Rationale: Among hospitalized patients with COVID 19 pneumonia, up to 32% may require admission in intensive care units (ICU) The aim of this prospective cohort study is to assess nutritional parameters in patients with COVID 19 following ICU Methods: All patients with COVID 19 requiring ICU stay (minimum 14 days) with mechanical ventilation were included after ICU discharge Recorded parameters at the time of post-ICU rehabilitation unit (PIRU) admission included demographics, body mass index (BMI), weight loss (%), Hand Grip Test (kg), nutrition therapy modalities, and albumin dosage (gr/l) Quantitative parameters were expressed in median and range Results: Until May 5th2020, 11 patients were included (age 58(33-75) years old, and 5 men (45%)) BMI at ICU admission was 25 7 (22 2-33 3) kg/m2 Duration regarding ICU stay and ventilation were respectively 29 (25-39) and 22 (13-28) days Three patients (27%) required extracorporeal membrane oxygenation and tracheostomy was performed in 5 (45%) During ICU stay, enteral nutrition was administered to all patients through a naso-gastric tube;a percutaneous endoscopic gastrostomy was placed in the ICU in two patients One patient required complementary parenteral nutrition At the time of admission in the PIRU, BMI was 22 9 (19 1-32 9) kg/m2 and nutrition dosage was calculated at a median of 2553kcal/day (≥28 kcal/kg/day) and 128 gr protein/day (≥1 3 gr/kg/day) Weight loss since ICU admission was estimated at 8 3% (4 3%>14%) Post-extubation dysphagia requiring texture adaptation was present in 5 patients (45%) Albumin levels were 30 (26-36) gr/L Hand-grip was 12 (8-26) kg and 0 (0-20) kg for respectively men and women, reflecting significant sarcopenia Conclusion: Critical illpatients with COVID 19 pneumonia are malnourished and have severe sarcopenia following ICU stay despite adequate nutrition management Optimal nutrition therapy remains crucial during the rehabilitation period Disclosure of Interest: None declared

The present study reports a case of biopsy-proven pulmonary veno-occlusive disease as a cause of severe pulmonary hypertension in a patient suffering from a chronic myeloproliferative disorder. The pulmonary disease evolved favourably under treatment with defibrotide, a pro-fibrinolytic medication used in hepatic veno-occlusive disease.

5. Hemodynamic responses to successful weaning from mechanical ventilation after cardiovascular surgery

Weaning from mechanical ventilation is usually associated with an increase in oxygen consumption (VO2), which may stress the cardiovascular system. We studied relative changes in the cardiac index and oxygen extraction ratio (EO2) during successful weaning in patients after cardiac surgery (n = 52), cardiac transplantation (n = 17), or abdominal aortic surgery (n = 11). Cardiac index was determined by the thermodilution technique and arterial and mixed venous blood gases were obtained before and 30 min after the start of weaning through a T-piece. The cardiovascular changes were evaluated in 42 patients in whom VO2 (calculated by Fick's equation) increased by more than 10%. Cardiac index increased more after abdominal aortic surgery (from 3.27 +/- 0.77 to 4.44 +/- 0.581 min(-1) m(-2), p < 0.01) than after cardiac surgery (from 2.53 +/- 0.59 to 2.87 +/- 0.46 1 min(-1) m(-2), p < 0.01) or cardiac transplantation (from 2.99 +/- 0.64 to 3.33 +/- 0.741 min(-1) m(-2), p < 0.05). EO2 remained stable in patients after aortic surgery (from 25.9 +/- 7.1 to 25.2 +/- 5.6 %, NS) but increased slightly after cardiac surgery (from 33.3 +/- 6.1 to 37.3 +/- 6.4%, NS) and significantly after cardiac transplantation (from 25.8 +/- 4.1 to 28.2 +/- 4.0%, p < 0.05). Hence the cardiovascular response to weaning from mechanical ventilation may vary according to the type of surgery.

Objective: To describe the practical aspects of nutritional man- agement in intensive care units (ICUs). Design: A 49-item questionnaire was sent to the physician members of the European Society for Inten- sive Care Medicine. The issues ad- dressed included: medical environ- ment, assessment of nutritional sta- tus and current practice for enteral and parenteral nutrition. Setting: 1608 questionnaires were sent in 35 European countries. Analysis: The answers were pooled and stratified by country. Results: 271 questionnaires were an- swered (response rate 17 %). As- sessment of nutritional status was generally based on clinical (99 %) and biochemical (82 %) parameters rather than on functional (24 %), anthropometric (23 %), immuno- logical (18 %) or questionnaire- based (11 %) data. Two thirds of 2774 patients hospital- ised in the corresponding ICUs at the time the questionnaire was an- swered were receiving nutritional support; 58 % of those were fed by the enteral route, 23 % by the par- enteral route and 19 % by combined enteral and parenteral. The presence of stress-induced hyperglycemia in critically ill patients, especially in those without evidence of antecedent diabetes, is a well established marker of poor outcomes [1, 2, 3, 4]. Two large single center studies comparing the standard strategy of permissive hyperglycemia to use of intravenous insulin to achieve a blood glucose between 80 and 110 mg/dl (intensive insulin therapy) demonstrated overall clinical benefit with the intensive insulin therapy [5, 6]. However, more recent studies of insulin therapy in critically ill patients have yielded conflicting results [7]. In addition, a growing awareness of the potential risks of hypoglycemia, which can occur more frequently when using intravenous insulin therapy, has raised concerns over the best way to control glucose. Despite this, reverting back to allowing hyperglycemia to continue unchecked is unlikely to be the correct approach either.

The tremendous interest in the physiological and pathological effects of nitric oxide (NO) is reflected in the enormous number of publications on this subject. Most of the studies investigating the effects of NO were performed in animal models. A Medline search from 1987 to November 1995, revealed that only 8 to 14% of the publications which include the keyword “nitric oxide” also include the keyword “human” (Fig. 1). However, there are several important differences between animal and human observations. The object of this chapter is to review the current knowledge about the role of NO in the hemodynamic alterations of septic shock, and to highlight the differences between animal and human studies.
