to reveal the most promising realistic export opportunities for a company, industry, country or region. Abstract: Safety and efficacy are of concern in developing new compounds. There is a paucity on the changes made by new compounds on inflammatory cytokines in combination with oxidative stress markers. Cytokines are potential markers of drug safety, focusing on cardiovascular, neurologic, and respiratory functions. In addition oxidative stress markers may indicate the impact of drugs on physiological processes. According to literature, a dysregulation of cytokines can be indicative of chronic inflammation. There is no formal reference of normal ranges of inflammatory markers. By the establishment of a baseline for preclinical safety studies, the effects of new compounds on cytokines can be quantified. When cancer cells were then exposed to an anti-cancer compound, the anti-oxidative enzymes' activity increased. Oxidative stress plays an important role in the regulation of physiological processes. The aim of this study was to determine the changes in the cytokines and anti-oxidant enzymes in the renal and hepatic systems in vivo (xenograft rats) after treatment with radiolabeled compounds (confidential, under development). Tissue was collected from xenografted RNU rats, from a control group and two radiolabeled compound exposed groups. The cytokines were analyzed using a CBA Flow Cytometry cytokine assay. Oxidative stress markers analyzed was intracellular reactive oxygen species (ROS), superoxide dismutase (SOD), catalase (CAT), protein carbonyls (PC), and lipid peroxidation (malondialdehyde, (MDA)). There were changes in the cytokines and oxidative stress markers post-exposure to radiolabeled compounds. In the kidneys, the copper-chelate (Cu-c) increased the pro- and anti-inflammatory cytokines (55% & 35%), where Pd-c decreased both (30% & 39%). In contrast liver cytokines for the Cu-c group (19 & 41%) decreased but increased for Pd-c (45 & 84%). In the kidneys, ROS was lower in Cu-c groups and showed an increase for Pd-c. The SOD levels in the kidneys was increased (30%) for Cu-c and decreased (20%) for Pd-c. A 120% increase for Cu-c and 328% decrease for Pd-c was seen for SOD in the liver. The CAT levels in the kidneys were elevated for Cu-c (45%) and Pd-c (67%) compared to the control. In contrast CAT levels in the liver showed a 50% decrease in both Cu-c and Pd-c. The Cu-c group was responsible for more protein damage, whereas Pd-c was responsible for more lipid peroxidation. A relationship between the anti-oxidant markers and the cytokines was seen. This suggest that cytokines in combination with oxidative stress markers may be used as indicators for drug safety

Abstract: Background: The skin provides a permeable barrier which may be impaired in oc‐ cupational settings. Transepidermal water loss (TEWL), stratum corneum hydration (SCH) and skin surface pH (SSpH) have been used in occupational settings to predict early onset of occupational skin diseases, to measure the effectiveness of prevention strategies for occupational skin diseases, and to assess skin condition during expo‐ sure. The aim was to compare the findings, identify shortcomings in the methodology and data reporting and furthermore, to make recommendations for future studies. Materials and Methods: A literature study was conducted on studies published be‐ fore December 2018 to provide a review on the measurement of TEWL, SCH and SSpH in occupational settings. Results: TEWL, SCH and SSpH were previously measured in a wide variety of indus‐ tries. Results between studies were highly variable, due to different study designs and different anatomical positions measured. Not all of the measurement conditions were reported and variations in study objectives led to data being reported and in‐ terpreted differently for most studies. Conclusion: Incomplete reporting of methodology hinders comparison of bioen‐ gineering measurements. No bioengineering method has been proven useful as a predictive tool for occupational skin diseases, however, it is useful in the acute as‐ sessment of skin condition. It is recommended that future studies on TEWL, SCH and SSpH adhere to guidelines for occupational settings as far as possible to enable comparison between studies

Abstract: The study investigated consumers’ overall liking and sensory profiling of 13 boiled Amaranthus genotypes by using the Check-all-that-apply question. Fifty consumers ranked their preference on a nine-point hedonic scale to determine overall liking of boiled samples. Additionally, 100 consumers completed a CATA question, which contained 19 descriptive terms related to Amaranthus leaves. Significant differences were found in the frequency in which consumers used 11 out of the 19 terms. Correspondence analysis, explaining 68.1% variance, illustrated differences in sensory characteristics between genotypes. Hierarchical cluster analysis identified three consumer segments, indicating the heterogeneous acceptance of samples. External mapping of the samples’ configuration identified regions of maximum liking. Sensory characteristics of Amaranthus leaves varied between genotypes and species, with no relationship between specific species and their sensory properties. The information obtained can be recommended to farmers to be incorporated into breeding programmes.

Abstract: Objective: Cancer patients in Africa face unique challenges such as poverty, access to health care and under-resourced health-care systems. Although quality of life (QoL) of cancer patients has been well researched, the perspectives of cancer patients living in Africa are unknown. The objective was to explore what constitutes QoL for cancer patients accessing public health care in South Africa. Methods: A qualitative exploratory design was used, and data were gathered by means of in-depth interviews. Purposive sampling selected the participants, and the sample size was determined by saturation (n = 22). The data were analyzed using Patton's method of content analysis. Results: The participants were aged between 20 and 79 years, with an average of 50 years. Most were female and represented seven cultural groups. Four themes that influence QoL arose from the data: psychosocial-, physical-, spiritual and financial factors. Conclusions: QoL remains a complex phenomenon, enhanced and diminished by various individual factors. Poverty was a major issue and influenced the physical aspects of QoL, as the participants had to be strong enough to work and earn a living. Support from family, friends, and church members enhanced QoL, as well as religion and religious practices. Measuring QoL would be the next step to enable nurses to implement measures to improve QoL. Whether existing QoL instruments would be suitable for this patient population is not known and should be investigated before implementation.

Abstract: A 38-year-old man, previously well, presents after being mauled by a Cape buffalo, while on Safari in South Africa. He recovered well but visited a cardiologist for infrequent palpitations lasting less than 10 min. A chest radiograph demonstrated increased cardiothoracic ratio raising the concern of possible traumatic mediastinal injury. His clinical examination was unremarkable. ECG demonstrated sinus rhythm with incomplete right bundle branch block. Echocardiography revealed a dilated, hyperdynamic right ventricle (RV) and dilated coronary sinus (CS). The left ventricle (LV) and aorta were normal with no evidence of aneurysm or dissection. An agitated saline bubble study was performed via the left antecubital fossa. Interestingly, all bubbles entered the right atrium (RA) via the superior vena cava (SVC) with no bubbles entering via the CS and no bubbles shunting to the left …

Abstract: ObjectivesWe investigated the management of travel fatigue and jet lag in athlete populations by evaluating studies that have applied non-pharmacological interventions (exercise, sleep, light and nutrition), and pharmacological interventions (melatonin, sedatives, stimulants, melatonin analogues, glucocorticoids and antihistamines) following long-haul transmeridian travel-based, or laboratory-based circadian system phase-shifts.DesignSystematic reviewEligibility criteriaRandomised controlled trials (RCTs), and non-RCTs including experimental studies and observational studies, exploring interventions to manage travel fatigue and jet lag involving actual travel-based or laboratory-based phase-shifts. Studies included participants who were athletes, except for interventions rendering no athlete studies, then the search was expanded to include studies on healthy populations.Data sourcesElectronic searches in PubMed, MEDLINE, CINAHL, Google Scholar and SPORTDiscus from inception to March 2019. We assessed included articles for risk of bias, methodological quality, level of evidence and quality of evidence.ResultsTwenty-two articles were included: 8 non-RCTs and 14 RCTs. No relevant travel fatigue papers were found. For jet lag, only 12 athlete-specific studies were available (six non-RCTs, six RCTs). In total (athletes and healthy populations), 11 non-pharmacological studies (participants 600; intervention group 290; four non-RCTs, seven RCTs) and 11 pharmacological studies (participants 1202; intervention group 870; four non-RCTs, seven RCTs) were included. For non-pharmacological interventions, seven studies across interventions related to actual travel and four to simulated travel. For pharmacological interventions, eight studies were based on actual travel and three on simulated travel.ConclusionsWe found no literature pertaining to the management of travel fatigue. Evidence for the successful management of jet lag in athletes was of low quality. Abstract: Although currently limited, services trade holds great potential for Bangladesh, as services already make a major contribution to GDP and employment. Services represent an important alternative (in the longer term) or complement (in the shorter term) to ready-made garments (RMGs), which have long dominated Bangladesh’s export mix. The country is poised to see declining RMG export revenues when the country graduates out of least developed country (LDC) status and loses its trade preferences in global markets. To build domestic capacity with a view to developing its services export sector, Bangladesh needs to open its market to services imports. But what approach would be best? Can a plurilateral trade agreement (PTA) like the Trade in Services Agreement (TiSA), whose members have sought to stimulate their services sectors through more liberalized trade, ever be an option? We use a dynamic computable general equilibrium (CGE) model to simulate the effects of TiSA membership on Bangladesh’s economy. The results show that, overall, Bangladesh would derive marginal benefit from TiSA, but employment and exports would suffer. The worst-affected sectors would be agriculture and textiles and clothing, the country’s largest employers. To lessen the impact of increased foreign competition, a regional trade approach is recommended, supported by a sound national services strategy which would include a roadmap for tackling the country’s myriad supply-side shortcomings. Abstract: Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m2 and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.
