Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). Diabetic retinopathy (DR) is the leading cause of blindness in patients with diabetes. However, biomarkers for early detection of DR are still lacking. MicroRNAs (miRNAs) regulate multiple biological functions and are often deregulated in DR. We aimed to investigate whether circulating miRNAs can be used as biomarkers of early-stage DR. We used RNA-seq and qRT-PCR to identify differential serum miRNAs in patients with type 2 diabetes mellitus with DR (T2DM-DR), T2DM without DR (T2DM-no-DR), and healthy controls. We validated differential circulating miRNAs in two phases using qRT-PCR assays. RNA-seq analysis identified 7 differential circulating miRNAs between T2DM-DR and T2DM-no-DR and 47 differential miRNAs between T2DM-DR and healthy subjects. Two-stage analysis verified that a profile of five serum miRNAs (hsa-let-7a-5p, hsa-miR-novel-chr5_15976, hsa-miR-28-3p, has-miR-151a-5p, has-miR-148a-3p) was significantly associated with T2DM-DR. Receiver-operator-characteristic analyses showed that a panel of three miRNAs (hsa-let-7a-5p, hsa-miR-novel-chr5_15976, and hsa-miR-28-3p) presented 0.92 sensitivity and 0.94 specificity for distinguishing T2DM-DR from T2DM-no-DR, and 0.93 sensitivity and 0.86 specificity for differentiating early-stage T2DM-DR (NPDR) from late-stage DR (PDR). Lentivirus-mediated overexpression of hsa-let-7a-5p in human retinal microvascular endothelial cells (HRMECs) significantly promoted proliferation rates of HRMECs. In conclusion, the three-miRNA signature from serum may serve as a noninvasive diagnostic biomarker for DR. Furthermore, we showed that DR-associated miRNAs may be involved in the pathogenesis of DR, at least in part, through modifying proliferation of HRMECs.

6. Dosimetric Study of Adaptive Proton Pencil-Beam Scanning Treatment Plans Using CBCT-Based Synthetic CTs Generated From Commercial Image Information Systems

PURPOSE/OBJECTIVE(S) Evaluate the accuracy of using CBCT-based synthetic CTs to trigger adaptive planning in pencil-beam scanning proton therapy. The CBCT-based synthetic CTs can decrease the number of necessary verification CT scans during treatment and trigger earlier adaptive planning for more effective treatment. MATERIALS/METHODS A retrospective study of 20 patients across 3 different sites: 10 head and neck, 5 breast, and 5 lung cases were studied. MIM Maestro 7.0.4 and Velocity 4.1 were used to generate synthetic CTs from CBCTs. The verification plans calculated on both the synthetic CT (synCT) and the verification CT (verCT) performed on the same day were compared in a treatment planning system. Dose metrics from each synCT plan generated are subtracted from the corresponding dose metrics from the same day verCT plan. Outliers with greater than 10% deviation on D95% were excluded from the analysis. RESULTS The CTV D95% is less than 2% different between the synCT and verCT plans, and 75% of Velocity patients and 55% of MIM patients falls within 95% confidence levels when comparing target D95%. The average difference from verCT D95 was 1.6% for Velocity and -0.88% for MIM. Serial OAR Dmax in synCT plans average deviation from verCT was less than 1Gy on average. Parallel OAR volumetric and mean dose in the synCT plans were consistent with the verCT plans with deviations on average less than 1% by volume and 0.5Gy mean. Maximum doses have significant variations ranged from -7.7Gy to +4.4Gy for the target and -11.76Gy to +9.83Gy for OARs. The D2% of treatment volume in synCT plans showed was consistent with less than 1% deviation from the verCT plans. CONCLUSION Both systems can automatically generate synthetic CTs from CBCTs. The dosimetric similarities of synCT plans compared with verCT plans were higher for treatment sites with less heterogeneities such as breast patients when compared with head and neck patients. Outliers can happen when there are significant anatomy changes which result in inaccurate synthetic CTs due to inadequate image deformation or HU corrections. Users should pay extra attention to tissue discrepancies while evaluating the images. The Dmax for critical structures can vary greatly between synCT and verCT, so a small volumetric analysis is recommended. Both systems can be used to reduce verification CT scans while a patient is on treatment and can improve the adaptive plan workflow. Synthetic CT generation can be a more effective way to discover significant dosimetric differences caused by changes in patient anatomy, and therefore a more efficient path to adaptive treatment.

Background: The human epidermal growth factor receptor-2 (HER2) is a member of a family of transmembrane tyrosine kinase receptors that play an important role in regulated normal cell growth and differentiation. The over-expression of HER2 in a subset of 15-20% of invasive breast cancers has an important bearing on prognosis, as HER2-positive tumors are associated with an aggressive clinical course and poor outcome. Targeting HER2-overexpression has been shown to be a remarkably effective therapeutic modality; however testing of tumor samples to assess the HER2 status of the patient's breast cancer is required. Clinical assays to assess the HER2 status in patients being considered for targeted therapy include immunohistochemistry (IHC), which detects protein over-expression, or fluorescence in situ hybridization (FISH), which detects gene amplification. Both the IHC and FISH methodologies have limitations. Given that the target of the currently approved drugs is the receptor protein, novel detections systems that could more accurately and quantitatively detect HER2 protein in clinical samples over a broad dynamic range would be advantageous and may be clinically helpful. Material and Methods: A novel detection technology using streptavidin-coated Phosphor Integrated Dot fluorescent nanoparticles (PID) has been developed that can be visualized by fluorescence microscopy and used for quantitative immunofluorescence detection of protein in clinical samples using computer assisted image analysis. In the current study, PID- nanoparticles were used to analyze HER2 protein expression in breast cancer cell lines and 120 well characterized breast cancer samples. These results have been compared with HER2 IHC and HER2 FISH analysis. Results: The expression levels of HER2 protein from 8 breast cancer cell lines was evaluated by antibody-binding capacity with FACS analysis. Formalin fixed paraffin embedded cell pellets for these cell lines were prepared and used for quantitative HER2 analysis by PID. The PID score/cell for each of these cell lines showed a strong linear correlation with antibody-binding capacity sites/cell by FACS analysis (R2 = 0.94). For the 120 breast cancer samples, PID score/cell was measured and compared against HER2 IHC membrane intensity measure by image analysis (Aperio) and HER2 FISH results. The HER2 PID score/cell showed a correlation coefficient of R2=0.72 versus the average HER2 copy number per cell by FISH, compared with a correlation coefficient of R2=0.41 for HER2 IHC membrane intensity measured by Aperio. For the HER2/CEP17 ratio, the correlation coefficient for the PID score/cell was R2=0.79 compared with a correlation coefficient of R2=0.32 for the HER2 IHC membrane intensity. Conclusions: PID-nanoparticles demonstrate great potential for the quantitative measurement of protein of clinical interest in routine clinical samples with morphologic confirmation of the tissue being studied. Further studies looking for PID-score thresholds for HER2 gene amplification and correlations with clinical outcome data are warranted and ongoing. 8. A Completion Technology of Multi-layer Hydraulic Fracturing in One Trip

A completion technology of multi-layer hydraulic fracturing in Xushen Gas Field of Daqing Oil Field was developed, which was of benefit to efficiency, safety, and cost of the fracturing operation, especially to the individual well production and the formation damage compared with that in conventional hydraulic fracturing due to the replacement of the pipe string. A fracturing completion integration pipe string, in which a new type of cemented carbide hydraulic anchor was developed and other matched downhole tools including pressure guide sandblast packers were improved, was used to meet the needs of multi-layer fracturing in the gas field. The pipe string can bear a maximum working temperature of 160 and a maximum pressure of 90 MPa, with advantages of strong stability and high abrasion resistance after fracturing, so as to well satisfy the requirements of completion and production done without replacing the string. Up to the end of 2016, the technology had been applied in ten wells in the gas field, with a success rate of 100%, which can be regarded as a breakthrough for multi-layer hydraulic fracturing in deep natural gas wells in Xushen Gas Field.

This document specifies a unique nickname approach. This approach gives unique nicknames to all TRILL switches across the multilevel TRILL campus.

Background and aims Therapeutic possibilities now exist to lower low-density lipoprotein cholesterol (LDL-C) to very low levels. However, substantial controversy remains in clinical practice with regard to its safety, and the question of whether low LDL-C levels per se may provoke adverse effects in humans arises. We aimed to explore the association of LDL-C with androgen and erectile dysfunction (ED) in a general population of men. Methods and results A total of 4203 men without hormone replacement therapy were enrolled from 22 sites in East China. Total testosterone (T) and Free T were assessed. Free androgen index (FAI) was calculated. The IIEF-5 questionnaire was used to assess ED. We found that free T and FAI gradually and markedly increased with increasing LDL-C levels. Using linear regression, after adjusting for age, educational level, economic status, smoking status, drinking status, BMI, diabetes, and use of lipid-lowering medication, LDL-C was positively associated with free T (B = 0.175, 95% CI: 0.084, 0.266) and FAI (B = 0.064, 95% CI: 0.016, 0.112). Meanwhile, there was a U-shaped curvilinear relationship between LDL-C and prevalence of ED. In the logistic regression analysis, compared to those with LDL-C among the 10th–90th percentile, the ORs of ED in men in the lowest and highest deciles were 1.938 (95% CI: 1.121, 3.349) and 1.804 (95% CI: 1.117, 2.916), respectively. Conclusion Lower LDL-C levels were significantly associated with lower free T and lower FAI in a general population of men. Moreover, both low and high levels of LDL-C might be risk factors for ED.

While increasing scientific evidence suggests that the ORMDL3 rs7216389 polymorphism may contribute to a higher susceptibility to asthma, many of the current studies have yielded inconclusive results. This meta-analysis aimed to assess the association between the ORMDL3 rs7216389 polymorphism and the risk of asthma. An extensive literature search for relevant studies was conducted in PubMed, Embase, the Web of Science, the Cochrane Library, Chinese National Knowledge Infrastructure, and Google Scholar. This meta-analysis was performed using the STATA 12.0 software. Crude odds ratios (OR) and their 95% confidence intervals (CI) were calculated. Thirteen studies were included with a total of 14,851 subjects, comprised of 6739 patients with asthma and 8112 healthy controls. Our meta-analysis results revealed that the ORMDL3 rs7216389 polymorphism may be associated with an increased risk of asthma (allele model: OR = 1.39, 95%CI = 1.27-1.52, P < 0.001; dominant model: OR = 1.46, 95%CI = 1.31-1.62, P < 0.001; recessive model: OR = 1.57, 95%CI = 1.37-1.81, P < 0.001; homozygous model: OR = 1.58, 95%CI = 1.32-1.90, P < 0.001; heterozygous model: OR = 1.54, 95%CI = 1.30-1.82, P < 0.001). We also found significant associations in our subgroup analyses based on ethnicity and type of asthma. However, in our subgroup analysis based on sources of controls, an association was found in the population-based case-control subgroup but not in the hospital-based case-control subgroup. This meta-analysis indicates that ORMDL3 rs7216389 may contribute to increasing susceptibility to asthma.

We present experimental studies on ion acceleration using an 800-nm circularly polarized laser pulse with a peak intensity of $6.9\ifmmode\times\else\texttimes\fi{}{10}^{19}\text{ }\text{ }\mathrm{W}/{\mathrm{cm}}^{2}$ interacting with an overdense plasma that is produced by a laser prepulse ionizing an initially ultrathin plastic foil. The proton spectra exhibit spectral peaks at energies up to 9 MeV with energy spreads of 30% and fluxes as high as $3\ifmmode\times\else\texttimes\fi{}{10}^{12}\text{ }\text{ }\mathrm{protons}/\mathrm{MeV}/\mathrm{sr}$. Two-dimensional particle-in-cell simulations reveal that collisionless shocks are efficiently launched by circularly polarized lasers in exploded plasmas, resulting in the acceleration of quasimonoenergetic proton beams. Furthermore, this scheme predicts the generation of quasimonoenergetic proton beams with peak energies of approximately 150 MeV using current laser technology, representing a significant step toward applications such as proton therapy.

It has been long known that reinforced concrete (RC) frames enhanced masonry panels’ performance, and masonry infill panels influenced strength and stiffness of the RC frames. Window openings are indispensable ingredients of masonry panels for functional reasons. However, few researches have been conducted on framed masonry panels with openings, although there are many numerical studies available on solid framed masonry panels’ behaviour. In this study, a three-dimensional numerical model, which is based on the surface based contact cohesive interaction model and friction principle, is proposed for non-linear static analysis of perforated framed masonry panels. High-density metallic implants can introduce considerable uncertainties in proton therapy treatment planning. These uncertainties eventually translate into proton range errors, which may cause significant underdosing to the target volume or overdosing to normal tissue beyond the target. This study investigated the dosimetric impact of a 0.6 mm titanium (Ti) mesh implant in passive-scattering proton beam therapy through the study of the depth dose and output in water, and the dose profiles in solid water at various depths. The measurements were performed for a beam with a range of 8.5 cm and a modulation of 7.5 cm. The titanium mesh was placed at a depth of 1 cm below the surface of the phantom for all measurements. A range reduction of 0.5 ± 0.1 mm was observed for a beam perpendicular to the mesh, with no further reductions when the incident angle increased to 60°. We conclude that the dosimetric effect of a 0.6 mm titanium mesh implant is small for a passive scattering proton beam. With proper correction applied to metal artifacts, consistent results were observed in the phantom study in the treatment planning system.

An intake duct for a waterjet system is treated to improve the hydraulic efficiency and the outflow nonuniformity together with its perpendicularity. A multi-objective optimization strategy is proposed by incorporating with design of experiments (DOE), computational fluid dynamics (CFD), inverse design method, Kriging model and non-dominated sorting genetic algorithm-II (NSGA-II). Optimal Latin hyper-cube design method is used in DOE to uniformly sample in variation ranges, and global optimization is then conducted by using NSGA-II based on the input-target approximation functions approximated by the Kriging model. Instead of randomly selecting one solution to implement, a technique for ordering preferences by similarity to ideal solution (TOPSIS) is introduced to select the best compromise solution (BCS) from pareto-front sets. Compared with the original duct, the optimized intake duct has a much better outflow quality at the design operation point with a comparative hydraulic efficiency. Thus, the authors believe the proposed method is helpful for optimizing the intake duct.

A double beam image (DBI) technique is coupled in the two-stage accelerating mechanism to simultaneously improve the spectra and maximum energy of the proton beam. A proton beam with a narrow-spectrum center at 5.4 MeV and a long tail up to 14.4 MeV is generated in the experiment. Experimental and simulation results show that spatial collineation, time synchronization, and real-time monitoring are needed for optimum two-stage proton acceleration and are realized by the DBI technique to a certain extent in our experiment. This DBI technique can be used to achieve optimum two-stage acceleration in a feasible manner and will allow precise manipulation of multistage acceleration to improve the energy and spectra of particle beams.

Fluidised hot melt granulation (FHMG) is a novel technology for granulation process in pharmaceutical industry, which has distinct advantages over other commercial techniques. The aim of this research was to investigate granulation and the effect of process parameters that may affect FHMG process. In this work, ballotini beads were used as the model particles and Lutrol® F 68 Poloxamer 188 was used as meltable solid binder. In order to determine the granulation and nucleation mechanism in this co-melt FHMG system, several parameters were investigated, such as binder content, particle size of binder and particle size and hydrophobicity of ballotini. These parameters were correlated to granule size distribution, mean granule size and granule shape. Furthermore, these experimental investigations were designed so that the coalescence model could be applied to the co-melt FHMG system. Fluidized hot melt granulation (FHMG) is an innovative granulation process that has distinct advantages over techniques that are typically used in the pharmaceutical industry for powder agglomeration and mixing. The aim of this research was to investigate process and formulation parameters that affect FHMG, in a pharmaceutical context, using Lutrol® F68 (copolymer of polyoxyethylene-polyoxypropylene) as a meltable binder with inert glass ballotini acting as a model pharmaceutical powder. Granulation process parameters investigated were: binder/bulk powder ratio, binder viscosity and granulation time. The experimental data obtained from FHMG were correlated with current granulation theory. De-fluidization occurred at >10% w/w Lutrol® F68 binder concentration and the resultant granules were highly irregular in shape. Furthermore, the dominant mechanism of granule growth altered from layering to coalescence with increasing binder content. Moreover, a modified growth regime map successfully described experimental data.

Crater-like structures are experimentally studied with an ultrashort, ultraintense laser pulse with an intensity of 1.5 × 1018 W/cm2, irradiating borosilicate glass targets, which extends laser-induced craters to the region of relativistic intensities. The morphology of the crater-like structures is measured accurately using a three-dimensional laser scanning confocal microscope and a scanning electron microscope. The experimental results indicate that a circular bowl shape is formed with a depth-to-diameter ratio of about 1/5, which is similar to that of meteorite impact craters. A plasma fireball model is applied to analyze the experimental results. Studies show that catastrophic asteroid strikes may be investigated by irradiating foils with intense laser pulses.

The authors studied the vascular anatomy of the buccinator muscle by dissecting fresh cadavers. The anatomy of the buccal branches of the facial artery consistently confirmed the existence of a posterior buccal branch, a few inferior buccal branches, and anterior buccal branches to the posterior, inferior, and anterior portions of the buccinator. The buccal artery and posterior buccal branch anastomose to each other and ramify over the muscle. Several veins originate from the lateral aspect of the muscle, converge into the buccal venous plexus, and drain into the facial vein (from two to four tributaries) or into the pterygoid plexus and the internal maxillary vein (from the buccal vein). These vessels and nerves enter the posterior half of the buccinator posterolaterally. The facial artery and vein are located at variable distances from each other around the oral commissure and the nasal base. Two patterns of buccinator musculomucosal island flaps supplied by these buccal arterial branches are proposed in this article. The buccal musculomucosal neurovascular island flap (posteriorly based), supplied by the buccal artery, its posterior buccal branch, and the long buccal nerve, can be passed through a tunnel under the pterygomandibular ligament for closure of mucosal defects in the palate, pharyngeal sites, the alveolus, and the floor of the mouth. The buccal musculomucosal reversed-flow arterial island flap (superiorly based), supplied by the distal portion of the facial artery through the anterior buccal branches, can be used to close mucosal defects in the anterior hard palate, alveolus, maxillary antrum, nasal floor and septum, lip, and orbit. The authors have used the flaps in 12 patients. There has been no flap necrosis, and results have been satisfactory, both aesthetically and functionally.

Guillain-Barre syndrome (GBS) is an acute, post-infectious, immune-mediated, demyelinating disease of peripheral nerves and nerve roots. Experimental autoimmune neuritis (EAN) is an animal model of GBS. Chrysin, which is a naturally occurring flavonoid, exhibits various biological activities. This study was designed to investigate the anti-inflammatory and neuroprotective properties of preventative and therapeutic chrysin treatment in EAN rats. For preventative treatment, chrysin was administered orally from day 1 to day 16 (50mg/kg once daily) while, for therapeutic treatment, rats received chrysin from day 7 to day 16 at the same dose once daily. Control animals received the same volume of the vehicle (phosphate-buffered saline/2% dimethylsulfoxide). Regardless of the treatment regimen, chrysin attenuated the severity and duration of the clinical course of EAN and reduced inflammatory cell infiltration and demyelination of sciatic nerves. In the sciatic nerves, the expression of inducible nitric oxide synthase, cyclooxygenase-2 and nuclear factor kappa B was reduced. Furthermore, chrysin inhibited the splenic mononuclear cell secretion of interleukin-1β, interleukin-2, interleukin-6, inteleukin-12, interferon γ and tumor necrosis factor α, and elevated the level of inteleukin-4. In summary, our data demonstrate that chrysin is a potentially useful agent for the treatment of EAN with its anti-inflammatory and neuroprotective effects.

Focal spot effects on the generation of proton beams are investigated by a high-intensity high-contrast laser irradiating on solid foil in target normal sheath acceleration experiments. Different spot size, transverse shape, and intensity of the laser are obtained by appropriately using deformable mirrors and parabolic mirrors. Experiments show that the maximum proton energy is mainly determined by the laser intensity if the focal spot size is not seriously changed. Compared with the previous experimental results, the optimum foil thickness d o is scaled by the laser intensity I as d o ~ I 0.33. The corresponding theoretical estimation is carried out as d o ~ I 0.25 for ultra-high intensity laser systems with similar contrast. MULTI and particle-in-cell simulations are used to interpret the experimental results.

Compared with traditional calibration model, the new one doesn't need any unpolarized target when calibrating a polarization correlation radiometer by regulating the physical of calibration targets. The design for polarizing grid and wide aperture radiator are also introduced in related section. The simulation results of calibration process prove validity of the new design.

A new calibration method for tri-polarization microwave radiometers is presented in this paper. The method can be used to calibrate the radiometers having the output of the first three Stokes parameters. By using two calibration targets with temperature adjusted function and a polarizing grid, the new means also may provide a full rank Stokes matrix to compute the elements in the gain matrix of polarization-correlating radiometers. The merit of this method is that need not nonpolarized target in radiometer calibration because the existence of temperature-adjusted function of two calibration loads. Hence, it will be helpful in some special spot such as thermal-vacuum chamber. An optimal criterion based on condition number is also provided to decrease the error limit of radiometer calibration. The simulation results show the method's validity.

Physical carrier sensing is an effective mechanism of medium access control (MAC) protocols to reduce collisions in wireless networks, and the size of the carrier sensing range has a great impact on the system performance. Previous studies have shown that the MAC layer overhead plays an important role in determining the optimal carrier sensing range. However, variable transmission ranges and receiver sensitivities for different channel rates and the impact of multihop forwarding have been ignored. In this paper, we investigate the impacts of these factors as well as several other important factors, such as SINR (signal to interference plus noise ratio), node topology, hidden/exposed terminal problems and bidirectional handshakes, on determining the optimum carrier sensing range to maximize the throughput through both analysis and simulations. The results show that if any one of these factors is not addressed properly, the system performance may suffer a significant degradation. Furthermore, considering both multirate capability and carrier sensing ranges, we propose to use bandwidth distance product as a routing metric, which improves end-to-end throughput by up to 27% in the simulated scenario.

It is not always practical for persons, whose occupation or activities mandate the use of irritants or allergens, to avoid their use. In order to reduce the risk of developing irritant contact dermatitis (ICD) and allergic contact dermatitis (ACD) in industry, prophylactic measures are indicated. Application of moisturizers as well as barrier creams (BCs) before or during work and wearing appropriate gloves and clothing may be protective. Oral and, to a lesser extent, topical corticoids have demonstrated significant efficacy in the treatment of ACD. Additional therapies include UVB/UVA treatment, other immunosuppressives, and Grenz ray.
