The main purpose of this study was to compare the parameters of computed tomography (CT) and the corresponding patient doses undergoing chest CT scan examinations in different regions of Brazil, providing the current scenario of how these procedures are being carried out in the country as well as the patient dose distribution. Thirty institutions, across 17 states and the Federal District participated in the survey. The evaluation included 30 multislice CTs of seven different models, manufactured by General Electric healthcare (GE). For each institution, data from 10 adult chest CT examinations, performed without contrast, were collected remotely. The analysis of the results showed a significant difference of the CTDIvol values, ranging from 1.1 mGy to 46.6 mGy. In 7 institutions. The mean value of CTDIvol was higher than values found in the literature and the UK Reference Levels. It was also observed that, regardless of the region of the country, for the same CT model, different scanning parameters were used, which resulted in CTDIvol up to five times higher among these institutions. Repetitions of CT acquisitions and scouts with radiation field dimensions larger than the region of interest were found in 25% of chest examinations, resulting in higher absorbed doses. The results of this work show a mapping of the chest CT procedures, which enable to establish strategic plans for the country. In addition, each institution will be able to implement an appropriate optimization program and establish institutional reference levels.

Purpose Optimization of CT scan practices can help achieve and maintain optimal radiation protection. The aim was to assess centering, scan length, and positioning of patients undergoing chest CT for suspected or known COVID-19 pneumonia and to investigate their effect on associated radiation doses. Methods With respective approvals from institutional review boards, we compiled CT imaging and radiation dose data from four hospitals belonging to four countries (Brazil, Iran, Italy, and USA) on 400 adult patients who underwent chest CT for suspected or known COVID-19 pneumonia between April 2020 and August 2020. We recorded patient demographics and volume CT dose index (CTDIvol) and dose length product (DLP). From thin-section CT images of each patient, we estimated the scan length and recorded the first and last vertebral bodies at the scan start and end locations. Patient mis-centering and arm position were recorded. Data were analyzed with analysis of variance (ANOVA). Results The extent and frequency of patient mis-centering did not differ across the four CT facilities (>0.09). The frequency of patients scanned with arms by their side (11–40% relative to those with arms up) had greater mis-centering and higher CTDIvol and DLP at 2/4 facilities (p = 0.027–0.05). Despite lack of variations in effective diameters (p = 0.14), there were significantly variations in scan lengths, CTDIvol and DLP across the four facilities (p Conclusions Mis-centering, over-scanning, and arms by the side are frequent issues with use of chest CT in COVID-19 pneumonia and are associated with higher radiation doses.

This work demonstrates the use of high-resolution 3D printing to fine-tune the low energy dependence of an eye lens dosimeter holder associated to a BeO OSL detector element (ezClip). Five geometries of the denominated iBe dosimeter were developed, three with a variation in the thickness of the wall in front of the sensitive element that tailor the response at low radiation energies; and three with variations of width and curvature in order to vary the angular response of the dosimeter badges. Additive manufacturing was accomplished using stereolithography which gave a high degree of accuracy and precision. The optimised dosimeter badges showed a low energy and angular dependence, within −20% to +20% in the energy range of 24 keV to 662 keV and from 0 to 60° incidence; and within −10% to +10% in the energy range of 24 keV to 164 keV and from 0 to 60° incidence. In contrast to other dosimeters with higher effective atomic numbers, the use of BeO as the sensitive element resulted in a flat energy and angular dependence response at low energies. A significant reduction in the measurement uncertainty in the diagnostic radiology energy range was achieved.

The aim of this multi-centre study was to evaluate the irradiation parameters and the radiation dose of abdomen CT exams without contrast, performed in adults In Brazil, in order to provide an overview of the CT installed base in the country and contribute for the establishment of optimization programs. Equipment of seven different models, with 16 and 64 channels, installed in 17 Brazilian states, plus the Federal District, were analyzed. The results show that the mean CTDIvol ranged from 6.60 mGy to 34.64 mGy. The results also showed that different irradiation parameters are used in equipment of the same model, resulting, in some cases, in three times higher CTDIvol values. These characteristics are not restricted to a given region of the country but distributed throughout the national territory. Repetitions of acquisitions and topograms with scan length larger than the region of interest were found in 13.5% of abdomen examinations, resulting in considerably higher absorbed doses than expected. This survey of the abdomen tomography exams, performed in about 74% of the Brazilian states, showed that in many clinics the procedures are not optimized. This study can be used for the planning of dose optimization programs and will contribute to the establishment of local diagnostic reference levels (DRL) for CT.

Computed tomography (CT) provides useful information in patients with known or suspected COVID-19 infection. However, there are substantial variations and challenges in scanner technologies and scan practices that have negative effect on the image quality and can increase radiation dose associated with CT. Objective: In this article, we present major issues and challenges with use of CT at five Brazilian CT facilities for imaging patients with known or suspected COVID-19 infection and offer specific mitigating strategies. Methods: Observational, retrospective and prospective study of five CT facilities from different states and regions of Brazil, with approval of research and ethics committees. Results: The most important issues include frequent use of CT, lack of up-to-date and efficient scanner technologies, over-scanning and patient off-centring. Mitigating strategies can include updating scanner technology and improving scan practices.

The aim of the present study was to evaluate the response of the MOSkin MOSFET dosimeter for X-ray diagnostic CT beams. Experiments were performed to investigate the sensitivity, energy dependence, reproducibility, fading and angular dependence of the dose response for the device. The dosimeter’s performance was evaluated for the standard radiation qualities RQT 8, RQT 9 and RQT 10 in a metrology laboratory. In a CT scanner, the MOSkin was used to assess the air kerma profile and the dose profile in a phantom. The integral of the dose profile was compared to the CPMMA,100 measured with a pencil ionization chamber. The results showed that the MOSkin response was linear and reproducible with doses in the CT range. Energy dependence varied up to a factor of 1.19 among the tested X-ray energies. Angular dependence of the response was not greater than 7.8% within the angle range from 0 to 90 degrees. Signal fading within 3 min was negligible. Additionally, the MOSkin was able to accurately assess the air kerma profile and the integral of the dose profile in a CT scanner. The integral of the dose profile in a phantom was in agreement with the CPMMA,100. The presented results demonstrated the potential of the MOSkin for application in CT dosimetry.

Recent epidemiological studies suggested to lower the threshold dose for radiation induced cataract in the eye lens. Therefore, eye lens radiation protection became to play a more important role in personal dosimetry. The main objective of this work is to propose a new methodology for prototyping and benchmarking of an eye lens dosiemter based on the equivalent dose to the sensitive part of the eye lens, using CAD Software and Geant 4 Monte Carlo simulations with mesh modeling and 3D printing. A 3D printed dosimeter was type tested based on IEC 62387:2012, in terms of energy and angular dependence for the measurements of Hp(3). The results show that the methodology employed is suitable for the development of new eye lens dosimeters.

A comparison exercise of Latin American and Caribbean Secondary Standards Dosimetry Laboratories (SSDLs) was jointly organized by the International Atomic Energy Agency (IAEA) and the Ionizing Radiation Metrology Laboratory at the Federal University of Pernambuco (LMRI-DEN/UFPE). This exercise was organized during an IAEA regional meeting on the review and update of calibration capabilities in Latin America, held in Recife, during the period from 23 to 27 April 2018 under the technical cooperation project ME-RLA 9085-170572. Fifteen participating SSDLs were required to irradiate optically stimulated personal dosimeters in terms of the personal dose equivalent Hp(10) in 137Cs radiation quality. In addition, the IAEA Dosimetry Laboratory in Seibersdorf, Austria, and the National Physical Laboratory in Teddington, Middlesex, UK participated in this exercise as reference institutes. Each participant received 10 dosimeters that were hand-carried directly to the SSDL. Two nominal dose values of 2 mSv and 4 mSv were selected for this exercise. The participants irradiated the dosimeters using the setup and the procedures which are normally used in their standard laboratory for Hp(10) dosimeter irradiations. The dosimeters were evaluated as they were received by the coordinating laboratory, using a single BeOSL Reader. The results show that, except for one laboratory, the differences between the dosimeter reading and the assigned values were within 10%; this is consistent with the expanded uncertainty. The results indicate that most of the participant laboratories have a good capability to irradiate personal dosimeters in the quantity Hp(10).

Cell or tissue renewal and regeneration are the two main developmental requirements of adult organisms.

The results obtained with a standard float zone (FZ) silicon diode, processed at the Helsinki Institute of Physics, used as on-line diagnostic X-ray dosimeter are described in this work. The device was connected in the short-circuit current mode to the input of an integrating electrometer. The response repeatability and the current sensitivity coefficient of the diode were measured with diagnostic X-ray beams in the range of 40-80 kV. The dose-response of the device, evaluated from 10 mGy up to 500 mGy, was linear with high charge sensitivity. Nevertheless, significant energy dependence was observed in the charge sensitivity of FZ device for energies below 70 kV. The dosimetric characteristics of this FZ diode were compared to those of an XRA-50 commercial Si diode, specially designed to X-ray dosimetry. The results obtained with the FZ diode evidenced that it can be an alternative choice for diagnostic X-ray dosimetry, although it needs to be calibrated for individual X-ray beam energies. The studies of long-term stability and the radiation hardness of these diodes are under way.

Tomography image analysis is one of the most important steps in clinical diagnosis for many different pathologies, as they provide a great amount of visual information of virtually inaccessible areas of the body, such as the brain.

Prostatic artery embolisation (PAE) is used to treat patients with benign prostatic hyperplasia and with lower urinary obstructive tract symptoms. It is an interventional procedure which uses fluoroscopy equipment and can result in exposure to high doses of radiation in patients and staff. We aimed to demonstrate the reduction of radiation doses received by staff during PAE by implementing an optimised protocol called Radiation Exposure Curtailment for Embolisation (RECiFE). This protocol was implemented in cooperation with the medical team and technical team using Siemens Combined Applications to Reduce Exposure (CARE) protocol. The results showed approximately 83% reduction in the radiation doses received by the main physician during PAE. Thus, by adjusting the acquisition parameters of the angiographic equipment and implementing the RECiFE protocol, it is possible to optimise the PAE procedure and reduce the staff radiation dose.

The response of thin diodes (SFH206k) as dosimeters has been investigated employing the beam of an electron accelerator within the dose rate range of 2–8 kGy/s and accumulated doses up to 100 kGy. These devices, operating in the short-circuit mode and under industrial irradiation conditions, deliver current signals nonlinearly dependent on the dose rate, whichever the dose history of the diodes, due to the high density of the generated electron-hole pairs herein achieved. Despite this nonlinearity, the dose rate response is stable and characterized by current signals with repeatability better than 2.0%, regardless of the accumulated dose. It is also found that the dose responses are quite linear with sensitivities slightly dependent on the accumulated dose at a constant dose rate. The decrease in the charge sensitivity, taking as reference that obtained before any radiation damage, reaches only 9% (k = 2) at 100 kGy, which is much smaller than the values reported in the literature. From this low aging and the repeatability of both dose rate and dose responses, it seems that the photodiode under investigation is a low budget alternative, good enough for routine dosimetry, provided it has been previously calibrated in the same processing facility.

This work reports the results obtained from the comparison involving 7 laboratories in Brazil. This exercise is about the calibration service of surface contamination monitors. The monitor has been calibrated to BS ISO 7503-3, the calibration factor in terms of surface emission rate. The comparison was conducted by the Brazilian National Laboratory of Ionizing Radiation Metrology (LNMRI / IRD) from October 2018 to July 2019. The extensive sources used were 14C, 137Cs, 60Co, 90Sr/90Y, 36Cl and 241Am. The result of this proficiency test was excellent and proved the calibration capacity of the Brazilian network in the calibration service for surface contamination monitors.

This study investigates patient and staff dose measurements in pediatric barium meal series fluoroscopic procedures. It aims to analyze radiographic techniques, measure the air kerma-area product (PKA), and estimate the staff's eye lens, thyroid and hands equivalent doses. The procedures of 41 patients were studied, and PKA values were calculated using LiF:Mg,Ti thermoluminescent dosimeters (TLDs) positioned at the center of the patient’s upper chest. Furthermore, LiF:Mg,Cu,P TLDs were used to estimate the equivalent doses. The results showed a discrepancy in the radiographic techniques when compared to the European Commission recommendations. Half of the results of the analyzed literature presented lower PKA and dose reference level values than the present study. The staff‘s equivalent doses strongly depends on the distance from the beam. A 55-cm distance can be considered satisfactory. However, a distance decrease of ~20% leads to, at least, two times higher equivalent doses. For eye lenses this dose is significantly greater than the annual limit set by the International Commission on Radiological Protection. In addition, the occupational doses were found to be much higher than in the literature. Changing the used radiographic techniques to the ones recommended by the European Communities, it is expected to achieve lower PKA values ​​and occupational doses.

The objective of this study was to assess the radiation doses received by anaesthetists from prostatic artery embolization (PAE) procedures. Ten PAE procedures conducted in a reference hospital in the city of Recife, Brazil were investigated. Occupational dosimetry was performed using thermoluminescent dosemeters which were located next to the eyes, close to the thyroid (over the shielding), on the thorax (under the apron), on the wrist and on the feet of the physician’s body. The results showed that the anaesthetist’s feet received the highest doses followed by the eyes and the hands. In some complex PAE procedures the doses received by anaesthetists on the lens of the eyes and the effective dose were higher than those received by the main operator due to the anaesthetist’s close position to the patient’s table and the use of oblique projections. The personal dose equivalent Hp(3) per procedure for the anaesthetist’s right eyebrow ranged from 20.2 μSv to 568.3 μSv. This result shows that anaesthetists assisting PAE procedures can exceeds the annual eye lens dose limit of 20 mSv recommended by the ICRP with only one procedure per week if radiation protection measures are not implemented during procedures.

The objective of this work was to evaluate the air kerma-area product (PKA) and the skin absorbed dose in the region of the eyes, salivary glands and thyroid of the patient from mandible examinations performed with three cone-beam computed tomography (CBCT) scanners, i.e. i-CAT classic, Gendex CB-500 and PreXion 3D. For the dosimetric evaluation, an anthropomorphic head phantom (model RS-250) was used to simulate an adult patient. The CBCT examinations were performed using standard and high-resolution protocols for mandible acquisitions for adult patients. During the phantom's exposure, the PKA was measured using an ionising chamber and the absorbed doses to the skin in the region of the eyes, thyroid and salivary glands were estimated using thermoluminescence dosemeters (TLDs) positioned on the phantom's surface. The PKA values estimated with the CBCT scanners varied from 26 to 138 µGy m(2). Skin absorbed doses in the region of the eyes varied from 0.07 to 0.34 mGy; at the parotid glands, from 1.31 to 5.93 mGy; at the submandibular glands, from 1.41 to 6.86 mGy; and at the thyroid, from 0.18 to 2.45 mGy. PKA and absorbed doses showed the highest values for the PreXion 3D scanner due to the use of the continuous exposure mode and a high current-time product.

This study investigates the optically stimulated luminescence (OSL) and the photo-transferred thermoluminescence (PTTL) signals in quartz single crystals showing a strong TL peak near 300 °C after being sensitized by irradiation with 25 kGy of gamma rays and heating at 400 °C. Natural and sensitized samples were prepared from two crystals with different sensitivity levels in the 300 °C TL region. Continuous-wave (CW) and linearly-modulated (LM) OSL signals were stimulated with blue light-emitting diodes during 40 and 1000 s, respectively. Two components were isolated from the CW-OSL signals of sensitized samples. These components were clearly seen in LM-OSL measurements together with two long-term components. LM-OSL showed that the sensitization process considered in this study sensitized an ultrafast OSL component of these crystals. The similar behavior found for the thermal stabilities of OSL and TL signals and the dependence of these signals with sample origins suggested that the trapping site related to the ultrafast component is also related to the TL process of the sensitized peak. The PTTL signal induced by blue LEDs increased the intensity of the sensitized glow peak. On the other hand, a remarkable reduction in the intensity of this peak as a result of the accumulated effect of blue light exposure was clearly seen in both kinds of crystals. These results were explained by a mechanism of competition between optically unstable deep traps and trap levels responsible to the sensitized TL peak.

The objective of this study was to evaluate the entrance surface air kerma in pediatric chest radiography. An evaluation of 301 radiographical examinations in anterior–posterior (AP) and posterior–anterior (PA) (166 examinations) and lateral (LAT) (135 examinations) projections was performed. The analyses were performed on patients grouped by age; the groups included ages 0–1 y, 1–5 y, 5–10 y, and 10–15 y. The entrance surface air kerma was determined with DoseCal software (Radiological Protection Center of Saint George׳s Hospital, London) and thermoluminescent dosimeters. Two different exposure techniques were compared. The doses received by patients who had undergone LAT examinations were 40% higher, on average, those in AP/PA examinations because of the difference in tube voltage. A large high-dose “tail” was observed for children up to 5 y old. An increase in tube potential and corresponding decrease in current lead to a significant dose reduction. The difference between the average dose values for different age ranges was not practically observed, implying that the exposure techniques are still not optimal. Exposure doses received using the higher tube voltage and lower current-time product correspond to the international diagnostic reference levels.

The aim of this article was to verify the performance of the Mirion InstadoseTM dosemeter under clinical conditions and to compare its response in typical X-ray fields used during interventional and cardiology procedures with the TLD-100, usually used for radiation dosimetry. It was also objective of this study to verify the feasibility of using the InstadoseTM dosemeter response at the chest level for estimation of occupational eye lens dose in cardiology and interventional radiology. Initially the response of the dosemeter was tested using continuous X-ray beams and the results showed that the Instadose dosemeter present a satisfactory behavior of the most important dosimetric properties based on the tests as described in the IEC 62387 standard. The measurements performed in clinical conditions showed that the InstadoseTM dosemeter response was comparable to that of TL dosemeters used in interventional radiology and cardiology procedures and there is a correlation between the eye lens doses and the chest doses measured with the InstadoseTM. Based on the results obtained, we recommend the use of the InstadoseTM dosemeter for purposes of occupational whole-body monitoring of medical staff in interventional radiology and cardiology procedures.

The dosimetric response of a multi guard ring structure (MGR) diode has been studied with clinical electron beam energies from 5 MeV to 15 MeV. The results showed that the MGR dose response is linear in the range of 5–320 cGy and presents reproducibility with variation coefficients less than 0.4%. The field output factors measured with the MGR agreed within 2% with those measured with an ionization chamber. This study evidences that this diode can be used for clinical electron beam dosimetry.

Introduction Digital mammography present many advantages in comparison to conventional mammography, such as high dynamic range and the post-processing of acquired images. One problem is that protocols may not be optimized, resulting in higher absorbed doses to patients. The objective of this work is to evaluate image quality and to estimate mean glandular doses (MGD) in patients submitted to mammography examinations with three digital systems and one screen-film system in Recife, Brazil. Methods To estimate the MGD, the parameters used to acquire images of 5475 patients, with ages between 40 and 64 years and compressed breasts between 2 and 9 cm, were registered. The MGD was calculated by multiplying the incident air kerma with conversion coefficients depending on the anode/filter, breast glandularity and half-value layer. The image quality evaluation of the digital systems was made using objective and subjective European criteria. Results The results showed MGDs in the range of 0.4-10.3 mGy and the higher values were observed with digital systems. It was also observed that in the digital systems the use of compression force is not adequate and the irradiation parameters are not optimized. The images failed to reproduce the pectoral muscle and the contrast-to-noise ratio was not adequate for one system, indicating the need to improve the patient’s positioning and the exposure parameters. Conclusion It can be concluded that the use of non-optimized irradiation parameters is causing the higher doses with digital systems, highlighting the insufficient compression force.

The purpose of bowtie filters in CT scanners is to homogenize the x-ray intensity measured by the detectors in order to improve the image quality and at the same time to reduce the dose to the patient because of the preferential filtering near the periphery of the fan beam. For CT dosimetry, especially for Monte Carlo calculations of organ and tissue absorbed doses to patients, it is important to take the effect of bowtie filters into account. However, material composition and dimensions of these filters are proprietary. Consequently, a method for bowtie filter simulation independent of access to proprietary data and/or to a specific scanner would be of interest to many researchers involved in CT dosimetry. This study presents such a method based on the weighted computer tomography dose index, CTDIw, defined in two cylindrical PMMA phantoms of 16 cm and 32 cm diameter. With an EGSnrc-based Monte Carlo (MC) code, ratios CTDIw/CTDI100,a were calculated for a specific CT scanner using PMMA bowtie filter models based on sigmoid Boltzmann functions combined with a scanner filter factor (SFF) which is modified during calculations until the calculated MC CTDIw/CTDI100,a matches ratios CTDIw/CTDI100,a, determined by measurements or found in publications for that specific scanner. Once the scanner-specific value for an SFF has been found, the bowtie filter algorithm can be used in any MC code to perform CT dosimetry for that specific scanner. The bowtie filter model proposed here was validated for CTDIw/CTDI100,a considering 11 different CT scanners and for CTDI100,c, CTDI100,p and their ratio considering 4 different CT scanners. Additionally, comparisons were made for lateral dose profiles free in air and using computational anthropomorphic phantoms. CTDIw/CTDI100,a determined with this new method agreed on average within 0.89% (max. 3.4%) and 1.64% (max. 4.5%) with corresponding data published by CTDosimetry (www.impactscan.org) for the CTDI HEAD and BODY phantoms, respectively. Comparison with results calculated using proprietary data for the PHILIPS Brilliance 64 scanner showed agreement on average within 2.5% (max. 5.8%) and with data measured for that scanner within 2.1% (max. 3.7%). Ratios of CTDI100,c/CTDI100, p for this study and corresponding data published by CTDosimetry (www.impactscan.org) agree on average within about 11% (max. 28.6%). Lateral dose profiles calculated with the proposed bowtie filter and with proprietary data agreed within 2% (max. 5.9%), and both calculated data agreed within 5.4% (max. 11.2%) with measured results. Application of the proposed bowtie filter and of the exactly modelled filter to human phantom Monte Carlo calculations show agreement on the average within less than 5% (max. 7.9%) for organ and tissue absorbed doses.

Latin American countries (Argentina, Brazil, Chile, Costa Rica, Cuba, Ecuador, El Salvador, Guatemala, Mexico, Nicaragua, Paraguay, Uruguay and Venezuela) working under International Atomic Energy Agency (IAEA) Technical Cooperation Programme: TSA3 Radiological Protection of Patients in Medical Exposures have joined efforts in the optimization of radiation protection in mammography practice. Through surveys of patient doses, the region has a unique database of diagnostic reference levels for analog and digital equipment that will direct future optimization activities, towards the early detection of breast cancer among asymptomatic women. During RLA9/057 (2007-2009) 24 institutions participated with analog equipment in a dose survey. Regional training on methodology and measurement equipment was addressed in May 2007. Mean glandular dose (DG) was estimated using the incident kerma in air and relevant conversion coefficients for both projections craneo caudal and mediolateral oblique (CC and MLO). For phase two, RLA9/067 (2010-2011), it was decided to include also digital systems in order to see their impact in future dose optimization activities. Any new country that joined the project received training in the activities through IAEA expert missions. 29 new institutions participated (9 analog and 20 digital equipment). A total of 2262 patient doses were collected during this study and from them DG (mGy) for both projections were estimated for each institution and country. Regional results (75 percentile in mGy) show for CC and MLO respectively: RLA9/057 (analog) 2.63 and 3.17; RLA/067: 2.57 and 3.15 (analog) and 2.69 and 2.90 (digital). Regarding only digital equipment for CC and MLO respectively, computed radiography (CR) systems showed 2.59 and 2.78 and direct digital radiography (DDR) systems 2.78 and 3.04. Based on the IAEA Basic Safety Standard (BSS) reference dose (3mGy), it can be observed that there is enough room to start optimization processes in Latin America (LA); several countries or even particular institutions have values much higher than the 3mGy. The aim of this work was to perform a dosimetric evaluation of two cone-beam computed tomography scanners. The study was performed with two scanners: i-CAT classic and PreXion 3D. Air kerma-area product (PKA) was measured for all full-arch dental implant planning protocols. Surface air kerma was also estimated at the region of the eyes, salivary glands and thyroid using thermoluminescence dosemeters positioned on an anthropomorphic phantom. The PKA values for the i-CAT classic ranged from 24 to 180 µGy m(2) and, for the PreXion, from 70 to 138 µGy m(2). The large variation of these values was mainly caused by acquisition time and field of views. The surface air kerma values were from 0.08 to 3.39 mGy at the eyes, 0.50 to 3.96 mGy at the parotids, 0.11 to 2.95 mGy at the submandibular glands and 0.05 to 1.32 mGy at the thyroid. These values are comparable with those found in the literature.

The purpose of this study is the development of paediatric reference phantoms for newborn and 1-year-old infants to be used for the calculation of organ and tissue equivalent doses in radiation protection. The study proposes a method for developing anatomically highly sophisticated paediatric phantoms without using medical images. The newborn and 1-year-old hermaphrodite phantoms presented here were developed using three-dimensional (3D) modelling software applied to anatomical information taken from atlases, textbooks and images provided by the Department of Anatomy of the Federal University of Pernambuco, Brazil. The method uses polygon mesh surfaces to model body contours, the shape of organs as well as their positions and orientations in the human body. Organ and tissue masses agree with corresponding data given by the International Commission on Radiological Protection for newborn and 1-year-old reference children. Bones were segmented into cortical bone, spongiosa, medullary marrow and cartilage to allow for the use of μCT images of trabecular bone for skeletal dosimetry. Anatomical results show 3D images of the phantoms' surfaces, organs and skeletons, as well as tables with organ and tissue masses or skeletal tissue volumes. Dosimetric results present comparisons of organ and tissue absorbed doses or specific absorbed fractions between the newborn and 1-year-old phantoms and corresponding data for other paediatric stylised or voxel phantoms. Most differences were found to be below 10%.

The International Atomic Energy Agency (IAEA) through the International Action Plan on Radiation Protection of Patients and the International Commission on Radiological Protection have for some time carried out important efforts to assure that in the medical applications of the ionising radiations, the optimisation of radiological protection of patients is fundamental, to such a point that the IAEA includes it directly as a requirement for these practices (in its International Basic Safety Standards for Protection against Ionising Radiation and for the Safety of Radiation Sources (BSS)-GSR Part 1, 2011). For this reason, among the objectives of Regional Project RLA/9/057 and Regional Project RLA/9/067, the intention was to establish the dose references in conventional radiology for Latin America, for the purposes of determining whether these doses comply with the requirements of the BSS and to tend to improve practices, in order to minimise the dose received by the patients.

Purpose To prospectively analyze the radiation exposure of patients and interventional radiologists during prostatic artery embolization (PAE). Materials and Methods Twenty-five consecutive PAE procedures performed with an Artis zee system in a single center by an interventional radiologist were prospectively monitored. The mean age, weight, and prostate volume of the patients were 65.7 year (range, 43–85 y), 71.4 kg (range, 54–88 kg), and 79 cm 3 (range, 36–157 cm 3 ), respectively. In addition to Digital Imaging and Communications in Medicine radiation data, direct measures were also obtained. Radiochromic film was used to evaluate peak skin dose (PSD). The radiologist wore a protective apron and a thyroid collar, and a ceiling-suspended screen and a table curtain were used. To estimate the absorbed doses, nine pairs of dosimeters were attached to the operator's body. Results The average fluoroscopy time was 30.9 minutes (range, 15.5–48.3 min). The mean total dose–area product (DAP) was 450.7 Gy·cm 2 (range, 248.3–791.73 Gy·cm 2 ) per procedure. Digital subtraction angiography was responsible for 71.5% of the total DAP, followed by fluoroscopy and cone-beam computed tomography. The mean PSD was 2,420.3 mGy (range, 1,390–3,616 mGy). The average effective dose for the interventional radiologist was 17 μSv (range, 4–47 μSv); values for the eyes, hands, and feet were obtained, and were all greater on the left side. Conclusions PAE may lead to high x-ray exposures to patients and interventional radiologists.

Performance comparison of selected photodiodes for usage as radiation detectors for radio-protection is presented. In this study, based on the criteria of minimum sensitive area of 5mm2, minimum half angle 60° and low cost, four commercial photodiodes are selected for evaluation: SFH205, SFH206, BPW34, and BPX90F. Photodiodes are low cost, small volume and lightweight detectors. As an electronic transducer, photodiode detector is an attractive approach for the development of low power portable electronic dosimeter for direct-reading real-time radiation dose measurement. The devices have been studied with respect to sensitivity (efficiency) in X-rays and gamma rays detection, repeatability and linearity in air kerma.

Objective: To estimate the entrance surface air kerma (Ka,e) and air kerma in the region of radiosensitive organs in radiographs of pediatric paranasal sinuses. Materials and Methods: Patient data and irradiation parameters were collected in examinations of the paranasal sinuses in children from 0 to 15 years of age at two children's hospitals in the city of Recife, PE, Brazil. We estimated the Ka,e using the X-ray tube outputs and selected parameters. To estimate the air kerma values in the regions of the eyes and thyroid, we used thermoluminescent dosimeters. Results: The Ka,e values ranged from 0.065 to 1.446 mGy in cavum radiographs, from 0.104 to 7.298 mGy in Caldwell views, and from 0.113 to 7.824 mGy in Waters views. Air kerma values in the region of the eyes ranged from 0.001 to 0.968 mGy in cavum radiographs and from 0.011 to 0.422 mGy in Caldwell and Waters views . In the thyroid region, air kerma values ranged from 0.005 to 0.932 mGy in cavum radiographs and from 0.002 to 0.972 mGy in Caldwell and Waters views. Conclusion: The radiation levels used at the institutions under study were higher than those recommended in international protocols. We recommend that interventions be initiated in order to reduce patient exposure to radiation and therefore the risks associated with radiological examination of the paranasal sinuses. Resumo Objetivo: Estimar o kerma no ar na superfície de entrada (Ka,e) e o kerma no ar na região de órgãos radiossensíveis em radiografias pediátricas dos seios paranasais. Materiais e Métodos: Foram coletados os dados dos pacientes e parâmetros de irradiação em exames de seios paranasais de crianças com 0 a 15 anos, em dois hospitais infantis na cidade do Recife, PE. O Ka,e foi estimado utilizando os rendimentos dos tubos de raios X e os parâmetros selecionados. Os valores de kerma no ar nas regiões dos olhos e tireoide foram estimados utilizando dosímetros termoluminescentes. Resultados: Os valores de Ka,e variaram de 0,065 a 1,446 mGy para radiografias cavum, 0,104 a 7,298 mGy para Caldwell e 0,113 a 7,824 mGy para Waters. Os valores de kerma no ar na região dos olhos variaram de 0,001 a 0,968 mGy para cavum e 0,011 a 0,422 mGy para Caldwell e Waters. Na região tireóidea, os valores de kerma no ar variaram de 0,005 a 0,932 mGy para cavum e 0,002 a 0,972 mGy para Caldwell e Waters. Conclusão: Os parâmetros de irradiação utilizados pelas instituições foram mais elevados do que os recomendados em protocolos britânicos. Ações de otimização são recomendadas de modo a contribuir para a redução da radiação nos pacientes e, portanto, os riscos.

The aim of this study was to evaluate the radiation doses to patients and staff received from the first cases of prostatic artery embolization (PAE) conducted in a public hospital in Recife, Brazil. Five PAE procedures for 5 men diagnosed with benign prostatic hyperplasia were investigated. In order to characterize patient exposure, dosimetric quantities, such as the air kerma-area product (P KA), the cumulative air kerma at the interventional reference point (Ka,r), the number of images, etc, were registered. To evaluate the possibility for deterministic effects, the peak skin dose (PSD) was measured using radiochromic films. For evaluation of personal dose equivalent and effective dose to the medical staff, thermoluminescent dosemeters (TLD-100) were used. The effective dose was estimated using the double dosimetry alghoritm of von Boetticher. The results showed that the mean patient's PSD per procedure was 2674.2 mGy. With regard to the medical staff, the mean, minimum and maximum effective doses estimated per procedure were: 18 μSv, 12 μSv and 21 μSv respectively. High personal equivalent doses were found for the feet, hands and lens of the eye, due to the use of multiple left anterior oblique projections and the improper use of the suspended lead screen and the lead curtain during procedures.

Computed tomography (CT) examinations have increased significantly in recent years due to technological innovations. In some industrialised countries, CT contributes to the population dose as much as background radiation. In developing countries, the uses and risks of CT have not been well characterised. The purpose of this investigation was to assess potential stochastic and deterministic radiation effects from common CT exams performed in six hospitals of Recife, Pernambuco. Scanning parameters and patient gender and age were collected for a total of 285 patients undergoing CT examinations of the head (90), chest (75), abdomen (60) and abdomen?pelvis (60). The organ doses, which were calculated using the ImPACT dosimetry calculator, varied significantly among institutions. Organs such as the brain, the heart and the eye lenses, which exhibited doses as high as 85, 42 and 100?mGy, respectively, are of concern for the production of cerebrovascular and cardiovascular diseases and cataracts. Effective cancer risks were calculated using Brenner methodology and BEIR-VII risk factors. They range from 1.8 to 110.2 cases per 100?000 persons for cancer induction and from 1.5 to 63.0 cases per 100?000 for cancer mortality. To reduce doses, a quality assurance programme that includes procedural justification and radiation protection optimisation should be implemented.

A homemade cylindrical ionization chamber was studied for routine use in therapy beams of (60)Co and X-rays. Several characterization tests were performed: leakage current, saturation, ion collection efficiency, polarity effect, stability, stabilization time, chamber orientation and energy dependence. All results obtained were within international recommendations. Therefore the homemade ionization chamber presents usefulness for routine dosimetric procedures in radiotherapy beams.

Two skeletal dosimetry methods using µCT images of human bone have recently been developed: the paired-image radiation transport (PIRT) model introduced by researchers at the University of Florida (UF) in the US and the systematic–periodic cluster (SPC) method developed by researchers at the Federal University of Pernambuco in Brazil. Both methods use µCT images of trabecular bone (TB) to model spongiosa regions of human bones containing marrow cavities segmented into soft tissue volumes of active marrow (AM), trabecular inactive marrow and the bone endosteum (BE), which is a 50 µm thick layer of marrow on all TB surfaces and on cortical bone surfaces next to TB as well as inside the medullary cavities. With respect to the radiation absorbed dose, the AM and the BE are sensitive soft tissues for the induction of leukaemia and bone cancer, respectively. The two methods differ mainly with respect to the number of bone sites and the size of the µCT images used in Monte Carlo calculations and they apply different methods to simulate exposure from radiation sources located outside the skeleton. The PIRT method calculates dosimetric quantities in isolated human bones while the SPC method uses human bones embedded in the body of a phantom which contains all relevant organs and soft tissues. Consequently, the SPC method calculates absorbed dose to the AM and to the BE from particles emitted by radionuclides concentrated in organs or from radiation sources located outside the human body in one calculation step. In order to allow for similar calculations of AM and BE absorbed doses using the PIRT method, the so-called dose response functions (DRFs) have been developed based on absorbed fractions (AFs) of energy for electrons isotropically emitted in skeletal tissues. The DRFs can be used to transform the photon fluence in homogeneous spongiosa regions into absorbed dose to AM and BE. This paper will compare AM and BE AFs of energy from electrons emitted in skeletal tissues calculated with the SPC and the PIRT method and AM and BE absorbed doses and AFs calculated with PIRT-based DRFs and with the SPC method. The results calculated with the two skeletal dosimetry methods agree well if one takes the differences between the two models properly into account. Additionally, the SPC method will be updated with larger µCT images of TB.

OBJETIVO: Estimar a dose extracraniana nos olhos, tireoide, tórax e pelve em pacientes submetidos a radiocirurgia com acelerador linear de 6 MV. MATERIAIS E MÉTODOS: Foram avaliados 11 pacientes com tumores cerebrais primários (7 pacientes) e secundários (4 pacientes), sendo que dois destes apresentavam duas lesões. Para a estimativa da dose extracraniana, foram utilizados dosímetros termoluminescentes. Foram utilizados cones de 1,50 a 3,75 cm e as doses de radiação variaram de 1300 a 2000 cGy. RESULTADOS: A idade média dos pacientes foi de 52 anos, sendo 63,6% do sexo feminino e 36,4% do sexo masculino. As localizações das lesões foram: nervo acústico direito (1), frontal (2), parietal (5), occipital direito (1), cerebelar (2) e parassagitais (2). Os valores médios das doses recebidas na região entre os olhos foram de 5,1 cGy; no olho direito, de 4,8 cGy; no olho esquerdo, de 6,5 cGy; na tireoide, de 4,2 cGy; no tórax, de 1,65 cGy; e na pelve, de 0,45 cGy. CONCLUSÃO: Estes resultados mostram que embora as doses não ultrapassem os limites de tolerância para ocorrência da opacidade do cristalino, é importante que os médicos radioterapeutas considerem os riscos de dose de radiação nessas regiões durante o planejamento de procedimentos de radiocirurgia craniana. OBJECTIVE: To estimate extracranial doses on eyes, thyroid, chest and pelvis in patients submitted to radiosurgery with 6 MV linear accelerator. MATERIALS AND METHODS: The present study evaluated 11 patients, 7 of them with primary, and 4 with secondary brain tumors. In the latter group, 2 patients had two lesions. Thermoluminescent dosimeters were utilized to estimate the extracranial dose. Radiosurgery cones ranges between 1.50 and 3.75 cm and doses between 1300 and 2000 cGy. RESULTS: Mean patients' age was 52 years, and 63.6% of them were women and 36.4%, men. Lesion locations were the following: right acoustic nerve (1), frontal (2), parietal (5), right occipital (1), cerebellum (2) and parasagittal (2). Mean received doses were the following: 5.1 cGy between the eyes; 4.8 cGy in the right eye; 6.5 cGy in the left eye; 4.2 cGy in the thyroid; 1.65 cGy in the chest; and 0.45 cGy in the pelvis. CONCLUSION: The results demonstrate that that although the eye doses do not exceed the tolerance limits for occurrence of lens opacity, it is important that the risks associated with radiation doses are taken into consideration by radiotherapists in the planning of cranial radiosurgery procedures.

The 5- and 10-year-old male and female phantoms presented here were developed using 3D modeling software applied to anatomical information taken from atlases and textbooks. The method uses polygon mesh surfaces to model body contours, the shape of organs as well as their positions, and orientations in the human body. Organ and tissue masses comply with the corresponding data given by the International Commission on Radiological Protection (ICRP) for the 5- and 10-year-old reference children. Bones were segmented into cortical bone, spongiosa, medullary marrow, and cartilage to allow for the use of micro computer tomographic ({mu}CT) images of trabecular bone for skeletal dosimetry. Results: The four phantoms, a male and a female for each age, and their organs are presented in 3D images and their organ and tissue masses in tables which show the compliance of the ICRP reference values. Dosimetric data, calculated for the reference pediatric phantoms by Montemore » Carlo methods were compared with corresponding data from adult mesh phantoms and pediatric stylized phantoms. The comparisons show reasonable agreement if the anatomical differences between the phantoms are properly taken into account. Conclusions: Pediatric phantoms were developed without using medical images of patients or volunteers for the first time. The models are reference phantoms, suitable for regulatory dosimetry, however, the 3D modeling method can also be applied to medical images to develop patient-specific phantoms.« less

The aim of this study was to evaluate the image quality of 29 computed tomography (CT) scanners in Brazil and to perform estimations of patient dose and image quality of common CT examinations at these equipment. The volume CT air kerma indexes (C(VOL)) were estimated, using normalised weighted air kerma indexes, supplied by the ImPACT group. The image quality tests were performed using the phantom and accreditation protocol from the American College of Radiology (ACR). The C(VOL) values for head scans varied between 8.7 and 108 mGy. The Hi-res chest examinations presented C(VOL) values varying from 0.4 to 32 mGy. For abdominal scans, the estimated C(VOL) values varied between 4.1 and 94 mGy. This wide variation of air kerma between different centres is related to the scanner type and also to the scanning parameters. The results also showed that the image quality did not attend all ACR CT accreditation requirements.

When the human body is exposed to ionizing radiation, among the soft tissues at risk are the active marrow (AM) and the bone endosteum (BE) located in tiny, irregular cavities of trabecular bone. Determination of absorbed fractions (AFs) of energy or absorbed dose in the AM and the BE represent one of the major challenges of dosimetry. Recently, at the Department of Nuclear Energy at the Federal University of Pernambuco, a skeletal dosimetry method based on µCT images of trabecular bone introduced into the spongiosa voxels of human phantoms has been developed and applied mainly to external exposure to photons. This study uses the same method to calculate AFs of energy and S-values (absorbed dose per unit activity) for electron-emitting radionuclides known to concentrate in skeletal tissues. The modelling of the skeletal tissue regions follows ICRP110, which defines the BE as a 50 µm thick sub-region of marrow next to the bone surfaces. The paper presents mono-energetic AFs for the AM and the BE for eight different skeletal regions for electron source energies between 1 keV and 10 MeV. The S-values are given for the beta emitters (14)C, (59)Fe, (131)I, (89)Sr, (32)P and (90)Y. Comparisons with results from other investigations showed good agreement provided that differences between methodologies and trabecular bone volume fractions were properly taken into account. Additionally, a comparison was made between specific AFs of energy in the BE calculated for the actual 50 µm endosteum and the previously recommended 10 µm endosteum. The increase in endosteum thickness leads to a decrease of the endosteum absorbed dose by up to 3.7 fold when bone is the source region, while absorbed dose increases by ∼20% when the beta emitters are in marrow.

The calibration procedure in radiotherapy treatments is very important and a sensitive task due to the high doses delivered to the patients. Generally, the air-kerma cavity standards for 60Co gamma rays are graphite cavity ionization chambers. In this work a new parallel-plate graphite ionization chamber was studied to analyze its potential use as a reference instrument. In order to evaluate its performance in 60Co beams, it was submitted to several characterization tests. Moreover, Monte Carlo simulations were undertaken using the EGSnrc code to study the influence of the chamber components on its response. The results obtained showed that this new ionization chamber presented a satisfactory performance in all evaluated tests.

The aim of this work was to study the performance of a pencil ionization chamber with a sensitive volume of only 1.06 cm3 and a length of 3.0 cm, developed at the Calibration Laboratory of the IPEN, in very low-energy radiotherapy X-ray beams. These beams are still used for certain skin cancer treatments due to their rapid attenuation in tissue. The dosimeter performance was evaluated in some tests proposed by the IEC 60731 standard: short- and long-term stability and linearity of response. For a complete analysis of the dosimeter response, the EGSnrc Monte Carlo simulation was utilized to investigate the influence of its different parts on the ionization chamber response. All results of the tests were in accordance with the recommended limits, and this work shows that it is possible to extend the application of this pencil-type ionization chamber developed at the LCI.

The results of the comparison involving 9 laboratories in Brazil are reported. The measured quantity was the air kerma in 137Cs and 60Co, at the level of radioprotection. The comparison was conducted by the National Laboratory Metrology of Ionizing Radiation (LNMRI/IRD) from October 2016 to March 2017. The largest deviation between the calibration coefficients was 0.8% for 137Cs and 0.7% for 60Co. This proficiency exercise proved the technical capacity of the Brazilian calibration network in radiation monitors and the results were used by some in the implementation of the standard ISO/IEC 17025.

Female and male adult human phantoms, called FASH (Female Adult meSH) and MASH (Male Adult meSH), have been developed in the first part of this study using 3D animation software and anatomical atlases to replace the image-based FAX06 and the MAX06 voxel phantoms. 3D modelling methods allow for phantom development independent from medical images of patients, volunteers or cadavers. The second part of this study investigates the dosimetric implications for organ and tissue equivalent doses due to the anatomical differences between the new and the old phantoms. These differences are mainly caused by the supine position of human bodies during scanning in order to acquire digital images for voxel phantom development. Compared to an upright standing person, in image-based voxel phantoms organs are often coronally shifted towards the head and sometimes the sagittal diameter of the trunk is reduced by a gravitational change of the fat distribution. In addition, volumes of adipose and muscle tissue shielding internal organs are sometimes too small, because adaptation of organ volumes to ICRP-based organ masses often occurs at the expense of general soft tissues, such as adipose, muscle or unspecified soft tissue. These effects have dosimetric consequences, especially for partial body exposure, such as in x-ray diagnosis, but also for whole body external exposure and for internal exposure. Using the EGSnrc Monte Carlo code, internal and external exposure to photons and electrons has been simulated with both pairs of phantoms. The results show differences between organ and tissue equivalent doses for the upright standing FASH/MASH and the image-based supine FAX06/MAX06 phantoms of up to 80% for external exposure and up to 100% for internal exposure. Similar differences were found for external exposure between FASH/MASH and REGINA/REX, the reference voxel phantoms of the International Commission on Radiological Protection. Comparison of effective doses for external photon exposure showed good agreement between FASH/MASH and REGINA/REX, but large differences between FASH/MASH and the mesh-based RPI_AM and the RPI_AF phantoms, developed at the Rensselaer Polytechnic Institute (RPI).

Skeletal dosimetry based on μ CT images of trabecular bone has recently been introduced to calculate the red bone marrow (RBM) and the bone surface cell (BSC) equivalent doses in human phantoms for external exposure to photons. In order to use the μ CT images for skeletal dosimetry, spongiosa voxels in the skeletons were replaced at run time by so-called micromatrices, which have exactly the size of a spongiosa voxel and contain segmented trabecular bone and marrow microvoxels. A cluster ( = parallelepiped ) of 2 × 2 × 2 = 8 micromatrices was used systematically and periodically throughout the spongiosa volume during the radiation transport calculation. Systematic means that when a particle leaves a spongiosa voxel to enter into a neighboring spongiosa voxel, then the next micromatrix in the cluster will be used. Periodical means that if the particle travels through more than two spongiosa voxels in a row, then the cluster will be repeated. Based on the bone samples available at the time, clusters of up to 3 × 3 × 3 = 27 micromatrices were studied. While for a given trabecular bone volume fraction the whole-body RBM equivalent dose showed converging results for cluster sizes between 8 and 27 micromatrices, this was not the case for the BSC equivalent dose. The BSC equivalent dose seemed to be very sensitive to the number, form, and thickness of the trabeculae. In addition, the cluster size and/or the microvoxel resolution were considered to be possible causes for the differences observed. In order to resolve this problem, this study used a bone sample large enough to extract clusters containing up to 8 × 8 × 8 = 512 micromatrices and which was scanned with two different voxel resolutions. Taking into account a recent proposal, this investigation also calculated the BSC equivalent dose on medullary surfaces of cortical bone in the arm and leg bones. The results showed (1) that different voxel resolutions have no effect on the RBM equivalent dose but do influence the BSC equivalent dose due to voxel effects by up to 5% for incident photon energies up to 200 keV, (2) that the whole-body BSC equivalent dose calculated with a cluster with 2 × 2 × 2 = 8 micromatrices is consistent with results received with clusters of up to 8 × 8 × 8 = 512 micromatrices, and (3) that for external whole-body exposure the inclusion of the BSC on medullary surfaces of cortical bone has a negligible effect on the whole-body BSC equivalent dose.

The introduction of routine patient dosimetry to Brazilian radiological institutions is very necessary in order to meet national and international standard requirements for radiation protection. This work presents a survey of the air kerma-area product (P(KA)), the entrance surface air kerma (K(e)) and the effective dose (E) in common radiographic examinations during the routine of a large public hospital in the city of Belo Horizonte, Brazil. Results draw attention to the use of field sizes larger than the cassette dimension, the lack of both the collimation X-ray beam and the standardisation of the exposure parameters by radiology technicians.

The aim of this paper is to report the sensitization of the TL peak appearing at 270°C in the glow curve of natural quartz by using the combined effect of heat-treatments and irradiation with high γ doses. For this, thirty discs with 6 x 1 mm 2 were prepared from plates parallell to a rhombohedral crystal face. The specimens were separated into four lots according to its TL read out between 160 and 320 °C. One lot was submitted to y doses of 60 Co radiation starting at 2kGy and going up until a cumulative dose of 25kGy. The other three lots were initially heat-treated at 500, 800 and 1000°C and then irradiated with a single dose of 25kGy. The TL response of each lot was determined as a function of test-doses ranging from 0.1 to 30 mGy. As a result, it was observed that heat-treatments themselves did not produce the strong peak at 270 °C that was observed after the administration of high γ doses. This peak is associated with the optical absorption band appearing at 470 nm which is due to the formation of [AlO 4 ]○ acting as electron-hole recombination centers. The formation of the 270°C peak was preliminary analyzed in relation to aluminum- and oxygen-vacancy-related centers found in crystalline quartz.

Micro computed tomography (microCT) images of human spongiosa have recently been used for skeletal dosimetry with respect to external exposure to photon radiation. In this previous investigation, the calculation of equivalent dose to the red bone marrow (RBM) and to the bone surface cells (BSC) was based on five different clusters of micro matrices derived from microCT images of vertebrae, and the BSC equivalent dose for 10 microm thickness of the BSC layer was determined using an extrapolation method. The purpose of this study is to extend the earlier investigation by using microCT images from eight different bone sites and by introducing an algorithm for the direct calculation of the BSC equivalent dose with sub-micro voxel resolution. The results show that for given trabecular bone volume fractions (TBVFs) the whole-body RBM equivalent dose does not depend on bone site-specific properties or imaging parameters. However, this study demonstrates that apart from the TBVF and the BSC layer thickness, the BSC equivalent dose additionally depends on a so-called trabecular bone structure (TBS) effect, i.e. that the contribution of photo-electrons released in trabecular bone to the BSC equivalent dose also depends on the bone site-specific structure of the trabeculae. For a given bone site, the TBS effect is also a function of the thickness of the BSC layer, and it could be shown that this effect would disappear almost completely, should the BSC layer thickness be raised from 10 to 50 microm, according to new radiobiological findings.

On October 26, 1991 a fatal radiological accident occurred in a 60Co irradiation facility in the town of Nesvizh in Belarus. Following a jam in the product transport system, the operator entered the facility to clear the fault. On entering the irradiation room the operator bypassed a number of safety features, which prevented him from perceiving that the source rack was in the irradiation position. After the accident average whole body absorbed doses between 8 and 16 Gy have been determined by TLD measurements, by isodose rate distributions, by biological dosimetry and by ESR measurements of clothes and teeth. In an earlier investigation the MAX/EGS4 exposure model had been used to calculate absorbed dose distributions for the radiological accident in Yanango/Peru, which actually represented the simulation of exposure from a point source on the surface of the body. After updating the phantom as well as the Monte Carlo code, the MAX/EGS4 exposure model was used to calculate the absorbed dose distribution for the worker involved in the radiological accident in Nesvizh/Belarus. For this purpose, the arms of the MAX phantom had to be raised above the head, and a rectangular 60Co source was designed to represent the source rack used in the irradiation facility. Average organ absorbed doses, depth-absorbed doses, maximum absorbed dose and average whole body absorbed dose have been calculated and compared with the corresponding data given in the IAEA report of the accident.

Under International Atomic Energy Agency regional programme TSA3 Radiological Protection of Patients in Medical Exposures, Latin American countries evaluated the image quality and glandular doses for digital mammography equipment with the purpose of seeing the performance and compliance with international recommendations. Totally, 24 institutions participated from Brazil, Chile, Costa Rica, El Salvador, Mexico, Paraguay and Venezuela. Signal difference noise ratio results showed for CR poor compliance with tolerances; better results were obtained for full-field digital mammography equipment. Mean glandular dose results showed that the majority of units have values below the acceptable dose levels. This joint Latin American project identified common problems: difficulty in working with digital images and lack of specific training by medical physicists from the region. Image quality is a main issue not being satisfied in accordance with international recommendations; optimisation processes in which the doses are increased should be very carefully done in order to improve early detection of any cancer signs.

The aim of this study is to evaluate organ and tissue absorbed doses to patients undergoing hepatic chemoembolization procedures performed in two hospitals in the city of Recife, Brazil. Forty eight patients undergoing fifty hepatic chemoembolization procedures were investigated. For the 20 cases with PA projection only, organs and tissues dose to KAP conversion coefficients were calculated using the mesh-based anthropometric phantom series FASH and MASH coupled to the EGSnrc Monte Carlo code. Clinical, dosimetric and irradiations parameters were registered for all patients. The maximum organ absorbed doses found were 2.4 Gy, 0.85 Gy, 0.76 Gy and 0.44 Gy for skin, kidneys, adrenals and liver, respectively.

The purpose of this study was to evaluate patient and medical staff absorbed doses received from transarterial chemoembolisation of hepatocellular carcinoma, which is the most common primary liver tumour worldwide. The study was performed in three hospitals in Recife, capital of the state of Pernambuco, located in the Brazilian Northeastern region. Two are public hospitals (A and B), and one is private (C). For each procedure, the number of images, irradiation parameters (kV, mA and fluoroscopy time), the air kerma-area product (PKA) and the cumulative air kerma (Ka,r) at the reference point were registered. The maximum skin dose (MSD) of the patient was estimated using radiochromic film. For the medical staff dosimetry, thermoluminescence dosemeters (TLD-100) were attached next to the eyes, close to the thyroid (above the shielding), on the thorax under the apron, on the wrist and on the feet. The effective dose to the staff was estimated using the algorithm of von Boetticher. The results showed that the mean value of the total PKA was 267.49, 403.83 and 479.74 Gy cm(2) for Hospitals A, B and C, respectively. With regard to the physicians, the average effective dose per procedure was 17 µSv, and the minimum and maximum values recorded were 1 and 41 µSy, respectively. The results showed that the feet received the highest doses followed by the hands and lens of the eye, since the physicians did not use leaded glasses and the equipment had no lead curtain.
